Abstract:Objectives: This study aimed to analyse trends in the prevalence and economic burden of hypertension and its socioeconomic disparities in rural southwestern China.Design: Two repeated cross-sectional studies were conducted to collect interview and health examination data among individuals aged >= 35 years in rural Yunnan Province, China.Setting: Three rural areas of Yunnan, China.Participants: We invited 8187 consenting participants in 2010-2011 and 7572 consenting participants in 2020-2021 to undergo interviews and health examinations.Results: The standardised prevalence of hypertension significantly increased from 26.1% in 2011 to 40.4% in 2021 (p<0.01), and the per capita direct, indirect and disease economic burdens increased from US$1323, US$46 and US$1369 to US$2196, US$49 and US$2244, respectively. In addition to the indirect economic burden, the direct and disease economic burdens increased significantly. The prevalence of hypertension was higher in illiterate population, among participants with low annual household income per capita, and participants with good access to medical services than in their counterparts who had good education, high annual household income per capita and poor access to medical services (all p<0.05). Moreover, the prevalence of hypertension showed a downward trend with improvement in socioeconomic position (SEP) (p<0.05). The per capita direct and disease economic burdens increased most in participants with low SEP, but the per capita indirect economic burden increased most in participants with upper-middle SEP.Conclusions: The prevalence and economic burden of hypertension have visibly accelerated in rural Yunnan Province over the 10 years studied, and socioeconomic disparities have been found in the prevalence and economic burden of hypertension. These findings highlight that socioeconomic differentials should be tailored to address the timing of effective interventions for hypertension prevention and control and reduce the economic burden of hypertension in rural southwestern China.

Abstract:This study investigates socioeconomic differences in prevalence, awareness, control and self-management of hypertension in rural China. A cross-sectional survey was conducted among four ethnic minority groups in Yunnan Province: Na Xi, Li Shu, Dai and Jing Po. Approximately 5532 consenting individuals aged ⩾35 years (48.4% of whom were male) were selected to participate in the study using a stratified, multistage sampling technique. Information about participants' demographic characteristics and hypertension awareness, treatment, control and self-management practices was obtained using a standard questionnaire. The age-standardised prevalence of hypertension in the study population was 33.6%. In hypertensive subjects, the overall levels of awareness, treatment and control of hypertension were 42.1%, 28.5% and 6.7%, respectively. Approximately 58.7% of hypertensive patients regularly self-monitored blood pressure (BP), 64.7% adhered to their physician-prescribed anti-hypertensive drugs, and 88.0% took at least one measure to control BP. Hypertensive patients of Jing Po ethnicity had the lowest rates of awareness, treatment, control and self-management of hypertension among the four ethnic minority groups studied. Individuals with lower levels of education were more likely to be hypertensive. Further, individuals with lower levels of education had a lower probability of awareness of their hypertensive status and of treatment with antihypertensive medication. Access to medical services was positively associated with awareness of suffering from hypertension, being treated with antihypertensive medication, and compliance with antihypertensive drug treatment. This study suggests that effective strategies to enhance awareness, treatment and management of hypertension should focus on individuals with low levels of education and poor access to medical services.
Epidemiology of Hypertension and Associated with Risk Factors in Rural Areas in Changchun of China

Abstract:Backgroud and aim: To evaluate the prevalence, awareness, treatment and control of hypertension and risk factors in rural areas in Changchun of China. Methods: A multistage cluster random sampling method was used to investigate the incidence of hypertensionin. Each participant underwent a standard questionare, biochemical tests and physical examinations. Logistic regression analysis was used to identify risk factors of hypertension. Results: Of 1603 (male=658) subjects,the overall prevalence of hypertension is 36.31% in rural area in Changchun of China, the awareness, treatment and the control is 20.96%, 13.75%, 1.55%, respectively. For male, the prevalence, awareness, treatment and control is, respectively, 17.59%, 7.22%, 5.67% and 0.69%; And for female is 18.71%, 13.75%, 8.08% and 0.86%. People with high-salt diet has a higher prevalence (P<0.001) than people with low-salt diet; And there are no significance for people with more meat, vegetables, sweet diet, current smoke or between white-collar and blue-collar. In a binary logistic regression, old age, drink, high-salt diet, central obesity, family history of hypertension and dyslipidemia were associated with increased odds of the hypertension, and the odd ratio (95% confidence interval [CI]) is 1.06 (95% CI:1.05-1.07), 1.55 (95% CI:1.18-2.03), 7.27 (95% CI:4.97-10.62), 1.98 (95% CI:1.54-2.55), 2.12 (95% CI:1.48-3.05) and 1.68 (95% CI:1.32-2.13), respectively, but no significance differernce with physical activity, monthly income, high carbohydrate intake, DM and education. Conclusion: The prevalence of hypertension is lower in rural area in Changchun than urban, but higher than southern rural areas in China, and the awareness, treatment especially control is lower than the other areas.

Abstract:This study aims to investigate the trends in prevalence, awareness, treatment and control of hypertension among rural residents aged 35–74 years in northern China during the country’s rapid economic development from 1991 to 2011. Two surveys, conducted in 1991and 2011, included 2196 and 1939 participants aged 35–74 years from same villages in Ji County, Tianjin of China, respectively. The prevalence of hypertension, adjusted by age and gender using the world standard population in 2000, increased 30% (39.9% vs 51.7%) between 1991 and 2011. The increase was greatest (68%) in women aged 35–44 years. Meanwhile, the prevalence of stage II hypertension increased by 75% overall, with a 4-fold increase in men aged 45–54 years. Although the awareness, treatment and control of hypertension increased significantly during the same period, they remained unacceptably poor. In conclusion, the community-based surveys showed that the prevalence of hypertension in rural residents of northern China aged 35–74 years increased rapidly over the past 20 years, and most dramatically in young women. Efforts in the primary prevention of hypertension, particularly for young women, and promoting education for hypertension awareness, treatment and control are of paramount importance in rural China.

Abstract:BACKGROUND:The aim of this study was to investigate trends in the distribution, prevalence, awareness, treatment, and control of hypertension in a Chinese population that has had the fastest growing gross domestic product in the world over the past 3 decades.METHODS:Four standardized cross-sectional health surveys were conducted between 2002 and 2010 in a population consisting of 85 million residents in Guangdong Province. Multistage cluster sampling was adopted to recruit representative samples. The data were obtained through on-site health examinations and face-to-face interviews.RESULTS:The aging trend in this population was not significant (P = 0.17) during the survey period, whereas body mass index/waist circumference increased significantly (P = 0.047 for body mass index and P < 0.001 for waist circumference). The age-standardized prevalence of hypertension increased from 10.5% to 13.3%, averaging a 0.35% increase per year. A higher risk was observed in younger residents over the survey period. Awareness and treatment increased by 22.0% and 19.0%, respectively, in the rural areas, whereas there was no significant change in the urban area (the corresponding figures were 1.8% and -3.1%, respectively). There was no improvement in hypertension control (the age-standardized control prevalence in 2002 was 50.3%, whereas it was 43.2% in 2010).CONCLUSIONS:The prevalence of hypertension increased slightly in this population with the fastest economic development. This increase mirrored the trend of increasing obesity. Awareness and treatment of hypertension have improved in the rural areas, although there were no significant changes in the urban areas. The prevalence of awareness and treatment remains at unacceptably low levels, suggesting that urgent and aggressive strategies are necessary to improve hypertension control and intervention.

Abstract:ObjectiveTo assess the recent trends in prevalence and management of hypertension in China, nationally and by population subgroups.DesignSix rounds of a national survey, China.SettingChina Chronic Disease and Risk Factors Surveillance, 2004-18.Participants642 523 community dwelling adults aged 18-69 years (30 501 in 2004, 47 353 in 2007, 90 491 in 2010, 156 836 in 2013, 162 293 in 2015, and 155 049 in 2018).Main outcome measuresHypertension was defined as a blood pressure of ≥140/90 mm Hg or taking antihypertensive drugs. The main outcome measures were hypertension prevalence and proportion of people with hypertension who were aware of their hypertension, who were treated for hypertension, and whose blood pressure was controlled below 140/90 mm Hg.ResultsThe standardised prevalence of hypertension in adults aged 18-69 years in China increased from 20.8% (95% confidence interval 19.0% to 22.5%) in 2004 to 29.6% (27.8% to 31.3%) in 2010, then decreased to 24.7% (23.2% to 26.1%) in 2018. During 2010-18, the absolute annual decline in prevalence of hypertension among women was more than twice that among men (−0.83 percentage points (95% confidence interval −1.13 to −0.52) v −0.40 percentage points (−0.73 to −0.07)). Despite modest improvements in the awareness, treatment, and control of hypertension since 2004, rates remained low in 2018, at 38.3% (36.3% to 40.4%), 34.6% (32.6% to 36.7%), and 12.0% (10.6% to 13.4%). Of 274 million (95% confidence interval 238 to 311 million) adults aged 18-69 years with hypertension in 2018, control was inadequate in an estimated 240 million (215 to 264 million). Across all surveys, women with low educational attainment had higher prevalence of hypertension than those with higher education, but the finding was mixed for men. The gap in hypertension control between urban and rural areas persisted, despite larger improvements in diagnosis and control in rural than in urban areas.ConclusionsThe prevalence of hypertension in China has slightly declined since 2010, but treatment and control remain low. The findings highlight the need for improving detection and treatment of hypertension through the strengthening of primary care in China, especially in rural areas.

Abstract:Objectives. Previous studies reported that there were disparities in hypertension management among different ethnic groups, and this study aimed to systematically determine the prevalence, awareness, treatment, and control rates of hypertension in multiple Chinese ethnic groups. Methods. We searched Embase, PubMed, and Web of Science for articles up to 25 October, 2022. The pooled prevalence, awareness, treatment, and control rates of hypertension were estimated with 95% confidence intervals (CI). The heterogeneity of estimates among studies was assessed by the Cochran Q test and I2 statistic. Meta-regression analyses were conducted to identify the factors influencing the heterogeneity of the pooled prevalence, awareness, treatment, and control rate of hypertension. Results. In total, 45 publications including 193,788 cases and 587,826 subjects were eligible for the analyses. The lowest prevalence was found in the Han group (27.0%), and the highest prevalence was in the Mongolian population (39.8%). The awareness rates ranged from 24.4% to 58.0% in the four ethnic groups. Both the highest treatment and control rates were found in the Mongolian population (50.6% and 16.0%, respectively), whereas the Yi group had the lowest control rate (8.0%). In addition, the study year, the mean age of subjects, mean body mass index of subjects, tobacco use (%), alcohol use (%), residence (urban%), and education (primary school%) had varied effects on heterogeneity. Conclusions. These findings highlight the disparities in prevalence, awareness, treatment, and control rates of hypertension in a different ethnic population of China, which could provide suggestions for making targeted prevention measures.

Trends in Population Blood Pressure and Prevalence, Awareness, Treatment, and Control of Hypertension among Middle-Aged and Older Adults in a Rural Area of Northwest China from 1982 to 2010.

Abstract:OBJECTIVES: To assess trends in average blood pressure levels and prevalence, awareness, treatment, and control of hypertension among adults in a rural area of Northwest China, and to determine associated risk factors. METHODS: Four cross-sectional population-based surveys were conducted between 1982 and 2010 among randomly selected adults in rural areas of Hanzhong, in Northwest China. Data on blood pressure, body mass index, family history of hypertension, and socio-demographic and lifestyle characteristics were collected in similar way by trained investigators in four surveys. Data of 8575 participants aged 35-64 years was analyzed. Averages and proportions were adjusted for age and sex. RESULTS: Average blood pressure in the population has increased since 1982 from 76.9 mm Hg to 79.6 mm Hg in 2010 (diastolic) and from 120.9 to 129.7 mm Hg (systolic). Prevalence of hypertension increased from 18.4% in 1982 to 30.5% in 2010, and awareness of hypertension increased from 16.8% to 38.4% in 2010. Treatment of hypertension increased from 1.0% in 1982 to 17.4% in 2010, and control of hypertension increased from 0.1% in 1982 to 3.5% in 2010. All these gradients were statistically significant (P<0.01 for trend). Population blood pressure and prevalence, awareness and treatment of hypertension were positively associated with increasing age, body mass index and having family history of hypertension. CONCLUSIONS: Average blood pressure levels and the prevalence, awareness, treatment and control of hypertension among adults in rural areas of Hanzhong have increased since 1982. However, awareness, treatment and control rates remain low. Public health programs and practical strategies are required to improve prevention and control of hypertension in rural Northwest China. In particular, attention should be given to the elderly and obese, and to those with a family history of hypertension, while raising awareness and treatment among younger adults.
[Effect of pulse wave velocity on coronary flow reserve in patients with essential hypertension].

Abstract:OBJECTIVE To observe the effect of pulse wave velocity (PWV) on coronary flow reserve (CFR) in patients with essential hypertension (EH). METHODS 108 patients with EH were divided into three groups according to the blood pressure (BP) status and antihypertensive drug therapy (ADT): group A (n = 41, with ADT and BP failing to reach the standard), group B (n = 27, with high BP and without ADT), and group C (n = 40, with ADT and BP reaching the standard). Twenty-five sex, age, body mass index, waist/hip ratio, heart rate, fasting glycemia and lipid level-matched healthy persons were used as controls (group D). PWV, CFR, left ventricle mass index (LVMI), E/A ratio, left ventricle isovolumetric relaxation time (IVRT), left ventricular ejection fraction (LVEF) were detected in each group. Correlation analysis was conducted. RESULTS (1) the PWV values in group A, B, and C were 12.7 m/s +/- 1.3 m/s, 11.9 m/s +/- 2.2 m/s, and 9.6 m/s +/- 0.9 m/s respectively, all significantly higher than that in the group D (8.9 m/s +/- 0.7 m/s, all P < 0.05) and with significant differences between group A and group C and between group B and group C (both P < 0.05). The CFR values in group A, B, and C were 2.56 +/- 0.18, 2.54 +/- 0.19, and 2.97 +/- 0.19 respectively, all significantly lower than that in group D (3.26 +/- 0.26, all P < 0.05), with significant differences between group A and group C and between group B and group C (both P < 0.05). The values of average peak velocity of baseline (APVb) in groups A, B, and C were 38 cm/s +/- 4 cm/s, 36 cm/s +/- 4 cm/s, and 34 cm/s +/- 4 cm/s respectively, significantly higher than that in group D (29 cm/s +/- 4 cm/s, all P < 0.05), without significant difference among group A, B, and C. (2) The values of LVMI in group A, B, and C were 117 g/m(2) +/- 8 g/m(2), 113 g/m(2) +/- 11 g/m(2), and 98 g/m(2) +/- 11 g/m(2) respectively, all significantly higher than that of group D (88 g/m(2) +/- 7 g/m(2), all P < 0.05), with significant differences between group A and group C and between group B and group C. The E/A ratios in groups A and B were 0.76 +/- 0.19 and 0.87 +/- 0.18 respectively, significantly lower than those in group C and D (1.08 +/- 0.18 and 1.19 +/- 0.15 respectively, all P < 0.05). The values of IVRT in group A, B and C were 134 ms +/- 11 ms, 138 ms +/- 11 ms, and 109 ms +/- 12 ms respectively, all significantly higher than that in group D (84 ms +/- 12 ms, all P < 0.05) with a significant difference between group A and group C and between group B and group C (both P < 0.05). There was no significant difference of LVEF among different groups. (3) Stepwise linear regression of PWV showed that the factors significantly influencing PWV were age, LVMI, E/A ratio and SBP, the factors significantly influencing CFR were LVMI and PWV, and PWV was significant negatively correlated to CFR and significant positively correlated to APVb. CONCLUSION PWV is increased in hypertensive patients. The increase of LVMI and increase of PWV contribute to the decrease of CFR in hypertensive patients. Increase of PWV that attributes to left ventricular hypertrophy and decrease of E/A ratio in patients with hypertension leads to further decrease of CFR. Decrease of CFR in hypertensive patients may cause cardiovascular diseases. Early detection and prevention of increased PWV may decrease the morbidity of cardiovascular diseases.
Prevalence, awareness, medication, control, and risk factors associated with hypertension in Yi ethnic group aged 50 years and over in rural China: the Yunnan minority eye study

Abstract:BackgroundHypertension is an important public health issue in China, but there are few studies examining hypertension in ethnic groups in Yunnan, China. This study, Yunnan Minority Eye Study (YMES), was initially designed to determine the prevalence and impact of eye diseases, including hypertension and diabetes mellitus. As a part of YMES, the prevalence, awareness, treatment, and control of hypertension and the associated risk factors among the Yi ethnic population in rural China are reported.MethodsA population-based survey was conducted in 2012 with adult participants over 50 from rural communities in Shilin Yi Autonomous County, Yunnan Province, located in southwest China. A random cluster sampling method was used to select a representative sample. The participants’ blood pressure, height, weight, and waist circumference were measured. Hypertension was defined as mean systolic blood pressure ≥140 mmHg and/or diastolic blood pressure ≥90 mmHg, and/or current use of antihypertensive medications.ResultsA total of 2208 adults were assessed. The prevalence of hypertension was 38.5%, and the age- and gender-adjusted prevalence was 37.0%. The proportion of patients who were aware of their hypertension among those diagnosed with hypertension was 24.8%. Of those aware of having hypertension, 23.6% took antihypertensive drugs. Among all hypertensive patients, only 7.2% had controlled their hypertension (<140/90 mmHg). Risk factors for hypertension were older age, smoking, alcohol consumption, family history of high blood pressure, overweight, and obesity. Protective factors included being slim and higher education.ConclusionsHypertension was highly prevalent among this population of the Yi ethnic group in China. The ratio of awareness, treatment, and control of hypertension were considerately low. Hypertension education and screening programs in rural China are recommended to improve the health status of this population.

Abstract:RUBELLA virus has been demonstrated to persist in the nasopharynx of children with congenital defects for at least a number of months. 1,2 These children also actively produce rubella neutralizing antibody. 3,4 The virus has been isolated from many tissues of infants dying with congenital rubella at 1 to 3 months of age. 5 Several recent studies have shown that the infected children are also infectious and pose a hazard to personnel working with these patients. 6,7 The present study was conducted to determine if virus persists in the nasopharynx of older children and young adults with rubella syndrome defects, and if neutralizing or complement fixing antibody is present in the sera of these patients. Abstract:Abstract Background The prevalence of hypertension is high and rising in China, but most people with hypertension do not have their blood pressure under control. This study investigated hypertension awareness, treatment, and control and their associated factors among Chinese adults. Methods Data collected from the 2011 China Health and Nutrition Survey (CHNS) from 12,991 Chinese adults were used. Hypertension was defined as systolic blood pressure ≥ 140 mmHg, diastolic blood pressure ≥ 90 mmHg, self-reported prior diagnosed hypertension, or taking antihypertensive medications. Hypertension awareness, treatment, and control were defined as a self-reported diagnosis of hypertension, current use of antihypertensive medication, and blood pressure < 140/90 mmHg, respectively. Multivariate logistic regression was performed to examine factors associated with hypertension awareness, treatment, and control. Results Overall, 3579 (27.6%) of the CHNS adult participants had hypertension, of whom 55.7% were aware of their diagnosis, 46.5% were treated with antihypertensive medications, but only 20.3% had their blood pressure under control. Higher hypertension treatment was associated with older age (OR = 2.57; 95%CI, 1.65–4.02), urban residency (1.50; 1.14–1.97), living in the Eastern region (1.52; 1.14–2.01), and being overweight/obese (1.99; 1.39–2.84). Hypertension awareness was associated with similar factors as hypertension treatment but was also associated with being female (1.37, 1.12–1.66). Poor hypertension control was associated with being overweight/obese (0.56; 0.42–0.76) and minority ethnicity (0.52; 0.31–0.86). Conclusion Hypertension is a major public health challenge in China. The prevalence of hypertension awareness, treatment, and control are still low despite existing public health policies and programs to reduce the burden of hypertension. More intensive hypertension screening and treatment programs are warranted in China.

Abstract:The aim of this study is to evaluate the developments in the treatment and prevalence of hypertension by demographic subgroups in least developed area of China in 2012 and 2022. This population‐based cross‐sectional study was conducted in 2012 and 2022, we applied stratified multistage random sampling to investigate residents aged 18 years or older in Gansu, the least developed province in the northwest of China. Questionnaires and anthropometric measurements were given to all respondents. The standardized prevalence of hypertension in adults in Gansu increased from 26.1% in 2012 to 28.8% in 2022. Compared with 2012, the control rate remains decreased despite the significantly improved awareness and treatment rates of hypertension in 2022. Apart from the reversal of the control rate, the trend of higher prevalence in men and higher awareness and treatment rates in women has not changed. There was an obviously increase in the proportion of participants who had received health education and hypertension management services from medical workers. The treatment was still primarily monotherapy, and there was no significant improvement in the prescription of medication. The prevalence of hypertension has increased mildly in the least developed region of China over the past decade, and the challenge of hypertension management has shifted from increasing awareness and treatment rates to increasing control rates. The onset and control of hypertension are affected by education methods, BMI, local economic conditions and other factors, and targeted strategies can be adopted to strengthen the management of hypertension in economically underdeveloped areas of China.

Abstract:BACKGROUND Hypertension is the most important risk factor for cardiovascular diseases. Little information exists on the status of hypertension among southern Chinese. We therefore investigated the hypertension prevalence, awareness, treatment, control, and associated factors in a southern Chinese population with 85 million residents. METHODS Stratified multistage cluster sampling with probability proportional to size method was used in this survey. A representative sample of 13,889 residents aged 20 years or above with completed questionnaire and blood pressure (BP) measurement was obtained. BP was measured in accordance with the 1999 World Health Organization/International Society of Hypertension Guidelines. Information related to history of diagnosis and treatment of hypertension was collected through questionnaire. RESULTS The prevalence of hypertension in this population was 20.5% (16.5%, 24.4%), which translated to 9.8 million adults suffering from hypertension in Guangdong province. The urban population had higher prevalence of hypertension than the rural population (25.1 vs. 16.1%). The prevalence of awareness, treatment, and control of hypertension in hypertensive patients living in urban regions were 42.8, 37.9, and 13.5%, respectively, which were higher than those in rural regions (the corresponding figures were 17.6, 10.4, and 3.4%, respectively). Nearly 50% urban adults and 80% rural adults did not measure their BP in the last 12 months. Frequency of BP measurement was associated with both awareness and treatment. CONCLUSIONS Hypertension was prevalent in southern China. The prevalence of awareness, treatment, and control of hypertension is low. Urgent strategies are needed to improve prevention, detection, and treatment of hypertension in this large Chinese population.

Abstract:We describe here 20 families of secondary (pmf‐driven) carriers which, in addition to nine families within the ATP‐dependent ABC superfamily, and seven families of Gram‐negative bacterial outer membrane porins, largely account for the stereospecific transport of sugars and their derivatives into and out of all living cells on earth. Family characteristics as well as struc‐tural and functional properties of the family constituents are described. By reference to our website (http://www‐biology.ucsd.edu/~msaier/transport/), phylogenetic relationships, detailed substrate specificity information and both primary and secondary references are also available. This review provides a comprehensive guide to the diversity of carriers that mediate the transport of sugar‐containing molecules across cell and organellar membranes.
Effect of Socioeconomic Status on Prevalence of Hypertension in Chinese Adults

Abstract:Objective To explore the effects of Socio-economic Status(SES) on prevalence of hypertension in Chinese adults(not in school) and provide evidence for further improving the strategy of chronic disease prevention and control in China.Methods The data used in this study came from part of China Health and Nutrition Survey(CHNS) which was a cross-sectional survey conducted in 2006.A non-student sample aged 18 to 50 years was selected and SES was determined by income,education and occupation.Binary logistic and multinomial logistic regression model were used to analysis the difference of prevalence and risk factors of hypertension among different Socioeconomic Status,respectively.Results After adjusted by age,gender and marital status,the risk for hypertension in the lowest SES group was 68.6% higher than that in the highest SES group(OR=1.686,95%CI:1.013～2.807).The distribution rate of high-salt diet and moderate & over drinking were different between SES groups(χ2=124.365,P<0.001 and χ2=21.003,P=0.007).After risk factors for hypertension controlled,the risks for hypertension in the lowest and in lower SES group were 80.6% and 74.6%,respectively,than that in the highest SES group(OR=1.806,95%CI:1.080～3.154;OR=1.686,95%CI: 1.009～2.885).Conclusion The people with low SES have higher risk for hypertension than the people with high SES do.
Epidemiological study of awareness, treatment and control rates of hypertension among elderly rural residents in Rongcheng area, China

Abstract:Objective To explore the current status of the prevalence, awareness, medical treatment, and control rates of hypertension among the elderly residents from rural areas in Rongcheng area. Method Cross-sectional study was conducted among 2125 adults ( ≥ 50 years) randomly selected from 10 spots of Rongcheng by cluster sampling method and a questionnaire survey was carried out of hypertension. Datas were statistically analyzed. Result 1309 out of 2125 adults suffered from hypertension with the prevalence of 61.6% and 516 (24.3%) were newly diagnosed. The awareness, treatment and control rates of hypertension were 60.6%, 40.7% and 12.9%, respectively, and varied significantly among the elderly with different age and gender (P < 0.05). Conclusion The awareness, treatment and control rate of hypertension among the elderly from rural areas in Rongcheng have increased, but still relatively low especially the treatment and control rates. The intervention should be used to improve the awareness, treatment and control rates of hypertension in Rongcheng area.
Factors associated with prevalence, awareness, treatment and control of hypertension in urban adults from 33 communities in China: the CHPSNE Study

Abstract:Using a randomized multistage cluster sampling, we studied the factors associated with the prevalence, awareness, treatment and control of hypertension (defined as systolic and diastolic blood pressure (BP) measurements ⩾140 and 90 mm Hg, respectively, or current drug treatment for hypertension) in a representative sample of the urban Chinese population. The participants were 18–74 years of age and had lived for at least 5 years in an area comprising 33 communities in three cities (Shenyang, Anshan and Jinzhou) in China. A total of 28 830 people were selected; the overall response rate was 87.4% (25 196/28 830). The overall prevalence of hypertension was 28.7% (7237/25 196). Of those with hypertension, 42.9% (3107/7237) were aware of their condition, 28.2% (2042/7237) were receiving treatment and 3.7% (264/7237) were considered controlled. More than half (57.8%) of the hypertensives did not think that high BP would endanger their lives. Multivariate analysis revealed that age, gender, education, occupation, income, body mass index, waist circumference and a family history of hypertension correlated significantly with the prevalence of hypertension. Among all the hypertensives, higher awareness (determined by odds ratios; 95% confidence intervals) was noted for persons who were white-collar workers (1.29; 1.08, 1.53, respectively), overweight (1.48; 1.30, 1.69), obese (3.37; 2.76, 4.11) or had a family history of hypertension (3.07; 2.76, 3.42). Among the individuals aware of their hypertension, treatment was more common in those with a higher level of education and less common among individuals consuming ⩾2 alcoholic drinks per day (0.65; 0.52, 0.83). Controlled hypertension was much less common among older persons, and participants who were former smokers (0.49; 0.26, 0.91). The results indicate that more attention is needed to improve the awareness of the potentially fatal nature of hypertension in urban China.

Abstract:China has undergone a rapid epidemiological transition from infectious to chronic diseases, a process characterized by widespread under-diagnosis of chronic diseases and low rates of treatment and control. This paper uses hypertension as an example and documents the association of socioeconomic status with various measures of this condition, i.e., prevalence, awareness, treatment and control. We find no wealth and education gradients in the prevalence of hypertension. Given education, wealth plays some roles in improving the treatment and control of hypertension. Some associations exist between education and diagnosis/treatment/control in urban areas but not in rural areas. We also find that the public health care services in China contribute little in informing patients of their hypertension status, suggesting that how to improve the effectiveness of the health care system in dealing with emerging chronic illnesses should be policy priority.
