Abstract:There are close functional and anatomical relationships between cranial nerves V and VII in both their sensory and motor divisions. Sensation on the face is innervated by the trigeminal nerves (V) as are the muscles of mastication, but the muscles of facial expression are innervated mainly by the facial nerve (VII) as is the sensation of taste. This article briefly reviews the anatomy of these cranial nerves, disorders of these nerves that are of particular importance to psychiatry, and some considerations for differential diagnosis.
Influence of Lenslet Configuration on Short-Term Visual Performance in Myopia Control Spectacle Lenses

Abstract:Purpose: This study aimed to evaluate short-term visual performance and optical quality of three different lenslet configurations on myopia control spectacle lenses. Materials and Methods: This study utilized a cross-over design. Distance visual acuity (VA) was measured in 50 myopic children; contrast sensitivity (CS) was measured in 36 myopic children. For each test, four spectacle lenses were evaluated in a random order: single-vision lens (SVL), lens with concentric rings of highly aspherical lenslets (HAL), lens with concentric rings of slightly aspherical lenslets (SAL), and lens with honeycomb configuration of spherical lenslets (HC). The modulation transfer function (MTF) and MTF area (MTFa) were used to determine optical quality. All tests were performed monocularly on the right eye with full correction. Results: HAL and SAL had larger MTFa than HC. VA in lenses with lenslets was significantly reduced compared to SVL (all p < 0.01). The reduction in VA was worse with HC than with SAL ( p = 0.02) and HAL ( p = 0.03); no effect of lenslet asphericity was found ( p > 0.05). VA changes induced by lenslets showed no correlation with spherical equivalent refraction (all p > 0.05) and were weakly positively associated with age for SAL ( r = 0.36, p = 0.01) and HC ( r = 0.31, p = 0.03), but not for HAL ( p = 0.30). The area under the log contrast sensitivity function (AULCSF) decreased with HAL and HC (all p < 0.001) in all illumination levels, and AULCSF with HAL was higher than that with HC in a photopic condition (1.17 ± 0.10 vs. 1.10 ± 0.13, p = 0.0004). The presence of lenslets did not affect CS at 3 cycles per degree (cpd) ( p = 0.80). At 6 to 18 cpd, CS was significantly reduced by HAL and HC (all p < 0.05), but not SAL ( p > 0.05) compared to SVL. At high spatial frequencies (>12 cpd) both SAL and HAL reduced CS significantly less than HC (all p < 0.01). Conclusion: Short-term visual performance was minimally impaired by looking through the lenslet structure of myopia control spectacle lenses. Concentric rings with aspherical lenslets had a significantly lower impact on both VA and CS than honeycomb configuration with spherical lenslets.

Peripheral myopic defocus signal affects the efficiency of visual information processing in myopic children

Abstract:Purpose: Spectacle lenses with peripheral lenslets have shown promise for myopia control by providing peripheral myopic defocus signals. Here, we aimed to investigate the impact of prolonged exposure (>6 months) to peripheral myopic defocus on visual information processing in myopic children. Methods: The study included 30 myopic children who habitually wore spectacle lenses with highly aspherical lenslets (HAL group) and 34 children who habitually wore single-vision (SV group) spectacles. The quick contrast sensitivity function (qCSF) was used to measure contrast sensitivity (CS) under conditions of no or high noise. Both groups were tested with HAL and SV lenses. The perceptual template model was utilised to fit the contrast sensitivity function (CSF) and determine differences in information processing efficiency through internal additive noise ( N add ) and perceptual template gain (β). Results: The areas under the log CSF in the SV group were significantly higher than for the HAL group in both zero-noise conditions with the SV test lens (p = 0.03) and high-noise conditions with the HAL test lens (p = 0.02). For 2 cycle per degree (cpd) stimuli, β was significantly higher in the SV group with the HAL test lens than in the HAL group (p = 0.02), while there was a trend towards a significant difference in β for 6 cpd stimuli (p = 0.07). However, there were no significant differences in N add between the two groups, with or without noise interference. Conclusion: The reduced CS observed in myopic children wearing HAL lenses for 6 months or more may be due to decreased β. This suggests that prolonged use of spectacle lenses with peripheral myopic defocus signals may compromise the central visual system's ability to process additional external noise, resulting in decreased efficiency in visual information processing.
One-year myopia control efficacy of spectacle lenses with aspherical lenslets

Abstract:Aims To evaluate the 1-year efficacy of two new myopia control spectacle lenses with lenslets of different asphericity. Methods One hundred seventy schoolchildren aged 8–13 years with myopia of −0.75 D to −4.75 D were randomised to receive spectacle lenses with highly aspherical lenslets (HAL), spectacle lenses with slightly aspherical lenslets (SAL), or single-vision spectacle lenses (SVL). Cycloplegic autorefraction (spherical equivalent refraction (SER)), axial length (AL) and best-corrected visual acuity (BCVA) were measured at baseline and 6-month intervals. Adaptation and compliance questionnaires were administered during all visits. Results After 1 year, the mean changes in the SER (±SE) and AL (±SE) in the SVL group were −0.81±0.06 D and 0.36±0.02 mm. Compared with SVL, the myopia control efficacy measured using SER was 67% (difference of 0.53 D) for HAL and 41% (difference of 0.33 D) for SAL, and the efficacy measured using AL was 64% (difference of 0.23 mm) for HAL and 31% (difference of 0.11 mm) for SAL (all p<0.01). HAL resulted in significantly greater myopia control than SAL for SER (difference of 0.21 D, p<0.001) and AL (difference of 0.12 mm, p<0.001). The mean BCVA (−0.01±0.1 logMAR, p=0.22) and mean daily wearing time (13.2±2.6 hours, p=0.26) were similar among the three groups. All groups adapted to their lenses with no reported adverse events, complaints or discomfort. Conclusions Spectacle lenses with aspherical lenslets effectively slow myopia progression and axial elongation compared with SVL. Myopia control efficacy increased with lenslet asphericity. Abstract:Objective To investigate the effects of a series of specially designed spectacle lenses on peripheral refraction,peripheral clear vision field and subjective evaluation. Methods Three subjects (4 eyes) aged 22 to 31 years,with a refraction of -2. 0 D were enrolled. They underwent peripheral refraction,peripheral clear vision field measurement and subjective evaluation with 5 consecutive lenses in situ in a randomized manner,including regular aspheric lens,MyoVision lens,Stop lenses with respectively small,medium and large optical zones. Subjective evaluation included the degrees of comfort,clearness and acceptability both at distance and at near. Results All tested eyes had a relatively hyperopic peripheral refraction with any of the five lenses in situ,with the degree of hyperopia increasing towards the periphery. Peripheral refraction is the most hyperopic with regular aspheric lens and Stop lens with large optical zone,followed by Stop lens with medium optical zone. The least peripheral hyperopia was seen in eyes with MyoVision lens and Stop lens with small optical zone. Peripheral clear vision field was largest with regular aspheric lens and Stop lens with large optical zone,moderate with Stop lens with medium optical zone,and smallest with MyoVision lens and Stop lens with small optical zone. Subjective evaluation at distance was the highest with regular aspheric lens, followed by Stop lens with large optical zone and medium optical zone,the lowest with MyoVision lens and Stop lens with small optical zone. Subjective evaluation at near was the highest with regular aspheric lens and Stop lens with large optical zone,followed by Stop lens with medium optical zone and MyoVision lens,the lowest with Stop lens with small optical zone. Conclusions Lenses with different mid-periphery additional designs differed in their effects on peripheral refraction,whose intensity was negatively correlated to adaptability with lenses in situ.

Abstract:Purpose: Spectacle lenses with highly aspherical lenslets (HAL) have been shown to effectively retard myopia progression in myopic children. This study aimed to investigate the impact of spectacle lenses with HAL on refractive and axial length (AL) changes in Chinese children with low amount of hyperopia. Design: Randomized controlled trial METHODS: : A total of 108 Chinese children aged 6.0 to 9.9 years and spherical equivalent refractive error (SERE) from 0.00 to +2.00 D were randomly allocated into two groups: the HAL group and the single vision spectacle lens (SVL) group. Cycloplegic refraction, AL, and uncorrected visual acuity (UCVA) was measured at baseline, three months, six months, and 12 months after lens dispensing. The duration of spectacle lens wear was monitored using a wearable device attached to the spectacle frame and by questionnaire logs provided by participants at each follow up visit. Results: The one-year SERE change was -0.19 [-0.32, 0.03] D and -0.23 [-0.36, 0.05] D in the SVL and HAL groups (P=0.883). The one-year AL elongation was 0.24 [0.18, 0.34] mm and 0.19 [0.12, 0.27] mm in the SVL and HAL groups (P=0.057). In the HAL group, changes in AL and SERE were significantly correlated to lens wearing time (P<0.001 and P=0.024, respectively). Participants in the HAL group who wore their lenses for more than 30 hours per week had significantly slower AL elongation (0.11 [0.05, 0.17] mm) compared to their SVL counterparts (0.27 [0.21, 0.33] mm) (P<0.001). Conclusion: Spectacle lenses with HAL significantly reduced AL elongation in low hyperopic children who wore lenses for over 30 hours per week. A dose-response relationship was evident with longer lens wearing time associated with less AL change.

Abstract:Purpose: To assess the visual impact of Diffusion Optics Technology™ 0.2 DOT lenses (SightGlass Vision Inc.) designed for myopia control on primary gaze. DOT spectacle lenses contain light scattering elements that scatter light as it passes through the lens which, in turn, reduces retinal image contrast. Methods: Fifty-one children (12.2 ± 1.3, range 10-14 years; 51% females) were randomly assigned to wear DOT spectacle (n = 27) or single vision lenses (n = 24) across six investigational sites in North America. Binocular high- and low-contrast distant visual acuities, near visual acuity, reading speed, contrast sensitivity, stereoacuity and glare were assessed in primary gaze after at least 3 years of wear, with the study 95% powered in all metrics to detect significant differences between the groups. Results: Mean binocular distance high-contrast (-0.09 ± 0.02 vs. -0.08 ± 0.02 logMAR, p = 0.81), low-contrast (0.05 ± 0.02 vs. 0.07 ± 0.02 logMAR, p = 0.52) and near visual acuity with glare sources (-0.06 ± 0.03 vs. -0.09 ± 0.03 logMAR, p = 0.32) were similar for DOT and single vision lens wearers, respectively. Contrast sensitivity was similar between children wearing DOT or single vision lenses across 11 of the 16 spatial frequencies (p > 0.05). Mean stereopsis was similar (p = 0.30) with the DOT lenses (33.2 ± 12.5″) and single vision lenses (38.1 ± 14.2″). Functional reading speed metrics were similar in both study groups, as was the objectively measured head tilt during reading (p > 0.05). The mean halo radius was 0.56° ± 0.17° with the DOT lenses compared with 0.50° ± 0.12° with single vision lenses (p = 0.02), but the statistically significant difference was smaller than the non-inferiority bound of 0.4°. Conclusion: Diffusion optics technology lenses provide a clinically equivalent visual experience to a standard single vision lens.

Abstract:Myopia has become an increasing health problem worldwide, particularly in some East Asian countries. This has led to many attempts to slow its progression in children. Many of these attempts have involved optical interventions, including blue- and red-light therapies, more outdoor time (increased illumination), orthokeratology (the overnight flattening of the central cornea to make optical correction unnecessary during the day), spectacle lenses, and contact lenses. The theoretical justifications of the last two include (1) that children who go on to develop myopia or already have myopia have poor accommodation response 1 so that the consequent hyperopic defocus with near vision tasks stimulates eye growth 2 and (2) that a relative hyperopic peripheral refraction is the cause of eye growth. The second of these theories is the basis for many of the available treatment options and is supported by animal studies. 3 The foundation of this theory is dubious, 4,5 however, and there is clinical evidence that relative peripheral hyperopia is purely a consequence of the development of myopia and not the other way around. 6–8 – 8 Nevertheless, correction of the relative peripheral hyperopia may still be of benefit, and recent lenses designed in accordance with the theory have established clinical benefits in slowing myopia progression. 9

Optical characterisation of two novel myopia control spectacle lenses

Abstract:Purpose: To quantify the amount of myopic defocus, contrast modulation and other optical characteristics of two novel spectacle lenses (MiYOSMART by Hoya and Stellest by Essilor) with the inclusion of lenslets in their designs were investigated computationally and experimentally. This paper examined the hypothesis that despite the non-coaxial nature of the optics, image degradation will exist due to the fragmented nature of the base optic when imaging through the lens regions populated by lenslets. Methods: Optical power was evaluated by computing wavefront vergence and curvature from wavefront slope measured with the Optocraft aberrometer within 1.0 and 6.0 mm apertures across MiYOSMART hexagons and Stellest rings. Point-spread functions (PSFs) were computed using physical (wave) optics and geometrical ray optics principles, and compared with experimental measurements using a 4f optical system. Simulated retinal images and modulation transfer functions (MTFs) were computed from PSF-derived optical transfer functions (OTFs). Results: Mean lenslet power in MiYOSMART was +3.95 ± 0.10 D through the hexagons and +6.00 ± 0.15 D in Stellest in rings 1-5 and decreased by 0.42 D/ring reaching 3.50 D in the final one. Stellest lenslets included up to -0.015 microns of primary spherical aberration. PSFs and retinal images revealed simultaneous contributions of the base optic and lenslets. MTFs showed a decrease in contrast at low (1-10 c/deg) spatial frequencies (SFs) comparable to 0.25 D of defocus, and retention of diminished levels of contrast at higher SFs. Conclusions: Varying sagittal power and consistent curvature power across the lenslets is an identifying signature of the novel non-coaxial lens design included in both spectacle lenses. Lenslet array structure itself plays a significant role in determining image characteristics. For both lenses, the blur created by the fragmented base optic contributes to the image quality. The reduced MTFs over a wide range of spatial frequencies result in lowered image contrast.
Myopia Control Efficacy of Spectacle Lenses with Aspherical Lenslets: Results of a 3-year Follow-up Study

Abstract:Purpose To investigate myopia control efficacy in children who continued wearing spectacle lenses with highly aspherical lenslets (HAL) or switched from spectacle lenses with slightly aspherical lenslets (SAL) and single-vision spectacle lenses (SVL) to HAL for one year following a 2-year myopia control trial. Design This was a one-year extension from a randomized clinical trial. Methods Of the 54 children who had worn HAL for two years, 52 continued wearing HAL (HAL1 group), and of the 53 and 51 children who had originally worn SAL or SVL, 51 and 48 switched to wearing HAL (HAL2 and HAL3 groups) in the 3 rd year, respectively. A new SVL (nSVL) group of 56 children was recruited, matched for age, sex, cycloplegic spherical equivalent refraction (SER), and axial length (AL) of the HAL3 group at extension baseline, and used for a comparison of third-year changes. SER and AL were measured every 6 months in the 3 rd year. Main Outcome Measures During the 3rd year, the mean (SE) myopia progression in the nSVL group was -0.56 (0.05) D. Compared with nSVL, the changes in SER were less in HAL1 (-0.38[0.05] D, P=0.02), HAL2 (-0.36[0.06] D, P=0.01) and HAL3 (-0.33[0.06] D, P=0.005). The mean (SE) AL elongation in the nSVL group was 0.28(0.02) mm. Compared with nSVL, the elongation in AL was less in HAL1 (0.17[0.02] mm, P<0.001), HAL2 (0.18[0.02] mm, P<0.001) and HAL3 (0.14[0.02] mm, P 0.05) in the third year. Conclusions Myopia control efficacy has remained in children who wore HAL in the previous 2 years. Children who switched from SAL or SVL to HAL in the 3rd year had slower myopia progression and axial elongation than that in control group.

Abstract:Purpose. The purpose of this study was to determine and compare visual quality indicators in eyes wearing multifocal center-distance, and dual-focus soft contact lenses that are specifically designed for the purpose of myopia progression management, as well as single-vision soft contact lenses. Material and Methods. Visual quality indicators of 34 eyes of healthy, young myopic adults ages 20 to 35 were determined. Non-cycloplegic measurements were conducted without contact lenses (baseline) and while subjects wore soft multi- focal center-distance soft contact lenses (ArtMost SoftOK SMR and SEED 1-day Pure UP Multistage daily disposable), dual-focus soft contact lenses (CooperVision MiSight 1 Day), and single-vision soft contact lenses (iLens Aqua Bi-weekly). In this study, the lenses were referred to as SoftOK, Pure UP, MiSight, and iLens. The measured and analyzed visual quality indicators included high-contrast distance and near visual acuity, stereopsis, accommodative amplitude, accommo- dative response, spherical aberration, and horizontal coma. Results. High-contrast distance and near visual acuities, accommodative responses, and stereopsis values achieved with the SoftOK, Pure UP, and MiSight contact lenses werecomparable to those achieved with the iLens single-vision soft contact lenses and to baseline values. Accommodative amplitude was slightly reduced with the MiSight contact lens and spherical aberration was significantly higher with the SoftOK lens compared to the other lenses. Both findings had statistical significance (P = 0.047 and P < 0.001, respec- tively). Horizontal coma, most likely resulting from slight lens decentration, was higher and more variable with the SoftOK lens compared to the other lenses, although no statistical significance was determined. Conclusion. Dual-focus and multifocal center-distance soft contact lenses allow for good visual quality, especially clear vision at distance and near. Small differences in accommoda- tive amplitude and spherical aberration were found between lens designs, however the clinical impact requires further investigation. Abstract:Purpose:To investigate the impact of vergence dysfunction on myopia progression in children with Defocus incorporated multiple segments (DIMS) spectacle lenses.Patients and Methods:We retrospectively enrolled children prescribed DIMS spectacle lenses to slow myopic progression. Baseline vergence dysfunction was determined according to phoria at distance and near. Axial length (AL) measurement and cycloplegic subjective refraction were performed before fitting the lenses and at six-month and one-year follow-ups. The six-month and one-year AL and spherical equivalent (SE) change from baseline were calculated and compared in subgroups stratified with the type of vergence dysfunction.Results:Two hundred and ninety-two myopic children were included. Significant AL elongation and SE progression were observed at six months and one year (P < 0.05 for all comparisons). Multiple regression demonstrated that AL elongation at six months (P < 0.001) and one year (P < 0.001) was negatively correlated with age, and SE progression at six months was associated with age (P = 0.002). The AL elongation at six months in children with convergence excess was significantly greater than in normal myopic subjects (P = 0.011) and subjects with convergence insufficiency (P = 0.008), divergence excess (P = 0.007), divergence insufficiency (P = 0.024) and basic esophoria (P = 0.048) at six months.Conclusion:The present research demonstrated that vergence dysfunction influences myopia progression for myopic children with DIMS, and the children with convergence excess suffer from the greatest myopia progression among different types of vergence dysfunction.

Methods: Children aged 5 to 12 with myopia up to −0.50 D, astigmatism and anisometropia under 1.50 D, and corrected visual acuity over 20/20 were randomized to either the study group (MPDL) or control group (SVL). The myopia progression was evaluated by measuring axial length (AL) growth (IOL Master; Zeiss) over a period of one year. Results: Ninety-two subjects were recruited. Forty-six children were randomly assigned to the control group, and 46 to the study group. In total, 83 children completed the clinical trial, with a mean age of 10.81 [9.53–11.92] years, among which 59.04% were female. After one year of treatment, there was less AL elongation in the study group compared to the control group (0.16 ± 0.16 mm vs. 0.24 ± 0.16 mm, p = 0.034). Conclusions: The MPDL significantly reduced the absolute growth of AL by 39% (p = 0.014) and relative growth of AL by 37.3% (p = 0.012) after 12 months in comparison to the control group in a Spanish population.

Abstract:Purpose: The purpose of this study was to compare the differences in myopic control effects between orthokeratology (OK) contact lenses and defocus incorporated multiple segments (DIMS) spectacle lenses. Methods: A retrospective cohort study was conducted that included patients who had received OK lens, DIMS spectacle lens or single-vision spectacle treatments. A total of 54 eyes from 27 individuals, 38 eyes from 19 individuals and 42 eyes from 21 individuals were enrolled into the OK lens, DIMS and control groups, respectively. The primary outcomes were the changes in the spherical equivalent refraction (SER) and axial length (AXL) among the groups. A repeated-measure ANCOVA was adopted to calculate the SER progression and AXL elongation of the OK lens group compared with the DIMS group. Results: The difference in the SER progression was clinically non-significant in the OK lens group compared with the DIMS and control groups (P = 0.001). The total AXL elongation results were similar between the OK lens and DIMS groups, but these were lower than in the control group (P = 0.005). The repeated-measure ANCOVA revealed that the SER progression difference during the study interval was clinically non-significant in the OK lens group when compared with the DIMS group (P = 0.028). The AXL elongation results between the OK lens and DIMS populations did not illustrate a significant difference (P = 0.607). In a subgroup analysis of moderate astigmatism, better AXL control was observed in the DIMS subgroup compared with the OK lens subgroup (P = 0.016). Conclusions: The OK lens demonstrated a clinically non-significant effect on the SER and AXL controls compared with the DIMS spectacle lens.

Abstract:The rising prevalence of myopia among children and adolescents necessitates effective interventions to mitigate long-term risks, such as retinal detachment and macular degeneration. Traditional approaches like undercorrection with single-vision lenses have proven ineffective. Recent research underscores the significance of peripheral vision in managing myopia progression. While hyperopic defocus in the peripheral retina may encourage axial elongation, multifocal lenses designed to induce peripheral myopic defocus can potentially curb this elongation. Randomized controlled trials (RCTs) have demonstrated that spectacle lenses with peripheral modifications, such as highly aspherical lenslets (HAL) or defocus incorporated multiple segments (DIMS), reduce myopia progression and axial elongation over 2 years. Similarly, multifocal contact lenses, especially those with high-add power (+ 2.50 D), have shown comparable reductions over 3 years. Safety is a critical consideration in myopia control. Multifocal spectacle lenses with peripheral plus designs are generally well tolerated, with only transient side effects, like dizziness and blurred vision. Soft multifocal contact lenses also exhibit a favorable safety profile, with low rates of serious adverse events. Thorough monitoring and reporting in these studies are essential to provide assurance about the safety of these interventions for children. Future research should aim to include more diverse populations in order to ensure the generalizability of findings across various demographic groups. Additionally, real-world data would offer valuable insights into the performance of these treatments in everyday scenarios. Observational studies, less influenced by industry sponsorship, could further validate these findings. Lastly, the clinical significance of the observed effects should be critically assessed to confirm that the reported benefits lead to meaningful reductions in myopia progression. Die zunehmende Prävalenz der Myopie bei Kindern und Jugendlichen erfordert effektive Maßnahmen, um langfristige Risiken wie Netzhautablösung und Makuladegeneration zu verringern. Herkömmliche Ansätze wie die Unterkorrektur (myoper Defokus) mit Einstärkengläsern haben sich als ineffektiv erwiesen. Aktuelle Forschungsergebnisse betonen die Bedeutung des peripheren Sehens, um das Fortschreiten der Myopie zu stoppen: Es wird angenommen, dass ein hyperoper Defokus auf der peripheren Netzhaut das axiale Längenwachstum des Auges fördert. Die Unterkorrektur der Myopie mit multifokalen Optiken, die den peripheren hyperopen Defokus reduzieren, wird daher als potenzielle Methode betrachtet, um das Fortschreiten der Myopie zu verlangsamen. Randomisierte kontrollierte Studien (RCTs) haben bereits gezeigt, dass multifokale Brillengläser, die einen peripheren myopischen Defokus erzeugen, die Myopieprogression und das axiale Längenwachstum über einen Zeitraum von 2 Jahren hemmen können. Auch weiche multifokale Kontaktlinsen weisen ein gutes Sicherheitsprofil mit nur sehr geringen Raten an schwerwiegenden unerwünschten Ereignissen auf. Zukünftige Studien zu multifokalen Optiken sollten jedoch eine breite Population einbeziehen, um die Übertragbarkeit der Ergebnisse auf verschiedene demografische Gruppen zu gewährleisten. Zudem könnten Real-World-Daten wertvolle Einblicke in die Wirksamkeit der Behandlungen im Alltag liefern. Solche Beobachtungsstudien wären auch weniger durch industrielles Sponsoring beeinflusst. Schließlich sollte die klinische Relevanz der Effekte sorgfältig bewertet werden, um sicherzustellen, dass der beschriebene Nutzen auch tatsächlich von klinischer Bedeutung ist. Electrophysiological response of myopic young adults with a novel ophthalmic lens for the control of myopia progression

Abstract:Aims/Purpose: To evaluate the changes in the electrophysiological response of the retina induced by a novel perifocal ophthalmic lens designed for myopia control. Methods: This experimental cross‐sectional study, evaluated eleven myopic participants (24.9 ± 4.5 years) with refractive errors lower than 6.00 D (spherical equivalent of −2.40 ± 1.32 D in the right eye and − 2.24 ± 1.56 D in the left eye). Refraction, best‐corrected visual acuity, and axial length were obtained at baseline. The global flash multifocal electroretinogram (gf‐mfERG) was assessed for the sensorial dominant eye with the RETI‐port/scan21 (Roland Consult, Wiesbaden, Germany). The gf‐mfERG recordings were performed with a stimulus array pattern of 61‐scaled hexagons displayed on a 19‐inch RGB monitor at a 33 cm distance (optically corrected). The retina was divided into three groups ‐ the fovea (that included concentric ring 1), the parafovea (rings 2 and 3) and the peripheral retina (rings 4 and 5). The peak‐time (in milliseconds, ms), the amplitude (in nano‐volts, nV) and the mean response density (nV/deg2) of the direct and implicit components (DC and IC, respectively) were analysed and compared between control (monofocal contact lenses) and test conditions (perifocal lenses). These lenses have an add power of +2.50 D on the temporal side, and + 2.00 D on the nasal side, intended to change the peripheral refraction. Results: No differences were found in the DC and IC peak time between the control and test measurements, for the fovea, parafovea and peripheral retina. The mean response density of the DC was higher with the perifocal lens for the perifovea ( p ‐value = 0.033) and for the peripheral retina ( p ‐value = 0.026). For the IC, the mean response density was also higher with the perifocal lens, only for the parafovea ( p ‐value = 0.003). Conclusions: Perifocal lenses for myopia control upregulate the parafoveal and peripheral retinal electrical activity.

Abstract:Background. This study tested short-term axial length changes with Lenstar using a simple peripheral plus-add spectacle design. Methods. The subjects for this study were current users of monofocal glasses or contact lenses, aged 18–25 years, with myopic spherical refractions ranging from −1.00 to −5.00 diopters in both eyes. This study tested subjects while using a pair of special defocus spectacles that possess a central zone with the distance myopic correction and a peripheral zone with +3.50 added diopters. The procedure consisted of reading an online book with black letters on white background on a desktop computer, in two periods—one with the usual spectacles and the second with special defocus spectacles. Before and after these periods, 10 axial length measurements of the right eye were made with Lenstar and averaged up to three decimal points. Results. Seventeen subjects (thirteen female and four male; mean age 22.3 ± 5.5 years) participated in this study. The mean spherical equivalent of their right eye was −2.31 ± 1.06 diopters. There was a significant difference of +8.1 microns which increased the axial length from baseline when reading the usual prescription levels during the first 40 min period. When subjects read in the same situation with the defocus spectacles, the axial length significantly returned to baseline measurements, which were shortened by −10.6 microns. Conclusions. The decrease in axial length with these spectacles might indicate an effect on axial elongation signals from the choroid. These spectacles could be tested for myopia control purposes.

Abstract:Objectives: Center-distance multifocal contact lenses (MFCLs) are used to slow myopia progression. We examined the effect of two MFCLs on intraocular straylight values in myopic individuals. Methods: Twenty-five young myopic adults were enrolled and were fit with three contact lenses (Biofinity sphere, Biofinity Multifocal, and NaturalVue Multifocal) in a random order over two study visits. Pupil size (NeurOptics VIP-300, Laguna Hills, CA) and contact lens centration were measured. Right eye intraocular straylight measurements were collected (OCULUS C-Quant; Wetzlar, Germany) and compared with a spectacle trial lens. Log straylight (LogSL) values and straylight residuals were analyzed using repeated-measures analyses of variance with Tukey-corrected post hoc t -tests. Results: The mean participant age (±SD) was 24.1±1.5 years, and right eye spherical equivalent refractive error was −3.38±1.53 DS. There was no difference in mesopic pupil size between visits ( P =0.68) and no difference in contact lens centration between lenses ( P =0.99). LogSL values differed by lens type ( P =0.004). LogSL with the spectacle trial lens was significantly greater than with each contact lens type (all P <0.05), but there were no significant differences in LogSL between the three contact lenses (all P >0.05). There was no difference between the three contact lens designs for straylight residuals ( P =0.33). Conclusions: Measured intraocular straylight for both MFCLs was not different than with a spherical soft contact lens. A significant increase in intraocular straylight with spectacle trial lens correction was observed compared with all contact lenses.
