Abstract:Objective.Intermittent hypoxia, the primary pathology of obstructive sleep apnea (OSA), causes cardiovascular responses resulting in changes in hemodynamic parameters such as stroke volume (SV), blood pressure (BP), and heart rate (HR). However, previous studies have produced very different conclusions, such as suggesting that SV increases or decreases during apnea. A key reason for drawing contrary conclusions from similar measurements may be due to ignoring the time delay in acquiring response signals. By analyzing the signals collected during hypoxia, we aim to establish criteria for determining the delay time between the onset of apnea and the onset of physiological parameter response.Approach.We monitored oxygen saturation (SpO2), transcutaneous oxygen pressure (TcPO2), and hemodynamic parameters SV, HR, and BP, during sleep in 66 patients with different OSA severity to observe body's response to hypoxia and determine the delay time of above parameters. Data were analyzed using the Kruskal-Wallis test, Quade test, and Spearman test.Main results.We found that simultaneous acquisition of various parameters inevitably involved varying degrees of response delay (7.12-25.60 s). The delay time of hemodynamic parameters was significantly shorter than that of SpO2and TcPO2(p< 0.01). OSA severity affected the response delay of SpO2, TcPO2, SV, mean BP, and HR (p< 0.05). SV delay time was negatively correlated with the apnea-hypopnea index (r= -0.4831,p< 0.0001).Significance.The real body response should be determined after removing the effect of delay time, which is the key to solve the problem of drawing contradictory conclusions from similar studies. The methods and important findings presented in this study provide key information for revealing the true response of the cardiovascular system during hypoxia, indicating the importance of proper signal analysis for correctly interpreting the cardiovascular hemodynamic response phenomena and exploring their physiological and pathophysiological mechanisms.
Altered Cardiovascular Variability in Obstructive Sleep Apnea

Abstract:Background —Altered cardiovascular variability is a prognostic indicator for cardiovascular events. Patients with obstructive sleep apnea (OSA) are at an increased risk for cardiovascular disease. We tested the hypothesis that OSA is accompanied by alterations in cardiovascular variability, even in the absence of overt cardiovascular disease. Methods and Results —Spectral analysis of variability of muscle sympathetic nerve activity, RR interval, and blood pressure were obtained during undisturbed supine rest in 15 patients with moderate-to-severe OSA, 18 patients with mild OSA, and 16 healthy control subjects in whom sleep disordered breathing was excluded by complete overnight polysomnography. Patients with OSA were newly diagnosed, never treated for OSA, and free of any other known diseases. Patients with moderate-to-severe OSA had shorter RR intervals (793±27 ms) and increased sympathetic burst frequency (49±4 bursts/min) compared with control subjects (947±42 ms; 24±3 bursts/min; P =0.008 and P <0.001, respectively). In these patients, total variance of RR was reduced ( P =0.01) and spectral analysis of RR variability showed an increase in low frequency normalized units, a decrease in high frequency normalized units, and an increase in the ratio of low to high frequency (all P <0.05). Even though blood pressure was similar to that of the control subjects, blood pressure variance in patients with moderate-to-severe OSA was more than double the variance in control subjects ( P =0.01). Patients with mild OSA also had a reduction in RR variance ( P =0.02) in the absence of any significant difference in absolute RR interval. For all patients with OSA, linear regression showed a positive correlation ( r =0.40; P =0.02) between sleep apnea severity and blood pressure variance. Conclusions —Cardiovascular variability is altered in patients with OSA. This alteration is evident even in the absence of hypertension, heart failure, or other disease states and may be linked to the severity of OSA. Abnormalities in cardiovascular variability may be implicated in the subsequent development of overt cardiovascular disease in patients with OSA.

Abnormal Cerebral Blood Flow and Volumetric Brain Morphometry in Patients With Obstructive Sleep Apnea

Abstract:Obstructive sleep apnea (OSA) is a serious breathing disorder, leading to myocardial infarction, high blood pressure, and stroke. Brain morphological changes have been widely reported in patients with OSA. The pathophysiological mechanisms of cerebral blood flow (CBF) changes associated with OSA are not clear. In this study, 20 patients with OSA and 36 healthy controls (HCs) were recruited, and then pseudo-continuous arterial spin labeling (pCASL) and voxel-based morphometry (VBM) methods were utilized to explore blood perfusion and morphological changes in the patients with OSA. Compared with the HC group, the OSA group showed increased CBF values in the right medial prefrontal cortex (mPFC), left precentral gyrus, and right insula and showed decreased CBF values in the right temporal pole (TP) and the right cerebellum_Crus2. Compared with the HC group, the patients with OSA showed decreased gray matter volume (GMV) in the right dorsal lateral prefrontal cortex (DLPFC), the right occipital pole, and the vermis. There were no significantly increased GMV brain regions found in patients with OSA. Pearson correlation analysis showed that the reduced GMV in the right DLPFC and the right occipital pole was both positively correlated with Mini-Mental State Examination (MMSE) (r = 0.755, p < 0.001; r = 0.686, p = 0.002) and Montreal Cognitive Assessment (MoCA) scores (r = 0.716, p = 0.001; r = 0.601, p = 0.008), and the reduced GMV in the right occipital pole was negatively correlated with duration of illness (r = -0.497, p = 0.036). Patients with OSA have abnormal blood perfusion metabolism and morphological changes in brain regions including the frontal lobe and the cerebellum and were closely related to abnormal behavior, psychology, and cognitive function, which play an important role in the pathophysiological mechanism of OSA.

Abstract:Purpose: It is well established that, together with a multitude of other adverse effects on health, severe obstructive sleep apnoea causes reduced cerebral perfusion and, in turn, reduced cerebral function. Less clear is the impact of moderate obstructive sleep apnoea (OSA). Our aim was to determine if cerebral blood flow is impaired in people diagnosed with moderate OSA. Methods: Twenty-four patients diagnosed with moderate OSA (15 ≤ apnoea-hypopnea index (AHI) < 30) were recruited (aged 32-72, median 59 years, 10 female). Seven controls (aged 42-73 years, median 62 years, 4 female) with an AHI < 5 were also recruited. The OSA status of all participants was confirmed at baseline by unattended polysomnography and they had an MRI arterial-spin-labelling scan of cerebral perfusion. Results: Neither global perfusion nor voxel-wise perfusion differed significantly between the moderate-OSA and control groups. We also compared the average perfusion across three regional clusters, which had been found in a previous study to have significant perfusion differences with moderate-severe OSA versus control, and found no significant difference in perfusion between the two groups. The perfusions were also very close, with means of 50.2 and 51.8 mL/100 g/min for the moderate-OSAs and controls, respectively, with a negligible effect size (Cohen's d = 0.10). Conclusion: We conclude that cerebral perfusion is not impaired in people with moderate OSA and that cerebral flow regulatory mechanisms can cope with the adverse effects which occur in moderate OSA. This is an important factor in clinical decisions for prescription of continuous positive airway pressure therapy (CPAP).

Abstract:Purpose: To evaluate capillaries perfusion and retinal nerve fiber layer (RNFL) thickness diurnal changes of macular/optic disc regions among participants with or without obstructive sleep apnea-hypopnea (OSA) using spectral-domain optical coherence tomography angiography (OCTA). Methods: In this study, we enrolled a cohort of 35 participants including 14 patients with mild-to-moderate OSA, 12 patients with severe OSA, and 9 healthy individuals. All participants had Berlin questionnaire filled. At 20:00 and 6:30, right before and after the polysomnography examination, a comprehensive ocular examination was conducted. The systemic and ocular clinical characteristics were collected, and OCTA scans were performed repeatedly. Blood flow and RNFL thickness parameters were then exported using built-in software and analyzed accordingly. Results: After sleep, the overall vessel density (VD) variables, especially macular and choriocapillaris VDs, were relatively comparative and stable. One exception was the RPC vessel density at the inside-disc region with a decreasing trend in the mild-to-moderate group (p=0.023). RNFL changes before and after sleep in the nasal-inferior and peripapillary region were statistically significant (p=0.003; p=0.043) among three groups. And multiple testing correction verified the significant difference in diurnal changes between the mild-to-moderate group and the control group in pairwise comparisons (p=0.006; p=0.02). Conclusions: The changes of imperceptible blood flow and RNFL thickness overnight around optic disc areas could be observed in OSA patients. Despite physiological fluctuations, aberrant diurnal changes might be useful for identifying a decrease in micro-environmental stability associated with the development of various ocular diseases such as glaucoma. Other VD variables, especially macular and choriocapillaris VDs, are relatively stable in eyes of patients having OSA with different severity.

Abstract:Objective To assess the blood pressure variability(BPV) in acute infarction with obstructive sleep apnea syndrome(OSAS),and to explore the relationship between BPV and stroke. Methods We studied patients with acute cerebral infarction.All of the patients were admitted to the department of neurology,Beijing Haidian Hospital,from Jan.2009 to Jun.2010.Overnight polysomnography(PSG) and 24-hour ambulatory blood pressure monitoring recordings were performed on all patients within the first week from the onset of the ischemic stroke.According to the PSG,patients were divided into OSAS group(n=126) and non-OSAS group(n=51) except the patients with central and mixed sleep apnea syndrome and taking anti-hypertension drugs.BPV was expressed as the standard deviation of blood pressure(BP) during a 24 period.The aim of study is to determine the relationships in BP,BPV,the National Institutes of Health Stroke Scale(NIHSS), and so on between groups.Data was analyzed by spssl 1.5.We used for nominal variables the x2 test or t test.Significance was set at P<0.05.The correlations between the baseline data,clinical complications and recurrent stroke were studied. Results There were significantly higher daytime systolic(138.9±17.8 mmHg vs 131.4±15.8 mmH g,and P-value is less than 0.01),nighttime BPV(12.1±4.7 mmHg vs 8.8±3.7 mmHg,and P-value is less than 0.01) in the OSAS group as compared to the non-OSAS group.There were significantly differences in NIHSS and the number of recurrence stroke between two groups.Hypertention, diabetes mellitus,OSAS,apnea hypopnea index(AHI) and nighttime BPV were related to recurrent stroke(P<0.05).After adjustment for the other factors,nighttime BPV(OR 0.910,95%CI 0.855- 0.969,P=0.003) was a independent risk factor of recurrent stroke. Conclusion The nighttime-BPV was higher in the OSAS group as compared to the non-OSAS group. The BPV was one of the reasons of recurrence of stroke in patients with OSAS.

Abstract:Background and Objectives: Obstructive sleep apnea (OSA) is a known risk factor for chronic coronary syndrome (CCS). CCS and OSA are separately associated with significant changes in heart rate variability (HRV). In this proof-of-concept study, we tested whether HRV values are significantly different between OSA patients with concomitant severe CCS, and OSA patients without known CCS. Material and Methods: The study comprised a retrospective assessment of the historical and raw polysomnography (PSG) data of 32 patients who presented to a tertiary university hospital with clinical complaints of OSA. A total of 16 patients (four females, mean age 62.94 ± 2.74 years, mean body mass index (BMI) 31.93 ± 1.65 kg/m2) with OSA (median apnea-hypopnea index (AHI) 39.1 (30.5–70.6)/h) and severe CCS were compared to 16 patients (four females, mean age 62.35 ± 2.06 years, mean BMI 32.19 ± 1.07 kg/m2) with OSA (median AHI 40 (30.6–44.5)/h) but without severe CCS. The short–long-term HRV (in msec) was calculated based on the data of a single-lead electrocardiogram (ECG) provided by one full-night PSG, using the standard deviation of the NN, normal-to-normal intervals (SDNN) and the heart rate variability triangular index (HRVI) methods, and compared between the two groups. Results: A significant reduction (p < 0.05) in both SDNN and HRVI was found in the OSA group with CCS compared to the OSA group without CCS. Conclusions: Severe CCS has a significant impact on short–long-term HRV in OSA patients. Further studies in OSA patients with less-severe CCS may shed more light onto the involved mechanistic processes. If confirmed in future larger studies, this physiologic metric has the potential to provide a robust surrogate marker of severe CCS in OSA patients.

Ventilatory Burden Predicts Change in Sleepiness Following Positive Airway Pressure in Sleep Apnea

Abstract:Excessive daytime sleepiness, an important symptom of obstructive sleep apnea (OSA), is commonly quantified using the Epworth Sleepiness Scale score (ESS). Baseline OSA severity measures (ventilatory burden, flow limitation, and hypoxemia) provide insights into OSA pathophysiology and could predict changes in sleepiness (i.e. change-in-ESS) following continuous positive airway pressure (CPAP) treatment. We hypothesized that change-in-ESS following CPAP treatment can be predicted from baseline polysomnography. Associations between OSA severity measures and ESS were evaluated in 2332 participants, adjusting for age, sex, BMI, and total sleep time. Change-in-ESS prediction was evaluated using 213 CPAP treatment studies (HomePAP, BestAIR, and ABC) in three steps: severity measures were compared (adjusted regression, n=64), a prediction model was developed using baseline ventilatory burden and baseline ESS (n=139), and then evaluated in holdout participants (n=74). In cross-sectional analysis, ESS was associated with ventilatory burden (0.45 points/SD; 95% CI 0.23-0.67), hypoxic burden (0.39; 0.17-0.62), the apnea-hypopnea index (AHI) (0.36; 0.14-0.59), and flow limitation severity (0.22; 0.01-0.43). Comparison analysis revealed that change-in-ESS was most strongly associated with baseline ventilatory burden (-1.08 points/SD; -2.13 to -0.05) and baseline ESS (-2.75; -3.83 to -1.69); the AHI association was weaker (-0.97; -2.01-0.05). Predicted change-in-ESS and actual change-in-ESS were correlated in holdout participants (adjusted R^2=0.313); median [IQR] actual change-in-ESS of predicted responders (>=2-point ESS improvement, n=54, 73.0%) was -5.0 [-10.0 to -2.0] and non-responders was 0.0 [-1.0-1.0] (P<0.001). Baseline ventilatory burden and baseline ESS were independently associated with change-in-ESS and could be used together to inform clinicians whether CPAP treatment will likely improve a patient's sleepiness.

Abstract:RATIONALE Atmospheric pressure chemical ionization (APCI) sources operated with point to plane DC discharges ('Coronas') frequently suffer from point electrode degradation and potentially lead to oxidation and/or fragmentation of the generated analyte ions. It is postulated that these adverse effects are caused by the interaction of these ions with the discharge chemistry as well as en route to the mass analyzer region. METHODS The corona discharge metal point electrode is replaced by the conically shaped liquid effluent evolving from a fused-silica capillary, which is analogous but not identical to the Taylor cone formation in electrospray ionization. The liquid consisting of either pure water or water containing 0.1 %V formic acid is fed via a nano-flow delivery stage at typical flow rates between 1-800 μL/h. The liquid flow is continuously replenishing the surface of the point electrode. The source is directly coupled to the inlet capillary of appropriate mass spectrometers, e.g., the Bruker Daltonics and Agilent varieties. RESULTS The actively pumped liquid flow is supplying a constant amount of the reagent gas (H2O) to the corona region in the 20 ppmV to 30 %V range, leading to controlled, very stable operation of the source. The typical light emission observed for corona discharges is in very close proximity to the aqueous surface. Analyte protonation is the dominating ionization pathway. The degree of primary analyte fragmentation is extremely low. CONCLUSIONS We have developed a novel atmospheric pressure chemical ionization source designed for the hyphenation of nano-flow liquid chromatography and gas chromatography with atmospheric pressure ionization mass spectrometry. The proposed reaction mechanism including the electrochemistry occurring in the source along with formation of protonated analyte molecules via collision-induced dissociation (CID) is in full accord with the experimental results. The system exhibits an extremely stable performance over prolonged operation times, sole generation of protonated molecules, and low degree of analyte ion fragmentation.
[Electroencephalogram Spectral Power Analysis of Obstructive Sleep Apnea Syndrome Patients Before and During Continuous Positive Airway Pressure Therapy].

Abstract:OBJECTIVE:To investigate the instant effects of continuous positive airway pressure (CPAP) on EEG spectral power changes in obstructive sleep apnea syndrome (OSAS) patients. METHODS:26 OSAS patients were included. The diagnosis was made by polysomnography (PSG). The CPAP pressure was titrated during the first night of therapy. During another night in two weeks after the diagnostic study, the patients accepted the whole night CPAP treatment while PSG was monitored. C(3)/A(2) was analyzed by using fast fourier transform (FFT). Spectral edge frequency (SEF), median power frequency (MPF), alpha index, beta index, delta index and theta; index during CPAP therapy were compared with those parameters before treatment. RESULTS:Sleep architecture was improved significantly during therapy. The ratio of slow wave sleep (SWS) deficiency reduced from 19/26 to 10/26 (P = 0.0250). There were more times of REM sleep (1.81 +/- 0.25) vs (2.65 +/- 0.17) (P = 0.023). A significant increase of total SWS time over total sleep time (TST) (2.9 +/- 1.1)% vs (6.0 +/- 1.2)% (P = 0.043) was observed, as well as total REM sleep time over TST (12.0 +/- 1.7)% vs (21.1 +/- 1.6)% (P = 0.001). Total stage I and stage II time over TST reduced greatly [from (85.1 +/- 2.1)% to (73.0 +/- 1.9)% (P = 0.000)]. So did the number of wake after sleep onset [from (12.2 +/- 1.3) to (9.4 +/- 1.0) (P = 0.033). The total number of stage I, stage II decreased from (46.4 +/- 4.2) to (36.7 +/- 2.4), but the difference was not significant (P = 0.051). Spectral analysis of EEG showed a decrease of mean SEF in total sleep period (TSP) (14.4 +/- 0.4) Hz vs (13.6 +/- 0.3) Hz (P = 0.003), stage I (15.4 +/- 0.4) Hz vs (14.8 +/- 0.5) Hz (P = 0.040), stage II (13.7 +/- 0.3) Hz vs (12.8 +/- 0.3) Hz (P = 0.007) and REM sleep (15.0 +/- 0.5) Hz vs (13.8 +/- 0.6) Hz (P = 0.028), as well as 0.018 and 0.047]. delta index was increased in stage II sleep (P = 0.030) but not in other sleep stages. CPAP had no obvious influence on theta; index (P > 0.05). The median of SEF in TSP (14 vs 13) Hz (P = 0.0056), stage I (16 vs 15) Hz (P = 0.04) and stage II (13 vs 13) Hz (P = 0.002). Changes of MPF were not as significant as that of SEF, but mean MPF was decreased during TSP and stage IV. CPAP significantly reduced mean beta index [from (8.4 +/- 0.5)% to (7.5 +/- 0.5)% (P = 0.012)] in total sleep period as well as in stage I, II, III and IV sleep (P < 0.015). alpha index in TSP, stage III and IV sleep was decreased significantly (P = 0.045, 0.018 and 0.047 respectively). delta index was increased in stage II sleep (P = 0.030) but not in other sleep stages. CPAP had no obvious influence on theta; index (P > 0.05). CONCLUSIONS:CPAP has an acute effect in improving the sleep architecture and EEG power spectrum. The SEF, beta index and alpha index are more sensitive than MPF, delta index and theta; index to CPAP therapy. These changes may be related to the recovery of cerebral function in OSAS patients after CPAP therapy. More studies need to be conducted to investigate how these changes happen and their relation to daytime cerebral function.

Abstract:ObjectiveTo characterize the association pattern between blood pressure (BP), metabolism changes, and obstructive sleep apnea (OSA) severity within male OSA patients. MethodsThe association between systolic BP (SBP), diastolic BP (DBP), glucose, lipids, apnea-hypopnea index (AHI), oxygen desaturation index (ODI), and arousal index (ArI) was evaluated after adjustment for BMI and waist circumference/hip circumference ratio (WHR) in 1,370 male OSA patients. ResultsIn the multiple linear regression models using SBP as an independent variable, SBP did not associate with the increase of any OSA indexes. BMI and glucose positively associated with AHI, ODI, and ArI elevation. WHR was also positively associated with increasing AHI and ODI. Total cholesterol levels increased with ODI and ArI increases. Triglyceride was associated with ArI. In the multiple linear regression models using DBP as an independent variable, DBP associated universally with AHI, ODI, and ArI with stable coefficients ranging from 0.19 to 0.20. The remaining independent variables were associated with AHI, ODI, and ArI with a similar trend to the models including SBP as an independent variable. ConclusionsApnea-hypopnea, hypoxemia, and arousal changed glycometabolism, fat metabolism, and BP profoundly in a particular pattern.

Abstract:Obstructive Sleep Apnea (OSA) is a common sleep disorder which causes poor sleep quality, daytime drowsiness, memory loss, and lack of attention, and can contribute to accidents. Long-term OSA is associated with cardiovascular disease, high blood pressure, arrhythmia and diabetes. Early discovery and treatment of OSA can significantly reduce the difficulty and cost of treatment. OSA detection uses a scale method which is prone to subjective errors. Polysomnography (PSG) requires long detection times, entails equipment which can be difficult to obtain and high cost, and does not provide immediate diagnosis or the establishment of preventative treatment. Human biosignals have been found to provide a faster and more objective means of sleep apnea diagnosis, allowing for the early treatment of OSA. By identifying key physiological characteristics and observing their change over time, one can assess the severity of the patient’s condition, and appropriate treatment can reduce the likelihood of disease onset or slow its progression. This study applies machine learning techniques to electroencephalography (EEG), electrocardiography (ECG), oxygen saturation (SpO2) and other complex physiological characteristics to establish a model for OSA severity and evaluate the effectiveness of various machine learning methods to detect sleep apnea. In addition, the fuzzy C-mean (FCM) algorithm is used to construct a predictive model for OSA to assist physicians in the early clinical detection of symptoms. The study found that patients with severe sleep apnea have a self-rescue condition which causes wakening due to respiratory arrest. Only relying on SpO2 and the Apnea Hypopnea Index (AHI) will cause severe OSA cases to be misidentified as mild, and this study found that the A5 band brain wave can help detect sleep apnea, reducing the possibility of miscategorization and assisting physicians in accurate clinical diagnosis.

Abstract:Abstract This study aimed to investigate the effect of a single night of continuous positive airway pressure (CPAP) treatment on spontaneous brain activity and the underlying neuropathological mechanisms in patients with severe obstructive sleep apnea (OSA). The study involved 30 severe OSA patients and 19 healthy controls (HC). Fractional amplitude of low-frequency fluctuation (fALFF) and regional homogeneity (ReHo) methods were employed to evaluate spontaneous brain activity in all participants. Following a single night of CPAP treatment, ReHo values increased in the bilateral caudate and decreased in the right superior frontal gyrus. The fALFF values increased in the left orbital part of the middle frontal gyrus and the right orbital of the inferior frontal gyrus (Frontal_Inf_Orb_R). However, fALFF values decreased in the medial part of the left superior frontal gyrus and the right supramarginal part of the inferior parietal lobe. Pearson correlation analysis revealed a positive relationship between the change in the fALFF in the Frontal_Inf_Orb_R and the change in REM sleep duration ( r = 0.437, p = 0.016) following a single night of CPAP treatment. We concluded that observing changes in abnormal fALFF and ReHo in OSA patients before and after a single night of CPAP treatment may enhance our understanding of the neurological mechanisms in patients with severe OSA.

Abstract:Purpose: Obstructive sleep apnea syndrome (OSAS) has mostly been examined using in-laboratory polysomnography (Lab-PSG), which may overestimate severity. This study compared sleep parameters in different environments and investigated the association between the plasma levels of neurochemical biomarkers and sleep parameters. Methods: Thirty Taiwanese participants underwent Lab-PSG while wearing a single-lead electrocardiogram patch. Participants' blood samples were obtained in the morning immediately after the recording. Participants wore the patch for the subsequent three nights at home. Sleep disorder indices were calculated, including the apnea-hypopnea index (AHI), chest effort index, and cyclic variation of heart rate index (CVHRI). The 23 eligible participants' derived data were divided into the normal-to-moderate (N-M) group and the severe group according to American Association of Sleep Medicine (AASM) guidelines (Lab-PSG) and the recommendations of a previous study (Rooti Rx). Spearman's correlation was used to examine the correlations between sleep parameters and neurochemical biomarker levels. Results: The mean T-Tau protein level was positively correlated with the home-based CVHRI (r = 0.53, p < 0.05), whereas no significant correlation was noted between hospital-based CVHRI and the mean T-tau protein level (r = 0.25, p = 0.25). The home-based data revealed that the mean T-Tau protein level in the severe group was significantly higher than that in the N-M group (severe group: 24.75 ± 6.16 pg/mL, N-M group: 19.65 ± 3.90 pg/mL; p < 0.05). Furthermore, the mean in-hospital CVHRI was higher than the mean at-home values (12.16 ± 13.66 events/h). Conclusion: Severe OSAS patients classified by home-based CVHRI demonstrated the higher T-Tau protein level, and CVHRI varied in different sleep environments.

Abstract:Obstructive sleep apnea (OSA) is a significant risk factor for cardiovascular disease (CVD) with increasing prevalence. An important mechanism of CVD development is a dysregulation of the autonomic nervous system (ANS). This prospective and controlled cohort study aimed to investigate ANS function in OSA including the response to long-term continuous positive airway pressure (CPAP) therapy by analyzing 24 h Holter electrocardiogram and 24 h Holter ambulatory blood pressure recording parameters. The study enrolled 57 patients who were newly diagnosed with severe OSA. After 6 months of CPAP therapy, 37 patients had a good therapy adherence (usage of CPAP device >4 h per night), and their data were analyzed. The difference in nocturnal diastolic blood pressure values before and after CPAP therapy reached statistical significance (76 (68–84) vs. 74 (63–80) mmHg, p = 0.0439). Lower nocturnal values after CPAP therapy of SDNN (101.5 vs. 95 ms, p = 0.0492) and RMSSD (29.5 vs. 26 ms, p = 0.0193) were found. An increase in diurnal spectral power (1742 vs. 2112 ms2, p = 0.0282) and a decrease in nocturnal spectral power (3256 vs. 2124 ms2, p = 0.0097), nocturnal VLF band (2493 vs. 1485.4 ms2, p = 0.0176), nocturnal LF band (638.7 vs. 473 ms2, p = 0.0097), and nocturnal HF band (234.9 vs. 135.7 ms2, p = 0.0319) was found. The results showed an imbalance of the ANS with a sympathetic predominance, especially during the night hours and in those with arterial hypertension. The impact of CPAP therapy on the improvement in ANS parameters was more pronounced at night, in men, and those with arterial hypertension.

Abstract:STUDY OBJECTIVES: To investigate changes in regional cerebral blood flow (rCBF) in patients with obstructive sleep apnea-hypopnea syndrome (OSAHS).DESIGN: We compared the 99mTc-ethylcysteinate dimer (ECD) single photon emission computed tomography (SPECT) images of patients with OSAHS with those of age- and sex-matched healthy volunteers.SETTING: University hospital.PATIENTS AND PARTICIPANTS: Twenty-seven patients with severe OSAHS and 27 healthy volunteers underwent 99mTc-ECD brain SPECT studies.INTERVENTION: For statistical parametric mapping analysis, all SPECT images were spatially normalized to the standard SPECT template and then smoothed using a 14-mm full-width at half-maximum Gaussian kernel. The Student t test was used for the statistical analysis.MEASUREMENTS AND RESULTS: The mean age of patients and subjects was 44.3 years (range 31-58). All patients underwent overnight polysomnography. The mean apnea-hypopnea index of patients was 60.4 +/- 17.6 per hour (range 33 -104), indicating severe OSAHS. All patients snored heavily and had daytime sleepiness (mean Epworth Sleepiness Scale score, 10.7 +/- 3.7, range 6-12). Statistical parametric mapping analysis showed that rCBF in patients with OSAHS was significantly reduced in bilateral parahippocampal gyri and in the right lingual gyrus, as compared with that of healthy volunteers (P < 0.05 with false discovery rate correction). Moreover, apnea-hypopnea indexes of patients were negatively correlated with rCBF in the right pericentral gyrus and right cuneus at uncorrected P < 0.001.CONCLUSIONS: Our results show the altered rCBF pattern in bilateral parahippocampal gyri, right lingual gyrus, pericentral gyrus, and cuneus in patients with severe OSAHS. These findings may partly explain the deficit in memory, spatial learning, executive function, and attention, which are frequently found in patients with OSAHS.

Abstract:STUDY OBJECTIVES Patients with obstructive sleep apnea (OSA) exhibit heterogeneous heart rate variability (HRV) during wakefulness and sleep. We investigated the influence of OSA severity on HRV parameters during wakefulness in a large international clinical sample. METHODS 1247 subjects (426 without OSA and 821 patients with OSA) were enrolled from the Sleep Apnea Global Interdisciplinary Consortium. HRV parameters were calculated during a 5-minute wakefulness period with spontaneous breathing prior to the sleep study, using time-domain, frequency-domain and non-linear methods. Differences in HRV were evaluated among groups using analysis of covariance, controlling for relevant covariates. RESULTS Patients with OSA showed significantly lower time-domain variations and less complexity of heartbeats compared to individuals without OSA. Those with severe OSA had remarkably reduced HRV compared to all other groups. Compared to non-OSA patients, those with severe OSA had lower HRV based on SDNN (adjusted mean: 37.4 vs. 46.2 ms; p<0.0001), RMSSD (21.5 vs. 27.9 ms; p<0.0001), ShanEn (1.83 vs. 2.01; p<0.0001), and Forbword (36.7 vs. 33.0; p=0.0006). While no differences were found in frequency-domain measures overall, among obese patients there was a shift to sympathetic dominance in severe OSA, with a higher LF/HF ratio compared to obese non-OSA patients (0.97 vs. 0.63; p=0.009). CONCLUSIONS Time-domain and non-linear HRV measures during wakefulness were associated with OSA severity, with severe patients having remarkably reduced and less complex HRV. Frequency-domain measures show a shift to sympathetic dominance only in obese OSA patients. Thus, HRV during wakefulness could provide additional information about cardiovascular physiology in OSA patients.

Abstract:OBJECTIVE:To investigate whether repeated arousals and hypoxia at the end of apnea contribute to nocturnal and diurnal elevation of blood pressure in obstructive sleep apnea syndrome (OSAS) patients with hypertension.METHODS:30 OSAS patients with hypertension and 30 OSAS patients without hypertension were studied. Movement arousal index (MAI), cortical arousal index (CAI), length of time during which nocturnal oxygen saturation decreased below 90% (OLT90%), apnea/hypopnea index (AHI), 24 h average systolic and diastolic blood pressure (ASBP and ADBP) were measured. Multiple stepwise regression analysis was performed to find the correlation between 24 h ASBP, ADBP and MAI, CAI, OLT90%, AHI, age, body mass index in OSAS patients with hypertension.RESULTS:MAI made the most significant contribution to 24 h ASBP and ADBP in OSAS patients with hypertension (ASBP: r = 0.485, P < 0.01; ADBP: r = 0.407, P < 0.05). OLT90% was the second most important contributing factor (ASBP: r = 0.382, P < 0.05; ADBP: r = 0.369, P < 0.05).CONCLUSION:Movement arousal and hypoxia may make an important contribution to circardian elevation of blood pressure in OSAS patients with hypertension. Both of them may contribute to the development of nocturnal and diurnal hypertension in patients with OSAS.

Abstract:BACKGROUND: Obstructive sleep apnea (OSA) is associated with an increased risk of cardiovascular complications. OSA and coronary artery disease (CAD) share the same risk factors and coexist in many patients. In previous studies, repeated nocturnal cardiac ischemic events in OSA patients with CAD have been reported.HYPOTHESIS: We hypothesized that OSA may precipitate myocardial ischemia, evidenced by ST-segment depression and elevated N-terminal brain natriuretic peptide (NT-proBNP) and high-sensitivity troponin T (hs-TropT) levels in patients with severe OSA and concomitant CAD. We also aimed to evaluate if the effects could be reversed by continuous positive airway pressure (CPAP) therapy.METHODS: Twenty-one patients with severe OSA (apnea-hypopnea index >15/h, nadir oxygen desaturation ≤ 80%), and coexisting CAD underwent in-hospital polysomnography at baseline and under CPAP. Blood samples for hs-TropT and NT-proBNP measurements were drawn prior and immediately after sleep. ST-segment depression was measured at the time of maximum oxygen desaturation during sleep.RESULTS: CPAP significantly decreased elevated NT-proBNP levels from 475 ± 654 pg/mL before sleep to 353 ± 573 pg/mL after sleep and attenuated ST-segment depression during sleep. hs-TropT was not elevated and did not differ after nocturnal oxygen desaturation at baseline and after CPAP.CONCLUSIONS: CPAP significantly reduced NT-proBNP in patients suffering from severe OSA and coexisting CAD. Repeated nocturnal myocardial ischemia did not cause myocyte necrosis evidenced by elevated hs-TropT or ST-segment depression.
