Abstract:Objective To evaluate the efficacy on root development of orthodontic traction for impacted maxillary central incisor in mixed dentition.Methods 9 cases of 8 to 11 years old impacted maxillary central incisor patients were treated with traction.CBCT measurement on root length,root canal wall thickness,and degree of root bending and the ratio of crown and root of the impacted teeth were implemented,and compared with the contralateral counterpart tooth.Results 9 teeth were treated to standard place.After traction,incisor root continues development,but root length is shorter than the control teeth.Crown root ratio and root wall thickness presented no significant difference compared to control teeth.Orthodontic treatment may not be able to change the original root dilaceration,and may cause new dilaceration during traction process.Conclusions Impacted maxillary central incisor after orthodontic traction can achieve satisfied clinical results,but indications for traction during mixed dentition should be carefully evaluated.
Periodontal status of maxillary central incisors after orthodontic traction: a longitudinal follow-up

Abstract:Background: The aim of this study was to evaluate the clinical and radiographic periodontal status of impacted permanent maxillary central incisors (Mx.1) after a long term of orthodontic traction. Methodology: This split-mouth study evaluated a sample of 11 patients (five females, six males) treated with Mx.1 unilateral traction one to 28 years after the removal of orthodontic appliances. The traction Group (TG) consisted of 11 Mx.1 and the Comparison Group (CG) comprised 11 spontaneously erupted contralateral Mx.1. High-resolution CBCT exams of central incisors were performed using Accuitomo (J. Morita, Kyoto, Japan). Cross-section imagens passing through the center of maxillary central incisors were used to measure buccal and lingual alveolar bone level. Presence of fenestration, root dilacerations, root coverage, and position of the root apex were also assessed in the same images. Clinical parameters included periodontal probing depth, attachment level, gingival bleeding index, plaque index, degree of gingival recession, amount of gingival mucosa, and evaluation of interproximal papilla and black triangle. Digital model analysis included an assessment of clinical crown height and width. Intergroup comparisons were performed using paired t-, McNemar's, and Wilcoxon tests (p<0.05). Results: Compared to CG, we found a significantly thinner labial bone plate thickness in TG at the middle (p=0.000) and apical (p=0.009) root level. We also observed an apical displaced labial bone crest level in TG (p=0.000). The Traction Group showed a greater frequency of root dilacerations and gingival recessions, a decreased amount of keratinized mucosa, and a decreased clinical attachment level at the labial aspect compared to contralateral teeth. Conclusions: A decreased thickness and height of labial alveolar bone and gingival recessions were found in maxillary central incisors 15 years after orthodontic traction. Though incisor traction might cause some periodontal impact, differences are acceptable under a clinical point of view considering the cost-benefit ratio.

Abstract:OBJECTIVE:To evaluate the orthodontic treatment outcome in patients with impacted maxillary central incisor in the mixed dentition.METHODS:Nine patients, aged 8 to 11 years, with impacted maxillary central incisor were treated orthodontically. The cone-beam CT(CBCT) was taken before treatment, after treatment and one year out of retention to evaluate the root length, root canal wall thickness, width of the apical foramen, and degree of root bending, alveolar bone height and thickness. The crown-to-root ratio was calculated. The periodontal and endodontic conditions were evaluated. The parameters of the treated incisors and contralateral ones served as controls were compared.RESULTS:Nine impacted teeth were treated successfully. Throughout the treatment, the root of impacted central incisor continued to develop and the alveolar bones also continued to develop and remodel. The condition of the alveolar bone of vertically impacted teeth was better than that of horizontally impacted ones. No periodontal pocket or pulp necrosis was found after treatment. Seven patients were examined one year after treatment. No significant difference was found in root length, palatal alveolar bone level and palatal alveolar bone thickness. The control group root length was (13.07±2.15) mm, the treatment group root length was (12.06±2.00) mm. No further alveolar bone loss, gingival recession and pulp necrosis were found. The control group labial and palatal alveolar bone levels were (0.90±0.62), (0.45±0.52) mm, labial and palatal alveolar bone thickness were (0.85±0.14), (1.21±0.41) mm. The treatment group labial and palatal alveolar bone levels were (2.18±1.59) mm, (0.57±0.71) mm, labial and palatal alveolar bone thickness were (0.48±0.29), (1.43±0.31) mm.CONCLUSIONS:Orthodontic therapy for impacted maxillary central incisor in the mixed dentition could promote root development and alveolar bone remodeling. Good periodontal and endodontic conditions were achieved.
[A retrospective study on incisor root resorption in patients treated with bracketless invisible appliance and straight wire appliance]

Abstract:Purpose: To assess root resorptions in patients treated with bracketless invisible appliance and straight wire appliance using cone-beam CT (CBCT). Methods: Twenty-eight patients treated with bracketless invisible appliance (as experimental group) or straight wire appliance (as control group) were randomly selected. CBCT images were analyzed at 3 time points (pre-operation, 6 month after operation, post-operation). Root resorption was calculated using root length at 3 time points. The difference between the 2 groups was analyzed with SPSS16.0 software package. To those teeth with no root length reduction, the bone defect was evaluated using CBCT. Results: After 6-month of treatment, 47.3% teeth from the experimental group had root resorption, while 68.3% in the control group. There was significant difference between the 2 groups (P<0.05). Root absorption of maxillary central incisors from the 2 groups was more severe than other teeth (55% in the experimental group, and 75% in the control group). After orthodontic treatment, the incidence of root resorption in the control groups increased to be 85.3% and 68.3% in the experimental group (P<0.05). To those teeth with no root resorption, bone defect was more common in the palatal side (18% in the experimental group, 14% in the control group). Conclusions: Compared with straight wire appliance, patients using bracketless invisible appliance suffer from less root resorption.
Evaluation of Root Resorption after Comprehensive Orthodontic Treatment Using Cone Beam Computed Tomography (CBCT): a Meta-Analysis

Abstract:Background: Orthodontic treatment can result in root resorption (RR). Traditional two-dimensional (2D) data exhibit magnification, deformation and positioning problems. Cone beam computed tomography (CBCT) contains more accurate three-dimensional (3D) information. This study identified and qualified the extent and location of root resorption using cone beam computed tomography (CBCT) after comprehensive orthodontic treatment. Methods: Studies comparing the RR before and after comprehensive orthodontic treatment using CBCT were identified using electronic searches of databases, including Cochrane, PubMed, EMBASE, China National Knowledge Infrastructure (CNKI) and Web of Science, and manual searches in relevant journals and the reference lists of the included studies until Oct 25, 2017. The extraction of data and the risk of bias evaluation were conducted by two investigators independently. The methodological quality of the included studies was assessed using the methodological index for non-randomized studies (MINORS). Studies that reported the length and volume of teeth were used for quantitative analyses. Results: Twelve studies were included in the meta-analysis. The length of all teeth after intervention was significantly shorter than that before treatment (MD = 0.80, 95% CI 0.56, 1.03, P < 0.00001). The sequence of RR from heaviest to lightest was maxillary lateral incisors, maxillary central incisors, mandibular anterior teeth, and maxillary canines. Studies were divided into two subgroups based on the use of tooth extraction. Root shortening after treatment was observed in both groups, and extraction caused more root resorption than was observed in the non-extraction group. Conclusions: There were different degrees of root resorption after orthodontics, but it was clinically acceptable. Root resorption established in CBCT research was less serious and more accurate than that observed in the two-dimensional research. Current evidence suggests that root length and volume were reduced after orthodontic treatment. The order of the amount of RR was maxillary lateral incisors, maxillary central incisors and mandibular anterior teeth. Most of the articles were complicated by different confounding factors. Therefore, more high-quality clinical trials are needed to determine the risk factors of root resorption and optimal protocols for treatment and to draw more reliable conclusions.
Three-dimensional root resorption evaluation of maxillary anterior teeth in skeletal class Ⅲ patients: a cone beam computed tomography study

Abstract:Objective:To investigate root resorption of maxillary anterior teeth in skeletal class Ⅲ patients using cone beam computed tomography (CBCT) three-dimensional reconstructed images.Methods:The samples consisted of 25 skeletal class Ⅲ patients who went through orthodontic-surgical treatment. CBCT data of each patient at T0 (pretreatment), T1(preoperative), T2(posttreatment) were acquired and 3D digital root models were reconstructed. Root length and segmented root volumes(whole root, cervical part and apical part) of maxillary central incisors and canines were calculated to investigate the characteristics of root resorption between different teeth positions during the orthodontic-surgical treatment.Results:(1) Maxillary central incisors exhibited significant decrease of root length and volume in presurgical stage (T0 to T1) and postsurgical stage (T1 to T2 )( P=0.000), Maxillary canines showed significant root length decrease during presurgical stage (T0 to T1) while apparent volume resorption happened during postsurgical stage (T1 to T2)( P=0.000). (2) Maxillary central incisors and canines showed less volume resorption in cervical root than apical root. The ratio of the mean amount of volume change were about 1: 2; (3) From stage T0 to T2, the changing amount of root volume showed no difference between maxillary central incisors and canines (maxillary central incisor (21.26±13.33) mm 3, maxillary canine (20.02±10.75) mm 3), but maxillary central incisors had greater proportion of changing amount (13.10±7.65%) than maxillary canines (8.91±4.88%) ( P=0.031). Conclusions:During the course of orthodontic-surgical treatment of skeletal class Ⅲ patients, maxillary central incisors and canines showed apparent root length decrease and volume resorption, and cervical root resorption could not be ignored. Maxillary central incisors resorb worse than canines. The volume measurement provides a new sensitive way to explore root resorption.
[Effect of different orthodontic techniques on alveolar bone changes in the upper incisor area of patients with periodontitis]

Abstract:Purpose: To explore the effects of different orthodontic techniques on the changes of alveolar bone density, height and gingival crevicular fluid (GCF) bleeding in upper incisor in patients with periodontitis. Methods: Twenty-three patients with moderate periodontitis who underwent orthodontic treatment from January 2016 to December 2019 in the Department of Stomatology of Jiangxi Pingxiang People's Hospital were divided into experimentalexperimental group(n=12) and control group(n=11). Patients in the experimental group were treated with bracket-less invisible appliance, while patients in the control group were treated with conventional lip-side fixed appliance. Changes of gingival crevicular hemorrhage index, probing depth, alveolar bone height and bone density in the incisor area were compared between the two groups before and after treatment. Statistical analysis was performed on data using SPSS 22.0 software package. Results: After treatment, the bone mineral density of the upper incisor alveolar crest (L1) of the two groups decreased, and the rate of decrease in the experimental group was significantly less than that in the control group (P<0.05). The bone density at the apical point of the anatomical root of the control group at 1 mm (L3) was also significantly reduced, but there was no significant change in the experimental group. There was no significant change in the alveolar bone height of the upper incisor area between the two groups before and after treatment (P>0.05). Root resorption of the upper incisors in the experimental group was significantly lower than that in the control group(P<0.05). After treatment, the probing depth and GCF bleeding index of the two groups decreased, and the change in the experimental group was significantly greater than that in the control group(P<0.05). Conclusions: Bracket-free invisible appliance for periodontitis is more conducive to the restoration of alveolar bone mineral density in the upper incisor than the conventional lip-side fixed appliance, and both of which have the same effect on bone height and are safe and effective orthodontic treatment.
Effect of Orthodontic Treatment on the Outcomes of Endodontically Treated Teeth: A Cone-Beam Computed Tomography Analysis.

Abstract:Introduction: Often there is the need of moving endodontically treated teeth. Orthodontic movement may have no effect on the prognosis of teeth with root canal treatment (RCT). To verify this subject, we evaluated the effect of orthodontic movement on the prognosis of RCT teeth using cone-beam computed tomography (CBCT) and further explored the influence of orthodontic movement on the prognosis of RCT teeth with and without apical periodontitis (AP). Methods: This retrospective study was conducted by evaluating 169 RCT teeth of 100 patients who had undergone fixed orthodontic treatment. AP was assessed and classified using the CBCT periapical index. Univariate analysis of RCT outcome was performed for the total RCT group, RCT without AP group and RCT with AP group. Multivariate logistic regression was performed for the total RCT group and RCT without AP group, respectively, but not for the RCT with AP group. Variables related to the prognosis of RCT were included, such as age, gender, tooth position, RCT quality, coronal restoration quality, periodontal condition, orthodontic traction distance, and orthodontic rotation angle. Results: The orthodontic traction distance and rotation angle were not significantly correlated to the RCT outcomes, regardless of the presence of AP. Among the total RCT group, teeth with unqualified RCT (odds ratio = 3.42, P = .004) and inadequate coronal restoration (odds ratio = 4.40, P = .031) had a lower success rate. Of the 97 RCT teeth without AP, unqualified RCT was a risk factor for treatment failure (odds ratio = 3.55, P = .041). Of the 72 RCT teeth with AP, the univariate analysis showed that RCT quality were significantly related to the outcome (P P = .042). Conclusions: Orthodontic movement had no effect on the prognosis of RCT teeth regardless of the presence of AP. (J Endod 2024;50:1091-1099.)
Micro-computed tomography evaluation of the effects of orthodontic force on immature maxillary first molars and alveolar bone mineral density of Sprague-Dawley rats

Abstract:Objective: To investigate changes in the immature teeth of Sprague-Dawley rats during orthodontic treatment and to explore the changes in the peri-radicular alveolar bone through micro-computed tomography (CT). Methods: Twenty-five 26-day-old male Sprague-Dawley rats were included. The maxillary left first molar was moved mesially under a continuous force of 30 cN, and the right first molar served as the control. After orthodontic treatment for 7, 14, 21, 28, and 42 days, the root length, tooth volume, and alveolar bone mineral density (BMD) around the mesial root were measured through micro-CT. Results: The immature teeth continued to elongate after application of orthodontic force. The root length on the force side was significantly smaller than that on the control side, whereas the differences in the volume change between both sides were not statistically significant. Alveolar bone in the coronal part of the compression and tension sides showed no difference in BMD between the experimental and control groups. The BMD of the experimental group decreased from day 14 to day 42 in the apical part of the compression side and increased from day 7 to day 42 in the apical part of the tension side. The BMD of the experimental group decreased in the root apex part on day 7. Conclusions: The root length and volume of immature teeth showed continued development under orthodontic forces. Alveolar bone resorption was observed on the compression side, and bone formation was observed on the tension side.
[Evaluation of Root Resorption after Surgical Orthodontic Treatment of Skeletal Class Ⅲ Malocclusion by Three-Dimensional Volumetric Measurement with Cone-Beam CT].

Abstract:OBJECTIVE:To explore the method of measuring root volume with cone-beam computed tomography (CBCT) three-dimensional reconstruction technology, and to study root length and root volume of upper and lower central incisors in patients with skeletal Class Ⅲ malocclusion treated by surgical orthodontic treatment.METHODS:Twenty patients with skeletal Class Ⅲ malocclusion undergoing surgical orthodontic treatment were selected. CBCT data at three time points, before decompensation treatment (T0), after decompensation treatment (before orthognathic surgery, T1), and the end of post-operative orthodontic treatment (T2) were collected. Three-dimensional reconstruction technology was used to measure the root length and root volume of the upper and lower central incisors (including total root volume, cervical root and apical root), calculate the percentage of reduction volume, and measure the distance of tooth movement after orthodontic treatment. Data were statistically analyzed by SPSS 20.0 software. Least significant difference (LSD) method was used for pair comparison between the groups subject to normal distribution, and non-parametric test was used for comparison between the groups not subject to normal distribution. The differences of root length and root volume of upper and lower incisors were compared, and the characteristics of root absorption were analyzed.RESULTS:Root length and root volume of the upper and lower central incisors were reduced during the surgical orthodontic treatment (P < 0.05) in cases. Both the root volume of cervical root and apical root were significantly reduced (P < 0.05), the reduction of apical root was more significant. The percentage of root volume reduction of the upper central incisor was (30.51±23.23)%, and lower central incisor (23.24±11.96)%. Compared with the upper central incisor, the root volume reduction amount and percentage of the lower central incisor were smaller, and apical root volume reduction of the upper central incisor was greater than that of the lower central incisor, which was statistically significant (P < 0.05). During pre-surgical orthodontic treatment, maxillary central incisor palatal moving was in a controlled tipping manner, and the mandibular central incisor tipped labially.CONCLUSION:In patients with skeletal Class Ⅲ malocclusion, root length and total root volume of upper and lower central incisors decreased during surgical orthodontic treatment. Root volume measurement indicated that the cervical root also had root resorption. The difference in root resorption of the upper and lower central incisors might be related to the distance and direction of teeth movement. CBCT three-dimensional reconstruction will compensate for the limitation of root length measurement in evaluating root resorption.
[Influential Factors Related to the Long-Term Effect of Periodontal-Orthodontic Treatment in Patients with Aggressive Periodontitis and Malocclusion].

Abstract:Objective: To investigate the influential factors related to the long-term effect of periodontal-orthodontic treatment in patients with aggressive periodontitis (AgP). Methods: A retrospective analysis was conducted in 25 AgP patients, who have received periodontal-orthodontic treatment in Peking University School and Hospital of Stomatology. Changes in the ratio of the residual alveolar bone height (RBH) was measured at three time points: baseline (T0), post orthodontic treatment (T1), and the last re-visit 3 years after orthodontic treatment (T2). Root abnormity was evaluated by observing periapical radiographs, and its relationship with alveolar bone loss after orthodontic treatment was analyzed. A multi-level analysis on factors related to the clinical outcome (alveolar bone height change) was performed. Results: Totally 693 teeth of 25 patients at T0 and T1 and 368 teeth of 14 patients at T2 were investigated. During the periodontal-orthodontic treatment, the RBH was mainly influenced by root abnormity (estimation value -2.392), tooth position (estimation value for upper teeth vs. lower teeth 3.139, and anterior teeth vs. posterior teeth -3.469) and the baseline RBH at T0 (estimation value -0.391) (P<0.05). Teeth with root abnormity, teeth in mandibular and anterior area, and teeth with higher RBH showed less change in T1-T0 RBH values. In the long-term follow-up, RBH was mainly influenced by tooth position (estimation value for upper teeth vs. lower teeth 3.735, and anterior teeth vs. posterior teeth -5.318), the baseline RBH and probing depth (PD) at T0. Teeth in mandibular and anterior area, teeth with higher RBH (estimation value -0.498) and PD (estimation value -1.594) (P<0.05) showed less change in T0-T2 RBH values. Conclusions: During orthodontic treatment, teeth with abnormal root, lower teeth, anterior teeth, and teeth with high RBH were unfavorable factors for bone gain. In the long-term observation, lower teeth, anterior teeth, teeth with high RBH and PD at first visit were unfavorable factors for bone gain.
[Extraction-orthodontic treatment on patients with chronicperiodontitis: a preliminary study].

Abstract:OBJECTIVE:To retrospectively evaluate clinical and radiographic records of chronic periodontitis patients who underwent extraction-orthodontic treatment, in order to determine the effect of the treatment on probing depth, alveolar bone height of teeth adjacent to the extraction sites. METHODS:In the study, 33 chronic periodontitis patients who had finished extraction-orthodontic treatment were selected, the periodontal examination system tables and panoramic tomography were recorded before treatment (T0) and after treatment (T1), and the periodontal probing depth (PD), residual alveolar bone height (RBH) of the teeth adjacent to extraction sites (TAES) and the non-teeth adjacent to extraction sites (NTAES) were measured at T0 and T1. RESULTS:There was insignificant difference in PD of TAES and NTAES at T0 [(2.40±0.51) mm vs. (2.42±0.55) mm,P>0.05], neither was that at T1 [(2.70±0.67) mm vs. (2.67±0.64) mm, P>0.05]; From T0 to T1, PD of TAES and NTAES had mean increases of 0.3 mm [(2.40±0.51) mm vs. (2.70±0.67) mm,P<0.01] and 0.25 mm [(2.42±0.55 mm vs. (2.67±0.64) mm, P<0.01], respectively. And PD of TAES and NTAES increased from T0 to T1 statistically in the same degree [(0.30±0.64) mm vs. (0.25±0.58) mm,P>0.05]; at T0, RBH of TAES was 0.024 smaller than that of NTAES (0.74±0.16 vs. 0.76±0.16,P<0.05), but there was no difference in RBH between the TAES and NTAES at T1 (0.78±0.14 vs. 0.79±0.12,P>0.05); From T0 to T1, RBH of TAES and NTAES had mean increases of 0.04 (0.74±0.16 vs.0.78±0.14,P<0.05) and 0.02 (0.76±0.16 vs. 0.79±0.12,P<0.05), respectively. And the change of RBH between TAES and NTAES from T0 to T1 had no statistical difference (0.04±0.11 vs. 0.02±0.08,P>0.05)RBH of TAES in the side close to extraction sites was as the same as that of TAES in the side away from the extraction sites at T0 (0.73±0.17 vs. 0.74±0.16,P>0.05). From T0 to T1, RBH of both sides of TAES had mean increases of 0.04 (0.73±0.11 vs. 0.77±0.11,P<0.05) and 0.04 (0.74±0.11 vs. 0.78±0.11,P<0.05), respectively. But for both sides of TAES, from T0 to T1, there was no significant difference in change of RBH (0.04±0.11 vs. 0.04±0.11,P>0.05). CONCLUSIONS:With strict control of periodontal inflammation and maintenance of oral hygiene, orthodontic treatment preserves the periodontal conditions in patients with chronic periodontitis, and the extraction-orthodontic treatment can preserve the bone height of the teeth adjacent to extraction sites.
Effect of Orthodontic Treatment on Root Shape in Endodontically Treated Teeth

Abstract:Objective To evaluate the clinical characters of root shape after orthodontic treatment among endodontically treated teeth.Methods Forty-five orthodontic patients were selected who had a tooth treated endodontically before initiation of the orthodontic movement The panoramic radiographs before and after orthodontic treatment were examined.Measurements were made by comparing the root resorption score taken before and after the orthodontic treatment Results ① There was great statistically significant difference (P < 0.001) in apical root resorption after orthodontic treatment in the endodontically treated teeth compared to before initiation of orthodontic movement.② A positive association between apical root resorption and gender was observed in the patients with endodontically treated teeth and female was more dangerous when exposed to orthodontic loading.③ There was no statistically difference in apical root resorption between endodontically treated teeth and natrual teeth.④ No root resorption score up to third degree was observed in selected cases.Conclusions After orthodontic treatment root resorption was common both in endodontically treated teeth and natural teeth, There was no significant difference between them.

Abstract:OBJECTIVE:To investigate the root length of posterior teeth after palatal expansion by magnetic expansion appliance.METHODS:A total of 32 patients who accepted palatal expansion with magnetic expansion appliance were enrolled in the study. Pretreatment and post-treatment cone-beam computed tomography (CBCT) images were obtained. A paired t test was used to evaluate the length change, and a group t test was used to compare the tooth lengths of developing and developed teeth.RESULTS:Anchorage teeth showed greater root resorption than non-anchorage teeth, whereas the developed posterior teeth showed reduced resorption. Moreover, the mesial buccal roots of the first molar and first premolar were reduced. No resorption was found in the developing teeth group.CONCLUSION:The magnetic palatal expansion can reduce the root length, particularly of developed root. The developing root of anchorage teeth show greater tolerance to resorption than the developed root. Early palatal expansion treatment with magnetic expansion appliance can reduce the risk of root resorption.
Influence of the maxillary sinus floor on the orthodontic movement of maxillary first molars: a cone beam computed tomography study

Abstract:Objective:To comparatively investigate whether the proximity to the maxillary sinus floor affects the mesial translation and root resorption of the maxillary first molars during orthodontic extraction treatment using cone beam computed tomography.Methods:Forty-four adult patients who received premolar extraction treatment were selected and divided into two groups according to the relationship between the maxillary first molars and the maxillary sinus floor: group CP, in which all roots of the maxillary first molars are in contact with the maxillary sinus floor/projecting into the sinus; and group IF, in which all roots of the maxillary first molars isolate from the maxillary sinus floor. The mesial translation, mesiodistal angulation changes and root resorption were measured and compared between two groups before and after the treatment.Results:The mesial movement distances of the maxillary first molars in group IF were (2.56±1.00) mm for crowns and (2.44±1.30) mm for roots, while (2.73±1.20) mm for crowns and (2.80±0.99) mm for roots in group CP. There was no significant difference in the mesial translation ( P=0.121, P=0.202) and the changes of the mesiodistal angulation (group CP: 1.72°±4.58°, group IF: 0.82°±5.47°, P=0.425) between the two groups. However, group CP [(16.45±9.52) mm 3] showed significantly higher root resorption than group IF [(7.85±4.94) mm 3]( P<0.001). Conclusions:The maxillary first molars could move through the maxillary sinus floor successfully even when the roots were in contact with the maxillary sinus floor or projected into the sinus. The relationship between the maxillary first molars to the maxillary sinus floor did not influence the mesiodistal angulation changes. But the magnitude of root resorption obviously increased when roots were in contact with the maxillary sinus floor or projected into the sinus.
Clinical Evaluation of Periodontal-Orthodontic Treatment in Patients with Aggressive Periodontitis and Malocclusion

Abstract:OBJECTIVE:To evaluate the clinical effect and safety of periodontal-orthodontic treatment in patients with aggressive periodontitis (AgP) and malocclusion.METHODS:A retrospective analysis was conducted in 25 AgP patients, who had received periodontal-orthodontic treatment in Peking University School and Hospital of Stomatology. Clinical indexes, including probing depth (PD), bleeding index (BI) and percentage of sites with bleeding on probing (BOP%) were evaluated at three time points: Baseline (T0); active periodontal treatment finished and before orthodontic treatment (T1); and after orthodontic treatment (T2). Also changes of ratio of the residual alveolar bone height (RBH) and the occurrence of root resorption were evaluated by periapical radiographs.RESULTS:(1) Compared with T0, all the clinical parameters including PD, BI, BOP% and percentage of sites with PD>3 mm were significantly improved (P<0.001). (2) Significant difference was observed in the average RBH between T0 (68.37%±15.60% and T2 (70.27%±14.23%). RBH in upper incisors [(58.79%±16.71% at T0, 65.54% (55.74%, 78.13%) at T2], upper canines [77.62% (66.06%, 87.17%) at T0, 79.57% (69.75%, 86.52%) at T2] and upper molars [74.30% (61.69%, 84.45%) at T0, 76.76% (68.12%, 85.09%) at T2] showed significant increase (P<0.05). (3) After orthodontic treatment, varying degrees of root resorption occurred in (23.94%±13.45%) of teeth per capita, among which the lower and upper incisors showed the highest incidence (68.48% and 65.31% in homogeneous teeth, respectively).CONCLUSION:After active periodontal treatment, orthodontic treatment in AgP patients had not aggravated inflammation and alveolar bone resorption; root resorption occurred in two-thirds of incisors approximately.
The relationship between apical root resorption and orthodontic tooth movement in growing subjects

Abstract:To investigate the relationship between apical root resorption and orthodontic tooth movement in growing subjects.58 growing subjects were collected randomly into the study sample and another 40 non-treated cases were used as control. The apical resoption of the upper central incisors was measured on periapical film and the incisor displacement was measured on lateral cephalogram. Using multiple linear regression analysis to examine the relationship between root resoption and the displacement of the upper incisor apex in each of four direction (retraction, advancement, intrusion and extrusion).The statistically significant negative association were found between resorption and both intrusion (P < 0.001) and extrusion (P < 0.05), but no significant association was found between resorption and both retraction and advancement. The regression analysis implied an average of 2.29 mm resorption in the absence of apical displacement.The likelihood that the magnitude of displacement of the incisor root is positively associated with root resoption in the population of treated growing subjects is very small.

Abstract:INTRODUCTION Reduced periodontal support is a challenge that clinicians often face during rehabilitation of compromised dentition. The close and intricate relationship between the periodontal tissues and the processes of tooth movement suggest that adjunct orthodontic therapy may play an important role in overcoming these problems. On the other hand, excessive movement of teeth beyond the anatomic boundaries of the alveolar process is commonly believed to contribute to further destruction of the periodontal tissues PURPOSE: To highlight the importance the periodontium health and root of the tooth during the orthodontic treatment METHOD: A literature review for the last five years was performed, with keywords such as: orthodontic appliances, healthy periodontium, recessions, systemic reviews. RESULTS: Root resorption can be observed during the orthodontic treatment which can follow up to tooth mobility as well as losing the tooth due to the treatment CONCLUSIONS: The correction of some orthodontic problems, such as excessively tipped molars, traumatic deep-bites and flared and spaced incisors, maybe particularly beneficial in periodontally compromised patients who are motivated to undergo treatment and demonstrate stable periodontal conditions. However, several factors may contribute to a harmful periodontal response including the use of heavy forces, inappropriate force systems and poor oral hygiene. Excessive and unrealistic tooth movements are also likely to result in reduced alveolar bone thickness, especially in patients with thin cortical plates. On the other hand, the response of the soft tissues is less predictable and likely to be influenced by multiple factors.
[An experimental study of the effects of root-canal treatment on orthodontic movement in cat cuspids]

Abstract:Purpose: To evaluate the effectiveness of orthodontic forces in moving teeth after root canal therapy and the risk of apical resorption which may happen during orthodontic movement in an animal model. Methods: Under general anesthesia, root canal therapy was performed on the mandibular canines on one side of 16 adult cats. Tipping movement of the canines was induced with an orthodontic spring (100 to 120 g). Eight weeks later, tooth movement was assessed from pre- and posttreatment mandibular casts, including measurement of root lengths in both radiographs and histological sections. All the data were analysed with SPSS 11.0 for windows. Student's t test was used for comparisons between paired groups. Results: The results showed that teeth after root canal therapy and vital teeth moved similar distances when subjected to the same forces (P>0.05). And teeth after root canal therapy had greater loss of cementum after tooth movement than vital teeth (P<0.05), but without significant differences in radiographic root length (P>0.05). Conclusions: Teeth after root canal therapy can be moved orthodontically as readily as vital teeth when subjected to the same orthodontic forces under the conditions of the study. However, teeth after root canal therapy have more root resorption than vital teeth during orthodontic movement which may not be detected form the radiography.
[Alveolar Bone Thickness and Root Length Changes in the Treatment of Skeletal Class III Patients Facilitated by Improved Corticotomy: a Cone-Beam CT Analysis].

Abstract:OBJECTIVE:To evaluate the alveolar bone thickness and root length changes of anterior teeth with cone-beam computed tomography (CBCT).METHODS:CBCT scans were taken for 12 skeletal Class III patients who accepted the improved corticotomy (IC) procedures during pre-surgical orthodontics. The CBCT data in T1 (the maxillary dental arch was aligned and leveled) and T2 (extraction space closure) were superimposed and the alveolar bone thickness at root apex level and root length measurements were done.RESULTS:From T1 to T2, the buccal alveolar bone thickness for the upper lateral incisors increased from (1.89±0.83) to (2.47±1.02) mm (P<0.05), and for central incisors and for canines from (2.32±0.71) to (2.68±1.48) mm and from (2.28±1.08) to (2.41±1.40) mm, respectively. According to Sharpe Grading System, the root resorption grade for 69 teeth of 72 was located in Grade 1, two teeth in Grade 2, one tooth in Grade 3.CONCLUSIONS:The improved corticotomy had the potential to increase the buccal alveolar bone thickness and the root resorption in most teeth was in Grade 1 according to Sharpe grading system.
