Abstract:STATEMENT OF PROBLEM:Recent systematic reviews have reported resorption of bone grafts after augmentation, but the influencing factors are numerous and uncertain. Different brands of bone graft and other factors may affect the bone formation effect after sinus floor augmentation.PURPOSE:The purpose of this retrospective clinical study was to evaluate the graft material height changes after sinus floor augmentation with cone beam computed tomography (CBCT) and to investigate the potential influencing factors related to graft resorption.MATERIAL AND METHODS:Four midsagittal cut CBCT images of 157 posterior maxillary implants after maxillary sinus floor elevation by using the lateral window approach with bone grafts from 116 patients were obtained. Four CBCT scans had been performed immediately (T0), 6 months (T1) and 12 months after bone grafting (T2), and 1.5 to 2 years after treatment (T3), and the distance between the implant platform and the grafted mucosa of the maxillary sinus floor was measured at 3 locations. Correlation coefficients of these parameters were calculated. A linear mixed model was used to investigate potential factors influencing graft height reduction, including the patient's sex, age, smoking status, periodontal status, graft brand, implant site, implant level, placement time, and residual bone height.RESULTS:Mean ±standard deviation graft height at the mesial side of the implant (MeGH) was significantly decreased by 0.32 ±0.88 mm in the first 6 months, was gradually reduced after 6 months (0.26 ±0.43 mm), and then 1 to 2 years later increased to 0.39 ±0.97 mm. The changes in graft height at the center of the implant (CeGH) and graft height at the distal side (DiGH) groups were similar to those in the MeGH group. A significantly positive correlation was found among the height alterations in 3 locations (rs=0.954, P<.001). The linear mixed model showed that smoking was more likely to cause graft height reduction in MeGH (P=.034).CONCLUSIONS:Graft height significantly decreased after maxillary sinus augmentation over 1 to 2 years. Smoking had a negative effect on graft height changes, while the 2 brands of graft and other factors had no significant effect.
Bone Volume and Height Changes for Lateral Window Sinus Floor Elevation Using Two Types of Deproteinized Bovine Bone Mineral: A Retrospective Cohort Study of 1-4 Years

Abstract:Objective: To compare bone volume and height changes of two types of deproteinized bovine bone mineral (DBBM) for lateral window sinus floor elevation (LSFE) with simultaneous implant placement. Materials and Methods: This retrospective cohort study involved 72 patients who underwent LSFE using low-temperature sintered cancellous bone-derived DBBM (C-DBBM) or high-temperature two-step sintered epiphyseal-derived DBBM (E-DBBM). Cone-beam computed tomography (CBCT) was acquired preoperatively, immediately postoperatively, 6 months and 1-4 years post-surgery. Bone volume (BV), apical bone height (ABH), endo-sinus bone gain (ESBG), and crestal bone level (CBL) were evaluated through three-dimensional fitting and superimposition. Linear mixed models (LMM) were employed to analyze factors influencing the reduction of BV (Delta BV) and ESBG (Delta ESBG). Results: The E-DBBM group showed no significant change in BV 1-4 years post-surgery, while the C-DBBM group demonstrated a significant reduction (p = .006) with volume stability of 85.86%. Bone height in the E-DBBM group increased at 6 months and subsequently decreased at 1-4 years (p = .003). In the C-DBBM group, it decreased at 6 months (p = .014), then further decreased at 1-4 years (p = .001). Delta ESBG was lower in the E-DBBM group than the C-DBBM group from immediate postoperative to 1-4 years (p = .009). LMM showed graft material type was the primary factor influencing Delta BV (p = .026) and Delta ESBG (p = .003). Conclusions: Within the limitations of this study, both types of DBBM could achieve favorable clinical outcomes. E-DBBM demonstrated enhanced stability in maintaining bone volume and height.
Comparative Evaluation of Endo-sinus Bone Augmentation after Osteotome Sinus Floor Elevation Without Grafting Using Two Radiographic Methods.

Abstract:PURPOSE Using panoramic radiograph and CBCT to compare the measurement errors of the residual bone height in the same site before implant placement in the posterior maxilla and endo-sinus bone gain after osteotome sinus floor elevation without grafting, and to evaluate endo-sinus bone augmentation before stage-two surgery with CBCT. Also, some related factors of new bone formation in the sinus were analyzed, such as small bone block elevated by osteotome at the implant apex intraoperatively. MATERIALS AND METHODS Patients were enrolled in the retrospective study; dental implants were placed in the maxillary posterior region using osteotome sinus floor elevation without grafting. The panoramic radiograph and CBCT were taken preoperatively, immediate postoperatively, and before the stage-two surgery. Endo-sinus bone augmentation was evaluated. A generalized linear model was made to explore the related factors of endo-sinus bone gain. Also, some clinical indexes, such as sinus membrane perforation rate, implant success, and failure criteria were assessed. RESULTS One hundred two dental implants were inserted in 91 patients. Preoperatively, the mean residual bone height was 8.53 ± 1.76 mm and 7.87 ± 1.45 mm measured using a panoramic radiograph and CBCT, respectively, with significant statistical differences (P < .05). Endo-sinus bone gains of 1.31 °æ 1.05 mm by panoramic radiograph and 1.80 ± 1.72 mm by CBCT were observed, with statistically significant differences (P < .05). The final endosinus bone gain was positively correlated with the implant protrusion length at baseline without any other relevant factors, such as elevated small bone block. CONCLUSION Measurement errors could be relatively minimized when using CBCT. Using the osteotome sinus floor elevation technique without grafting, approximately 2 mm of endo-sinus bone could be acquired. The final endo-sinus bone gain was positively correlated with implant protrusion length at baseline. Small bone block elevated intraoperatively did not gain more endo-sinus bone.
Sinus lift grafting with anorganic bovine bone vs 50% autologous bone mixed with 50% anorganic bovine bone: 2 years after loading results from a randomised controlled trial

Abstract:Purpose: To compare the outcome of implants inserted in maxillary sinuses augmented with anorganic bovine bone (ABB) grafts vs mixed 50% ABB and 50% autologous bone graft, using a lateral window approach. Materials and methods: This study was designed as a randomised controlled trial of parallel groups. Patients in need of an implant-supported prosthesis in a maxillary posterior area with a residual alveolar bone height no greater than 4 mm (range 0-4 mm) were recruited for lateral sinus grafting. Patients were randomly allocated to receive 50% ABB and 50% autogenous bone (group A) or 100% ABB (group B). After 7 months, tapered implants were inserted with an insertion torque between 20 and 45 Ncm. After 3 months, implants were loaded with screw-retained temporary crowns. Definitive crowns were delivered 3 months later. Outcome measures were implant survival, complications, radiographic marginal bone-level changes, probing pocket depths (PPD) and bleeding on probing (BOP). Clinical data were collected at definitive prosthesis delivery, 1 and 2 years after loading. Results: Thirty-two consecutive patients were treated with 32 sinus lift procedures (16 group A, 16 group B). A total of 46 implants were installed. No patient dropped out. No crown/implant failed by the end of the study. Three complications (one sinus membrane perforation and two chipping of the ceramic) were observed in three patients in group A, vs none in group B (RR 0.81; 95% CI 0.64 - 1.03 mm; P = 0.225). At the 2-year after final loading follow-up, the mean marginal bone loss was 1.18 ± 0.50 mm (95% CI 0.95 - 1.45 mm) in group A and 1.28 ± 0.48 mm (95% CI 0.97 - 1.43 mm) in group B, with no statistically significant differences between the two groups (difference 0.11 ± 0.22 mm; 95% CI -0.06 - 0.16 mm; P = 0.586). At the same follow-up, the mean PPD value was 2.70 ± 0.39 for group A and 2.54 ± 0.66 for group B, with no statistically significant difference between groups (difference 0.17 ± 0.39 mm; 95% CI 0.06 - 0.32 mm; P = 0.456), while the mean BOP value was 1.21 ± 0.79 for group A and 1.28 ± 0.68 for group B, (difference: 0.06 ± 0.49 mm; 95% CI -0.23 - 0.25 mm; P = 0.297). Conclusions: Within the limitations of this study, the present data seem to confirm the hypothesis that the clinical outcome of implants inserted in sinuses grafted with ABB vs implants inserted in sinuses grafted with mixed 50% ABB and 50% autologous bone are comparable. At the 5-year follow-up, the mean PPD value was 3.20 ± 0.44 mm for group A and 3.32 ± 0.49 mm for group B (difference 0.12 ± 0.43 mm; 95% CI 0.02-0.22 mm; P = 0.672). At the 2-year follow-up, the mean BOP value was 1.21 ± 0.79 for group A and 1.28 ± 0.68 for group B (difference 0.06 ± 0.49; 95% CI -0.23-0.25; P = 0.297), and at the 5-year follow-up, the mean BOP value was 1.77 ± 0.62 for group A and 1.91 ± 0.48 for group B (difference: 0.14 ± 0.51; 95% CI -0.05-0.33; P = 0.492). Conclusions: Within the limitations of this study, the present data confirm similar clinical outcomes of implants inserted in sinuses grafted with ABB versus implants inserted in sinuses grafted with mixed 50% ABB and 50% autologous bone.
The Influence of Residual Alveolar Bone Height on Graft Composition after Maxillary Sinus Augmentation Using Two Different Xenografts: A Histomorphometric Comparative Study

Abstract:Aim: To evaluate the hypothesis of a correlation between the preoperative residual alveolar bone height (RBH) and graft maturation after maxillary sinus floor augmentation procedures using two different bone substitutes. Methods: A total of 20 patients who underwent unilateral maxillary sinus floor augmentation with either mineralized deproteinized bovine bone (DBBM) or a xenograft enriched with polymer and gelatin (NBS) were included in this prospective study. Six months after sinus surgery, bone biopsies were harvested with a 3.2 mm diameter trephine bur, prior to dental implant placement. Histomorphometric analysis was performed, and the results were correlated with the individual RBH. Implants were loaded after 5 months of insertion, and 1-year implant success and marginal bone level change were assessed. Results: RBH was 2.17 ± 1.11 mm (range 0.5–3.5 mm) and 2.14 ± 0.72 mm (range 0.5–3.0 mm) in the NBS and DBBM group, respectively. The biopsy analyses for the DBBM group showed woven bone increases by 5.08% per 1-mm increment of RBH; medullary spaces decreased by 9.02%, osteoid decreased by 4.4%, residual biomaterial decreased by 0.34%, and lamellar bone increased by 5.68% per 1-mm increase of RBH. In the NBS group, samples showed woven bone increases by 8.08% per 1-mm increase of RBH; medullary spaces decreased by 0.38%; osteoid increased by 1.34%, residual biomaterial decreased by 0.58%, and lamellar bone decreased by 5.50% per 1-mm increase of RBH. There was no statistically significant difference in the correlation between RBH and lamellar bone, woven bone, and osteoid, independently of the material used. Implant success was 100% in both groups, and marginal bone loss was 1.02 ± 0.42 mm in DBBM and 0.95 ± 0.31 mm in the NBS group after the 1-year follow-up. Conclusion: In spite of the absence of significance, the observed trend for woven bone to increase and medullary spaces to decrease when RBH increases deserves attention. Residual bone dimension might be a determinant in the bone graft maturation after maxillary sinus augmentation.

Influence of Lateral Windows with Decreased Vertical Height Following Maxillary Sinus Floor Augmentation: A 1-Year Clinical and Radiographic Study.

Abstract:PURPOSETo verify whether lateral sinus floor elevation (LSFE) with the vertical height of the osteotomy decreased to 3 to 5 mm can achieve improvement compared with a conventional lateral window in terms of implant survival, graft stability, and related complications.MATERIALS AND METHODSOf the 49 patients who were selected for inclusion in this retrospective study, 25 patients were in the test group and 24 were in the control group based on the vertical height of the lateral window. Each patient received the LSFE treatment using deproteinized bovine bone mineral (DBBM) based on the two different vertical heights of the lateral window (test group: 3 to 5 mm; control group: 6 to 8 mm in vertical height). Residual bone dimensions were measured preoperatively, and graft dimensions were measured immediately after the procedure (baseline), 6 months (T1), and 1 year (T2) postoperatively on the basis of cone beam computed tomography (CBCT) scans. CBCT scan data at the baseline were analyzed with image processing software to evaluate the volume of augmented material, and they were transformed into three-dimensional reconstruction images to calibrate the window dimension. Implant failure and recorded intraoperative and postoperative complications were counted.RESULTSA prominent augmented graft height was obtained in both groups (test group: 9.48 ± 3.27 mm; control group: 10.44 ± 3.01 mm) with no significant difference; likewise, no difference was observed concerning augmented graft volume. Neither cumulative survival rate (CSR) nor augmented graft height decrement had any statistical intergroup difference during each follow-up interval. CSRs were detected as 100% for the test group and 94.28% and 91.67% for the control group based on implants and patients, respectively. Intraoperative membrane perforation was reported in two cases with a few nosebleeds the first night in the test group, while one case occurred in the control group. All the perforations were successfully sealed intraoperatively by a collagen membrane.CONCLUSIONOsteotomy of LSFE with decreased vertical height yielded similar results to the control group with conventional dimensions in terms of implant survival, graft stability, and related complications with 1 year of follow-up. Notwithstanding, a prospective split-mouth randomized controlled trial with long-term observation is imperative to further investigate the influence of decreased vertical height of the lateral window on maturation and consolidation of DBBM.
Clinical evaluation of maxillary sinus floor elevation with or without bone grafts: a systematic review and meta-analysis of randomised controlled trials with trial sequential analysis

Abstract:Introduction Our goal was to systematically review the current evidence comparing the relative effectiveness of two maxillary sinus floor elevation (MSFE) approaches (internal and external) without bone grafts with that of conventional/grafted MSFE in patients undergoing implantation in the posterior maxilla. Material and Methods Medical databases (PubMed/Medline, Embase, Web of Science, and Cochrane Library) were searched for randomised controlled trials published between January 1980 and May 2023. A manual search of implant-related journals was also performed. Studies published in English that reported the clinical outcomes of MSFE with or without bone material were included. The risk of bias was assessed using the Cochrane Handbook Risk Assessment Tool. Meta-analyses and trial sequence analyses were performed on the included trials. Meta-regression analysis was performed using pre-selected covariates to account for substantial heterogeneity. The certainty of evidence for clinical outcomes was assessed using GRADEpro GDT online (Guideline Development Tool). Results Seventeen studies, including 547 sinuses and 696 implants, were pooled for the meta-analysis. The meta-analysis showed no statistically significant difference between MSFE without bone grafts and conventional MSFE in terms of the implant survival rate in the short term (n = 11, I2 = 0%, risk difference (RD): 0.03, 95% confidence intervals (CI): –0.01–0.07, p = 0.17, required information size (RIS) = 307). Although conventional MSFE had a higher endo-sinus bone gain (n = 13, I2 = 89%, weighted mean difference (WMD): –1.24, 95% CI: –1.91– –0.57, p = 0.0003, RIS = 461), this was not a determining factor in implant survival. No difference in perforation (n = 13, I2 = 0%, RD = 0.03, 95% CI: –0.02–0.09, p = 0.99, RIS = 223) and marginal bone loss (n = 4, I2 = 0%, WMD = 0.05, 95% CI: –0.14–0.23, p = 0.62, no RIS) was detected between the two groups using meta-analysis. The pooled results of the implant stability quotient between the two groups were not robust on sensitivity analysis. Because of the limited studies reporting on the visual analogue scale, surgical time, treatment costs, and bone density, qualitative analysis was conducted for these outcomes. Conclusions This systematic review revealed that both non-graft and grafted MSFE had high implant survival rates. Owing to the moderate strength of the evidence and short-term follow-up, the results should be interpreted with caution.

Radiographic Assessment of Deproteinized Bovine Bone Mineral (DBBM) and Collagen-Stabilized DBBM for Transalveolar Sinus Floor Elevation: A 2-Year Retrospective Cohort Study

Abstract:Background: Theoretically, collagen-stabilized deproteinized bovine bone mineral (DBBM-C) has better operability compared with DBBM. DBBM-C avoids dispersing during the transalveolar sinus floor elevation (TSFE) because of its block shape. Purpose: To evaluate radiological changes of using DBBM-C in TSFE. Materials and Methods: Patients who received TSFE using DBBM (Bio-Oss (R)) or DBBM-C (Bio-Oss (R) collagen) with simultaneous implantation were recruited. Graft bone height apically (aGH), endo-sinus bone gain (ESBG), and crest bone level (CBL) were assessed through panoramic radiograph and cone beam computed tomography (CBCT). Results: A total of 138 patients (138 implants) were retrospectively enrolled. After 2 years of implantation, the incidence of postoperative complications was 4.2% (95% CI: 0.9%-11.7%) and 4.5% (95% CI: 0.9%-12.7%) for DBBM and DBBM-C groups, respectively. Measured in panoramic radiograph, Delta aGH of DBBM-C (1.8 mm, SD: 1.4, 95% CI: 1.2-2.4, P = 0.044) group was significantly higher than that of DBBM (1.2 mm, SD: 1.4, 95% CI: 0.7-1.7) after 24 months. No significant differences for Delta CBL were noted during the entire observation period. Measured through CBCT, ESBG was 5.0 (SD: 1.8, 95% CI: 4.1-5.8) mm in DBBM group and 4.6 (SD: 1.6, 95% CI: 3.9-5.3) mm in DBBM-C group 24 months after implantation. The aGH value of DBBM-C group was significantly higher compared with DBBM in CBCT (OR = 1.4, 95% CI: 1.1-1.9, P = 0.020). Conclusions: DBBM-C could achieve similar bone generation as DBBM in TSFE. Both materials could maintain aGH, ESBG, and CBL relatively stable 2 years after implantation.
Repositioning of the Bone Window in Lateral Sinus Floor Elevation with Simultaneous Implant Placement: A Retrospective Radiographic Study.

Abstract:Objectives To retrospectively evaluate whether repositioning the bone window leads to a better outcome of three-dimensional sinus augmentation in lateral sinus floor elevation (LSFE) with simultaneous implant placement. Methods 34 patients with a total of 40 implants (14: test group, 26: control group) receiving LSFE with simultaneous implant placement were included in this retrospective research. CBCT images were taken before surgery, immediately and 6 months after surgery. The two-dimensional augmentation parameters, including apical bone height (ABH), endo-sinus bone gain (ESBG), and palatal/buccal bone height (PBH/BBH), and three-dimensional parameters, including augmentation volume (AV) and palatal/buccal augmentation volume (PAV/BAV), were measured. The lateral defect length (LDL) and lateral window length (LWL) were also measured to evaluate the lateral antrostomy recovery. Results At the 6-month follow-up, the reduction rates at ABH, ESBG, and BBH of the test group (ABH: 10.41% +/- 30.30%, ESBG: 2.55% +/- 8.91%, BBH: 2.50% +/- 8.65%) were significantly lower than those of the control group (ABH: 25.10% +/- 22.02%, ESBG: 11.47% +/- 9.79%, BBH: 7.10% +/- 5.37%; p < .05). In addition, the test group showed better three-dimensional augmentation stability on the buccal side (BAV reduction: 15.51% +/- 10.86% vs. 27.15% +/- 12.61%; p < .05). Moreover, the LDL/LWL ratio of the test group was significantly lower than that of the control group (p < .05). Conclusion Within the limitations of this study, repositioning of the bone window in LSFE with simultaneous implant placement could contribute to endo-sinus augmentation stability on the buccal side at the 6-month follow-up. Osteotome sinus floor elevation with or without grafting: a 3-year randomized controlled clinical trial.

Abstract:Aims To evaluate the clinical and radiographic results of dental implant placed using osteotome sinus floor elevation (OSFE) with and without simultaneous grafting. Materials & Methods Forty-five patients were randomly assigned into two groups: Group1: OSFE with deproteinized bovine bone mineral (DBBM) mixed with autogenous bone chips, and Group2: OSFE without grafting. The endo-sinus bone gain (ESBG) was assessed on radiographs at 6, 12, 24, 36months following surgery as primary outcome measurement. Implant survivals and marginal bone loss (MBL) were assessed as secondary outcome measurements. Results Twenty-one implants in Group1 and 20 implants in Group2 were analysed. The residual bone height (RBH) was 4.63 +/- 1.31mm in average (4.67 +/- 1.18mm for Group1 and 4.58 +/- 1.47mm for Group2). The 3-year cumulative survival rates of implants were 95.2% for Group1 and 95.0% for Group2. The ESBG in Group1 reduced from 5.66 +/- 0.99mm at 6months to 3.17 +/- 1.95mm at 36months, whereas the ESBG in Group2 increased from 2.06 +/- 1.01mm at 6months to 3.07 +/- 1.68mm at 36months. The MBL after 3years was 1.33 +/- 0.46mm in Group1 and 1.38 +/- 0.23mm in Group2. Conclusions OSFE and simultaneous implant installation with and without grafting both resulted in predictable results. The application of grafting materials has no significant advantage in terms of clinical success.

Transcrestal maxillary sinus floor elevation with injectable xenogeneic bone substitute in gel form: A clinical, radiological and histological analysis

Abstract:Purpose: This retrospective study measured the increase in bone tissue using the transcrestal maxillary sinus floor elevation with injectable xenogeneic bone substitute in gel form with simultaneous implant placement. This procedure allows elevation of the sinus floor atraumatically, reducing the risk of perforation of the Schneiderian membrane. Methods: 52 subjects needing unilateral sinus floor elevation, with a residual crestal height from 2 mm to 5 mm, and a request for at least one implant-prosthetic rehabilitation in the posterior maxillary area were enrolled. Transcrestal maxillary sinus floor elevation was performed with injectable xenogeneic bone substitute in gel form. The sinus elevation was measured after the surgery and 6 months later with a CBCT. Average values were calculated for each measure. Results: 46 implants were simultaneously placed; six implants were placed after 4 months because of the lack of primary stability. All the placed implants, with a follow-up varying from 3 to 5 years after loading, osseointegrated successfully resulting in a survival rate of 100%. Average pre-operative bone height was 4.2 mm while after the surgery the average value reached was 10.1 mm with an average value of new bone gain of 6.43 mm. Histological analysis revealed the presence of 33.2% of vital bone. Clinical significance: Transcrestal sinus floor elevation with injectable xenogenic bone substitute in gel form is a minimally invasive technique that can reduce the incidence of Schneider membrane perforations, making a widely used method, such as sinus floor elevation, safer and less operator dependent.

Abstract:OBJECTIVES To compare clinical and radiographic outcomes between transcrestal sinus floor elevation (TSFE) and lateral sinus floor elevation (LSFE) approaches of simultaneous implant placement in atrophic maxilla. MATERIALS AND METHODS Patients with a residual bone height (RBH) ≤6 mm were enrolled and randomly assigned to TSFE and LSFE groups. Patients in both groups simultaneously underwent sinus floor elevation with bovine-derived xenograft and implant placement. Clinical and radiographic results were evaluated immediately after surgery and after 6, 12, 18, and 24 months. The endo-sinus bone gain (ESBG), apical implant bone height (ABH), endo-sinus bone-implant contact rate (EBICR), and crestal bone level (CBL) were assessed using panoramic radiographs. RESULTS Forty-one implants (TSFE: 21, LSFE: 20) were placed in cases with a mean RBH of 3.77 ±1.16 mm. All implants obtained clinical success and satisfactory ESBG at 24 months. No significant differences were found in ESBG and ABH between two groups immediately after surgery, but LSFE group showed significantly higher values than TSFE group thereafter. Grafts in TSFE group reached stability 6 months earlier than that in LSFE group. In both groups, EBICR was almost 100%, and CBL showed no detectable changes. CONCLUSIONS LSFE can achieve higher ESBG 2 years after surgery. Otherwise, TSFE could be an alternative to LSFE, when the access for lateral window preparation is limited. Both approaches were highly predictable for RBH ≤6 mm during 24-month observation period for the implants placed simultaneously.

Abstract:Objective: The combination of calcium phosphate with blood-derived growth factors (BDGF) has been widely used in bone regeneration procedures although its benefits are still unclear. The purpose of this study was to evaluate whether or not BDGF improves the efficacy of a modified carbonated calcium phosphate biomaterial in sinus floor augmentation. Material and methods: Ten patients underwent 20 sinus floor augmentation procedures using nanostructured carbonated hydroxyapatite (cHA) microspheres alone or associated with BDGF in a randomized controlled clinical trial. The in vitro release of growth factors was assessed by an elution assay. Bone grafts were randomly implanted in the right and left maxillary sinuses of each participant, associated either with a 0.9% saline solution or BDGF. Bone gain was evaluated through cone beam tomography after 180 days. Results: Nine women and one man composed the sample. The blood-derived concentrates were able to release high levels of growth factors and cytokines. A significant clinical advantage was observed in the use of the BDGF after fibrin polymerization around the biomaterial microspheres, optimizing the surgical procedures, thereby reducing the time and displacement, and improving the adaptation of the biomaterial in the maxillary sinus. No synergistic effect was observed in bone formation when cHA was associated with BDGF (p > 0.05). Conclusion: Equivalent new bone formation was observed for cHA in the presence or absence of the BDGF concentrate in bilateral sinus floor elevation after 6 months. Blood-derived growth factors did not improve bone repair when associated with calcium phosphate in sinus lift procedures.

Abstract:OBJECTIVE: The aim of this population-based retrospective study was to compare the osteogenic effect of newly formed bone after maxillary sinus floor elevation (MSFE) and simultaneous implantation with or without bone grafts by quantitatively analyzing trabecular bone parameters.METHODOLOGY: A total of 100 patients with missing posterior maxillary teeth who required MSFE and implantation were included in this study. Patients were divided into two groups: the non-graft group (n=50) and the graft group (n=50). Radiographic parameters were measured using cone beam computed tomography (CBCT), and the quality of newly formed bone was analyzed by assessing trabecular bone parameters using CTAn (CTAnalyzer, SkyScan, Antwerp, Belgium) software.RESULTS: In the selected regions of interest, the non-graft group showed greater bone volume/total volume (BV/TV), bone surface/total volume (BS/TV), trabecular number (Tb. N), and trabecular thickness (Tb. Th) than the graft group (p<0.001). The non-graft group showed lower trabecular separation (Tb. Sp) than the graft group (p<0.001). The incidence of perforation and bleeding was higher in the graft group than in the non-graft group (p<0.001), but infection did not significantly differ between groups (p>0.05). Compared to the graft group, the non-graft group showed lower postoperative bone height, gained bone height and apical bone height (p<0.001).CONCLUSION: MSFE with and without bone grafts can significantly improve bone formation. In MSFE, the use of bone grafts hinders the formation of good quality bone, whereas the absence of bone grafts can generate good bone quality and limited bone mass.

Abstract:Abstract Purpose To evaluate the clinical effects of concentrated growth factors (CGFs) combined with bone substitutes for alveolar ridge preservation (ARP) in the maxillary molar area. Methods Thirty-six patients who underwent extraction of the upper molars were recruited and randomly divided into three groups: 1. Grafted with CGFs combined with deproteinized bovine bone mineral (DBBM) and covered with CGFs membrane (CGFs/DBBM group), 2. Grafted with DBBM alone and covered with collagen membrane (DBBM group), 3. Control group spontaneous healing. The area of the alveolar bone in center (C-), mesial (M-) and distal (D-) section was compared with preoperative in radiography. Bone cores were obtained for histopathology observation and comparison. Results In C-, M- and D-section, the alveolar ridge area in all three groups was significantly reduced at 8 months postoperatively compared to the baseline ( P < 0.05). The alveolar ridge area declines in the CGFs/DBBM group (C-12.75 ± 2.22 mm 2 , M-14.69 ± 2.82 mm 2 , D-16.95 ± 4.17 mm 2 ) and DBBM group (C-14.08 ± 2.51 mm 2 , M-15.42 ± 3.47 mm 2 , D-16.09 ± 3.97 mm 2 ) were non-significant differences. They were significantly less than the decline in the control group (C-45.04 ± 8.38 mm 2 M-31.98 ± 8.34 mm 2 , D-31.85 ± 8.52 mm 2 ) ( P < 0.05). The percentage of newly formed bone in the CGFs/DBBM group (41.99 ± 12.99%) was significantly greater than that in DBBM group (30.68 ± 10.95%) ( P < 0.05). The percentage of residual materials in the CGFs/DBBM group (16.19 ± 6.63%) was significantly less than that in the DBBM group (28.35 ± 11.70%) ( P < 0.05). Conclusion Combined application of CGFs and DBBM effectively reduced the resorption of alveolar ridge and resulted in more newly formed bone than the use of DBBM with collagen membranes.

Abstract:I read with interest the recent Editorial published by Forconi et al. [6]. Hyperviscosity is classically admitted to disturb blood flow and to represent a risk factor for the development of cardiovascular disease. However, recent experimental results tackle this view and support the hypothesis that hyperviscosity might represent a potential compensatory mechanism in the regulation of peripheral resistances and tissue perfusion in the setting of disease [6]. The resulting increased wall shear stress may stimulate nitric oxide production by endothelial cells to create a vasodilatory compensation [8]. Exercise induces important hematological/hemorheological changes such as an increase of hematocrit and blood viscosity [1]. Connes et al. [3] observed that athletes who developed exercise induced hypoxemia (EIH; i.e. with a significant decrease of arterial pressure in oxygen of more than 4% as compared to resting value) exhibited a greater increase of blood viscosity during exercise than those without EIH. It was hypothesized that the higher blood viscosity reached might increase the pulmonary artery pressure and damage the alveolar-capillary membrane through the stress-failure mechanism resulting in pulmonary oxygen diffusion impairment [2]. However, recent data demonstrated that muscle oxygenation is greater in EIH athletes than in athletes without EIH during exercise and that is the greater muscle oxygenation that could explain the decrease of arterial pressure in oxygen [7]. The mechanisms at the origin of this greater muscle oxygenation are unknown. We recently studied hemodynamical, respiratory and hemorheological changes during exercise, independently of EIH, and we found that the increase of blood viscosity between rest and exercise was positively correlated with the enlargement of the arteriovenous oxygen content difference suggesting that the greater the increase of blood viscosity, the greater muscle oxygen consumption [5]. From a classical view point it could seem surprising but, in fact, the increase of blood viscosity, in association with the increase of blood flow, may increase shear stress and stimulate vasodilation through a NO-dependent mechanism and allow adequate oxygen delivery to tissues. The same mechanism may be advanced in athletes prone to EIH. The higher blood viscosity, and the resulting higher shear stress, during exercise might stimulate the endothelial production of nitric oxide and increase the functional capillary density at the muscle level leading to better tissue oxygenation. Another paradox concerns the case of sickle cell trait (SCT) carriers. Several studies reported elevated resting and exercising blood viscosity (see [4] for review) and it is usually advanced that it could reduce the aerobic physical fitness in this population. Abstract:Background: Implant failures are more common when multiple missing posterior teeth need lateral sinus floor elevation owing to inadequate tissue maturation after grafting. Effects of lateral window dimensions on vital bone formation have rarely been compared. Purpose: To compare endo-sinus bone formation between two- and solo-window techniques to rehabilitate multiple missing posterior teeth that need substantial augmentation. Methods and materials: Patients with severely atrophic posterior maxilla were randomized to receive lateral sinus floor elevation via solo or two bony windows. Bone core specimens harvested from lateral aspect of the augmentation sites were histomorphometrically analyzed. Proportions of mineralized bone (MB), bone substitute materials (BS), and nonmineralized tissue (NMT) were quantified. Results: Twenty-one patients underwent 23 maxillary sinus augmentations. One patient in each group dropped out during the follow-up period. Lateral window dimensions were 81.65 +/- 4.59 and 118.04 +/- 19.53 mm(2) in the test and control groups, respectively. Histomorphometric analysis revealed mean MB of 42.32%+/- 13.07% and 26.00%+/- 15.23%, BS of 40.34%+/- 9.52% and 60.03%+/- 10.13%, and NMT of 18.14%+/- 14.24% and 14.75%+/- 10.38% in test and control groups, respectively, with significant differences. Conclusion: The two-window technique could facilitate faster maturation and consolidation of the grafted volume and is an effective alternative for rehabilitation of severely atrophic posterior maxilla with multiple missing posterior teeth.
Does a Severely Resorbed Subantral Ridge Decrease Long-Term Implant Survival Rate with Sinus Floor Augmentation?

Abstract:Purpose: To identify the impact of residual bone height on 5-year implant survival and prosthetic complication rates in patients who underwent maxillary sinus grafting. Materials and Methods: A total of 87 consecutive patients were treated with 104 lateral approach maxillary sinus floor augmentation procedures with 100% deproteinized bovine bone and received 169 implants. The analysis considered patient age, sex, time of implant placement, and residual bone height. Patients with < 3 mm residual bone height were assigned to the study group; otherwise, they were placed in the control group. Results: The mean follow-up was 68.2 months (0 to 103 months). The mean residual bone height was 1.8 mm in the study group and 4.1 mm in the control group. The 5-year implant survival and prosthetic complication rates were, respectively, 97.4% and 8.0% in the study group and 100% and 12.5% in the control group. Residual bone height, sex, age, and time of implant placement were not significant factors for the 5-year implant survival or prosthetic complication rate. The lateral bone wall was significantly thinner in the study group. The grafted bone height reduction was significantly different at 6 months and 2 years postoperation in both groups, but there was no difference in the change in grafted bone height reduction over time between the two groups. Condusion: A residual bone height < 3 mm did not impact the survival rates of implants placed in grafted maxillary sinuses or the prosthetic complication rate after 5 years of functional loading.
