Abstract:BACKGROUND:Fundus changes associated with high myopia (HM) may mask those associated with primary open-angle glaucoma (POAG). This study aim to determine the characteristics of RNFL thickness changes in patients with both POAG and HM and compare these to changes in patients with only HM. The diagnostic capabilities of both OCT and GDxVCC in this subset of patients are also evaluated.METHODS:Twenty-two eyes with POAG and HM (spherical equivalent (SE) between -6.0 and -12.0 D) were evaluated, and 22 eyes with HM were used for comparison. Characteristic retinal nerve fiber layer (RNFL) thickness profiles in patients with POAG and HM were examined using optical coherence tomography (OCT) and scanning laser polarimetry with variable corneal compensation (GDxVCC), and the diagnostic capabilities of these imaging modalities were compared. RNFL parameters evaluated included superior average (Savg-GDx), inferior average (Iavg-GDx), temporal-superior-nasal- inferior-temporal (TSNIT) average, and nerve fiber indicator (NFI) on GDxVCC and superior average (Savg-OCT), inferior average (Iavg-OCT), nasal average (Navg-OCT), temporal average (Tavg-OCT), and average thickness (AvgThick-OCT) on OCT (fast RNFL scan). Visual field testing was performed and defects were evaluated using mean defect (MD) and pattern standard deviation (PSD).RESULTS:The RNFL parameters (P < 0.05) significantly different between groups included Savg-GDx, Iavg-GDx, TSNIT average, NFI, Savg-OCT, Iavg-OCT, Tavg-OCT, and AvgThick-OCT. Significant correlations existed between TSNIT average and AvgThick-OCT (r = 0.778), TSNIT average and MD (r = 0.749), AvgThick-OCT and MD (r = 0.647), TSNIT average and PSD (r = -0.756), and AvgThick-OCT and PSD (r = -0.784). The area under the receiver operating characteristic curve (AUROC) values of TSNIT average, Savg-GDx, Iavg-GDx, NFI, Savg-OCT, Iavg-OCT, Navg-OCT, Tavg-OCT, and AvgThick-OCT were 0.947, 0.962, 0.973, 0.994, 0.909, 0.917, 0.511, 0.906, and 0.913, respectively. The NFI AUROC was the highest value.CONCLUSIONS:RNFL thickness was significantly lower in all but the nasal quadrant in patients with POAG and HM, compared to patients with only HM. Measurements with OCT and GDxVCC were well-correlated, and both modalities detected RNFL thickness changes. However, GDxVCC was better than OCT in detecting POAG in HM patients.
Diagnostic Capability of Different Morphological Parameters for Primary Open‐angle Glaucoma in the Chinese Population

Abstract:Background To assess the diagnostic capability of novel Bruch’s membrane opening (BMO)-based disc parameters, the BMO-minimum rim width (BMO-MRW) and the BMO-minimum rim area (BMO-MRA) in the Chinese population and compare them to the retinal nerve fiber layer (RNFL) from optical coherence tomography (OCT) and the rim area (RA) from the Heidelberg retinal tomograph-III (HRT-III). Methods In total, 200 eyes of 77 healthy and 123 primary open-angle glaucoma (POAG) subjects were included in this cross-sectional study. All participants underwent the visual field test and structural measurements by OCT and HRT-III. The areas under the receiver operating characteristic curves (AUCs) of different structural parameters were calculated to assess their diagnostic power and compared using the DeLong test. Results In populations with different characteristics, the BMO-MRW and BMO-MRA had better diagnostic power than the RA. In discriminating between all POAG subjects and healthy controls and between early-stage patients and controls, the global BMO-MRW had comparable AUCs with the RNFL, but the BMO-MRA had lower AUCs than the RNFL. In healthy subjects with macrodiscs, both the global and sectoral BMO-MRW were thinner than those in healthy subjects with normal disc size. The AUCs of BMO-MRA, BMO-MRW and RNFL in subjects with macrodiscs were comparable. Additionally, in the myopic population, the BMO-MRA and BMO-MRW had comparable AUCs with the RNFL. Conclusions The BMO-MRW had comparable diagnostic power with the RNFL, and compared with BMO-MRW, the BMO-MRA might have advantages in certain populations, such as macrodiscs. All OCT-derived parameters exceeded the RA in diagnostic capability.

Abstract:Purpose The aim of this study was to investigate changes in the retinal and choroidal thickness between high myopic amblyopia (HMA), low myopia (LM), moderate myopia (MM), high myopia (HM), and normal group (NG) using a spectral-domain optical coherence tomography (SD-OCT). Materials and Methods A total of 75 Chinese children (128 eyes; mean age 10.5 years) were recruited. Retinal thickness (RT) and choroidal thickness (CT) were measured at different locations including subfoveal (SF), and at 0.5 mm/1.0 mm/1.5 mm/2.0 mm/2.5 mm/3.0 mm to the fovea in superior, nasal, inferior, and temporal sectors using enhanced depth imaging (EDI) system of SD-OCT. Axial length (AL), best-corrected visual acuity (BCVA), and refraction errors were also collected. Results No significant differences were found in subfoveal retinal thickness (SFRT). Moreover, a significantly thinner subfoveal choroidal thickness (SFCT) was found in HMA compared to NG, LM, and MM, but not compared to HM. RT at 0.5 mm to fovea, HMA was significantly thinner compared to LM and MM in the three sectors (superior, inferior, and temporal). Nevertheless, no significant differences were found compared to NG and HM. CT at 0.5 mm to fovea, HMA was the significantly thinnest in all four sectors compared to NG, LM, and MM. RT at 1.0 mm/1.5 mm/2.0 mm/2.5 mm/3.0 mm to fovea, HMA was thinner compared to NG, LM, and MM. CT at 1.0 mm/1.5 mm/2.0 mm/2.5 mm/3.0 mm to fovea, HMA was thinner compared to NG, LM, and MM. At the superior and inferior sectors, HMA showed to be statistically thinner compared with HM. Moreover, SFCT in the HMA, HM, and NG were negatively correlated with AL. Conclusions Thinner retina and choroidal tissue appear to be related to HMA, and thus can be used as useful parameters for discovering the underlying mechanisms of the disease.

Distribution and Classification of Peripapillary Retinal Nerve Fiber Layer Thickness in Healthy Young Adults

Abstract:Purpose: To report the distribution of peripapillary retinal nerve fiber layer (RNFL) thickness in healthy young adults, investigate factors associated with RNFL thickness, and report the percentage of outside normal limits (ONL) and borderline (BL) RNFL thickness classifications based on the optical coherence tomography (OCT) manufacturer reference database.Methods: Participants of the Raine Study Generation 2 cohort (aged 18-22 years) underwent spectral domain OCT imaging with an RNFL circle scan. Eyes with inadequate scans or optic nerve pathology were excluded. Linear mixed models were used to analyze associations.Results: Data were available for 1288 participants (mean age, 20.0 years). Mean RNFL thicknesses in right and left eyes, respectively, were global = 100.5 µm, 100.3 µm (P = 0.03); temporal = 73.1 µm, 68.9 µm (P < 0.001); superotemporal = 140.6 µm, 136.3 µm (P < 0.001); superonasal = 104.9 µm, 115.1 µm (P < 0.001); nasal = 79.7 µm, 79.1 µm (P = 0.09); inferonasal = 109.8 µm, 111.5 µm (P < 0.001); and inferotemporal = 143.2 µm, 143.6 µm (P = 0.51). Longer axial length was associated with thinner RNFL globally, nasally, inferotemporally, superotemporally, superonasally, and inferonasally, as well as thicker RNFL temporally. The prevalence of ONL and BL classifications was generally higher than the expected rates of 1% and 4%, respectively, in temporal sectors and lower than expected in nasal sectors. The prevalence of global BL classifications was lower than expected (right eye, 2.3%; left eye, 2.6%).Conclusions: Measured RNFL thickness differs with axial length and between right and left eyes. More reference data are needed to better define the normal limits of RNFL variation in different populations.Translational Relevance: This study provides an improved understanding of normal variation in RNFL thickness in young adults.

Diagnostic Specificities of Retinal Nerve Fiber Layer, Optic Nerve Head, and Macular Ganglion Cell-Inner Plexiform Layer Measurements in Myopic Eyes

Abstract:PURPOSE: To evaluate and compare the diagnostic specificities of peripapillary retinal nerve fiber layer (RNFL) thickness, macular ganglion cell-inner plexiform layer (GC-IPL) thickness, and optic nerve head (ONH) measurements in nonglaucomatous myopic individuals.METHODS: In a prospective, cross-sectional study, participants underwent a complete ophthalmic examination, a screening automated visual field test, and axial length measurement. The study eye then underwent optic nerve head and macular scanning using spectral-domain optical coherence tomography (OCT) instrumentation to determine RNFL thickness, GC-IPL thickness, and ONH measurements. False-positive rates for each of the OCT-derived parameters, using predefined criteria for an abnormal test, were calculated. Comparative analysis was performed using the McNemar test.RESULTS: Data from 43 eligible subjects were analyzed. The mean age was 30±6.8 years (range, 22 to 50 y) with an average axial length of 25.26±1.21 mm (range, 23.06 to 29.07 mm) and mean spherical equivalent of -4.50±1.93 D (range, -1.00 to -9.00 D). The false-positive rate was higher when using RNFL parameters compared with both ONH (47% vs. 7%, respectively; P<0.001) and GC-IPL (47% vs. 26%, respectively; P=0.049) parameters. The false-positive rate was higher when using GC-IPL parameters, compared with ONH parameters (26% vs. 7%, respectively; P=0.039).CONCLUSIONS: Caution should be exercised when relying on OCT-derived RNFL and GC-IPL thickness values to diagnose glaucoma in myopic individuals. OCT-derived ONH parameters perform better than RNFL and GC-IPL parameters and may increase diagnostic specificity in this population.

Abstract:Purpose To seek the threshold value of Bruch’s membrane opening-minimum rim width (BMO-MRW) where visual field (VF) damage occurs in open-angle glaucoma (OAG) and explore whether there are structural differences between primary open-angle glaucoma (POAG) and normal-tension glaucoma (NTG). Methods We recruited 83 healthy and 106 glaucoma (49 with POAG and 57 with NTG) subjects for this study. All subjects underwent optical coherence tomography (OCT), BMO-MRW scans, and Humphrey visual field (VF) analyzer examination. Global and sectoral BMO-MRW was correlated with the corresponding VF according to the Garway-Heath map. Using a broken-stick statistical model, the structure–function relationship of VF values and BMO-MRW, the tipping point where VF defects were associated with a reduction in BMO-MRW and the slopes above and below the tipping point were determined and compared between POAG and NTG. Results The tipping point of global BMO-MRW for VF impairment was 234.38 μm, 228.09 μm, and 249.68 μm in the OAG, POAG, and NTG groups, respectively. The slope below the tipping point was significantly steeper than the slope above it in all quadrants of each group ( p < 0.001). The tipping point in NTG in the inferotemporal and nasal quadrants was smaller than that of POAG, especially in the inferotemporal quadrant. Conclusion In OAG, BMO-MRW loss seems to occur before the onset of perimetric impairment. Compared with POAG, NTG appears to have more severe rim damage, especially in the inferotemporal quadrant at the onset of detectable VF defects.
Compartmental analysis of three-dimensional choroidal vascularity and thickness of myopic eyes in young adults using SS-OCTA

Abstract:Purpose: We aimed to investigate the change of three-dimensional (3D) choroidal thickness (ChT), choroidal vessel volume (CVV), and choroidal vessel index (CVI) in young myopic adults using swept-source optical coherence tomography angiography (SS-OCTA) and compare the difference of these indicators in different quadrants of the macula and optic disc. Methods: A total of 248 eye samples from 135 participants were used in this cross-sectional study. Each participant underwent detailed history taking and ocular examinations. Based on axial length (AL), patients were divided into the emmetropia (EM) group, mild-myopia (MIM) group, moderate-myopia (MOM) group, and high-myopia (HM) group. 6 mm × 6 mm (1,024 × 1024 B-scans) SS-OCTA scans were performed centered on the fovea and optic disc. 3D ChT, CVV, and CVI were measured based on a built-in deep learning algorithm. Differences in ChT, CVV, and CVI were analyzed in different regions and different myopic groups. Results: Significant reduction in the global CVV were found in the HM group (1.930 ± 0.865) in comparison with the EM (3.486 ± 0.992), MIM (3.238 ± 1.033), and MOM (2.589 ± 1.083) groups ( p < 0.001). The global CVI was also lower in the HM group (0.258 ± 0.061) than in the EM (0.320 ± 0.055), MIM (0.320 ± 0.051), and MOM (0.286 ± 0.066) groups ( p < 0.001). The ChT was thinner in eyes with HM (242.753 ± 65.641) than in eyes with EM (377.532 ± 80.593), MIM (348.367 ± 78.191), or MOM (300.197 ± 87.175) ( p < 0.001). Compartmental analysis revealed that ChT, CVV, and CVI in the nasal quadrant of the macula and temporal and inferior quadrants of the optic disc were much lower than those in other quadrants ( p < 0.05). Correlation analyses found that ChT, CVV, and CVI were negatively correlated with AL and spherical equivalence. Conclusion: 3D ChT, CVV, and CVI gradually decreased as the degree of myopia increased. The changes were more dramatic on the nasal side of the macula and the temporal and inferior sides of the optic disc. These findings demonstrated the 3D choroidal change and highlighted the papillo-macular bundle as a sensitive region in myopic development.

Comparison of two retinal nerve fibre layer thickness measurement patterns of RTvue optical coherence tomography

Abstract:Background: To evaluate the agreement and repeatability between operator-dependent centring and automatic centring retinal nerve fibre layer thickness measurement patterns (RNFL3.45 and NHM4) of RTvue optical coherence tomography in normal and glaucomatous eyes. Design: Cross-sectional observational study. Participants: A total of 153 eyes from 149 normal subjects and subjects with glaucoma were analysed. Methods: The retinal nerve fibre layer thickness was measured using RNFL3.45 and NHM4 three times on the same day to determine the repeatability and agreement between the two scan patterns. Main outcome measures: Student's paired t-testing, intra-class correlation coefficient, coefficient of variation, test-retest viability, Pearson's correlation coefficient and Bland-Altman analysis of retinal nerve fibre layer thickness measurements. Results: The difference between RNFL3.45 and NHM4 measurements was statistically significant by paired t-testing (P=0.003) only in severe glaucoma group. The Pearson's correlation test showed a high degree of correlation of the mean retinal nerve fibre layer thickness (r=0.949). Bland-Altman plots showed that the differences between RNFL3.45 and NHM4 were smaller at thicker retinal nerve fibre layer values, but larger at thinner retinal nerve fibre layer values. The intra-class correlation coefficient for RNFL3.45 (and lower 95% confidence interval) in normal and glaucomatous eyes was 0.990 (0.985 confidence interval) and 0.997 (0.995 confidence interval), respectively. The intra-class correlation coefficient for NHM4 in normal and glaucomatous eyes was 0.989 (0.983 confidence interval) and 0.995 (0.992 confidence interval), respectively. The test-retest variability for RNFL3.45 and NHM4 ranged from 3.98 to 9.75µm in normal eyes, and from 4.10 to 13.34µm in glaucomatous eyes. Conclusion: Measurements of retinal nerve fibre layer thickness by RNFL3.45 and NHM4 are in good agreement.
Value of optical coherence tomography measurement of macular thickness and optic disc parameters for glaucoma screening in patients with high myopia

Abstract:BACKGROUND: The basic method of glaucoma diagnosis is visual field examination, however, in patients with high myopia, the diagnosis of glaucoma is difficult.AIM: To explore the value of optical coherence tomography (OCT) for measuring optic disc parameters and macular thickness as a screening tool for glaucoma in patients with high myopia.METHODS: Visual values (contrast sensitivity, color vision, and best-corrected visual acuity) in three groups, patients with high myopia in Group A, patients with high myopia and glaucoma in Group B, and patients with high myopia suspicious for glaucoma in Group C, were compared. Optic disc parameters, retinal nerve fiber layer (RNFL) thickness, and ganglion cell layer (GCC) thickness were measured using OCT technology and used to compare the peri-optic disc vascular density of the patients and generate receiver operator characteristic (ROC) test performance curves of the RNFL and GCC for high myopia and glaucoma.RESULTS: Of a total of 98 patients admitted to our hospital from May 2018 to March 2022, totaling 196 eyes in the study, 30 patients with 60 eyes were included in Group A, 33 patients with 66 eyes were included in Group B, and 35 patients with 70 eyes were included in Group C. Data were processed for Groups A and B to analyze the efficacy of RNFL and GCC measures in distinguishing high myopia from high myopia with glaucoma. The area under the ROC curve was greater than 0.7, indicating an acceptable diagnostic value.CONCLUSION: The value of OCT measurement of RNFL and GCC thickness in diagnosing glaucoma in patients with high myopia and suspected glaucoma is worthy of development for clinical use.

Comparison of Retinal Nerve Fiber Layer Thickness Between Mild to Moderately Myopic Eyes with Normal Eyes

Abstract:Aim: To access the difference between the mean thickness of the retinal nerve fiber layer (RNFL) in myopic eyes (down to -6.00 D) and the normal eye. Study Design: An Observational and Descriptive study. Place duration: In the department of Ophthalmology, Al Shifa Trust Eye Hospital, Rawalpindi for six-months duration from June 2020 to November 2020. Methods: In this study, the mean comparison of RNFL among 70 myopic eyes (down to -6.00 D) and 70 normal eyes of the same age was done. 15-45 years was the patients’ age range. A comprehensive eye exam was performed and thickness of RNFL was evaluated by optical coherence tomography. Data analysis was done with t-test for independent samples using SPSS 21.0; p <0.05 as the significant value. Results: The RNFL mean variation between both groups was 5.901 μm (SE: 1.930). The mean RNFL thickness in the group of people with myopia was 96.31 ± 11.121 μm with (SE: 1.310) and mean age was 28.32 ± 7.452 years. The mean thickness of RNFL was 94.210 ± 9.521 μm in the right eye and 99.175 ± 11.142 μm in the left eye. The RNFL mean score of thickness for eyes with myopia distributed normally as p <0.04. The RNFL mean score of thickness for normal group was (102.81 ± 11.48 μm) with (SE: 1.40), and the mean thickness of RNFL evaluated in healthy eyes did not show a normal distribution as P <0.20. This study outcomes exhibited a statistically substantial variance between the mean thickness of RNFL assessed in myopic and normal eyes (p <0.003). Conclusion: There is a significant variation in mean thickness of RNFL between normal and myopic eyes as assessed by optical coherence tomography. Appropriate understanding of data of RNFL in eyes with myopia is suggested to evade glaucoma misdiagnosis. Abstract:With the aim of understanding the influence of fluorine on the double bond of the cis-stilbene moiety of combretastatin derivatives and encouraged by a preliminary molecular modeling study showing a different biological environment on the interaction site with tubulin, we prepared, through various synthetic approaches, a small library of compounds in which one or both of the olefinic hydrogens were replaced with fluorine. X-ray analysis on the difluoro-CA-4 analogue demonstrated that the spatial arrangement of the molecule was not modified, compared to its nonfluorinated counterpart. SAR analysis confirmed the importance of the cis-stereochemistry of the stilbene scaffold. Nevertheless, some unpredicted results were observed on a few trans-fluorinated derivatives. The position of a fluorine atom on the double bond may affect the inhibition of tubulin polymerization and cytotoxic activity of these compounds.
Choroidal Morphologic and Vascular Features in Patients With Myopic Choroidal Neovascularization and Different Levels of Myopia Based on Image Binarization of Optical Coherence Tomography

Abstract:Purpose: To characterize the choroidal morphologic and vascular features in different levels of myopes and patients with myopic choroidal neovascularization (mCNV).Methods: A total of 148 subjects were enrolled in this cross-sectional study, including 78 low-to-moderate myopes (LMM), 53 high myopes (HM), and 17 high myopic patients with mCNV. Ocular biometrics were measured using an optical low-coherence reflectometry device. Retinal and choroidal imaging was performed using enhanced depth imaging (EDI) spectral domain optical coherence tomography (OCT). Retinal parameters including retinal thickness and retinal volume were obtained from a built-in software. Binarization technique was adopted to investigate choroidal parameters including choroidal thickness (CT), vascular area, stromal area, and choroidal vascularity index (CVI). Choroidal parameters were measured at five locations to cover as much area of choroid as possible, and their patterns of distribution were further analyzed.Results: Patients with mCNV had an atrophic retina of comparable thickness to HM (273.65 +/- 17.28 vs. 276.49 +/- 13.29 mu m, p = 0.47), but the choroid was thinner than that of HM (153.94 +/- 15.12 vs. 236.09 +/- 38.51 mu m, p < 0.001). Subfoveal CVI was greatest in the mCNV eyes (0.651 +/- 0.009), followed by HM (0.645 +/- 0.012) and LMM eyes (0.636 +/- 0.012). Similar to CT, CVI was also found significantly different among these three groups at all five locations (p for trend < 0.001 for all locations). Axial length (AL) was negatively correlated with retinal volume (r = -0.236, p = 0.009), which is the only significant finding in associations between ocular factors and retinal parameters. Strong, negative correlations were identified between AL and subfoveal choroidal thickness (SFCT, r = -0.820, p < 0.001). However, AL was positively correlated with subfoveal CVI (r = 0.668, p < 0.001). CVI was greater in myopic eyes with thinner choroid (r = -0.578, p < 0.001). BCVA exhibited no significant association with CVI (r = 0.139, p = 0.092), but was negatively correlated with SFCT (r = -0.386, p < 0.001) and positively correlated with AL (r = 0.351, p < 0.001).Conclusion: Choroid in patients with mCNV was thinner yet more vascularized than that in HM and LMM subjects. CVI increased with a longer AL which was associated with a smaller SFCT, choroidal vascular area (VA), and total choroidal area (TCA). Better BCVA was achieved in subjects with thicker SFCT and shorter AL.
Ganglion cell-inner plexiform layer and retinal nerve fiber layer thickness according to myopia and optic disc area: a quantitative and three-dimensional analysis

Abstract:BackgroundTo determine the influences of myopia and optic disc size on ganglion cell-inner plexiform layer (GCIPL) and peripapillary retinal nerve fiber layer (RNFL) thickness profiles obtained by spectral domain optical coherence tomography (OCT).MethodsOne hundred and sixty-eight eyes of 168 young myopic subjects were recruited and assigned to one of three groups according to their spherical equivalent (SE) values and optic disc area. All underwent Cirrus HD-OCT imaging. The influences of myopia and optic disc size on the GCIPL and RNFL thickness profiles were evaluated by multiple comparisons and linear regression analysis. Three-dimensional surface plots of GCIPL and RNFL thickness corresponding to different combinations of myopia and optic disc size were constructed.ResultsEach of the quadrant RNFL thicknesses and their overall average were significantly thinner in high myopia compared to low myopia, except for the temporal quadrant (all Ps ≤0.003). The average and all-sectors GCIPL were significantly thinner in high myopia than in moderate- and/or low-myopia (all Ps ≤0.002). The average OCT RNFL thickness was correlated significantly with SE (0.81 μm/diopter, P < 0.001), axial length (-1.44 μm/mm, P < 0.001), and optic disc area (5.35 μm/mm2, P < 0.001) by linear regression analysis. As for the OCT GCIPL parameters, average GCIPL thickness showed a significant correlation with SE (0.84 μm/diopter, P < 0.001) and axial length (-1.65 μm/mm, P < 0.001). There was no significant correlation of average GCIPL thickness with optic disc area. Three-dimensional curves showed that larger optic discs were associated with increased average RNFL thickness and that more-myopic eyes were associated with decreased average GCIPL and RNFL thickness.ConclusionMyopia can significantly affect GCIPL and RNFL thickness profiles, and optic disc size has a significant influence on RNFL thickness. The current OCT maps employed in the evaluation of glaucoma should be analyzed in consideration of refractive status and optic disc size.

Abstract:AIM: To investigate the distribution characteristics of retinal nerve fiber layer thickness and vessel density in macular area of eyes with high myopia, using optical coherence tomography angiography (OCTA). METHODS: Forty eyes in 20 patients diagnosed with high myopia, age 29.90±7.92 years old, with a spherical equivalent of -8.95±2.01 D, were recruited. The retinal nerve fiber layer (RNFL) thickness and the vessel density of the superficial retinal capillary plexus, deep retinal capillary plexus, and choroidal capillary were measured by OCTA. Macular regions within a 6-mm diameter circle were divided into 9 subfields: the central subfield, and the pericentral and peripheral regions of superior, inferior, nasal, and temporal quadrants. The retinal nerve fiber layer thicknesses of different subfields were compared, and their relationships with spherical equivalent, axial length, and vessel density were analyzed. RESULTS: In the pericentral region, the retinal nerve fiber layer thickness of the superior quadrant was the lowest, whereas thickness was highest in the inferior quadrant (all P<0.05). In the peripheral region, the retinal nerve fiber layer thickness of the temporal quadrant was the lowest (all P<0.05). A negative correlation was found between the retinal nerve fiber layer thickness and spherical equivalent (r=-0.356, P=0.024) in the peripheral superior subfield. The vessel density of superficial retinal capillary plexus was positively correlated with retinal nerve fiber layer thickness in the nasal and inferior quadrants of the pericentral region and in the superior, nasal, and inferior quadrants of the peripheral region (r=0.314, 0.408, 0.467, 0.655, and 0.737 respectively; and all P<0.05), whereas the vessel density of choroidal capillary was negatively correlated with retinal nerve fiber layer thickness in the peripheral superior subfield (r= -0.356, P=0.024). CONCLUSION: The RNFL of macular areas is not uniformly distributed in high myopia (HM). As the spherical equivalent (SE) increases, the RNFL thickness decreases in certain areas, which correlates with the vessel density(VD) of superficial retinal capillary plexus (SCP) and choroidal capillary (CC) layers.
The measurement of retinal nerve fiber layer thickness in 213 myopia using optical conhenrence tomography and its associated factors

Abstract:Objective To study the characteristics of retinal nerve fiber layer(RNFL)thicknes and its associated factors. Methods According to the myopia degree 213 subjects were divided into 3 groups:51 subjects were in slight myopia group,91 subjects were in medium myopia group and 71 subjects were in high myopia group. The different quadrants RNFL thickness, diskarea,rimarea cup/disk area ratio ,intraocular pressure (IOP)and pupil diameters in each groups were measured by optical conhenrence tomography(OCT) , noncontact tonometer and Corneal topograph respectively.The data were analyzed by SPSS statistics software system. Results Compared with each group, the difference between the different quadrants RNFL thickness in myopia groups was significant (P<0.01), except the superior quandrant RNFL thickness in slight and medium myopia groups(P>0.05). The RNFL thicknesses in inferior, superior , nasal and temporal quadrant are associated with spherical equivalent diopter(r=-0.259、-0.316、-0.183、0.318, P <0.01) . The mean RNFL thickness is associated with diskarea,rimarea and cup/disk area ratio (r =0.165, P <0.05;r =0.262, P <0.01;r =-0.140, P <0.05). The mean RNFL thickness is not related with I0P and pupil diameters(r =-0.088、0.050, P >0.05). Conclusion The RNFL thicknesses in inferior, superior and nasal quadrant are thinner and thinner with an increase of myopia degree.But the RNFL thickness in temporal quadrant was thicker. The RNFL thickness become thicker with an increase of diskarea,rimarea and become thinner with an increase of cup/disk area ratio.
Relationship between peripapillary choroidal thickness and retinal nerve fiber layer in young people with myopia

Abstract:Aim To study the characteristics and relationship between peripapillary retinal nerve fiber layer (RNFL) and choroidal thickness in young people with myopia. Methods We retrospectively analyzed 92 cases (52 myopia, 40 emmetropia) regarding age, sex, refractive power, axial length (AL), and intraocular pressure. Peripapillary RNFL and choroidal thicknesses were measured by optical coherence tomography (OCT) in six sectors. Differences in thicknesses between the two groups were compared by single-factor analysis. Results RNFL was thickest in the inferotemporal sector (157.3 ± 19.66 μm) and thinnest in the nasal sector (58.78 ± 18.41 μm). Peripapillary choroid was thickest in the superonasal sector (176.37 ± 33.92 μm) and thinnest in the inferotemporal sector (131.79 ± 25.22 μm). The RNFL was thinner in the myopia group (99.04 ± 8.23 μm) vs the emmetropia group (103.25 ± 8.32 μm); significantly different in the superotemporal and inferonasal sectors. Peripapillary choroid thickness in the myopia group (148.65 ± 26.64 μm) was lower vs the emmetropia group (160.88 ± 29.06 μm); significantly different in the nasal, inferonasal, and inferotemporal sectors. RNFL thickness was negatively correlated with choroidal thickness in the nasal sector (r = −0.288). Conclusion Peripapillary RNFL and choroidal thicknesses showed regional distributions. RNFL was negatively correlated with PCT in the nasal sector, possibly related to eye axis growth and choroidal compensation.

Abstract:Purpose: To evaluate the RNFL thickness by optical coherence tomography (OCT) and correlate it with the axial length and refractive error in myopes. Methods: Patients with myopia −1D to −10D attending ophthalmology OPD at a tertiary hospital from October 2013 to April 2015 for evaluation underwent ophthalmic examination including refraction, axial length, and OCT RNFL thickness measurements. The patients were divided into two groups; group A included patients with AL ≤24 mm and group B AL >24 mm. Results : The study included 100 eyes with myopia ranging from −1D to −10D. The mean (±SD) age was 26.87 (±5.93) years with a range of 21–48 years and male: female ratio of 2:3. There was a statistically significant difference in the average peripapillary RNFL thickness between the two axial length groups ( P = 0.01); RNFL thickness in group A being 91.40 (±10.17) and group B 86.06 (±10.09); and in the average RNFL thickness between the 3 degrees of myopia groups, with higher myopic group having thinner RNFL ( P = 0.001). Conclusion: There is a significant decrease in RNFL thickness with an increase in the grade of myopia and axial length. This polar RNFL thinning could be wrongly attributed to glaucomatous change. We recommend careful interpretation of RNFL data in myopes with axial length >24 mm, when applying the current OCT nomograms.
