Abstract:Background Longitudinal data in health informatics studies often presented challenges due to sparse observations from each subject, limiting the application of contemporary deep learning for prediction. This issue was particularly relevant in predicting birthweight, a crucial factor in identifying conditions such as macrosomia and large-for-gestational age (LGA). Previous approaches had relied on empirical formulas for estimated fetal weights (EFWs) from ultrasound measurements and mixed-effects models for interim predictions. Method The proposed novel supervised longitudinal learning procedure features a three-step approach. Firstly, EFWs were generated using empirical formulas from ultrasound measurements. Subsequently, nonlinear mixed-effects models were employed to create augmented sequences of EFWs, spanning daily gestational timepoints. This augmentation transformed sparse longitudinal data into a dense parallel sequence suitable for training recurrent neural networks (RNNs). A tailored RNN architecture was then devised to incorporate the augmented sequential EFWs along with non-sequential maternal characteristics. Results The RNN algorithms were trained on augmented data to predict birthweights, which were further classified for macrosomia and LGA. Application of this supervised longitudinal learning procedure to the Successive Small-for-Gestational-Age Births study yielded improved performance in classification metrics. Specifically, sensitivity, area under the receiver operation characteristic curve, and Youden's Index demonstrated enhanced results, indicating the effectiveness of the proposed approach in overcoming sparsity challenges in longitudinal health informatics data. Conclusions The integration of mixed-effects models for temporal data augmentation and RNNs on augmented sequences showed effective in accurately predicting birthweights, particularly in the context of identifying excessive fetal growth conditions.

Abstract:There has been no machine learning study with a rich collection of clinical, sonographic markers to compare the performance measures for a variety of newborns’ weight-for-height indicators. This study compared the performance measures for a variety of newborns’ weight-for-height indicators based on machine learning, ultrasonographic data and maternal/delivery information. The source of data for this study was a multi-center retrospective study with 2949 mother–newborn pairs. The mean-squared-error-over-variance measures of five machine learning approaches were compared for newborn’s weight, newborn’s weight/height, newborn’s weight/height2 and newborn’s weight/hieght3. Random forest variable importance, the influence of a variable over average node impurity, was used to identify major predictors of these newborns’ weight-for-height indicators among ultrasonographic data and maternal/delivery information. Regarding ultrasonographic fetal biometry, newborn’s weight, newborn’s weight/height and newborn’s weight/height2 were better indicators with smaller mean-squared-error-over-variance measures than newborn’s weight/height3. Based on random forest variable importance, the top six predictors of newborn’s weight were the same as those of newborn’s weight/height and those of newborn’s weight/height2: gestational age at delivery time, the first estimated fetal weight and abdominal circumference in week 36 or later, maternal weight and body mass index at delivery time, and the first biparietal diameter in week 36 or later. These six predictors also ranked within the top seven for large-for-gestational-age and the top eight for small-for-gestational-age. In conclusion, newborn’s weight, newborn’s weight/height and newborn’s weight/height2 are more suitable for ultrasonographic fetal biometry with smaller mean-squared-error-over-variance measures than newborn’s weight/height3. Machine learning with ultrasonographic data would be an effective noninvasive approach for predicting newborn’s weight, weight/height and weight/height2.

Constructing small for gestational age prediction models: A retrospective machine learning study

Abstract:Objective To develop machine learning prediction models for small for gestational age with baseline characteristics and biochemical tests of various pregnancy stages individually and collectively and compare predictive performance. Study design This retrospective study included singleton pregnancies with infants born between May 2018 and March 2023. Small for gestational age was defined as a birth weight below the 10th percentile according to the Intergrowth-21st fetal growth standards. The pregnancy data were categorized into four datasets at different gestational time points (14 and 28 weeks and admission). The LightGBM framework was utilized to assess the variable importance by employing a five-fold cross-validation. RandomizedSearchCV and sequential feature selection were applied to estimate the optimal number of features. Seven machine learning algorithms were used to develop prediction models, with an 8:2 ratio for training and testing. The model performance was evaluated using receiver operating characteristic curve analysis and sensitivity at a false positive rate of 10 %. Results We included data of 4,394 women with singleton pregnancies, including 148 (3.4%) small for gestational age infants. Women delivering small for gestational age infants exhibited significantly shorter stature and lower fundal height and abdominal circumference at admission. Maternal height, age, and pre-pregnancy weight consistently ranked among the top 20 features in prediction models with any dataset. The models incorporated variables of admission stage have strong predictive performance with the area under the curves exceeding 0.8. The prediction model developed with variables of admission stage yielded the best performance, achieving an area under the curve of 0.85 and a sensitivity of 73% at the false positive rate of 10%. Conclusions By machine learning, various pregnancy stages' prediction models for small for gestational age showed good predictive performance, and the predictive value of variables at each pregnancy stage was fully explored. The prediction model with the best performance was established with variables of admission stage and emphasized the significance of prenatal physical examinations.

Abstract:The results of four studies suggest that contamination concerns involved in prejudice towards male homosexuals may be expressed in the increased need for physical cleansing after an imagined contact with a homosexual man. Participants in Study 1 completed word fragments according to the theme of cleansing, and in Study 2, they chose a cleansing wipe more often after imagining using the mobile of a homosexual (vs. heterosexual) man. The need for cleansing was specific to the body parts engaged in the contact. In Study 3, participants evaluated hand and mouth cleansing products as more desirable after imagining using a mobile phone of a homosexual (vs. heterosexual) man. The specific need for cleansing, but not the accessibility of cleansing related words, was more pronounced among political conservatives (Study 4). The results are discussed with reference to the behavioral immune system hypothesis, research on moral disgust, and the embodiment literature.

Abstract:Background Improving the accuracy of estimated fetal weight (EFW) calculation can contribute to decision-making for obstetricians and decrease perinatal complications. This study aimed to develop a deep neural network (DNN) model for EFW based on obstetric electronic health records. Methods This study retrospectively analyzed the electronic health records of pregnant women with live births delivery at the obstetrics department of International Peace Maternity & Child Health Hospital between January 2016 and December 2018. The DNN model was evaluated using Hadlock’s formula and multiple linear regression. Results A total of 34824 live births (23922 primiparas) from 49896 pregnant women were analyzed. The root-mean-square error of DNN model was 189.64 g (95% CI 187.95 g—191.16 g), and the mean absolute percentage error was 5.79% (95%CI: 5.70%—5.81%), significantly lower compared to Hadlock’s formula (240.36 g and 6.46%, respectively). By combining with previously unreported factors, such as birth weight of prior pregnancies, a concise and effective DNN model was built based on only 10 parameters. Accuracy rate of a new model increased from 76.08% to 83.87%, with root-mean-square error of only 243.80 g. Conclusions Proposed DNN model for EFW calculation is more accurate than previous approaches in this area and be adopted for better decision making related to fetal monitoring.
Early Prediction of High Risk Gestational Diabetes Mellitus <i>via</i> Machine Learning Models

Abstract:Aims: Gestational diabetes mellitus (GDM) is a pregnancy-specific disorder that can usually be diagnosed after 24 gestational weeks. So far, there is no accurate method to predict GDM in early pregnancy. Methods: We collected data extracted from the hospital’s electronic medical record system included 73 features in the first trimester. We also recorded the occurrence of GDM, diagnosed at 24-28 weeks of pregnancy. We conducted a feature selection method to select a panel of most discriminative features. We then developed advanced machine learning models, using Deep Neural Network (DNN), Support Vector Machine (SVM), K-Nearest Neighboring (KNN), and Logistic Regression (LR), based on these features. Results: We studied 16,819 women (2,696 GDM) and 14,992 women (1,837 GDM) for the training and validation group. DNN, SVM, KNN, and LR models based on the 73-feature set demonstrated the best discriminative power with corresponding area under the curve (AUC) values of 0.92 (95%CI 0.91, 0.93), 0.82 (95%CI 0.81, 0.83), 0.63 (95%CI 0.62, 0.64), and 0.85 (95%CI 0.84, 0.85), respectively. The 7-feature (selected from the 73-feature set) DNN, SVM, KNN, and LR models had the best discriminative power with corresponding AUCs of 0.84 (95%CI 0.83, 0.84), 0.69 (95%CI 0.68, 0.70), 0.68 (95%CI 0.67, 0.69), and 0.84 (95% CI 0.83, 0.85), respectively. The 7-feature LR model had the best Hosmer-Lemeshow test outcome. Notably, the AUCs of the existing prediction models did not exceed 0.75. Deep Learning in Predicting Preterm Birth: A Comparative Study of Machine Learning Algorithms

Abstract:Objective: To determine whether deep learning algorithms are suitable for predicting preterm birth.Methods: A retrospective study was conducted at Peking University Third Hospital from January 2018 to June 2023. Birth data were divided into two parts based on the date of delivery: the first part was used for model training and validation, while real world viability was evaluated using the second part. Four machine learning algorithms (logistic regression, random forest, support vector machine, and transformer) were employed to predict preterm birth. Receiver operating characteristic curves were plotted, and the area under the curve (AUC), sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and accuracy were calculated.Results: This research included data on 30,965 births, where 24,770 comprised the first part, and included 3164 (12.77%) in the preterm birth group, with 6195 in the second part, including 795 (12.83%) in the preterm birth group. Significant differences in various factors were observed between the preterm and full-term birth groups. The transformer model (AUC = 79.20%, sensitivity = 73.67%, specificity = 72.48%, PPV = 28.21%, NPV = 94.95%, and accuracy = 72.61 % in the test dataset) demonstrated superior performance relative to logistic regression (AUC = 77.96% in the test dataset), support vector machine (AUC = 71.70% in the test dataset), and random forest (AUC = 75.09% in the test dataset) approaches.Conclusion: This study highlights the promise of deep learning algorithms, specifically the transformer algorithm, for predicting preterm birth.

Abstract:Abstract Fetal macrosomia is associated with maternal and newborn complications due to incorrect fetal weight estimation or inappropriate choice of delivery models. The early screening and evaluation of macrosomia in the third trimester can improve delivery outcomes and reduce complications. However, traditional clinical and ultrasound examinations face difficulties in obtaining accurate fetal measurements during the third trimester of pregnancy. This study aims to develop a comprehensive predictive model for detecting macrosomia using machine learning (ML) algorithms. The accuracy of macrosomia prediction using logistic regression, k-nearest neighbors, support vector machine, random forest (RF), XGBoost, and LightGBM algorithms was explored. Each approach was trained and validated using data from 3244 pregnant women at a hospital in southern China. The information gain method was employed to identify deterministic features associated with the occurrence of macrosomia. The performance of six ML algorithms based on the recall and area under the curve evaluation metrics were compared. To develop an efficient prediction model, two sets of experiments based on ultrasound examination records within 1-7 days and 8-14 days prior to delivery were conducted. The ensemble model, comprising the RF, XGBoost, and LightGBM algorithms, showed encouraging results. For each experimental group, the proposed ensemble model outperformed other ML approaches and the traditional Hadlock formula. The experimental results indicate that, with the most risk-relevant features, the ML algorithms presented in this study can predict macrosomia and assist obstetricians in selecting more appropriate delivery models.

Selection of Methods Predicting Fetal Weight Using Ultrasonography

Abstract:Objective The neural network and regression methods were used to compare the accuracy of predicting fetal weight. Methods Of a total of 390 cases, ultrasonographic parameters as BPD, HD, ThD, AD, FL, LL, STT and SCT were measured at 72 hour before delivery. Among them, 250 cases utilized training neural network systems and establish a series of regression formulae for fetal weight prediction. In the other 140 cases, fetal weights were by neural network, regression equation and the traditional method for comparison of accuracy. Results The 6 variable neural network has the highest coincide rate, 88.6%, with a relative error of 0.8±5.2%. The regression methods have a little lower coincident rate. Among them, the 6 variable regression equation has a coincident rate of 85.7%, and the 2 variable has a rate of 83.6%. AD, FL and BPD are useful variables which affect the accuracy in the fetal weight prediction. Conclusions 1. The neural network is a new method to predict fetal body weight with high accuracy. The regression equations, with their ease of use and preciseness, are still the mainstay of predicting fetal body weight as well. 2. AD, FL and BPD are important parameters in predicting fetal weight. 3. It is necessary to use different method in predicting different fetal weight, such as over weight and under weight.
Predicting the birth weight by pregnant women characteristics and fetal ultrasonic parameters at 20～24 weeks of gestation

Abstract:Objective:To evaluate the factors of predicting birth weight and develop a mathematical equation to predict birth weight on the pregnant women characteristics and fetal parameters at 20~24 weeks of gestation.Methods:235 healthy singleton pregnancy women were recruited at Prenatal Diagnosis Center of Jiangsu province from March to May in 2011.All pregnant women were in 20 to 24 weeks gestational age.They all had regular menstrual cycle of 28 days before pregnancy.Maternal weight(pre-pregnancy,mid-pregnancy,and at delivery)and body mass index(BMI),maternal age,height,newborn birth weight,the interval from ultrasound examination to term delivery were recorded.At the same time,the fetal biometrics including biparietal diameter(BPD),head circumference(HC),abdominal circumference(AC),femur length(FL)and humerus length(HL)were measured.Stepwise multiple regression analysis was used to develop the equation for term birth weight estimation by 173 women's data,the left 62 cases were used to verify the equation.Results:Maternal weight(pre-pregnancy,mid-pregnancy and at delivery)and body mass index(BMI),fetal AC,FL,HL,gestation at birth and the interval from ultrasound scan to term delivery were factors affecting birth weight significantly(P≤0.10).Using the stepwise regression analysis,an optimal formula was derived:estimated birth weight=-501.14+12.52×[time interval from mid-pregnancy to term delivery(days)]+34.19×[mid-pregnancy BMI(kg/m2)]+8.61×[abdominal circumference(mm)].Validation the regression equation,in 62 cases,there were 56 cases had absolute error less than±250g(90.32%,56/62).The cases which had an average error below 10% between predictive and actual birth weight accounted for 91.93%(57/ 62).Conclusion:Mid-pregnancy BMI,meanwhile fetal abdominal circumference and time interval from mid-pregnancy to term delivery are significant factors for birth weight estimation than other factors in this study.

Abstract:A newborn with a birth weight above the 90th percentile of same gestational age is termed as large for gestational age. Large for gestational age suffers from serious complications during and after the antepartum period because they do not get earlier identification of the disease. Earlier recognition of large for gestational age infants could slow progression and prevent further complication of the disease. In medical science, prevention and mitigation of disease require examination of biochemical indicators. Machine learning has been evolved and envisioned as a tool to predict large for gestational age infants with most deterministic characteristics. This study aims to identify most deterministic biochemical indicators for large for gestational age prediction with minimal computational overhead. To the best of my knowledge, this is the first time a study is carried out to identify the most deterministic risk factors associated with large for gestational age and to develop large for gestational age prediction model using machine learning techniques. To develop an efficient large for gestational age prediction model, we conducted three group of experiments that considered basic machine learning methods; feature selection; and imbalanced data, respectively. Support vector machine, logistic regression, Naive Bayes and Random Forest were trained using tenfold cross-validation on large for gestational age dataset; we selected precision and area under the curve as a performance evaluation metrics; information gain an entropy-based feature selection method was adopted to rank features; we introduced an ensemble data imbalance technique in the last group of experiments. For each group of experiments, support vector machine performed best compared to other machine learning classifiers by producing the highest prediction precision score of 85%. All of the classifiers performed best with thirty ranked features subset, which validates the applied method to recognize the most deterministic risk factors associated with large for gestational age prediction.
Comparative effectiveness of explainable machine learning approaches for extrauterine growth restriction classification in preterm infants using longitudinal data

Abstract:Introduction Preterm birth is a leading cause of infant mortality and morbidity. Despite the improvement in the overall mortality in premature infants, the intact survival of these infants remains a significant challenge. Screening the physical growth of infants is fundamental to potentially reducing the escalation of this disorder. Recently, machine learning models have been used to predict the growth restrictions of infants; however, they frequently rely on conventional risk factors and cross-sectional data and do not leverage the longitudinal database associated with medical data from laboratory tests. Methods This study aimed to present an automated interpretable ML-based approach for the prediction and classification of short-term growth outcomes in preterm infants. We prepared four datasets based on weight and length including weight baseline, length baseline, weight follow-up, and length follow-up. The CHA Bundang Medical Center Neonatal Intensive Care Unit dataset was classified using two well-known supervised machine learning algorithms, namely support vector machine (SVM) and logistic regression (LR). A five-fold cross-validation, and several performance measures, including accuracy, precision, recall and F1-score were used to compare classifier performances. We further illustrated the models’ trustworthiness using calibration and cumulative curves. The visualized global interpretations using Shapley additive explanation (SHAP) is provided for analyzing variables’ contribution to final prediction. Results Based on the experimental results with area under the curve, the discrimination ability of the SVM algorithm was found to better than that of the LR model on three of the four datasets with 81%, 76% and 72% in weight follow-up, length baseline and length follow-up dataset respectively. The LR classifier achieved a better ROC score only on the weight baseline dataset with 83%. The global interpretability results revealed that pregnancy-induced hypertension, gestational age, twin birth, birth weight, antenatal corticosteroid use, premature rupture of membranes, sex, and birth length were consistently ranked as important variables in both the baseline and follow-up datasets. Discussion The application of machine learning models to the early detection and automated classification of short-term growth outcomes in preterm infants achieved high accuracy and may provide an efficient framework for clinical decision systems enabling more effective monitoring and facilitating timely intervention.

Abstract:Accurate estimation of fetal weight before delivery is of great benefit to limit the potential complication associated with the low-birth-weight infants. Although the regression analysis has been used as a daily clinical means to estimate the fetal weight on the basis of ultrasound measurements, it still lacks enough accuracy for low-birth-weight fetuses. The ineffectiveness is mainly due to the large inter- or intraobserver variability in measurements and the inappropriateness of the regression analysis. A novel method based on the support vector regression (SVR) is proposed to improve the weight estimation accuracy for fetuses of less than 2500 g. Here, fuzzy logic is introduced into SVR (termed FSVR) to limit the contribution of inaccurate training data to the model establishment, and thus, to enhance the robustness of FSVR to noisy data. To guarantee the generalization performance of the FSVR model, the nondominated sorting genetic algorithm (NSGA) is utilized to obtain the optimal parameters for the FSVR, which is referred to as the evolutionary fuzzy support vector regression (EFSVR) model. Compared with regression formulas, back-propagation neural network, and SVR, EFSVR achieves the lowest mean absolute percent error (6.6%) and the highest correlation coefficient (0.902) between the estimated fetal weight and the actual birth weight. The EFSVR model produces significant improvement (1.9%-4.2%) on the accuracy of fetal weight estimation over several widely used formulas. Experiments show the potential of EFSVR in clinical prenatal care.
Fibromyalgia treatment with intravenous tropisetron administration.

Abstract:A prospective, randomized, placebo-controlled, multicenter, double-blind trial in fibromyalgia patients demonstrated that peroral daily treatment with 5 mg tropisetron for 10 days produced a significant reduction in pain and other symptoms. The aim of the present study was to determine whether intravenous administration of 2 mg tropisetron daily for a limited period of time would produce quicker and more favorable results. In the first cohort 18 fibromyalgia patients received a single intravenous injection of 2 mg tropisetron. In the second cohort 24 fibromyalgia patients were treated with 2 mg intravenous tropisetron daily for 5 days. Pain intensity was measured with the visual analog scale and the pain score. Pain at tender and control points (dolorimeter) as well as 17 ancillary symptoms before and after treatment were evaluated. Pain intensity was followed-up by means of a patient diary until recurrence. Dolorimetry revealed that a single intravenous injection of 2 mg tropisetron significantly reduced pain and enhanced pain threshold. These effects, however, lasted for only a few days. Of 18 patients in the first cohort, only three showed no response to therapy. Of the 24 patients in the second cohort, 23 showed pain reduction when 2 mg tropisetron was administered daily for 5 days. Pain relief lasted for 2 weeks to 2 months in 20 of these patients. Two patients stopped filling in the pain diary. Twelve ancillary symptoms such as sleep disturbances, fatigue, morning stiffness were also significantly improved by the latter treatment. In the global assessment 16 out of 24 patients showed significant improvement and seven showed slight improvement. Only one patient experienced no improvement. Tolerability was good. In conclusion, intravenous injection of 2 mg of the 5-hydroxytryptamine3 receptor antagonist tropisetron once daily for 5 days produced a longer-lasting therapeutic effect on fibromyalgia symptoms than did peroral daily treatment with 5 mg of this drug. The results achieved are currently being evaluated in a randomized, placebo-controlled, double-blind trial.
Identifying First-Trimester Risk Factors for SGA-LGA Using Weighted Inheritance Voting Ensemble Learning

Abstract:The classification of fetuses as Small for Gestational Age (SGA) and Large for Gestational Age (LGA) is a critical aspect of neonatal health assessment. SGA and LGA, terms used to describe fetal weights that fall below or above the expected weights for Appropriate for Gestational Age (AGA) fetuses, indicate intrauterine growth restriction and excessive fetal growth, respectively. Early prediction and assessment of latent risk factors associated with these classifications can facilitate timely medical interventions, thereby optimizing the health outcomes for both the infant and the mother. This study aims to leverage first-trimester data to achieve these objectives. This study analyzed data from 7943 pregnant women, including 424 SGA, 928 LGA, and 6591 AGA cases, collected from 2015 to 2021 at the Third Affiliated Hospital of Sun Yat-sen University in Guangzhou, China. We propose a novel algorithm, named the Weighted Inheritance Voting Ensemble Learning Algorithm (WIVELA), to predict the classification of fetuses into SGA, LGA, and AGA categories based on biochemical parameters, maternal factors, and morbidity during pregnancy. Additionally, we proposed algorithms for relevance determination based on the classifier to ascertain the importance of features associated with SGA and LGA. The proposed classification solution demonstrated a notable average accuracy rate of 92.12% on 10-fold cross-validation over 100 loops, outperforming five state-of-the-art machine learning algorithms. Furthermore, we identified significant latent maternal risk factors directly associated with SGA and LGA conditions, such as weight change during the first trimester, prepregnancy weight, height, age, and obstetric factors like fetal growth restriction and birthing LGA baby. This study also underscored the importance of biomarker features at the end of the first trimester, including HDL, TG, OGTT-1h, OGTT-0h, OGTT-2h, TC, FPG, and LDL, which reflect the status of SGA or LGA fetuses. This study presents innovative solutions for classifying and identifying relevant attributes, offering valuable tools for medical teams in the clinical monitoring of fetuses predisposed to SGA and LGA conditions during the initial stage of pregnancy. These proposed solutions facilitate early intervention in nutritional care and prenatal healthcare, thereby contributing to enhanced strategies for managing the health and well-being of both the fetus and the expectant mother.

Abstract:Background Machine learning could be used for prognosis/diagnosis of maternal and neonates' diseases by analyzing the data sets and profiles obtained from a pregnant mother. Purpose We aimed to develop a prediction model based on machine learning algorithms to determine important maternal characteristics and neonates' anthropometric profiles as the predictors of neonates' health status. Methods This study was conducted among 1280 pregnant women referred to healthcare centers to receive antenatal care. We evaluated several machine learning methods, including support vector machine (SVM), Ensemble, K-Nearest Neighbor (KNN), Naïve Bayes (NB), and Decision tree classifiers, to predict newborn health state. Results The minimum redundancy-maximum relevance (MRMR) algorithm revealed that variables, including head circumference of neonates, pregnancy intention, and drug consumption history during pregnancy, were top-scored features for classifying normal and unhealthy infants. Among the different classification methods, the SVM classifier had the best performance. The average values of accuracy, precision, recall, F1-score, and area under the receiver operating characteristic curve (AUC) in the test group were 75%, 75%, 76%, 76%, and 65%, respectively, for SVM model. Conclusion Machine learning methods can efficiently forecast the neonate's health status among pregnant women. This study proposed a new approach toward the integration of maternal data and neonate profiles to facilitate the prediction of neonates' health status.

Abstract:<span>Background: Fetal weight is an important parameter to ensure maternal and child safety. The purpose of this study was to use three-dimensional (3D) limb volume ultrasound combined with fetal abdominal circumference (AC) measurement to establish a model to predict fetal weight and evaluate its efficiency.Methods: A total of 211 participants with single pregnancy (28–42 weeks) were selected between September 2017 and December 2018 in the Beijing Obstetrics and Gynecology Hospital of Capital Medical University. The upper arm (AVol)/thigh volume (TVol) of fetuses was measured by the 3D limb volume technique. Fetal AC was measured by two-dimensional ultrasound. Nine cases were excluded due to incomplete information or the interval between examination and delivery >7 days. The enrolled 202 participants were divided into a model group (134 cases, 70%) and a verification group (68 cases, 30%) by mechanical sampling method. The linear relationship between limb volume and fetal weight was evaluated using Pearson Chi-squared test. The prediction model formula was established by multivariate regression with data from the model group. Accuracy of the model formula was evaluated with verification group data and compared with traditional formulas (Hadlock, Lee2009, and INTERGROWTH-21st) by paired t-test and residual analysis. Receiver operating characteristic curves were generated to predict macrosomia.Results: AC, AVol, and TVol were linearly related to fetal weight. Pearson correlation coefficient was 0.866, 0.862, and 0.910, respectively. The prediction model based on AVol/TVol and AC was established as follows: Y = −481.965 + 12.194TVol + 15.358AVol + 67.998AC, R2adj = 0.868. The scatter plot showed that when birth weight fluctuated by 5% (i.e., 95% to 105%), the difference between the predicted fetal weight by the model and the actual weight was small. A paired t-test showed that there was no significant difference between the predicted fetal weight and the actual birth weight (t = −1.015, P = 0.314). Moreover, the residual analysis showed that the model formula's prediction efficiency was better than the traditional formulas with a mean residual of 35,360.170. The combined model of AVol/TVol and AC was superior to the Lee2009 and INTERGROWTH-21st formulas in the diagnosis of macrosomia. Its predictive sensitivity and specificity were 87.5% and 91.7%, respectively.Conclusion: Fetal weight prediction model established by semi-automatic 3D limb volume combined with AC is of high accuracy, sensitivity, and specificity. Machine learning model-based preterm birth prediction and clinical nomogram: A big retrospective cohort study

Abstract:Objective: This study sought to develop a multifactorial predictive model for preterm birth risk, with the goal of providing clinical practitioners with early prevention. Methods: This retrospective cohort study utilized 2022 and 2018 National Vital Statistics System (NVSS) birth data, with the 2022 cohort arbitrarily split into training (70%) and internal verification (30%) subsets, and the 2018 cohort for external validation. Four machine learning algorithms-logistic regression, adaptive lasso regression, bootstrap forest, and boosted trees-identified features associated with preterm birth. The study then integrated the consensus features identified across the four models to construct a logistic regression-based preterm birth prediction nomogram. To evaluate the model's efficacy, calibration, receiver operating characteristic (ROC), and decision curve analysis were applied to both the internal and external validation sets. Results: The study included 2 567 040 mother-infant pairs from the 2022 cohort and 2 688 568 mother-infant pairs from the 2018 cohort. All four machine learning models demonstrated high accuracy (area under the curve [AUC] >0.7) in predicting preterm birth, and the internal validation results indicated good model generalizability. Feature selection identified nine common risk factors associated with preterm birth. The prediction nomogram based on these nine common features achieved AUCs of 0.701, 0.702, and 0.704 in the training, internal validation, and external validation sets, respectively. The calibration curves showed good agreement, and the decision curve analysis confirmed the model's net clinical benefits. Conclusion: This study developed a reliable preterm birth prediction tool using large-scale birth cohort data, filling the gap of lacking external validation for existing preterm birth prediction models.
