Abstract:Gestational diabetes mellitus (GDM) is common during pregnancy, with the prevalence reaching as high as 31.0％ in some European regions (McIntyre et al., 2019). Dysfunction of the glucose metabolism in pregnancy can influence fetal growth via alteration of the intrauterine environment, resulting in an increased risk of abnormal offspring birth weight (McIntyre et al., 2019). Infants with abnormal birth weight will be faced with increased risks of neonatal complications in the perinatal period and chronic non-communicable diseases in childhood and adulthood (Mitanchez et al., 2015;McIntyre et al., 2019). Therefore, accurate esti-mation of birth weight for neonates from women with GDM is crucial for more sensible perinatal decision-making and improvement of perinatal outcomes. Timely antenatal intervention, with reference to accurately estimated fetal weight, may also decrease the risks of adverse long-term diseases.
Accurate birth weight prediction from fetal biometry using the Gompertz model

Abstract:Objectives: Monitoring of fetal growth and estimation of birth weight is of clinical importance. During pregnancy, ultrasound fetal biometry values including femur length, head circumference, abdominal circumference, biparietal diameter are measured and used to place fetuses on "growth charts". There is no simple growth-model-based, predictive formula in use for fetal biometry. Estimation of fetal weight at birth currently depends on ultrasound data taken a short time before birth. Study design: Our cohort ("Seethapathy cohort") consists of ultrasound biometry measurements and other data for 774 pregnant women in Chennai, India, 2015-2017. We use the Gompertz model, a standard model for constrained growth, with just three intuitive parameters, to model the growth of fetal biometry, and a machine learning (ML) model trained on these parameters to predict birth weight (BW). Results: The Gompertz model convincingly fits the growth of fetal biometry values. Two Gompertz parameters- t 0 (inflection time) and c (rate of decrease of growth rate)-seem universal to all fetuses, while the third, A , is an overall scale specific to each fetus, capturing individual variation. On the Seethapathy cohort we can infer A for each fetus from ultrasound data available by the 24 or 35 weeks. Our ML model predicts birth weight with < 8 % error, outperforming published methods that have access to late-term ultrasound data. The same model gives an 8.4 % error in BW prediction on an independent validation cohort of 365 women. Conclusions: The Gompertz model fits fetal biometry growth and enables birth weight estimation without need of late-term ultrasounds. Aside from its clinical predictive value, we suggest its use for future growth standards, with almost all variation described by a single scale parameter A .
Selection of Methods Predicting Fetal Weight Using Ultrasonography

Abstract:Objective The neural network and regression methods were used to compare the accuracy of predicting fetal weight. Methods Of a total of 390 cases, ultrasonographic parameters as BPD, HD, ThD, AD, FL, LL, STT and SCT were measured at 72 hour before delivery. Among them, 250 cases utilized training neural network systems and establish a series of regression formulae for fetal weight prediction. In the other 140 cases, fetal weights were by neural network, regression equation and the traditional method for comparison of accuracy. Results The 6 variable neural network has the highest coincide rate, 88.6%, with a relative error of 0.8±5.2%. The regression methods have a little lower coincident rate. Among them, the 6 variable regression equation has a coincident rate of 85.7%, and the 2 variable has a rate of 83.6%. AD, FL and BPD are useful variables which affect the accuracy in the fetal weight prediction. Conclusions 1. The neural network is a new method to predict fetal body weight with high accuracy. The regression equations, with their ease of use and preciseness, are still the mainstay of predicting fetal body weight as well. 2. AD, FL and BPD are important parameters in predicting fetal weight. 3. It is necessary to use different method in predicting different fetal weight, such as over weight and under weight.
Ultrasonographic estimation of fetal weight: development of new model and assessment of performance of previous models

Abstract:Objectives: To develop a new formula for ultrasonographic estimation of fetal weight and evaluate the accuracy of this and all previous formulae in the prediction of birth weight. Methods: The study population consisted of 5163 singleton pregnancies with fetal biometry at 22-43 weeks' gestation and live birth of a phenotypically normal neonate within 2 days of the ultrasound examination. Multivariable fractional polynomial analysis was used to determine the combination of variables that provided the best-fitting models for estimated fetal weight (EFW). A systematic review was also carried out of articles reporting formulae for EFW and comparing EFW to actual birth weight. The accuracy of each model for EFW was assessed by comparing mean percentage error, absolute mean error (AE), proportion of pregnancies with AE ≤ 10% and Euclidean distance. Results: The most accurate models, with the lowest Euclidean distance and highest proportion of AE ≤ 10%, were provided by the formulae incorporating ≥ 3 rather than < 3 biometrical measurements. The systematic review identified 45 studies describing a total of 70 models for EFW by various combinations of measurements of fetal head circumference (HC), biparietal diameter, femur length (FL) and abdominal circumference (AC). The most accurate model with the lowest Euclidean distance and highest proportion of AE ≤ 10% was provided by the formula of Hadlock et al., published in 1985, which incorporated measurements of HC, AC and FL; there was a highly significant linear association between EFW and birth weight (r = 0.959; P < 0.0001), and EFW was within 10% of birth weight in 80% of cases. The performance of the best model developed in this study, utilizing HC, AC and FL, was very similar to that of Hadlock et al. CONCLUSION: Despite many efforts to develop new models for EFW, the one reported in 1985 by Hadlock et al., from measurements of HC, AC and FL, provides the most accurate prediction of birth weight and can be used for assessment of all babies, including those suspected to be either small or large. Abstract:Up to 78% of the birth weight can be inferred from supervised prenatal ultrasound. Birth weight resulted ±16% of the estimated fetal weight in 95% of the cases. Parity had the most significant effect on ultrasound measurement bias. Objectives To evaluate the accuracy of ultrasound estimates of fetal weight made by residents. Methods A cross‐sectional study was conducted on consecutive pregnancies attended in an obstetric ward. All participants provided their consent to participate. Inclusion criteria were singleton pregnancies with ultrasound evaluation and delivery within 14 days. Congenital abnormalities and stillbirths were excluded. Ultrasound scans were performed by two residents, one observer per subject, following standardized methodology and supervised by senior staff. Fetal weight was calculated using the four‐parameter Hadlock 1985 formula. Birth weight was measured using a standard clinical scale. Bland–Altman and mixed‐effects analyses were performed. Results In a 6‐month period, 95 gestations were recruited at 30–41 weeks, and birth weights ranged from 1200 to 4080 g. The bias between methods was −28 g (CI95%, −74 to 18) with 95% limits of agreement of ±440 g. The 95% deviation between methods was of ±16%. Parity had the most significant effect on the bias between methods; operator and maternal weight had no significant effect. Conclusions Supervised measurement of fetal weight by residents demonstrated high accuracy. Parity bias may be associated with visceral fat deposition independent of maternal weight gain. A methodology for medical evaluation is presented.

Abstract:Objective To evaluate the accuracy of the fetal weight estimation system in terms of estimating the fetal weight.Methods The fetal weight estimation system developed by the Department of Electronic Engineering,Fudan University was applied to estimate the fetal weight in many hospitals,and the differences between the estimated results of the system and the actual weight of newborn fetuses,as well as the accuracy of the estimation system and the traditional estimation methods were compared.Results A total number of 610 cases were employed,with each case including the ultrasound images of fetus before birth,the corresponding measurement data and the newborn weight.The correlation coefficients between the weight of the newborn babies and the fetal parameters,which were measured by the estimation system in terms of the biparietal diameter,the head circumference,the abdomen circumference and the femur length,were higher than those obtained by manual annotations,with improvements of 0.08,0.17,0.09 and 0.05,respectively.The absolute error of the estimated weight of the system with respect to the actual weight was 44 g smaller than those estimated by traditional method,and the variance of the absolute error was 53 g smaller;while the corresponding absolute percentage error was 1.8％ lower,and the corresponding variance percentage was 1.5％ lower.Conclusions The fetal weight estimation system improves the accuracy of the prenatal ultrasound assessment and provides reliable information to clinical diagnosis.

Abstract:Objective: To predict birth weight at various potential gestational ages of delivery based on data routinely available at the first antenatal visit. Design: Individual participant data meta-analysis. Data sources: Individual participant data of four cohorts (237 228 pregnancies) from the International Prediction of Pregnancy Complications (IPPIC) network dataset. Eligibility criteria for selecting studies: Studies in the IPPIC network were identified by searching major databases for studies reporting risk factors for adverse pregnancy outcomes, such as pre-eclampsia, fetal growth restriction, and stillbirth, from database inception to August 2019. Data of four IPPIC cohorts (237 228 pregnancies) from the US (National Institute of Child Health and Human Development, 2018; 233 483 pregnancies), UK (Allen et al, 2017; 1045 pregnancies), Norway (STORK Groruddalen research programme, 2010; 823 pregnancies), and Australia (Rumbold et al, 2006; 1877 pregnancies) were included in the development of the model. Results: The IPPIC birth weight model was developed with random intercept regression models with backward elimination for variable selection. Internal-external cross validation was performed to assess the study specific and pooled performance of the model, reported as calibration slope, calibration-in-the-large, and observed versus expected average birth weight ratio. Meta-analysis showed that the apparent performance of the model had good calibration (calibration slope 0.99, 95% confidence interval (CI) 0.88 to 1.10; calibration-in-the-large 44.5 g, -18.4 to 107.3) with an observed versus expected average birth weight ratio of 1.02 (95% CI 0.97 to 1.07). The proportion of variation in birth weight explained by the model (R2) was 46.9% (range 32.7-56.1% in each cohort). On internal-external cross validation, the model showed good calibration and predictive performance when validated in three cohorts with a calibration slope of 0.90 (Allen cohort), 1.04 (STORK Groruddalen cohort), and 1.07 (Rumbold cohort), calibration-in-the-large of -22.3 g (Allen cohort), -33.42 (Rumbold cohort), and 86.4 g (STORK Groruddalen cohort), and observed versus expected ratio of 0.99 (Rumbold cohort), 1.00 (Allen cohort), and 1.03 (STORK Groruddalen cohort); respective pooled estimates were 1.00 (95% CI 0.78 to 1.23; calibration slope), 9.7 g (-154.3 to 173.8; calibration-in-the-large), and 1.00 (0.94 to 1.07; observed v expected ratio). The model predictions were more accurate (smaller mean square error) in the lower end of predicted birth weight, which is important in informing clinical decision making. Conclusions: The IPPIC birth weight model allowed birth weight predictions for a range of possible gestational ages. The model explained about 50% of individual variation in birth weights, was well calibrated (especially in babies at high risk of fetal growth restriction and its complications), and showed promising performance in four different populations included in the individual participant data meta-analysis. Further research to examine the generalisability of performance in other countries, settings, and subgroups is required. Trial registration: PROSPERO CRD42019135045.

Abstract:Introduction: We aim to evaluate the supplementary predictive value of soft tissue markers, including fetal limb volumes, for fetal birth weight and fat tissue weight. Methods: This is a prospective study of 60 patients undergoing term induction of labor. Ultrasound was performed 48 h before birth, and 2D sonographic measurements, subcutaneous tissue thickness, and 3D fetal limb volumes were taken. Birth weight and neonatal fat weight were assessed by plethysmography. Clinical data were collected. The relation between ultrasound and neonatal outcomes was assessed by univariate and multivariate predictive models. The estimated and actual birth weights were compared applying different published formulas, and systematic and random error were collected and compared. Results: 3D fetal limb volumes showed a strong relation to birth weight, absolute weight, and relative fat weight. The Lee 6 formula performed better than either Hadlock 3 or Lee 3 with the lowest random error. Fractional limb volumes involve a highly reproducible technique, with excellent correlation (intra-class coefficient >0.90) for both inter- and intra-observer reliability. The prevalence of estimated EFW measures within 10% error from the actual birth weight was 71.7% with the Hadlock 3 model and 95.0% with the Lee 6 model ( p = 0.09). Conclusion: Late assessment of 3D fetal limb volume in upper and lower extremities is not only useful for accurately predicting birth weight but is a useful marker for prediction of birth fat tissue weight.

The early prediction of neonatal morbidity and mortality in singleton small for gestational age infants with a birthweight < 1,500 g

Abstract:Objective: Identify factors for discrimination of "high" and "low risk" small for gestational age infants. Study design: Singleton infants born small for gestational age with a birthweight <1,500 g between 1999 and 2007 were included. Maternal, placental, and infant related factors were analyzed with regard to mortality and morbidity. Patients who died or suffered from complications were defined "high risk" as opposed to "low risk". Parameters associated with "high risk" were identified and an equation established to predict the minimal expected probability to die or suffer from neonatal morbidity. Results: Around 231 patients showed a mortality rate of 12.6 %, respiratory distress syndrome in 35.5 %, necrotizing enterocolitis in 8.2 % and neurological morbidities in 6.5 %. Of these, 58.9 % survived without complications. The factors for discrimination of "high" and "low risk" were Z-score of birth weight, gestational age, and pH. Conclusion: We facilitate prognostication by classifying small for gestational age preterms into "low" and "high risk".

Abstract:This editorial refers to ‘Cardiac mesenchymal stromal cells are a source of adipocytes in arrhythmogenic cardiomyopathy’[†][1], by E. Sommariva et al ., on page 1835. There is an apocryphal story about the famous American bank robber ‘Slick’ Willie Sutton. When asked by a reporter, ‘Why do you rob banks?’ he replied, ‘Because that's where the money is.’ Willie's wise-crack led to Sutton's Law, or the principle that when searching for something one is well advised to look for it in the obvious place first. Thus, in developing an understanding of cardiomyopathies it stands to reason that cardiomyocytes should be the focus of disease modelling. Following Sutton's Law has indeed led investigators to multiple insights, including successful disease modelling using induced pleuripotent stem cells (iPSCs), and development of potential diagnostic tests for arrhythmogenic cardiomyopathy (ACM).1,2 Genetic diagnosis in inherited cardiomyopathies also favours myocyte-specific disease modelling as the protein products of genes with pathogenic mutations identified in families with inherited cardiomyopathy are nearly universally important in myocyte function. Chief among these are proteins responsible for force generation and transmision, cytoskeletal structure, cell–cell adhesion, and transmembrane ion traffic. However, as in all things, context matters. Evaluating the accuracy of sonographic fetal weight estimations using the Hadlock IV formula in a Chinese population

Abstract:Background: Despite being the most generalized formula in China, the Hadlock IV formula has never been examined to determine if it is suitable for Chinese newborns, nor have the factors that might affect its performance been investigated. However, previous studies have reported varying results about other formulas in other nationalities. This study sought to evaluate the performance of the Hadlock IV formula in estimating fetal weight (FW) in pregnant Chinese women and use ultrasound to identify the factors affecting the accuracy of estimations of newborn weight; through these means, we aimed to create a reference for predicting neonatal weight for obstetricians.Methods: A retrospective observational study comprising data from 976 cases of live-birth singleton pregnancies at the Shanghai General Hospital was conducted. The participants' clinical data were examined and subjected to a logistic regression analysis to identify the multitude of possible factors affecting the estimation of FW. The proportions and correlations between the accurate and inaccurate estimation groups were compared to determine the different prognosis of these 2 groups. The correlations between the accuracy of the sonographic-based fetal weight estimation (SFWE) and newborns with different weight ranges were also analyzed.Results: The overall accuracy rate of the SFWE predicted by the Hadlock IV formula was 79.61%, while that of the inaccurate estimation group was only 20.39%. The incidence of spontaneous vaginal delivery (VD) was lower in the inaccurate estimation group than in the accurate estimation group (40.7% vs. 48.13%; P=0.041). In the inaccurate estimation group, 11.56% (23/199) of the participants underwent a secondary cesarean section (sCS), compared to only 6.44% (50/777) in the accurate estimation group. The low birth weight (LBW) rates and macrosomia rates were lower in the accurate estimation group than in the inaccurate estimation group, with odds ratios (ORs) of 0.483 and 0.459, respectively (P<0.05). The results indicated that the SFWE was more accurate for newborns weighing 2,500-4,000 g than those weight out of this range. In relation to macrosomia, the SFWE was likely to be underestimated, but it was usually overestimated in the LBW group.Conclusions: The overall performance of the Hadlock IV formula in predicting the birth weight of Chinese newborns remains suboptimal. Extra caution should be exercised in cases of suspected large-for-gestational age (LGA) infants, small-for-gestational age (SGA) infants, infants with macrosomia, or LBW fetuses in the Chinese population.

Abstract:Background and Aims: The incidence of twin pregnancies has risen over the last few decades, mainly attributed to assisted reproduction techniques (ART). Fetal reduction (FR) can significantly reduce low birthweight (LBW) risk in twins. However, the LBW risk is still higher in singletons reduced from twins than for primary singletons. Many factors, such as maternal factors, FR timing and approach may affect LBW risks after FR and no relevant prediction models have been reported to date. The main objective of this study is to develop a nomogram to predict LBW risk in singleton pregnancies reduced from dichorionic (DC) twins. Method: We retrospectively reviewed and analyzed the data on women with (DC) twin pregnancies who underwent FR in Women’s Hospital, Zhejiang university, School of medicine in July 2005 to August 2021. Logistic LASSO regression was used to identify the most relevant variables associated with LBW. The nomogram was constructed and receiver operating characteristic (ROC) curve, Hosmer-Lemeshow test, and decision clinical analysis were used for model performances assessment and visualization. The model was evaluated by cohorts produced by 500 resampling bootstrap analysis to testify the stability. Results: A total of 471 patients were enrolled in the analysis. At last, seven independent predictive factors for LBW were identified and integrated to construct the nomograms, including maternal height, nulliparous, conception mode, reasons for FR, gestational age at FR, gestational diabetes, pregnancy hypertensive disease. The AUC of our prediction model was 0.793, which was validated by internal (AUC 0.762) confirmation with bootstrap analysis. The nomogram had well-fitted calibration curves. Decision curve analysis demonstrated that the nomogram was clinically useful. Conclusion: We made the first attempt to develop a reliable predictive nomogram for the risk of LBW in DC twin pregnancies reduced to singletons, providing a useful guide for clinicians and patients in making appropriate FR decisions.
Influence of perinatal factors on full-term low-birth-weight infants and construction of a predictive model

Abstract:BACKGROUND Being too light at birth can increase the risk of various diseases during infancy. AIM To explore the effect of perinatal factors on term low-birth-weight (LBW) infants and build a predictive model. This model aims to guide the clinical management of pregnant women’s healthcare during pregnancy and support the healthy growth of newborns. METHODS A retrospective analysis was conducted on data from 1794 single full-term pregnant women who gave birth. Newborns were grouped based on birth weight: Those with birth weight < 2.5 kg were classified as the low-weight group, and those with birth weight between 2.5 kg and 4 kg were included in the normal group. Multiple logistic regression analysis was used to identify the factors influencing the occurrence of full-term LBW. A risk prediction model was established based on the analysis results. The effectiveness of the model was analyzed using the Hosmer–Leme show test and receiver operating characteristic (ROC) curve to verify the accuracy of the predictions. RESULTS Among the 1794 pregnant women, there were 62 cases of neonatal weight < 2.5 kg, resulting in an LBW incidence rate of 3.46%. The factors influencing full-term LBW included low maternal education level [odds ratio (OR) = 1.416], fewer prenatal examinations (OR = 2.907), insufficient weight gain during pregnancy (OR = 3.695), irregular calcium supplementation during pregnancy (OR = 1.756), and pregnancy hypertension syndrome (OR = 2.192). The prediction model equation was obtained as follows: Logit (P ) = 0.348 × maternal education level + 1.067 × number of prenatal examinations + 1.307 × insufficient weight gain during pregnancy + 0.563 × irregular calcium supplementation during pregnancy + 0.785 × pregnancy hypertension syndrome − 29.164. The area under the ROC curve for this model was 0.853, with a sensitivity of 0.852 and a specificity of 0.821. The Hosmer–Leme show test yielded χ 2 = 2.185, P = 0.449, indicating a good fit. The overall accuracy of the clinical validation model was 81.67%. CONCLUSION The occurrence of full-term LBW is related to maternal education, the number of prenatal examinations, weight gain during pregnancy, calcium supplementation during pregnancy, and pregnancy-induced hypertension. The constructed predictive model can effectively predict the risk of full-term LBW.

Abstract:Objective:To evaluate the factors of predicting birth weight and develop a mathematical equation to predict birth weight on the pregnant women characteristics and fetal parameters at 20~24 weeks of gestation.Methods:235 healthy singleton pregnancy women were recruited at Prenatal Diagnosis Center of Jiangsu province from March to May in 2011.All pregnant women were in 20 to 24 weeks gestational age.They all had regular menstrual cycle of 28 days before pregnancy.Maternal weight(pre-pregnancy,mid-pregnancy,and at delivery)and body mass index(BMI),maternal age,height,newborn birth weight,the interval from ultrasound examination to term delivery were recorded.At the same time,the fetal biometrics including biparietal diameter(BPD),head circumference(HC),abdominal circumference(AC),femur length(FL)and humerus length(HL)were measured.Stepwise multiple regression analysis was used to develop the equation for term birth weight estimation by 173 women's data,the left 62 cases were used to verify the equation.Results:Maternal weight(pre-pregnancy,mid-pregnancy and at delivery)and body mass index(BMI),fetal AC,FL,HL,gestation at birth and the interval from ultrasound scan to term delivery were factors affecting birth weight significantly(P≤0.10).Using the stepwise regression analysis,an optimal formula was derived:estimated birth weight=-501.14+12.52×[time interval from mid-pregnancy to term delivery(days)]+34.19×[mid-pregnancy BMI(kg/m2)]+8.61×[abdominal circumference(mm)].Validation the regression equation,in 62 cases,there were 56 cases had absolute error less than±250g(90.32%,56/62).The cases which had an average error below 10% between predictive and actual birth weight accounted for 91.93%(57/ 62).Conclusion:Mid-pregnancy BMI,meanwhile fetal abdominal circumference and time interval from mid-pregnancy to term delivery are significant factors for birth weight estimation than other factors in this study.

Abstract:Background Birth weight influences profiles of dried blood amino-acids and acylcarnitines in newborn screening. This study aimed to define a more appropriate cut-off value to reduce the false positive rate and the number of recalled patients in newborn screening. Methods All babies who underwent newborn screening in our center were included; they were divided into groups by birth weight: 2500-3999 g (comparator group), <1000 g (group 1), 1000-1499 g (group 2), 1500-2499 g (group 3), and >4000 g (group 4). The 0.5th and 99.5th percentiles were used as the cut-off values. Comparisons were done on amino acid and acylcarnitines concentrations between the groups. False positive rate, positive predictive value, corrected false positive rate by birth weights were determined. Results Data on a total of 578,287 newborn infants were included in the analysis. The total false positive rate was 0.75%, and positive predictive value 2.89%. The false positive rate was 0.69%, 0.54% and 5.31% in infants with normal birth weight, birth weight of >4000 (group 4) and low birth weight of < 2500 g (groups 1, 2 and 3), respectively. Low-birth weight infants had much higher phenylalanine, tyrosine, methionine, arginine, propionylcarnitine, isovalerylcarnitine and octadecanoylcarnitine concentrations. Free carnitines and palmitoylcarnitine concentrations were lower. After adjusting for birth weight, false positive rate of all indices decreased to 0.53%, and positive predictive value increased to 4.31%. Conclusions Amino acid and carnitine concentrations in low-birth weight newborn infants may differ from the normal term newborn infants. The cut-off values of individual metabolites should be adjusted based on birth weight, to reduce false positive rate and increase positive predictive value.

Abstract:The results of four studies suggest that contamination concerns involved in prejudice towards male homosexuals may be expressed in the increased need for physical cleansing after an imagined contact with a homosexual man. Participants in Study 1 completed word fragments according to the theme of cleansing, and in Study 2, they chose a cleansing wipe more often after imagining using the mobile of a homosexual (vs. heterosexual) man. The need for cleansing was specific to the body parts engaged in the contact. In Study 3, participants evaluated hand and mouth cleansing products as more desirable after imagining using a mobile phone of a homosexual (vs. heterosexual) man. The specific need for cleansing, but not the accessibility of cleansing related words, was more pronounced among political conservatives (Study 4). The results are discussed with reference to the behavioral immune system hypothesis, research on moral disgust, and the embodiment literature.
