Abstract:Background: Person-centered language places a person's identity before any disability or medical condition they may have. Using person-centered language reduces stigma and improves the patient-physician relationship, potentially optimizing health outcomes. Patients with psoriasis often feel stigmatized due to their chronic skin condition. Objective: We seek to evaluate the use of person-centered language in psoriasis literature and to explore whether certain article characteristics were associated with non-person-centered language. Methods: We performed a systematic search on PubMed for recently published articles in journals that regularly publish psoriasis studies. After article reduction procedures, randomization, and screening, we reached our target sample of 400 articles. The following non-person-centered language terms were extracted from each article: "Psoriasis Patient," "Psoriasis subject," "Affected with," "Sufferer," "Suffering from," "Burdened with," "Afflicted with," and "Problems with." Screening and data extraction occurred in a masked duplicate fashion. Results: Of the 400 included articles, 272 (68%) were not adherent to person-centered language guidelines according to the American Medical Association Manual of Style. The most frequent non-person-centered language term was "Psoriasis Patient," found in 174 (43.5%) articles. The stigmatizing language was associated with the type of article and funding status, with original investigations and funded studies having higher rates of stigmatizing language. Conclusions: Articles about psoriasis commonly use non-person-centered language terms. It is important to shift away from using stigmatizing language about patients with psoriasis to avoid potential untoward influences. We recommend using "patients with psoriasis" or "patient living with psoriasis" to emphasize the importance of person-centered care.

Abstract:Bovine adrenocortical cells in primary culture express the basic fibroblast growth factor (bFGF) gene and their proliferation is stimulated by this growth factor. We report here the characterization of bFGF receptors on these cells. Binding studies revealed the presence of two bFGF receptor types: a limited number (4,300 sites/cell) of high affinity sites (Kd congruent to 2 pM) and a larger number (230,000 sites/cell) of lower affinity sites (Kd congruent to 400 pM). Cross-linking of 125I-bFGF to adrenocortical cells revealed two bands at 145 kDa and 125 kDa which are attributed to molecular complexes between the high affinity receptors and their ligand. These high affinity receptors possess N-linked carbohydrate chains that are important for proper cell surface expression but are devoid of glycosaminoglycan chains. The low-affinity (2 M NaCl-sensitive) binding sites are totally degraded by heparitinase treatment of adrenocortical cells indicating that low-affinity sites are borne by heparan sulfate proteoglycans. However, heparitinase treatment also reduced partially the binding of bFGF to high-affinity (2 M NaCl-resistant) sites. This argues for a contribution of heparan sulfate proteoglycans to the binding of bFGF to high-affinity receptors. Exogenous soluble heparin or heparan sulfate did not restore normal high affinity bFGF binding onto heparitinase-treated cells, suggesting that heparan sulfate proteoglycans either must be membrane-anchored or must contain specific structural features to enhance FGF binding to high-affinity receptors. Taken together with previous reports, this work supports the hypothesis that bFGF may act as an autocrine growth factor in the adrenal cortex.
Person- and identity-first language in autism research: A systematic analysis of abstracts from 11 autism journals

Abstract:Autism, Ahead of Print. We examined the use of person- and identity-first language (PFL, IFL) in scholarly writing about autism by reviewing 12,962 journal abstracts from 11 autism research journals (mostly covering the years 2001–2022). We found a preference for PFL (64.68%) over IFL (15.83%) when considering aggregated, within-journal breakdowns (with abstracts using both representing 19.50%). However, when examining language use longitudinally, we observed a general trend toward increasing use of IFL after years of stable trends showing predominant use of PFL. These trends were not consistent across all reviewed journals, as some journals demonstrated preference for either PFL or IFL across all observed years. Notably, we highlight the Journal of Autism and Developmental Disorders as a special case that offers a 51-year perspective (1971–2022); here, IFL was predominant until the 1990s, when PFL became a stable majority, and in the late 2010s, IFL started to increase. Journal differences in aggregate breakdowns and longitudinal trends are discussed in line with recent policy changes, journal submission guidelines, and shifting perspectives in autism research. Implications for writing about autistic individuals and the role that language plays within the broader autism research scholarly community are discussed.Lay There are many ways to refer to an individual who is on the autism spectrum. A recommended approach has been to use person-first language (PFL), such as "person with autism." A different approach is to use identity-first language (IFL), such as "autistic person." Recent studies focused on different groups of people (e.g. autistic self-advocates, parents, and practitioners) show that some groups prefer PFL (practitioners) while others prefer IFL (autistic self-advocates). However, less is known about how researchers use PFL and IFL in academic writing (e.g. studies published in scientific journals) involving autistic research participants. Our study examined 12,962 journal abstracts (short summaries of scientific articles) from 11 academic journals that publish autism research findings. We wanted to know (a) about the use of PFL and IFL across abstracts, and (b) how PFL and IFL use has changed annually over time. We examined data for all journals individually and grouped together. Our findings showed that journal abstracts generally use PFL (65%) with some using either IFL (16%) or both PFL and IFL (20%). However, journals varied, with some showing a clear majority for PFL and a couple for IFL. Examining trends over time across journals showed that while PFL appeared to be the majority for most journals, IFL has steadily increased in the recent few years. Our study helps us understand how autism researchers write about autistic individuals and offers implications for helping researchers intentionally make choices about the language used in their autism research studies.

Participants reported perceived personal responsibility for obesity, and 3 operationalizations of obesity stigma: prejudice, stereotypes, and support for punitive policies. Mediation analyses were used to test if the manipulation affected obesity stigma, through perceived personal responsibility.ResultsThere was no indirect effect of PFL vs IFL on the 3 outcomes (95% CIs contained zero). However, the indirect effects of PFL vs no-obesity condition were significant (prejudice: β = −0.10, SE = 0.05, 95% CI [−0.22, −0.01]; stereotypes: (β = 0.07, SE = 0.03, 95% CI [0.01, 0.14]); punitive punishment: (β = −0.06, SE = 0.04, 95% CI [−0.15, −0.01]). Also, the indirect effects of IFL vs no-obesity condition on stereotypes (β = 0.07, SE = 0.04, 95% CI [0.0003, 0.15]) and punitive punishment (β = −0.06, SE = 0.04, 95% CI [−0.15, −0.0002]) were significant.ConclusionPFL may not affect obesity stigma as it does in the context of other marginalized groups. The effect of PFL and IFL, compared to the no-obesity condition, suggests future routes for intervention.

Words Matter: Reducing Stigma in Online Conversations about Substance Use with Large Language Models

Abstract:Stigma is a barrier to treatment for individuals struggling with substance use disorders (SUD), which leads to significantly lower treatment engagement rates. With only 7% of those affected receiving any form of help, societal stigma not only discourages individuals with SUD from seeking help but isolates them, hindering their recovery journey and perpetuating a cycle of shame and self-doubt. This study investigates how stigma manifests on social media, particularly Reddit, where anonymity can exacerbate discriminatory behaviors. We analyzed over 1.2 million posts, identifying 3,207 that exhibited stigmatizing language towards people who use substances (PWUS). Using Informed and Stylized LLMs, we develop a model for de-stigmatization of these expressions into empathetic language, resulting in 1,649 reformed phrase pairs. Our paper contributes to the field by proposing a computational framework for analyzing stigma and destigmatizing online content, and delving into the linguistic features that propagate stigma towards PWUS. Our work not only enhances understanding of stigma's manifestations online but also provides practical tools for fostering a more supportive digital environment for those affected by SUD. Code and data will be made publicly available upon acceptance.

Abstract:Background: People who use drugs face entrenched stigma, which fosters shame, restricts service access, and exacerbates inequalities. The use of mass media in anti-stigma interventions offers an opportunity to challenge stigmatising attitudes at scale. There are, however, inconsistencies in messaging approaches used in mass media anti-stigma interventions, and how authors conceptualise and measure 'stigma'. Methods: This scoping review maps literature on the development and/or evaluation of mass media interventions intended to reduce stigma towards people who use drugs. We systematically searched seven databases for reports about: (i) people who use drugs, (ii) stigma, (iii) mass media. We charted data about intervention (i) subjects and recipients, (ii) format, (iii) authors, (iv) content; and (v) conceptualisation and measurement of stigma. We narratively synthesised findings with qualitative content analyses. Results: From 14,256 records, we included 49 reports about 35 interventions. 25/35 were from the last five years and 19/35 were from the United States. Intended recipients included the public and/or specified sub-populations, often including healthcare workers. Most interventions were intended to reduce stigma towards people with patterns of drug use perceived to be problematic, as opposed to people who use drugs in general. Interventions ranged from single pieces of media to complex multi-format campaigns. People who use(d) drugs contributed to 22/35 interventions. Professionals working in medical disciplines co-authored 29/35 interventions. Intervention content often had a medical focus, describing dependence as a 'disease' or medical issue, and emphasised the benefits of recovery. Other interventions, however, criticised medical framings. In some interventions drug use and people who use drugs were described in markedly negative terms. 'Stigma' was often under-theorised, and measurement approaches were inconsistent, with 42 instruments used to measure phenomena associated with stigma across 19 quantitative evaluations. Conclusion: We found inconsistencies in approaches to reduce and measure stigma, potentially reflecting different motivations for intervention development. The primary motivation of many interventions was seemingly to promote drug service engagement and recovery.
Lived Experience Matters: Automatic Detection of Stigma on Social Media Toward People Who Use Substances

Abstract:Stigma toward people who use substances (PWUS) is a leading barrier to seeking treatment.Further, those in treatment are more likely to drop out if they experience higher levels of stigmatization. While related concepts of hate speech and toxicity, including those targeted toward vulnerable populations, have been the focus of automatic content moderation research, stigma and, in particular, people who use substances have not. This paper explores stigma toward PWUS using a data set of roughly 5,000 public Reddit posts. We performed a crowd-sourced annotation task where workers are asked to annotate each post for the presence of stigma toward PWUS and answer a series of questions related to their experiences with substance use. Results show that workers who use substances or know someone with a substance use disorder are more likely to rate a post as stigmatizing. Building on this, we use a supervised machine learning framework that centers workers with lived substance use experience to label each Reddit post as stigmatizing. Modeling person-level demographics in addition to comment-level language results in a classification accuracy (as measured by AUC) of 0.69 -- a 17% increase over modeling language alone. Finally, we explore the linguist cues which distinguish stigmatizing content: PWUS substances and those who don't agree that language around othering ("people", "they") and terms like "addict" are stigmatizing, while PWUS (as opposed to those who do not) find discussions around specific substances more stigmatizing. Our findings offer insights into the nature of perceived stigma in substance use. Additionally, these results further establish the subjective nature of such machine learning tasks, highlighting the need for understanding their social contexts.

Abstract:Parents with substance use disorders are highly stigmatized by multiple systems (e.g., healthcare, education, legal, social). As a result, they are more likely to experience discrimination and health inequities [1, 2]. Children of parents with substance use disorders often do not fare any better, as they frequently experience stigma and poorer outcomes by association [3, 4]. Calls to action for person-centered language for alcohol and other drug problems have led to improved terminology [5,6,7,8]. Despite a long history of stigmatizing, offensive labels such as "children of alcoholics" and "crack babies," children have been left out of person-centered language initiatives. Children of parents with substance use disorders can feel invisible, shameful, isolated, and forgotten—particularly in treatment settings when programming is centered on the parent [9, 10]. Person-centered language is shown to improve treatment outcomes and reduce stigma [11, 12]. Therefore, we need to adhere to consistent, non-stigmatizing terminology when referencing children of parents with substance use disorders. Most importantly, we must center the voices and preferences of those with lived experience to enact meaningful change and effective resource allocation.

Physician assessments of drug seeking behavior: A mixed methods study

Abstract:Background: Pain complaints are common, but clinicians are increasingly concerned about overuse of opioid pain medications. This may lead patients with actual pain to be stigmatized as "drug-seeking," or attempting to obtain medications they do not require medically. We assessed whether patient requests for specific opioid pain medication would lead physicians to classify them as drug-seeking and change management decisions. Methods and findings: Mixed-methods analysis of interviews with 192 office-based primary care physicians after viewing video vignettes depicting patients presenting with back pain. For each presentation physicians were randomly assigned to see either an active request for a specific medication or a more general request for help with pain. The main outcome was assignment by the physician of "drug-seeking" as a potential diagnosis among patients presenting with back pain. Additional outcomes included other actions the physician would take and whether the physician would prescribe the medication requested. A potential diagnosis of drug-seeking behavior was included by 21% of physicians seeing a specific request for oxycodone vs. 3% for a general request for help with back pain(p<0.001). In multivariable models an active request was most strongly associated with a physician-assigned diagnosis of drug-seeking behavior(OR 8.10; 95% CI 2.11-31.15;p = 0.002); other major patient and physician characteristics, including gender and race, did not have strong associations with drug-seeking diagnosis. Physicians described short courses of opioid medications as a strategy for managing patients with pain while avoiding opioid overuse. Conclusions: When patients make a specific request for opioid pain medication, physicians are far more likely to suspect that they are drug-seeking. Physician suspicion of drug-seeking behavior did not vary by patient characteristics, including gender and race. The strategies used to assess patients further varied widely. These findings indicate a need for the development of better clinical tools to support the evaluation and management of patients presenting with pain.

Abstract:Stigma facilitates negative outcomes for those with substance use disorders (SUDs), such as restricted healthcare access, job loss, and social ostracization. Many professionals advocate for reducing stigmatizing language in their conduct. However, there remains a notable degree of stigmatizing language in self-report measures. Thus, I aimed to examine psychometric changes from replacing stigmatizing language in self-report measures and test if these changes affect the respondents' stigmatizing beliefs. I conducted a randomly assigned double-masked experiment with undergraduates ( n = 125; M age = 19.18, SD = 1.05; 62% female; 58% White), Reddit users ( n = 224; M age = 31.28, SD = 9.21; 58% male; 82% White), and Buddhist Recovery Network members ( n = 136; M age = 50.48, SD = 11.50; 61% male; 94% White). The participants completed either the original version of the Addictions Belief Inventory consisting of stigmatizing terms (e.g., drug abuser) or a modified version consisting of non-stigmatizing terms (e.g., those with SUDs). I found no changes to the psychometric properties when I compared the factor structures, distributions, and construct validities. There was a minor reduction in one subscale's internal consistency and a large change in the mean scores, which varied by sample type. Removing stigmatizing language did not change the measure's psychometric properties notably. However, I found that removing stigmatizing language reduces stigmatizing beliefs and that this reduction was substantial, especially for those who have no history of SUDs, such as some undergraduates or Reddit users. These findings support an ongoing effort to reduce stigmatizing language among professionals and academics.

Abstract:Stigma associated with alcohol use disorder (AUD) and alcohol-associated liver disease (ALD) can cause delayed disease detection and intervention and may influence liver transplant allocation. 1 Although multiple societies recommended use of nonstigmatizing language, 2 -4 it is unclear how well these recommendations are followed in materials from transplant centers. Although stigma surrounding AUD and ALD is multifaceted, we investigated the adoption of these language recommendations among liver transplant centers in the US. We conducted a cross-sectional study systematically reviewing accredited US liver transplant centers and addiction psychiatry websites at the same institutions. Pediatric centers were excluded due to their low ALD rates. These websites were evaluated by 2 independent reviewers (R.M. and A.O.) for use of stigmatizing language ("alcoholism," "alcoholic," "alcohol abuse") and nonstigmatizing language ("alcohol use disorder," "alcohol misuse," "alcohol associated," "alcohol related"). Websites using both types were categorized as using mixed language. Language use was analyzed with a 2-sided χ 2 test with P < .05 considered significant, and interrater reliability was measured with the Cohen κ. The STROBE reporting guideline was followed. Data were obtained from public websites, and no institutional review board review was needed per the Common Rule. Of 114 liver transplant centers reviewed, 82 (71.9%) described 1 or more of the following: AUD, ALD, alcohol-associated hepatitis, and alcohol-associated cirrhosis. Of the 104 addiction psychiatry websites reviewed, 39 (37.5%) described AUD while none mentioned liver diseases. The Cohen κ coefficient was 0.731. Seventy-two of the 82 transplant websites (87.8%) and 18 of the 39 addiction psychiatry websites (46.2%) used stigmatizing language. For AUD, only stigmatizing language was used 79.2% of the time (42 of 53) compared with 20.8% (11 of 53) for only nonstigmatizing language and 0% for mixed language. This was significantly higher than on addiction psychiatry websites, which used only stigmatizing language at a rate of 30.8% (12 of 39; P < .001). Of 60 centers that described AUD on their own and corresponding addiction psychiatry department websites, 19 (31.7%) had consistent language use between the 2. For ALD (n = 60), 66.7% of websites (n = 40) used only stigmatizing language, 20.0% (n = 12) used only nonstigmatizing language, and 13.3% (n = 8) used mixed language. When discussing alcohol-associated hepatitis (n = 47), 95.7% of websites (n = 45) used only stigmatizing language, while 2.1% (n = 1) used only nonstigmatizing language, and 2.1% (n = 1) used mixed language. For alcohol-associated cirrhosis (n = 28), 85.7% of websites (n = 24) used "alcoholic cirrhosis," while 10.7% (n = 3) consistently used nonstigmatizing language and 3.6% (n = 1) used mixed language (Figure). Our findings reveal limited and inconsistent adoption of nonstigmatizing language for AUD and ALD across transplant websites compared with addiction psychiatry websites within the same institutions, suggesting the variability is not due to a system-wide policy but is center specific. This disparity underscores the need for the use of more patient-centered language in transplant centers to reduce stigma. The slow adoption of nonstigmatizing language may be due to a lack of awareness about its association with health care and resistance to change. Patients increasingly seek health-related information online, which can significantly influence behavior. 5 ,6 Although it is encouraging that academic societies advocate for nonstigmatizing language to describe ALD, our results call for hospital systems to reassess and update their language to align with nonstigmatizing terminology, which may enhance patient care and improve patients' willingness for treatments. This study's limitations include the challenge in defining stigmatizing language comprehensively. Although we did not encompass all stigmatizing language for ALD, our study shows more frequent use of terms such as alcoholic , alcoholism , and alcohol abuse on transplant websites compared with psychiatry counterparts. Abstract:Addictive substances are prevalent world-wide, and their use presents a substantial and persistent public health problem. A wide range of digital interventions to decrease use and negative consequences thereof have been explored, differing in approach, theoretical grounding, use of specific technologies, and more. The current study was designed to comprehensively map the recent (2015-2022) extant literature in a systematic manner, and to identify neglected and emerging knowledge gaps. Four major databases (Medline, Web of Science Core Collection, and PsychInfo) were searched using database-specific search strategies, combining terms related to clinical presentation (alcohol, tobacco or other drug use), technology and aim. After deduplication, the remaining n=13,917 unique studies published were manually screened in two stages, leaving a final n=3,056 studies, the abstracts of which were subjected to a tailored coding scheme. Findings revealed an accelerating rate of publications in this field, with randomized trials being the most common study type. Several meta-analyses on the topic have now been published, revealing promising and robust effects. Digital interventions are being offered on numerous levels, from targeted prevention to specialized clinics. Detailed coding was at times made difficult by inconsistent use of specific terms, which has important implications for future meta-analyses. Moreover, we identify several gaps in the extant literature – few health economic assessments, unclear descriptions of interventions, weak meta-analytic support for some type of interventions, and limited research on many target groups, settings and new interventions like video calls, chatbots and artificial intelligence – that we argue are important to address in future research.

High-risk polysubstance use among LGBTQ+ people who use drugs in the United States: An application of syndemic theory

Abstract:Background: Compared to heterosexual and cisgender people, lesbian, gay, bisexual, transgender, queer, and other sexual and gender minority (LGBTQ+) people are more likely to develop problems with high-risk polysubstance use. According to syndemic theory, this disparity in high-risk polysubstance use is produced by the LGBTQ+ community's increased vulnerability to experiencing psychosocial (e.g., discrimination, unwanted sex) and structural (e.g., food insecurity, homelessness) conditions, greater likelihood of coping with concurrent health problems (e.g., human immunodeficiency virus [HIV]), and decreased opportunities to develop protective factors (e.g., social support, resilience). Methods: Data from 306 LGBTQ+ participants living in the United States (U.S.) with a lifetime history of alcohol and drug use were analyzed; 21.2% reported lifetime problems with 10 different drugs. Bootstrapped hierarchical multiple regression was used to test demographic correlates and syndemic predictors of high-risk polysubstance use. One-way ANOVA and post-hoc comparison tests were used to test subgroup differences by gender. Results: Results indicated that income, food insecurity, sexual orientation-based discrimination, and social support were associated with high-risk polysubstance use, explaining 43.9% of the variance of high-risk polysubstance use. Age, race, unwanted sex, gender identity-based discrimination, and resilience were not significant. Group comparison tests revealed that, compared to nonbinary people and cisgender sexual minority men and women, transgender individuals experienced significantly higher levels of high-risk polysubstance use and sexual orientation-based discrimination but significantly lower levels of homelessness and social support. Conclusion: This study provided further evidence for conceptualizing polysubstance use as an adverse outcome of syndemic conditions. Harm reduction strategies, anti-discrimination laws, and gender-affirming residential treatment options should be considered in U.S. drug policy. Clinical implications include targeting syndemic conditions to reduce high-risk polysubstance use among LGBTQ+ people who use drugs.

Abstract:Background: People with a history of injection drug use face discrimination in healthcare settings that may impede their use of routine care, leading to greater reliance on the emergency department (ED) for addressing health concerns. The relationship between discrimination in healthcare settings and subsequent ED utilization has not been established in this population. Methods: This analysis used longitudinal data collected between January 2014 and March 2020 from participants of the ALIVE (AIDS Linked to the IntraVenous Experience) study, a community-based observational cohort study of people with a history of injection drug use in Baltimore, Maryland. Logistic regressions with generalized estimating equations were used to estimate associations between drug use-related discrimination in healthcare settings and subsequent ED utilization for the sample overall and six subgroups based on race, sex, and HIV status. Results: 1,342 participants contributed data from 7,289 semiannual study visits. Participants were predominately Black (82%), mostly male (66%), and 33% were living with HIV. Drug use-related discrimination in healthcare settings (reported at 6% of study visits) was positively associated with any subsequent ED use (OR = 1.40, 95% CI: 1.15–1.72). Positive associations persisted after adjusting for covariates, including past sixth-month ED use and drug use, among the overall sample (aOR = 1.28, 95% CI: 1.04–1.59) and among some subgroups. Conclusions: Drug use-related discrimination in healthcare settings was associated with greater subsequent ED utilization in this sample. Further exploration of mechanisms driving this relationship may help improve care and optimize healthcare engagement for people with a history of injection drug use.

Practices and Barriers in Developing and Disseminating Plain-Language Resources Reporting Medical Research Information: A Scoping Review

Abstract:Background: The intent of plain-language resources (PLRs) reporting medical research information is to advance health literacy among the general public and enable them to participate in shared decision-making (SDM). Regulatory mandates coupled with academic and industry initiatives have given rise to an increasing volume of PLRs summarizing medical research information. However, there is significant variability in the quality, format, readability, and dissemination channels for PLRs. In this scoping review, we identify current practices, guidance, and barriers in developing and disseminating PLRs reporting medical research information to the general public including patients and caregivers. We also report on the PLR preferences of these intended audiences. Methods: A literature search of three bibliographic databases (PubMed, EMBASE, Web of Science) and three clinical trial registries (NIH, EMA, ISRCTN registry) was performed. Snowball searches within reference lists of primary articles were added. Articles with PLRs or reporting topics related to PLRs use and development available between January 2017 and June 2023 were identified. Evidence mapping and synthesis were used to make qualitative observations. Identified PLRs were quantitatively assessed, including temporal annual trends, availability by field of medicine, language, and publisher types. Results: A total of 9116 PLRs were identified, 9041 from the databases and 75 from clinical trial registries. The final analysis included 6590 PLRs from databases and 72 from registries. Reported barriers to PLR development included ambiguity in guidance, lack of incentives, and concerns of researchers writing for the general public. Available guidance recommendations called for greater dissemination, increased readability, and varied content formats. Patients preferred visual PLRs formats (e.g., videos, comics), which were easy to access on the internet and used short jargon-free text. In some instances, older audiences and more educated readers preferred text-only PLRs. Preferences among the general public were mostly similar to those of patients. Psychology, followed by oncology, showed the highest number of PLRs, predominantly from academia-sponsored research. Text-only PLRs were most commonly available, while graphical, digital, or online formats were less available. Preferred dissemination channels included paywall-free journal websites, indexing on PubMed, third-party websites, via email to research participants, and social media. Conclusions: This scoping review maps current practices, recommendations, and patients' and the general public's preferences for PLR development and dissemination. The results suggest that making PLRs available to a wider audience by improving nomenclature, accessibility, and providing translations may contribute to empowerment and SDM. Minimizing variability among available guidance for PLR development may play an important role in amplifying the value and impact of these resources.
Exploring the Potential Impact of GLP-1 Receptor Agonists on Substance Use, Compulsive Behavior, and Libido: Insights from Social Media Using a Mixed-Methods Approach

Abstract:Glucagon-like peptide-1 (GLP-1) is involved in a range of central and peripheral pathways related to appetitive behavior. Hence, this study explored the effects of glucagon-like peptide-1 receptor agonists (GLP-1 RAs) on substance and behavioral addictions, including alcohol, caffeine, nicotine, cannabis, psychostimulants, compulsive shopping, and sex drive/libido. Data were collected from various social platforms. Keywords related to GLP-1 RAs and substance/behavioral addiction were used to extract relevant comments. The study employed a mixed-methods approach to analyze online discussions posted from December 2019 to June 2023 and collected using a specialized web application. Reddit entries were the focus here due to limited data from other platforms, such as TikTok and YouTube. A total of 5859 threads and related comments were extracted from six subreddits, which included threads about GLP-1 RAs drugs and associated brand names. To obtain relevant posts, keywords related to potential substance use and compulsive behavior were selected. Further analysis involved two main steps: (1) manually coding posts based on users' references to the potential impact of GLP-1 RAs on substance use and non-substance habits, excluding irrelevant or unclear comments; (2) performing a thematic analysis on the dataset of keywords, using AI-assisted techniques followed by the manual revision of the generated themes. Second, a thematic analysis was performed on the keyword-related dataset, using AI-assisted techniques followed by the manual revision of the generated themes. In total, 29.75% of alcohol-related; 22.22% of caffeine-related; and 23.08% of nicotine-related comments clearly stated a cessation of the intake of these substances following the start of GLP-1 RAs prescription. Conversely, mixed results were found for cannabis intake, and only limited, anecdotal data were made available for cocaine, entactogens, and dissociative drugs' misuse. Regarding behavioral addictions, 21.35% of comments reported a compulsive shopping interruption, whilst the sexual drive/libido elements reportedly increased in several users. The current mixed-methods approach appeared to be a useful tool in gaining insight into complex topics such as the effects of GLP-1 RAs on substance and non-substance addiction-related disorders; some GLP-1 RA-related mental health benefits could also be inferred from here. Overall, it appeared that GLP-1 RAs may show the potential to target both substance craving and maladaptive/addictive behaviors, although further empirical research is needed.

Abstract:Personalized Normative Feedback (PNF) may help address addictive disorders. PNF highlights discrepancies between perceived and actual peer norms, juxtaposed against self-reported behavior. PNF can be self-directed and cost-efficient. Our study estimates the efficacy of PNF alone, and in combination with other self-directed interventions, to address frequency and symptom severity of hazardous alcohol use, problem gambling, illicit drug and tobacco use. We searched electronic databases, grey literature, and reference lists of included articles, for randomized controlled trials published in English (January 2000-August 2019). We assessed study quality using the Cochrane Risk of Bias tool. Thirty-four studies met inclusion criteria (k = 28 alcohol, k = 3 gambling, k = 3 cannabis, k = 0 tobacco). Thirty studies provided suitable data for meta-analyses. PNF alone, and with additional interventions, reduced short-term alcohol frequency and symptom severity. PNF with additional interventions reduced short-term gambling symptom severity. Effect sizes were small. PNF did not alter illicit drug use. Findings highlight the efficacy of PNF to address alcohol frequency and symptom severity. The limited number of studies suggest further research is needed to ascertain the efficacy of PNF for gambling and illicit drug use. Cost-effectiveness analyses are required to determine the scale of PNF needed to justify its use in various settings.

Abstract:Importance The overdose epidemic continues in the US, with 107 941 overdose deaths in 2022 and countless lives affected by the addiction crisis. Although widespread efforts to train and support physicians to implement medications and other evidence-based substance use disorder interventions have been ongoing, adoption of these evidence-based practices (EBPs) by physicians remains low. Objective To describe physician-reported reasons for reluctance to address substance use and addiction in their clinical practices using screening, treatment, harm reduction, or recovery support interventions. Data Sources A literature search of PubMed, Embase, Scopus, medRxiv , and SSRN Medical Research Network was conducted and returned articles published from January 1, 1960, through October 5, 2021. Study Selection Publications that included physicians, discussed substance use interventions, and presented data on reasons for reluctance to intervene in addiction were included. Data Extraction and Synthesis Two reviewers (L.N., M.C., L.F., J.P., C.S., and S.W.) independently reviewed each publication; a third reviewer resolved discordant votes (M.C. and W.C.). This systematic review was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-analyses guidelines and the theoretical domains framework was used to systematically extract reluctance reasons. Main Outcomes and Measures The primary outcome was reasons for physician reluctance to address substance use disorder. The association of reasons for reluctance with practice setting and drug type was also measured. Reasons and other variables were determined according to predefined criteria. Results A total of 183 of 9308 returned studies reporting data collected from 66 732 physicians were included. Most studies reported survey data. Alcohol, nicotine, and opioids were the most often studied substances; screening and treatment were the most often studied interventions. The most common reluctance reasons were lack of institutional support (173 of 213 articles [81.2%]), knowledge (174 of 242 articles [71.9%]), skill (170 of 230 articles [73.9%]), and cognitive capacity (136 of 185 articles [73.5%]). Reimbursement concerns were also noted. Bivariate analysis revealed associations between these reasons and physician specialty, intervention type, and drug. Conclusions and Relevance In this systematic review of reasons for physician reluctance to intervene in addiction, the most common reasons were lack of institutional support, knowledge, skill, and cognitive capacity. Targeting these reasons with education and training, policy development, and program implementation may improve adoption by physicians of EBPs for substance use and addiction care. Future studies of physician-reported reasons for reluctance to adopt EBPs may be improved through use of a theoretical framework and improved adherence to and reporting of survey development best practices; development of a validated survey instrument may further improve study results.
