Abstract:Background: Obstructive sleep apnea-hypopnea syndrome (OSAHS) is a common sleep disorder. The lower atmospheric pressure and decreased oxygen levels of high-altitude areas can exacerbate the severity of OSAHS, but research into OSAHS in high-altitude areas remains limited. This study, from June 2015 to January 2020, involved 4,667 patients with suspected OSAHS and 38 healthy volunteers. The non-OSAHS group (AHI <5/h) had 395 patients, while the larger OSAHS group (AHI ≥5/h) comprised 4,272 patients. The significant size difference between the groups emphasized the study's focus on OSAHS, using the non-OSAHS mainly for comparison. Methods: Sleep technicians monitored the OSAHS patient group overnight by polysomnography (PSG), the apnea-hypopnea index (AHI), the mean oxygen saturation (MSpO 2 ), lowest oxygen saturation (LSpO 2 ), the oxygen desaturation index (ODI) and the total sleep time with oxygen saturation less than 90% (TST-SpO 2 <90%). Healthy volunteers self-monitored sleep patterns at home, using the CONTEC RS01 respiration sleep monitor with a wristwatch sleep apnea screen meter. The RSO1 wristwatch-style device has already been studied for consistency and sensitivity with the Alice-6 standard multi-lead sleep monitor and can be used for OSAHS screening in this region. Results: LSpO 2 recordings from healthy volunteers (86.36 ± 3.57%) and non-OSAHS (AHI <5/h) cohort (78.59 ± 11.99%) were much lower than previously reported normal values. Regression analysis identified no correlations between AHI levels and MSpO 2 or TST-SpO 2 <90%, weak correlations between AHI levels and LSpO 2 or MSpO 2 , and a strongly significant correlation between AHI levels and the ODI ( r = 0.76, p < 0.05). The data also indicated that the appropriate clinical thresholds for OSAHS patients living at mild high altitude are classified as mild, moderate, or severe based on LSpO 2 saturation criteria of 0.85–0.90, 0.65–0.84, or <0.65, respectively. Conclusion: The study findings suggest that individuals with an AHI score below 5 in OSAHS, who reside in high-altitude areas, also require closer monitoring due to the elevated risk of nocturnal hypoxia. Furthermore, the significant correlation between ODI values and the severity of OSAHS emphasizes the importance of considering treatment options. Additionally, the assessment of hypoxemia severity thresholds in OSAHS patients living in high-altitude regions provides valuable insights for refining diagnostic guidelines.

Extreme altitude‐induced central sleep apneas lasting more than 100 seconds in a healthy 23‐year‐old man

Abstract:Summary Central sleep apneas (CSA) can occur de novo at high‐altitude in individuals without sleep‐disordered breathing at low altitude. These apneas are usually brief, lasting only 5–15 s. This report presents the first documented case of a man experiencing extreme altitude‐induced CSA lasting more than 100 s in the absence of any sleep breathing disorder in normoxia. A 23‐year‐old male with no pre‐existing health conditions was recruited for a study examining the work of breathing during sleep at a simulated altitude of 3500 m (FiO2:13%). A lowland polysomnography was first conducted to exclude moderate to severe sleep‐disordered breathing and showed an apnea–hypopnea index (AHI) of 7.6/h, an oxygen desaturation index (ODI) of 4.8/h, and a mean pulse oximetry‐based oxygen saturation (SpO2) of 93.9%. During the recording in the hypoxic chamber, the participant experienced prolonged CSA lasting up to 1 min and 49 s. These apneas were associated with significant oxygen desaturations (nadir: 44%). To investigate the origin of these atypical CSA, the participant underwent a new low‐altitude polysomnography with transcutaneous CO2 measurement (mean PaCO2:46 mmHg) and diurnal arterial blood gas analysis (pH: 7.42, pCO2: 35.1 mmHg, pO2: 79.9 mmHg, HCO3−: 22.4 mmol/L). These results indicated no signs of chronic hypercapnia or hypocapnia. A hypoxia tolerance test (FiO2: 11.5%) demonstrated a good ventilatory response to hypoxia during exercise (1.004 L/min/kg). A rebreathing test according to the Read protocol in hyperoxia demonstrated an impaired ventilatory response to CO2 (<0.6 L/min/mmHg). This report documents a rare form of extreme hypoxia‐induced CSA, potentially caused by impaired CO2 chemoreceptor sensitivity and an increased arousal threshold.

Abstract:Purpose: This study sought to identify the changes and potential association between sleep characteristics and short-term memory, and mood states among volunteers at different altitudes and times. Method: A total of 26 healthy volunteers were recruited from the PLA General Hospital, and we conducted a longitudinal prospective survey for over 1 year from November 2019 to April 2021. First, we collected demographic data, sleep parameters by overnight polysomnography (PSG), short-term memory by digit span test, and mood states by completing a questionnaire with a brief profile of mood states among participants in the plain (53 m). Then, we continuously followed them up to collect data in the 3rd month at an altitude of 1,650 m (on the 3rd month of the 1-year survey period), the 3rd month at an altitude of 4,000 m (on the 6th month of the 1-year survey period), and the 9th month at an altitude of 4,000 m (on the 12th month of the 1-year survey period). Multiple linear regression analysis was used to construct models between sleep parameters and short-term memory, and mood states. Results: The prevalence of sleep apnea syndrome (SAS) significantly increased with rising elevation (P < 0.01). The apnea-hypopnea index (AHI), the mean apnea time (MAT), the longest apnea time (LAT), and the duration of time with SaO2 < 90% (TSA90) were increased (P < 0.05), and the mean pulse oxygen saturation (MSpO2), the lowest pulse oxygen saturation (LSpO2), and heart rate were significantly decreased with increasing altitude (P < 0.05). Digit span scores were decreased with increasing altitude (P < 0.001). A negative mood was more severe and a positive mood increasingly faded with rising elevation (P < 0.001). Additionally, linear correlation analysis showed that higher AHI, LAT, and MAT were strongly associated with a greater decline in short-term memory (in the 3rd and 9th month at an altitude of 4,000 m, respectively: r s = -0.897, -0.901; r s = -0.691, -0.749; r s = -0.732, -0.794, P < 0.001), and also were strongly associated with more severe negative mood (in the 3rd month at altitudes of 1,650 m and 4,000 m, respectively: r s = 0.655, 0.715, 0.724; r s = 0.771, 0.638, 0.737, P < 0.000625). Multiple linear regression pointed out that AHI was a significant predictor of negative mood among people at different altitudes (in the 3rd month at an altitude of 1,650 m: TMD = 33.161 + 6.495*AHI; in the 3rd month at an altitude of 4,000 m: TMD = 74.247 + 1.589*AHI, P < 0.05). Conclusion: SAS developed easily in high altitudes, most often in CSA (central sleep apnea, CSA). The sleep, short-term memory, and negative mood were significantly more damaged with elevation in volunteers. Sleep parameters were closely associated with short-term memory and mood states in volunteers at high altitudes; the higher the sleep parameters (AHI, LAT, and MAT) scores, the more significant the mood disorders and the more obvious impairment of short-term memory. AHI was a critical predictor of the negative mood of volunteers at different altitudes. This study provides evidence that could help with the prevention and control of sleep disorder, cognitive disorder, and negative mood among populations with high altitudes.
Effect of mild obstructive sleep apnea in mountaineers during the climb to Mount Aconcagua

Abstract:Objective: to compare mountaineers with and without asymptomatic sleep apnea (OSA) before the ascent and to study high altitude-related sleep disorders, its interaction with metabolic, neuroendocrine and immunological components. Material and methods: During an expedition to Mount Aconcagua, researchers assessed the respiratory polygraphy (RP), clinical condition and inflammatory parameters, and rhythm of cortisol secretion in mountaineers sleeping at different altitude camps. Results: 8 athletes (4 women), 36 years old (25-51) participated. Baseline and final BMI were; 23.6 (20.9-28.7) and 22.77 (20.9-27.7), respectively: p<0.01. 40 valid RP recordings were analyzed. At 746 m.a.s.l. (baseline), only 2 mountaineers presented mild asymptomatic OSA. The OSA group presented baseline apnea-hypopnea index (AHI) values between 5-15 events per hour, which evidence a mild respiratory sleep disorder with AHI increased by altitude depending of central apneas and hypopneas (p<0.05) as high altitude periodic breathing pattern but no increase in obstructive apneas (p<0.01). The circadian rhythm of cortisol was maintained in all cases in which they had not received treatment with dexamethasone and their values increased with the altitude reached. Increased systolic blood pressure was observed in the OSA group. Conclusion: In a context of hypobaric hypoxia, individuals with pre-existing asymptomatic OSA are prone to experiencing lower oxygen saturations and clinical deterioration.
Sleep characteristics in an adult with sleep complaints in three cities at different altitudes

Abstract:Sleep studies conducted at an altitude that is different from the home altitude can yield misleading results regarding the severity of obstructive sleep apnea (OSA). The objective of the present study was to determine the sleep characteristics of a patient undergoing polysomnography (PSG) in three Colombian cities at different altitudes (Bogotá, at 2,640 m above sea level [ASL]; Bucaramanga, at 959 m ASL; and Santa Marta, at 15 m ASL). The patient was an obese man with diabetes and suspected OSA. All PSG recordings were scored and interpreted in accordance with American Academy of Sleep Medicine criteria. In Bogotá, PSG revealed moderate OSA (an apnea-hypopnea index [AHI] of 21 events/h); in Bucaramanga, PSG revealed increased upper airway resistance (an AHI of 2 events/h); in Santa Marta, PSG revealed mild OSA (an AHI of 7 events/h). The reduction in the AHI was predominantly a reduction in hypopneas and obstructive apneas. The respiratory events were shorter in duration in the city at an intermediate altitude. Given that the AHI varied widely across cities, we can assume that the patient is normal or has moderate OSA depending on the city where he is. Central apneas were found to have no influence on the AHI.
A narrative review of periodic breathing during sleep at high altitude: From acclimatizing lowlanders to adapted highlanders

With acclimatization in lowlanders (i.e. day to weeks at high altitude), periodic breathing persists and there appears to be a lengthening of cycle length. This increase in periodic breathing does not coalesce with poor sleep quality and has minimal impact on the average sleeping oxygen saturation. Despite persistence of periodic breathing in acclimatized lowlanders, high altitude adapted highlanders appear to demonstrate a blunting of periodic breathing, compared to lowlanders. This observation is suggestive that periodic breathing during sleep is an adaptive trait to high altitude. Periodic breathing during sleep at high altitude is almost universal among sojourners. Here, in the context of acclimatization and adaptation, we provide a contemporary review on periodic breathing at high altitude, and explore whether this is an adaptive or maladaptive process. The mechanism(s), prevalence and role of periodic breathing in acclimatized lowlanders at high altitude are contrasted with the available data from adapted indigenous populations (e.g. Andean and Tibetan highlanders). It is concluded that (1) periodic breathing persists with acclimatization in lowlanders and the severity is proportional to sleeping altitude; (2) periodic breathing does not seem to coalesce with poor sleep quality such that, with acclimatization, there appears to be a lengthening of cycle length and minimal impact on the average sleeping oxygen saturation; and (3) high altitude adapted highlanders appear to demonstrate a blunting of periodic breathing, compared to lowlanders, comprising a feature that withstands the negative influences of chronic mountain sickness. These observations indicate that periodic breathing persists with high altitude acclimatization with no obvious negative consequences; however, periodic breathing is attenuated with high altitude adaptation and therefore potentially reflects an adaptive trait to this environment.

Abstract:Objective To explore the effect of different altitude on the patients with sleep apnea-hypopnea syndrome.Methods Five subjects underwent respiration and pulse oximeter tests.Apnea-hypopnea index (AHI),hypopnea index (HI),mean saturation of arterial blood oxygen (MSaO 2 ),the lowest saturation of arterial blood oxygen (LSaO 2 ),the time spent at SaO 2 below 90% (SIT90 ),the number of desaturations≥4% per hour(ODI4 )and blood pressure were calculated at different altitude (include 500 m,1 000 m,2 261 m,2 800 m,3 650 m and 4 200 m).Results The apnea-hypopnea index, hypopnea index,oxygen desaturation index,MSaO 2 ,LSaO 2 ,systolic pressure and diastolic pressure were significantly different among five different altitude groups (all P <0.05).The AHI rised from (1 1.0 ± 5.4)times/h to (28.6±1 1.3)times/h,the HI rised from (7.6±3.4)times/h to (1 9.6 ±9.0)times/h and ODI4 rised from (1 1.2±5.2)times/h to (29.8±1 1.0)times/h as altitude increases from 500 m to 4 200 m.Conclusions The influence of highter altitude on the respiratory function are predominantly central apneas and hypopneas.We should pay attention the effects of high altitude on the sleep apnea.
Effectiveness of Continuous Positive Airway Pressure in Alleviating Hypoxemia and Improving Exertional Capacity at Altitude

High Alt Med Biol. 00:000-000, 2024. Introduction: Decreased oxygen saturation and exercise tolerance are commonly experienced at high altitude. Continuous positive airway pressure (CPAP) devices have become increasingly portable and battery powered, providing a potentially unique new therapeutic modality for treatment of altitude-related illnesses. This study evaluated the potential use of CPAP devices to improve and maintain oxygen saturation at altitude, both at rest and with exertion, to evaluate the feasibility of using this device at altitude. Methods: Subjects were taken to Mount Blue Sky and monitored while they hiked to the summit (4,350 m), maintaining a consistent level of exertion. Subjects hiked for 0.7 km both with and without CPAP set to 10 cmH2O pressure. Continuous vital signs were collected during the hike and recovery period. Results: All subjects completed the hike wearing CPAP devices at a vigorous level of exertion. Mean oxygen saturation of the CPAP group (M = 83.8%, SD = 3.72) was significantly higher than that of the control group during exertion (M = 78.7%, SD = 2.97); p = 0.005. Recovery after exertion was quicker in the CPAP group than the control group. Three subjects experienced claustrophobia requiring a brief pause, but were able to complete their exercise trial without removing equipment or experiencing adverse events. When pauses from claustrophobia were excluded, there was no difference in completion time between the groups (p = 0.06). Conclusion: CPAP reliably improved oxygen saturation at rest and during vigorous exertion at high altitude. Its ability to correct hypoxemia, even with physical exertion, may prove useful after further study as a portable self-carried device to prevent and treat altitude-related illness, or to improve safety in high-altitude rescues.

Abstract:BackgroundHeart rate variability (HRV) during sleep in normal subjects at high altitude shows a decrease in parasympathetic tone associated with an increase in the sympathetic one, which tends to be reversed with acclimatization. However, periodic breathing (PB) during sleep may influence this effect detected by HRV spectral analysis.PurposeThe aim of our study was to investigate HRV during sleep periodic breathing (PB) at high altitude in normal subjects at two different times of acclimatization, i.e., two different levels of hypoxemia.MethodsRecordings of six healthy climbers (aged between 33 and 40 years), at sea level (SL) and at Everest North Base Camp (5180 m), during the first (BC1) and the tenth (BC2) overnight unattended polygraphy, were analyzed. PB was commonplace in all subjects at high altitude to a variable extent. At SL and at BC1 and BC2, HRV was evaluated overnight and separately during clear regular breathing (RB) and PB.ResultsA mean overnight beat-by-beat series interval (RR) reduction at acute environmental hypoxic exposure that resumed to SL values after 10-day sojourn was observed. This reduction was mostly due to RR during RB, while during PB, RR values were not different from SL. Higher peaks of tidal volume were associated with higher HRV.ConclusionsThe present study shows that in healthy subjects, PB with central apneas increases the amplitude of RR oscillations, and these oscillations are tightly related to respiratory amplitude. Oxygenation does not influence this phenomenon. Therefore, oscillations in ventilation itself should be taken into account when investigating HRV.

Abstract:Objectives: The adverse effects of hypoxia and sleep deprivation (SD) have been studied separately but there is little literature on their effects when acting in conjunction. Hence, the effects of SD on hypoxia tolerance and cognitive performance when exposed to altitude were investigated. Material and Methods: In a cross-sectional repetitive measure design, 15 healthy volunteers assessed on cognitive performance were made to perform cognitive tests (Dual Task Test, Stroop Test [ST], and Digit Symbol Substitution Test [DSST] of Psychometric Evaluation Designed for Aviators test battery) on the ground and at a simulated altitude of 17,000 ft in a hypobaric chamber on 4 days. On the 2 nd and 3 rd day, the tests were done following 2 h of SD the previous night, and on the 4 th day, the same was repeated following normal recovery sleep. Heart rate (HR) and respiratory rate (RR) were measured on the ground and 17,000 ft. The Lake Louise Acute Mountain Sickness (LLAMS) scores were used to assess the severity of symptoms of hypoxia and time to arterial oxygen desaturation of 75% was used to measure hypoxia tolerance. Results: Although not statistically significant, the LLAMS scores showed an increasing trend on SD days with an improvement in recovery of sleep. No statistically significant differences in the HR (F = 0.569, P = 0.637) and RR (F = 0.569, P = 0.637) were observed across the days of SD or recovery. The time to oxygen desaturation to 75% also showed no significant effect (F = 0.587, P = 0.625). A statistically significant increase in response times of two cognitive tests (ST and DSST) was observed on exposure to altitude on all 4 days. The three cognitive tests did not show any statistically significant effect following 2 h of SD or recovery. Conclusion: The study could not elicit a statistically significant effect of 2 h of SD across 2 consecutive nights on the cognitive performance measures on exposure to hypoxia.

Abstract:Objective To analysis the respiratory disturbance in patients with obstructive sleep apnea-hypopnea syndrome(OSAHS)in Karamaycity(altitude 500 m)and Kunming city(altitude 1800 m).Methods 288 subjects(114 cases of OSAHS from Karamay city and 174 cases of OSAHS from Kunming city) were underwent respiration and pulse oximeter tests.Apnea-hypopnea index(AHI), hypopnea index(HI),mean SaO2(MSaO2%),the lowest SaO2(LSaO2%),the time spent at SaO2below 90%,the number of desaturations≥4% per hour(ODI4)were calculated at different altitude district (Karamay city altitude 500 m and Kunming city altitude 1800 m).The two groups were matched age, gender,body mass index(BMI).We selected 42 cases OSAHS of Han nationlity and 38 cases OSAHS of Uyghur nationlity from Karamay city.The two groups were matched for age and gender.Results The AHI[(52.7±28.3)/h vs(33.1±19.9)/h]、ODI4[(55.1±29.4)/h vs(33.5±19.9)/h]、The MSaO2 [(86.1±10.1)% vs(90.6±7.3)%]、the LSaO2[(65.3±25.6)% vs(72.4±12.4)%]、The arterial oxygen partial pressure(PaO2)[(61.4±12.9)mmHg vs(83.3±15.8)mmHg]and arterial carbon dioxide partial pressure (PaCO2)[(33.5 ± 7.4) mmHg vs (40.8 ± 5.7) mmHg]were significant difference between Kunming and Karamay's OSAHS groups(all P <0.05).The PaO2[(76.0±17.9)mmHg vs(85.5 ± 15.9)mmHg]and PaCO2[(43.1 ± 5.5)mmHg vs(40.5 ± 5.6)mmHg]were significant difference between Han Nationality and Uyghur Nationality OSAHS groups(both P<0.05). Conclusions Sleep disorder breathing were related to different altitude and nationality,the high altitude may induce the PaO2and PaCO2of OSAHS during the daytime,which may be attribute to the function of respiratory control.

Abstract:Abstract The objective of the study was to observe brain function changes in Obstructive Sleep Apnoea Hypopnoea Syndrome (OSAHS) patients at high altitude. Resting-state functional magnetic resonance imaging (rs-fMRI) in patients with OSAHS was assessed using regional homogeneity (ReHo), amplitude of low frequency fluctuation (ALFF) and functional connectivity (FC). In this study, 36 male patients with OSAHS and 38 healthy male subjects were recruited from high-altitude areas, specifically, altitudes of 2,000–3,000 m. OSAHS was diagnosed by polysomnography (PSG). The blood oxygen level-dependent (BOLD) signals of OSAHS patients and healthy controls in the resting state were obtained and compared using ReHo, ALFF and FC methods. The posterior cingulate cortex (PCC) was selected as the seed region in the comparison of FC between the two groups. Compared with the healthy control group, multiple brain functions in the OSAHS patient group were different. There were correlations between the brain function values of some brain regions and demographic data. We also found that in contrast to earlier findings with individuals in plains areas, the brain function at the frontal lobe and the precuneus were higher in OSAHS patients, and the PCC showed higher FC with the left caudate, which may be due to the high-altitude hypoxic environment.

Abstract:Abstract Obstructive sleep apnea syndrome (OSAS) can lead to alterations in right ventricular (RV) performance and pulmonary vascular haemodynamics. Additionally, altitude-related hypoxia is associated with pulmonary vasoconstriction, and the effect of high-altitude on the pulmonary circulation in OSAS patients can be further altered. We sought to assess alterations in RV morphology and function in OSAS patients living at high altitude by way of 2-dimensional speckle tracking echocardiography (2D-STE), real-time 3- dimensional echocardiography (RT-3DE) and cardiac biomarkers. We also evaluate the impact of continuous positive airway pressure (CPAP) treatment on RV performance. Seventy-one patients with newly diagnosed OSAS and thirty-one controls were included in this study. All individuals were assessed for cardiac biomarkers as well as underwent 2D-STE and RT-3DE. Forty-five OSAS patients underwent CPAP therapy for at least 24 weeks and were studied before and after CPAP treatment. RT-3DE was used to measure RV volume, and calculate RV 3D ejection fraction (3D RVEF). Peak systolic strain was determined. Cardiac biomarkers, including C-reactive protein (CRP), N-terminal pro-B-type natriuretic peptide, and cardiac troponin T were also measured. Right atrium volume index, RV volume, RV volume index, systolic pulmonary artery pressure (sPAP), pulmonary vascular resistance (PVR) and level of serum CRP were significantly higher in OSAS group, while OSAS patients showed lower 3D RVEF and RV longitudinal strains. Compared to the patients with sPAP < 40 mmHg, RV longitudinal strains in patients with sPAP ≥ 40 mmHg were lower. Both RV global longitudinal strain and sPAP were associated with apnea–hypopnea index. Patients treated with 6 months of CPAP therapy had significant improvement in RV geometry and performance. RV structural abnormalities and RV function impairments were observed in OSAS patients living at moderate high altitude compared to control highlanders. The reversibility of these changes after application of CPAP were further confirmed.

Abstract:Abstract Introduction Given conflicting evidence in the literature, we sought to evaluate the relative importance of central sleep apnea (CSA) and obstructive sleep apnea (OSA) on incident atrial fibrillation (AF) risk in a cohort of patients referred for sleep evaluation. Methods We reviewed the medical records of patients without AF at baseline who underwent in-lab diagnostic polysomnography (PSG) at six sleep laboratories within the University of Pittsburgh Medical Center (UPMC) between 2004 and 2018. Baseline characteristics including apnea hypopnea index (AHI), central apnea index (CAI), and obstructive apnea index (OAI) were extracted from the electronic health record. Among those with AHI≥15, patients were categorized as CSA if CAI≥5 and CAI>OAI or OSA if OAI≥5 and OAI>CAI. Patients with AHI< 15 were included as the reference group. Incident AF was identified based on AF as a primary or secondary diagnosis on either inpatient or outpatient UPMC visits. Results We identified 24,199 patients (425 CSA, 7559 OSA, 16,215 no sleep-disordered breathing [reference]) for analysis who had a mean age of 49, mean body mass index (BMI) 34, and 48% male. The incidence rates for AF ranged from 1.07 per 100 person-years in the reference group to 1.91 and 2.60 in the OSA and CSA groups (p< 0.01). Using Cox proportional hazards models, the hazard ratio for incident AF in the OSA group compared to reference was 1.78 (95% CI [1.63-1.94], p< 0.01) and in the CSA group was 2.48 (95% CI [1.92-3.21], p< 0.01). After adjusting for age, sex, race, BMI, and baseline diagnoses of diabetes, heart failure, prior heart attack, and prior stroke, OSA was no longer associated with incident AF risk (HR=1.01, 95% CI [0.92-1.12], p=0.76). However, a potentially important association between CSA and incident AF persisted (HR=1.28, 95% CI [0.99-1.67], p=0.06). Conclusion We found that CSA was associated with a 28% increased rate of incident AF independent of traditional risk factors, which was nearly statistically significant. Intermittent Oxygen Inhalation with Proper Frequency Improves Overall Health Conditions and Alleviates Symptoms in a Population at High Risk of Chronic Mountain Sickness with Severe Symptoms

Abstract:BACKGROUND: Oxygen inhalation therapy is essential for the treatment of patients with chronic mountain sickness (CMS), but the efficacy of oxygen inhalation for populations at high risk of CMS remains unknown. This research investigated whether oxygen inhalation therapy benefits populations at high risk of CMS.METHODS: A total of 296 local residents living at an altitude of 3658 m were included; of which these were 25 diagnosed cases of CMS, 8 cases dropped out of the study, and 263 cases were included in the analysis. The subjects were divided into high-risk (180 ≤ hemoglobin (Hb) <210 g/L, n = 161) and low-risk (Hb <180 g/L, n = 102) groups, and the cases in each group were divided into severe symptom (CMS score ≥6) and mild symptom (CMS score 0-5) subgroups. Severe symptomatic population of either high- or low-risk CMS was randomly assigned to no oxygen intake group (A group) or oxygen intake 7 times/week group (D group); mild symptomatic population of either high- or low-risk CMS was randomly assigned to no oxygen intake group (A group), oxygen intake 2 times/week group (B group), and 4 times/week group (C group). The courses for oxygen intake were all 30 days. The CMS symptoms, sleep quality, physiological biomarkers, biochemical markers, etc., were recorded on the day before oxygen intake, on the 15th and 30th days of oxygen intake, and on the 15th day after terminating oxygen intake therapy.RESULTS: A total of 263 residents were finally included in the analysis. Among these high-altitude residents, CMS symptom scores decreased for oxygen inhalation methods B, C, and D at 15 and 30 days after oxygen intake and 15 days after termination, including dyspnea, palpitation, and headache index, compared to those before oxygen intake (B group: Z = 5.604, 5.092, 5.741; C group: Z = 4.155, 4.068, 4.809; D group: Z = 6.021, 6.196, 5.331, at the 3 time points respectively; all P < 0.05/3 vs. before intake). However, dyspnea/palpitation (A group: Z = 5.003, 5.428, 5.493, both P < 0.05/3 vs. before intake) and headache (A group: Z = 4.263, 3.890, 4.040, both P < 0.05/3 vs. before intake) index decreased significantly also for oxygen inhalation method A at all the 3 time points. Cyanosis index decreased significantly 30 days after oxygen intake only in the group of participants administered the D method (Z = 2.701, P = 0.007). Tinnitus index decreased significantly in group A and D at 15 days (A group: Z = 3.377, P = 0.001, D group: Z = 3.150, P = 0.002), 30 days after oxygen intake (A group: Z = 2.836, P = 0.005, D group: Z = 5.963, P < 0.0001) and 15 days after termination (A group: Z = 2.734, P = 0.006, D group: Z = 4.049, P = 0.0001), and decreased significantly in the group B and C at 15 days after termination (B group: Z = 2.611, P = 0.009; C group: Z = 3.302, P = 0.001). In the population at high risk of CMS with severe symptoms, oxygen intake 7 times/week significantly improved total symptom scores of severe symptoms at 15 days (4 [2, 5] vs. 5.5 [4, 7], Z = 2.890, P = 0.005) and 30 days (3 [1, 5] vs. 5.5 [2, 7], Z = 3.270, P = 0.001) after oxygen intake compared to no oxygen intake. In the population at high risk of CMS with mild symptoms, compared to no oxygen intake, oxygen intake 2 or 4 times/week did not improve the total symptom scores at 15 days (2 [1, 3], 3 [1, 4] vs. 3 [1.5, 5]; χ2 = 2.490, P = 0.288), and at 30 days (2 [0, 4], 2 [1, 4.5] vs. 3 [2, 5]; χ2 = 3.730, P = 0.155) after oxygen intake. In the population at low risk of CMS, oxygen intake did not significantly change the white cell count and red cell count compared to no oxygen intake, neither in the severe symptomatic population nor in the mild symptomatic population.CONCLUSIONS: Intermittent oxygen inhalation with proper frequency might alleviate symptoms in residents at high altitude by improving their overall health conditions. Administration of oxygen inhalation therapy 2-4 times/week might not benefit populations at high risk of CMS with mild CMS symptoms while administration of therapy 7 times/week might benefit those with severe symptoms. Oxygen inhalation therapy is not recommended for low-risk CMS populations.

Abstract:Objective To investigate the relationship between hypoxia at high altitude and arterial compliance of healthy men on acute exposure to high altitude and clinical importance.Methods 38 subjects who had lived in sea-level were included in our research.All subjects underwent non-invasive office BP measurement ( Dynapulse 200M;Pulse Metric,Inc.,USA) in sea-level or in high altitude (4000m)to obtain data of the arterial compliance of healthy men on acute exposure to high altitude.Results On acute exposure to high altitude in the sea-level natives,SBp was increased to 6.6%,and SV Resistanc(SVR) decreased significantly at both altitude(P0.05);but BA compliance(BAC) did not affected by change between both altitude(P0.05).Logistic regression showed that significantly charged SVR was an importantly dangerous factor for AMS (F=16.04,P=0.000).Conclusions out results demonstrate the continual decreasing of SaO 2 in arterial blood and SVR were observed on acute exposure to high altitude in the sea-level natives,SBp increased temporarily.Estimate of SVR may be an important screening to high danger population of AMS in clinic.

Abstract:Myasthenia gravis (MG) is a T-cell-regulated autoimmune disease in which a pathological autoantibody response is mounted against the nicotinic acetylcholine receptor of the neuromuscular junction. Our laboratory previously identified a T cell epitope, p195-212, derived from the human acetylcholine receptor alpha subunit, which triggered PBL to proliferate from about 70% of MG patients tested. p195-212 was also found to be an immunodominant T cell epitope in SJL mice and a cryptic epitope in C3H.SW mice. Inoculation of naive SJL mice with cells from a p195-212-specific syngeneic T cell line caused MG-related autoimmune manifestations in those mice. In these studies we analyzed TCR alpha and beta chain sequences used by T cell lines and clones from both high- and low-responder mouse strains in response to p195-212. T cell lines generated from either strain expressed single TCR V beta gene segments (V beta 17 in SJL mice and V beta 8 in C3H.SW mice). By deleting V beta 17-expressing T cells in p195-212-immunized SJL mice we established a T cell line that expressed the V beta 6 gene product, suggesting that SJL mice are not limited to using a single V beta gene segment in response to p195-212. In addition, we found that N- and/or C-terminal-truncated peptides of p195-212, presented under the same culture conditions to different clones bearing the same TCR alpha beta chain, could elicit very different proliferative responses from the clones. Thus, even within a constrained system, factors other than TCR sequence contribute to the differential stimulation of T cell responses.

Abstract:Abstract Introduction Obstructive sleep apnea (OSA) affects more than 10% of older Americans and is associated with future comorbid conditions. The current method for measuring OSA severity, the apnea-hypopnea index (AHI), is poorly correlated to symptom burden and clinical improvement. Hypoxic burden (HB), which quantifies and characterizes OSA-related hypoxemia, has been suggested as an alternative OSA measure. However, more research is needed to understand HB’s predictive ability. Thus, this study investigated whether HB could predict which OSA patients, prescribed CPAP, will have the greatest improvement in measures of sleepiness and cognition. Methods This is a secondary analysis from a trial that investigated the effects of CPAP versus sham treatment on sleepiness [Epworth Sleepiness Scale (ESS), psychomotor vigilance test (PVT)] and cognition [Buschke Selective Reminding Test (BSRT), Sustained Working Memory Test (SWMT), Cogscreen Pathfinder Number (PN)]. Using the SpO2 signal, HB was calculated as the area under the desaturation curve during all desaturation events (sustained >3% dip in SpO2). For categorical analyses, we classified “sleepy” and “not sleepy” in two different ways: ESS≥10 or < 10 and PVT lapses≥4 and < 4, respectively. Results Of the 426 (66% male) participants included, ESS and PVT significantly decreased after two months of treatment; 10.2±4.2 vs 8.0±4.2 (p< 0.001) and 2.5±1.6 vs 2.3±1.4 (p< 0.01), respectively. After correcting for BMI, age, sex, and AHI, baseline HB was associated with a change in sleepiness after treatment (∆ESS R2=0.04, p< 0.001; ∆PVT R2=0.01, p< 0.05). Moreover, having higher baseline HB increased odds for transitioning from “sleepy” to “non-sleepy” based on ESS (1.006 [1.001, 1.012]) or PVT (1.07 [1.003, 1.011]). HB was not associated with change in cognitive variables at follow-up (p>0.05), before correcting for any covariates. Conclusion HB had a significant yet weak effect on subjective (ESS) and objective (PVT) measures of sleepiness after correcting for covariates. HB did not significantly explain changes in any cognitive measure after two months of CPAP treatment. Whether this lack of an association is due to the absence of an overall improvement in cognition or the inability of HB to explain variability remains to be tested.
