Abstract:Objective:To evaluate prospectively the accuracy of ultrasonography in diagnosis of carpal tunnel syndrome(CTS).Methods:Forty-four wrists in 37 CTS patients and 80 wrists in 40 asymptomatic volunteers were imaged using a real-time ultrasound scanner with a 7～14 MHz linear-array transducer.The cross-sectional area and flattening ratio at the level of the distal radius.The pisiform bone and the hook of hamate bone of the proximal carpal tunnel were measured.Data from the patient group and control group were compared to determine the statistical significance.The accuracy of the ultrasonographic diagnostic criteria for carpal tunnel syndrome was evaluated using receiver-operating characteristic(ROC) analysis.Results:All measurements showed significant differences between patients and controls.Increased cross-sectional area of the median nerve was the most predictive value of carpal tunnel syndrome.Using the ROC curve,a cut-off value of 9.5mm2 at the level of pisiform bone provided a diagnostic sensitivity of 88% and specificity of 87.5%.Conclusion:Sonographic measurement of the median nerve cross-sectional area is useful and dependent for the diagnosis of carpal tunnel syndrome.
Carpal Tunnel Syndrome: Rapid Evidence Review

Abstract:Carpal tunnel syndrome (CTS) is caused by compression of the median nerve as it travels through the carpal tunnel. Patients commonly experience pain, paresthesia, and, less often, weakness in the distribution of the median nerve. Provocative maneuvers, such as the Phalen test and Tinel sign, have varying sensitivity and specificity for the diagnosis of CTS. Thenar atrophy is a late finding and highly specific for CTS. Although patients with a classic presentation of CTS do not need additional testing for diagnosis, electrodiagnostic studies can confirm the diagnosis in atypical cases, exclude other causes, and gauge severity for surgical prognosis. An abnormal nerve conduction study is useful for ruling in CTS, but a normal test does not necessarily exclude it. Over-the-counter analgesics, such as nonsteroidal anti-inflammatory drugs and acetaminophen, have not shown benefit for CTS. Patients with mild to moderate CTS initially may be offered nonsurgical treatments, such as splinting or local corticosteroid injections. Night-only splinting is as effective as continuous wear. A neutral wrist splint may be more effective than an extension splint. In patients with recent onset of CTS, corticosteroid injections provide slightly greater improvement of symptoms compared with splinting at 6 weeks, with similar outcomes at 6 months. Patients with severe CTS, including objective weakness or sensory deficits, should be offered surgical decompression. Endoscopic and open carpal tunnel release techniques are equally effective.
The spectrum of cutaneous lesions in pediatric patients with leukemia.

Abstract:This is a clinical study that attempts to determine the incidence and patterns of cutaneous lesions in 109 pediatric patients with leukemia. Non-specific lesions, namely, adverse reactions to chemotherapy, complications of immunosuppression, and hemorrhagic diathesis were seen in 88.7% of those patients with acute lymphocytic leukemia and in 88.9% of patients with acute non-lymphocytic leukemia. Leukemia cutis was seen in three patients with acute non-lymphocytic leukemia, and in one patient with congenital leukemia. It seems that although skin complications are a frequent event in the course of childhood leukemia, leukemic infiltration of the skin a is rare event.
Electrophysiological Analysis in Carpal Tunnel Syndrome in 22 Cases

Abstract:Objective:To find out the neural electrophysiological features in patients with carpal tunnel syndrome(CTS).Methods: We performed the measurement of motor nerve conduction velocity and sensory nerve conduction velocity on median nerve and ulnar nerve as well as electromyogram of abductor pollcis brevis and abductor digiti minimi in 22 cases of CTS patients.Results: Of the 22 patients,bilateral lesions were found in 7 and unilateral in 15,and totally there were 29 lesions.The latency of 29 median nerves sensory conduction became delayed and the sensory conduction velocity were decreased.Reduced evoked amplitude was seen in 21 median nerves sensory nerves and delayed distal motor latency(DML) in 28 median nerves.In addition,28 median nerves compound muscle action potential(CMAP) were found decreased.The DML and the evoked amplitude remained normal in only 1 median nerve.The neurogenic injury was seen in 19 abductor pollicis brevis controlled by the median nerve.Conclusion: Neural electrophysiological examination has great value in the diagnosis and differential diagnosis of CTS.
Clinical and electrodiagnostic testing of carpal tunnel syndrome: a narrative review

Abstract:Carpal Tunnel Syndrome (CTS) is a pressure-induced neuropathy that causes sensorimotor disturbances of the median nerve, which impair functional ability. A clear history that elicits relevant personal and work exposures and the nature of symptoms can lead to a high probability of a correct diagnosis. Hand diagrams and diagnostic questionnaires are available to provide structure to this process. A variety of provocative tests have been described and have variable accuracy. The Phalen's wrist flexion and the carpal compression tests have the highest overall accuracy, while Tinel's nerve percussion test is more specific to axonal damage that may occur as a result of moderate to severe CTS. Sensory evaluation of light touch, vibration, or current perception thresholds can detect early sensory changes, whereas 2-point discrimination changes and thenar atrophy indicate loss of nerve fibers occurring with more severe disease. Electrodiagnosis can encompass a variety of tests and is commonly used to assess the presence/severity of neuropathic changes and to preclude alternative diagnoses that overlap with CTS in presentation. The pathophysiologic changes occurring with different stages of nerve compression must be considered when interpreting diagnostic test results and predicting response to physical therapy management.

Abstract:Carpal tunnel syndrome (CTS), the most common mononeuropathy, results from compression of the median nerve within the fibro-osseous carpal tunnel. Diagnosis is typically based on clinical evaluation and confirmed by nerve conduction studies. However, ultrasound (US) has emerged as a valuable noninvasive adjunct for CTS confirmation, offering potential advantages over electrodiagnostic testing in terms of patient comfort and diagnostic accuracy. This review begins with a concise summary of carpal tunnel anatomy and CTS pathophysiology as a foundation for exploring the diverse applications of US in CTS evaluation. B-mode US assessment is presented with a focus on cross-sectional imaging and dynamic evaluations, including the transverse translocation and longitudinal gliding of the median nerve. We also review current methods for assessing vascularization in CTS and explore the usefulness of elastography in CTS evaluation. The advantages and limitations of each US method are elucidated, highlighting their practical utility in clinical practice. * Shared first authorship . Abstract:ABSTRACT Background: Carpal tunnel syndrome (CTS) is the most common entrapment neuropathy, but not adequately studied in India. Objectives: To study clinical tests, nerve conduction studies (NCS), ultrasonography (USG), and magnetic resonance imaging (MRI) in diagnosing CTS. Materials and Methods: We diagnosed CTS in 54 patients (93 hands) out of 60 screened patients with symptoms compatible with CTS, including 19 control patients (23 hands). We conducted provocative tests and calculated Boston Carpal tunnel Questionnaire (BCTQ) symptom (S) and function (F) scores. NCS positive patients were classified into mild, mild-to-moderate, moderate, severe, and all-CTS groups. Median nerve anteroposterior, transverse, circumference (CIR), and cross-sectional area (CSA) at inlet (I), middle (M), and outlet (O) each was measured by USG in all patients. MRI was done in 26 patients (39 hands). Results: Phalen, hand elevation and pressure provocation tests had higher sensitivity, Tinel's test had higher specificity and tethered median nerve and tourniquet tests had low sensitivity and moderate specificity. USG had low sensitivity but high specificity, and MRI had moderate sensitivity. USG in patients compared to controls was significantly abnormal in CSA-I, CIR-I, and CSA-O. Significant correlation was found between BCTQ-S and NCS and BCTQ-S and CIR-O. CIR-M, CIR-O, CSA-M, and CSA-I had correlation with NCS. MRI was significant in moderate and in moderate + severe groups combined and associated pathologies were detected in 59% patients. Conclusion: NCS remain gold standard but USG and MRI help increase sensitivity and detect mass lesions amenable to surgery.

Abstract:This study evaluates the usefulness of ultrasonography in diagnosing carpal tunnel syndrome (CTS). The cross-sectional area of the median nerve was measured at the forearm and its entry into the carpal tunnel and compared with clinical symptoms and electroneuromyography. A total of 124 patients were examined, 77 patients with clinically confirmed CTS and 47 controls. A significant correlation was found between the ultrasonography cross-sectional area values at entry points into the carpal tunnel and electroneuromyography. The highest sensitivity (87%) and specificity (91%) for different cut-off values (8.5–12.5 mm 2 ) of the cross-sectional area was for 11.5 mm 2 . The use of ultrasonography may provide a quick and reliable differential diagnostic tool for the primary diagnosis of CTS in patients with classical symptoms. Level of evidence: II

Abstract:Abstract Carpal tunnel syndrome (CTS) is the most common entrapment neuropathy caused by impairment of the median nerve due to compression as it passes through the carpal tunnel. The current gold standard in diagnosing CTS and nerve damage is by electrophysiological nerve conduction study (NCS). However, 10 to 25% of NCS results are falsely negative. Moreover, NCS remains an expensive and time-consuming procedure for patients. Ultrasonography serves as a real-time, well-tolerated, portable, and noninvasive tool for assessing the carpal tunnel. This study aims to assess the role of high-frequency ultrasound of the median nerve at the wrist in evaluating CTS and correlate with NCS to determine whether sonography can be used as an alternative to NCS in diagnosing and grading CTS.
A Case of Atraumatic Acute Carpal Tunnel Syndrome Requiring Emergency Surgery

Abstract:Acute carpal tunnel syndrome (ACTS) is an urgent condition in which symptoms progress rapidly on an hourly basis, and emergency surgery may be required to treat it. ACTS often occurs after a traumatic event such as a fracture of the distal radius, and rarely occurs non-traumatically. We present a case of a 60-year-old male with ACTS secondary to acute synovitis due to rheumatoid arthritis. The patient complained of strong numbness from the thumb to the ring finger and pain in the palm, and he was unable to actively flex or extend his fingers. In addition, severe tenderness was observed in the palm; on the contralateral side, no obvious tenderness of the forearm and wrist joint was observed. Due to the intolerable pain and numbness, ACTS was suspected. Internal pressure from the forearm to the palm was measured, and it was found that the internal pressure of the carpal tunnel was elevated at 150 mmHg. Based on these findings, non-traumatic ACTS was diagnosed, and emergency surgery was performed. The transverse carpal ligament was exposed, an incision was made from the distal end, and the proximal part was fully incised to the forearm fascia so that the carpal tunnel was completely released. The synovial membranes around the median nerve were peeled off, confirming that the nerve had been loosened sufficiently. After the operation, finger pain and numbness improved dramatically from the day after surgery. Proper diagnosis and prompt treatment with surgical median nerve decompression are crucial for good functional recovery in these patients.

Abstract:Abstract Background Carpal tunnel syndrome (CTS) is the commonest entrapment neuropathy. The aim of this study was to assess the accuracy and validity of high resolution musculoskeletal ultrasound (US) in the diagnosis of CTS in the Saudi population. Methods Sixty patients were diagnosed clinically to have CTS involving 89 wrists that were confirmed by neurophysiologic studies. Each affected wrist was characterized as idiopathic or associated with either diabetes mellitus or hypothyroidism and were assigned a severity grade based on results of neurophysiologic studies. Seventy-six healthy wrists from fifty age, sex and BMI matched healthy subjects were included in the control group. High resolution ultrasound (US) was performed to assess median nerve cross sectional area distal (CSAd) at the entry to the carpal tunnel and proximally (CSAp) at the level of pronator quadratus muscle with a further calculation of their difference (ΔCSA) and their mean average or CSAd+CSAp/2 (CSApd). Results There was a significant difference between both groups regarding mean ± SD of CSAd, CSAp, ∆CSA, and CSApd ( p = 0.0001). A positive significant correlation was also found between the CSAd, ∆ CSA and the CSApd measurements with neurophysiologic severity grade of CTS ( P = 0.001). A ∆CSA threshold of 2.5 mm 2 showed the highest sensitivity and specificity to diagnose CTS in Saudis. Conclusion High resolution ultrasound is a valid and accurate diagnostic modality in carpal tunnel syndrome and correlated to CTS severity. A ∆CSA greater than 2.5 mm 2 is considered a valid diagnostic value for CTS in our Saudi population. CTS in our patients with diabetes tend to have greater median nerve US measurement values.

Abstract:Background Neuromuscular ultrasound plays an increasing role in diagnosing carpal tunnel syndrome (CTS). There are limited data supporting the correlation between the electrodiagnostic studies and ultrasound measurements in CTS. Objective To assess the association between different electrodiagnostic severities and ultrasound measurements of the median nerve in CTS. Design A retrospective cohort study. Abstract:Abstract Objectives To comparatively assess the sonographic spectrum of carpal tunnel syndrome (CTS) in patients with rheumatoid arthritis (RA) and in patients with idiopathic CTS. Methods Fifty-seven RA patients and 25 idiopathic CTS patients were consecutively enrolled. The diagnosis of CTS in RA patients was made according to clinical history and examination. The following sonographic findings were assessed at carpal tunnel level: median nerve cross-sectional area (CSA) at the carpal tunnel proximal inlet, finger flexor tendons tenosynovitis, radio-carpal synovitis and intraneural power Doppler (PD) signal. Results CTS was diagnosed in 15/57 RA patients (26.3%). Twenty-three RA wrists with CTS, 84 RA wrists without CTS and 34 idiopathic CTS wrists were evaluated. The average CSA of the median nerve was higher in idiopathic CTS than in RA wrists with CTS (17.7 mm 2 vs 10.6 mm 2 , p < 0.01). A higher rate of inflammation of synovial structures (flexor tendons sheath and/or radio-carpal joint) was found in RA wrists with CTS compared with those without CTS ( p = 0.04) and idiopathic CTS ( p = 0.02). Intraneural PD signal was more common in CTS (in both RA and idiopathic CTS) wrists compared with wrists without CTS ( p < 0.01). Conclusion The sonographic spectrum of CTS in RA patients is characterized by an inflammatory pattern, defined by the presence of finger flexor tendons tenosynovitis and/or radio-carpal joint synovitis. Conversely, a marked median nerve swelling is the dominant feature in idiopathic CTS. Intraneural PD signal is a frequent finding in both conditions. Key Points • Carpal tunnel syndrome (CTS) associated with rheumatoid arthritis (RA) and idiopathic CTS have distinct ultrasound patterns. • The most characteristic sonographic features of CTS in RA patients are those indicative of synovial tissue inflammation at carpal tunnel level. Conversely, marked median nerve swelling is the dominant finding in idiopathic CTS. • Intraneural power Doppler signal is a frequent finding in both conditions. • In patients with CTS, differently from electrophysiology, US can provide clues prompting a rheumatology referral in case of prominent inflammatory findings at carpal tunnel level.

Abstract:Objective To evaluate the role of ultrasonography in the diagnosis and treatment of carpal tunnel syndrome (CTS). Methods Sixty wrists from fifty patients with CTS confirmed by operations and fifty asymptomatic controls underwent ultrasonography by the same operator. The configurations and echo of the median nerve were observed in sagittal plane. The maximum and minimum anterior-posterior diameter of the median nerve were measured and the thickness ratio was calculated. The cross-sectional area at the level of pisiform bone was measured in transverse plane. Results The most compressed site located at the hook of the hamate bone. The proximal and the distal part of the compressed nerve swelled up. The echo of the swollen nerve was hypoechoic. Comparing with the control group, the quantitative criteria including the thickness ratio and the cross-sectional area at the level of pisiform bone was significant. The cut-off value of thickness ratio and the cross-sectional area at the level of pisiform bone was ≤0.81 and 0.11 cm~2. The sensitivity was 90.2% and 80.2% , and the specificity was 88.2% and 88.4%, respectively. Conclusion Ultrasonography could detect the pathological nerve from its configuration, determine the existence of CTS on quantitive criteria, and be helpful to choose operation methods by discovering some local cause of CTS and anatomic variations.

Abstract:Carpal tunnel syndrome (CTS) is the most common form of entrapment neuropathy. The ultrasonography is comparable to electrodiagnostic study for diagnosis and evaluation of CTS. The aim of this study was to evaluate the ultrasonography parameters for surgical outcome in patients with CTS. A prospective study evaluated 60 patients with CTS who received surgical procedure for relieving transverse carpal ligament. The ultrasonography of wrist was performed before and one month after operation. Surgical outcome was measured by Historical Objective Scale and Boston Questionnaire. The cross-sectional area (CSA) of median nerve at the wrist was positively correlated with the distal latency of median compound motor action potential (CMAP) and median sensory nerve action potential (SNAP). The CSA of median nerve at the wrist was significantly larger in patients with poor surgical outcome compared with those with good outcome (p < 0.05). Furthermore, the CSA of median nerve at the wrist was significantly reduced one month after operation when compared to baseline level in CTS patients (P < 0.05). The larger CAS of median nerve at the wrist indicated poor surgical outcome in patient with CTS. The ultrasonography provides a useful value in assessment of CTS patients who received surgical procedure for median nerve release.

Abstract:Objectives: Evaluating peripheral neuropathy mainly relies on physical examination, patient history, and electrophysiological studies. High-resolution ultrasound is a fast, noninvasive modality for dynamic nerve assessment that enables the length of the nerve to be examined. Magnetic resonance imaging is preferred for examining deeper nerves with a high contrast resolution; its use shows excellent benefit in patients with atypical presentation, equivocal diagnosis, suspected secondary causes, and postsurgical relapse. We aimed to assess the measurements and criteria for both ultrasound and magnetic resonance neurography for the diagnosis of carpal tunnel syndrome, based mainly on the three measurements assessed by Buchberger et al. Methods: This prospective study was conducted to test diagnostic accuracy. Thirty-two patients who presented clinically with, and were diagnosed by electrophysiological tests as having, carpal tunnel syndrome participated. Superficial ultrasound of the wrist joint was performed on all participants, followed by magnetic resonance imaging within 1 week of ultrasonography. Results: The three main parameters of cross-sectional area measurement, distal nerve flattening, and flexor retinaculum bowing indices showed positive occurrences of 93.7%, 59.4%, and 59.4%, respectively; 90.6% of patients had decreased nerve echotexture. The diagnostic ability of magnetic resonance imaging was decreased when cross-sectional area measurements were used: positive results were achieved in 81.2% of patients, but the positive results showing the distal tunnel nerve increased flattening and bowed flexor retinaculum slightly decreased to 56.2% for each. A high T2 signal of the median nerve was observed in 90.6% of patients. In an agreement analysis, we found a statistically significant difference that supported the use of ultrasound as a primary diagnostic modality for carpal tunnel syndrome. However, magnetic resonance imaging improved tissue characterization and was a good diagnostic modality, with a statistically significant difference, for cases of secondary carpal tunnel syndrome, detection of the underlying entrapping cause, and early abnormality detection in the innervated muscle. Conclusions: Our results demonstrate that ultrasound examination can be used as the first imaging modality after physician evaluation, with results comparable to those of electrophysiological studies for evaluating carpal tunnel syndrome and determining its cause. Magnetic resonance neurography examination is the second step in detecting secondary causes in cases with suspected early muscle denervation changes that cannot be elicited by ultrasound or in cases with equivocal results.
The Application of Ultrasonography in the Diagnosis of Carpal Tunnel Syndrome

Abstract:Objective Our aim is to evaluate the diagnostic value of ultrasonography in patients with carpal tunnel syndrome (CTS). Methods 50 patients who had clinically and electrophysiologically confirmed CTS underwent ultrasonographic examination of the wrist. 30 asymptomatic normal wrists served as control. Data from the patient and the control groups were compared to determine the diagnostic parameters. Results The thickness of palmar retinaculum in CTS patients was 0.39 cm on average while that of normal control 0.33 cm. The flattening ratio of the median nerve at hamate hook level was 4.56 versus 3.12. Cross-sectional area of the median nerve at radiocarpal joint level, pisiform level, hamate hook level, and distal hamate level was larger in CTS patients than in normal individuals. There was high correlation between the electrophysiological results and ultrasonographic findings. Conclusion Ultrasonography examination is a valuable and practical tool in defining the cause and severity of CTS. It is simple, reliable and atraumatic.
Evaluation of electrophysiological examinations for the diagnosis of carpal tunnel syndrome

Abstract:Abstract To investigate the clinical significance of results in electrodiagnostic evaluations for carpal tunnel syndrome(CTS) and analyse the correlation between electrodiagnostic evaluations and physical examinations, 236 CTS patients and 96 healthy volunteers were included to accept electrodiagnostic evaluations. There were significant differences between CTS and control groups, mild and moderate patients, moderate and severe patients in CTS groups respectively, in terms of compound muscle action potential(CMAP), sensory conduction velocity of middle finger(SCV), distal motor latency(DML) difference between median nerve and ulnar nerve on the ring finger(△DML) (p<0.05).△DML has larger area under curve of ROC curve (0.974) than CMAP, SCV and DML(0.748, 0.950, 0.958), then △DML could be included as valuable test for diagnosing and classification of carpal tunnel syndromes. There was low correlation between electrodiagnostic evaluations and physical examinations (rho <0.4),so electrodiagnostic evaluation and physical examination should be combined to diagnose CTS.

Abstract:Background Numerous sonographic modalities and parameters have been used to diagnose carpal tunnel syndrome (CTS), with varying accuracy. Our umbrella review aimed to summarize the evidence from systematic reviews and meta-analyses regarding the use of ultrasound imaging to diagnose CTS. Methods Systematic reviews and meta-analyses meeting the inclusion criteria were searched in PubMed, Embase, Medline, Web of Science, and Cochrane databases from inception to March 2022. Critical appraisal, data extraction, and synthesis were performed in accordance with the criteria for conducting an umbrella review. Results Sixteen reviews were included. Three reviews were classified as high quality, one as moderate, four as low, and eight as critically low. The cross-sectional area (CSA) of the median nerve at the carpal tunnel inlet demonstrated the best reliability and diagnostic accuracy among multiple parameters. A cutoff CSA value of 9–10.5 mm 2 gave the highest diagnostic performance in the general population. The degree of CSA enlargement was correlated with CTS severity. Sonoelastography and Doppler ultrasound might provide additional insights into CTS evaluation as median nerve stiffness and vascularity at the wrist were increased in these patients. Conclusions Sonography is a reliable tool to diagnose CTS, with inlet CSA being the most robust parameter. Sonoelastography and Doppler ultrasound can serve as auxiliary tools to confirm CTS diagnoses. Further studies are needed to expand the use of sonography for diagnosing CTS, especially in the presence of concomitant neuromuscular disease(s).

Abstract:Objectives: The goal of this work is to determine the clinical value of the transverse carpal ligament (TCL) in carpal tunnel syndrome (CTS) for guiding subsequent treatment. Methods: This study analyzed patients who underwent median nerve (MN) ultrasound (US) examination of the wrist from April 2020 to April 2021. The cross-sectional area and anteroposterior diameter of the MN, as well as the TCL thickness and stiffness, were measured from images. The intra-group and intra-patient subgroup differences were compared using a t- test and a rank test. We also utilized receiver operating characteristic (ROC) curves to diagnose CTS and evaluate the severity. Results: The final cohort consisted of 120 wrists (bilateral) from 60 samples, evenly balanced across the patient and control groups according to their CTS diagnosis. In the unilateral positive patient subgroup, the MN and TCL of the positive hand were significantly thicker and stiffer than the negative counterparts (both, p < 0.05). The values from the right were also thicker and stiffer than the left (both, p < 0.05) in patients with bilateral CTS. The MN and TCL of the patient group were also significantly thicker and stiffer than those of the control group (both, p < 0.001). For diagnosing CTS, the area under the curve (AUC) of TCL thickness and stiffness at the distal carpal tunnel (DCT) ranged between 0.925 and 0.967. For evaluating CTS severity, we found that the optimal TCL stiffness is sufficient for diagnosing mild and non-mild patient cases (AUC: Emean = 0.757, Emax = 0.779). Conclusions: Shear wave elastography is therefore an effective method for CTS diagnosis and management.
