Abstract:Background: Injury to the inferior alveolar nerve can occur during surgical removal of the mandibular third molars (M3Ms), resulting in numbness of the mandibular teeth, chin, and lower lip. This occurs when the roots of M3M compress the inferior alveolar canal (IAC) against the lingual cortical plate. Cone-beam computed tomography (CBCT) reveals the buccolingual relationship of the IAC and M3M, but the panoramic radiograph (PR) has not been evaluated to determine whether the plane film can reveal this relationship. Aims: The aim of the study is to determine whether PR could predict buccal or lingual IAC position or compression near M3M. Design: This retrospective study evaluated 200 M3M sites in 42 women and 67 men, aged 17–28 years. Ninety-one bilateral measurements were taken; 98 left and 102 right halves were analyzed. Methods: IAC position and degree of compression were interpreted from the CBCT. Utilizing imaging software, 13 different measurements were obtained. Statistical Analyses: Fisher's exact test, t-test, principal component analysis, and multivariate analysis were utilized. Results: Male and female canal positions significantly differed. Canal position was significantly different comparing partially erupted to erupted and fully bony impacted molars. Lingual canal position, versus buccal, was more frequently associated with moderate-to-severe IAC compression. “Anterior-posterior ramus at the occlusal plane” and “anterior ramus to anterior IAC at the occlusal plane” contributed most to the severity of IAC compression. Conclusion: The variables evaluated did not identify a significant relationship. A larger data set is needed to evaluate any role the dimension of the ramus has on IAC compression.
Radiographic predictors of post-operative inferior alveolar nerve injury in mandibular third molar surgery

Abstract:Background Cone-beam computed tomography (CBCT) provides additional three-dimensional information on the relationship between the mandibular third molar (M3M) and the inferior alveolar nerve (IAN). As such, CBCT is being increasingly utilized in pre-operative M3M assessment. Purpose The purpose of the study was to compare the radiographic findings on panoramic and CBCT and their association with post-operative IAN paresthesia. Study design, setting and sample We conducted a retrospective cohort study in a sample of patients referred to the Department of Oral and Maxillofacial Surgery at the Royal Dental Hospital of Melbourne, for management of impacted M3Ms. Patients were included in this study if they had one or more high risk findings on OPG, had both OPG and CBCT imaging taken and if at least one M3M had been extracted. Subjects were excluded from this study if their M3Ms were not extracted or if a CBCT was not indicated. Independent variables The independent variable was radiographic features identified on OPG (Rood and Shehab's radiographic signs, root morphology, impaction type, Pell and Gregory classification) and CBCT (number and location of roots, severity of IAN compression and the presence of ankylosis). Outcome variable The outcome variable was post-operative neurosensory function defined as any reported paresthesia at 2-week post-procedural review. Covariates The covariates were patient demographic information (age, sex). Analyses Variables were initially assessed with univariate logistic regression analysis to determine factors related to developing post-operative paresthesia. Multivariate logistic regression analysis was then used to assess the association between positive univariate study variables and post-operative paresthesia, while adjusting for potential confounders. Covariates were assessed using an omnibus likelihood ratio test and included if they were statistically significant at the p<0.05 level. Results The study sample consisted of 257 subjects who underwent surgical removal of n=386 M3Ms. The mean age was 25.9 (SD = 7.05). The panoramic features of narrowing of canal, diversion of canal and dark/bifid roots were identified as statistically significant associations of post-operative paresthesia. No CBCT features were significantly associated of post-operative paresthesia. Conclusions and Relevance Certain panoramic features, along with patient age, are significant predictors of IAN paresthesia. CBCT findings were not significant predictors upon multivariate analysis, thus additional CBCT imaging did not significantly improve ability to predict paresthesia.

Evaluation Of The Relationship Between The Impacted Mandibular Third Molar And The Inferior Alveolar Canal On Panoramic Radiograph

Abstract:Objective: To evaluate the diagnostic accuracy of the panoramic radiograph in judging the relationship between the impacted mandibular third molar (IMTM) and the inferior alveolar canal (IAC) by cone-beam computed tomography (CBCT) and give guidelines for using radiological examinations before IMTM extraction.Methods: A total of 1735 IMTMs on panoramic radiographs were analysed and classified into three classes according to the relationship between the IMTMs and the IAC. A total of 126 IMTMs superimposing on the canal partially or touching the canal in line on panoramic radiographs were examined by CBCT. Nine radiographic signs were observed by two radiologists respectively and multivariate logistic regression analysis was performed.Results: 8.7% of IMTMs have superimposition or a linear touching relationship with the IAC on panoramic radiographs. Of these IMTMs, 52.4% impinged and broke the IAC in CBCT images. Two radiologic signs on panoramic radiographs, including interruption of the radiopaque border of the canal (P = 0.009), and interruption of the alveolar lamina dura and periodontal space (P = 0.007), were statistically significant in predicting the impingement of the IAC.Conclusion: Panoramic radiography can be used to screen out high-risk cases of inferior alveolar nerve injury before the IMTM extraction. Those IMTMs superimposing the canal partially or touching the canal in line on panoramic radiographs should be examined by CBCT further. Two features on panoramic radiographs, including interruption of the alveolar lamina dura and periodontal space, and interruption of the radiopaque border of the canal, were more valuable than other signs to predict impingement and interruption of the IAC wall.
Comparison of panoramic radiograph and cone beam computed tomography findings for impacted mandibular third molar root and inferior alveolar nerve canal relation

Abstract:Objectives: This study was aimed at determining the specific signs of close relationship between impacted mandibular third molar (IMTM) root and inferior alveolar nerve (IAN) canal as visualised on digital panoramic radiograph or orthopantomograph (OPG) which should indicate as well as contraindicate the need for cone beam computed tomography (CBCT) imaging based on hierarchical model of Fryback and Thornbury. Materials and methods: The sample consisted of 120 individuals (200 IMTM) who had undergone OPG as well as CBCT imaging as preoperative radiographic evaluation before surgical extraction. On panoramic radiographs; Rood and Sheehab's radiographic signs of IMTM root and IAN canal, impacted third molar position based on Winter's classification and depth of impaction based on Pell and Gregory's classification were evaluated. On CBCT; presence/absence of corticalization and the status of the buccal and lingual cortices (thinning/perforation) were evaluated. These findings of OPG were compared to CBCT findings to determine the better modality based on hierarchical model of Fryback and Thornbury. Results: Statistically significant association was found between radiographic signs of Rood and Sheehab such as 'no relation' and 'superimposition' on OPG and presence of corticalization between IMTM root and IAN canal on CBCT. Statistically significant association was also found between 'mesioangular' and 'vertical' positions of Winter's classification as well as 'class 1A' of Pell and Gregory's classification of IMTM on OPG and presence of corticalization on CBCT. CBCT was found to be having lesser variance and greater F value as compared to OPG for evaluation of IMTM. Conclusions: On OPG; Rood and Sheehab's radiographic signs such as 'no relation' and 'superimposition', Winter's 'mesioangular' and 'vertical' and Pell and Gregory's 'Class 1A' are not indicative for CBCT examination before surgical extraction is attempted. On OPG; Rood and Sheehab's radiographic signs such as interruption of white line, darkening of root, darkening of canal, deflection of root, narrowing of canal as well as diversion of canal are all frequently associated with absence of corticalization between IMTM root and IAN canal and hence require CBCT examination before surgical extraction is attempted so that post-operative neurological complications can be minimised. Also, Winter's horizontal, distoangular and others as well Pell and Gregory's classes 1B,1C,2A,2B,2C,3A,3B,3C are indicative for CBCT examination before surgical extraction is attempted. Fryback and Thornbury model-based comparison proves that CBCT is a better radiographic modality as compared to OPG for evaluation of IMTM relation with IAN canal.
Is Cone Beam Computed Tomography Accurate in Predicting Inferior Alveolar Nerve Exposure During Mandibular Third Molar Extraction?

Abstract:Objectives This study aims to evaluate the accuracy of cone beam computed tomography (CBCT) in predicting the exposure of inferior alveolar nerve (IAN) during complicated mandibular third molars (M3M) extraction. Methods 115 M3Ms with canal cortical defect signs on preoperative CBCT were extracted. Candidate variables included sex, age, types of CBCT machine, the Winter classification of M3Ms, the direct contact size between IAN and tooth root on CBCT, the size of cortical defect on CBCT. The primary outcome was the exposure of IAN and the exposed neurovascular bundle size which was recorded measured under endoscope. Bland-Altman analysis was performed to assess the agreement between the CBCT and endoscopic measurements. Results 85/115 (73.9%) M3Ms with canal cortical defect signs on preoperative CBCT had intraoperative exposure of IAN. The average length and width of the exposed IAN were 5.89 ± 1.72mm and 2.48 ± 0.79 mm, which were significantly smaller than the direct contact size between IAN and tooth root on CBCT (9.69 ± 3.05mm and 3.26 ± 0.87 mm, P < 0.001) but larger than the cortical defect size (5.06 ± 2.05mm and 2.10 ± 0.54 mm, P < 0.05). The probability of intraoperative IAN exposure was statistically different among different Winter classifications of M3M and the probability of IAN exposure was higher in non-horizontal impacted type. Conclusions Not all M3Ms with tooth-IAN contact signs on preoperative CBCT indicated intraoperative IAN exposure. The contact sizes on CBCT were always larger than the intraoperative endoscopic measurements. IAN exposure can be accurately predicted by the length of cortical defect on CBCT. Non-horizontal impaction predisposed the M3M to a higher risk of intraoperative IAN exposure. Clinical Relevance: Endoscope provides the possibility to observe and record the IAN exposure directly. IAN exposure can be accurately predicted by the length of cortical defect on CBCT instead of the direct contact size between IAN and tooth root. Non-horizontal impaction predisposed the M3M to a higher risk of intraoperative IAN exposure.

Abstract:INTRODUCTION: The present study was aimed to identify the radiographic signs between impacted third molar root and inferior alveolar canal (IAC) on cone-beam CT (CBCT) images as risk factors and prognostic predictors associated with inferior alveolar nerve (IAN) damage following tooth removal.MATERIAL AND METHODS: A retrospective clinical study was performed involving 136 patients with 257 impacted lower third molars from January 2013 to December 2014. The neurosensory function of the lower lip and chin was subjectively evaluated and assessed by neurosensory tests before and after surgery. The preoperative CBCT data were retrieved and analyzed to identify the radiographic signs associated with postoperative IAN injury.RESULTS: The overall incidence of IAN injury in our patient cohort was 13.2%. Multiple radiographic features on coronal CBCT images including contact between IAC and root, IAC position relative to root, IAC shape and cortication status were found to be significantly associated with IAN damage (p < 0.05, χ2 test). Furthermore, buccolingual position, teardrop/dumbbell shape and cortication status of IAC were identified as independent prognostic predictors for IAN damage.CONCLUSIONS: Our findings indicate that radiographic signs including direct contact between IAC and root, buccal/lingual IAC position relative to root, teardrop/dumbbell shape and cortication absence of the IAC on presurgical CBCT images are associated with high risk of IAN injury and postoperative neurosensory disturbance.

Abstract:In most underdeveloped countries, the initial contact with Epstein Barr virus (EBV) usually happens in the first decade of life and results in an asymptomatic infection, whereas in developed areas, primary infection in adolescence or adulthood is accompanied by infectious mononucleosis in 50% cases. Although it is generally a harmless passenger, in some individuals, it is associated with B‐cell lymphoma. In Argentina, EBV primary infection shows the classical pattern observed in developing populations, given that nearly 70% of patients are seropositive by the age of 2 years. However, EBV association with pediatric Hodgkin and Burkitt lymphoma resembles that observed in developed regions. Concerning diffuse large B‐cell lymphoma, our series demonstrated higher EBV association than other adult ones from either developed or underdeveloped countries. Interestingly, the early EBV primary infection observed, characteristic of an underdeveloped population, together with the statistically significant EBV association with patients ≤10 years old demonstrated in all types of lymphoma studied, suggest a relationship between low age of EBV seroconversion and B‐cell lymphoma development risk.
Evaluating the Risk of Post-Extraction Inferior Alveolar Nerve Injury Through the Relative Position of the Lower Third Molar Root and Inferior Alveolar Canal.

Abstract:The aim of this study was to introduce a method to evaluate the risk of inferior alveolar nerve (IAN) injury following the extraction of impacted lower third molars. Two hundred impacted lower third molars adjacent to the IAN were evaluated. These were divided into four classification groups according to preoperative cone beam computed tomography (CBCT) findings: AR, apical region; LT, lateral region of the tapered root; LE, lateral region of the enlarged root; AE, adjacent to the enlarged root. All teeth were dislocated along the long axis or arc of the root by tooth sectioning technique and extracted by a single surgeon. The primary outcome variable was postoperative neurosensory impairment of the IAN. The χ2 test was used to evaluate differences in postoperative IAN injury between the classifications. Logistic regression analysis was used to evaluate the risk factors for postoperative IAN injury. The overall incidence of postoperative IAN injury was 7%. Specifically, most injuries involved classification AE (AE 36%, LE 8.6%, LT 3.6%, AR 0%), and the difference was statistically significant (P< 0.05). Logistic regression showed that classification AE was the only risk factor for postoperative IAN injury (P< 0.001). According to preoperative CBCT, the risk of postoperative IAN injury is higher when the IAN is adjacent to the enlarged part of the root.
Retrospective evaluation of sensory neuropathies after extraction of mandibular third molars with confirmed "high-risk" features on cone beam computed topography scans

Abstract:Objectives: The objective of this study was to identify the incidence of inferior alveolar nerve (IAN) injury after extraction of mandibular third molars with confirmed "high-risk" features on cone beam computed topography scans and establish any risk factors that significantly increase the chances of sustaining an IAN injury. Study design: This study is a retrospective analysis of outcomes after surgical extraction of 500 mandibular third molars over a 5-year period. All teeth showed signs of contact with or compression of the IAN on cone beam computed topography scans. Results: The overall incidence of IAN injury was 6.6%, with permanent neuropathies accounting for 1.8% within this high-risk cohort. Statistically significant factors shown to increase the risk of nerve injury included increasing age (P = .002), compression of the nerve evident on cone beam computed topography scan (P = .005), and buccal or interradicular position of the nerve (P = .042). Conclusions: Results indicate a low incidence of IAN injury, particularly given the high-risk nature of all teeth that were extracted. It highlights the benefits of cone beam computed topography scans in surgical planning. These data are an important contribution to the existing literature and valuable in the consent procedure for patients undergoing surgical removal of mandibular third molars in contact with the IAN.

Diagnostic accuracy of CBCT compared to panoramic radiography in predicting IAN exposure: a systematic review and meta-analysis

Abstract:Objectives: The aim of this study is to verify whether the diagnostic accuracy of cone beam computed tomography (CBCT) is superior to panoramic radiography (PR) in predicting inferior alveolar nerve (IAN) exposure during the lower third molar extraction. Materials and methods: Eight electronic databases were searched up to September 2020. Studies that evaluated the accuracy (sensitivity, specificity, positive-predictive value, and negative predictive value) of both imaging methods were included. The gold standard was the visualization of the IAN exposure during the extraction of lower third molars. The gray literature was also used to include any other paper that might meet the eligibility criteria. The meta-analysis was performed with OpenMeta-Analyst and ReviewManager v.5.3 software. The methodology of the studies was evaluated using QUADAS-2. Results: Among the search, three studies met all the eligibility criteria and were included in the qualitative and quantitative synthesis. The meta-analysis was conducted with all included studies. Accuracy values for CBCT were 95.1% for sensitivity (p=0.666) and 64.4% for specificity (p<0.001). For PR sensitivity and specificity, we observed 73.9% (p=0.101) and 24.8% (p<0.001), respectively. Conclusions: Both exams were reliable for detecting positive cases of exposure of the IAN. However, CBCT had a better performance compared to PT in predicting IAN exposure during surgery. Clinical relevance: To better understand the CBCT accuracy in predicting the IAN exposure during surgery, since this event can increase the likelihood of IAN injury and, consequently, cause neurosensory disturbances.
Comparison of Digital OPG and CBCT in Assessment of Risk Factors Associated with Inferior Nerve Injury during Mandibular Third Molar Surgery

Abstract:Background: Pre-operative radiographic assessment of the anatomical relationship between the roots of the mandibular third molar and the inferior alveolar nerve (IAN) is a must to minimize the risk of IAN injury during surgery. Objectives: To compare the radiographic signs of digital orthopantomogram (OPG) and cone-beam computed tomography (CBCT). An additional objective was to assess the cortex status between the mandibular canal and third molar on CBCT images in relation to the demographic characteristics, region (right or left side), and angulation of mandibular molar. Methodology: In this retrospective study, a total of 350 impacted mandibular third molars with a close relationship between the inferior alveolar canal (IAC) and impacted mandibular third molars on digital OPG were further referred for CBCT imaging for assessment of the position of the mandibular canal. The study was conducted between August 2018 and February 2020. Digital OPGs were evaluated for radiographic signs like interruption of the mandibular canal wall, darkening of the roots, diversion of the mandibular canal, and narrowing of the mandibular canal. The age and sex of patients, site of impacted third molar, Winter’s classification of mandibular third molar, position of IAC relative to impacted molar, and the radiographic markers of OPG were assessed for cortical integrity using CBCT. Chi square testing was applied to study the values of difference and binomial logistic regression was done to assess the factors associated with cortication. Statistical significance was set at p ≤ 0.05. Results: Among 350 patients, 207 (59.1%) were male and 143 (40.9%) were female with a mean age of 36.8 years. The most common OPG sign was interruption of white line, seen in 179 (51.1%) cases. In total, 246 cases (70.3%) showed an absence of canal cortication between the mandibular canal and the impacted third molar on CBCT images. Cortication was observed in all cases with a combination of panoramic signs which was statistically significant (p = 0.047). Cortication was observed in 85 (50.6%) cases where IAC was positioned on the buccal side, 11 (16.9%) in cases of inferiorly positioned IAC, and just 8 (7.6%) for cases of lingually positioned IAC which was statistically significant (p = 0.003). Statistically insignificant (p > 0.05) results were noted for cortex status in CBCT images with regards to the age, sex, site, and angulation of impacted third molars. Conclusion: CBCT imaging is highly recommended for those cases where diversion of the mandibular canal is observed on OPG and when the roots are present between canals.

Risk assessment of inferior alveolar nerve injury after wisdom tooth removal using 3D AI-driven models: A within-patient study

Abstract:Objective: To compare a three-dimensional (3D) artificial intelligence (AI)- driven model with panoramic radiography (PANO) and cone-beam computed tomography (CBCT) in assessing the risk of inferior alveolar nerve (IAN) injury after mandibular wisdom tooth (M3M) removal through a within-patient controlled trial. Methods: From a database of 6,010 patients undergoing M3M surgery, 25 patients met the inclusion criteria of bilateral M3M removal with postoperative unilateral IAN injury. In this within-patient controlled trial, preoperative PANO and CBCT images were available, while 3D-AI models of the mandibular canal and teeth were generated from the CBCT images using the Virtual Patient Creator AI platform (Relu BV, Leuven, Belgium). Five examiners, who were blinded to surgical outcomes, assessed the imaging modalities and assigned scores indicating the risk level of IAN injury (high, medium, or low risk). Sensitivity, specificity, and area under receiver operating curve (AUC) for IAN risk assessment were calculated for each imaging modality. Results: For IAN injury risk assessment after M3M removal, sensitivity was 0.87 for 3D-AI, 0.89 for CBCT versus 0.73 for PANO. Furthermore, the AUC and specificity values were 0.63 and 0.39 for 3D-AI, 0.58 and 0.28 for CBCT, and 0.57 and 0.41 for PANO, respectively. There was no statistically significant difference (p>0.05) among the imaging modalities for any diagnostic parameters. Conclusion: This within-patient controlled trial study revealed that risk assessment for IAN injury after M3M removal was rather similar for 3D-AI, PANO, and CBCT, with a sensitivity for injury prediction reaching up to 0.87 for 3D-AI and 0.89 for CBCT. Clinical significance: This within-patient trial is pioneering in exploring the application of 3D AI-driven models for assessing IAN injury risk after M3M removal. The present results indicate that AI-powered 3D models based on CBCT might facilitate IAN risk assessment of M3M removal.

Abstract:Objective To compare the predictive effects of the sensory disturbances of inferior alveolar nerve ( IAN) after re-moval of impacted mandibular third molars and IAN exposure during surgery between cone beam computed tomography ( CBCT ) and panoramic radiography ( PAN) .Methods Eighty-six patients with impacted third molars in close relation to the IAN as determined by PAN and judged as a “moderate” risk of IAN damage were divided into CBCT and PAN groups .The sensory disturbances of the IAN af-ter teeth removal and the IAN exposure at surgery were evaluated with CBCT and PAN , respectively .Results Postoperative sensory disturbances occurred in one patient in the CBCT group and one patient in the PAN group .There were no significant differences between the two groups .However ,correct diagnostic rates of neurovascular bundle exposure at the extraction of impacted teeth were 56% and 35%by CBCT and PAN,respectively.The differences were statistically significant (P =0.029).Conclusion CBCT was not superior to PAN in predicting postoperative sensory disturbances but was superior in predicting IAN exposure during third molar removal in cases judged as having “moderate” risk.

Abstract:Backround: This study was designed to analyse the value of preoperative Cone Beam CTs (CBCT) prior to the surgical removal of complex lower third molars. Furthermore, the aim was to assess injuries to the inferior alveolar nerve (IAN) bundle and postoperative neurological disorders depending on the position of the lower third molar and the inferior alveolar nerve bundle. Methods: In this retrospective examination preoperative Cone Beam CTs and Orthopantomographs (OPT) of 324 patients were analysed concerning the location of the lower third molars in relation to the mandible and the inferior alveolar nerve bundle. Surgery protocols of all patients who underwent the surgical removal of at least one complex lower third molar were analysed concerning patient data, length of surgery, intraoperative haemorrhage, intraoperative exposure of the inferior alveolar nerve bundle, postoperative swelling and postoperative neurological disorders. The data was then compared to data from international studies. Results: In all 324 patients a permanent neurological damage was not found. Temporary neurological damage was recorded in 13 cases (2.6%). A caudal nerve position with no measurable distance to the root of the lower third molar was associated with the highest risk of a temporal neurological damage. A vestibular touching nerve route also correlated with postoperative sensitivity impairment. If a mesioangulation (Winter) or a Pell and Gregory Type IIIC appears in the OPT, risk of neurological damage is at its highest. Conclusions: Three-dimensional radiographic imaging, in our patient group, does not significantly affect the risk for complications during the surgical removal of complex lower third molars. Therefore, it should only be utilized for risk assessment, especially in cases of symptom-free lower third molars. A preoperative orthopantomogram still can be accepted as standard for radiographic imaging. An intraoperative exposure of the IAN bundle does not necessarily predict simultaneous neurological damage. Exposure of the IAN bundle is no indication for a discontinuation of the surgery.

Abstract:ObjectivesThe present study aimed to assess whether anatomical variations of the mandibular canal are associated with neurosensory disturbances of the inferior alveolar nerve (IAN) following mandibular third molar removal.MethodsTwo observers compared the detection of third molar root-nerve relations and bifurcations of the mandibular canal on panoramic radiographs and CBCT images of 201 patients undergoing removal of 357 mandibular third molars. Potential neurosensory disturbances of the IAN were surveyed ten days after surgery. Fisher's Exact was performed to correlate presence of canal variations to postoperative neurosensory disturbances. Positive and negative predictive values (PPV, NPV) and likelihood ratios (LR + , LR–) were calculated.ResultsThirteen patients reported postoperative altered sensation of the lower lip, with 2 of them having mandibular canal bifurcations on the ipsilateral side of the injury. Fisher's Exact showed that the studied mandibular canal variations were not related to postoperative neurosensory disturbances. CBCT was superior in visualization of anatomical variations of the mandibular canal. Prevalence of bifurcations was 14% on CBCT and 7% on panoramic radiographs. In both imaging modalities and for all parameters, PPVs were low (0.04 − 0.06) and NPVs were high (0.92 − 0.98), with LR ranging around 1.ConclusionIn the present study, the assessed mandibular canal variations had limited predictive value for IAN neurosensory disturbances following third molar removal.Clinical relevanceWhile a close relation between the third molar and the mandibular canal remains a high risk factor, mandibular canal variations did not pose an increased risk of postoperative IAN injury after third molar removal.

Abstract:Risks of lower third molar surgery like the inferior alveolar nerve injury may result in permanent consequences. Risk assessment is important prior to the surgery and forms part of the informed consent process. Traditionally, plain radiographs like orthopantomogram have been used routinely for this purpose. Cone beam computed tomography (CBCT) has offered more information from the 3D images in the lower third molar surgery assessment. The proximity of the tooth root to the inferior alveolar canal, which harbours the inferior alveolar nerve, can be clearly identified on CBCT. It also allows the assessment of potential root resorption of the adjacent second molar as well as the bone loss at its distal aspect as a consequence of the third molar. This review summarized the application of CBCT in the risk assessment of lower third molar surgery and discussed how it could aid in the decision-making of high-risk cases to improve safety and treatment outcomes.

Abstract:Hypertrophic chondrocytes, which are the terminally differentiated form of chondrocytes, play a key role in endochondral ossification. In order to investigate the functions of hypertrophic chondrocytes during bone development, we generated a new transgenic line expressing Cre recombinase under the control of a 8.2 kb mouse type X collagen gene promoter (Col10a1(8.2)-Cre). Microinjection was employed to introduce the 11.5 kb transgenic fragment into 328 oocytes, from which 51 progenies were obtained. Three mice carrying the transgene in genome were identified by PCR genotyping. PCR detected expression of Col10a1(8.2)-Cre transgene within tissues containing hypertrophic chondrocytes. To examine the activity and specificity of Cre recombinase in vivo, transgenic line was crossed with ROSA26 report line. As indicated by LacZ staining, ROSA26; Col10a1(8.2)-Cre double transgenic mice showed efficient expression of Cre recombinase within hypertrophic chondrocytes. In situ hybridization analyses further confirmed the transcription of Col10a1(8.2)-Cre transgene within the upper zone of hypertrophy, indicating a better activity and specificity in contrast to the previously constructed Col10a1(1.0)-Cre transgenic line. These results showed that this Col10a1(8.2)-Cre transgenic line could be used as a powerful tool to achieve conditional gene knockout in hypertrophic chondrocytes.
Endoscopic Visualization of the Inferior Alveolar Nerve Associated with Somatosensory Changes after Impacted Mandibular Third Molar Extraction.

Abstract:The aim of this study is to assess the relationship between somatosensory functional changes and inferior alveolar nerve (IAN) exposure after impacted mandibular third molars (M3M) removal. We recruited 35 patients who underwent impacted M3M extraction near the IAN. The M3Ms were extracted by combined endoscopy, piezosurgery, and contra-angle high-speed turbine handpiece. All IAN canal perforations and exposed regions were recorded and measured by endoscopy after extraction and on cone-beam computed tomography (CBCT) images before extraction. The patients were followed up 1, 7, and 35 days after surgery. A standardized quantitative sensory testing (QST) battery was performed on the lower lip skin. All of 35 cases had exposed IAN on CBCT images, 5 of which had no exposed IAN under endoscopy. For the other 30 cases, the endoscopy-measured IAN length and width were shorter than the CBCT measurements (P < 0.001). The warm and mechanical detection thresholds (MDT) on the operation side were significantly higher than the contralateral side after surgery (P < 0.05). Thermal sensory limen, MDT, and cold pain threshold were strongly correlated with the exposed IAN length and MDT also with the exposed IAN width one day after surgery. In conclusion, it was found that not all exposed IAN in CBCT images were real exposure after surgery. The intraoperative exposed IAN endoscopic measurements were smaller than by CBCT and strongly correlated with some QST parameters.
