Abstract:Few studies have focused on the toxicological risks of dairy fat intake. A standard dairy fat (SDF) with a 70% SFA content and a naturally enriched dairy fat (EDF) in vaccenic, rumenic and α-linolenic acids and low in SFA (54%) have been examined in a 4-week repeated dose oral toxicity study as a daily dose of 2000 mg/kg bw by gavage in rats. Comparisons were established with a third group of rats (control) which did not receive fat administration. Both fats were well tolerated, and no adverse events or mortality were observed during the treatment nor after a 2-week observation period. EDF and SDF did not cause significant differences with respect to a control group in body weight gain, food consumption, clinical observations, organ weight ratios, histopathological findings and most of the hematological and biochemical parameters including total cholesterol and cholesterol fractions in plasma. In rats treated with SDF, a significant increase of triglycerides was observed as compared to the control group. By contrast, in rats treated with EDF, a significant decrease in triglycerides was detected. EDF orally administered to rats was safe, and no treatment-related toxicity was detected. The results also suggest that EDF could protect against the increase of triglyceride concentrations in plasma.
Advantages of Autologous Platelet-Rich Fibrin Membrane on Gingival Crevicular Fluid Growth Factor Levels and Periodontal Healing: A Randomized Split-Mouth Clinical Study

Abstract:Background: This study evaluates contributions of platelet-rich fibrin (PRF) combined with conventional flap surgery on growth factor levels in gingival crevicular fluid (GCF) and periodontal healing. Methods: Twenty-six patients (52 sites) with chronic periodontitis were treated either with autologous PRF with open flap debridement (OFD+PRF) or OFD alone. Growth factor levels in GCF at baseline and 2, 4, and 6 weeks after surgery were analyzed, and clinical parameters such as probing depth (PD), relative clinical attachment level (rCAL), and gingival margin level (GML) at baseline and 9 months after surgery were measured. Results: Mean PD reduction and rCAL gain were significantly greater in OFD+PRF sites than in OFD sites. Mean GML change was -0.38 + 0.10 mm in OFD sites and 0.11 + 0.08 mm in the test group; difference between the two groups was statistically significant (P <0.05). Both groups demonstrated increased expression levels of fibroblast growth factor-2, transforming growth factor-β1, and platelet-derived growth factor-BB at 2 weeks compared with baseline, followed by reductions at 4 and 6 weeks. The OFD+PRF group showed significantly higher growth factor levels compared with the OFD group at 2 and 4 weeks. Conclusion: PRF membrane combined with OFD provides significantly higher GCF concentrations of angiogenic biomarkers for ≈2 to 4 weeks and better periodontal healing in terms of conventional flap sites.
Quantification of growth factors in advanced platelet-rich fibrin and concentrated growth factors and their clinical efficiency as adjunctive to the GTR procedure in periodontal intrabony defects

Abstract:Background The development of platelet concentrated biomaterials has gained increasing awareness for regenerative medicine. With different protocol, derivatives such as advanced platelet-rich fibrin (A-PRF), injected platelet-rich fibrin, and concentrated growth factor (CGF) have been demonstrated effectively in preclinical and clinical studies. The aim of this study was to compare the level of growth factors releasing from A-PRF and CGF, and their clinical efficacy in the regenerative management of intrabony defects (IBDs). Methods Thirty-two blood samples were collected from eight healthy donors and assessed for platelet-derived growth factor-alpha beta, vascular endothelial growth factor, bone morphogenetic protein-2, and transforming growth factor-beta 1 release at indicated times. In addition, the clinical records of 45 patients (15 per group) who had undergone guided tissue regeneration (GTR) with or without A-PRF/CGF were retrieved. The probing depth (PD) and clinical attachment level (CAL) were recorded preoperatively and 6 months postoperatively. Intrabony component (IC) depth, radiographic bone level (RBL), and bone defect filling were assessed radiographically. Results A-PRF had a looser fibrin network than the CGF but presented larger amounts of growth factors with a more sustained release period. Although there was no difference in PD reduction, CAL gain, RBL height change and defect filling (%) between A-PRF and CGF group, both achieved a more favorable clinical result in IC height reduction and defect filling (%) than the control. Conclusions A-PRF and CGF have the ability to stimulate a continual and steady release of total growth factors over a 14-day period. A-PRF and CGF show a similar effectiveness in periodontal bone regeneration with a potential benefit of improving GTR outcomes in IBD treatment.
Angioedema in the emergency department: an evidence-based review.

Abstract:Angioedema is the end result of a variety of pathophysiological processes resulting in transient, localized, nonpitting swelling of the subcutaneous layer of the skin or submucosal layer of the respiratory or gastrointestinal tracts. It is now generally accepted that the swelling is mediated by either histamine or bradykinin. Angioedema may result in severe upper airway compromise or-less commonly recognized-compromise in the gastrointestinal tract often associated with severe abdominal pain. A variety of new therapeutic options are becoming available for use in the United States that have the potential to greatly impact the management and outcomes for those with severe clinical manifestations. This review assesses the evidence on the causes and treatments of angioedema in the emergency department and reviews the new therapeutic options available for treatment of angioedema based on their effectiveness, price, and availability.

Abstract:Abstract Introduction: Platelet-rich fibrin (PRF) accelerates wound healing by promoting faster cicatricial tissue remodeling and excellent neovascularization. Hyaluronic acid (HA) being biocompatible, anti-inflammatory, and proangiogenic leads to improvement in the rate of wound healing. Aim: The aim of this study was to compare the effect of PRF membrane and 0.2% HA gel on wounds after gingival depigmentation surgery. Materials and Methods: This study was carried out on 30 systemically healthy individuals recruited from the pool of patients who visited the department of periodontology. After depigmentation procedure in every patient, the sites were divided into three even groups. Group A received PRF membrane and periodontal dressing, Group B received 0.2% HA gel application and periodontal dressing, and Group C served as a control group in which only periodontal dressing was placed. The individuals were evaluated for the healing index (HI) and Numerical Rating Scale (NRS) on the 3 rd and 5 th day. Epithelialization test (ET) was performed on the 5 th day. Gingival biotype was reassessed and compared to preoperative value after 3 months. The clinical trial was expressed in terms of mean and standard deviation. Intra-group comparison and inter-group comparison were done through the Kruskal–Wallis ANOVA test. All statistical tests were performed through SPSS version 25.0 (IBM). Results: The inter-group statistical analysis concerning NRS, HI, and ET showed statistically significant results in Groups A and B compared to Group C ( P < 0.05), while gingival biotype showed statistically significant results in Group A compared to Groups B and C. Conclusion: The use of PRF membrane and HA gel to protect the raw wound site of depigmented gingiva proved to be an effective approach, resulting in faster healing.

Abstract:Background The primary purpose of this two‐arm, parallel design, randomized controlled study is to compare healing of the palatal tissue donor site when platelet‐rich fibrin (PRF) is used as a wound dressing compared to the use of a hemostatic agent. Secondary outcomes of patient pain perception and analgesic intake were also evaluated. Methods Seventy‐four patients receiving free gingival grafts were randomized to receive either PRF (test) or hemostatic agent (control) as a palatal wound dressing by patients selecting a sealed envelope containing their group assignment (initially 37 envelopes for PRF group and 37 for hemostatic agent group). Patient pain assessment and analgesic consumption were documented using a 21‐point numerical scale (NMRS‐21) at 24, 48, and 72 hours post‐surgery. At 1‐, 2‐, 3‐, and 4‐week follow‐up appointments palatal early healing index (PEHI) scores including wound color, epithelialization, presence or absence of swelling, granulation tissue, and bleeding on gentle palpation were generated by direct intraoral examination by a blinded examiner unaware of the patients' treatment group. Results NMRS‐21 pain scores showed a significant reduction in pain over time in both groups, with no significant difference between groups at any time point. No significant between‐group difference was found in the amount of analgesics taken by patients at 24, 48, and 72 hours. There was significant improvement in PEHI scores over the 4‐week time period in both groups, but there was no significant difference in PEHI score at each time point (1, 2, 3, 4 weeks) between groups. Conclusions Study findings suggest that there is no difference in early palatal wound healing, patient pain perception, or analgesic consumption between use of PRF or a hemostatic agent as donor‐site wound dressings.

Abstract:ObjectivesThe aim of this study is to determine the use of platelet-rich fibrin (PRF) in the management of soft tissue donor site healing after harvesting connective tissue graft (CTG) from the palate and evaluate the postoperative discomfort (pain, bleeding, analgesic consumption, tissue color match) of patients.Materials and methodsForty patients were randomly assigned to PRF or control group. In the PRF group, PRF membrane was placed into CTG donor site. After surgery, delayed bleeding, early healing index (EHI), tissue color match, and analgesic consumption were recorded. The visual analog scale (VAS) was used to observe the postoperative pain and tissue color match. Data were analyzed using the independent sample t test and the repeated measure ANOVA test.ResultsThe patients in the PRF group reported significantly lower pain scores at all-time points. Postoperative 3rd and 7th day, EHI scores were lower in the favor of the PRF group. VAS score values of tissue color match were lower in the control group at 7th and 14th day, compared with the PRF group. Analgesic intake was significantly lower in the PRF group postoperatively at 1st and 3rd day that of control group.ConclusionPRF application at the palatal donor site demonstrates promising results in terms of better wound healing and reduced postoperative discomfort in the patients after harvesting CTG.Clinical relevancePatients may avoid surgical operations because of the discomfort or pain feeling. Reducing postoperative pain and discomfort and accelerating recovery meet the wishes of every patient and physician. PRF can provide these requirements as an easy method to obtain and implement.

Levels of platelet activating factor in gingival crevice fluid following periodontal surgical therapy

Abstract:Background and objective: Elevated levels of platelet activating factor (PAF), a potent inflammatory phospholipid mediator, have been previously detected in gingival tissues and gingival crevice fluid (GCF) in periodontal disease. However, the role of this mediator during wound healing after periodontal surgery remains unclear. The hypothesis, a relationship between PAF levels and periodontal healing, was tested by measuring PAF levels in GCF samples collected from sites that had undergone guided tissue regeneration (GTR) or flap surgery. Material and methods: Using a split-mouth design, 30 intrabony defects were randomly assigned to treatment with GTR (group 1) or to flap surgery (group 2). GCF was sampled pre-operatively and at 6-, 12- and 24-wk follow-up evaluation visits. PAF levels in GCF were analyzed by high-performance liquid chromatography (HPLC). Results: Both treatment modalities significantly reduced the probing pocket depth and improved the clinical attachment level (p < 0.01). Compared with pre-operative values, the GCF volume and PAF levels were significantly decreased at postoperative weeks 6, 12 and 24 in both groups (p < 0.01). There were also significant differences in GCF volume and PAF levels at all time points up to 24 wks in both groups (p < 0.01). No statistically significant differences were observed in any of the parameters investigated between the two groups (p > 0.05). Conclusion: PAF is detectable in GCF by HPLC and showed a continuous decrease at all the time points monitored following periodontal surgical therapy. This suggests that changes in the levels of this mediator in GCF might be useful for monitoring the progress of periodontal repair and regeneration.

Abstract:Introduction: Fibrin, fibronectin, platelet derived growth factor, and transforming growth factors from platelet concetrate are crucial for tissue reparation and regeneration. Objective: This study was designed to evaluate clinical effectiveness of activated platelet-rich fibrin (PRF) membrane in treatment of gingival recession. Methods: 19 gingival recessions Miller class I or II were treated with a coronally advanced flap and the PRF membrane (PRF group). Following the elevation of the flap, bone and root surfaces were covered with the PRF membrane. After suturing, the PRF membrane was covered with a coronally advanced flap. In the same patients, 19 other gingival recessions were treated with CTG in combination with the coronally advanced flap (the CTG group). Clinical recordings were made of vertical recession depth (VRD), probing depth (PD), clinical attachment level (CAL) and keratinized tissue width (KTW) before and 12 months after mucogingival surgical treatment. Clinical evaluation of healing events was estimated with recordings of the healing index (HI). Recordings of HI were performed in the 1st, 2nd and 3rd week post-surgically. Results: Mean root coverage was significant in both groups (the PRF group 79.94% and the CTG group 88.56% %; p < 0.01). The difference between the two tested groups was not statistically significant. Results of the keratinized tissue width showed significant increase (p < 0.05) 12 months after the surgery in both, the PRF and CTG groups. Results of KTW showed statistical significance of recorded differences obtained in the two evaluated groups (p < 0.05). There was no statistical significance in reduction of PD and CAL recorded in the PRF and CTG groups. The values of HI recorded in the 1st and 2nd week postoperatively were significantly enhanced in the PRF group (p < 0.05). Conclusion: Results of this study confirm both procedures as effective with equivalence of clinical results in solving gingival recession problems. The utilization of the PRF resulted in a decreased postoperative discomfort and advanced tissue healing.

Abstract:This experimental study aimed to evaluate the effects of injectable platelet-rich fibrin (i-PRF) on mucosal healing and the release of growth factors in rats. 40 rats were used; i-PRF was administered in the right buccal area while saline was injected in the left. Cytokeratin, FGF, PDGF, TGF, and VEGF expressions were determined with immunohistochemistry. Gene expressions of EGF, TGF-β, and VEGF were analysed. Epithelialization started on the 3rd day, and connective tissue maturation was more prominent in the i-PRF-applied group. Also, the releases of VEGF, EGF, TGF-β, PDGF, and FGF were higher in the i-PRF group during the 14 days. Gene expression analysis showed that changes in TGF-β at 14 days after i-PRF injection and VEGF after 21 days were statistically significant. The results of this study suggested that autologous i-PRF application enhanced the healing of oral mucosal wounds by increasing the release of growth factors for 21 days.

Abstract:Background: The combination of the coronally positioned flap with connective tissue grafting in treating gingival recession defects has been shown to demonstrate the highest success rate. Aim: The aim of the present study was to compare and evaluate the efficiency between autologous fibrin glue (AFG) and conventional absorbable sutures in the treatment of marginal tissue recessions using connective tissue grafts (CTGs) and the coronally advanced flap (CAF) technique. Methods: Twenty Miller's class I and II recession sites in maxillary or mandibular areas in chronic periodontitis patients were treated as group I (test): CTG + CAF + AFG; and group II (control): CTG + CAF with conventional absorbable sutures. Clinical parameters in both groups were recorded at baseline (t0) and six months (t1) postoperatively in terms of gingival recession height (RH), root coverage esthetic score (RCES), visual analogue scale (VAS), and healing index (HI). Results: The RH, RCES, VAS, and HI were significantly reduced at the end of six months compared to baseline ( P < 0.005) in both groups; however, there was no statistical difference between the groups, indicating the equivalent efficacy of AFG and conventional sutures in healing. Conclusion: All the treated sites were found to have appreciable root coverage with satisfactory post-operative healing. Hence, it may be suggested that both AFG and absorbable sutures are equally efficient in post-surgical soft tissue healing.

Abstract:OBJECTIVES This study compared the effects of L-PRF and A-PRF on patients' quality of life and the healing of palatal wounds after free gingival graft harvesting. METHOD AND MATERIALS After FGG harvesting, palatal donor sites of 36 patients were assigned one of the three group: 1)L-PRF group, 2) A-PRF group and 3) Palatal stent+periodontal pack group (control group).Wound healing was evaluated by H2O2 test. Pain was evaluated by visual analog scale (VAS)and number of analgesics consumed. For patient quality of life, the Oral Health Impact Profile-14 (OHIP-14) questionnaire was utilized. RESULTS Complete epithelialization was higher in thecontrol group on the 7th day than test groups (p< 0.05). Complete epithelization was achievedin all groups 14th day postoperatively. VAS scores for pain and the number of analgesic tabletsconsumed were similar in all groups (p >0.05). OHIP-14 total scores were similar in all studygroups at 7th day postoperatively (p>0.05). OHIP-14 total and physical pain scores was lowerin L-PRF group than A-PRF group at 14th day postoperatively (p< 0.05). CONCLUSION Asidefrom the slight superiority of L-PRF over A-PRF, it can be concluded that both PRF procedureshave similar effects on palatal wound healing and quality of life.

Abstract:The transition to the integral-quantum-Hall-effect regime through the suppression of backscattering is studied theoretically on a model system by calculating the two-terminal conductance of a quasi-one-dimensional quantum wire as a magnetic field is applied in the presence of disorder. The quantum Hall regime is entered when there is negligible overlap between electron edge states localized at opposite sides of the wire. Levitation of the quantized plateaus with increasing disorder is observed. The conductance is calculated from a multichannel Landauer-B\"uttiker formula with transmission matrices determined by a recursive Green's-function technique on a tight-binding lattice.

Abstract:ObjectiveTitanium-prepared platelet-rich fibrin (T-PRF) is activated with titanium, which results in a more mature and aggregated form than PRF. In our previous studies, we established that the fibrin carpet formed with titanium had a firmer network structure, and longer resorption time in the tissue than the fibrin carpet formed with glass. The purpose of this randomized controlled clinical trial is to compare the effects of autogenous T-PRF and connective tissue graft (CTG).Materials and methodsA total of 114 Miller Class I/II gingival recessions with abrasion defects were treated either T-PRF (63 teeth) or CTG (51 teeth) using a modified tunnel technique. Clinical periodontal indexes, keratinized tissue (KTW), gingival thickness, and recession depth were recorded before surgery and at 6- and 12-month follow-up examinations. The visual analog scale and healing index scores were assessed.ResultsThe mean root coverages were 93.29 and 93.22% in the T-PRF and CTG groups, respectively, at 12 months post-operatively. CTG resulted in greater gingival thickness than T-PRF at 6 and 12 months post-surgery compared to baseline. Furthermore, the mean amounts of KTW increased by 1.97 and 0.75 mm in the T-PRF and CTG groups, respectively.ConclusionWithin the limits of this study, the results demonstrated that T-PRF is safe and effective for treatment of multiple Miller Class I/II gingival recession defects.Clinical relevanceT-PRF can serve as a good autogenous alternative to CTG, which is the gold standard for root coverage.

Abstract:In this paper we provide a simple proof of the extension theorem for partial orderings due to Suzumura [1983] when the domain of the partial order is finite. The extension theorem due to Szpilrajn [1930] follows from this theorem. Szpilrajns extension theorem is used to show that an asymmetric binary relation is contained in the asymmetric part of a linear order if and only if it is acyclic. This theorem is then applied to prove three results. Finally we introduce the concept of a threshold choice function, and our third result says that such choice functions are the only ones to satisfy a property called functional acyclicity.
