Abstract:Purpose: This study presents a rare case of primary tubercular osteomyelitis of the zygoma, and addresses how combined surgical and medical treatments are effective managing rare presentations of tubercular osteomyelitis in facial bones. Methods: A 57-year-old man presented with pain and purulent discharge from the right malar region following trauma. Initial treatments with empirical antibiotics had poor response, hence further investigations were done. Comprehensive diagnostic workup included lab tests, Computed tomography with contrast, and histopathological examination. The patient underwent surgical debridement of the zygomatic bone through an intraoral approach, and debrided tissue was sent for microbiological and histopathological examination, which confirmed tubercular osteomyelitis. Post-surgery, the patient was treated with a four-drug anti-tubercular regimen. Results: Following anti-tubercular therapy, the patient showed significant improvement, with no signs of disease recurrence after a year of follow-up. Conclusion: Primary tubercular osteomyelitis of the zygoma is extremely rare but should be considered in differential diagnoses of facial swellings and discharging sinuses. This case underscores the importance of a multidisciplinary approach in treating rare presentations of tubercular osteomyelitis.
A Rare Case of Tubercular Osteomyelitis of Mandible in a 5-year-old Child

Abstract:Tuberculosis (TB), also known as Koch's disease, is a chronic granulomatous disease typically caused by Mycobacterium tuberculosis (M. tuberculosis). On 24th March 1882, Dr Robert Koch discovered M. tuberculosis that caused TB. In humans, M. bovis and atypical mycobacterium may also cause this disease. According to the World Health Organization (WHO) Global Report 2022, published on 27th October 2022, the incidence of TB in India for the year 2022 is 210/100,000 population. Primarily, TB affects the pulmonary region in humans, whereas secondarily, it may affect extrapulmonary sites such as the bones and intestines via lymph nodes. In this article, we are reporting a rare case of tubercular osteomyelitis of the mandible, in which the patient reported swelling of the cheek, mimicking an odontogenic infection that led to mandibular osteomyelitis. The definitive diagnosis of tubercular osteomyelitis was made by cartridge-based nucleic acid amplification test (CB-NAAT) when the cheesy material was found during surgical debridement and curettage of the mandible. Following that diagnosis, antitubercular treatment (ATT) was started immediately for the patient, which led to the complete resolution of the disease. The patient has been on regular follow-up for the last six months with no evidence of relapse of disease. Primary TB of the mandible is very rare, with only a few reported cases in the literature. Bony cystic lesion with associated submandibular lymphadenopathy on a background of breast carcinoma: an unexpected case of cervicofacial actinomycosis

Abstract:Actinomycosis is an uncommon, chronic suppurative granulomatous infection and needs to be considered as a differential diagnosis. A 56-year-old woman with a background of type 2 diabetes mellitus and breast carcinoma was referred to the Oral and Maxillofacial Surgery 2-week wait clinic, regarding a tender sublingual mass and firm erythematous swelling in the right submandibular and submental region. This was slowly progressive and had not responded to oral co-amoxiclav. An orthopantomogram showed a well-defined radiolucency and smaller radiolucent lesions throughout the edentulous right body of the mandible. A contrast-enhanced CT confirmed a right submandibular abscess communicating with cavitating lesions. The differentials included osteomyelitis, bony metastases, multiple myeloma or other cystic lesions. The patient underwent incision and drainage of the abscess, alongside biopsies, and intravenous co-amoxiclav was given. Microbiology cultures confirmed the presence of Actinomyces israelii and a diagnosis of cervicofacial actinomycosis with mandibular osteomyelitis. The patient was successfully treated with prolonged antibiotics.
Bilateral Jaw Mycobacterium Abscessus Mimicking Actinomycosis: A Postoperative Complication of Wisdom Teeth Extraction

Abstract:The incidence of nontuberculous mycobacteria infections has surged over recent decades. Mycobacterium abscessus is one example that can present unique diagnostic challenges due to its variable antibiotic resistance profile and its clinical similarities to Actinomycoses israelii in postodontogenic infections. The authors report a case of a 22-year-old healthy female presenting with bilateral mandibular nodules following wisdom teeth extraction. After a presumptive diagnosis of actinomycosis, cultures revealed a Mycobacterium abscessus infection susceptible to macrolides. Magnetic resonance imaging depicted bilateral sinus tracts without osteomyelitis. The patient opted for dual antibiotic therapy, consisting of azithromycin and omadacycline, without surgical intervention. Given her clinical and radiographic improvement after three months, the patient elected to continue dual antibiotic therapy for 12 months with appropriate clinical and radiographic monitoring. This case underscores the importance of early microbial cultures to guide diagnosis and treatment, particularly considering Mycobacterium abscessus's similarities with other pathogens and its variable macrolide susceptibility due to genetic mutations. As highlighted in this case, clinicians must successfully differentiate between and appropriately treat various nontuberculous mycobacteria.

Giant Mandibular Ameloblastoma with Rare Hypercalcemia: A Case Report and Literature Review

Abstract:Ameloblastoma is the most common benign odontogenic tumor with local invasion and high recurrence, which generally occurs in the jaw bones. Hypercalcemia is a common paraneoplastic syndrome that is commonly observed in patients with malignancies but rarely encountered in patients with benign tumors. Thus far, not many cases of ameloblastoma with hypercalcemia have been reported, and the pathogenic mechanism has not been studied in depth. This paper presents a case report of a 26-year-old male diagnosed with giant ameloblastoma of the mandible, accompanied by rare hypercalcemia. Additionally, a review of the relevant literature is conducted. This patient initially underwent marsupialization, yet this treatment was not effective, which indicated that the selection of the appropriate operation is of prime importance for improving the prognosis of patients with ameloblastoma. The tumor not only failed to shrink but gradually increased in size, accompanied by multiple complications including hypercalcemia, renal dysfunction, anemia, and cachexia. Due to the contradiction between the necessity of tumor resection and the patient’s poor systemic condition, we implemented a multi-disciplinary team (MDT) meeting to better evaluate this patient’s condition and design an individualized treatment strategy. The patient subsequently received a variety of interventions to improve the general conditions until he could tolerate surgery, and finally underwent the successful resection of giant ameloblastoma and reconstruction with vascularized fibular flap. No tumor recurrence or distance metastasis was observed during 5 years of follow-up. Additionally, the absence of hypercalcemia recurrence was also noted.

Abstract:Tuberculosis is a disease with presentations both in the lungs and at other extrapulmonary sites. While pulmonary tuberculosis constitutes a significant proportion of total tuberculosis cases, extrapulmonary cases with infections at rare sites are also documented. Herein, an exceedingly rare case of tuberculosis of the cervical vertebrae with retropharyngeal and parapharyngeal abscesses due to Staphylococcus aureus and Mycobacterium tuberculosis in a young Indian male is presented. The rarity of the locations of the lesions with coinfections with two bacteria made the diagnosis challenging. Besides, the potential for a retropharyngeal abscess to compress the airway is an emergency situation. However, the ultimate diagnosis was achieved with the help of a radiograph of the neck, contrast-enhanced computed tomography of the neck, fine-needle aspiration cytology, and a cartridge-based nucleic acid amplification test. He was initiated on appropriate antibiotics and antituberculous chemotherapy per his weight.

Abstract:Tuberculosis is still considered a cause of death, especially in developing countries. Primary tonsillar tuberculosis in the absence of pulmonary tuberculosis in an immunocompetent patient is a rare entity. Its diagnosis is difficult because it simulates tonsillar cancer, and histopathological examination is often needed for confirmation. We report the case of a 30-year-old woman with no history of tuberculosis and a normal lung CT. The lack of response to the initial treatment suggested either tumor pathology or germ-specific tonsillitis. The diagnosis of tonsillar tuberculosis was established based on the histological aspects provided by the tonsil biopsy and the QuantiFERON-TB test.

Abstract:A 6-year-old boy presented with a left mandibular swelling that had been progressing for 18 months. Initially, he received antibiotics without any positive results. Subsequently, teeth 74 and 75 were extracted in an attempt to alleviate the condition. However, the swelling persisted one month after the extractions, and the patient experienced mild pain only during chewing. A panoramic radiograph revealed a rounded radiolucent lesion involving the entire tooth germ of 74 and partially affecting the germ of 75.

A pituitary mass is not always an adenoma: A rare case of pituitary tuberculoma in an adolescent

Abstract:Introduction and importance: Tuberculosis of the central nervous system is unusual and accounts for 1 % of all cases of tuberculosis in the world. The pituitary location is even scarcer. Case presentation: A 14-year-old girl presented with polyuria-polydipsia syndrome and menstrual irregularity. MRI showed an intrasellar lesion of the pituitary gland. She underwent transsphenoidal surgery for histopathological diagnosis and removal of the lesion. Histological findings were consistent with a tuberculoma. She was put on anti-tuberculosis drugs and is being followed up. Clinical discussion: In endemic areas, pituitary tuberculosis should be considered in the differential diagnosis of pituitary tumors. The histological examination will guide the diagnosis. Sometimes, other complementary examinations such as the tuberculin skin test can be of great help when the histology is not conclusive. Medical treatment can be curative, however, surgery can be necessary for decompression. Conclusion: In addition to being the first case of histologically proven primary pituitary tuberculosis in a child reported in Morocco, the present case is unique in the way that the extensive radiological examinations did not reveal any evidence of other systemic or pulmonary tuberculosis.

Abstract:We report a significant excess of heterozygotes at an alpha-tubulin locus in individuals with endometrial carcinoma. Although the heterozygous state was not specific for cancer, there was a 2.6-fold increase in heterozygote frequencies in endometrial tumors over those of normal human endometrial tissues. Among normal control tissues, the two alleles of 2.1 and 2.2 kb apparent size were found to be in a Hardy-Weinberg equilibrium. The results with tumor samples were significantly different from those expected for a Hardy-Weinberg distribution. The alleles identified using MspI digestion and a 2.5-kb alpha-tubulin cDNA probe have been previously reported by Gatti et al. (Nucleic Acids Res. 15, 8119, 1987).

Abstract:Adenoid cystic carcinoma (ACC) is an aggressive form of salivary gland cancer that mostly affects the accessory, parotid, and submandibular salivary glands. This tumor is characterized by slower development, perineural invasion, and possible local recurrence in clinical and pathological findings. A male patient, aged 71, who was from a remote area, appeared with a lesion affecting the right submandibular gland and had been experiencing discomfort in the same region for four months. Following a biopsy and the histological confirmation of ACC in the right submandibular gland, the tumor was widely excised locally.
Mycobacterium tuberculosis and Pseudoramibacter alactolyticus coinfection in brain after dental extraction: A case report

Abstract:Introduction: More than 1200 different types of microbes were found in the human mouth, only some of these microorganisms were associated with intracranial bacterial infection. However, there are limited data available about the Pseudoramibacter alactolyticus (P alactolyticus) or Mycobacterium tuberculosis (MTB) intracranial infections oral origin. Patient concerns: Here, we reported a rarely case with P alactolyticus and MTB coinfection in central nervous after dental extraction. The 44-year-old man presented with progressive headache over the last 2 weeks and a sustained fever >39°C, with a dental extraction performed 2 days before the onset of headache. Diagnosis: P alactolyticus and MTB were confirmed by real-time polymerase chain reaction targeting the16S ribosomal RNA gene. The presence of MTB was also demonstrated by positive acid-fast staining of the purulent discharge. Interventions: The patient was treated by metronidazole and anti-TB treatment OUTCOMES:: The patient fully recovered without sequela. Conclusion: In conclusion there should be awareness of the possibility of P alactolyticus or MTB intracranial infections following tooth extraction.
Multidisciplinary Approach to Mandibular Ameloblastoma: A Case Report on Surgical and Prosthetic Management

Abstract:BACKGROUND Ameloblastoma is a locally aggressive, benign, odontogenic tumor. Reports suggest that the chances of recurrence of this tumor are high if treated with a conservative approach. Concordantly, surgical removal of the lesion along with the affected adjacent tissues and bone structure is recommended to reduce the chances of recurrence. Post-surgical prosthetic rehabilitation is advised to improve speech, mastication, and aesthetic appearance. This case report highlights the treatment and reconstruction challenges that maxillofacial surgeons and their teams face in managing cases of large ameloblastoma. CASE REPORT A 41-year-old Sudani man was referred for the management of a large ameloblastoma associated with the left border of the mandible. Management consisted of surgical removal of the affected mandible along with prosthetically preserving the mandible with grafts and screws. Histopathological, computed tomography, and incisional biopsy evaluation confirmed the presence of ameloblastoma. Postoperatively, no complications were reported. Six months postoperatively, no sign of recurrence was seen. The patient was referred to a surgeon for placement of an endosseous implant. CONCLUSIONS When dealing with large ameloblastoma, an interdisciplinary dental team is essential for improving the treatment results. This case highlights the importance of precise and timely primary care diagnosis and a collaborative approach to treatment. By embracing advancements in digital technologies, surgeons can enhance functional and aesthetic results, improving long-term quality of life.
Gingiva as the primary site of extrapulmonary tuberculosis: A rare case report with brief review of literature

Abstract:Multiple strains of Mycobacteria cause tuberculosis (TB), a chronic, specific infectious granulomatous disease. It mainly occurs with pulmonary involvement when compared to extrapulmonary involvement. Primary oral occurrence is uncommon and oral lesions are usually secondary to pulmonary involvement. When there are no active pulmonary clinical manifestations of TB, the diagnosis of the very rare entity of primary gingival TB poses a great challenge to clinicians. In this case report, we discuss a case of primary gingival TB in a 24-year-old lactating mother. This article briefs the onset and course of the lesion during pregnancy and postpartum, elaborates the pathway to diagnosis, various investigations performed and the regimen of antitubercular therapy for 6 months, followed by complete resolution of the lesion without recurrence. This report also describes the significance of considering TB as a differential diagnosis in oral lesions and the various diagnostic methods available. It also emphasizes the sole importance of histopathology in the early detection of the lesion and its management.
Actinomyces-induced Osteomyelitis of the Mandible - A Rare Disease?

Abstract:Background: Actinomyces species are commensal oral cavity flora that can cause jaw osteomyelitis. Osteomyelitis of the jaw by Actinomyces is rare, and its presentation can be confused with many different pathologies. Case presentation: This is the case of a 61-year-old female with breast cancer and on chemotherapy as well as non-invasive carcinoma of the tongue who initially presented to the dentist with white spots in the right mandible near the incisors associated with right mandible pain and swelling. Actinomyces-induced osteomyelitis of the mandible was diagnosed. The patient was treated with penicillin V for 6 weeks along with a course of hyperbaric oxygen therapy, which resulted in the complete resolution of the infection. Conclusion: In summary, jaw osteomyelitis caused by Actinomyces should always be part of the differential diagnosis; as these organisms are commensal flora. The symptoms manifested are non-specific, and such a diagnosis could be easily missed, resulting in delay of care and disease progression.
Ameloblastic Fibro-odontoma - A Case Report of Two Uncommon Cases

Abstract:Rationale: Ameloblastic fibro-odontoma (AFO) is a rare mixed odontogenic tumour with a preferential location in the posterior mandible and with a variety of radiological aspects. We report two clinical cases of AFO in two rare locations and with unusual radiological aspects. Patient concerns: The first patient is a 37-year-old female with an asymptomatic intraoral swelling located in the anterior mandibular. The second patient is a 16-year-old girl with a chief complaint of missing maxillary canine. Diagnosis: Both cases were diagnosed with AFO. Treatment: For the first patient, a biopsy was performed, and for the second one, the lesion was surgically excised. Outcomes: The first patient is under regular surveillance and the other was followed up for one year without any evidence of recurrences. Take-away lessons: Despite many efforts, the nature, histology and therapy of these lesions remain very confusing.
Characterization of an outbreak of tetM-containing Neisseria gonorrhoeae in Argentina

Abstract:Phenotypic and molecular characterization of an outbreak of 9 Neisseria gonorrhoeae (NG) isolates exhibiting high-level plasmid mediated resistance to penicillin and tetracycline (PP-TRNG) that took place in Tandil, Argentina between February and April 1995. Comparison with the patterns of the 3 PP-TRNG strains previously isolated were made. We determined the following markers for each strain: antimicrobial susceptibility, serogroup, auxotype, plasmid profile, presence of tet M determinant and restriction pattern of the tetM-containing plasmid. Antimicrobial tests values were: tetracycline disk diameter 12-14 mm, minimum inhibitory concentration (MIC) 32 mug/ml; penicillin disk diameter 6 mm, MIC 32 mug/ml and sensitive by both methods to spectinomycin, cefuroxime, ceftriaxone and ciprofloxacin. All isolates were of the same serogroup (WI). Ten of the strains, including the 9 from Tandil outbreak, were arginine-requiring, while the other 2 were methionine and arginine-requiring. All of them demonstrate the same plasmid profile (2.6, 3.2, 25.2 MDa). They were positive for the tetM determinant and the restriction analysis identified it as a Dutch-type plasmid. In spite of the temporal and geographical dispersion, PP-TRNG strains in Argentina seem to be highly homogeneous in terms of antimicrobial susceptibility, serogroup, plasmid profiles and even auxotype.
Extraspinal osteoarticular tuberculosis: A diagnostic and therapeutic challenge

Abstract:Tuberculosis (TB) is a major cause of morbidity and mortality, representing a challenge for health-care providers worldwide. Extraspinal osteoarticular tuberculosis (ESOTB) represents a rare location of TB. We aim to describe ESOTB focusing on clinical, diagnostic, and therapeutic characteristics of this entity. We report a retrospective case series of patients diagnosed with ESOTB, treated, and followed up between 2015 and 2022. The diagnosis was based either on bacteriological results (culture and polymerase chain reaction [PCR]) or histological analysis. Five patients with confirmed ESOTB, three women and two men, with a mean age of 46.4 (16-72), were enrolled in study. The affected sites were the elbow (one case), the shoulder (one case), the greater trochanter (one case), the second metatarsal (one case), and the distal interphalangeal joint (one case). The mean delay to diagnosis was 5.8 months (3-10)]. The most common symptoms on presentation were pain (all cases), swelling (all cases), and limited joint range of motion (all cases). One case presented with a draining sinus (20%). Radiological findings were soft tissue swelling (two cases) and periarticular bone destruction (all cases). Four patients presented with pathognomonic histology. PCR was performed in two patients and was positive in both of them. All cases were cured after 9-12 months of oral TB treatment without relapse for the 12-18 months of follow-up. Only one patient underwent surgery for abscess drainage to gain local infection control. ESOTB is a mysterious condition that must not be overlooked and should be suspected in cases of long-standing bone and joint pain and swelling.
Silent Jaw Expansion: A Rare Case of Posterior Maxillary Ameloblastoma

Abstract:Ameloblastoma is a rare odontogenic tumour that develops from the epithelial remains of the dental lamina. It is a benign but locally aggressive neoplasm that typically manifests as slow-growing tumours in the jaw, with posterior maxilla ameloblastoma being the rarer occurrence compared to mandibular ameloblastoma. This case report is about a 46-year-old Malay man who presented with a two-month history of left palatal and cheek swelling, along with symptoms such as left eye blurring, nasal block, and dysphagia. Imaging revealed a massive cystic lesion in the left maxillary sinus, which was diagnosed as a multicystic ameloblastoma after biopsy. The patient underwent surgical resection with a partial maxillectomy and was treated postoperatively with antibiotics, steroids, and regular follow-up appointments. The histopathological examination confirmed the diagnosis, and the patient was given a dental obturator to cover the defect intraorally. He is currently under annual surveillance with no signs or symptoms of recurrence.
