Abstract:Connected speech samples elicited by a picture description task are widely used in the assessment of aphasias, but it is not clear what their interpretation should focus on. Although such samples are easy to collect, analyses of them tend to be time-consuming, inconsistently conducted, and impractical for non-specialist settings. Here, we analysed connected speech samples from patients with the three variants of primary progressive aphasia (semantic, svPPA N = 9; logopenic, lvPPA N = 9; non-fluent, nfvPPA N = 9), progressive supranuclear palsy (PSP Richardson's syndrome N = 10), corticobasal syndrome (CBS N = 13), and age-matched healthy controls. There were three principal aims: (i) to determine the differences in quantitative language output and psycholinguistic properties of words produced by patients and controls; (ii) to identify the neural correlates of connected speech measures; and (iii) to develop a simple clinical measurement tool. Using data-driven methods, we optimised a 15-word checklist for use with the Boston Diagnostic Aphasia Examination "cookie theft" and Mini Linguistic State Examination "beach scene" pictures and tested the predictive validity of outputs from Least Absolute Shrinkage and Selection Operator (LASSO) models using an independent clinical sample from a second site. The total language output was significantly reduced in patients with nfvPPA, PSP and CBS relative to those with svPPA and controls. The speech of patients with lvPPA and svPPA contained a disproportionately greater number of words of both high frequency and high semantic diversity. Results from our exploratory voxel-based morphometry analyses across the whole group revealed correlations between grey matter volume in (i) bilateral frontal lobes with overall language output, (ii) the left frontal and superior temporal regions with speech complexity, (iii) bilateral frontotemporal regions with phonology, and (iv) bilateral cingulate and subcortical regions with age of acquisition. With the 15-word checklists, the LASSO models showed excellent accuracy for within-sample k-fold classification (over 93%) and out-of-sample validation between patients and controls (over 90%), and moderately good (59% - 74%) differentiation between the motor disorders (nfvPPA, PSP, CBS) and lexico-semantic groups (svPPA, lvPPA). In conclusion, we propose that a simple 15-word checklist provides a suitable screening test to identify people with progressive aphasia, while further specialist assessment is needed to differentiate accurately some groups (e.g., svPPA versus lvPPA and PSP versus nfvPPA).

Abstract:Morphosyntactic assessments are important for characterizing individuals with nonfluent/agrammatic variant primary progressive aphasia (nfvPPA). Yet, standard tests are subject to examiner bias and often fail to differentiate between nfvPPA and logopenic variant PPA (lvPPA). Moreover, relevant neural signatures remain underexplored. Here, we leverage natural language processing tools to automatically capture morphosyntactic disturbances and their neuroanatomical correlates in 35 individuals with nfvPPA relative to 10 healthy controls (HC) and 26 individuals with lvPPA. Participants described a picture, and ensuing transcripts were analyzed via part-of-speech tagging to extract sentence-related features (e.g., subordinating and coordinating conjunctions), verbal-related features (e.g., tense markers), and nominal-related features (e.g., subjective and possessive pronouns). Gradient boosting machines were used to classify between groups using all features. We identified the most discriminant morphosyntactic marker via a feature importance algorithm and examined its neural correlates via voxel-based morphometry. Individuals with nfvPPA produced fewer morphosyntactic elements than the other two groups. Such features robustly discriminated them from both individuals with lvPPA and HCs with an AUC of 0.95 and 0.82, respectively. The most discriminatory feature corresponded to subordinating conjunctions was correlated with cortical atrophy within the left posterior inferior frontal gyrus across groups ( p FWE <.05). Automated morphosyntactic analysis can efficiently differentiate nfvPPA from lvPPA. Also, the most sensitive morphosyntactic markers correlate with a core atrophy region of nfvPPA. Our approach, thus, can contribute to a key challenge in PPA diagnosis.

Abstract:Primary progressive aphasia (PPA) is a neurodegenerative disorder characterized by progressive loss of speech and language. Although speech perception and language comprehension deficits are observed in individuals with PPA, these deficits have been understudied relative to production deficits. Recent work has examined receptive language processing at sublexical, lexical, and semantic levels in PPA; however, systematic investigation of these levels of processing within a single PPA cohort is lacking. The current study sought to fill this gap. Individuals with logopenic, nonfluent, and semantic variants of PPA and healthy, age-matched controls completed minimal pairs syllable discrimination, auditory lexical decision, and picture-word verification tasks to assess sublexical, lexical, and semantic processing. Distinct profiles were observed across PPA variants. Individuals with logopenic variant PPA had impaired performance on auditory lexical decision and picture-word verification tasks, with a trend toward impaired performance on the syllable discrimination task. Individuals with nonfluent and semantic variant PPA had impaired performance only on auditory lexical decision and picture-word verification. Evaluation of the types of errors made on the picture-word verification task (phonological and semantic) provided further insight into levels of deficits across the variants. Overall, the results indicate deficits in receptive processing at the lexical-phonological, lexical-semantic, and semantic levels in logopenic variant PPA, with a trend toward impaired sublexical processing. Deficits were observed at the lexical-semantic and semantic levels in semantic variant PPA, and lexical-phonological deficits were observed in nonfluent PPA, likely reflecting changes both in lexical-phonological processing as well as changes in predictive coding during perception. This study provides a more precise characterization of the linguistic profile of each PPA subtype for speech perception and language comprehension. The constellation of deficits observed in each PPA subtype holds promise for differential diagnosis and for informing models of intervention.

15N-tracer investigations into the nitrogen metabolism of preterm infants fed mother's milk and a formula diet.

Abstract:Protein synthesis, protein breakdown, protein-N turnover, and other parameters describing the nitrogen metabolism were measured in five male preterm infants. The weight of the subjects at birth was 2,064 +/- 107 g and the measurements were performed at age 16.0 +/- 4.5 days in the case of the mother's milk diet and 27.4 +/- 6.8 days in the case of the formula diet containing 1.8% protein. The parameters were measured by means of the 15N-tracer technique using [15N]glycine (95 atom %) applied in a single oral dose of 20 mg/kg as a tracer. The three-pool model proposed by Winkler and Faust was used to calculate the whole body protein parameters. No difference in net protein gain, protein synthesis, protein breakdown, or the other protein metabolism parameters were recorded despite the different protein inputs. Renal nitrogen excretion and the rate of endogenous urea N excretion were significantly higher for the formula diet than for the mother's milk diet. The protein synthesis rate of 7.9 g X kg-1 X day-1 was, as has previously been observed, higher than in other age groups. The protein metabolism of the preterm infant older than 33 weeks of gestational age does not benefit from a formula diet based on cow's milk that is richer in protein than mother's milk.

Abstract:Primary progressive aphasia (PPA) is a clinical syndrome in which patients progressively lose speech and language abilities. Three variants are recognized: logopenic (lvPPA), associated with phonology and/or short-term verbal memory deficits accompanied by left temporo-parietal atrophy; semantic (svPPA), associated with semantic deficits and anterior temporal lobe (ATL) atrophy; non-fluent (nfvPPA) associated with grammar and/or speech-motor deficits and inferior frontal gyrus (IFG) atrophy. Here, we set out to investigate whether the three variants of PPA can be dissociated based on error patterns in a single language task. We recruited 21 lvPPA, 28 svPPA, and 24 nfvPPA patients, together with 31 healthy controls, and analyzed their performance on an auditory noun-to-verb generation task, which requires auditory analysis of the input, access to and selection of relevant lexical and semantic knowledge, as well as preparation and execution of speech. Task accuracy differed across the three variants and controls, with lvPPA and nfvPPA having the lowest and highest accuracy, respectively. Critically, machine learning analysis of the different error types yielded above-chance classification of patients into their corresponding group. An analysis of the error types revealed clear variant-specific effects: lvPPA patients produced the highest percentage of "not-a-verb" responses and the highest number of semantically related nouns (production of baseball instead of throw to noun ball ); in contrast, svPPA patients produced the highest percentage of "unrelated verb" responses and the highest number of light verbs (production of take instead of throw to noun ball ). Taken together, our findings indicate that error patterns in an auditory verb generation task are associated with the breakdown of different neurocognitive mechanisms across PPA variants. Specifically, they corroborate the link between temporo-parietal regions with lexical processing, as well as ATL with semantic processes. These findings illustrate how the analysis of pattern of responses can help PPA phenotyping and heighten diagnostic sensitivity, while providing insights on the neural correlates of different components of language.

Abstract:Purpose This study aimed to characterize the quantitative (total number of correct words generated) and qualitative (psycholinguistic properties of correct words generated) performance patterns on the animal fluency task in individuals with the logopenic variant of primary progressive aphasia and to investigate the influence of phonological and semantic abilities to these patterns. Methods Fifteen participants with lvPPA and twenty neurotypical adults completed the animal fluency task and an assessment battery to characterize their phonological and semantic abilities. We recorded the total number of correct words produced and their psycholinguistic properties. Group differences were analyzed using independent samples t -tests and analysis of covariance. Stepwise and multiple linear regression analyses were implemented to investigate the contribution of psycholinguistic properties on word generation as well as the role of phonological and semantic abilities on performance. We also investigated the mediating role of phonological and semantic abilities on the relationship between relevant psycholinguistic properties and word generation output. Results Compared to neurotypical controls, participants with lvPPA produced fewer correct responses and more words with lower age of acquisition. The total number of correct words generated was predicted by the age of word acquisition, such that individuals who generated more responses, produced words acquired later in life. Phonology and semantics influenced the number of correct words generated and their frequency, age of acquisition, and semantic neighborhood density. Familiarity and arousal were driven by semantic abilities. Phonological abilities partially mediated the relationship between age of acquisition and word generation output. Conclusions This study provides valuable insights into the performance patterns of the animal fluency task in lvPPA. Individuals with lvPPA with more intact phonological and semantic abilities generated greater number of words with more complex psycholinguistic properties. Our findings contribute to the understanding of language processes underlying word retrieval in lvPPA.

Abstract:We investigated semantic cognition in the logopenic variant of primary progressive aphasia (lvPPA), including (i) the status of verbal and non-verbal semantic performance; and (ii) whether the semantic deficit reflects impaired semantic control. Our a priori hypothesis that individuals with lvPPA would exhibit semantic control impairments was motivated by the anatomical overlap between the temporoparietal atrophy typically associated with lvPPA and lesions associated with post-stroke semantic aphasia (SA) and Wernicke's aphasia (WA), which cause heteromodal semantic control impairments. We addressed the presence, type (semantic representation and semantic control; verbal and non-verbal), and progression of semantic deficits in lvPPA. Since most people with lvPPA have Alzheimer's disease (AD) pathology and are part of a broader multidimensional phenotype space encompassing AD subtypes, we compared semantic performance in lvPPA and typical amnestic AD (tAD). Given the differences in lesion and atrophy patterns in SA and WA versus semantic dementia/semantic-variant PPA patients, our second aim was to examine atrophy patterns in people with lvPPA and tAD compared to age-matched controls. Twenty-seven patients participated in the study. People were grouped into those meeting consensus criteria for lvPPA (n = 10) and others who may have previously satisfied definitions of lvPPA but had progressed with multi-domain cognitive impairments (herein referred to as "lvPPA+"; n = 8). People with tAD (n = 9) were relatively preserved across verbal and non-verbal semantic assessments. LvPPA patients were impaired on both verbal and non-verbal semantic tasks and their impairments showed the hallmark characteristics of a semantic control deficit. LvPPA and lvPPA+ patients showed effects of varying semantic control demands, positive cueing effects, and correlated performance between semantic and executive tasks. Whole-brain voxel-based morphometry, comparing each of the patient groups to age-matched controls, revealed significantly reduced grey and white matter in the bilateral hippocampi and lateral temporal regions in tAD patients. The lvPPA group exhibited an asymmetric pattern of reduced grey and white matter intensity in the language-dominant left hemisphere, including a significant portion of the lateral and medial temporal lobe. LvPPA+ patients demonstrated reduced grey and white matter in the left temporal lobe extending subcortically, anteriorly and posteriorly, as well as right temporal involvement. Our findings could aid diagnostic subtyping of PPA by adopting semantic control features and offer improved clinical characterisation of lvPPA in the trajectory of semantic decline.

Abstract:Current classification of primary progressive aphasia (PPA) encompasses three variants: non-fluent (nfvPPA), semantic (svPPA) and logopenic (lvPPA). Previously lvPPA was regarded as aphasic form of Alzheimer's disease (AD). However, not all patients with lvPPA phenotype present with AD pathology. Despite abundant literature on differentiation of lvPPA from svPPA and nfvPPA, studies comparing lvPPA with AD and mild cognitive impairment (MCI) are scarce. This study aimed at analyzing written descriptive output in lvPPA, AD and MCI. Thirty-five patients participated in the study: 9 with lvPPA, 13 with AD and 13 with MCI. Most aspects of writing performance were comparable in three groups. However, letter insertion errors appeared in 44% patients with lvPPA, while they were absent in AD and MCI. Patients with lvPPA used more verbs than patients with AD. Writing profile may complement other neuropsychological assessment results in the differential diagnosis of lvPPA. Letter insertion errors and frequent verb use may raise a query of lvPPA.

Speech-language within and between network disruptions in primary progressive aphasia variants

Abstract:Primary progressive aphasia (PPA) variants present with distinct disruptions in speech-language functions with little known about the interplay between affected and spared regions within the speech-language network and their interaction with other functional networks. The Neurodegenerative Research Group, Mayo Clinic, recruited 123 patients with PPA (55 logopenic (lvPPA), 44 non-fluent (nfvPPA) and 24 semantic (svPPA)) who were matched to 60 healthy controls. We investigated functional connectivity disruptions between regions within the left-speech-language network (Broca, Wernicke, anterior middle temporal gyrus (aMTG), supplementary motor area (SMA), planum temporale (PT) and parietal operculum (PO)), and disruptions to other networks (visual association, dorsal-attention, frontoparietal and default mode networks (DMN)). Within the speech-language network, multivariate linear regression models showed reduced aMTG-Broca connectivity in all variants, with lvPPA and nfvPPA findings remaining significant after Bonferroni correction. Additional loss in Wernicke-Broca connectivity in nfvPPA, Wernicke-PT connectivity in lvPPA and greater aMTG-PT connectivity in svPPA were also noted. Between-network connectivity findings in all variants showed reduced aMTG-DMN and increased aMTG-dorsal-attention connectivity, with additional disruptions between aMTG-visual association in both lvPPA and svPPA, aMTG-frontoparietal in lvPPA, and Wernicke-DMN breakdown in svPPA. These findings suggest that aMTG connectivity breakdown is a shared feature in all PPA variants, with lvPPA showing more extensive connectivity disruptions with other networks.

Abstract:INTRODUCTION Epidural haematomas are one of the most common complications of closed head injuries. Bilateral extradural haematomas are rare, usually acute, and generally associated with severe trauma and a high mortality. OBJECTIVE The purpose of this paper is to present six cases of bilateral extradural haematomas seen at our service during the last 24 years and found an incidence of 2.5% of all cases of extradural haematomas surgically treated. RESULTS There were 5 males and 1 female with a mean age of 32.6 years (range 16-55). In 3 cases haematoma was across the midline and in the other 3 cases haematomas was found at different locations on either side. Skull fracture was present in all cases. Surgical approach was the primary treatment in all cases. Mortality in our series was 50%. CONCLUSIONS Bilateral extradural haematomas is a rare condition and the prognosis is mainly dependent of the pre-operative neurological state.
[Primary progressive aphasia: its clinical variability: an analysis of 15 cases]

Abstract:Introduction: Primary progressive aphasia (PPA) represents a clinical syndrome linked to multiple degenerative diseases. The diagnosis of PPA is made when language is the only area of salient and progressive dysfunction for at least the first two years of the disease. Aim: To evaluate the neuropsychological, neuropsychiatric and language characteristics of the PPA. Patients and methods: 15 patients with PPA underwent language, neuropsychological and neuropsychiatric evaluation, magnetic resonance imaging, computerized tomography and single photon emission computerized tomography. Results: We observed a clear distinction between the oral expression patterns; the patients were classificated by type of aphasia. The most common sign of PPA was a word finding deficit, also known as anomia. Seven aphasia type Broca, four sensorial transcortical aphasia, two aphasia type Wernicke and two anomic aphasia were found in our patients. Depression, apathy, anxiety and irritability were the most prevalent neuropsychiatric sign. Conclusions: PPA is a language-based syndrome, that include fluent (normal articulation, flow and number of words per utterance) and nonfluent form of aphasia. It has been considered a cognitive term, however, PPA is associated with high prevalence of psychiatric manifestations. More research it will be necessary to evaluate the prognostic value of them. The slow and progressive deterioration of language provides an interesting model to understand the mechanisms and biological bases involved in the linguistic process.
Logopenic aphasia in Alzheimer's disease: clinical variant or clinical feature?

Abstract:BACKGROUND: Primary progressive aphasia (PPA) is a clinical syndrome characterised by progressive decline in components of the language system. Recent evidence suggests that the logopenic/phonological (LPA) variant is a reliable in vivo marker of Alzheimer related pathology. The aim of this study was to determine if patients with clinically typical early stage Alzheimer's disease (AD) display a characteristic language disorder that resembles LPA, or if LPA is a clinical manifestation of an atypical form of AD.METHODS: Spoken language samples were obtained using the Cookie Theft picture description task from 18 post mortem confirmed cases of AD, where speech samples were taken at the first point of clinical diagnosis, and 18 post mortem confirmed healthy controls. Spoken samples were transcribed from tape recordings and analysed using the scoring system described by Wilson et al.RESULTS: Group comparisons between normal controls and AD patients showed no significant overall differences. Individual review of the linguistic variables compared with the PPA variants showed that a third of patients had normal language (n=6). The remainder showed varied patterns of linguistic impairment. In the majority of the affected group, the most salient feature was a reduction in one or more measures of syntactic complexity. One patient's deficit was comparable to that found in LPA.CONCLUSIONS: The impairment found in clinically typical early stage AD did not correspond consistently to the linguistic profiles described in any of the sub-syndromes of PPA. The only reliably distinguishing feature was a reduction across a range of syntactic complexity measures. The findings suggest that LPA represents an atypical clinical presentation of AD rather than a common clinical feature of typical AD.
Artificial intelligence classifies primary progressive aphasia from connected speech

Abstract:Neurodegenerative dementia syndromes, such as Primary Progressive Aphasias (PPA), have traditionally been diagnosed based in part on verbal and nonverbal cognitive profiles. Debate continues about whether PPA is best divided into three variants and also regarding the most distinctive linguistic features for classifying PPA variants. In this cross-sectional study, we first harnessed the capabilities of artificial intelligence (AI) and Natural Language Processing (NLP) to perform unsupervised classification of short, connected speech samples from 78 PPA patients. We then used NLP to identify linguistic features that best dissociate the three PPA variants. Large Language Models (LLMs) discerned three distinct PPA clusters, with 88.5% agreement with independent clinical diagnoses. Patterns of cortical atrophy of three data-driven clusters corresponded to the localization in the clinical diagnostic criteria. In the subsequent supervised classification, seventeen distinctive features emerged, including the observation that separating verbs into high and low-frequency types significantly improves classification accuracy. Using these linguistic features derived from the analysis of short, connected speech samples, we developed a classifier that achieved 97.9% accuracy in classifying the four groups (three PPA variants and healthy controls). The data-driven section of this study showcases the ability of LLMs to find natural partitioning in the speech of patients with PPA consistent with conventional variants. In addition, the work identifies a robust set of language features indicative of each PPA variant, emphasizing the significance of dividing verbs into high and low-frequency categories. Beyond improving diagnostic accuracy, these findings enhance our understanding of the neurobiology of language processing.

Abstract:Objective: To explore the utility of neuropsychological testing for patients with Primary Progressive Aphasia and compare testing data for a Logopenic and Semantic PPA variants within the medically complex Veteran population. Participants and Methods: Both participants were referred by their psychiatrist due to memory concerns. The case studies testing data will be compared to look at the differences on testing between different PPA presentations within the Veteran population. Patient A is a 77 year old, right handed, African American, divorced man with approximately 14 years of formal education. Patient B is a 76 year old, right handed, Caucasian, widowed man with approximately 16 years of formal education. Results: Patient A displayed problems with single-word retrieval, repetition of nonsense words and sentences, comprehension, reading, spelling, and naming. He also displayed impairments in aspects of working memory, along with learning and memory. His cognitive profile raises concern for a logopenic variant of primary progressive aphasia, which often has Alzheimer's disease pathology. Patient B displayed empty speech, impairments in fluency and reduced semantic knowledge that raises concern for a semantic variant of primary progressive aphasia. However, aspects of his presentation are not consistent with this diagnosis, specifically intact confrontation visual naming. Patient has a history of significant alcohol abuse, although he has mostly remained sober since moving to Texas. This evaluation cannot rule out the contribution of sustained alcohol use on his cognitive functioning; however, this is likely not the primary etiology given his significant language issues. Conclusions: Patients with medically complex histories and unclear timelines of symptom progressive make it difficult for diagnostic clarity. Diagnoses can be additionally difficult to determine at times when the clinical presentation is not as clearly defined in textbooks. This case study comparison displays the importance of integrating all data to determine the proper diagnosis to optimize patient care and provide recommendations tailored to that individual.

Abstract:Abstract Background Primary Progressive Aphasia (PPA) is a neurodegenerative syndrome that presents with speech and language impairments and is generally classified into three variants: semantic (svPPA), nonfluent/ agrammatic (nfvPPA), and logopenic variants (lvPPA). Current understanding of PPA has been mostly derived from English language patients. Expanding PPA research into non‐English languages encourages language‐specific linguistic‐adapted assessments and enriches our knowledge in linguistic features distinct from English language. This presentation focuses on three Chinese linguistic features that differ from English language: lexical tones, classifier, and logographic script. Chinese is a tonal and classifier language in which tones are lexically contrastive, and classifiers are obligatory to quantify nouns numerally. It also adopts logographic script that has complex visuospatial configurations and weak grapheme‐phoneme correspondence. Method The CLAP ( C hinese L anguage A ssessment for P PA) battery is designed to characterize the neurolinguistics features of Chinese speaking PPA individuals. In CLAP classifier task, participants are asked to spontaneously produce classifiers specific to numerous nouns and presented with multiple choices when failed. To evaluate Chinese writing skills, participants are tasked with dictating 60 Chinese characters of varying lexicality, character frequency, and stroke number. The motor speech function is assessed by multiple repetitions of Chinese characters that differ in articulation places, manners, and tones. Results The CLAP project enrolled 20 controls and 33 PPA individuals (7 nfvPPA, 10 svPPA, and 16 lvPPA). In classifier task, svPPA and lvPPA scored lower (p<0.01), though lvPPA were more responsive to choices than svPPA (p<0.001). The classifier score significantly correlated with volumetric changes over the left middle, inferior temporal, and fusiform gyri, brain regions typically associated with semantic functions. For orthographic dictation task, all three PPA variants performed worse than controls (p<0.001), and distinctive dysgraphia phenotypes were noted in each variant (svPPA: phonological plausible and perseveration errors; nfvPPA: motoric, radical and morphemic errors; lvPPA: visual and stroke errors, P<0.001). In motor speech task, nfvPPA showed lower performance when tasked with repeating characters that vary in articulation places and tones (p=0.018, p<0.001) but not in articulation manners (p=0.076). Conclusion The clinical features and assessment strategies for PPA syndrome may vary across languages and should ideally be tailored linguistically.

Abstract:Many obvious practical and ethical obstacles severely limit the scope for experiments using humans in biomedical science, thus much of what we know about the underlying biology of cells and tissues comes from studies using model organisms such as mice, and the focus of this article, the fruit fly Drosophila melanogaster (Fig. 1a). Drosophila has been used productively as a model organism for over a century to study a diverse range of biological processes including genetics and inheritance, embryonic development, learning, behavior, and aging. Although humans and fruit flies may not look very similar, it has become well established that most of the fundamental biological mechanisms and pathways that control development and survival are conserved across evolution between these species. The first documented use of Drosophila in the laboratory was by William Castle’s group at Harvard in 1901, although the “father” of Drosophila research is undoubtedly Thomas Hunt Morgan1. Morgan greatly refined the theory of inheritance first proposed by Gregor Mendel, by using Drosophila to define genes and establish that they were found within chromosomes (long before it was even established that DNA is the genetic material). Morgan won the Nobel Prize in Physiology or Medicine in 1933 “for his discoveries concerning the role played by the chromosome in heredity”2. One of Morgan's protégés, Hermann Muller, won the Nobel Prize in Physiology or Medicine in 1946 “for the discovery of the production of mutations by means of x-ray irradiation”3. Using Drosophila in the 1920s, Muller discovered that x-rays caused a massive increase in the mutation rate of genes, and could actually break chromosomes4. Although irradiated flies looked normal, their offspring frequently showed the effects of mutation. This led to the realization that radiation causes harmful genetic defects in the offspring of exposed humans – a timely observation given that this was at the advent of man’s attempts to harness and exploit nuclear fission. The fruit fly Drosophila melanogaster is a versatile model organism that has been used in biomedical research for over a century to study a broad range of phenomena. There are many technical advantages of using Drosophila over vertebrate models; they are easy and inexpensive to culture in laboratory conditions, have a much shorter life cycle, they produce large numbers of externally laid embryos and they can be genetically modified in numerous ways. Research using Drosophila has made key advances in our understanding of regenerative biology and will no doubt contribute to the future of regenerative medicine in many different ways.
A Clinical, Neurolinguistic, and Radiological Study of a Chinese Follow-Up Case with Primary Progressive Aphasia

Abstract:Primary progressive aphasia (PPA) is a progressive neurodegenerative disorder characterized by the deterioration of language functions. The Han language bears some unique features from the Latin languages; however, the features of PPA in the Han language-speaking population are not well understood. In this study, we performed a 3-year follow-up on a Han language-speaking PPA patient with corresponding changes in magnetic resonance imaging (MRI). During the early stage, linguistic analysis revealed several symptoms including difficulty with auditory comprehension, right-left disorientation, reading disorders, and agraphia, specifically the execution of serial oral instructions. This Chinese PPA patient presented with a reading disorder, but his word comprehension ability remained intact. There are two different possible modalities of incorrect writing in this case. The patient also presented with noun-verb double dissociation. The early-stage MRI showed atrophy of the left frontal lobe, which was most severe in the inferior frontal gyrus. Three years later, the patient was found to have progressive atrophy in the parietal, frontal, and temporal lobes, among which the frontal lobe remained the most severely affected region. The brain imaging of the Chinese-speaking PPA patient showed changes similar to those of a Latin language-speaking PPA patient. The prominent change was asymmetrical atrophy in the frontal and temporal lobes. This is the first report of noun-verb double dissociation existing in a Chinese-language speaking PPA patient. The dissociation may be related to an impaired function of the inferior frontal gyrus, which is likely associated with verb-naming in Chinese-speaking people. Several unique features were observed in this case, including impairment in writing ability.

Abstract:Background and purpose Logopenic variant primary progressive aphasia (lvPPA) is a major variant presentation of Alzheimer's disease (AD) that signals the importance of communication dysfunction across AD phenotypes. A clinical staging system is lacking for the evolution of AD‐associated communication difficulties that could guide diagnosis and care planning. Our aim was to create a symptom‐based staging scheme for lvPPA, identifying functional milestones relevant to the broader AD spectrum. Methods An international lvPPA caregiver cohort was surveyed on symptom development under an 'exploratory' survey (34 UK caregivers). Feedback from this survey informed the development of a 'consolidation' survey (27 UK, 10 Australian caregivers) in which caregivers were presented with six provisional clinical stages and feedback was analysed using a mixed‐methods approach. Results Six clinical stages were endorsed. Early symptoms included word‐finding difficulty, with loss of message comprehension and speech intelligibility signalling later‐stage progression. Additionally, problems with hearing in noise, memory and route‐finding were prominent early non‐verbal symptoms. 'Milestone' symptoms were identified that anticipate daily‐life functional transitions and care needs. Conclusions This work introduces a new symptom‐based staging scheme for lvPPA, and highlights milestone symptoms that could inform future clinical scales for anticipating and managing communication dysfunction across the AD spectrum.
