Abstract:Aim: The chronic lack of obstetrics-gynecology (Ob/Gyn) doctors is a serious problem to be tackled in Japan. To address this issue, it is necessary for medical students to be interested in working as an Ob/Gyn doctor and more junior residents need to specialize in Ob/Gyn. This study aims to identify the preferences concerning working conditions among students and discusses what information should be provided and what conditions should be changed. Methods: A questionnaire survey was conducted among participants in the Summer School 2023 of the Japanese Society of Obstetrics and Gynecology. The respondents were medical students and junior residents interested in Ob/Gyn. Ob/Gyn specialists contributing to the summer school were also asked the same questions for comparison. The resulting data focused on important working conditions, concerns about future work, and expected future working conditions. Results: Responses from 132 medical students (97.8%, N = 135), 122 residents (99.2%, N = 123), and 76 specialists (95.0%, N = 80) were obtained. There were three main findings. First, large gaps between students/residents and specialists were observed in terms of preferences. Second, differences between male and female students/residents were not negligible, although the difference was relatively small in specialists. Third, students/residents were more concerned about risks related to medical malpractice than specialists. Conclusions: Gaps in preferences and perceptions regarding working conditions between students/residents and specialists should be narrowed through sufficient information provision. Working conditions should become gender-inclusive rather than focus on female persons.

Abstract:Background/Objectives: The Ministry of Health, Labor and Welfare in Japan has been promoting physicians' working style reforms since 2019. This study aimed to update the relationship between working hours, depressive symptoms, and suicidal ideation among obstetricians and gynecologists, based on the physicians' working style reforms. Methods: A questionnaire-based survey was conducted among obstetricians and gynecologists, and valid responses were obtained from 1164 physicians. Multivariable logistic regression analysis was performed to identify significant associations. Results: Of the respondents, 49.8% were female, and most physicians were aged 30–39 (32.1%). Precisely, 57.4% worked in public hospitals, and 47.9% worked in urban areas. Physicians working 40 ≤ x 100 h per week, and working in rural areas. None of these variables was significantly associated with suicidal ideation. Conclusions: The physicians' working style reforms have reduced the number of working hours for obstetricians and gynecologists. However, rates of depressive symptoms and suicidal ideation have not improved.

Task Shifting of Traditional Birth Attendants in Rural China: a Qualitative Study of the Implementation of Institution-Based Delivery Policy

Abstract:BackgroundInstitution-based delivery, with the ultimate goal of universal access to skilled birth attendance, has been selected as a key strategy to reduce maternal mortality rate in many developing countries. However, the question of how to engage traditional birth attendants in the advocacy campaign for skilled birth attendance poses several challenges. Here, we use the experience of task shifting of traditional birth attendants in rural Guangxi Zhuang Autonomous Region, China, as a case study to illustrate how traditional birth attendants could be well integrated into the health system in rural regions.MethodsWe used qualitative research methods, including 28 document reviews (17 published manuscripts, six policy documents, one research report, one doctoral dissertation, and three websites) and 32 individual in-depth interviews with regional and local politicians, policy makers, health-care managers, health-care providers, civil society members, village women cadres, traditional birth attendants, village maternal health workers, mothers, and mother-in-laws. The research has been approved by the Ethical Board of School of Public Health, Fudan University, China.FindingsFrom 1998 to 2012, the rate of institutional-based childbirth increased from 45·7% to 99·7%, and maternal mortality rate decreased from 86·0 per 100 000 livebirths to 27·2 per 100 000 livebirths in Guangxi Zhuang Autonomous Region. Traditional birth attendants' role of providing in-home care during childbirth was restructured, and their social role was strengthened as a result of task shifting. Traditional birth attendants were shifted to act as village maternal health workers to promote perinatal care and institution-based delivery. They were designed to function at the bottom level of the rural health network (county–township–village), acting as the link between women and the health system. A key policy document formalised the role of village maternal-health workers and supported the task-shifting process, which involved engaging with all government and non-governmental organisations (eg, women's federation, youth organisations, and academic associations), training traditional birth attendants for their new role, and providing incentives and sanctions for human resources management.InterpretationThe task-shifting experience, implemented in accordance with the key policy document, in Guangxi was an example of successfully engaging traditional birth attendants in the promotion of institution-based delivery.FundingThe study was supported by the China–UK Global Health Support Programme (GHSP-CS-OP1-02) and partly by the European Union Sixth Framework Programme HEPVIC (EU517746).
Factors affecting marriage and pregnancy/childbirth among women physicians in Japan

Abstract:Background Previous studies have not elucidated the factors affecting marriage and pregnancy/childbirth. This study identifies whether work characteristics and conditions affect marriage and pregnancy/childbirth among women physicians. Methods This nationwide survey was conducted on women members of three major Japanese medical societies: internal medicine, obstetrics and gynecology, and surgery. Results A total of 7747 women participated in this study. Although the mean age at marriage was the same, the age at the first childbirth in all specialties, especially in surgery, was significantly higher than that for the general Japanese population. In addition, being in a junior position, being board‐certified, and working in a workplace other than a university hospital were positively associated with the experience of pregnancy/childbirth, whereas working hours showed no impact. Conclusions We should change the mindset regarding pregnancy/childbirth and build effective strategies in academic institutions to improve the environment for safer pregnancy/childbirth while advancing careers.

Abstract:In May 2024, we requested all obstetric institutes (n = 1970) that are members of the Japan Association of Obstetricians and Gynecologists (JAOG) to provide information concerning the collaboration between obstetric institutes and psychiatrists in Japan, such as (1) pregnancy and delivery management of all pregnant women with psychiatric complications, and (2) availability of smooth collaboration with associated psychiatry (or psychiatrists) regarding pregnant women with psychiatric complications. Of the 1970 obstetric institutes, 1483 (75.2%) responded with valid information (completion of data submitted). In Japan, only 21% (308/1483) of all obstetric institutes are able to see all pregnant women with psychiatric complications who attend their hospital, and 32% (480/1483) of the institutes responded that they have smooth cooperation with associated psychiatrists. The response to pregnant women with mental health problems in Japanese obstetric institutes is not adequately equipped yet.

Abstract:Workstyle reform for Japanese doctors is a priority of Japanese labor policy. Factors influencing this include the low birthrate and longevity of the population and the long working hours experienced by Japanese workers. Long working hours imperil the health of doctors and create discord between their work and other life roles. The Japanese government enacted measures to promote workstyle reform for doctors, including a provision that capped legal overtime work. The limit was set to sustain healthcare services in the community. However, the allowed level of overtime is substantially higher than the so-called “line of karoshi” at which point worker compensation for work-related medical disorders is approved. Despite limited research, several measures can be considered to reduce health risks of overworked doctors, such as recommending psychological detachment from clinical work and maintaining work-life balance. Doctors should realize that self-care is a professional imperative. Organizational efforts from public administration entities or hospitals are required to secure the health of doctors. The number of surgeons and obstetricians has not recently increased. Gender role stereotyping has encouraged male Japanese doctors to work long hours and makes it difficult for female doctors to continue their professional life after childbirth. Without reforming the workstyle and protecting the health of doctors, younger workers, particularly women, are liable to avoid entering medical professions. To ensure the provision of good healthcare, it is necessary to change traditional values and improve the work environment. Academics in occupational fields should produce evidence that will help improve Japanese labor policies.

Abstract:Background: Japan is reviewing how physicians operate and plans to implement a work-style reform for physicians in 2024. This study examined how outsourcing housework cleaning tasks changed the daily lives of university hospital physicians.Methods: A total of 18 physicians participated in the study, outsourcing cleaning tasks either once or thrice.Results: Fourteen out of 18 respondents reported a decrease in the burden of household chores. Additionally, 10 respondents reported having more time for family contact, and nine respondents reported having more time for their own hobbies and diversions. Meanwhile, only five respondents reported that they had more time to work.Conclusion: Outsourcing housework cleaning tasks and using the newly created time for family and self may improve work performance.

Abstract:Background: Telemedicine, or online medical care, has gained considerable attention worldwide. However, it has not been widely adopted in Japan, and the detailed status of received and provided online medical care and the reasons for its lack of popularity remain unknown. Objective: This study aims to investigate the current status of online medical care in Japan and the factors limiting its adoption from the perspective of both patients receiving and medical professionals providing online medical care. Methods: In total, 2 nationwide questionnaire surveys were conducted. The first survey, targeting both patients and healthy individuals, screened approximately 40,000 participants among 13 million people. The participants were selected to match the age distribution of the Japanese population based on government data, and their online medical care experience and medical visit status were recorded. To further investigate online medical care use and satisfaction, a web-based survey was conducted with 15% (6000/40,000) of the screened participants. The second survey, targeting medical professionals, was administered to a physician, a nurse, and a member of the administrative staff in each of 4900 randomly selected medical facilities to inquire about their online medical care practices and impressions. In addition, both surveys investigated the factors limiting online medical care expansion in Japan. Results: The response rates among patients and healthy individuals targeted for the screening and main surveys were 92.5% (36,998/40,000) and ˃80% (1312/1478, 88.77%; 1281/1522, 84.17%; 404/478, 84.5%; and 2226/2522, 88.26% in 4 survey groups), respectively. The survey of medical professionals yielded 1552 responses (n=618, 39.82% physicians; n=428, 27.58% nurses; n=506, 32.6% administrative staff). Although the facility-level response rate was low (794/4900, 16.2%), some facility categories had relatively high response rates. Only 5.29% (1956/36,998) of the patients and healthy individuals had online medical care experience. When there were more hospitals nearby and they felt it was more work to see a physician in person, they were more likely to use online medical care (more nearby hospitals: adjusted odds ratio [aOR] 1.33, 95% CI 1.18-1.50; more work: aOR 1.48, 95% CI 1.35-1.63 per survey response point in the patient group). Similarly, these factors were substantially associated with satisfaction (more nearby hospitals: aOR 1.40, 95% CI 1.14-1.73; more work: aOR 1.50, 95% CI 1.27-1.76 per survey response point in the patient group). In both surveys, the most frequently selected factor preventing the widespread use of online medical care was patients' need to switch to face-to-face medical care for mandatory tests and procedures. Inadequate awareness of and education on online medical care were also frequently selected. Conclusions: Our nationwide surveys provided insights into the current status of online medical care in Japan and simultaneously identified several problems and issues related to it, which will be useful in promoting its wider adoption.

Population aging and physician maldistribution: A longitudinal study in Japan

Abstract:Background: Over the past two decades, population aging and the introduction of the new postgraduate medical education program in 2004 have impacted on the geographic maldistribution of physicians in Japan. The purpose of this study was to evaluate recent changes in physician distribution across municipalities from 1996 to 2012 using Gini coefficients and to clarify the impact of the new medical education program on physician distribution.Methods: We extracted the number of physicians classified by type of medical institution and municipal bodies. Gini coefficients were calculated using both population and demand for medical services. We calculated the contribution ratio (CR) of maldistribution within each type of medical institution to the whole maldistribution using Rao’s method. In addition, we calculated the incremental difference in Gini coefficients between 2002 and 2010, and calculated the CR of the incremental Gini coefficient difference for each medical institution type using Seki’s method.Results: Both Gini coefficients decreased from 1996 to 2002, and increased after 2006. The CR of other hospitals increased from 2004. The incremental difference in the Gini coefficient using demand between 2002 and 2010 was 0.012, and the CR of each type of medical institution was -25.1% (university hospitals), 131.0% (other hospitals) and -5.9% (clinics).Conclusions: Our analysis showed that the geographic maldistribution of physicians has worsened since the introduction of the new postgraduate medical education program, and the CR of maldistribution in other hospitals was high. Our study suggested that new medical resource distribution policies should be discussed to improve maldistribution.

Evidence-based Obstetrics in Four Hospitals in China: an Observational Study to Explore Clinical Practice, Women's Preferences and Provider's Views.

Abstract:BACKGROUND: Evidence-based obstetric care is widely promoted in developing countries, but the success of implementation is not known. Using selected childbirth care procedures in four hospitals in Shanghai, we compared practice against evidence-based information, and explored user and provider views about each procedure. METHODS: Observational study. Using the Cochrane Library, we identified six procedures that should be avoided as routine and two that should be encouraged. Procedure rate determined by exit interviews with women, verified using hospital notes. Views of women and providers explored with in depth interviews.The study sites were three hospitals in Shanghai and one in neighbouring province of Jiangsu. 150 women at each centre for procedure rate, and 48 in-depth interviews with women and providers. RESULTS: Vaginal births were 50% (303/599) of the total. Of the six practices where evidence suggests they should be avoided as routine, three were performed with rates above 70%: pubic shaving (3 hospitals), rectal examination (3 hospitals), and episiotomy (3 hospitals). Most women delivered lying down, pain relief was rarely given, and only in the urban district hospital did women routinely have a companion. Most women wanted support or companionship during labour and to be given pain relief; but current practice is insufficient to meet women's needs. CONCLUSION: Obstetric practice is not following best available evidence in the hospitals studied. There is a need to adjust hospital policy to support the use of interventions proven to be of benefit to women during childbirth, and develop approaches that ensure clinical practice changes.
Midwives’ Views and Experiences of Providing Midwifery Care in the Task Shifting Context: a Meta-Ethnography Approach

Abstract:Objective: This study aimed to explore the existing knowledge about midwives' views and experiences of providing care for women in the context of task shifting.Methods: We conducted a qualitative systematic review using meta-ethnography to describe the views and experiences of midwives on providing care in the context of task shifting. Comparative textual analysis of published qualitative studies involved translation of first-order key concepts and meanings from included studies to generate second-and third-order concepts. A grid was made to identify core findings and compare them reciprocally. Results: Thirty-six qualitative studies met the inclusion criteria. The literature comprised of 32 first key concepts. Eight second-order constructs emerged, and three third-order interpretations were generated. The three overarching themes were: (1) midwives perceived themselves as providing culturally competent and high quality women-centered care; (2) they valued their profession but saw it as complex and challenging; (3) as health professionals, they reported a variety of organizational, cultural, and professional barriers to providing women-centered care. Conclusions: While performing a specific task in the task shifting context, midwives perceived their crucial roles and responsibilities, along with achieved value and reward. However, due to a range of existing barriers, the caring task posed great challenges in completely implementing women-centered care. It is essential for systems to identify and eliminate these barriers early, to consider midwives' emotional well-being, and to develop overall strategies to better support the midwifery workforce. Policy makers and administrators should establish a supportive environment to facilitate midwives to perform women-centered caring tasks in more effective and efficient ways.

Temporal trends and regional variations in operative vaginal deliveries in Japan: A national representative cohort study

Abstract:Aim: To analyze temporal trends and regional variations in operative vaginal delivery (OVD) in Japan. Methods: Using the National Database of Health Insurance Claims and Specific Health Checkups of Japan from 2014 to 2021, we identified the numbers of vacuum and forceps deliveries. We analyzed annual totals and proportions of OVDs and calculated the mean age of women undergoing these deliveries. We also predicted trends in OVD for the next 20 years and compared geographical differences in the proportions of forceps deliveries among OVDs. Results: During the observation period, out of 7 368 814 total births, 8.4% were through OVD, including 7.6% by vacuum and 0.8% by forceps. Both delivery methods showed an increasing trend from 2014 to 2021: vacuum deliveries rose from 7.0% to 8.7%, and forceps deliveries increased from 0.6% to 1.0%. Notably, the proportion of forceps deliveries in OVD increased from 8.1% to 10.5%. The mean age was higher for forceps deliveries than vacuum deliveries. According to our predictions, vacuum deliveries may continue to increase, but forceps deliveries may stabilize. The proportion of forceps deliveries among OVDs ranged from 0% to 38% across Japanese prefectures. Conclusions: This study shows an increase in the use of OVD in Japan from 2014 to 2021. There are large regional differences in the choice between vacuum and forceps deliveries. These findings can help us understand the practice of OVD in Japan.
Work engagement influencing factors of Japanese midwives working in prenatal medical centers

Abstract:Introduction: There have been significant changes in the work environment of midwives, such as the establishment of prenatal medical centers and shared wards for obstetrics and other departments. These changes in the work environment pose a psychological burden on midwives. Under these circumstances, the concept of work engagement is essential for overcoming the ensuing difficulties. However, no studies have yet investigated midwives working in prenatal medical centers in Japan. This study examined the factors affecting work engagement among midwives working in prenatal medical centers in Japan. Methods: A cross-sectional survey of 498 midwives working at prenatal medical centers nationwide was conducted. Data were analyzed using the Mann-Whitney U test or Kruskal-Wallis test, correlation analysis using Spearman's rank correlation coefficient, and multiple regression analysis as linear regression. Results: The median of work engagement score was 3.00 (IQR: 2.40-3.60). The multiple linear regression analysis showed that professional identity (β=0.494, p<0.001), resilience (β=0.243, p<0.001), presence of in-hospital midwifery in the facility (β=0.138, p<0.011), and age (β=0.085, p=0.021) were significant. The adjusted R2 value was 0.490 (p<0.001). Conclusions: These results offer management insights into improving work engagement among midwives working in prenatal medical centers in Japan. The proposed measures include hospital/ward administrators establishing the professional identity of midwives and providing them with opportunities to demonstrate their expertise, for instance, within in-hospital midwifery systems at perinatal medical centers.
Long‐term impact of overnight shiftwork implementation on pediatric residents' mental wellness: A repeated cross‐sectional survey

Abstract:Objectives The Japanese government partially enacted the "Work Style Reform Bill" in 2019. The National Center for Child Health and Development (NCCHD) introduced an Overnight Call Shift (OCS) system for pediatrician training. We conducted a follow‐up survey in 2019 to investigate the long‐term effectiveness of the OCS system to improve the pediatric residents' mental wellness at NCCHD. Methods We conducted a questionnaire‐based cross‐sectional survey for pediatric residents in 2019 to compare the data with those of the previous survey in 2012. The questionnaire includes demographic data, working conditions data, and mental wellness assessment by the Center for Epidemiologic Studies Depression scale (CES‐D) and the Maslach Burnout Inventory (MBI). Results The collection rate for the 2019 survey was 94.5% (37 participants/39 eligible). Compared to 2012, there were no significant changes in demographic data and working hours, a significant increase by about 30% in residents who took daytime off after night work, about 10% decrease in residents who scored 16 and above on the CES‐D, and a significant decrease in the mean score for depersonalization (DP) in the MBI. Multiple regression analyses showed that daytime off after night work was the decreasing factor for CES‐D and Emotional exhaustion (EE). Conclusions The overnight shiftwork system shortened the pediatric residents' duty hours somewhat, and imposed an impact on the pediatric residents' mental wellness.

Abstract:We performed a prospective, randomised double-blind study in 24 patients undergoing high tibial osteotomy to evaluate the effectiveness of human recombinant osteogenic protein (OP-1) on a collagen type-I carrier in a critically-sized fibular defect. The study had two phases, each evaluated by clinical, radiological and DEXA methods during the first postoperative year. The first concerned the validation of the model of the fibular defect, using positive (demineralised bone) and negative (untreated) controls. The second phase concerned the osteogenic potential of OP-1 on collagen type-I v collagen type-I alone. The results of the first phase established the critically-sized nature of the defect. In the untreated group no bony changes were observed while, in the demineralised bone group, formation of new bone was visible from six weeks onwards. The results of the second phase showed no significant formation of new bone in the presence of collagen alone, while in the OP-1 group, all patients except one showed formation of new bone from six weeks onwards. This proved the osteogenic activity of OP-1 in a validated critically-sized human defect.

Abstract:Background: Highly-competent patient care is paramount to medicine. Quality training and patient accessibility to physicians with a wide range of specializations is essential. Yet, poor quality of life for physicians cannot be ignored, being detrimental to patient care and leading to personnel leaving the medical profession. In 2004, the Japanese government reformed postgraduate training for medical graduates, adding a 2-year, hands-on rotation through different specialties before the specialization residency was begun. Residents could now choose practice location, but it sparked concerns that physician distribution disparities had been created. Japanese media reported that residents were choosing specialties deemed to offer a higher quality of life, like Ophthalmology or Dermatology, over underserved areas like Obstetrics or Cardiology. To explore the consequences of Japan's policy efforts, through the residency reform in 2004, to improve physician training, analyzing ophthalmologist supply and distribution in the context of providing the best possible patient care and access while maintaining physician quality of life. Methods: Using secondary data, we analyzed changes in ophthalmologist supply at the secondary tier of medical care (STM). We applied ordinary least-squares regression models to ophthalmologist density to reflect community factors such as residential quality and access to further professional development, to serve as predictors of ophthalmologist supply. Coefficient equality tests examined predictor differences before and after 2004. Similar analyses were conducted for all physicians excluding ophthalmologists (other physicians). Ophthalmologist coverage in top and bottom 10% of STMs revealed supply inequalities. Results: Change in ophthalmologist supply was inversely associated with baseline ophthalmologist density before (P < .01) and after (P = .01) 2004. Changes in other physician supply were not associated with baseline other physician density before 2004 (P = 0.5), but positively associated after 2004 (P < .01). Inequalities between top and bottom 10% of ophthalmologist supply in STMs were large, with best-served areas maintaining roughly five times greater coverage than least-served areas. However, inequalities gradually declined between 1998 and 2012. Conclusions: Ophthalmologist supply increased both before and after the 2004 reform, yet contrary to media reports, proceeded at a lesser rate than supply increases for other physicians. After 2004, geographical disparities decreased for ophthalmologists, while increasing for other physicians.
Task shifting in Dutch nursing practice: A repeated cross‐sectional analysis of nurses' experiences

Abstract:Aim This study aims firstly to identify shifts in the execution of medical tasks by nurses in the past decade. Secondly, it aims to explore nurses' perspectives on task shifting: how they think task shifting affects the quality of care, the attractiveness of nursing practice and their collaboration with physicians. Design A quantitative repeated cross‐sectional study. Methods A nationwide survey was conducted among Dutch registered nurses (RNs) working in hospitals and home care, first in 2012 and again in 2022, with sample sizes of 359 and 362, respectively. Analyses were based on descriptive statistics and logistic and linear regressions. Results Between 2012 and 2022, there was a significant increase in the execution of only one medical task by nurses, namely prescribing over‐the‐counter medication. The majority reported in both years that task shifting has positive impact on their professional autonomy and the attractiveness of nursing practice. However, most nurses also reported that task shifting increased their workload (72.7% in 2022) could lead to conflicts in care teams (20.9% in 2022 compared to 14.7% in 2012) and may cause physicians to feel threatened (32.8% in 2022 and 29.9% in 2012). There were no significant changes in nurses' perception of the impact of task shifting on quality of care, the attractiveness of nursing practice and the nurse–physician relationship. Conclusion There was an increase in the execution of prescribing over‐the‐counter‐medication by nurses between 2012 and 2022. However, both in 2012 and in 2022, as the majority of nurses reported that task shifting increased their workload, there is reason to worry about this negative consequence of task shifting, e.g. with regard to labour market issues. Further research, also among the medical profession, is needed to better understand and address the implications of task shifting for the nursing profession. Implications for the Profession Implications for the nursing profession include potential scope expansion with complex tasks, attracting more individuals to nursing careers, although an eye must also be kept on what that means for the workload of nurses and the relationship with physicians. Impact Nurse prescribing medicines was more executed in 2022 compared to 2012. Nurses had a predominantly positive perspective on task shifting, but still felt it can cause conflicts in care teams, high workload and physicians feeling threatened. These results can help during implementation of task shifting and in monitoring the perceived effects of task shifting among nurses.
