Bilateral Alterations in Isokinetic Strength and Knee Biomechanics During Side-Cutting 1 Year after Unilateral ACL Reconstruction.

Abstract:BACKGROUND:Individuals with anterior cruciate ligament (ACL) reconstruction (ACLR) are a population that has a higher risk for ACL injury compared with the general population. To reduce the reinjury rate and improve the rehabilitation outcome after ACLR, risk factors for ACL injury have to be addressed.PURPOSE:To compare the knee biomechanics during side-cutting and isokinetic strength of the thigh muscle of the reconstructed leg with those of the contralateral leg and healthy controls and investigate the knee movement asymmetries in individuals with ACLR.STUDY DESIGN:Controlled laboratory study.METHODS:A total of 16 participants with ACLR (ACLR group; 11.8 ± 1.1 months after reconstruction) and 16 healthy controls (control group) were recruited. Landmark coordinates and ground-reaction forces during side-cutting and isokinetic strength of hamstring and quadriceps were collected. Two-way analysis of variance with the mixed design was performed to compare each dependent variable between groups and across legs.RESULTS:The reconstructed leg had a significantly smaller knee flexion angle (P = .004) and less quadriceps strength (P = .003) than the contralateral leg. The knee extension moment and knee external rotation angle were decreased compared with both the contralateral leg (P = .001, P = .003, respectively) and the healthy control leg (P = .001, P = .001, respectively). The ACLR group showed greater knee abduction angles (P = .004) and smaller knee external rotation moments (P = .006) than the control group. The ACLR group also demonstrated greater asymmetries of knee flexion angle (P = .015), knee external rotation angle (P = .001), knee extension moment (P = .013), knee abduction moment (P = .001), and quadriceps strength (P = .046) than the control group.CONCLUSION:Knee biomechanics in the leg with ACLR were altered mainly in the sagittal plane during side-cutting compared with the contralateral leg. The altered movement patterns between the ACLR and control groups were primarily observed in the frontal and transverse planes. The ACLR group also demonstrated greater asymmetries of sagittal knee movement and concentric quadriceps strength than the control group.CLINICAL RELEVANCE:Individuals with ACLR showed different alterations in the reconstructed and contralateral leg compared with healthy controls. These results suggest that rehabilitation programs after ACLR should further focus on restoring the knee flexion angle and quadriceps strength. Injury prevention programs need to be further targeted in the altered movement patterns observed between the ACLR and the healthy groups.

Abstract:UV-curable chitosans (UVCC-7-10) were synthesized using less-toxic agents. The UVCC-7 was completely cured by UV spot irradiation for 4 s. The UVCC-7 was implanted into murine subcutaneous tissues, and the response to the implantation was observed by histological examination at 7 days after implantation. In the histological findings, the implant was surrounded by thin fibrous granulating tissue with no inflammatory cellular infiltration. Fibroblasts infiltrate between the cured implant. The novel synthesized UVCC-7 showed good biocompatibility.
Analysis of Limb Loading and Lower Extremity Strength Recovery Across Time After Anterior Cruciate Ligament Reconstruction

Abstract:Background: Evidence as to how patient thigh muscle strength and limb loading (LL) during a squatting task recovers throughout rehabilitation after anterior cruciate ligament reconstruction (ACLR) is lacking. Hypothesis: Patients will improve LL and strength throughout rehabilitation. Changes in LL and strength over time will be positively correlated. Study design: Prospective cohort study. Level of evidence: Level 3. Methods: A total of 60 participants (28 male/32 female; age, 22.5 ± 9.35 years) participated in 2 visits post-ACLR, assessing LL and strength. Using an instrumented pressure mat, patients completed 3 sets of 3 repetitions of bodyweight squats. Peak force (N), unilateral cumulative load (%), and quadriceps and hamstring isokinetic peak torque (N·m) were calculated and recorded bilaterally. LL and peak torque were compared over time and between limbs. Results: A significant limb-by-time interaction was observed for LL peak force (N), where patients underloaded the ACLR limb at visit 1 compared with the contralateral limb (P < 0.01). Patients increased their ACLR LL across visits (P = 0.04). A limb-by-time interaction for quadriceps peak torque (N·m) was observed where the ACLR limb increased peak torque across visits (P < 0.01); however, strength deficits persisted at visit 2 (P < 0.01) when compared with the nonoperative limb. Weak correlations were observed between all change scores metrics (r, 0.20-0.25). Conclusion: Patients recovering from ACLR exhibited more symmetric loading during a squatting task and improved their lower extremity strength over time. Changes in strength were not related to changes in LL during a squatting task over time. Clinical relevance: Squatting tasks are safe and easily implemented throughout ACLR recovery. As changes in functional LL and strength recovery are not related, both should be considered in serial postoperative testing for more comprehensive function and strength assessments.
Quadriceps Strength Asymmetry After Anterior Cruciate Ligament Reconstruction Alters Knee Joint Biomechanics and Functional Performance at Time of Return to Activity

Abstract:Background: Quadriceps strength deficits are observed clinically after anterior cruciate ligament (ACL) injury and reconstruction and are often not overcome despite rehabilitation. Given that quadriceps strength may be important for achieving symmetrical joint biomechanics and promoting long-term joint health, determining the magnitude of strength deficits that lead to altered mechanics is critical. Purpose: To determine if the magnitude of quadriceps strength asymmetry alters knee and hip biomechanical symmetry as well as functional performance and self-reported function. Study Design: Cross-sectional study; Level of evidence, 3. Methods: A total of 73 patients were tested at the time they were cleared for return to activity after ACL reconstruction. Quadriceps strength and activation, scores on the International Knee Documentation Committee form, the hop for distance test, and sagittal plane lower extremity biomechanics were recorded while patients completed a single-legged hop. Results: Patients with high and moderate quadriceps strength symmetry had larger central activation ratios as well as greater limb symmetry indices on the hop for distance compared with patients with low quadriceps strength symmetry ( P < .05). Similarly, knee flexion angle and external moment symmetry were higher in the patients with high and moderate quadriceps symmetry compared with those with low symmetry ( P < .05). Quadriceps strength was found to be associated with sagittal plane knee angle and moment symmetry ( P < .05). Conclusion: Patients with low quadriceps strength displayed greater movement asymmetries at the knee in the sagittal plane. Quadriceps strength was related to movement asymmetries and functional performance. Rehabilitation after ACL reconstruction needs to focus on maximizing quadriceps strength, which likely will lead to more symmetrical knee biomechanics.
Beyond weakness: Exploring intramuscular fat and quadriceps atrophy in ACLR recovery

Abstract:Muscle weakness following anterior cruciate ligament reconstruction (ACLR) increases the risk of posttraumatic osteoarthritis (OA). However, focusing solely on muscle weakness overlooks other aspects like muscle composition, which could hinder strength recovery. Intramuscular fat is a non-contractile element linked to joint degeneration in idiopathic OA, but its role post-ACLR has not been thoroughly investigated. To bridge this gap, we aimed to characterize quadriceps volume and intramuscular fat in participants with ACLR (male/female = 15/9, age = 22.8 ± 3.6 years, body mass index [BMI] = 23.2 ± 1.9, time since surgery = 3.3 ± 0.9 years) and in controls (male/female = 14/10, age = 22.0 ± 3.1 years, BMI = 23.3 ± 2.6) while also exploring the associations between intramuscular fat and muscle volume with isometric strength. Linear mixed effects models assessed (I) muscle volume, (II) intramuscular fat, and (III) strength between limbs (ACLR vs. contralateral vs. control). Regression analyses were run to determine if intramuscular fat or volume were associated with quadriceps strength. The ACLR limb was 8%-11% smaller than the contralateral limb (p < 0.05). No between-limb differences in intramuscular fat were observed (p = 0.091-0.997). Muscle volume but not intramuscular fat was associated with strength in the ACLR and control limbs (p < 0.001-0.002). We demonstrate that intramuscular fat does not appear to be an additional source of quadriceps dysfunction following ACLR and that muscle size only explains some of the variance in muscle strength.
Angle-Specific Analysis of Isokinetic Quadriceps and Hamstring Strength at 6 and 12 Months after Unilateral ACL Reconstruction.

Abstract:Background: Quadriceps and hamstring strength deficits are related to the increased risk of reinjury after anterior cruciate ligament reconstruction (ACLR). Hypothesis: Knee angle-specific quadriceps and hamstring strength differences would be observed in patients with ACLR 6 and 12 months after surgery. Study Design: Case-series. Level of Evidence: Level 4. Methods: A total of 23 postprimary unilateral ACLR patients followed-up at 6 and 12 months postoperatively and 25 controls were included. Isokinetic knee extension and flexion strength were evaluated at 60 deg/s from 20 degrees to 90 degrees. Statistical parametric mapping were performed to explore the angle-specific strength and the limb symmetry index (LSI). Results: At 6 months postoperatively, the reconstructed leg demonstrated lower knee extension and flexion strength than the contralateral (20 degrees-77 degrees, 24 degrees-90 degrees) (P < 0.01) and control legs (22 degrees-90 degrees, 40 degrees-82 degrees) (P < 0.01). From 6 months to 12 months, knee extension (60 degrees-90 degrees) and flexion (20 degrees-79 degrees) strength improved in the reconstructed leg (P < 0.05), while LSI remained unchanged (P > 0.02). At 12 months, knee extension strength differences persisted in the reconstructed leg compared with the contralateral (20 degrees-81 degrees) and controls (25 degrees-63 degrees) (P < 0.01). ACLR patients had lower LSI of knee extension strength at 6 (20 degrees-59 degrees) and 12 (24 degrees-57 degrees) months postoperatively than the controls (P < 0.02). Conclusion: The reconstructed leg exhibited differences in knee extension strength compared with the contralateral and control legs. Although bilateral knee extension strength increased from 6 to 12 months postoperatively, LSI did not show improvement during this period. Clinical Relevance: Quadriceps restoration was observed only in knee flexion angles greater than 60 degrees compared with controls. Future studies should investigate whether knee extension strength, especially in lower flexion angles, can be enhanced through rehabilitation programs. Furthermore, assessing the impact of this improvement on long-term outcomes and reinjury risk in ACLR patients is warranted.

Abstract:Introduction: It is well documented that marked weakness of the quadriceps is present after knee joint injury. This joint trauma induces a presynaptic reflex inhibition of musculature surrounding the joint, termed arthrogenic muscle inhibition (AMI). The extent to which anterior cruciate ligament (ACL) injury affects thigh musculature motor unit activity, which may affect restoration of thigh muscle strength after injury, is undetermined. Methods: A randomized protocol of knee flexion and extension isometric contractions (10%-50% maximal voluntary isometric contraction) were performed for each leg on 54 subjects with electromyography array electrodes placed on the vastus medialis, vastus lateralis, semitendinosus, and biceps femoris. Longitudinal assessments for motor unit recruitment and average firing rate were acquired at 6-month intervals for 1 year post ACL injury. Results: The ACL-injured population demonstrated smaller quadriceps and hamstrings motor unit size (assessed via motor unit action potential peak-to-peak amplitude) and altered firing rate activity in both injured and uninjured limbs compared to healthy controls. Motor unit activity remained altered compared to healthy controls at 12 months post ACL reconstruction (ACLR). Discussion: Motor unit activity was altered after ACLR up to 12 months post-surgery. Further research is warranted to optimize rehabilitation interventions that adequately address altered motor unit activity and improve safety and success with return to sport after ACLR. In the interim, evidence based clinical reasoning with a focus on development of muscular strength and power capacity should be the impetus behind rehabilitation programming to address motor control deficits.
A Case of Interstitial Pneumonia Induced by Rituximab Therapy

Abstract:Rituximab, a chimeric human/murine monoclonal antibody directed against CD20, is an effective therapy for B-cell type non-Hodgkin’s lymphoma. Most of the adverse responses are mild to moderate nonhematological toxicities, which include fever, chill, or skin reactions. Severe respiratory adverse events have been infrequent [1,2]. Recently, Burton et al reported 2 interstitial pneumonia (IP) cases related to rituximab therapy [3]. IP is rarely induced by rituximab, and its etiological roles are still unclear. Here, we report an IP case related to rituximab therapy, for which we investigated its etiology as well as possible. An 80-year-old man with a diagnosis of diffuse large Bcell lymphoma was treated with rituximab and cyclophosphamide, vincristine, and prednisolone chemotherapy (RCOP). No severe adverse effects were observed during the first course, and after 3 weeks, he was given a second course. With the second rituximab infusion, he had a grade 2 fever and a systemic skin rash. The infusion reaction was abated with hydroxycortisone, and COP therapy was scheduled. Ten days after the second rituximab infusion, the patient complained of general fatigue and had a high fever.A chest x-ray revealed bilateral ground-glass opacities, and a high-resolution computed tomography scan of the chest confirmed the diagnosis of interstitial pneumonia. As respiratory insufficiency advanced, he needed noninvasive positive-pressure ventilation. He was treated with methylprednisolone pulse therapy after a bronchoalveolar lavage (BAL) examination. He made a dramatic recovery, corticosteroid was tapered, and he was discharged after 3 weeks. No pathogens were detected in the patient’s blood, sputum, or BAL fluids, and results for Pneumocystis carini and Mycobacterium tuberculosis were also negative. Abstract:The mechanisms behind the persistent Quadriceps weakness observed after an Anterior Cruciate Ligament reconstruction (ACLr) remain debatable. We described the time course of changes in Quadriceps thickness, strength, and thigh circumference over six months following an ACLr. We also studied the causal relation between Quadriceps thickness, total thickness and strength. Quadriceps thicknesses, Quadriceps strength, and thigh circumference were measured preoperative, 3, and 6 months post-ACLr surgery in 103 patients (77 men and 26 women). Limbs and time were compared with repeated-measures ANOVA and causal relations through mediation analysis (α = 5%). From 0 to 3 months post-surgery, Quadriceps strength (p<0.05), and VI (p<0.05) and VL (p<0.001) thicknesses was reduced. From 3 to 6 months, Quadriceps strength (p<0.05), total Quadriceps thickness (p<0.001), and VI (p<0.001) and VL (p<0.05) thicknesses increased. VM and RF thicknesses increase to 3 months after ACLr (p<0.05). Quadriceps strength, circumference at 5 cm, VI, VL, VM, and total Quadriceps thickness remain lower than the contralateral limb 6 months following an ACLr (p<0.05). The VI thickness determine indirectly (p<0.001) through the total Quadriceps thickness the Quadriceps strength pattern, while the VL thickness directly determined it (p=0.034). Quadriceps thickness adapts heterogeneously, with VI and VL atrophying for up to three months, explaining the post-ACLr quadriceps weakness. By 6 months, VI does not recover its thickness proportion, and VI and VL do not return to pre-surgery conditions. Both vastii muscles primarily determine Quadriceps strength changes, eliciting the VI thickness as a key biomarker for Quadriceps weakness following ACLr.

Abstract:Kurt Goldstein is regarded as one of the major proponents of the holistic movement at the beginning of the 20th century. He rejected the strong localization hypothesis in the field of aphasiology and attempted to link language disturbances to an underlying general intellectual impairment. Goldstein's criticism was based on his subtle symptomatology, his organismic biology, and his philosophical reflections. In his concept of abstract attitude Goldstein searched for a general psychological function that might explain a variety of aphasic symptoms. Abstract attitude bridges the gap between cognitive and linguistic structures. According to Goldstein, it is the basis for words to have a meaning, to be employed in a categorical sense. Since amnesic aphasics are confined to a concrete attitude, their words have lost their representational function. Although Goldstein's concept of abstract attitude is no longer used in scientific discourse, it is analyzed for its heuristic value. It led Goldstein to questions about the relation between cognition and language and to fragments of a semantic theory.

Abstract:BACKGROUND:The incidence of the patellofemoral joint chondral lesions after anterior cruciate ligament reconstruction (ACLR) is disturbingly high. Few studies have assessed the factors affecting patellofemoral joint chondral lesions postoperatively.HYPOTHESIS:The recovery of quadriceps strength after ACLR could be associated with patellofemoral joint cartilage damage.STUDY DESIGN:Cohort study; Level of evidence, 3.METHODS:A total of 88 patients who underwent arthroscopic anatomic double-bundle ACLR with hamstring autografts received second-look arthroscopy at the time of metal staple removal at an average of 24.1 months (range, 12-51 months) postoperatively. All patients underwent standardized isokinetic strength testing for bilateral quadriceps and hamstrings 1 to 2 days before second-look arthroscopy. The patients were divided into 2 groups: Patients in group 1 had a ≥20% deficit on the peak torque measures for quadriceps compared with that of the contralateral knee, whereas those in group 2 had a <20% deficit on peak torque. Cartilage status at the patellofemoral joint and tibiofemoral joint were evaluated by second-look arthroscopy and the Outerbridge classification. Other assessments included the International Knee Documentation Committee (IKDC) score, Tegner and Lysholm scores, side-to-side difference on KT-2000 arthrometer, and range of motion.RESULTS:There were 42 patients included in group 1 and 46 patients in group 2. The mean postoperative quadriceps peak torque of the involved knee compared with the contralateral knee was 70% (range, 57%-80%) in group 1 and 95% (range, 81%-116%) in group 2. For all patients, a significant worsening was seen in the patellar and trochlear cartilage (P = .030 and <.001, respectively) but not at the medial or lateral tibiofemoral joint after ACLR. A significant worsening in the status of both patellar and trochlear cartilage was seen after ACLR in group 1 (P = .013 and =.011, respectively) and of trochlear cartilage in group 2 (P = .006). Significantly fewer severe chondral lesions of the patella were found in group 2 than in group 1 (proportion of patients whose cartilage grade worsened: 26% vs 48%, P < .05; difference in cartilage grade: 0.09 vs 0.62, P < .05). There was no significant difference for trochlear chondral worsening between the 2 groups. No significant differences were detected between the 2 groups in terms of hamstring strength; Lysholm, Tegner, and IKDC scores; KT-2000 arthrometer anterior laxity; or range of motion.CONCLUSION:Greater than 80% recovery of quadriceps strength after ACLR is associated with less severe patellar cartilage damage at short-term follow-up.

J Strength Cond Res XX(X): 000–000, 2023—The purpose of this study was to investigate the relationship between muscle fiber type–specific properties of the vastus lateralis and quadriceps muscle performance in individuals after an anterior cruciate ligament (ACL) tear. 26 subjects (22.0 ± 5.4 years) were included in this cross-sectional study, and all data were collected before ACL reconstruction. Quadriceps peak torque (QPT) and early (0–100 ms) and late (100–200 ms) rate of torque development (RTD) were obtained from maximal voluntary isometric quadriceps strength testing. Muscle fiber cross-sectional area (fCSA) and percent fiber type distribution (FT%) were evaluated through immunohistochemical analysis of a muscle biopsy. Between-limb differences in fiber characteristics were assessed using paired t-tests (with α-level 0.05). Relationships between fiber-specific properties and quadriceps muscle performance were determined using separate multiple linear regression analyses for ACL-injured and noninjured limbs. There were significant differences in fCSA between ACL-injured and noninjured limbs across all fiber types, but no differences in FT%. Type 1 fCSA, type 2a fCSA, and their interaction effect were the explanatory variables with the strongest relationship to all performance outcomes for the ACL-injured limb. The explanatory variables in the ACL-injured limb had a significant relationship to QPT and late RTD, but not early RTD. These findings suggest that QPT and late RTD are more heavily influenced by fCSA than FT% in ACL-injured limbs. This work serves as a foundation for the development of more specific rehabilitation strategies aimed at improving quadriceps muscle function before ACL reconstruction or for individuals electing nonsurgical management.

Abstract:PurposeThe purpose of this study was to determine the effect of concomitant meniscal surgery on the recovery of quadriceps activation and strength at a time when individuals return to sport following anterior cruciate ligament (ACL) reconstruction.MethodsForty-six individuals that were cleared for participation following ACL reconstruction were invited to participate in this study. Participants were placed into groups according to surgical reports (ACL-only, n = 24; meniscal repair, n = 12; meniscectomy, n = 10). Quadriceps strength was quantified using isokinetic and isometric measures. Isokinetic strength was collected at 60°/s in concentric mode. Isometric strength was collected at 90° of knee flexion. Quadriceps activation was assessed using the burst superimposition technique and quantified via the central activation ratio. One-way ANOVAs were utilized to detect whether differences existed in quadriceps activation and strength between groups. Where appropriate, post hoc Bonferroni multiple comparison procedures were used.ResultsQuadriceps activation (P = n.s.) and strength (isokinetic: P = n.s.; isometric: P = n.s.) were not different between groups.ConclusionConcomitant meniscectomy or meniscal repair did not affect the recovery of quadriceps activation and strength at a time when individuals return to sport following ACL reconstruction. Though group differences in quadriceps function were not detected, all participants demonstrated levels of quadriceps activation failure that are below healthy individuals at a time when they were returned to sport. Given that persistent quadriceps activation failure is detrimental to knee function, rehabilitation protocols that target quadriceps activation failure should be developed and employed post-reconstruction.Level of evidenceIII.

Abstract:Background Quadriceps dysfunction is common following anterior cruciate ligament reconstruction and contributes to aberrant gait biomechanics. Changes in quadriceps composition also occur in these patients including greater concentrations of non-contractile tissue. The purpose of this study was to evaluate associations between quadriceps composition, function, and gait biomechanics in individuals with anterior cruciate ligament reconstruction. Methods Forty-eight volunteers with anterior cruciate ligament reconstruction completed gait biomechanics and quadriceps function and composition assessments. Gait biomechanics were sampled during downhill walking (−10° slope) on an instrumented treadmill. Quadriceps function (peak torque and rate of torque development) was assessed via maximal isometric contractions, while composition was evaluated via ultrasound echo intensity. Findings Greater quadriceps peak torque was associated with a greater peak knee extension moment ( r = 0.365, p = 0.015). Greater vastus lateralis echo intensity (i.e. poorer muscle quality) was associated with less knee flexion displacement ( r = −0.316, p = 0.032). Greater echo intensity of the vastus lateralis ( r = −0.298, p = 0.044) and rectus femoris ( r = −0.322, p = 0.029) was associated with a more abducted knee angle at heel strike. Quadriceps peak torque explained 11–16% of the variance in echo intensity. Interpretation Both quadriceps function and composition influence aberrant gait biomechanics following anterior cruciate ligament reconstruction. Quadriceps composition appears to provide insight into quadriceps dysfunction independent of muscle strength, as they associated with different gait biomechanics outcomes and shared minimal variance. Future research is necessary to determine the influence of changes in quadriceps composition on joint health outcomes.

Abstract:Background Despite quadriceps weakness in individuals after quadriceps tendon anterior cruciate ligament reconstruction (QT‐ACLR), and its association with knee joint mechanics, no studies have addressed gait mechanics in both partial‐thickness (PT‐Q) and full‐thickness (FT‐Q) options for QT‐ACLR. Objective To assess gait mechanics across a QT‐ACLR cohort. We hypothesized that QT‐ACLR would show changes in knee joint mechanics compared to control participants (CON) and nonoperated limbs. Additionally, we hypothesized that FT‐Q operated limbs would show greater changes compared to PT‐Q and CON. Design Retrospective cohort study. Setting University‐affiliated sports medicine institute. Participants Sixteen patients who underwent QT‐ACLR (7 FT‐Q: Age (years) = 28.6 ± 7.3, post‐op (months) = 23.5 ± 10.7, 9 PT‐Q: Age = 25.2 ± 4.3, post‐op = 24.4 ± 11.7) were recruited and compared to 11 CON (age = 23.4 ± 4.8). Intervention Participants underwent gait testing with force plate integrated motion capture. Main Outcome Measures Mixed repeated‐measures analyses of covariance, adjusted for gait speed, were used to determine significant main effects or interactions in peak knee flexion angle, sagittal knee range of motion, peak internal knee extension moment (KEM), and peak internal knee flexion moment. Results When measured an average of 2 years after surgery, no main effect for limb or limb by depth interaction were detected. A significant effect by group was observed for peak KEM (p = .03, η2 = .27) and peak knee flexion angle (p = .04, η2 = .24) in the loading response phase. FT‐Q (p = .02) and PT‐Q (p = .03) showed lower KEM compared to the CON group in both limbs. The FT‐Q group showed lower peak knee flexion angle compared to the CON group (p = .01). Conclusions Knee joint symmetry may be recovered 2 years following QT‐ACLR, but lower KEM compared to CON for both graft options and lower peak knee flexion angle than CON for the FT‐Q group may indicate a need for further investigation in QT‐ACLR.

Abstract:Background: Lower extremity movement asymmetries may lead to re-injury and knee osteoarthritis after anterior cruciate ligament (ACL) reconstruction surgery. However, there is no consensus regarding the effect of quadriceps strength asymmetry on lower extremity movement asymmetry after ACL reconstruction. Research question: What is the relationship between quadriceps strength asymmetry and asymmetries in lower extremity kinematics and kinetics during walking in individuals who underwent ACL reconstruction surgery?. Methods: Isometric quadriceps strength, kinematic, and kinetic data during walking were collected from 24 men with unilateral ACL reconstruction. Knee joint angles and moments were reduced. Pearson correlation coefficients between asymmetry in selected knee biomechanics and isometric quadriceps strength asymmetry were determined. Results: The isometric quadriceps strength of the injured leg was significantly lower than that of the uninjured leg (P < 0.001). Knee flexion angles and knee extension moments were smaller in the injured leg than that in the uninjured leg during both loading response (P = 0.007, P = 0.047) and mid-stance phases (P = 0.005, P = 0.028). Isometric quadriceps strength asymmetry was significantly correlated with asymmetry in the peak knee flexion angle during loading response and mid-stance phases (r = -0.48, P = 0.017, r = -0.48, P = 0.017). Isometric quadriceps strength asymmetry was also significantly correlated with asymmetry in the peak knee extension moment during the mid-stance phase (r = -0.44, P = 0.033). Significance: Individuals with ACL reconstruction demonstrate knee movement asymmetry in the sagittal plane. Isometric quadriceps strength asymmetry is significantly correlated with asymmetry in knee flexion angles during the early stance phase and knee extension moments during the mid-stance phase. Rehabilitation programs should emphasise eccentric exercise to beneficially modify quadriceps neuromuscular control.

Abstract:Background: Release for full activity and return to sport after anterior cruciate ligament reconstruction (ACLR) is often dictated by time from surgery and subjective opinion by the medical team. Temporal guidelines for return to sport may not accurately identify impaired strength and neuromuscular control, which are associated with increased risk for second injury (contralateral and/or ipsilateral limb) after ACLR in athletes. Hypotheses: Athletes undergoing ACLR and returning to sport would demonstrate functional deficits that would not be associated with time from surgery. Study Design: Controlled laboratory study. Methods: Thirty-three male (n = 10) and female (n = 23) athletes with unilateral ACLR, who were cleared by a physician to return to their sport after surgery and rehabilitation, performed the single-legged vertical hop test for 10 seconds on a portable force plate. Matched teammates of each patient were recruited to serve as sex-, sport-, and age-matched controls (CTRL; n = 67). Maximum vertical ground-reaction force (VGRF) was measured during each single-limb landing. Single-limb symmetry index (LSI) was calculated as the ratio of the involved divided by uninvolved limb, expressed as a percentage. Results: The single-limb vertical jump height LSI was reduced in the ACLR group, 89% (95% confidence interval [CI], 83%-95%), compared with the matched CTRL group, 101% (95% CI, 96%-105%; P < .01). The LSI for VGRF normalized to potential energy achieved during flight of the hop was increased in ACLR at 112% (95% CI, 106%-117%) relative to the CTRL group at 102% (95% CI, 98%-106%; P .05; R 2 = .002-.01). Conclusion: Deficits in unilateral force development (vertical jump height) and absorption (normalized VGRF) persist in an athlete’s single-limb performance after ACLR and full return to sports. These symmetry deficits appear to be independent of time after reconstruction. Clinical Relevance: On the basis of these results, clinicians should consider assessment of single-limb power performance in the decision-making process for return-to-sport release. Persistent side-to-side asymmetries may increase the risk of contralateral and/or ipsilateral injury.
