Abstract:BACKGROUND:Individuals with anterior cruciate ligament (ACL) reconstruction (ACLR) are a population that has a higher risk for ACL injury compared with the general population. To reduce the reinjury rate and improve the rehabilitation outcome after ACLR, risk factors for ACL injury have to be addressed.PURPOSE:To compare the knee biomechanics during side-cutting and isokinetic strength of the thigh muscle of the reconstructed leg with those of the contralateral leg and healthy controls and investigate the knee movement asymmetries in individuals with ACLR.STUDY DESIGN:Controlled laboratory study.METHODS:A total of 16 participants with ACLR (ACLR group; 11.8 ± 1.1 months after reconstruction) and 16 healthy controls (control group) were recruited. Landmark coordinates and ground-reaction forces during side-cutting and isokinetic strength of hamstring and quadriceps were collected. Two-way analysis of variance with the mixed design was performed to compare each dependent variable between groups and across legs.RESULTS:The reconstructed leg had a significantly smaller knee flexion angle (P = .004) and less quadriceps strength (P = .003) than the contralateral leg. The knee extension moment and knee external rotation angle were decreased compared with both the contralateral leg (P = .001, P = .003, respectively) and the healthy control leg (P = .001, P = .001, respectively). The ACLR group showed greater knee abduction angles (P = .004) and smaller knee external rotation moments (P = .006) than the control group. The ACLR group also demonstrated greater asymmetries of knee flexion angle (P = .015), knee external rotation angle (P = .001), knee extension moment (P = .013), knee abduction moment (P = .001), and quadriceps strength (P = .046) than the control group.CONCLUSION:Knee biomechanics in the leg with ACLR were altered mainly in the sagittal plane during side-cutting compared with the contralateral leg. The altered movement patterns between the ACLR and control groups were primarily observed in the frontal and transverse planes. The ACLR group also demonstrated greater asymmetries of sagittal knee movement and concentric quadriceps strength than the control group.CLINICAL RELEVANCE:Individuals with ACLR showed different alterations in the reconstructed and contralateral leg compared with healthy controls. These results suggest that rehabilitation programs after ACLR should further focus on restoring the knee flexion angle and quadriceps strength. Injury prevention programs need to be further targeted in the altered movement patterns observed between the ACLR and the healthy groups.

Abstract:Individuals post-ACL reconstruction (ACLR) are at elevated risk for ACL re-injury. While several studies have examined biomechanical asymmetries post-ACLR during landing, less is known about asymmetries during a sidestep cut. Therefore, the purpose of this study was to compare sagittal and frontal plane biomechanics at the hip and knee during a 45° sidestep cut in post-ACLR participants and healthy controls. Nineteen athletes post-ACLR and nineteen healthy controls performed a bilateral 45° sidestep cut while three-dimensional kinematics and kinetics were measured. Sagittal and frontal plane kinematics and kinetics were examined at the hip and knee during stance phase. A linear mixed model compared biomechanical differences between the limbs of ACLR and healthy control participants (α = 0.05). In the post-ACLR group, peak hip extension, peak knee flexion, sagittal hip and knee excursion, and the peak knee extensor moment were significantly lower in the ACLR surgical limb compared to the non-surgical limb ( p < 0.05). The peak knee flexion angle and peak knee extensor moment were also lower in the ACLR surgical limb compared to the matched control limb ( p < 0.05). In summary, post-ACLR participants exhibited altered sagittal plane movement in their surgical limb that was not demonstrated in the non-surgical limb or in control participants, which may suggest avoidance, or reduced utilization of the ACLR limb.

Abstract:Background: Lower extremity movement asymmetries may lead to re-injury and knee osteoarthritis after anterior cruciate ligament (ACL) reconstruction surgery. However, there is no consensus regarding the effect of quadriceps strength asymmetry on lower extremity movement asymmetry after ACL reconstruction. Research question: What is the relationship between quadriceps strength asymmetry and asymmetries in lower extremity kinematics and kinetics during walking in individuals who underwent ACL reconstruction surgery?. Methods: Isometric quadriceps strength, kinematic, and kinetic data during walking were collected from 24 men with unilateral ACL reconstruction. Knee joint angles and moments were reduced. Pearson correlation coefficients between asymmetry in selected knee biomechanics and isometric quadriceps strength asymmetry were determined. Results: The isometric quadriceps strength of the injured leg was significantly lower than that of the uninjured leg (P < 0.001). Knee flexion angles and knee extension moments were smaller in the injured leg than that in the uninjured leg during both loading response (P = 0.007, P = 0.047) and mid-stance phases (P = 0.005, P = 0.028). Isometric quadriceps strength asymmetry was significantly correlated with asymmetry in the peak knee flexion angle during loading response and mid-stance phases (r = -0.48, P = 0.017, r = -0.48, P = 0.017). Isometric quadriceps strength asymmetry was also significantly correlated with asymmetry in the peak knee extension moment during the mid-stance phase (r = -0.44, P = 0.033). Significance: Individuals with ACL reconstruction demonstrate knee movement asymmetry in the sagittal plane. Isometric quadriceps strength asymmetry is significantly correlated with asymmetry in knee flexion angles during the early stance phase and knee extension moments during the mid-stance phase. Rehabilitation programs should emphasise eccentric exercise to beneficially modify quadriceps neuromuscular control.

Abstract:Background: Anterior cruciate ligament (ACL) injury predisposes individuals to early-onset knee joint osteoarthritis (OA). Abnormal joint loading is apparent after ACL injury and reconstruction. The relationship between altered joint biomechanics and the development of knee OA is unknown. Hypothesis: Altered knee joint kinetics and medial compartment contact forces initially after injury and reconstruction are associated with radiographic knee OA 5 years after reconstruction. Study Design: Case-control study; Level of evidence, 3. Methods: Individuals with acute, unilateral ACL injury completed gait analysis before (baseline) and after (posttraining) preoperative rehabilitation and at 6 months, 1 year, and 2 years after reconstruction. Surface electromyographic and knee biomechanical data served as inputs to an electromyographically driven musculoskeletal model to estimate knee joint contact forces. Patients completed radiographic testing 5 years after reconstruction. Differences in knee joint kinetics and contact forces were compared between patients with and those without radiographic knee OA. Results: Patients with OA walked with greater frontal plane interlimb differences than those without OA (nonOA) at baseline (peak knee adduction moment difference: 0.00 ± 0.08 N·m/kg·m [nonOA] vs −0.15 ± 0.09 N·m/kg·m [OA], P = .014; peak knee adduction moment impulse difference: −0.001 ± 0.032 N·m·s/kg·m [nonOA] vs −0.048 ± 0.031 N·m·s/kg·m [OA], P = .042). The involved limb knee adduction moment impulse of the group with osteoarthritis was also lower than that of the group without osteoarthritis at baseline (0.087 ± 0.023 N·m·s/kg·m [nonOA] vs 0.049 ± 0.018 N·m·s/kg·m [OA], P = .023). Significant group differences were absent at posttraining but reemerged 6 months after reconstruction (peak knee adduction moment difference: 0.02 ± 0.04 N·m/kg·m [nonOA] vs −0.06 ± 0.11 N·m/kg·m [OA], P = .043). In addition, the OA group walked with lower peak medial compartment contact forces of the involved limb than did the group without OA at 6 months (2.89 ± 0.52 body weight [nonOA] vs 2.10 ± 0.69 body weight [OA], P = .036). Conclusion: Patients who had radiographic knee OA 5 years after ACL reconstruction walked with lower knee adduction moments and medial compartment joint contact forces than did those patients without OA early after injury and reconstruction.

Abstract:Objective The debate on the superiority of single- or double-bundle for anterior cruciate ligament reconstruction has not ceased. The comparative studies on intra-articular biomechanics after different surgical reconstructions are rare. This study is to evaluate the biomechanical stress distribution intra-knee after single- and double-bundle anterior cruciate ligament reconstruction by three-dimensional finite element analysis, and to observe the change of stress concentration under the condition of vertical gradient loads. Methods In this study, magnetic resonance imaging data were extracted from patients and healthy controls for biomechanical analysis. Patients included in the three models were matched in age and sex. The strength and distribution of induced stresses were analyzed in two frequently used procedures, anatomical single-bundle anterior cruciate ligament reconstruction and anatomical double-bundle anterior cruciate ligament reconstruction, using femoral-graft-tibial system under different loads, to mimic a post-operation mechanical motion. The three-dimensional finite-element models for normal ligament and two surgical methods were applied. A vertical force simulating daily walking was performed on the models to assess the interfacial stresses and displacements of intra-articular tissues and ligaments. The evaluation results mainly included the stress of each part of ligament and meniscus. The stress values of different parts of three models were extracted and compared. Results The stress of ligament/graft at femoral side of three finite-element models was significantly higher than at tibial side, while the highest level was observed in single-bundle reconstruction finite-element model. With the increase of force, the maximum stress in the medial (7.1-7.1 MPa) and lateral (4.9-7.4 MPa) meniscus of single-bundle reconstruction finite-element model shifted from the anterior horn to the central area (p = 0.0161, 0.0479, respectively). The stress was shown to be at a lower level at femoral side and posterior cruciate ligament of intra-knee in two reconstruction finite-element models than that in normal finite-element models, while presented higher level at the tibial side than normal knee (p = 0.3528). The displacement of the femoral side and intra-knee areas in reconstruction finite-element models was greater than that in normal finite-element model (p = 0.0855). Conclusion Compared with the single-bundle technique, the graft of double-bundle anterior cruciate ligament reconstruction has better stress dissipation effect and can prevent postoperative meniscus tear more effectively.

Abstract:Background: After anterior cruciate ligament reconstruction (ACLR), athletes have demonstrated performance asymmetries as compared with healthy cohorts, but little research has investigated if biomechanical asymmetries are also different during jump and change-of-direction (CoD) tasks between groups. Purpose: To identify if differences in magnitude of asymmetry of biomechanical and performance variables exist between these groups. Study Design: Controlled laboratory study. Methods: Analysis was conducted between 156 male patients 9 months after surgery and 62 healthy participants. Three-dimensional motion capture and analysis were carried out on a double-legged drop jump, a single-legged drop jump, a single-legged hop for distance, and planned and unplanned CoD. Asymmetry between limbs was calculated for each variable with root mean square difference between limbs. Statistical parametric mapping was used to identify the between-group differences in magnitude of asymmetry of performance and biomechanical variables. Results: There were differences in asymmetry of biomechanical variables across all jump and CoD tests, with greater asymmetries in the ACLR group. The majority of differences between groups were in the sagittal and frontal planes, with more differences found in the jump than CoD tests. The single-legged drop jump demonstrated large differences in performance asymmetry (effect size, 0.94) with small differences for both CoD tests (0.4) and none for the single-legged hop for distance. Conclusion: This study demonstrated greater asymmetry of biomechanical variables 9 months after ACLR as compared with healthy participants across all tests, suggesting insufficient rehabilitation. Clinical Relevance: This study highlights the importance of including biomechanical as well as performance variables when assessing rehabilitation status after ACLR. Patellofemoral and tibiofemoral joint loading during a single‐leg forward hop following ACL reconstruction

Abstract:Altered biomechanics are frequently observed following anterior cruciate ligament reconstruction (ACLR). Yet, little is known about knee‐joint loading, particularly in the patellofemoral‐joint, despite patellofemoral‐joint osteoarthritis commonly occurring post‐ACLR. This study compared knee‐joint reaction forces and impulses during the landing phase of a single‐leg forward hop in the reconstructed knee of people 12‐24 months post‐ACLR and uninjured controls. Experimental marker data and ground forces for 66 participants with ACLR (28±6 years, 78±15 kg) and 33 uninjured controls (26±5 years, 70±12 kg) were input into scaled‐generic musculoskeletal models to calculate joint angles, joint moments, muscle forces, and the knee‐joint reaction forces and impulses. The ACLR group exhibited a lower peak knee flexion angle (mean difference: ‐6 degrees; 95% confidence interval: [‐10,‐2] degrees), internal knee extension moment (‐3.63 [‐5.29,‐1.97] percentage of body weight * participant height (BW*HT)), external knee adduction moment (‐1.36 [‐2.16,‐0.56] %BW*HT) and quadriceps force (‐2.02 [‐2.95,‐1.09] body weights (BW)). The ACLR group also exhibited a lower peak patellofemoral‐joint compressive force (‐2.24 [‐3.31,‐1.18] BW), net tibiofemoral‐joint compressive force (‐0.74 [‐1.20, 0.28] BW), and medial compartment force (‐0.76[‐1.08,‐0.44] BW). Finally, only the impulse of the patellofemoral‐joint compressive force was lower in the ACLR group (‐0.13 [‐0.23,‐0.03] body weight‐seconds). Lower compressive forces are evident in the patellofemoral‐ and tibiofemoral‐joints of ACLR knees compared to uninjured controls during a single‐leg forward hop‐landing task. Our findings may have implications for understanding the contributing factors for incidence and progression of knee osteoarthritis after ACLR surgery.This article is protected by copyright. Abstract:Purpose: Postural instability and decreased balance control ability have been observed in patients after anterior cruciate ligament (ACL) reconstruction. Herein, we examined the abnormal balance control mechanisms of these patients during dynamic reaching forward and quiet standing, providing a quantitative index for rehabilitation assessment. Methods: We enrolled ACL reconstruction patients 6–8 months after surgery, and 14 gender- and age-matched healthy volunteers. The IKDC and Lysholm were applied in each patient after ACL reconstruction. All participants conducted the quiet standing and reaching forward (RF) tests at the specified locations on force plates. The ground reaction force, center of pressure (COP), and kinematics signals were recorded. The maximal reach distance (MRD), speed of RF, length of COP, peak speed of COP in anterior-posterior direction (AP-COP), and weight bearing ratio (WBR) of the affected limb were calculated in the RF test. The COP speed, COP amplitude, frequency components of COP and WBR were extracted during quiet standing. Results: We observed that the speed of RF in the patients after ACL reconstruction was significantly lower than that of controls ( p < 0.05). The COP length during RF was positively correlated with the Lysholm scale in the affected limb of patients (r = 0.604, p < 0.05). The peak of AP-COP speed during RF in the affected limb of patients was significantly lower than that of the healthy controls ( p < 0.05), and positively correlated with the IKDC scale (r = 0.651, p < 0.05). WBR on the affected limb of patients during RF were significantly lower than that of controls ( p < 0.05). The mean (r = −0.633, p < 0.05) and peak (r = −0.643, p < 0.05) speeds of COP during quiet standing were negatively correlated with the IKDC scale value. The amplitude of AP-COP on the contralateral side of patients was significantly higher than that of controls during quiet standing ( p < 0.05). Conclusion: Patients after ACL reconstruction performed decreased postural control capacity, especially in dynamic balance, and were accompanied by deficiencies in proprioception. The COP length, peak speed of COP during RF and COP speed during quiet standing could be considered as quantitative index of balance function assessment after ACL reconstruction.

Abstract:ObjectivesTo investigate the biomechanical characteristics of patients with anterior cruciate ligament (ACL) injury by gait analysis, surface electromyography (SEMG), and proprioception test, and provide rehabilitation suggestions according to the results. MethodsIn this retrospective cohort study, 90 adults with unilateral ACL injury, ranging in age from 19 to 45 years (66 men and 24 women, average age: 30.03 ± 7.91) were recruited for this study form May 2018 to July 2019. They were divided into three groups according to the time after the injury: group A (3‐week to 1.5‐month), group B (1.5‐month to 1 year), and group C (more than 1 year). The SEMG signals were collected from the bilateral rectus femoris (RF), vastus medialis (VM), and vastus lateralis (VL) and the root mean square (RMS) were used to assess muscular activity. SEMG were used to analyze muscles function, gait analysis was used to evaluate the walking stability, balance and location assessment were used to analyze the proprioception. ResultsThrough the comparison between bilateral limbs, all muscles strength shown decreased (RF: 239.94 ± 129.70 vs 364.81 ± 148.98, P = 0.001; VM: 298.88 ± 175.41 vs 515.79 ± 272.49, P = 0.001; VL:389.54 ± 157.97 vs 594.28 ± 220.31, P < 0.001) and the division of proprioception became larger (tandem position: 7.79 ± 1.57 vs 6.33 ± 1.49, P = 0.001; stance with one foot: 8.13 ± 0.84 vs 7.1 ± 0.57, P = 0.003; variance of 30°: 6.96 ± 3.15 vs 4.45 ± 1.67, P = 0.03; variance of 60°: 4.64 ± 3.38 vs 2.75 ± 1.98, P = 0.044) in the injured side when compared to the non‐injured and 26 gait parameters were shown difference in group A. In group B, the muscle strength of VL shown decreased (VL: 381.23 ± 142.07 vs 603.9 ± 192.72, P < 0.001) and the division of location of 30° became larger (7.62 ± 4.98 vs 4.33 ± 3.24, P = 0.028) in the injured side when compared to the non‐injured side and there were eight gait parameters that showed differences. In group C, the muscle strength and proprioception showed no differences and only 16 gait parameters showed differences between the bilateral limbs. ConclusionThe results proved the deterioration of proprioception in 30° of injured side will not recover and non‐injury side and will become worse after 1 year from the injury; among the VL, VM, and RF, the recovery rate of VL is the slowest and bilateral straight leg raising (SLR) (30°) is the best way to train it; the gait stability will be worse after 1 year from the injury. Therefore, we suggest that the training for proprioception in 30° and VL are important for the rehabilitation, and the ACL reconstruction should be performed within 1 year.

Abnormal Rotational Knee Motion during Running after Anterior Cruciate Ligament Reconstruction

Abstract:Background The effectiveness of anterior cruciate ligament reconstruction for restoring normal knee kinematics is largely unknown, particularly during sports movements generating large, rapidly applied forces. Hypothesis Under dynamic in vivo loading, significant differences in 3-dimensional kinematics exist between anterior cruciate ligament-reconstructed knees and the contralateral, uninjured knees. Study Design Prospective, in vivo laboratory study. Methods Kinematics of anterior cruciate ligament-reconstructed and contralateral (uninjured) knees were evaluated for 6 subjects during downhill running 4 to 12 months after anterior cruciate ligament reconstruction, using a 250 frame/s stereoradiographic system. Anatomical reference axes were determined from computed tomography scans. Kinematic differences between the uninjured and reconstructed limbs were evaluated with a repeated-measures analysis of variance. Results Anterior tibial translation was similar for the reconstructed and uninjured limbs. However, reconstructed knees were more externally rotated on average by 3.8 ± 2.3° across all subjects and time points (P = .0011). Reconstructed knees were also more adducted, by an average of 2.8 ± 1.6° (P = .0091). Although differences were small, they were consistent in all subjects. Conclusions Anterior cruciate ligament reconstruction failed to restore normal rotational knee kinematics during dynamic loading. Clinical Relevance Although further study is required, these abnormal motions may contribute to long-term joint degeneration associated with anterior cruciate ligament injury/reconstruction.
In Vivo Gait Kinematics of the Knee after Anatomical and Non-Anatomical Single-Bundle Anterior Cruciate Ligament Reconstruction-a Prospective Study

Abstract:Background: The factors that influence functions of knees after anterior cruciate ligament reconstruction (ACLR) still remains uncertain. The functional restoration of knees after ACLR can be reflected on gait kinematics restoration. The purpose of this study was to evaluate the gait kinematics and clinical outcomes of knees after anatomical and non-anatomical single-bundle ACLR during level walking. Methods: Thirty-four patients with unilateral primary single-bundle ACLR and 18 healthy people were recruited. Patients were divided into anatomical reconstruction group (AR group; n=13) and non-anatomical reconstruction group (Non-AR group; n=21) according to Bernard Quadrant method. The ACL graft orientations on coronal and sagittal planes were measured on 3D models from medical images. The 6 degrees of freedom (DOF) kinematics of knees and range of motion (ROM) of 6 DOF kinematics were measured with a portable optical tracking system. The comparison of 6 DOF kinematics and ROM of 6 DOF kinematics were performed between the ACLR knees and contralateral knees. The following assessments were also performed including clinical examination, KT-2000 arthrometer measurement, International Knee Documentation Committee (IKDC) and Lysholm scores. Results: All patients reached a minimum follow-up of 6 months (10 +/- 4 months). For AR group and Non-AR group, no statistically significant differences were observed in gait kinematics between the ACLR knees and contralateral knees. No statistically significant differences between the ACLR knees and contralateral knees were observed in terms of ROM of 6 DOF kinematics in AR group. However, in Non-AR group, the ACLR knees exhibited significant ROM of anterior-posterior translation by approximately 0.5 cm than contralateral knees (P=0.0080). No statistically significant differences between the two groups were observed regarding IKDC subjective score, Lysholm score and KT-2000 arthrometer test. Conclusions: The anatomical ACLR can restore close to normal gait kinematics and ROM of 6 DOF kinematics compared with non-anatomical ACLR. The ACL graft after anatomical ACLR simulated native ACL fibers to function in terms of graft orientation.

Abstract:Background: Athletes who have undergone anterior cruciate ligament reconstruction (ACLR) have a high risk of reinjury upon the return to sports participation. While the mechanisms behind this increased risk of reinjury are unknown, it has been suggested that altered knee biomechanics during sports-specific activities may be a contributing factor. Purpose/Hypothesis: To compare frontal plane knee joint angles and moments during a sidestep cutting maneuver in female soccer athletes who have undergone ACLR with those in athletes with no history of knee injury. It was hypothesized that athletes with a history of ACLR would exhibit increased knee abduction angles and knee adductor moments compared with those with no history of injury. Study Design: Controlled laboratory study. Methods: Twelve female soccer players with a history of ACLR served as the experimental group, and 12 female soccer players with no history of knee injury constituted the control group. Three-dimensional kinematics and ground-reaction forces were collected while each participant performed a sidestep cutting maneuver. Variables of interest included the knee abduction angle and knee adductor moment during the early deceleration phase of the cutting maneuver. Independent-samples t tests were used to evaluate differences between groups ( P ≤ .05). Results: Participants in the ACLR group exhibited increased average knee abduction angles (ACLR: 3.8° vs control: 1.8°; P = .03) and peak knee adductor moments (ACLR: 1.33 N·m/kg vs control: 0.80 N·m/kg; P = .004) compared with the control group. Conclusion: Female soccer players who have undergone ACLR and returned to sports participation exhibited increased knee abduction angles and knee adductor moments during the early deceleration phase of cutting compared with their healthy counterparts with no history of knee injury. Clinical Relevance: Even though athletes are able to return to sport after ACLR, they are at an increased risk for reinjury. It may be the case that the increased frontal plane knee angles and moments exhibited by these athletes after ACLR could be contributing to this risk for reinjury. Therefore, it is important that rehabilitation programs after ACLR include the restoration of frontal plane knee mechanics.

Abstract:Individuals with anterior cruciate ligament (ACL) reconstruction often display abnormal gait mechanics reflective of a "stiff-knee" gait (i.e., reduced knee flexion angles and moments). However, dynamic knee stiffness, which is the dynamic relationship between the position of the knee and the moment acting on it, has not been directly examined during walking in individuals with ACL reconstruction. Here, we aimed to evaluate dynamic knee stiffness in the involved compared to the uninvolved limb during weight-acceptance and mid-stance phases of walking. Twenty-six individuals who underwent ACL reconstruction (Age: 20.2 ± 5.1 yrs., Time post-op: 7.2 ± 0.9 mo.) completed an overground walking assessment using a three-dimensional motion capture system and two force plates. Dynamic knee stiffness (Nm/°) was calculated as the slope of the regression line during weight-acceptance and midstance, obtained by plotting the sagittal plane knee angle versus knee moment. Paired t-tests with Bonferroni corrections were used to compare differences in dynamic stiffness, knee excursions, and moment ranges between limbs during both stance phases. Greater dynamic knee stiffness was found in the involved compared with the uninvolved limb during weight-acceptance and mid-stance (p < 0.01). Knee flexion and extension excursions were reduced in the involved limb during both weight-acceptance and mid-stance, respectively (p < 0.01). Sagittal plane knee moment ranges were not different between limbs during weight-acceptance (p = 0.1); however, the involved limb moment range was reduced relative to the uninvolved limb during mid-stance (p < 0.01). These results indicate that individuals with ACL reconstruction walk with a stiffer knee throughout stance, which may influence knee contact forces and could contribute to the high propensity for post-traumatic knee osteoarthritis development in this population.

Abstract:OBJECTIVE To explore the impact of thrombolytic therapy time delay on in-hospital major adverse cardiac event rate in patients with acute ST-segment elevation myocardial infarction (STEMI). METHODS This survey was performed in 101 level 2 hospitals from 15 provinces or autonomous region in China. Patients admitted to these hospitals with acute STEMI during November 2011 to November 2014 and received thrombolytic therapy were eligible for this study. Multivariate logistic regression modeling was used to estimate the rate of in-hospital major adverse cardiac events (re-infarction, stroke and all-cause mortality) in patients with different thrombolytic time delay. RESULTS A total of 3 719 STEMI patients received thrombolytic therapy, 88%(3 270 cases) of them were treated within 12 hours. After controlling for the impact of confounding factors, such as a variety of risk factors, past disease histories, the severity of STEMI, medication, interventions, etc, MACE rates significantly increased along with the thrombolysis time delay, no matter thrombolysis succeeded or not (trend test P<0.05). In group of successful thrombolysis, multivariate adjusted rate of MACE for 0-5.9 hours, 6.0-11.9 hours and ≥12.0 hours were 3.2%, 3.9% and 7.8%; in group of unsuccessful thrombolysis, it was 11.6%, 14.1% and 25.2% respectively (trend test P<0.05). The multivariate adjusted rates of re-infarction in group of successful thrombolysis as well as in group of unsuccessful thrombolysis significantly increased in proportion to thrombolysis time delay ≥ 12.0 h (trend test P<0.05). In group of successful thrombolysis, multivariate adjusted rate of re-infarction for 0-5.9 hours, 6.0-11.9 hours and ≥12.0 hours were 1.9%, 1.5% and 6.8%; in group of unsuccessful thrombolysis, it was 3.9%, 3.1% and 13.6%, respectively. Within the same time delay group, MACE and re-infarction rates were significantly lower in successful thrombolysis group than in unsuccessful thrombolysis group (all P<0.05). CONCLUSION Thrombolytic therapy should be conducted within 6 hours after the attack. Both re-infarction or MACE rates are significantly increased in patients with more than 12 hours thrombolysis time delay.
Atypical Lower Limb Mechanics During Weight Acceptance of Stair Descent at Different Time Frames After Anterior Cruciate Ligament Reconstruction

Abstract:The American Journal of Sports Medicine, Ahead of Print. Background:An anterior cruciate ligament (ACL) rupture may result in poor sensorimotor knee control and, consequentially, adapted movement strategies to help maintain knee stability. Whether patients display atypical lower limb mechanics during weight acceptance of stair descent at different time frames after ACL reconstruction (ACLR) is unknown.Purpose:To compare the presence of atypical lower limb mechanics during the weight acceptance phase of stair descent among athletes at early, middle, and late time frames after unilateral ACLR.Study Design:Controlled laboratory study.Methods:A total of 49 athletes with ACLR were classified into 3 groups according to time after ACLR—early ( 18 months; n = 16)—and compared with asymptomatic athletes (control; n = 18). Sagittal plane hip, knee, and ankle angles; angular velocities; moments; and powers were compared between the ACLR groups' injured and noninjured legs and the control group as well as between legs within groups using functional data analysis methods.Results:All 3 ACLR groups showed greater knee flexion angles and moments than the control group for injured and noninjured legs. For the other outcomes, the early group had, compared with the control group, less hip power absorption, more knee power absorption, lower ankle plantarflexion angle, lower ankle dorsiflexion moment, and less ankle power absorption for the injured leg and more knee power absorption and higher vertical ground reaction force for the noninjured leg. In addition, the late group showed differences from the control group for the injured leg revealing more knee power absorption and lower ankle plantarflexion angle. Only the early group took a longer time than the control group to complete weight acceptance and demonstrated asymmetry for multiple outcomes.Conclusion:Athletes with different time frames after ACLR revealed atypically large knee angles and moments during weight acceptance of stair descent for both the injured and the noninjured legs. These findings may express a chronically adapted strategy to increase knee control. In contrast, atypical hip and ankle mechanics seem restricted to an early time frame after ACLR.Clinical Relevance:Rehabilitation after ACLR should include early training in controlling weight acceptance. Including a control group is essential when evaluating movement patterns after ACLR because both legs may be affected.

Abstract:Background The effects of anterior cruciate ligament (ACL) reconstruction on postoperative gait biomechanics remain controversial, and the influence of meniscus pathology on pre- and post-ACL reconstruction biomechanical changes has not yet been studied. Here, we aimed to clarify the difference in knee joint moment between pre- and post-ACL reconstruction conditions in subjects with and without meniscal pathology. Methods Twenty-four subjects with unilateral ACL reconstruction injuries participated in this study. A total of 13 of the subjects had concomitant meniscus injuries. Gait analysis was performed preoperatively and at 11 months postoperatively. Three-dimensional knee joint angles and moments were calculated based on motion-capture data. The total joint moment and contribution of individual moments during the stance phase were assessed. For statistical analysis, we compared pre- and postoperative alterations, and differences were assessed in subjects with and without meniscal pathology. Results Tibial rotation excursion was lower in subjects with meniscal pathology than in those with intact menisci postoperatively. An significant increase in the contribution of the knee flexion moment and a significant decrease in the contribution of the knee adduction moment postoperatively were observed in subjects with intact menisci. Conclusions Patients with ACL injury showed different postoperative alterations in joint moments depending on the concomitant meniscal injury.

Abstract:Background: Individuals with a recent anterior cruciate ligament reconstruction may demonstrate an altered movement strategy for protecting the knee and maintaining stability. Altered knee movement might lead to abnormal intra-articular load, potentially contributing to early knee osteoarthritis onset. A protective strategy may be particularly evident during active tasks that induce a pivot-shift manoeuvre, such as a step-down and cross-over task. In this study, we investigated whether knee joint mechanics and muscle activity differed between participants early (∼3 months) following reconstruction (n = 35) to uninjured controls (n = 35) during a step-down and cross-over task with a 45° change-of-direction. Methods: We used motion capture, force plates and surface electromyography to compare time-normalised curves of sagittal and transverse-plane knee mechanics and muscle activity during the cross-over phase between groups using functional t-tests. We also compared knee mechanics between sides within the injured group and compared discrete outcomes describing the cross-over phase between groups. Findings: Compared to controls, the injured participants had greater knee flexion angle and moment, lower internal rotation moment, more preparatory foot rotation of the pivoting leg, a smaller cross-over angle, and a longer cross-over phase for both the injured and uninjured sides. The injured leg also had greater biceps femoris and vastus medialis muscle activity compared to controls and different knee mechanics than the uninjured leg. Interpretation: Individuals with anterior cruciate ligament reconstruction showed a knee-stabilising and pivot-shift avoidance strategy for both legs early in rehabilitation. These results may reflect an altered motor representation and motivate considerations early in rehabilitation.
A Case of Interstitial Pneumonia Induced by Rituximab Therapy

Abstract:Rituximab, a chimeric human/murine monoclonal antibody directed against CD20, is an effective therapy for B-cell type non-Hodgkin’s lymphoma. Most of the adverse responses are mild to moderate nonhematological toxicities, which include fever, chill, or skin reactions. Severe respiratory adverse events have been infrequent [1,2]. Recently, Burton et al reported 2 interstitial pneumonia (IP) cases related to rituximab therapy [3]. IP is rarely induced by rituximab, and its etiological roles are still unclear. Here, we report an IP case related to rituximab therapy, for which we investigated its etiology as well as possible. An 80-year-old man with a diagnosis of diffuse large Bcell lymphoma was treated with rituximab and cyclophosphamide, vincristine, and prednisolone chemotherapy (RCOP). No severe adverse effects were observed during the first course, and after 3 weeks, he was given a second course. With the second rituximab infusion, he had a grade 2 fever and a systemic skin rash. The infusion reaction was abated with hydroxycortisone, and COP therapy was scheduled. Ten days after the second rituximab infusion, the patient complained of general fatigue and had a high fever.A chest x-ray revealed bilateral ground-glass opacities, and a high-resolution computed tomography scan of the chest confirmed the diagnosis of interstitial pneumonia. As respiratory insufficiency advanced, he needed noninvasive positive-pressure ventilation. He was treated with methylprednisolone pulse therapy after a bronchoalveolar lavage (BAL) examination. He made a dramatic recovery, corticosteroid was tapered, and he was discharged after 3 weeks. No pathogens were detected in the patient’s blood, sputum, or BAL fluids, and results for Pneumocystis carini and Mycobacterium tuberculosis were also negative.
