Abstract:The purpose of the study was to investigate the influence of late gadolinium enhancement (LGE) on the diastolic volume recovery of left ventricle in patients with hypertrophic cardiomyopathy (HCM).Twenty-four HCM patients were studied through report-card 4.0. The presence or absence of late gadolinium enhancement was recorded according to a standardized methodology with a threshold value of six standard deviations above background. The LGE positive and negative groups were correlated to left ventricular end diastolic volume index (EDVI), left ventricular mass, left ventricular ejection fraction (EF), peak filling rate (PFR), peak ejecting rate (PER), normalized peak filling or ejecting rate (NPFR or NPER), time to peak filling or ejecting rate (TPFR or TPER), and diastolic volume recovery (DVR).PFR, NPFR, SV, SVI, EF, CO, CI, FS in LGE positive group were lower than LGE negative group, DVR10-40, DVR100, end systolic volume (ESV), end systolic volume index (ESVI), ESD were higher in LGE positive group, and the differences were statistically significant. The average LGE mass (ROI, region of interest) was 20.78 g, about 13.67% of left ventricle mass in LGE positive HCM group. Pearson correlation was noted between the LGE percent (ROI%) and ESV (0.692, P<0.05), ROI% and EF (-0.718, P<0.05), ROI% and PFR (-0.534, P<0.05), DVR20-40 (0.547, 0.544, 0.906, P<0.05) etc. The correlation between ROI% and DVR40 was best (0.906, P<0.05), and the correlation between ROI% and ESVI, ROI% and EF were both bigger than 0.7, showed the correlation was good.In addition to common quotas used to assess the structure and function of left ventricle in HCM, volume-time curve parameters may have potential to evaluate cardiac function in HCM. The correlation between DVR generated from volume-time curve with LGE was good, and may be a marker of effect of enhancement/scar tissue on diastolic function.

Assessment of Left Ventricle Volume Filling Curve with Magnetic Resonance Imaging: a Preliminary Study

Abstract:OBJECTIVE:To conduct a preliminary study on the reliability of left ventricle volume filling curve through the commercial medical program Report-Card 4.0.METHODS:A total pf 22 normal volunteers underwent the examination. Images of standard 2-chamber view and short-axis view were acquired at end-expiration by electrocardiography-gated FIESTA CINE sequence. Then one experienced doctor manually contoured the endocardium during end-systole and end-diastole phases respectively, obtained the results of the volume of end-diastole (EDV) and end-systole (ESV), ejection fraction (EF), stroke volume (SV) and cardiac output (CO), processing time and mean processing time per phase. Papillary muscle was not included into left ventricle volume. Another two observers utilized LV ANALYSIS of Report-Card, generated the left ventricle volume filling curve and recorded the processing time and mean processing time per phase. From the curve, EDV, ESV, SV, EF, CO, peak ejecting rate (PER) and peak filling rate (PFR) were also acquired. One observer repeated the procedures a week later.RESULTS:The difference of results from two methods were insignificant (P > 0.05) and the correlation was excellent (EDV 0.963, ESV 0.944). Intra-observer and inter-observer variability for measurements (EDV, ESV) were assessed by Bland-Altman analysis and interclass correlation coefficient (0.985, 0.987, 0.959 and 0.957 respectively). The mean processing time (179 ± 51) s by means of manually contouring was significantly less than the mean processing time (331 ± 99) s through REPORT-CARD 4.0 (P < 0.001). However, the mean processing time per phase (17 ± 5) s by means of REPORT-CARD 4.0 was significantly less than the mean processing time per phase (89 ± 26) s through manually contouring (P < 0.001).CONCLUSION:The reliability of left ventricle volume filling curve generated through Report-Card is excellent. Left ventricle volume filling curve may be a reliable method of further studying the functions of left ventricle.
Evaluation of Left and Right Ventricular Ejection Fraction and Volumes from Gated Blood-Pool SPECT in Patients with Dilated Cardiomyopathy: Comparison with Cardiac MRI

Abstract:This prospective study evaluated the accuracy of electrocardiogram-gated blood-pool SPECT (GBPS) for the assessment of left ventricular (LV) and right ventricular (RV) ejection fraction (EF), end-diastolic volume (EDV), and end-systolic volume (ESV) in patients with dilated cardiomyopathy (DCM), using cardiac magnetic resonance (CMR) imaging as the reference standard. Methods: Thirty-two patients (24 men and 8 women; mean age, 51 y) with a diagnosis of idiopathic DCM underwent GBPS and CMR. LV and RV parameters including EDV, ESV, and EF from GBPS were calculated using fully automated gradient software and compared with those obtained by CMR. Results: Biventricular volumes were underestimated by GBPS, compared with CMR (P < 0.001). We found no statistical difference between these 2 methods in the assessment of LV EF (P = 0.23), whereas RV EF was overestimated by GBPS (P < 0.001 vs. CMR). Regression analysis yielded significant correlations between GBPS and CMR in the assessments of biventricular parameters (r = 0.83 for LV EDV, 0.88 for LV ESV, 0.89 for LV EF, 0.86 for RV EDV, 0.86 for RV ESV, and 0.62 for RV EF; all P < 0.001). Comparison of the deviations of RV indices between GBPS and CMR with the ratio of RV EDV to LV EDV showed that there was a statistically significant trend for RV volumes to be underestimated and for RV EF to be overestimated as the biventricular volumetric ratio decreased (r = 0.61 for RV EDV, 0.68 for RV ESV, and −0.55 for RV EF; all P < 0.001). Conclusion: For patients with DCM, GBPS correlated well with CMR for the assessment of biventricular parameters, but RV indices should be cautiously interpreted.
Right ventricular function and determining factors of dysfunction in ST-segment-elevation myocardial infarction: a cross-sectional study with cardiac magnetic resonance imaging (MRI)

Abstract:Background: Over the past few decades, left ventricular (LV) dysfunction in ST-segment elevation myocardial infarction (STEMI) patients has been the focus of research. Recently, co-occurring right ventricular (RV) dysfunction has received more attention in clinical practice. We aimed to assess RV function using cardiac magnetic resonance (CMR) imaging and identify factors that may contribute to RV dysfunction in STEMI patients. Methods: We retrospectively studied 189 patients with STEMI who underwent CMR 1-7 days after successful percutaneous coronary intervention (PCI). The ejection fraction (EF), wall thickening rate (WTR), peak radial strain (RS), circumferential strain (CS) and longitudinal strain (LS) of the LV, interventricular septum (IVS) and RV were measured with cine images. The location and extent of the infarct were determined using late gadolinium enhancement (LGE) imaging. The differences of function between STEMI patients with right ventricular ejection fraction (RVEF) <50% and those with RVEF ≥50% were compared using an independent-sample t-test. Linear regression analyses were used to determine independent predictors of RVEF. Results: RVEF <50% was observed in 32.28%% STEMI patients, who also demonstrated significantly lower left ventricular ejection fraction (LVEF), WTR, RS, CS, LS and larger infarct sizes than those with RVEF ≥50%. Patients with RVEF <50% also demonstrated a higher incidence of RV infarction, higher RV end-systolic volume (ESV) index, and lower RV RS and CS. Multivariable linear regression analysis revealed LV EF, IVS WTR and IVS RS as significant predictors for RVEF, while male gender, the culprit lesion in the right coronary artery (RCA), peak troponin were negative predictors for RVEF. Notably, peak troponin, LV EF, LV RS, LV CS, LV WTR, and IVS WTR demonstrated higher area under the curve (AUC) values for predicting RV dysfunction. Conclusions: RV dysfunction was detected in 32.28% of STEMI patients. Patients with acute STEMI and RVEF <50% had impaired LV and IVS functions. Systolic function of the LV and IVS, peak troponin, and culprit lesions in the RCA were independent predictors of RV dysfunction in STEMI patients.
Coalescing drops in microfluidic parking networks: A multifunctional platform for drop-based microfluidics.

Abstract:Multiwell plate and pipette systems have revolutionized modern biological analysis; however, they have disadvantages because testing in the submicroliter range is challenging, and increasing the number of samples is expensive. We propose a new microfluidic methodology that delivers the functionality of multiwell plates and pipettes at the nanoliter scale by utilizing drop coalescence and confinement-guided breakup in microfluidic parking networks (MPNs). Highly monodisperse arrays of drops obtained using a hydrodynamic self-rectification process are parked at prescribed locations in the device, and our method allows subsequent drop manipulations such as fine-gradation dilutions, reactant addition, and fluid replacement while retaining microparticles contained in the sample. Our devices operate in a quasistatic regime where drop shapes are determined primarily by the channel geometry. Thus, the behavior of parked drops is insensitive to flow conditions. This insensitivity enables highly parallelized manipulation of drop arrays of different composition, without a need for fine-tuning the flow conditions and other system parameters. We also find that drop coalescence can be switched off above a critical capillary number, enabling individual addressability of drops in complex MPNs. The platform demonstrated here is a promising candidate for conducting multistep biological assays in a highly multiplexed manner, using thousands of submicroliter samples.
Quantitation of Diffuse Myocardial Fibrosis Using Cardiac CT in Heart Failure: a Pilot Study

Abstract:Objective To evaluate the feasibility and accuracy of cardiac CT (CCT) in quantitation of extracellular volume (ECV) fraction in patients with heart failure, with 3 T Cardiac MR (CMR) as the reference. Methods Twenty?eight patients with variety reasons of heart failure were enrolled in this study. ECVs was calculated, the correlation between CCT and CMR ECV value and other cardiac function parameters (left ventricular end systolic volume LVESV, left ventricular end diastolic volume LVEDV, cardiac output CO and ejection fraction LVEF, and clinical bio?marker BNP) was determined. Interclass correlation coefficient (ICC) was used to evaluate the agreement of measurement by two radiologists. Results The average of ECV on CCT and CMR was 33% ± 8% and 31% ± 6%, respectively. A good correlation was revealed between myocardial ECV at CCT and that at CMR (r=0.854, P<0.001). Bland?Altman analysis between CCT and CMR showed a small bias (4.6%), with 95% limits of agreement of-18.2% to 27.4%. ICC for ECV at CCT was excellent (ICC=0.910). For both CCT and CMR, ECV was inversely related to LVEF. The radiation dose for CCT?ECV was (1.60±0.04) mSv. Conclusions ECV at CCT and that at CMR showed good correlation, suggesting the potential for myocardial tissue characterization using CCT. However, CCT?ECV would possibly overestimate the extent of ECV.
Evaluation of Left Ventricular Volumes and Ejection Fraction by Gated SPECT and Cardiac MRI in Patients with Dilated Cardiomyopathy

Abstract:PURPOSE:The goal of this study was to evaluate the accuracy of gated single photon emission computed tomography (SPECT) in the assessment of left ventricular (LV) end-diastolic/end-systolic volumes (EDV, ESV) and ejection fraction (LVEF) in patients with dilated cardiomyopathy, using cardiac magnetic resonance imaging (MRI) as the reference method. Furthermore, software-specific characteristics of Quantitative Gated SPECT (QGS), Emory Cardiac Toolbox (ECTB) and 4D-MSPECT were analysed.METHODS:Thirty-six patients with dilated cardiomyopathy who underwent gated (99m)Tc-methoxyisobutylisonitrile SPECT and cardiac MRI were included. LV EDV, ESV and LVEF values of gated SPECT were calculated using QGS, ECTB and 4D-MSPECT.RESULTS:The correlation between the results of gated SPECT and cardiac MRI was excellent for EDV [R = 0.872 (QGS), R = 0.879 (ECTB), R = 0.869 (4D-MSPECT)], ESV [R = 0.908 (QGS), R = 0.897 (ECTB), R = 0.880 (4D-MSPECT)] and LVEF [R = 0.794 (QGS), R = 0.763 (ECTB), R = 0.710 (4D-MSPECT)]. EDV and ESV assessed by QGS did not differ significantly from those assessed by cardiac MRI (all p = NS), whereas EDV and ESV were overestimated by ECTB and 4D-MSPECT compared with cardiac MRI (all p < 0.05). LVEF was overestimated by QGS, ECTB and 4D-MSPECT compared with cardiac MRI (all p < 0.05).CONCLUSIONS:The correlation between gated SPECT and cardiac MRI is excellent for LV volume and LVEF values calculated by QGS, ECTB and 4D-MSPECT in patients with dilated cardiomyopathy. However, algorithm-varying over- or underestimation of LV volumes and LVEF should be accounted for in the clinical context.
Clinical Study of Left Ventricular Ejection Fraction Measured from Gated Myocardial Perfusion SPECT with 99Tcm-Mibi by ECToolbox Software

Abstract:Objective To evaluate clinical value of ECToolbox software measuring left ventricular ejection fraction(LVEF) by gated myocardial imaging.Methods Rest gated myocardial SPECTS with 99Tcm-MIBI was performed on fifty-eight patients by ECToolbox software.All patients were divided into three groups according to EDV.R0,R1 and R2 were recorded.Equilibrium gated blood-pool imaging was also performed within a week.The correlation and concordance of the two methods measuring LVEF were observed.Results When EDV70 ml,there were no correlations between the two methods.The LVEF measured by ECToolbox software was significantly higher than that of gated blood-pool imaging.R1 was better.When EDV was between 70 and 100 ml,there was positive correlation between two methods and there was no significant difference of LVEF measured by R1 and gated blood-pool imaging.When EDV≥100 ml,there was strong correlation between the two methods and there was no significant difference of LVEF measured by R0 and gated blood-pool imaging.Conclusions The correlations and concordances of ECToolbox software and gated blood-pool imaging when measuring LVEF were all affected by EDV.R1 should be used when EDV was smaller than 100 ml,otherwise R0 should be used.

Abstract:Echocardiography is a widely used modality for the assessment of right ventricular (RV) function; however, few studies have comprehensively compared the accuracy of echocardiographic parameters using invasively obtained reference values. Therefore, this exploratory study aimed to compare the accuracy of echocardiographic parameters of RV function and RV–pulmonary artery (PA) coupling. We calculated four indices of RV function (end‐systolic elastance [Ees] for systolic function [contractility], τ for relaxation, and β and end‐diastolic elastance [Eed] for stiffness), and an index of RV–PA coupling (Ees/arterial elastance [Ea]), using pressure catheterization, cardiac magnetic resonance imaging, and a single‐beat method. We then compared the correlations of RV indices with echocardiographic parameters. In 63 participants (54 with pulmonary hypertension (PH) and nine without PH), Ees and τ correlated with several echocardiographic parameters, such as RV diameter and area, but the correlations were moderate (|correlation coefficients (ρ)| 0.65. In addition, RVFW‐LS/eSPAP yielded high sensitivity (0.84) and specificity (0.75) for detecting reduced Ees/Ea. The present study indicated a limited accuracy of echocardiographic parameters in assessing RV systolic and diastolic function. In contrast to RV function, they showed high accuracy for assessing RV–PA coupling, with RVFW‐LS/eSPAP exhibiting the highest accuracy.

Evaluation of right ventricular regional volume and systolic function in normal adults using real-time three-dimensional echocardiography

Abstract:Objective To explore the characteristics of right ventricular(RV)regional volume and systolic function in normal adults using real-time three-dimensional echocardiography(RT3DE).Methods RT3DE images were acquired from 53 normal adults for evaluation and analysis to obtain RV global and regional end-diastolic volume(EDV),end-systolic volume(ESV),systolic volume(SV)and ejection fraction(EF)in three compartments(inflow,body and outflow).The RV dyssynchrony parameters were calculated as the standard deviation(SD)and maximum difference(dif)of time to minimum systolic volume(Tmsv)and Tmsv corrected by heart rate(Tmsv%)in three RV compartments.Conventional echocardiographic parameters were calculated and recorded.Results Higher RV global and regional(body and outflow)EDV and SV,higher global and regional ESV and lower inflow EF were found in the male group when compared with the female group(all P<0.05).It was found that EDV and SV in the inflow compartment were the highest in the three compartments of the right ventricle(all P<0.05),and the difference of regional EF were ordered from larger to smaller as inflow>outflow>body(all P<0.05).The differences among global and regional Tmsv and Tmsv% were not significant.RV global EDV was positively correlated with RV end-diastolic diameters and area(all P<0.05).RV global ESV was positively correlated with RV end-systolic diameters and area(all P<0.05).Conclusions In normal adults,there were distinct characters of regional volume and systolic function in the three compartments of the right ventricle,with high systolic synchronicity and certain differences between males and females.RT3DE may play a potential role in the evaluation of RV regional volume and systolic function.
Evaluation of right ventricular functions in patients with ischemic cardiomyopathy by speckle-tracking echocardiography

Abstract:Background: It is widely recognized that the right ventricle plays a significant role in the prognosis of numerous diseases. However, the assessment of right ventricular function (RV) has not been given much attention until recently. This study used speckle-tracking echocardiography (STE) to assess RV functions in ischemic cardiomyopathy (ICM) patients. Results: This study included 74 patients diagnosed with ischemic cardiomyopathy (ICM) and an ejection fraction (EF) of less than 50%. Although all the selected patients had normal RV systolic function by tricuspid annular plane systolic excursion (TAPSE), a considerable percentage of them had subtle RV systolic dysfunction, which could be identified by right ventricular free wall longitudinal strain (RV FWLS) (36.5%) and right ventricular global longitudinal strain (RV GLS) (55.4%). Moreover, the mean RV FWLS was significantly higher than RV GLS (- 20.4 ± 5.08% vs. - 17.5 ± 6.89%), respectively. Advanced left ventricle (LV) adverse remodeling was associated with subtle RV dysfunction. Using multivariate regression analysis, increased E/e' (p = 0.016, CI 1.135-3.423) and RV myocardial performance index (MPI) (p = 0.007, CI 0.000-0.007) were identified as independent factors of impaired RV FWLS with the greatest effectiveness. Conclusion: When standard RV measures are normal in patients with ICM, RV systolic strain analysis offers an incremental utility to detect subtle abnormalities in RV function, especially in resource-constrained settings where cardiac magnetic resonance (CMR) is not practical.
Right Ventricular Diastolic Performance in Patients With Chronic Thromboembolic Pulmonary Hypertension Assessed by Echocardiography

Abstract:Background: There have been no systemic studies about right heart filling pressure and right ventricular (RV) distensibility in patients with chronic thromboembolic pulmonary hypertension (CTEPH). Therefore, we aimed to explore combinations of echocardiographic indices to assess the stages of RV diastolic dysfunction. Methods and Results: We recruited 32 healthy volunteers and 71 patients with CTEPH. All participants underwent echocardiography, cardiac catheterization (in patients with CTEPH), and a 6-min walk test (6MWT). The right atrial (RA) end-systolic area was adjusted for body surface area (BSA) (indexed RA area). RV global longitudinal diastolic strain rates (SRs) and RV ejection fraction (EF) were measured by speckle tracking and three-dimensional echocardiography (3D echo), respectively. All 71 patients with CTEPH underwent pulmonary endarterectomy. Of the 71 patients, 52 (73%) had decreased RV systolic function; 12 (16.9%), 26 (36.6%), and 33 (46.5%) patients had normal RV diastolic pattern, abnormal relaxation (stage 1), and pseudo-normal patterns (stage 2), respectively. The receiver operating characteristic curve analysis showed that the optimal cut-off values of early diastolic SR <0.8 s-1 and indexed RA area > 8.8 cm2/BSA had the best accuracy in identifying patients with RV diastolic dysfunction, with 87% sensitivity and 82% specificity. During a mean follow-up of 25.2 months after pulmonary endarterectomy, the preoperative indexed RA area was shown as an independent risk factor of the decreased 6MWT distance. Conclusions: Measuring early diastolic SR and indexed RA area would be useful in stratifying RV diastolic function.

Abstract:PURPOSE: To explore the usefulness of myocardial late iodine enhancement (LIE) and extracellular volume (ECV) quantification by using dual-energy cardiac CT.MATERIALS AND METHODS: In this single-center retrospective study, a total of 40 patients were evaluated with LIE CT by using a dual-layer spectral detector CT system. Among these, 21 also underwent cardiac MRI. Paired image sets were created by using standard imaging at 120 kVp, virtual monochromatic imaging (VMI) at 50 keV, and iodine density imaging. The contrast-to-noise ratio and image quality were then compared. Two observers assessed the presence of LIE and calculated the interobserver agreements. Agreement between CT and cardiac MRI when detecting late-enhancing lesions and calculating the ECV was also assessed.RESULTS: The contrast-to-noise ratio was significantly higher by using VMI than by using standard 120-kVp imaging, and the mean visual image quality score was significantly higher by using VMI than by using either standard or iodine density imaging. For interobserver agreement of visual detection of LIE, the agreement for VMI was excellent and the κ value (κ, 0.87) was higher than that for the standard 120-kVp (κ, 0.70) and iodine density (κ, 0.83) imaging. For detecting late-enhancing lesions, agreement with cardiac MRI was excellent by using VMI (κ, 0.90) and iodine density imaging (κ, 0.87) but was only good by using standard 120-kVp imaging (κ, 0.66). Quantitative comparisons of the ECV calculations by using CT and cardiac MRI showed excellent correlation (r 2 = 0.94).CONCLUSION: Dual-energy cardiac CT can assess myocardial LIE and quantify ECV, with results comparable to those obtained by using cardiac MRI.© RSNA, 2019See also the commentary by Litt in this issue.

Abstract:Background: Regional myocardial work (MW) is not measured in the right ventricle (RV) due to a lack of high spatial resolution regional strain (RS) estimates throughout the ventricle. We present a cineCT-based approach to evaluate regional RV performance and demonstrate its ability to phenotype three complex populations: end-stage LV failure (HF), chronic thromboembolic pulmonary hypertension (CTEPH), and repaired tetralogy of Fallot (rTOF). Methods: 49 patients (19 HF, 11 CTEPH, 19 rTOF) underwent cineCT and right heart catheterization (RHC). RS was estimated from full-cycle ECG-gated cineCT and combined with RHC pressure waveforms to create regional pressure-strain loops; endocardial MW was measured as the loop area. Detailed, 3D mapping of RS and MW enabled spatial visualization of strain and work strength, and phenotyping of patients. Results: HF patients demonstrated more overall impaired strain and work compared to the CTEPH and rTOF cohorts. For example, the HF patients had more akinetic areas (median: 9%) than CTEPH (median: <1%, p=0.02) and rTOF (median: 1%, p<0.01) and performed more low work (median: 69%) than the rTOF cohort (median: 38%, p<0.01). The CTEPH cohort had more impairment in the septal wall; <1% of the free wall and 16% of the septal wall performed negative work. The rTOF cohort demonstrated a wide distribution of strain and work, ranging from hypokinetic to hyperkinetic strain and low to medium-high work. Impaired strain (-0.15<=RS) and negative work were strongly-to-very strongly correlated with RVEF (R=-0.89, p<0.01; R=-0.70, p<0.01 respectively), while impaired work (MW<=5 mmHg) was moderately correlated with RVEF (R=-0.53, p<0.01). Conclusions: Regional RV MW maps can be derived from clinical CT and RHC studies and can provide patient-specific phenotyping of RV function in complex heart disease patients.

End-Systolic Eccentricity Index Obtained by Enhanced Computed Tomography Is a Predictor of Pulmonary Vascular Resistance in Patients with Chronic Thromboembolic Pulmonary Hypertension

Abstract:The usefulness of the parameters of biventricular function simultaneously measured using enhanced multi-detector computed tomography (MDCT) pulmonary angiography in patients with chronic thromboembolic pulmonary hypertension (CTEPH) has not been clarified. This study aimed to verify the correlation between left and right ventricular (RV) parameters and pulmonary vascular resistance (PVR). Patients who underwent enhanced MDCT before diagnostic right heart catheterization at Nagoya University Hospital between October 2014 and April 2021 were enrolled. The correlation of biventricular function and volume parameters with PVR was assessed. Eighty patients were retrospectively analyzed. Patients’ mean age was 65 ± 13 years, mean PVR was 9.1 (range, 6.1−11.3) Wood units, and mean end-systolic eccentricity index (esEI) was 1.76 ± 0.50. RV end-systolic volume (ESV) (p = 0.007), RV cardiac output (CO) (p < 0.001), RV ejection fraction (p < 0.001), LV end-diastolic volume (EDV) (p < 0.001), left ventricular (LV) ESV (p = 0.006), LVCO (p < 0.001), end-diastolic EI (p < 0.001), and esEI (p < 0.001) were significantly correlated with PVR. The LVEDV (p = 0.001) and esEI (p < 0.009) were independent predictors of PVR. Systolic pulmonary arterial pressure (PAP) (p < 0.001), diastolic PAP (p < 0.001), mean PAP (p < 0.001), right atrial pressure (p < 0.023), and PVR (p < 0.001) were significantly higher in the high esEI group than in the low esEI group. The esEI was a simple predictor of CTEPH severity.
Quantitative Assessment of Right Ventricle by Dynamic Three Dimensional Voxel Imaging of Echocardiography

Abstract:Purpose To evaluate the right ventricular function by dynamic three-dimensional reconstruction of multiplane transesophageal echocardiography with voxel imaging. Methods Thirty-four patients with pulmonary hypertension were divided into two groups, group A included patients with volume overload, group B included patients with pressure overload. Control group included 15 normal subjects. Results Right ventricle(RV) volume and free wall mass of all the normal subjects and patients had been successfully reconstructed. Compared with normal subjects', RV of pulmonary hypertension patients was bigger and the excursion of apex and infundibulum was reduced. The mean free wall mass of normal subjects and pulmonary hypertension patients were (18.17±5.23) and (42.09±17.90)g/m2 respectively. The mean end of distolic volume of normal subjects and patients were (34.57±11.01) and (73.26±38.71)mL/m2 respectively. The mean end systolic volume of normal subjects and patients were (19.17±6.34) and (45.40±26.30)mL/m2 respectively. Quantitative analysis revealed that both of RV volme and free wall mass of pulmonary hypertension patients were greater than those of normal people(P0.05). The RV volume and free wall mass in the volume overload group was compared with those of pressure overload. The result showed that the free wall mass of RV due to pressure overload[(50.27±20.87)mL/m2] were significantly greater than that of RV due to volume overload[(33.91±9.14)mL/m2] (P0.05). As for right ventricular vo-lume and ejection fraction, there was no significant difference between these two groups. Conclusions The dynamic 3D reconstruction of multiplane transesophageal echocardiography with Voxel imaging can precisely evaluate the right ventricular volume, free wall mass and ejection fraction in patients with pulmonary hypertension. It can be applied clinically to assess the right heart function, the severity, treatment outcomes and prognosis of right heart diseases.

Abstract:PURPOSE: To evaluate the relationship between myocardial extracellular volume (ECV) fraction measured using dual-energy computed tomography with late iodine enhancement (LIE-DECT) and risk of heart failure (HF) in patients without coronary artery disease (CAD), and to evaluate the relationship between ECV and left ventricular structure and function.MATERIALS AND METHODS: Sixty consecutive HF patients without CAD and 60 consecutive participants without heart disease who underwent coronary CT angiography (CCTA) following LIE-DECT were included. ECV of the left ventricle was calculated from the iodine maps and hematocrit levels using the American Heart Association (AHA) 16-segment model. Cardiac structural and functional parameters were collected including left ventricular end-systolic volume (LVESV), left ventricular end-diastolic volume (LVEDV), left ventricular ejection fraction (LVEF), left atrial volume (LAV), interventricular septal thickness (IVST), and N-terminal pro-brain natriuretic peptide (NT-pro-BNP).RESULTS: ECV in HF patients without CAD (31.3 ± 4.0 %) was significantly higher than that in healthy subjects (27.1 ± 3.7 %) (P < 0.001). Multivariate linear analysis revealed that ECV was associated with age (β = 0.098, P = 0.010) and hypertension (β = 2.093, P = 0.011) in all participants. Binary logistic regression analysis showed that after adjusting for age, sex, body mass index (BMI), smoking, and drinking, ECV was a risk factor affecting the occurrence of HF in those without CAD (OR = 1.356, 95 %CI:1.178-1.561, P < 0.001). A positive correlation was found between ECV and NT-pro-BNP, LVEDV, LVESV, and LAV (r = 0.629, 0.329, 0.346, and 0.338, respectively; all P < 0.001) in all participants.CONCLUSIONS: ECV could be measured using LIE-DECT iodine maps. ECV elevation was a risk factor for HF without CAD and correlated with cardiac structure and function.

Abstract:OBJECTIVE To validate a Doppler index of diastolic filling (E/Em) during exercise in the early stage of the patients with acute myocardial infarction,and investigate its association with exercise capacity. METHODS The study enrolled consecutive 60 patients with first ST segment elevation myocardial infarction(STEMI)in Killip I-II stage and who underwent percutaneous coronary intervention (PCI) from March 2008 to August 2008(as study group), At 10-14 days after acute myocardial infarction, Cardiopulmonary Exercising Testing (CPET, Modified Bruce flat treadmill protocol, symptom-limited flat exercise tolerance test) was conducted with Tissue Doppler Imaging (TDI) evaluation at rest and instantly during exercising peak. Doppler index of diastolic filling was also measured at rest and immediately after Cardiopulmonary Exercising Testing in 30 age-matched patients as control whose coronary angiography (CAG) or coronary computed tomography was negative. RESULTS All patients completed the TDI evaluation at rest and after CPET. There was no significant difference in E/Em measured by TDI in control group at rest and during exercise. In study group, 29(48%,29/60) patients had a normal E/Em both at rest and exercise, 21(35%,21/60) patients had elevation of E/Em at rest, and 10(17%,10/60) patients had elevation of E/Em only during exercise. There was a good negative correlation between E/Em and metabolic equivalent at rest (r=-0.54, P<0.01) and during exercise (r=-0.40, P<0.01). The patients which E/Em>10 at rest(METs is 4.6+/-1.6, P<0.01) and patients who had E/Em>10 only during exercise (METs is 5.8+/-1.6, P<0.01) had a reduced exercise capacity compared with the patients with normal E/Em both at rest and during exercise (METs is 6.2+/-1.8). CONCLUSION The ratio of early diastolic transmitral velocity to early diastolic tissue velocity (E/Em) by the stress echocardiography can be used to reliably identify the patients with acute myocardial infarction with elevated LVFP during exercise and reduced exercise capacity.
Effective RV ejection fraction by CMR: a new index of RV function in patients with severe TR

Abstract:Background Right ventricle (RV) dysfunction represent an formal indication for intervention in patients with significant tricuspid regurgitation (TR). RV ejection fraction (RVEF) by Cardiac Magnetic Resonance (CMR) is considered the gold standard of RV function; however it is influenced by changes of preload conditions and may remain unaffected until late stages in severe TR. Effective RV ejection fraction (eRVEF) is corrected by the amount of TR and may reflect an earlier parameter of RV dysfunction. Purpose To compare the prognostic impact of both indices of RV function: RVEF and eRVEF in a multicentre cohort of patients with severe TR. Methods Patients with at least severe TR assessed by 2D echocardiography with a contemporary cardiac magnetic resonance (CMR) study were included in this study. In addition to conventional parameters of biventricular volume and function, eRVEF was assessed in all patients. eRVEF was calculated according to the formula: eRVEF=net pulmonary forwardflow/RV− EDV (figure). A combined endpoint of hospital admission due to right heart failure and cardiovascular mortality was defined. Results 271 patients with severe TR recruited from 5 tertiary care hospitals were included in this study (age 71 ± 11 years, 67% female, 83% NYHA I or II). Regarding the etiology, 10% were primary TR, 1% CIED-related TR, 46 % atrial secondary TR and 43% ventricular secondary TR. After a median follow-up of 27 months (IQR: 10-53 months), 41% of the patients (n=112) experienced the combined endpoint (n=97 were admitted due to HF, n=49 died). After adjusting for age, NYHA class, comorbidities, type of TR and LVEF in multivariate Cox proportional models, both RVEF and eRVEF were independently associated with all-cause mortality and heart failure (adj HR for RVEF HR per 1%=0.96 [0.94-0.98], p<0.001 and for eRVEF per 1%=0.98 [0.96-0.99], p<0.001). A value of eRVEF of ≤37% were associated with impaired prognosis (figure). eRVEF identified higher rates of RV dysfunction compared to RVEF (49% vs 38%, p<0.001). eRVEF was superior to RVEF to predict outcomes (higher AUC 0.70 [0.63-0.76] vs 0.66 [0.59-0.73, p<0.001; Δ C-statistic 0.139 [0.040-0.237], p=0.05). Conclusion RV function is crucial for determining optimal timing for TR intervention. eRVEF is corrected by the amount of TR and identify higher rates of RV dysfunction in severe TR. eRVEF held stronger association with outcome compared to RVEF.eRVEF calculationKaplan Meier Curves
