Abstract:Purpose: To compare the outcome of implants inserted in maxillary sinuses augmented with anorganic bovine bone (ABB) grafts vs mixed 50% ABB and 50% autologous bone graft, using a lateral window approach. Materials and methods: This study was designed as a randomised controlled trial of parallel groups. Patients in need of an implant-supported prosthesis in a maxillary posterior area with a residual alveolar bone height no greater than 4 mm (range 0-4 mm) were recruited for lateral sinus grafting. Patients were randomly allocated to receive 50% ABB and 50% autogenous bone (group A) or 100% ABB (group B). After 7 months, tapered implants were inserted with an insertion torque between 20 and 45 Ncm. After 3 months, implants were loaded with screw-retained temporary crowns. Definitive crowns were delivered 3 months later. Outcome measures were implant survival, complications, radiographic marginal bone-level changes, probing pocket depths (PPD) and bleeding on probing (BOP). Clinical data were collected at definitive prosthesis delivery, 1 and 2 years after loading. Results: Thirty-two consecutive patients were treated with 32 sinus lift procedures (16 group A, 16 group B). A total of 46 implants were installed. No patient dropped out. No crown/implant failed by the end of the study. Three complications (one sinus membrane perforation and two chipping of the ceramic) were observed in three patients in group A, vs none in group B (RR 0.81; 95% CI 0.64 - 1.03 mm; P = 0.225). At the 2-year after final loading follow-up, the mean marginal bone loss was 1.18 ± 0.50 mm (95% CI 0.95 - 1.45 mm) in group A and 1.28 ± 0.48 mm (95% CI 0.97 - 1.43 mm) in group B, with no statistically significant differences between the two groups (difference 0.11 ± 0.22 mm; 95% CI -0.06 - 0.16 mm; P = 0.586). At the same follow-up, the mean PPD value was 2.70 ± 0.39 for group A and 2.54 ± 0.66 for group B, with no statistically significant difference between groups (difference 0.17 ± 0.39 mm; 95% CI 0.06 - 0.32 mm; P = 0.456), while the mean BOP value was 1.21 ± 0.79 for group A and 1.28 ± 0.68 for group B, (difference: 0.06 ± 0.49 mm; 95% CI -0.23 - 0.25 mm; P = 0.297). Conclusions: Within the limitations of this study, the present data seem to confirm the hypothesis that the clinical outcome of implants inserted in sinuses grafted with ABB vs implants inserted in sinuses grafted with mixed 50% ABB and 50% autologous bone are comparable. Conclusions: Within the limitations of this study, the present data confirm similar clinical outcomes of implants inserted in sinuses grafted with ABB versus implants inserted in sinuses grafted with mixed 50% ABB and 50% autologous bone.
Changes in Bone Graft Height and Influencing Factors after Sinus Floor Augmentation by Using the Lateral Window Approach: A Clinical Retrospective Study of 1 to 2 Years.

Abstract:STATEMENT OF PROBLEM:Recent systematic reviews have reported resorption of bone grafts after augmentation, but the influencing factors are numerous and uncertain. Different brands of bone graft and other factors may affect the bone formation effect after sinus floor augmentation.PURPOSE:The purpose of this retrospective clinical study was to evaluate the graft material height changes after sinus floor augmentation with cone beam computed tomography (CBCT) and to investigate the potential influencing factors related to graft resorption.MATERIAL AND METHODS:Four midsagittal cut CBCT images of 157 posterior maxillary implants after maxillary sinus floor elevation by using the lateral window approach with bone grafts from 116 patients were obtained. Four CBCT scans had been performed immediately (T0), 6 months (T1) and 12 months after bone grafting (T2), and 1.5 to 2 years after treatment (T3), and the distance between the implant platform and the grafted mucosa of the maxillary sinus floor was measured at 3 locations. Correlation coefficients of these parameters were calculated. A linear mixed model was used to investigate potential factors influencing graft height reduction, including the patient's sex, age, smoking status, periodontal status, graft brand, implant site, implant level, placement time, and residual bone height.RESULTS:Mean ±standard deviation graft height at the mesial side of the implant (MeGH) was significantly decreased by 0.32 ±0.88 mm in the first 6 months, was gradually reduced after 6 months (0.26 ±0.43 mm), and then 1 to 2 years later increased to 0.39 ±0.97 mm. The changes in graft height at the center of the implant (CeGH) and graft height at the distal side (DiGH) groups were similar to those in the MeGH group. A significantly positive correlation was found among the height alterations in 3 locations (rs=0.954, P<.001). The linear mixed model showed that smoking was more likely to cause graft height reduction in MeGH (P=.034).CONCLUSIONS:Graft height significantly decreased after maxillary sinus augmentation over 1 to 2 years. Smoking had a negative effect on graft height changes, while the 2 brands of graft and other factors had no significant effect.

Abstract:PURPOSE Using panoramic radiograph and CBCT to compare the measurement errors of the residual bone height in the same site before implant placement in the posterior maxilla and endo-sinus bone gain after osteotome sinus floor elevation without grafting, and to evaluate endo-sinus bone augmentation before stage-two surgery with CBCT. Also, some related factors of new bone formation in the sinus were analyzed, such as small bone block elevated by osteotome at the implant apex intraoperatively. MATERIALS AND METHODS Patients were enrolled in the retrospective study; dental implants were placed in the maxillary posterior region using osteotome sinus floor elevation without grafting. The panoramic radiograph and CBCT were taken preoperatively, immediate postoperatively, and before the stage-two surgery. Endo-sinus bone augmentation was evaluated. A generalized linear model was made to explore the related factors of endo-sinus bone gain. Also, some clinical indexes, such as sinus membrane perforation rate, implant success, and failure criteria were assessed. RESULTS One hundred two dental implants were inserted in 91 patients. Preoperatively, the mean residual bone height was 8.53 ± 1.76 mm and 7.87 ± 1.45 mm measured using a panoramic radiograph and CBCT, respectively, with significant statistical differences (P < .05). Endo-sinus bone gains of 1.31 °æ 1.05 mm by panoramic radiograph and 1.80 ± 1.72 mm by CBCT were observed, with statistically significant differences (P < .05). The final endosinus bone gain was positively correlated with the implant protrusion length at baseline without any other relevant factors, such as elevated small bone block. CONCLUSION Measurement errors could be relatively minimized when using CBCT. Using the osteotome sinus floor elevation technique without grafting, approximately 2 mm of endo-sinus bone could be acquired. The final endo-sinus bone gain was positively correlated with implant protrusion length at baseline. Small bone block elevated intraoperatively did not gain more endo-sinus bone.

Abstract:Objective The objective was to assess the one year implant treatment outcome and patient‐related outcome measures (PROMs) following maxillary sinus floor augmentation (MSFA) with autogenous bone graft (ABG) from the zygomatic buttress (control) compared with 1:1 mixture of ABG and anorganic porcine bone mineral (APBM) (Test I) or biphasic bone graft material (BBGM) (Test II). Materials and Methods Sixty healthy patients (34 females, 26 males) were randomly allocated to either control or test groups. Outcome measures included survival of suprastructures and implants, implant stability quotient, health status of peri‐implant tissue, peri‐implant marginal bone loss, frequency of complications, and PROMs using Oral Health Impact Profile‐14 combined with questionnaire assessing patient perception of peri‐implant soft tissue, prosthetic solution, implant function, and implant treatment outcome using visual analogue scale. Mean differences were expressed with standard deviation and 95% confidence interval. Level of significance was .05. The Influence of Residual Alveolar Bone Height on Graft Composition after Maxillary Sinus Augmentation Using Two Different Xenografts: A Histomorphometric Comparative Study

Abstract:Aim: To evaluate the hypothesis of a correlation between the preoperative residual alveolar bone height (RBH) and graft maturation after maxillary sinus floor augmentation procedures using two different bone substitutes. Methods: A total of 20 patients who underwent unilateral maxillary sinus floor augmentation with either mineralized deproteinized bovine bone (DBBM) or a xenograft enriched with polymer and gelatin (NBS) were included in this prospective study. Six months after sinus surgery, bone biopsies were harvested with a 3.2 mm diameter trephine bur, prior to dental implant placement. Histomorphometric analysis was performed, and the results were correlated with the individual RBH. Implants were loaded after 5 months of insertion, and 1-year implant success and marginal bone level change were assessed. Results: RBH was 2.17 ± 1.11 mm (range 0.5–3.5 mm) and 2.14 ± 0.72 mm (range 0.5–3.0 mm) in the NBS and DBBM group, respectively. The biopsy analyses for the DBBM group showed woven bone increases by 5.08% per 1-mm increment of RBH; medullary spaces decreased by 9.02%, osteoid decreased by 4.4%, residual biomaterial decreased by 0.34%, and lamellar bone increased by 5.68% per 1-mm increase of RBH. In the NBS group, samples showed woven bone increases by 8.08% per 1-mm increase of RBH; medullary spaces decreased by 0.38%; osteoid increased by 1.34%, residual biomaterial decreased by 0.58%, and lamellar bone decreased by 5.50% per 1-mm increase of RBH. There was no statistically significant difference in the correlation between RBH and lamellar bone, woven bone, and osteoid, independently of the material used. Implant success was 100% in both groups, and marginal bone loss was 1.02 ± 0.42 mm in DBBM and 0.95 ± 0.31 mm in the NBS group after the 1-year follow-up. Conclusion: In spite of the absence of significance, the observed trend for woven bone to increase and medullary spaces to decrease when RBH increases deserves attention. Residual bone dimension might be a determinant in the bone graft maturation after maxillary sinus augmentation.

Radiographic graft changes following maxillary sinus floor augmentation with autogenous bone compared with composite grafts: a one-year single-blind randomized controlled trial

Abstract:This study evaluated radiographic graft changes following maxillary sinus floor augmentation with (A) autogenous bone, (B) 1:1 autogenous bone and deproteinized porcine bone mineral, or (C) 1:1 autogenous bone and biphasic bone graft material. Sixty patients were randomly allocated to groups A, B, and C (20 in each). CBCT scans were obtained at enrolment, after surgery (T1), after prosthetic rehabilitation (T2), and 1 year after implant loading (T3). Significant decreases in graft volume (3D) and graft height (2D) from T1 to T3 were observed in all groups (P < 0.05). However, at T2 and T3, graft volume was significantly higher in group B than in groups A and C (P < 0.05), and graft height was significantly higher in group B than in group A (P < 0.05). Bone density increased significantly from T1 to T2 in all groups (P < 0.001). However, bone density was significantly higher in group B than in groups A and C, at T2 and T3 (P < 0.05). No significant correlation between graft volume or height and implant protrusion length or residual bone height was found. In conclusion, 1:1 autogenous bone and porcine bone resulted in significantly higher graft volume, height, and bone density when compared with autogenous bone or a 1:1 ratio of autogenous bone and biphasic bone. However the higher graft volume, height, and bone density did not appear to lead to improved outcomes at 1 year of functional loading when compared to the other groups.

Elevated blood viscosity during exercise: what are the consequences?

Abstract:I read with interest the recent Editorial published by Forconi et al. [6]. Hyperviscosity is classically admitted to disturb blood flow and to represent a risk factor for the development of cardiovascular disease. However, recent experimental results tackle this view and support the hypothesis that hyperviscosity might represent a potential compensatory mechanism in the regulation of peripheral resistances and tissue perfusion in the setting of disease [6]. The resulting increased wall shear stress may stimulate nitric oxide production by endothelial cells to create a vasodilatory compensation [8]. Exercise induces important hematological/hemorheological changes such as an increase of hematocrit and blood viscosity [1]. Connes et al. [3] observed that athletes who developed exercise induced hypoxemia (EIH; i.e. with a significant decrease of arterial pressure in oxygen of more than 4% as compared to resting value) exhibited a greater increase of blood viscosity during exercise than those without EIH. It was hypothesized that the higher blood viscosity reached might increase the pulmonary artery pressure and damage the alveolar-capillary membrane through the stress-failure mechanism resulting in pulmonary oxygen diffusion impairment [2]. However, recent data demonstrated that muscle oxygenation is greater in EIH athletes than in athletes without EIH during exercise and that is the greater muscle oxygenation that could explain the decrease of arterial pressure in oxygen [7]. The mechanisms at the origin of this greater muscle oxygenation are unknown. We recently studied hemodynamical, respiratory and hemorheological changes during exercise, independently of EIH, and we found that the increase of blood viscosity between rest and exercise was positively correlated with the enlargement of the arteriovenous oxygen content difference suggesting that the greater the increase of blood viscosity, the greater muscle oxygen consumption [5]. From a classical view point it could seem surprising but, in fact, the increase of blood viscosity, in association with the increase of blood flow, may increase shear stress and stimulate vasodilation through a NO-dependent mechanism and allow adequate oxygen delivery to tissues. The same mechanism may be advanced in athletes prone to EIH. The higher blood viscosity, and the resulting higher shear stress, during exercise might stimulate the endothelial production of nitric oxide and increase the functional capillary density at the muscle level leading to better tissue oxygenation. Another paradox concerns the case of sickle cell trait (SCT) carriers. Several studies reported elevated resting and exercising blood viscosity (see [4] for review) and it is usually advanced that it could reduce the aerobic physical fitness in this population. Abstract:Aims To evaluate the clinical and radiographic results of dental implant placed using osteotome sinus floor elevation (OSFE) with and without simultaneous grafting. Materials & Methods Forty-five patients were randomly assigned into two groups: Group1: OSFE with deproteinized bovine bone mineral (DBBM) mixed with autogenous bone chips, and Group2: OSFE without grafting. The endo-sinus bone gain (ESBG) was assessed on radiographs at 6, 12, 24, 36months following surgery as primary outcome measurement. Implant survivals and marginal bone loss (MBL) were assessed as secondary outcome measurements. Results Twenty-one implants in Group1 and 20 implants in Group2 were analysed. The residual bone height (RBH) was 4.63 +/- 1.31mm in average (4.67 +/- 1.18mm for Group1 and 4.58 +/- 1.47mm for Group2). The 3-year cumulative survival rates of implants were 95.2% for Group1 and 95.0% for Group2. The ESBG in Group1 reduced from 5.66 +/- 0.99mm at 6months to 3.17 +/- 1.95mm at 36months, whereas the ESBG in Group2 increased from 2.06 +/- 1.01mm at 6months to 3.07 +/- 1.68mm at 36months. The MBL after 3years was 1.33 +/- 0.46mm in Group1 and 1.38 +/- 0.23mm in Group2. Conclusions OSFE and simultaneous implant installation with and without grafting both resulted in predictable results. The application of grafting materials has no significant advantage in terms of clinical success.
Autogenous bone grafts in oral implantology-is it still a "gold standard"? A consecutive review of 279 patients with 456 clinical procedures

Abstract:Background: This study assessed the clinical outcomes of graft success rate and early implant survival rate after preprosthetic alveolar ridge reconstruction with autologous bone grafts. Methods: A consecutive retrospective study was conducted on all patients who were treated at the military outpatient clinic of the Department of Oral and Plastic Maxillofacial Surgery at the military hospital in Ulm (Germany) in the years of 2009 until 2011 with autologous bone transplantation prior to secondary implant insertion. Intraoral donor sites (crista zygomatico-alveolaris, ramus mandible, symphysis mandible, and anterior sinus wall) and extraoral donor site (iliac crest) were used. A total of 279 patients underwent after a healing period of 3-5 months routinely computer tomography scans followed by virtual implant planning. The implants were inserted using guided oral implantation as described by Naziri et al. All records of all the consecutive patients were reviewed according to patient age, history of periodontitis, smoking status, jaw area and dental situation, augmentation method, intra- and postoperative surgical complications, and surgeon's qualifications. Evaluated was the augmentation surgical outcome regarding bone graft loss and early implant loss postoperatively at the time of prosthodontic restauration as well a follow-up period of 2 years after loading. Results: A total of 279 patients underwent 456 autologous augmentation procedures in 546 edentulous areas. One hundred thirteen crista zygomatico-alveolaris grafts, 104 ramus mandible grafts, 11 symphysis grafts, 116 grafts from the anterior superior iliac crest, and 112 sinus lift augmentations with bone scrapes from the anterior facial wall had been performed. There was no drop out or loss of follow-up of any case that had been treated in our clinical center in this 3-year period. Four hundred thirty-six (95.6%) of the bone grafts healed successfully, and 20 grafts (4.4%) in 20 patients had been lost. Fourteen out of 20 patients with total graft failure were secondarily re-augmented, and six patients wished no further harvesting procedure. In the six patients, a partial graft resorption was detected at the time of implantation and additional simultaneous augmentation during implant insertion was necessary. No long-term nerve injury occurred. Five hundred twenty-five out of 546 initially planned implants in 259 patients could be inserted into successfully augmented areas, whereas 21 implants in 20 patients due to graft loss could not be inserted. A final rehabilitation as preplanned with dental implants was possible in 273 of the 279 patients. The early implant failure rate was 0.38% concerning two out of the 525 inserted implants which had to be removed before the prosthodontic restoration. Two implants after iliac crest augmentation were lost within a period of 2 years after loading, concerning a total implant survival rate after 2 years of occlusal loading rate of 99.6% after autologous bone augmentation prior to implant insertion. Conclusions: This review demonstrates the predictability of autologous bone material in alveolar ridge reconstructions prior to implant insertion, independent from donor and recipient site including even autologous bone chips for sinus elevation. Due to the low harvesting morbidity of autologous bone grafts, the clinical results of our study indicate that autologous bone grafts still remain the "gold standard" in alveolar ridge augmentation prior to oral implantation.
PRF versus xenograft in sinus augmentation in case of HA-coating implant placement: A 36-month retrospective study

Abstract:Background: Sinus lift with a simultaneous implant placement in the residual maxilla is a common technique used worldwide. Nevertheless, choosing an ideal grafting material remains an object of dispute. The use of an autologous blood-derived graft, known as platelet-rich fibrin (PRF), has not yet been recognized to be as good as xenografts and alloplastic materials. However, initial results have been promising. Objectives: To conduct a clinical and radiological comparison of implantation with a simultaneous sinus lift using xenograft or PRF clots. Material and methods: Thirty sinus lifts with simultaneous implantation were conducted using a lateral window approach and the tent pole technique, with xenograft (group 1 (G1)) or PRF (group 2 (G2)) as a filling material. To be included in the study, patients must have had an alveolar ridge height of 4-5 mm, no signs of inflammatory processes, good oral hygiene, and no other grafting procedures performed in region of implant insertion. In each case, the measurements taken were probing pocket depth (PPD), height of keratinized tissue (HKT), clinical attachment level (CAL), recession depth/width (RD/RW), and, on panoramic X-rays, marginal bone loss (MBL), grafted sinus high (GSH), and bone gain (BG). Preand post-operative treatment was applied to reduce the chance of infection. Results: During the study, 30 implants (hydroxyapatite-coated implants manufactured by SGS - 10 mm in length and 4.2 mm in diameter) were placed. The survival rate of implants in both groups was 100% with no implant mobility, pain, paresthesia, or inflammatory processes in the direct vicinity of the implants observed, except in 1 patient. After 36 months of follow-up, the radiological assessments for G1 were: GSH 4.5 mm, MBL 0.46 mm and BG 4.53 mm; and for G2: 3.4 mm, 0.6 mm and 3.4 mm, respectively. Results of the clinical measurements were for G1: HKT after 36 months (HKT36) 2.46 mm, CAL 0.47 mm and PPD 2 mm; and for G2: HKT36 3.13 mm, CAL 0.6 mm and PPD 2.07 mm. Conclusions: After 3 years of follow-up, the results of sinus lifting solely using PRF with simultaneous implantation were promising, especially in terms of soft tissue management. Therefore, PRF can be regarded as an alternative to previously used materials.

Abstract:The cuticle, a hydrophobic protective layer on the aerial parts of terrestrial plants, functions as a versatile defensive barrier to various biotic and abiotic stresses and also regulates water flow from the external environment.1 A biopolyester (cutin) and long-chain fatty acids (waxes) form the principal structural framework of the cuticle; the functional integrity of the cuticular layer depends on the outer 'epicuticular' layer as well as the blend consisting of the cutin biopolymer and 'intracuticular' waxes.2 Herein, we describe a comprehensive protocol to extract waxes exhaustively from commercial tomato (Solanum lycopersicum) fruit cuticles or to remove epicuticular and intracuticular waxes sequentially and selectively from the cuticle composite. The method of Jetter and Schäffer (2001) was adapted for the stepwise extraction of epicuticular and intracuticular waxes from the fruit cuticle.3,4 To monitor the process of sequential wax removal, solid-state cross-polarization magic-angle-spinning (CPMAS) 13C NMR spectroscopy was used in parallel with atomic force microscopy (AFM), providing molecular-level structural profiles of the bulk materials complemented by information on the microscale topography and roughness of the cuticular surfaces. To evaluate the cross-linking capabilities of dewaxed cuticles from cultivated wild-type and single-gene mutant tomato fruits, MAS 13C NMR was used to compare the relative proportions of oxygenated aliphatic (CHO and CH2O) chemical moieties. Exhaustive dewaxing by stepwise Soxhlet extraction with a panel of solvents of varying polarity provides an effective means to isolate wax moieties based on the hydrophobic characteristics of their aliphatic and aromatic constituents, while preserving the chemical structure of the cutin biopolyester. The mechanical extraction of epicuticular waxes and selective removal of intracuticular waxes, when monitored by complementary physical methodologies, provides an unprecedented means to investigate the cuticle assembly: this approach reveals the supramolecular organization and structural integration of various types of waxes, the architecture of the cutin-wax matrix, and the chemical composition of each constituent. In addition, solid-state 13C NMR reveals differences in the relative numbers of CHO and CH2O chemical moieties for wild-type and mutant red ripe tomato fruits. The NMR techniques offer exceptional tools to fingerprint the molecular structure of cuticular materials that are insoluble, amorphous, and chemically heterogeneous. As a noninvasive surface-selective imaging technique, AFM furnishes an effective and direct means to probe the structural organization of the cuticular assembly on the nm-μm length scale.

Abstract:Aims The aim of this randomized controlled clinical trial was to compare the gene expression, micro‐CT, histomorphometrical analysis between biphasic calcium phosphate (BCP) of 70/30 ratio and deproteinized bovine bone mineral (DBBM) in sinus augmentation. Materials and Methods Twenty‐four patients in need for sinus floor augmentation through lateral approach were randomized into BCP 70/30 ratio or DBBM. After at least 6 months of healing, a total of 24 bone specimens were collected from the entire height of the augmented bone at the area of implant placement and underwent micro‐CT, histomorphometric and gene expression analysis. The 12 bone specimens of BCP 70/30 ratio were equally allocated to micro‐CT and histologic analysis (test group, n = 6) and gene expression analysis (test group, n = 6). Similarly, the 12 bone specimens of DBBM were also allocated to micro‐CT and histologic analysis (control group, n = 6) and gene expression analysis (control group, n = 6). The newly formed bone, remaining graft materials and relative change in gene expression of four target genes were assessed. Results The micro‐CT results showed no statistically significant difference in the ratio of bone volume to total volume (BV/TV ratio) for the two groups (BCP 41.51% vs. DBBM 40.97%) and the same was true for residual graft material to total volume (GV/TV ratio, BCP 9.97% vs. DBBM 14.41%). Similarly, no significant difference was shown in the histological analysis in terms of bone formation, (BCP 31.43% vs. DBBM was 30.09%) and residual graft area (DBBM 40.76% vs. BCP 45.06%). With regards to gene expression, the level of ALP was lower in both groups of bone grafted specimens compared with the native bone. On the contrary, the level of OSX, IL‐1B and TRAP was higher in augmented bone of both groups compared with the native bone. However, the relative difference in all gene expressions between BCP and DBBM group was not significant. Conclusions The BCP, HA/β‐TCP ratio of 70/30 presented similar histological and micro‐CT outcomes in terms of new bone formation and residual graft particles with DBBM. The gene expression analysis revealed different gene expression patterns between augmented and native bone, but showed no significant difference between the two biomaterials.

Sinus Floor Elevation and Simultaneous Dental Implantation: A Long Term Retrospective Study of Sinus Bone Gain

Abstract:OBJECTIVE:To evaluate the sinus bone gains after sinus floor elevation procedures with or without grafts when implants were placed simultaneously.METHODS:The research included 26 edentulous patients in the maxillary posterior region, who were divided into 2 different groups according to the sinus lift procedures employed: group of osteotome sinus floor elevation with bone grafts (group A) and group of osteotome sinus floor elevation without bone grafts (group B). The implants were placed simultaneously, and were followed for a period of 30-50 months after sinus floor elevation as routine and then were observed for sinus bone change.RESULTS:There were 27 implants (13 implants for group A and 14 implants for group B) and the following time was 40.23 (36.20, 48.07) months. The residual bone height (RBH) was (6.64±1.21) mm for group A and ( 6.96±1.36) mm for group B; the difference between the two groups was not statistically significant (P=0.459).The sinus bone gain (SBG) when followed was (2.20±1.71) mm for group A, and 1.77 (0.94, 2.05) mm for group B; the difference between the two groups was not statistically significant (P = 0.583). The quality of apical bone around implant apex was divided into 3 categories which were excellent, fine and fair.The quality of apical bone around implant apex was excellent and fine for 9 implants in group A and 8 implants in group B, and the difference between the two groups was not statistically significant (Fisher exact test, P=0.695).The implant apex when followed was (0.09±1.32) mm below the new sinus floor for group A and (0.03±0.91) mm for group B, and the difference between the two groups was not statistically significant (P = 0.898). The SBG for the total was 1.85 (1.10, 2.20) mm. The SBG was statistically significantly only related to the protrusion height of the implant apex (r=0.383, P=0.049).CONCLUSION:Sinus floor elevation procedures with or without grafts can all gain good results. The SBG is related to the protrusion height of the implant apex significantly.
A 5-year clinical and computerized tomographic implant follow-up in sinus-lifted maxillae and native bone

Abstract:Objectives: The present study analysed apical and marginal bone remodelling around dental implants placed in both maxillary (sinus elevated with particulated autogenous osseous graft) and corresponding native bone areas, with a follow-up of 5 years. The clinical survival of implants was also observed. Materials and methods: In this retrospective chart review, 27 patients were enrolled, with 55 dental implants inserted from 2000 to 2006, 26 of which were followed (one implant per patient); if required, patients were treated via sinus lift with autogenous bone and particulate technique. The internal controls were implants positioned in native areas beneath the sinus. Radiologic survey was assessed via computerized tomographic analysis measuring apical bone level (ABL) and marginal bone level (MBL), at 1- (T1 ), 3- (T2 ) and 5 years (T3 ), around implants (buccal, b; palatal, p; mesial, m; and distal sides, d). Clinical probing depth (CPD) and clinical attachment level (CAL) for all the four peri-implant aspects were measured. Cumulative survival rate (CSR) and survival rate (SR) of implants were calculated. Significances for paired and unpaired comparisons were searched for. Results: A significant degree of apical resorption was recorded between T1 and T3 for the mesial particulate group; again, a significant difference was discovered between the native and particulate procedures for mABL. A further feature was discovered for the particulate procedure, for which ABLs resulted negative at least for three of the aspects. Regarding MBL measurements, similar behaviours were revealed using time-comparison analysis for the two procedures at the buccal aspect. Comparisons among diameters, irrespective of the procedure, showed that resorption times for the bMBL were shorter as the diameter of the implant became wider. The implant CSR was 92% in native areas (two failures/25 implants) and 93.3% in sinuses lifted with particulate bone (two failures/30 implants). Conclusions: The results suggest that a protrusion of the implant apices into augmented sinus lift occurred, whereas the bone remodelling of the coronal areas was not influenced by the events in the maxillary sinus. A diameter ranging from 4 to 5 mm might better guarantee a conservation of marginal peri-implant bone level compared with implants with smaller diameters.
Microcomputed tomographic and histologic analysis of anorganic bone matrix coupled with cell-binding peptide suspended in sodium hyaluronate carrier after sinus augmentation: a clinical study

Abstract:Purpose: Anorganic bovine hydroxyapatite matrix (ABM), when coupled with synthetic cell-binding peptide (P15), mimics the cell-binding region of type 1 collagen and is commercially available suspended in a sodium hyaluronate carrier. The aim of the present study, therefore, was to test the efficacy of ABM/P-15 Putty (DENTSPLY Friadent CeraMed) as a sole graft material for sinus augmentation in patients with severely resorbed posterior maxillae. Materials and methods: Sinus augmentation was performed in 10 patients using ABM/P-15 Putty and two provisional dental implants (3.0 mm in diameter). The graft and implants were placed simultaneously with the aid of a surgical stent. After 8 or 16 weeks, the implants were removed using a 4.25-mm trephine bur; this was followed by immediate placement of wider-diameter (5.5-mm) implants. All 20 implants were scanned by microcomputed tomography to determine bone mineral density (BMD), percent bone volume (PBV), and percent bone contact (PBC). Results: There was a significant increase in the BMD of bone around the implants at 8 weeks and 16 weeks compared to native residual (control) bone. There was no significant difference in PBV or PBC between 8 weeks and 16 weeks. The average increase in bone height at 16 weeks was 9.63 ± 1 mm. Microcomputed tomographic images and histologic sections showed dense graft particles surrounded by vital trabecular bone. Conclusion: BMD increases as early as 8 weeks and does not show an additional increase after 16 weeks. PepGen P-15 Putty was found to be a promising osteoconductive graft for sinus augmentation, supporting immediate placement of implants.

Abstract:Aims To assess the long-term clinical and radiographic results of implants placed using osteotome sinus floor elevation (OSFE) with or without bone grafting.Materials and methods Forty-five patients were randomly assigned into two groups: Group 1 (OSFE with deproteinized bovine bone mineral) and Group 2 (OSFE without grafting). The patients were recalled at 1, 3, 5 and 10 years after surgery. The implant survival, endo-sinus bone gain (ESBG), marginal bone loss (MBL), peri-implant bone height (PBH, distance from the most coronal level to the most apical level of bone-to-implant contact), prosthesis survival and hardware complications, and peri-implant soft tissue conditions were assessed.Results Forty patients attended the 10-year examination. Mean residual bone height was 4.58 +/- 1.28 mm. The 10-year cumulative survival rate was 90.7% for Group 1 and 95.0% for Group 2. The PBH was 5.89 +/- 1.24 mm for Group 1 and 5.74 +/- 1.43 mm for Group 2 at 10 years. The ESBG of both groups remained stable after 3 years. Two-thirds of the implants were free of hardware complications. No significant differences in MBL and peri-implant tissue parameters were found.Conclusion OSFE with or without grafting both yielded predictable clinical outcomes with similar PBH (ClinicalTrials.gov. NCT01619956).

Abstract:Aim To assess the 5‐year effects of grafting connective tissue while undertaking single immediate implant placement and provisionalization at the mid‐buccal mucosa level (MBML). Secondary outcomes were buccal bone wall thickness (BBT), marginal bone level (MBL) and patient satisfaction. Materials and Methods Sixty patients with a single failing tooth in the maxillary anterior region were provided with an immediately placed and provisionalized implant. At implant placement, the patients randomly received either a connective tissue graft from the maxillary tuberosity (n = 30, test group) or no graft (n = 30, control group). The alveolar socket classification was mainly Type 2A. Data were collected before removing the failing tooth (T0), and at 1 (T1), 12 (T12) and 60 (T60) months after final crown placement. The primary outcome was the change in MBML compared with the pre‐operative situation. Additionally, the change in BBT, MBL, aesthetics (using the Pink Aesthetic Score–White Aesthetic Score), soft‐tissue peri‐implant parameters and patient satisfaction were assessed. Results At the 5‐year follow‐up, 27 patients could be analysed from each group. In each group, one implant was lost during the osseointegration period, within 3 months of placement, resulting in an implant survival rate of 96.7% in both groups. MBML change at T60 was −0.6 (−1.1 to –0.1) mm in the control group and 0.1 (−0.4 to 0.5) mm in the test group (p = .008). BBT and MBL, aesthetics, soft‐tissue peri‐implant parameters and patient satisfaction showed stable results and satisfied patients, without clinically relevant differences between the groups. Conclusions This 5‐year follow‐up study shows that grafting connective tissue when replacing a single failing tooth with immediately placed and provisionalized implant results in favourable peri‐implant tissues and fewer MBML changes.
