Abstract:Aphallia (Penile agenesis) is a very rare genitourinary anomaly that has profound surgical and psychological implications. Aphallia cases have 46, XY karyotype, except one reported case of penile agenesis having XX/XY mosaic karyotype. Early assignment of female gender and feminizing reconstruction of the perineum is the goal of treatment in cases of penile agenesis. This disorder results from partial or complete failure of development of genital tubercle. It usually coexists with other serious anomalies, which are incompatible with normal life. We present here with a 1 day old neonate who presented with aphallia and urethro-rectal fistula along with horse shoe kidney.

Abstract:Growth of S49 wild-type (WT) lymphoma cells for 24 hr with 3 nM epinephrine produces a very pronounced attenuation of cAMP accumulation in response to subsequent challenges with much higher concentrations of the catecholamine [Mol. Pharmacol. 36:459-464 (1989)]. We report here the effects of this treatment, in S49 WT, cyc-, and kin- cells, on the responsiveness of adenylate cyclase in partially purified membranes. The desensitization of adenylate cyclase in the S49 WT cells after 24-hr treatment was homologous, in that only responses to epinephrine were attenuated. The EC50 for epinephrine stimulation of adenylate cyclase was 54 +/- 8% (mean +/- standard error) higher in treated cells than in controls, and there was a 32 +/- 3% reduction in Vmax at supramaximal epinephrine concentrations. The treatment also caused a 34 +/- 9% reduction in the levels of the beta-adrenergic receptor (beta AR), which was of a sufficient magnitude to account for the homologous desensitization seen. The 24-hr treatment had similar effects in S49 kin- cells, where we observed a 28 +/- 4% decrease in Vmax, a 35 +/- 6% increase in EC50 for epinephrine stimulation of adenylate cyclase, and a 25 +/- 3% decrease in beta AR. In contrast, the 24-hr treatment had no measurable effect on adenylate cyclase activity in S49 cyc- cells. That is, the responsivity of adenylate cyclase reconstituted with Gs from S49 WT cells was not attenuated, although beta AR levels were significantly decreased. The desensitization of S49 cells with the 24-hr treatment was additive with that mediated by the cAMP-dependent protein kinase (cAPK). Further, unlike the cAPK-mediated attenuation, it was relatively insensitive to the levels of free Mg2+ in the adenylate cyclase reaction mixture. The characteristics of the desensitization produced by 24-hr treatment with 3 nM epinephrine, together with the observation that it is similar in S49 WT and kin- cells, demonstrates that the process in WT cells is, at least in part, independent of the rapid cAPK-mediated desensitization. It is also most likely that it is unrelated to the rapid cAMP-independent processes involving sequestration/internalization or the beta AR kinase, because those mechanisms require much higher receptor occupancies than the 0.2% occurring with 3 nM epinephrine. Thus, 24-hr treatment appears to produce attenuation of adenylate cyclase by causing down-regulation of beta AR, without involving any other known form of desensitization.

Abstract:This case report presents a rare occurrence of isolated bladder exstrophy with a normal phallus in a one-day-old male newborn. The patient also presented with imperforated anus, congenital heart disease, and a left ectopic kidney. Surgical interventions included cut-back anoplasty, bilateral ureteral reimplantation, and primary bladder closure in a single operation, resulting in a successful outcome without complications. Three months of follow-up showed satisfactory results.

Abstract:Urethral atresia is a rare but clinically significant cause of congenital lower urinary tract obstruction . Initial management options include urinary diversion until definitive urethral reconstruction or progressive urethral dilation. Given the overall rarity of the condition, there are no evidence-based guidelines for the immediate and long-term management of urethral atresia, and clinical practice varies widely. We present an illustrative case managed with progressive urethral dilation alongside urinary diversion to highlight key factors in shared clinical decision making. Ultimately, pooled multi-institutional long-term outcomes data are needed to better guide practice for these patients and their families.

Abstract:Introduction: Patent urachus refers to a rare condition called urachus anomaly. These conditions are caused by the failure of the normal fetal tissue used to empty the fetal bladder to degenerate. Some of these urachal abnormalities are obvious at birth, while others are more subtle and are not diagnosed until adulthood or are discovered only incidentally after imaging for other reasons. Urachal anomalies in children with wet umbilical cord, persistent drainage, recurrent urinary tract infections, and recurrent umbilical cord infections require accurate and timely diagnosis to help relieve symptoms, prevent recurrent serious infections, and ensure timely treatment. Clinical case: The case of a 7-month-old male patient is presented. The mother reports umbilical granuloma of several months of evolution. He goes to a doctor where he suggests an ultrasound examination that rules out persistence of urachus. Mass in umbilical region persists, which is why she comes again. Evolution: By laparoscopic vision, umbilical hernia plus persistence of urachus is evident, findings such as the presence of indirect bilateral inguinal hernia are found. After surgery, the patient recovered satisfactorily. Conclusions: Diagnosis of urachal anomalies can be elusive, but continued drainage of the umbilical cord is a concern in the case of urachal anomalies. Although this condition is usually an isolated finding, the possibility of posterior urethral valves in infants with a patent urachus should not be ruled out.

Abstract:Introduction: This report describes a case with an ectopic ejaculatory duct opening into the bladder trigone in Zinner syndrome, congenital unilateral renal agenesis, and an ipsilateral seminal vesicle cyst. Case presentation: The patient was identified when no left kidney was detected in the fetal period. Abdominal ultrasonography and pelvic plain MRI at 6 months old revealed a 10-mm cystic lesion on the dorsal aspect of the bladder. Cysto-urethroscopy at 1 year old revealed a rather short posterior urethra and right and left inferior crests extending from the posterior urethra beyond the bladder neck. The ejaculatory duct opening was identified on the bladder trigone. Conclusion: Anatomical abnormality of the ejaculatory duct may represent a cause of infertility and ejaculatory dysfunction in Zinner syndrome. Endoscopic evaluation should be performed for this rare anomaly, even in children.

Abstract:Androgen insensitivity syndrome (AIS) is a very uncommon genetic disorder that results from the resistance of androgen receptor (AR) to androgen, which influences the formation of the male genitalia and in turn presents with female phenotype.Surgical resection of undesceaded testicle and different kinds of genitoplasty are crucial methods to correct the deformity of reproductive system, as well as hormone replacement therapy, which is an essential therapy for postoperational rehabilitation in AIS patients.A 43-year-old patient, who was socially female, was first admitted to gastroenterology department due to recurrent ascites and occasional abdominal pain with unknown origin.Taking physical examination, ultrasonography, karyotype analysis and sex hormone levels into consideration, the overall manifestations revealed the typical clinical features of complete androgen insensitivity syndrome.After that she was transferred to urology department for laparoscopic gonadectomy.During the surgery, doctors found that there was a vesical fistula on the upper wall near the conjunction between the bladder and ligamenta umbilicale medium, which explained the recurrent ascites for more than 4 years.After resecting the testicles and the tissues around the vesical fistula for histopathology, the result suggested Sertoli cell adenoma, hyperplastic Leydig cells and urothelium atypical hyperplasia.Hormone replacement therapy was given right after discharge.The hormone levels of follicle-stimulating hormone, luteinizing hormone, estradiol and progesterone were modulated by the dysfunction of androgen production after gonadectomy and hormone replacement therapy together with psychotherapy could stabilize her hormone levels and improve the quality of her life.The patient was suspicious of AIS family history and the pedigree was made to analyze her family which was possibly X-linked recessive pattern.We propose three possible hypotheses of the fistula, which are direct surgical injury, recurrence of bladder cancer and congenital urachal anomalies.But whether it is relevant between urachal anomalies and AIS is yet to be discovered.

Abstract:Congenital obstructive uropathy is a rare cause of ascites in infants. Majority of reported cases of genitourinary causes of ascites were due to posterior urethral valve. Here, we report a 6-month-old boy who presented with progressive tense ascites and peritonitis attributed by unilateral left distal ureteric obstruction and acute pyonephrosis. He underwent left nephrostomy placement, after which there was a remarkable improvement of ascites. He then underwent left ureteral diversion procedure a month later with a tentative plan for ureteral reanastomosis in 6 months. To date, there are no reports describing ascites secondary to distal ureteric obstruction beyond the neonatal period. The objective of this case report is to highlight unilateral urinary tract obstruction as a potential cause of transudative ascites. Additionally, the superimposed infection in the obstructed collecting system can lead to acute peritonitis likely due to translocation of bacteria into the peritoneal cavity.
Urethral duplication as cause of incontinence in a 5-years-old female: Surgical approach

Abstract:Introduction: Duplication of the urethra is a rare congenital anomaly in females which, if left unidentified, might cause refractory urinary incontinence. This video demonstrates its identification and treatment. Patient & methods: A 5-year-old female was referred for primary refractory diurnal incontinence without any dry moment since birth. Initial clinical examination showed permanent urine leakage from an opening cranial to the clitoris. Ultrasound did not show any bladder or kidney abnormalities. A diagnostic cystoscopy under anesthesia revealed an accessory meatus located cranially to the orthotopic meatus and to the clitoris. Results: Lying in dorsal decubitus under general anesthesia, catheters were inserted in respectively the orthotopic and accessory meatus. The accessory meatus and urethra were dissected away into the bladder dome from a circumferential skin incision. Defects were closed and bladder drainage per urethram was left in situ for 10 days. P ostoperative recovery was uncomplicated. Complete resolution of the incontinence was immediately achieved at catheter removal. Daytime and night-time continence are observed with 3 years follow-up. Conclusion: Urethral duplication is a rare finding in females but should be excluded during the workup of primary incontinence. In girls with isolated urethral duplication, resection of the accessory urethra is the best surgical option.

Abstract:Improving the dexterity of active prostheses is a major research area amalgamating machine learning algorithms and biosignals. A recent research niche has emerged from thisproviding proportional control to a prosthetic hand by modelling the force applied at the fingertips using surface electromyography (sEMG). The publicly released NinaPro database contains sEMG recording for 6 degree-of-freedom force activations for 40 intact subjects. In this preliminary study the authors successfully perform multivariate force regression using Bayesian mixture of experts (MoE). The accuracy of the model is compared to the benchmark set by the authors of NinaPro; comparable performance is achieved, however in this work a lower dimensional feature extraction representation obtains the best modelling accuracies, hence reducing training time. Inherent to the Bayesian framework is the inclusion of uncertainty in the model structure, providing a natural step in obtaining confidence bounds on the predictions. The MoE model used in this paper provides a powerful method for modelling force regression with application to actively controlling prosthetic and robotic arms for rehabilitation purposes, resulting in highly refined movements.

Abstract:Congenital true diphallia, complete duplicate bladder, bladder exstrophy and anorectal malformation in a child are uncommon. Here, we present the case of a three-year-old boy with multiple genitourinary malformation, including true diphallia, complete duplicate bladder, bladder exstrophy, epispadias, and anorectal malformation. Multi-departmental collaborative treatment for complex conditions ultimately achieved an ideal appearance for this patient. All vital signs were stable after the surgery and they remained consistent during follow-up. In such cases, surgical correction is individualized to achieve adequate urinary continence and erection with adequate esthetics.

Bladder Agenesis: A Systematic Review

Abstract:Bladder agenesis is a rare congenital deformity characterized by the absence of the bladder. It is primarily observed in postmortem dissections of stillbirths rather than live births. The condition is often associated with other congenital anomalies, leading to the hypothesis that most affected fetuses do not survive to term. However, the exact cause and specific associated anomalies remain unclear and poorly described in the literature. The limited mention of bladder agenesis in textbooks and literature underscores the importance of creating a comprehensive source for future research in this field. Therefore, our objective is to collect and analyze data on bladder agenesis, focusing on associated anomalies and potential causes, to enhance our understanding of the condition. We conducted a thorough review of reports collected from three databases, Google Scholar, PubMed, and Science Direct, last searched on July 30, 2023, starting with 327 reports. Excluding duplicates and records written in languages other than English, veterinary studies, irrelevant reports, or stillbirths. Inclusion criteria were the following: cases must have proven bladder agenesis, not hypoplasia, and must have most of the information, including the age of diagnosis, presenting symptoms, gender, associated anomalies, and management or outcome of the patient. A quality assessment was conducted according to the Joanna Briggs Institute checklist for case reports. A total of 65 case reports from 56 articles were included in the review. Through our manual analysis, we documented a wild array of malformations associated with bladder agenesis. Among the reports reviewed, 93% exhibited urinary system malformations beside bladder agenesis, 77% were found to have reproductive malformations, 44% had gastrointestinal anomalies, 38% showed musculoskeletal malformations, 28% had cardiac malformations, and another 28% had vascular anomalies. The overall mortality rate was 38%, with a higher rate of 74% for males compared to 20% for females. By collating and analyzing those case reports, we aim to contribute to a better understanding of bladder agenesis and its associated anomalies, facilitating further investigations and advancements in the field.
Transverse testicular ectopia in a newborn with transposition of the great arteries: A unique case report

Abstract:Introduction: Transverse testicular ectopia (TTE) is an extremely rare though well-documented congenital anomaly. In males with a 46XY karyotype, it is characterised by the herniation of both testes and part of the Müllerian organs into a single processus vaginalis. TTE is one of the three main clinical presentations of persistent Müllerian duct syndrome (PMDS). Transposition of the great arteries (TGA) is another rare congenital anomaly and severe cardiac condition. We present the likely first reported case of TTE with an accompanying malformation of TGA in a newborn. Case presentation: A 3-day-old Caucasian 46XY newborn with TGA was referred to the paediatric surgeons and endocrinologists for possible variations of sex characteristics (VSC). Despite a clinical examination revealing phenotypical male genitalia, an early postnatal ultrasound (US) suggestive of a uterine structure raised the suspicion of VSC. This patient had an arterial switch operation at 2 weeks of age before undergoing an exploration of the left groin at 8 weeks of age. Intraoperative findings revealed bilateral testes either side of a rudimentary uterus with fallopian tubes in the left inguinal canal. To avoid de-vascularising any structures, modified bilateral orchidopexy was performed placing each testis in the respective hemiscrotum with the uterus placed across the scrotal septum. Conclusion: We present the first reported case of TGA accompanying TTE. Early and accurate diagnosis, combined with the coordinated care by the specialist paediatric surgeon, cardiothoracic team, endocrinologist, and radiologist are essential for delivering timely, optimal care. This unique case raises the possibility of there being a link between TTE and TGA.

Abstract:IP-based networks such as the Internet traverse paths of widely ranging bandwidths and load factors. Several investigators have proposed the use of hierarchical video as a solution for dissemination of video across heterogeneous networks of this sort. In particular, such approaches avoid some the "least common denominator" problems inherent in single-layer heterogeneous multicast. We describe an experimental video delivery system that makes use of several new Internet technologies such as the resource reservation protocol (RSVP), integrated services (IntServ), the realtime transport protocol (RTF) and the IP multicast to control the dissemination of a hierarchical media streams across heterogeneous IP multicast networks.
Tryptophan methylester modulation of poult responses to Bordetella avium.

Abstract:On the day of hatching, four groups of poults [Control, L-tryptophan methylester (LTME), Bordetella avium-infected, and B. avium-infected plus LTME] were established and placed into heated metal brooding batteries. Bordetella avium infection caused a significant depression in body temperature within 24 h after intranasal challenge with the W strain, and the hypothermia persisted through 21 d of age. L-Tryptophan methylester, a water-soluble form of tryptophan, was given by oral gavage daily in saline at a concentration of 50 mg per poult beginning 4 d after hatch. Within 2 d after initiation of LTME treatments, colonic temperature of B. avium-infected poults was elevated to the level of Controls and remained at that level throughout the experimental period. The BW of B. avium-infected poults were reduced significantly. The LTME treatment caused a significant BW increase in the B. avium-infected poults, but the increase was not to the level of Controls. The anti-sheep red blood cell antibody titers in B. avium-infected poults were not affected significantly. However, LTME treatment induced a significant increase in anti-sheep red blood cell antibody titers in both the infected and Control poults. Based upon data reported herein, it was concluded that feed intake depression associated with development of bordetellosis caused the poults to react more specifically to a mild tryptophan deficiency than to other nutrient deficiencies. The tryptophan deficiency caused a growth depression that was only partially alleviated by daily supplementation of LTME. The physiological responses to daily supplementation of LTME to B. avium-infected poults suggested that growth depression and poor performance was not limited to dietary deficiency of tryptophan.

Abstract:Bladder exstrophy (BE) is a very rare congenital abnormality, especially in girls. We reported a 17-year-old girl with protruded bladder and urinary incontinence. Her vaginal orifice appeared stenotic and shifted anteriorly. She had an incomplete labia minora, short perineum, and anterior displacement of the anus. The pelvic radiography showed a pubic diastasis with a manta ray sign, and the ultrasonography examination showed a bicornuate uterus. She had undergone a functional reconstruction surgery with cystectomy, ileal conduit, appendectomy, and vaginoplasty along with the advancement of abdominal flap reconstruction with multiple Z-plasty. Functionally, she was able to urinate through the ileal conduit comfortably, had a regular menstrual cycle with minimal pain, had a wider vaginal canal, and had a smooth blood flow. Aesthetically, she had a good lower abdomen appearance, remained dry, and had an odorless urine. In clinical practice, this rare case report can provide additional knowledge and management of the same cases with effective results.
A Rare Case of an Irreducible Inguinoscrotal Hernia (Scrotal Cystocele) With a Left Vaginal Hydrocele: A Case Report

Abstract:Inguinal bladder hernia, also known as scrotal cystocele, first described by Dr. Levine, is considered among the rare cases that may or may not present as scrotal swelling depending on the size of the herniated part. In this particular patient, he presented with scrotal swelling and symptoms related to difficulties in micturition, such as incomplete voiding and increased frequency of voiding urine. On contrast-enhanced computed tomography (CECT), it was revealed that the bladder had been herniated into the inguinoscrotal region, accompanied by a left vaginal hydrocele. The hydrocele was then drained, and the left scrotal sac was everted. The hernial contents were reduced, and an open mesh repair was done to reduce the incidences of recurrence. These cases can generally be seen in obese male patients who are more than 50 years of age and males above 50 years with associated comorbidities. The diagnosis plays a very crucial role in these cases; if the presence of the bladder is not diagnosed preoperatively, then there is a fair chance that the bladder can undergo iatrogenic trauma during surgery. Hence, the clinician should be aware and should consider this situation if the patient comes under the criteria of being above 50 years old, with obesity and/or comorbidities, and with/without symptoms. Although rare, the condition can be treated by undergoing an open surgical repair, wherein the hernial contents are reduced and mesh is put in to avoid reoccurrence.
