Abstract:Background The primary purpose of this two‐arm, parallel design, randomized controlled study is to compare healing of the palatal tissue donor site when platelet‐rich fibrin (PRF) is used as a wound dressing compared to the use of a hemostatic agent. Secondary outcomes of patient pain perception and analgesic intake were also evaluated. Methods Seventy‐four patients receiving free gingival grafts were randomized to receive either PRF (test) or hemostatic agent (control) as a palatal wound dressing by patients selecting a sealed envelope containing their group assignment (initially 37 envelopes for PRF group and 37 for hemostatic agent group). Patient pain assessment and analgesic consumption were documented using a 21‐point numerical scale (NMRS‐21) at 24, 48, and 72 hours post‐surgery. At 1‐, 2‐, 3‐, and 4‐week follow‐up appointments palatal early healing index (PEHI) scores including wound color, epithelialization, presence or absence of swelling, granulation tissue, and bleeding on gentle palpation were generated by direct intraoral examination by a blinded examiner unaware of the patients' treatment group. Results NMRS‐21 pain scores showed a significant reduction in pain over time in both groups, with no significant difference between groups at any time point. No significant between‐group difference was found in the amount of analgesics taken by patients at 24, 48, and 72 hours. There was significant improvement in PEHI scores over the 4‐week time period in both groups, but there was no significant difference in PEHI score at each time point (1, 2, 3, 4 weeks) between groups. Conclusions Study findings suggest that there is no difference in early palatal wound healing, patient pain perception, or analgesic consumption between use of PRF or a hemostatic agent as donor‐site wound dressings.

Abstract:Objective To study the results of petrosal bone imaging and audiometric long-term follow-up of two patients with branchio-oto-renal (BOR) syndrome and relate them to the clinical features, including caloric responses. Study Design Longitudinal case study. Setting Tertiary referral center. Patients A father and son with the BOR syndrome. Main Outcome Measures Both patients underwent imaging studies to detect and evaluate inner ear anomalies. Longitudinal audiometric analysis of the hearing threshold data over the previous 23 years was performed. Caloric tests were performed at various ages. Results The son had a short, wide internal acoustic canal, a hypoplastic cochlea, a plump vestibule, and a wide vestibular aqueduct on both sides; the semicircular canals and endolymphatic sac were of normal size. He showed progressive fluctuant sensorineural hearing loss. Caloric tests disclosed hyporeflexia on the left side. The father had a plump internal acoustic canal and hypoplastic cochlea on both sides. The left vestibule was hypoplastic, and the left vestibular aqueduct was marginally enlarged. He showed severe hearing impairment, without substantial progression or fluctuation, and caloric areflexia on the left side. Conclusion These findings suggest a correlation between progressive fluctuant sensorineural hearing loss with caloric hypofunction and the presence of an enlarged vestibular aqueduct in the BOR syndrome. Additional longitudinal case studies are needed to further evaluate such a correlation.

Abstract:Aims This study aimed to evaluate the effectiveness of a chitosan‐based dressing (CD) in achieving early wound healing and hemostasis at palatal donor sites in patients undergoing free gingival graft (FGG) surgery. Methods Thirty‐two patients requiring FGG were treated in this randomized controlled clinical trial. Complete epithelialization (CE) and color match (CM) at donor sites were assessed by a blinded examiner on postoperative days 7, 14, 21, and 28. Donor sites were compressed for 2 min with wet gauze (WG) alone in control group (CG) or WG + CD in test group (TG) immediately after graft harvesting, and immediate bleeding (IB) was recorded (yes/no). Delayed bleeding (DB) (for 1 week), and number of analgesic tablets consumed, and VAS scores for pain (for 2 weeks) were recorded by patient every day. Results Twenty‐eight patients (14 in each group) were included in final analysis. The prevalence of CE (at weeks 2 and 3) and VAS scores for CM scores were higher in TG but the intergroup differences were statistically significant only for CM (at week 4). Abstract:Abstract Introduction: Platelet-rich fibrin (PRF) accelerates wound healing by promoting faster cicatricial tissue remodeling and excellent neovascularization. Hyaluronic acid (HA) being biocompatible, anti-inflammatory, and proangiogenic leads to improvement in the rate of wound healing. Aim: The aim of this study was to compare the effect of PRF membrane and 0.2% HA gel on wounds after gingival depigmentation surgery. Materials and Methods: This study was carried out on 30 systemically healthy individuals recruited from the pool of patients who visited the department of periodontology. After depigmentation procedure in every patient, the sites were divided into three even groups. Group A received PRF membrane and periodontal dressing, Group B received 0.2% HA gel application and periodontal dressing, and Group C served as a control group in which only periodontal dressing was placed. The individuals were evaluated for the healing index (HI) and Numerical Rating Scale (NRS) on the 3 rd and 5 th day. Epithelialization test (ET) was performed on the 5 th day. Gingival biotype was reassessed and compared to preoperative value after 3 months. The clinical trial was expressed in terms of mean and standard deviation. Intra-group comparison and inter-group comparison were done through the Kruskal–Wallis ANOVA test. All statistical tests were performed through SPSS version 25.0 (IBM). Results: The inter-group statistical analysis concerning NRS, HI, and ET showed statistically significant results in Groups A and B compared to Group C ( P < 0.05), while gingival biotype showed statistically significant results in Group A compared to Groups B and C. Conclusion: The use of PRF membrane and HA gel to protect the raw wound site of depigmented gingiva proved to be an effective approach, resulting in faster healing.

Abstract:ObjectivesThe aim of this study was to compare the effects of a hemostatic collagen sponge and a collagen sponge sealed with a bio-adhesive material on the palatal donor sites with the purpose of minimizing postoperative pain after epithelialized gingival graft (EGG) harvesting.Material and methodsThe present study consisted of 44 EGGs harvested in 44 patients. In the control group, a hemostatic collagen sponge was applied over the palatal wound, while the test group was treated with additional cyanoacrylate. Patients were observed for 14 days, evaluating the pain level by using the visual analogic scale. The consumption of analgesic during the postoperative period, the willingness for retreatment and the characteristic of the graft were also analyzed.ResultsStatistically significant differences in pain perception were found between test and control groups in each of the studied days (p < 0.01). Analgesic consumption was lower in the test group (p < 0.01). Graft width < 14 mm was found to be associated with lower discomfort (p < 0.01).ConclusionsAdding an additional layer of cyanoacrylate over a hemostatic collagen sponge on the palatal wound following EGG harvesting was found to be successful in minimizing the postoperative discomfort and the need for analgesics.Clinical relevancePostoperative pain after palatal tissue harvesting can be successfully minimized if the donor site open wound is protected with an external layer of cyanoacrylate over a collagen sponge.

Abstract:ObjectivesThe aim of this study is to determine the use of platelet-rich fibrin (PRF) in the management of soft tissue donor site healing after harvesting connective tissue graft (CTG) from the palate and evaluate the postoperative discomfort (pain, bleeding, analgesic consumption, tissue color match) of patients.Materials and methodsForty patients were randomly assigned to PRF or control group. In the PRF group, PRF membrane was placed into CTG donor site. After surgery, delayed bleeding, early healing index (EHI), tissue color match, and analgesic consumption were recorded. The visual analog scale (VAS) was used to observe the postoperative pain and tissue color match. Data were analyzed using the independent sample t test and the repeated measure ANOVA test.ResultsThe patients in the PRF group reported significantly lower pain scores at all-time points. Postoperative 3rd and 7th day, EHI scores were lower in the favor of the PRF group. VAS score values of tissue color match were lower in the control group at 7th and 14th day, compared with the PRF group. Analgesic intake was significantly lower in the PRF group postoperatively at 1st and 3rd day that of control group.ConclusionPRF application at the palatal donor site demonstrates promising results in terms of better wound healing and reduced postoperative discomfort in the patients after harvesting CTG.Clinical relevancePatients may avoid surgical operations because of the discomfort or pain feeling. Reducing postoperative pain and discomfort and accelerating recovery meet the wishes of every patient and physician. PRF can provide these requirements as an easy method to obtain and implement.

Abstract:Background/aim: Caffeic acid phenethyl ester (CAPE) and Ankaferd Blood Stopper (ABS) are considered to contribute to wound healing. The purpose of this study was to investigate the effect of ABS and CAPE on secondary wound healing of oral mucosal tissue. Materials and methods: In total, 63 male Sprague-Dawley rats were used in this study. The animals were randomly divided into three groups and anaesthetized with ketamine (8 mg/100 g, intraperitoneally): a control group, CAPE group, and ABS group. A full-thickness excisional wound was created using a 4 mm punch biopsy tool. Topical ABS and CAPE were then applied in each group for 7, 14, and 21 days (n = 7 in each group). The animals in each group were sacrificed after 7, 14, and 21 days. Palatal specimens were stained with haematoxylin-eosin. Vascular endothelial growth factor (VEGF) and tumour necrosis factor-inducible gene 6 (TSG-6) protein expressions were determined using the Western blot method. Results: Inflammation, vessel dilatation, and haemorrhages were significantly lower in the CAPE group as compared with these parameters in the other groups (P < 0.05). Fibrosis was significantly higher in the ABS group as compared with that in the other groups (P < 0.05). VEGF protein levels were elevated in the 21-day CAPE group and 7-day ABS group. The expression of TSG-6 increased in the 7-day CAPE group and 21-day ABS group. Conclusion: Based on our findings, ABS and CAPE had positive effects on the oral wound healing process.
Evaluation of the safety and efficacy of periodontal applications of a living tissue-engineered human fibroblast-derived dermal substitute. I. Comparison to the gingival autograft: a randomized controlled pilot study

Abstract:Background: Periodontists have found the gingival autograft to be an effective and predictable technique to increase the amount of attached gingiva around teeth, but this technique requires the surgeon to harvest donor tissue from a remote surgical site. The present study seeks to evaluate the safety and effectiveness of a tissue-engineered skin equivalent, a living human fibroblast-derived dermal substitute (HF-DDS), compared to a gingival autograft (GA) consisting of donor tissue harvested from the patient's palate in a procedure designed to increase the amount of keratinized tissue around teeth that do not require root coverage. Methods: Twenty-five patients with insufficient attached gingiva associated with at least two teeth in contralateral quadrants of the same jaw were treated. One tooth in each patient was randomized to receive either a GA (control) or a HF-DDS graft (test). Clinical parameters measured at baseline and 3, 5, 7, 9, and 12 months included recession, clinical attachment level, keratinized tissue height, and plaque index. Probing depth was measured at 7, 9, and 12 months. Inflammation of each site was scored and texture and color of the grafted tissue were compared to the surrounding tissue. Resistance to muscle pull was evaluated and a questionnaire was used to determine patient preference. Surgical position of the graft and alveolar bone level were recorded at the surgical visit and patients were evaluated weekly for the first 4 weeks at which time recession and level of oral hygiene were measured. Biopsies and persistence studies were performed on a subset of the patients. Results: Results for both test and control groups were similar for all measured clinical parameters with the exception of amount of keratinized tissue and percent shrinkage of keratinized tissue. The control group exhibited an average of 1.0 to 1.2 mm more keratinized tissue over time than the test group (P <0.001) and the control group had about half as much shrinkage as the test group over time (P <0.001). Test sites demonstrated significantly better color match over time compared to control sites. Similarly, tissue texture for test sites was significantly better than control sites over time. Conclusions: Based on the results of this investigation, the tissue engineered HF-DDS graft was safe and capable of generating keratinized tissue without the morbidity and potential clinical difficulties associated with donor site surgery. The GA generated more keratinized tissue and shrank less than the HF-DDS graft, but the test graft generated tissue that appeared more natural.

Abstract:In this paper we provide a simple proof of the extension theorem for partial orderings due to Suzumura [1983] when the domain of the partial order is finite. The extension theorem due to Szpilrajn [1930] follows from this theorem. Szpilrajns extension theorem is used to show that an asymmetric binary relation is contained in the asymmetric part of a linear order if and only if it is acyclic. This theorem is then applied to prove three results. Finally we introduce the concept of a threshold choice function, and our third result says that such choice functions are the only ones to satisfy a property called functional acyclicity.

Abstract:Few studies have focused on the toxicological risks of dairy fat intake. A standard dairy fat (SDF) with a 70% SFA content and a naturally enriched dairy fat (EDF) in vaccenic, rumenic and α-linolenic acids and low in SFA (54%) have been examined in a 4-week repeated dose oral toxicity study as a daily dose of 2000 mg/kg bw by gavage in rats. Comparisons were established with a third group of rats (control) which did not receive fat administration. Both fats were well tolerated, and no adverse events or mortality were observed during the treatment nor after a 2-week observation period. EDF and SDF did not cause significant differences with respect to a control group in body weight gain, food consumption, clinical observations, organ weight ratios, histopathological findings and most of the hematological and biochemical parameters including total cholesterol and cholesterol fractions in plasma. In rats treated with SDF, a significant increase of triglycerides was observed as compared to the control group. By contrast, in rats treated with EDF, a significant decrease in triglycerides was detected. EDF orally administered to rats was safe, and no treatment-related toxicity was detected. The results also suggest that EDF could protect against the increase of triglyceride concentrations in plasma.
Nano-emodin mediated photodynamic therapy for wound healing of donor site after free gingival graft: A parallel clinical trial

Abstract:Aim: This study aimed to evaluate the effectiveness of nano-emodin (n-Emo) mediated photodynamic therapy (PDT) as an adjunctive therapy to conventional wound care in patients undergoing free gingival graft (FGG) treatment. The study employed a single-center, parallel, two-blind, randomized, controlled trial design. Method and material: A total of 53 patients requiring FGG treatment were randomly assigned to one of three groups: the n-Emo mediated PDT group, the n-Emo gel group, and the control group. The n-Emo mediated PDT group received n-Emo gel on donor site followed by LED (450 nm, 1000 ± 1400 mW/cm2, 60-80 J/cm2, 60 s), the n-Emo gel group received only n-Emo gel on donor site, and the control group received a placebo gel on donor site. All groups received treatment immediately and 48 h after FGG surgery. Wound healing was assessed based on wound area, bleeding, color match, and epithelialization. Postoperative pain perception was evaluated using a Visual Analogue Scale (VAS), and the number of non-steroidal anti-inflammatory drug (NSAID) intakes was recorded. Results: The results showed that all groups demonstrated significant improvement in wound healing by day 28, with the n-Emo mediated PDT exhibiting significantly better healing on the 14th, 21st, and 28th day compared to the control group. Additionally, on the 28th day, the n-Emo mediated PDT showed significantly better healing than the n-Emo gel group. The n-Emo mediated PDT also had significantly lower pain scores than the control group. There were no significant differences in the number of NSAID intakes, bleeding, or color match among the groups, but all groups showed improved color match during follow-up. By the third week, all groups had fully epithelialized without significant differences, and no secondary bleeding incidents were reported. Conclusion: The study concludes that n-Emo mediated PDT is an effective adjunctive therapy to conventional wound care for managing complication after surgery at donor site after FGG surgery.
Minimizing Patient Morbidity Following Palatal Gingival Harvesting: A Randomized Controlled Clinical Study

Abstract:This clinical study was conducted to evaluate the impact of different hemostatic treatments following palatal gingival harvesting on patient discomfort. Fifty patients who needed a mucogingival surgery requiring gingival graft harvesting were enrolled and randomly assigned to one of five groups: (1) a control group in which only sutures were applied; (2) a cyanoacrylate group; (3) a periodontal dressing material group; (4) a hemostatic gelatin sponge group; and (5) a group in which the gelatin sponge and cyanoacrylate were combined. In the 2 weeks following the procedures, perception of pain, healing, consumption of drugs, and willingness to repeat the procedure were recorded through visual analog scale (VAS) by patients. Over the 2 weeks, lower pain (VAS) was found in all test groups compared to the control group (P < .01, value for time-group interaction). Notably, the gelatin sponge combined with cyanoacrylate group had very low pain (VAS ≤ 0.5 points) throughout the 14 days. The lowest healing scores at day 10 were associated with the control group (6.8 VAS points) in contrast to the four test groups (8.2 to 9.0 VAS points, P = .0001). Pain was inversely correlated with age (P < .05). Pain also depended on the apicocoronal dimension of the graft: the higher the graft, the more pain was experienced by the participants (0.4 VAS points per 1 mm, P < .05). Within the limitations of this study, palatal coverage appears to result in better outcomes when compared to suture alone. In particular, a double-layered protection of the palatal wound with a gelatin sponge combined with cyanoacrylate appeared to be the best option in reducing pain and postoperative discomfort.

Abstract:OBJECTIVES This study compared the effects of L-PRF and A-PRF on patients' quality of life and the healing of palatal wounds after free gingival graft harvesting. METHOD AND MATERIALS After FGG harvesting, palatal donor sites of 36 patients were assigned one of the three group: 1)L-PRF group, 2) A-PRF group and 3) Palatal stent+periodontal pack group (control group).Wound healing was evaluated by H2O2 test. Pain was evaluated by visual analog scale (VAS)and number of analgesics consumed. For patient quality of life, the Oral Health Impact Profile-14 (OHIP-14) questionnaire was utilized. RESULTS Complete epithelialization was higher in thecontrol group on the 7th day than test groups (p< 0.05). Complete epithelization was achievedin all groups 14th day postoperatively. VAS scores for pain and the number of analgesic tabletsconsumed were similar in all groups (p >0.05). OHIP-14 total scores were similar in all studygroups at 7th day postoperatively (p>0.05). OHIP-14 total and physical pain scores was lowerin L-PRF group than A-PRF group at 14th day postoperatively (p< 0.05). CONCLUSION Asidefrom the slight superiority of L-PRF over A-PRF, it can be concluded that both PRF procedureshave similar effects on palatal wound healing and quality of life.

Clinical and histological evaluation of concentrated growth factor membrane bandage after gingival depigmentation: A case series

Abstract:Concentrated growth factor (CGF) is a newer form of autologous platelet concentrate with an extra fibrin matrix and more growth factors than that observed with platelet-rich fibrin, due to its different centrifugation process. This case series evaluates the early wound healing with CGF membrane as a periodontal bandage after depigmentation. Three individuals were selected for a split-mouth study. After surgical depigmentation, the test site received CGF membrane. Both groups were covered by a periodontal pack. Participants were assessed on the 3rd and 5th days using a Visual Analog Scale and a Healing Index. Epithelization test was done with toluidine blue on the 5th day and tissue samples were taken for histological evaluation. The application of CGF membrane showed better healing and lesser inflammatory cells. Thus, a CGF membrane is helpful in protecting the raw wound region of depigmented sites, improving patient comfort, and for better healing.

Abstract:Background: This study evaluates contributions of platelet-rich fibrin (PRF) combined with conventional flap surgery on growth factor levels in gingival crevicular fluid (GCF) and periodontal healing. Methods: Twenty-six patients (52 sites) with chronic periodontitis were treated either with autologous PRF with open flap debridement (OFD+PRF) or OFD alone. Growth factor levels in GCF at baseline and 2, 4, and 6 weeks after surgery were analyzed, and clinical parameters such as probing depth (PD), relative clinical attachment level (rCAL), and gingival margin level (GML) at baseline and 9 months after surgery were measured. Results: Mean PD reduction and rCAL gain were significantly greater in OFD+PRF sites than in OFD sites. Mean GML change was -0.38 + 0.10 mm in OFD sites and 0.11 + 0.08 mm in the test group; difference between the two groups was statistically significant (P <0.05). Both groups demonstrated increased expression levels of fibroblast growth factor-2, transforming growth factor-β1, and platelet-derived growth factor-BB at 2 weeks compared with baseline, followed by reductions at 4 and 6 weeks. The OFD+PRF group showed significantly higher growth factor levels compared with the OFD group at 2 and 4 weeks. Conclusion: PRF membrane combined with OFD provides significantly higher GCF concentrations of angiogenic biomarkers for ≈2 to 4 weeks and better periodontal healing in terms of conventional flap sites.

Abstract:Purpose: To evaluate the gain in peri-implant keratinized mucosa (KM) using a combination of a free gingival graft (FGG) strip with a xenogeneic collagen matrix (XCM) and compare it to that of an FGG alone. Materials and Methods: This randomized controlled clinical study included a total of 30 patients with a minimal amount of peri-implant KM (≤ 1 mm) in the maxillary and mandibular anterior regions (including the premolars). The study population was divided into two groups using a randomization protocol; group A (n = 15) received a combination of FGG strip and XCM, and group B (n = 15) received FGG only to augment the KM. Clinical parameters included width of KM (WKM) measured at baseline, 1 month, and 6 months; a customized pink esthetic score (PES) measured at baseline and 6 months; and patient morbidity using visual analog scale (VAS) at days 1, 7, and 14 postoperatively. Results: After 1 month, both groups showed statistically significant gain in KM that reduced slightly by the 6-month follow-up but remained statistically significant when compared to baseline. When the gain was compared between the two groups, the difference was not statistically significant. Better esthetic outcomes and lower morbidity were reported in group A compared to group B, and this difference was statistically significant. Conclusions: Within the limitations of the study, it can be concluded that the combination of FGG strip with XCM is a viable alternative to the use of FGG in augmenting peri-implant KM.

Abstract:Background The aim of this systematic review and network meta‐analysis (NMA) was to assess the efficacy of different wound‐healing agents for palatal donor area management after soft tissue graft harvesting. Methods Electronic searches in six databases were conducted for publications up to October 2023. Studies with data from patients undergoing therapeutic approaches using agents for palatal healing after gingival graft harvesting were included. Data about postoperative pain, wound‐healing and postoperative complications reported for each agent were extracted. Three different tools were used for the risk of bias within studies evaluation (Murad tool for case series and case report, RoB 2.0 tool for randomized studies and ROBINS‐I tool for non‐randomized studies). A Bayesian random effects NMA was conducted for postoperative pain levels and wound healing. Results Eighty‐four publications were included in the systematic review (qualitative analysis), with 14 of these subjected to NMA (quantitative analysis). The summarized results from the qualitative and quantitative analysis showed that all wound‐healing agents evaluated promoted better pain control and wound healing compared to spontaneous healing and hemostatic sponges alone. The NMA outcomes reveal that leucocyte‐ and platelet‐rich fibrin (L‐PRF) was the most effective agent in reducing postoperative pain in all analyzed periods. Moreover, the L‐PRF seems to accelerate wound healing and reduce postoperative complications compared to other agents. Conclusion In conclusion, the L‐PRF was the most effective agent in reducing postoperative pain, accelerating wound healing and reducing postoperative complications after harvesting soft tissue grafts from the palatal area.
