Abstract:OBJECTIVES The aim of the study was to evaluate both the effectiveness of ureteropelvic junction stenting with the use of a polymeric "double J" stent after laparoscopic pyeloplasty, as well as intra- and postoperative complications of the procedure. MATERIAL AND METHODS From October 2001 to November 2010 laparoscopic pyeloplasty was performer in 150 patients with primary UPJO. In all cases an attempt has been made to insert the stent into the operated kidney before the operation. All but one operations were performed using a transperitoneal approach. Anderson-Hynes pyeloplasty was carried out in 85 cases, Y-V plasty in 61 cases and Fenger plasty in 2 patients. Open conversion was performer in two cases. RESULTS In 13 cases the complications observed in the postoperastive period were connected with the polymeric stent. In 12 patients the obstruction of the stent lead to massive urinary leakage and in 3 cases from this group urinoma developed. Urinoma was also observed in a patient, who was left without stenting because of the difficulties in inserting the stent to the operated kidney. In 10 cases the stent was successfully replaced but in 3 cases the placement of a percutaneous nephrostomy tube was necessary. It was left in place for 3 weeks. CONCLUSIONS Our data indicate, that the stent obstruction is the main cause of postoperative complications in patients after laparoscopic pyeloplasty. So far no better method of upper urinary tract drainage after laparoscopic pyeloplasty has been worked out. Experimenting with various types of stents has been of no avail.

Abstract:We developed neratinib-resistant HER2-mutant cancer cells by gradual dose escalation. RNA sequencing identified TORC1 signaling as an actionable mechanism of drug resistance. Primary and acquired neratinib resistance in HER2-mutant breast cancer patient-derived xenografts (PDXs) was also associated with TORC1 hyperactivity. Genetic suppression of RAPTOR or RHEB ablated P-S6 and restored sensitivity to the tyrosine kinase inhibitor. The combination of the TORC1 inhibitor everolimus and neratinib potently arrested the growth of neratinib-resistant xenografts and organoids established from neratinib-resistant PDXs. RNA and whole-exome sequencing revealed RAS-mediated TORC1 activation in a subset of neratinib-resistant models. DNA sequencing of HER2-mutant tumors clinically refractory to neratinib, as well as circulating tumor DNA profiling of patients who progressed on neratinib, showed enrichment of genomic alterations that converge to activate the mTOR pathway.

Abstract:Abstract Introduction: The anaplastic lymphoma kinase (ALK) gene encodes a receptor tyrosine kinase involved in cellular proliferation, differentiation, and cell death. ALK becomes oncogenic when it forms a fusion gene. The EML4-ALK fusion gene has been identified mainly in non-small cell lung cancer (NSCLC). It is unknown whether neratinib, an irreversible pan-HER tyrosine kinase inhibitor (TKI) targeting EGFR, HER2 and HER4, is effective in cancers harboring EML4-ALK gene fusion. The objective of this study was to assess in vitro efficacy of neratinib in combination with dasatinib (Src/Abl TKI) or crizotinib (ALK TKI) in EML4-ALK+ NSCLC. Methods: The antiproliferative effects of neratinib, dasatinib and crizotinib were assessed in the CRISPR/Cas9-modified EML4-ALK fusion-A549 (EML4-ALK+) and parental A459 (A549-Par) NSCLC cell lines by 5-day acid phosphatase assay. IC50 values were calculated using CalcuSyn software. To assess the synergy between neratinib and dasatinib, and neratinib and crizotinib, matrix assays were performed and analyzed using Combenefit software. To further assess the efficacy of the TKIs, changes in signaling pathways were assessed by Western blotting, and apoptosis induction and cell migration were measured with the Incucyte® S3 imaging system. Results: Neratinib, crizotinib, and dasatinib displayed nanomolar IC50 values in both cell lines. As expected, EML4-ALK+ cells were more sensitive to crizotinib than A549-Par cells (IC50 = 595 nM vs 1 μM, p ≤ 0.05). EML4-ALK expression led to numerical increases in neratinib IC50 value (326.37 ± 44.34 nM in EML4-ALK+ vs 247 ± 32.65 nM in A549-Par) and dasatinib IC50 value (39.95 ± 5.67 nM in EML4-ALK+ vs 27.75 ± 18.53 nM in A549-Par) but these changes were not statistically significant. Matrix assays showed that neratinib-crizotinib (NC) was more effective than crizotinib alone in both cell lines, with more potent sensitivity in EML4-ALK+ cells. The neratinib-dasatinib (ND) combination was synergistic in the A549-Par cell line but only additive in the EML4-ALK+ cell line, suggesting a reduced sensitivity to the ND combination. EML4-ALK+ cells had sustained pERK1/2 levels after treatment with all TKIs, except NC, compared to A549-Par cells. Neratinib alone was significantly less effective at inhibiting pAkt (99.2 ± 5.3 % in EML4-ALK+ vs 64.9 ± 19.0 %, p ≤ 0.05) and pERK1/2 (91.3 ± 4.6 % in EML4-ALK+ vs 54.9 ± 15.9 %, p ≤ 0.01) in EML4-ALK+ cells than in A549-Par cells at 24h. In A549-Par cells, ND caused the highest apoptotic induction, followed by NC. Apoptosis levels induced by ND in EML4-ALK+ cells were 4-fold lower at 72hr than in A549-Par cells (p ≤ 0.0001). Dasatinib and ND were more effective than crizotinib or NC at preventing cell migration in both cell lines. Conclusion: EML4-ALK potentially decreases sensitivity to the neratinib-dasatinib combination. This warrants further investigation in other EML4-ALK+ models. Citation Format: Myra E. Castel, Neil T. Abstract:Abstract Acquired resistance to Osimertinib continues to pose a major limitation to achieving durable therapeutic responses in EGFR mutant non-small cell lung cancer (NSCLC). While multiple actionable genetic mutations have been identified as the underlying molecular basis of resistance, a genomic mechanism cannot be identified in >50% of Osimertinib resistant tumors and thus an effective combinatorial treatment strategy cannot be deployed for these patients. Recently, we identified YAP/TEAD activation and ERK1/2 reactivation as two major non-mutational counter-regulatory pathways that diminish the efficacy of Osimertinib, promoting establishment of the drug tolerant persister (DTP) and resistant states. Moreover, it is well known that cancer cells escape tyrosine kinase inhibitor (TKI) therapy by developing dependence on alternate, non-mutated receptor tyrosine kinases (RTKs) through a treatment-induced “RTK switch” phenomenon. The link however between alternate RTK signaling and YAP/TEAD/ERK1/2 has never been explored in the context of Osimertinib resistance. In this study, we systemically uncover transcriptional de-repression of, and critical dependence on PDGFRβ as part of a novel “RTK switch” mechanism deployed by cells to evade Osimertinib-induced death. Genetic or pharmacologic co-inhibition of PDGFRβ renders NSCLC cells more sensitive to Osimertinib during drug naïve, DTP and resistant phases of treatment through suppression of ERK1/2 reactivation, YAP activity/nuclear translocation and YAP-mediated induction of EMT factors, SLUG and Zeb2. Importantly, compared to Osimertinib alone, co-targeting EGFR and PDGFRβ robustly suppresses tumor regrowth following treatment cessation both in vitro and in vivo which is consistent with a reduction in the DTP cell population. These data auspiciously demonstrate a two-fold utility of concomitant PDGFRβ inhibition: to enhance the initial efficacy of Osimertinib and to eliminate the DTP and resistant cells following chronic Osimertinib treatment. Further mechanistic studies from integration of ATAC-seq, CUT&RUN and RNA-seq reveal that Osimertinib induces the dynamic cooperativity of chromatin modulator BRD4 with pluripotent POU-domain transcription factors Oct-4, Pit-1 and Brn-3 at putative proximal enhancer elements, facilitating upregulation of PDGFRβ. Collectively, we demonstrate a novel treatment-induced BRD4/POUF epigenetic/transcriptional mediated “RTK switch” that involves co-opting PDGFRβ signaling to activate YAP/TEAD and ERK1/2 as a means of driving adaptive resistance to Osimertinib. These data provide compelling rationale for combined anti-EGFR and PDGFRβ targeted therapy to improve patient outcomes. Abstract:Introduction: Approximately 50% of melanomas harbor an activating BRAFV600E mutation. Standard of care involves a combination of inhibitors targeting mutant BRAF and MEK1/2, the substrate for BRAF in the MAPK pathway. PTEN loss-of-function mutations occur in ~40% of BRAFV600E melanomas, resulting in increased PI3K/AKT activity that enhances resistance to BRAF/MEK combination inhibitor therapy. Methods: To compare the response of PTEN null to PTEN wild-type cells in an isogenic background, CRISPR/Cas9 was used to knock out PTEN in a melanoma cell line that harbors a BRAFV600E mutation. RNA sequencing, functional kinome analysis, and drug synergy screening were employed in the context of BRAF/MEK inhibition. Results: RNA sequencing and functional kinome analysis revealed that the loss of PTEN led to an induction of FOXD3 and an increase in expression of the FOXD3 target gene, ERBB3/HER3 . Inhibition of BRAF and MEK1/2 in PTEN null, BRAFV600E cells dramatically induced the expression of ERBB3/HER3 relative to wild-type cells. A synergy screen of epigenetic modifiers and kinase inhibitors in combination with BRAFi/MEKi revealed that the pan ERBB/HER inhibitor, neratinib, could reverse the resistance observed in PTEN null, BRAFV600E cells. Conclusions: The findings indicate that PTEN null BRAFV600E melanoma exhibits increased reliance on ERBB/HER signaling when treated with clinically approved BRAFi/MEKi combinations. Future studies are warranted to test neratinib reversal of BRAFi/MEKi resistance in patient melanomas expressing ERBB3/HER3 in combination with its dimerization partner ERBB2/HER2.

Abstract:Purpose: Neratinib is an irreversible, pan-HER tyrosine kinase inhibitor that is FDA approved for HER2-overexpressing/amplified (HER2 + ) breast cancer. In this preclinical study, we explored the efficacy of neratinib in combination with inhibitors of downstream signaling in HER2 + cancers in vitro and in vivo . Experimental Design: Cell viability, colony formation assays, and Western blotting were used to determine the effect of neratinib in vitro . In vivo efficacy was assessed with patient-derived xenografts (PDX): two breast, two colorectal, and one esophageal cancer (with HER2 mutations). Four PDXs were derived from patients who received previous HER2-targeted therapy. Proteomics were assessed through reverse phase protein arrays and network-level adaptive responses were assessed through Target Score algorithm. Results: In HER2 + breast cancer cells, neratinib was synergistic with multiple agents, including mTOR inhibitors everolimus and sapanisertib, MEK inhibitor trametinib, CDK4/6 inhibitor palbociclib, and PI3Kα inhibitor alpelisib. We tested efficacy of neratinib with everolimus, trametinib, or palbociclib in five HER2 + PDXs. Neratinib combined with everolimus or trametinib led to a 100% increase in median event-free survival (EFS; tumor doubling time) in 25% (1/4) and 60% (3/5) of models, respectively, while neratinib with palbociclib increased EFS in all five models. Network analysis of adaptive responses demonstrated upregulation of EGFR and HER2 signaling in response to CDK4/6, mTOR, and MEK inhibition, possibly providing an explanation for the observed synergies with neratinib. Conclusions: Taken together, our results provide strong preclinical evidence for combining neratinib with CDK4/6, mTOR, and MEK inhibitors for the treatment of HER2 + cancer.

Abstract:Abstract Background. Neratinib is an orally available irreversible pan-HER receptor tyrosine kinase inhibitor that has clinical benefit in patients with HER2-positive and HER2-mutated breast cancer. Neratinib also targets the other two members of the HER family, namely EGFR and HER4. However, tentative evidence suggests that neratinib may affect kinases of non-HER family. In the present study, we aimed to explore the pattern of kinases targeted by neratinib, in the context of HER2 non-amplified cancer cells and explored the potential clinical implications. Method. A panel of HER2-low/medium-expressing (MCF-7 and ZR-75-1) and HER2- negative (MDA-MB-231 and BT549) human breast cancer cells were used. The response of a panel of 304 kinases to neratinib was screened using the kinase antibody array. Cellular toxicity was quantified using a colorimetric method. Candidate neratinib-responsive kinases were subject to analyses against the survival of a breast cancer cohort as well as a public database for additional validation. The protein expression level of the kinase targets identified was also validated on tissue arrays from breast cancer by immunohistochemistry. Result. Of all the kinases tested, thirty-six were found to be influenced by neratinib, either via activation or suppression of their phosphorylation. Based on the differential responses to neratinib in HER2 negative and positive breast cancer cells, a small panel of the kinases were classified as HER2 independent. Using clinical cohorts, these kinases collectively had predictive value for the overall survival of the patients (p<0.0001). We further analysed one of the candidate kinases, namely salt inducible kinase-1 (SIK1) and the SIK family in relation with HER2. HER2 expression was found significantly correlated with both SIK1 (r=0.46, p<0.001) and also other two SIK family members. SIK1 protein staining in the tissue array was found to be aberrant in breast tumours. Conclusion. Neratinib has an effect on a subset of HER2 independent kinases in breast cancer cells. These kinases had a prognostic value on patient's survival and correlated with HER2 expression, as in the case of the SIK family.This finding may have important implication in understanding the biology of HER2 inhibitors such as neratinib. TGFβ2-mediated epithelial–mesenchymal transition and NF-κB pathway activation contribute to osimertinib resistance

Abstract:Osimertinib (AZD9291) has been widely used for the treatment of EGFR mutant non-small cell lung cancer. However, resistance to osimertinib is inevitable. In this study we elucidated the molecular mechanisms of resistance in osimertinib-resistant NCI-H1975/OSIR cells. We showed that NCI-H1975/OSIR cells underwent epithelial–mesenchymal transition (EMT), which conferred sensitivity to the GPX4 inhibitor 1<i>S</i>, 3<i>R</i>-RSL3 to induce ferroptotic cell death. The EMT occurrence resulted from osimertinib-induced upregulation of TGFβ2 that activated SMAD2. On the other hand, we revealed that NCI-H1975/OSIR cells were highly dependent on NF-κB pathway for survival, since treatment with the NF-κB pathway inhibitor BAY 11–7082 or genetic silence of p65 caused much greater cell death as compared with the parental NCI-H1975 cells. In NCI-H1975 cells, osimertinib activated NF-κB pathway, evidenced by the increased p65 nuclear translocation, which was abolished by knockdown of TGFβ2. In the cancer genome atlas lung adenocarcinoma data, TGFB2 transcript abundance significantly correlated with EMT-associated genes and NF-κB pathway. In addition, coexistence of EMT and activation of NF-κB pathway was observed in several NCI-H1975/OSIR clones. These findings shed new light on distinct roles of TGFβ2 in osimertinib-resistant cells and provide new strategies for treatment of this resistant status.

Abstract:Abstract Background. The EGF receptor (EGFR) kinase family plays important role in tumour growth and progression and has been a validated therapeutic target for lung cancers. Here, we examined the activity of an irreversible pan-HER tyrosine kinase inhibitor, neratinib, on kinase phosphorylation and cellular response in human gastric cancer. The study also examined the expression of the neratinib responsive kinases in relation to chemoresistance in human gastric cancer. Method. Tumour and normal gastric tissues from a cohort of 85 patients were obtained along with clinical outcome and therapeutic response to chemotherapy. The level of expression of a panel of protein kinases of interest, including EGFR family members, was quantitatively analysed using a protein based kinase array. Human gastric cancer cells (AGS and HGC27) were assessed for their sensitivity to neratinib, inhibitors of other candidate kinases and combinations thereof. Cellular growth, migration and matrix adhesiveness were evaluated using multiple in vitro platforms. Result. We identified a panel of kinases that were highly responsive to the treatment by neratinib in cell lines. In the gastric cancer cohort, expression of these kinases significantly correlated with the overall survival of the patients (p<0.001) and, together with node involvement, invasion, staging and type of surgery, was an independent prognostic indicator for the clinical outcome. Expression of the EGFR kinase family members EGFR, HER2, and HER4 was linked to poor clinical outcome. Expression of both neratinib response kinases and EGFR family members was also associated with therapeutic resistance to chemotherapies in the cohort of patients analyzed. In vitro, neratinib caused a concentration dependent inhibition of migration, matrix adhesiveness and growth in human gastric cancer cells. The migratory pace of the gastric cancer cells was particularly sensitive to neratinib. The role of two neratinib response kinases, Protein Kinase C iota (PKCi) and Mouse Double Minute 2 homologue (MDM2), was then further investigated. Interestingly, both PKCi and MDM2 were aberrantly expressed in gastric cancer tissues. In addition, the PKCi inhibitor Oncrasin-1 and the MDM2 antagonist Nutlin-3 both demonstrated synergy with neratinib in inhibiting adhesiveness and migration of the gastric cancer cells. Conclusion. Our data indicates that EGFR family members and other neratinib responsive kinases are aberrantly expressed in human gastric cancer and may be of prognostic value for the patients. In addition, gastric cancer cells are sensitive to neratinib, and displayed reduced migration when combined with inhibitors of PKCi or MDM2, two of the neratinib responsive kinases. Abstract:Abstract Breast cancer can be classified into complicated malignant subtypes based on advances of clinical and research evidence. There are still unmet needs for the more effective treatment of breast cancer patients under specific pathological conditions (stages/subtypes). Neratinib is considered as a second-generation inhibitor of the EGFR family members (EGFR, HER2 and HER4) of receptor kinases. It can irreversibly inhibit the EGFR and HER2 tyrosine kinases by targeting a cysteine residue in the ATP-binding site of the receptor. Recently neratinib has been approved by FDA for the extended adjuvant treatment of early-stage HER2+ breast cancer. In this study, to further dissect the molecular mechanisms of neratinib, the global gene expression profiling analysis of breast cancer cells treated with neratinib was performed using RNA AmpliSeq transcriptome (including 20816 gene candidates) in the Ion PI sequencing system. We selected two breast cancer cell lines, the triple negative MDA-MB-231 cell line and the low HER2-expressing, but not-amplified, ER+ MCF-7 cell line. The RNA-Seq data show differential gene profiling heatmap from these two breast cancer cell lines. The RNA-Seq data confirmed the downregulation of HER2 in MCF-7 cells, but there was no change of gene expression of EGFR, HER3 and HER4. In both MDA—MB-231 and MCF-7 cell lines, there was a panel of upregulating genes including KIAA1024, ZNF550, MESDC1, TMC8, ZNF524, AGBL2, HIST2H2BC and PPARGC1B. In contrast, with response to neratinib, another cluster of genes was downregulated in both cell lines including TAS2R5, KRT14, VWCE, LY9, PAPLN and STK4-AS1. In MDA-MB-231 cells, neratinib intended to regulate LPS/IL-1 and eNOS mediated signaling pathways. However, in MCF-7 cells, neratinib regulated ephrin receptor (EPH) signalling and PPARα activation. We then investigated the possible HER2-independent effect of neratinib in breast cancer cells such as regulation of EPH. Our data using the electric cell-substrate impedance sensing (ECIS) indicated that MCF-7 cells responded to an EphB4 inhibitor, NVP-HPG-712, by a marked reduction of cellular migration, in contrast to MDA MB-231 which does not respond to the inhibitor. Similarly, MCF-7 cells also responded to the combinational treatment of neratinib and NVP-HPG-712 with a decrease in cellular migration. Again, MDA-MB-231 failed to yield a response. In summary, differential analysis of transcripts by high throughput RNA-seq could be valuable to provide deeper insight into the gene regulation and signaling pathway prediction of breast cancer cells with response to neratinib when the large scale of patient samples and multiple types of breast cancer cell lines are integrated. Abstract:Lazertinib, a novel third-generation EGFR-tyrosine kinase inhibitor, has shown marked efficacy in patients with EGFR-mutant lung cancer. However, after various periods of therapeutic intervention, most patients eventually acquire resistance to lazertinib. A potential therapeutic approach to address this clinical issue is to overcome the initial drug resistance mechanism. To determine the underlying adaptive lazertinib resistance mechanisms, we evaluated signal transduction, cell apoptosis, and drug sensitivity in mutant EGFR-expressing non-small cell lung cancer (NSCLC) cells treated with lazertinib. The siRNA screening assay demonstrated that AXL knockdown significantly inhibited cell viability in the presence of lazertinib, indicating that AXL activation contributes to lazertinib resistance. However, long-term culture with a combination of lazertinib and AXL inhibitors eventually led to the proliferation of residual cells and increased MCL-1 expression, which was mediated by the nuclear translocation of the transcription factor YAP. Cell-based assays showed that triple therapy with an MCL-1 or YAP inhibitor, in combination with EGFR-TKI lazertinib, an AXL inhibitor, significantly reduced cell viability and increased apoptosis. These results demonstrate that AXL and YAP/MCL-1 signals contribute to the adaptive lazertinib resistance in EGFR-mutant NSCLC cells, suggesting that the initial dual inhibition of AXL and YAP/MCL-1 might be highly effective in eliminating lazertinib-resistant cells. Abstract:A series of molecular targeted drugs have been developed for the treatment of non-small cell lung cancer (NSCLC) harboring driver gene alterations. Although the use of these drugs has substantially improved the clinical outcomes of patients with the genetically altered NSCLC, these patients ultimately develop resistance to the drugs. Various molecular mechanisms of resistance to the molecular target therapies have been reported, it is still a crucial challenge to overcome the drug resistance. Osimertinib, the most used third-generation EGFR tyrosine kinase inhibitor to target both common EGFR mutations and secondary T790M mutation, and sotorasib, a recently developed KRAS inhibitor to target KRAS G12C mutation, are no exception. In this study, we aimed to investigate the mechanisms of acquired resistance to osimertinib and sotorasib in NSCLC by profiling drug-resistant lung cancer cell lines, which were established from EGFR-mutant HCC827(del E756_A750) and KRAS G12C-mutant H23 through exposure to each of the molecular targeted drugs. Phospho-RTK array and western blot analyses revealed MET overexpression and phosphorylation in the osimertinib-resistant HCC827 (HCC827OR) as previous studies reported. Notably in our study, HCC827OR cells were incompletely sensitive to a selective MET inhibitor savolitinib without osimertinib, suggesting that HCC827OR partially lost its dependency on mutant EGFR and got addiction to MET signaling. When sotolasib was administered to the sotolasib-resistant H23 (H23SR) cells, KRAS-GTP, and the downstream ERK and AKT phosphorylation were suppressed. To identify novel mechanisms of the resistance, we performed epigenomic profiling of active enhancer in those cells and found a candidate transcription factor, at whose gene locus H3K27 acetylation signal was increased in H23SR cells compared to parent H23 cells. Immunoblotting showed that protein expression of the gene was indeed higher in H23SR cells than the parent cells. To confirm the involvement of this gene to the resistance, further investigation is warranted. Abstract:Abstract Purpose: In metastatic colorectal cancer (mCRC), HER2 (ERBB2) gene amplification is implicated in anti-EGFR therapy resistance. We sought to determine the recommended phase II dose (RP2D) and efficacy of neratinib, a pan-ERBB kinase inhibitor, combined with cetuximab, in patients with progressive disease (PD) on anti-EGFR treatment. Patients and Methods: Twenty-one patients with quadruple-wild-type, refractory mCRC enrolled in this 3+3 phase Ib study. Standard dosage cetuximab was administered with neratinib at 120 mg, 160 mg, 200 mg, and 240 mg/day orally in 28-day cycles. Samples were collected for molecular and pharmacokinetic studies. Results: Sixteen patients were evaluable for dose-limiting toxicity (DLT). 240 mg was determined to be the RP2D wherein a single DLT occurred (1/7 patients). Treatment-related DLTs were not seen at lower doses. Best response was stable disease (SD) in 7 of 16 (44%) patients. HER2 amplification (chromogenic in situ IHC) was detected in 2 of 21 (9.5%) treatment-naïve tumors and 4 of 16 (25%) biopsies upon trial enrollment (post-anti-EGFR treatment and progression). Compared with matched enrollment biopsies, 6 of 8 (75%) blood samples showed concordance for HER2 CNV in circulating cell-free DNA. Five SD patients had HER2 amplification in either treatment-naïve or enrollment biopsies. Examination of gene-expression, total protein, and protein phosphorylation levels showed relative upregulation of ≥2 members of the HER-family receptors or ligands upon enrollment versus matched treatment-naïve samples. Conclusions: The RP2D of neratinib in this combination was 240 mg/day, which was well tolerated with low incidence of G3 AEs. There were no objective responses; SD was seen at all neratinib doses. HER2 amplification, detectable in both tissue and blood, was more frequent post-anti-EGFR therapy.

Abstract:The clinical efficacy of EGFR tyrosine kinase inhibitors (TKIs) in non-small cell lung cancer (NSCLC) harbouring activating EGFR mutations is limited by the emergence of acquired resistance, mostly ascribed to the secondary EGFR-T790M mutation. Selective EGFR-T790M inhibitors have been proposed as a new, extremely relevant therapeutic approach. Here, we demonstrate that the novel irreversible EGFR-TKI CNX-2006, a structural analog of CO-1686, currently tested in a phase-1/2 trial, is active against in vitro and in vivo NSCLC models expressing mutant EGFR, with minimal effect on the wild-type receptor. By integration of genetic and functional analyses in isogenic cell pairs we provide evidence of the crucial role played by NF-κB1 in driving CNX-2006 acquired resistance and show that NF-κB activation may replace the oncogenic EGFR signaling in NSCLC when effective and persistent inhibition of the target is achieved in the presence of the T790M mutation. In this context, we demonstrate that the sole, either genetic or pharmacologic, inhibition of NF-κB is sufficient to reduce the viability of cells that adapted to EGFR-TKIs. Overall, our findings support the rational inhibition of members of the NF-κB pathway as a promising therapeutic option for patients who progress after treatment with novel mutant-selective EGFR-TKIs.

Abstract:Several aspects of EGFR TKI treatment have been discussed.The issue of TKIs resistance has been treated in some papers. In their Opinion, Bronte G et al discussed the topic of Osimertinib resistance and the possible combination strategies or the use of fourth generation EGFR-TKIs to overcome it. They supposed that in the next future oncologists will be able to address each patients who experience resistance to upfront osimertinib toward a different treatment strategy on the basis of molecular alterations highlighted by liquid biopsy or tumor biopsy. In this line, Carlo Bao et al presented two clinical cases harboring concomitant EGFR and BRAF alterations and treated with Osimertinib. Both cases were responsive to treatment and liquid biopsy results were in accordance with the clinical behaviour. Patients with advanced NSCLC with secondary T790M can benefit from Osimertinib, but the role of this drug in patients who exhibith resistance without T790M or with T790M unknown status is not well established. From the systematic review and meta-analysis performed by Yi XF and colleagues emerged that patients who undergo progression with brain metastasis on first or second generation TKIs can benefit from subsequent Osimertinib regardless T790M status. This represent an important finding that could orientate clinical decision management after Osimertinib resistance.Other types of resistance mechanisms are known, and the role of tumor-associated macrophages (TAMs) is also emerging (1). Previous studies have demonstrated that high infiltration of TAMs is significantly associated with an unfavorable prognosis in NSCLC patients treated with EGFR-TKIs (2). So, the inhibition of TAMs may a potential approach to improving resistance to EGFR-TKIs. In the review reported by Cheng D et al, a number of preclinical studies have been discussed, analysing the combination of EGFR-TKIs with several compound directed versus TAMs. In particular, inhibiting mTOR, AKT and STAT3 pathways, such as the lipid metabolic pathway, could all be possible potenatial strategies to overcome EGFR-TKI resistance.The addition of antiangiogenic treatments to EGFR-TKis was also studied. The systematic review and meta-analysis by Zheng H et al highlighted that the addition of bevacizumab to EGFR-TKI provide significant better PFS and OS, and the benefit is more evident for patients who have ever smoked, aged <75 years and of Asian population. Hsu PC and colleagues reported the results of a study in which first-line bevacizumab was combined with erlotinib or afatinib in EGFR-mutated NSCLC, demonstrating the development of T790M as resistance mechanism in 57.9% of cases. The addition of anti-angiogenic treatment in patients with an acquired resistance to Osimertinib was also demonstrated in a clinical case described by He W et al. In particular, a NSCLC patients harboring acquired EGFR 19Del/T790M/cis-C797S mutation resistance was treated with sintilimab, an anti-VEGF drug and chemotherapy. The patient remained progression-free for 15 months and the regimen was well tolerated. The addition of immunotherapy in EGFR-mutated patients is a controversial topic (3). Microsatellite instability is a rare event in NSCLC, but Yang M et al reported a case of apatients with a rare pulmonary enteric adenocarcinoma with EGFR mutation and MSI-H, who receive benefit from a combination approach of EGFR-TKI plus immune check point inhibitor.Rare EGFR mutations are another important issue object of several studies in the last years. It was reported that afatinib, an irreversible ErbB family inhibitor, is more efficacious in treating patients with uncommon EGFR mutations (4). Dong W et al performed a literature search on studies evaluating the efficacy of afatinib in any line of treatment in NSCLC patients with uncommon EGFR mutations. Their conclusion was that afatinib seems to be effective in patients with the more frequent uncommon EGFR mutations, whereas inconsistent data are present with regard to the other uncommon EGFR alterations, due to the high heterogeneity of them. Christopoulos P et al described seven clinical cases of patients carrying different uncommon EGFR mutations treated with afatinib. Overall, patients responded to afatinib, with six partial response and three durable responses. Interestingly, co-mutations did not preclude sensitivity to the drug, with durable response observed also in patients exhibiting cooccurring TP53 or CDKN2A mutations. Another case report was reported by Wang P et al, describing a patient carrying two exon 18 mutations and who acquired an EGFR amplification after treatment with osimertinib. The patient showed a partial response to neratinib. Also, a further case report of a patient carrying an EGFR kinase domain duplication was reported by Lin H and colleagues. Abstract:Activating mutations in the epidermal growth factor receptor (EGFR) gene in non-small cell lung cancer (NSCLC) patients are associated with clinical benefit in the metastatic and adjuvant settings when treated with EGFR inhibitors, such as osimertinib. Despite its dramatic efficacy, most patients are partial responders and refractive disease leave limited treatment options. The objective was to dissect the mechanism of osimertinib resistance in EGFR mutant NSCLC models to identify a means of re-sensitization. Imaging mass cytometric analysis of 76 NSCLC patients demonstrated EGFR expression was inversely correlated to CD105 membrane expression. CD105 is a transforming growth factor beta (TGF-β) family co-receptor that promotes bone morphogenic protein (BMP) signaling and inhibits TGF-β signaling. Significant elevation in cell surface CD105 by osimertinib was furthered by EGFR knockdown in two NSCLC lines with EGFR activating mutations. These lines were subjected to a program of increasing osimertinib concentrations to generate isogenic resistant lines. Heterogeneous drug-tolerant persister cells had significantly higher mutational load, increased CD105 cell surface expression as well as greater expression of bypass signaling factors such as ERK, PI3K, and BMP ligands compared to their parental counterparts. Single cell RNA-seq analysis of parental NSCLC revealed a small population of cells that shared elevated endothelial and pyrimidine metabolism, with a slow-cycling signature that was enriched in the osimertinib-resistant cells. Combining osimertinib with a CD105 neutralizing antibody, carotuximab, reduced the slow-cycling population and restored osimertinib sensitivity in resistant cell lines. The knockdown of CD105 had a more pronounced decrease in cell viability when combined with osimertinib. The osimertinib-resistant lines demonstrated more condensed chromatin and glycolytic state compared to the respective parental lines by ATAC-seq and Single Cell ENergetIc metabolism profiling, respectively. The addition of carotuximab reversed the chromatin and metabolic reprograming of osimertinib resistance. Carotuximab was discovered to initiate CD105 interaction with EGFR and its dominant active variant, EGFRv3. Ultimately, combination therapy of carotuximab and osimertinib resulted in significantly reduced tumor expansion compared to mice treated with either drug alone. Paradoxically, the small tumors from combination therapy had greater mitosis compared to the larger tumors of the osimertinib single agent treated mice. Inhibition of CD105 with carotuximab can overcome resistance to osimertinib and inhibit NSCLC tumor progression. This is a first-in-class pre-clinical foundation for a novel synthetic lethality treatment strategy resulting from the observation of global changes in EGFR-antagonist resistance as opposed to individual mutations identified in tumor subpopulations. SOS1 and KSR1 modulate MEK inhibitor responsiveness to target resistant cell populations based on PI3K and KRAS mutation status

Abstract:KRAS is the most commonly mutated oncogene. Targeted therapies have been developed against mediators of key downstream signaling pathways, predominantly components of the RAF/MEK/ERK kinase cascade. Unfortunately, single-agent efficacy of these agents is limited both by intrinsic and acquired resistance. Survival of drug-tolerant persister cells within the heterogeneous tumor population and/or acquired mutations that reactivate receptor tyrosine kinase (RTK)/RAS signaling can lead to outgrowth of tumor-initiating cells (TICs) and drive therapeutic resistance. Here, we show that targeting the key RTK/RAS pathway signaling intermediates SOS1 (Son of Sevenless 1) or KSR1 (Kinase Suppressor of RAS 1) both enhances the efficacy of, and prevents resistance to, the MEK inhibitor trametinib in KRAS -mutated lung (LUAD) and colorectal (COAD) adenocarcinoma cell lines depending on the specific mutational landscape. The SOS1 inhibitor BI-3406 enhanced the efficacy of trametinib and prevented trametinib resistance by targeting spheroid-initiating cells in KRAS G12/G13 -mutated LUAD and COAD cell lines that lacked PIK3CA comutations. Cell lines with KRAS Q61 and/or PIK3CA mutations were insensitive to trametinib and BI-3406 combination therapy. In contrast, deletion of the RAF/MEK/ERK scaffold protein KSR1 prevented drug-induced SIC upregulation and restored trametinib sensitivity across all tested KRAS mutant cell lines in both PIK3CA -mutated and PIK3CA wild-type cancers. Our findings demonstrate that vertical inhibition of RTK/RAS signaling is an effective strategy to prevent therapeutic resistance in KRAS -mutated cancers, but therapeutic efficacy is dependent on both the specific KRAS mutant and underlying comutations. Thus, selection of optimal therapeutic combinations in KRAS -mutated cancers will require a detailed understanding of functional dependencies imposed by allele-specific KRAS mutations.

Abstract:A tri-block nanoparticle (TBN) comprising of an enzymatically cleavable porous gelatin nanocore encapsulated with gefitinib (tyrosine kinase inhibitor (TKI)) and surface functionalized with cetuximab-siRNA conjugate has been synthesized. Targeted delivery of siRNA to undruggable KRAS mutated non-small cell lung cancer cells would sensitize the cells to TKI drugs and offers an efficient therapy for treating cancer; however, efficient delivery of siRNA and releasing it in cytoplasm remains a major challenge. We have shown TBN can efficiently deliver siRNA to cytoplasm of KRAS mutant H23 Non-Small Cell Lung Cancer (NSCLC) cells for oncogene knockdown; subsequently, sensitizing it to TKI. In the absence of TKI, the nanoparticle showed minimal toxicity suggesting that the cells adapt a parallel GAB1 mediated survival pathway. In H23 cells, activated ERK results in phosphorylation of GAB1 on serine and threonine residues to form GAB1-p85 PI3K complex. In the absence of TKI, knocking down the oncogene dephosphorylated ERK, and negated the complex formation. This event led to tyrosine phosphorylation at Tyr627 domain of GAB1 that regulated EGFR signaling by recruiting SHP2. In the presence of TKI, GAB1-SHP2 dissociation occurs, leading to cell death. The outcome of this study provides a promising platform for treating NSCLC patients harboring KRAS mutation.
Neratinib overcomes trastuzumab resistance in HER2 amplified breast cancer

Abstract:Trastuzumab has been shown to improve the survival outcomes of HER2 positive breast cancer patients. However, a significant proportion of HER2-positive patients are either inherently resistant or develop resistance to trastuzumab. We assessed the effects of neratinib, an irreversible panHER inhibitor, in a panel of 36 breast cancer cell lines. We further assessed its effects with or without trastuzumab in several sensitive and resistant breast cancer cells as well as a BT474 xenograft model. We confirmed that neratinib was significantly more active in HER2-amplified than HER2 non-amplified cell lines. Neratinib decreased the activation of the 4 HER receptors and inhibited downstream pathways. However, HER3 and Akt were reactivated at 24 hours, which was prevented by the combination of trastuzumab and neratinib. Neratinib also decreased pHER2 and pHER3 in acquired trastuzumab resistant cells. Neratinib in combination with trastuzumab had a greater growth inhibitory effect than either drug alone in 4 HER2 positive cell lines. Furthermore, trastuzumab in combination with neratinib was growth inhibitory in SKBR3 and BT474 cells which had acquired resistance to trastuzumab as well as in a BT474 xenograft model. Innately trastuzumab resistant cell lines showed sensitivity to neratinib, but the combination did not enhance response compared to neratinib alone. Levels of HER2 and phospho-HER2 showed a direct correlation with sensitivity to neratinib. Our data indicate that neratinib is an effective anti-HER2 therapy and counteracted both innate and acquired trastuzumab resistance in HER2 positive breast cancer. Our results suggest that combined treatment with trastuzumab and neratinib is likely to be more effective than either treatment alone for both trastuzumab-sensitive breast cancer as well as HER2-positive tumors with acquired resistance to trastuzumab.

Abstract:The development of acquired resistance to small molecule tyrosine kinase inhibitors (TKIs) targeting epidermal growth factor receptor (EGFR) signaling has hindered their efficacy in treating non-small cell lung cancer (NSCLC) patients. Our previous study showed that constitutive activation of the 70 kDa ribosomal protein S6 kinase 1 (S6K1) contributes to the acquired resistance to EGFR-TKIs in NSCLC cell lines and xenograft tumors in nude mice. However, the regulatory mechanisms underlying S6K1 constitutive activation in TKI-resistant cancer cells have not yet been explored. In this study, we recapitulated this finding by taking advantage of a gefitinib-resistant patient-derived xenograft (PDX) model established through a number of passages in mice treated with increasing doses of gefitinib. The dissociated primary cells from the resistant PDX tumors (PDX-R) displayed higher levels of phosphor-S6K1 expression and were resistant to gefitinib compared to cells from passage-matched parental PDX tumors (PDX-P). Both genetic and pharmacological inhibition of S6K1 increased sensitivity to gefitinib in PDX-R cells. In addition, both total and phosphorylated mechanistic target of rapamycin kinase (MTOR) levels were upregulated in PDX-R and gefitinib-resistant PC9G cells. Knockdown of MTOR by siRNA decreased the expression levels of total and phosphor-S6K1 and increased sensitivity to gefitinib in PDX-R and PC9G cells. Moreover, a transcription factor ELK1, which has multiple predicted binding sites on the MTOR promoter, was also upregulated in PDX-R and PC9G cells, while the knockdown of ELK1 led to decreased expression of MTOR and S6K1. The chromatin immunoprecipitation (ChIP)-PCR assay showed the direct binding between ELK1 and the MTOR promoter, and the luciferase reporter assay further indicated that ELK1 could upregulate MTOR expression through tuning up its transcription. Silencing ELK1 via siRNA transfection improved the efficacy of gefitinib in PDX-R and PC9G cells. These results support the notion that activation of ELK1/MTOR/S6K1 signaling contributes to acquired resistance to gefitinib in NSCLC. The findings in this study shed new light on the mechanism for acquired EGFR-TKI resistance and provide potential novel strategies by targeting the ELK1/MTOR/S6K1 pathway.
