Abstract:ObjectiveTo assess the recent trends in prevalence and management of hypertension in China, nationally and by population subgroups.DesignSix rounds of a national survey, China.SettingChina Chronic Disease and Risk Factors Surveillance, 2004-18.Participants642 523 community dwelling adults aged 18-69 years (30 501 in 2004, 47 353 in 2007, 90 491 in 2010, 156 836 in 2013, 162 293 in 2015, and 155 049 in 2018).Main outcome measuresHypertension was defined as a blood pressure of ≥140/90 mm Hg or taking antihypertensive drugs. The main outcome measures were hypertension prevalence and proportion of people with hypertension who were aware of their hypertension, who were treated for hypertension, and whose blood pressure was controlled below 140/90 mm Hg.ResultsThe standardised prevalence of hypertension in adults aged 18-69 years in China increased from 20.8% (95% confidence interval 19.0% to 22.5%) in 2004 to 29.6% (27.8% to 31.3%) in 2010, then decreased to 24.7% (23.2% to 26.1%) in 2018. During 2010-18, the absolute annual decline in prevalence of hypertension among women was more than twice that among men (−0.83 percentage points (95% confidence interval −1.13 to −0.52) v −0.40 percentage points (−0.73 to −0.07)). Despite modest improvements in the awareness, treatment, and control of hypertension since 2004, rates remained low in 2018, at 38.3% (36.3% to 40.4%), 34.6% (32.6% to 36.7%), and 12.0% (10.6% to 13.4%). Of 274 million (95% confidence interval 238 to 311 million) adults aged 18-69 years with hypertension in 2018, control was inadequate in an estimated 240 million (215 to 264 million). Across all surveys, women with low educational attainment had higher prevalence of hypertension than those with higher education, but the finding was mixed for men. The gap in hypertension control between urban and rural areas persisted, despite larger improvements in diagnosis and control in rural than in urban areas.ConclusionsThe prevalence of hypertension in China has slightly declined since 2010, but treatment and control remain low. The findings highlight the need for improving detection and treatment of hypertension through the strengthening of primary care in China, especially in rural areas.

Abstract:RUBELLA virus has been demonstrated to persist in the nasopharynx of children with congenital defects for at least a number of months. 1,2 These children also actively produce rubella neutralizing antibody. 3,4 The virus has been isolated from many tissues of infants dying with congenital rubella at 1 to 3 months of age. 5 Several recent studies have shown that the infected children are also infectious and pose a hazard to personnel working with these patients. 6,7 The present study was conducted to determine if virus persists in the nasopharynx of older children and young adults with rubella syndrome defects, and if neutralizing or complement fixing antibody is present in the sera of these patients. Abstract:Abstract Background The prevalence of hypertension is high and rising in China, but most people with hypertension do not have their blood pressure under control. This study investigated hypertension awareness, treatment, and control and their associated factors among Chinese adults. Methods Data collected from the 2011 China Health and Nutrition Survey (CHNS) from 12,991 Chinese adults were used. Hypertension was defined as systolic blood pressure ≥ 140 mmHg, diastolic blood pressure ≥ 90 mmHg, self-reported prior diagnosed hypertension, or taking antihypertensive medications. Hypertension awareness, treatment, and control were defined as a self-reported diagnosis of hypertension, current use of antihypertensive medication, and blood pressure < 140/90 mmHg, respectively. Multivariate logistic regression was performed to examine factors associated with hypertension awareness, treatment, and control. Results Overall, 3579 (27.6%) of the CHNS adult participants had hypertension, of whom 55.7% were aware of their diagnosis, 46.5% were treated with antihypertensive medications, but only 20.3% had their blood pressure under control. Higher hypertension treatment was associated with older age (OR = 2.57; 95%CI, 1.65–4.02), urban residency (1.50; 1.14–1.97), living in the Eastern region (1.52; 1.14–2.01), and being overweight/obese (1.99; 1.39–2.84). Hypertension awareness was associated with similar factors as hypertension treatment but was also associated with being female (1.37, 1.12–1.66). Poor hypertension control was associated with being overweight/obese (0.56; 0.42–0.76) and minority ethnicity (0.52; 0.31–0.86). Conclusion Hypertension is a major public health challenge in China. The prevalence of hypertension awareness, treatment, and control are still low despite existing public health policies and programs to reduce the burden of hypertension. More intensive hypertension screening and treatment programs are warranted in China.

Abstract:This study aims to investigate the trends in prevalence, awareness, treatment and control of hypertension among rural residents aged 35–74 years in northern China during the country’s rapid economic development from 1991 to 2011. Two surveys, conducted in 1991and 2011, included 2196 and 1939 participants aged 35–74 years from same villages in Ji County, Tianjin of China, respectively. The prevalence of hypertension, adjusted by age and gender using the world standard population in 2000, increased 30% (39.9% vs 51.7%) between 1991 and 2011. The increase was greatest (68%) in women aged 35–44 years. Meanwhile, the prevalence of stage II hypertension increased by 75% overall, with a 4-fold increase in men aged 45–54 years. Although the awareness, treatment and control of hypertension increased significantly during the same period, they remained unacceptably poor. In conclusion, the community-based surveys showed that the prevalence of hypertension in rural residents of northern China aged 35–74 years increased rapidly over the past 20 years, and most dramatically in young women. Efforts in the primary prevention of hypertension, particularly for young women, and promoting education for hypertension awareness, treatment and control are of paramount importance in rural China.

Abstract:Aims: The present study aimed to assess the prevalence of hypertension among Chinese adults.Methods: Data were obtained from sphygmomanometer measurements and a questionnaire administered to 46239 Chinese adults >= 20 years of age who participated in the 2007-2008 China National Diabetes and Metabolic Disorders Study. Hypertension was defined as blood pressure >= 140/90 mm Hg or use of antihypertensive medication.Results: A total of 26.6% of Chinese adults had hypertension, and a significantly greater number of men were hypertensive than women (29.2% vs 24.1%, p<0.001). The age-specific prevalence of hypertension was 13.0%, 36.7%, and 56.5% among persons aged 20 to 44 years (young people), 45 to 64 years (middle-aged people), and >= 65 years (elderly people), respectively. In economically developed regions, the prevalence of hypertension was significantly higher among rural residents than among urban residents (31.3% vs 29.2%, p = 0.001). Among women or individuals who lived in the northern region, the disparity in the prevalence of hypertension between urban and rural areas disappeared (women: 24.0% vs. 24.0%, p = 0.942; northern region: 31.6% vs. 31.2%, p = 0.505). Among hypertensive patients, 45.0% were aware of their condition, 36.2% were treated, and 11.1% were adequately controlled.Conclusions: The prevalence of hypertension in China is increasing. The trend of an increase in prevalence is striking in young people and rural populations. Hypertension awareness, treatment, and control are poor. Public health efforts for further improving awareness and enhancing effective control are urgently needed in China, especially in emerging populations.

Abstract:OBJECTIVES: To assess trends in average blood pressure levels and prevalence, awareness, treatment, and control of hypertension among adults in a rural area of Northwest China, and to determine associated risk factors. METHODS: Four cross-sectional population-based surveys were conducted between 1982 and 2010 among randomly selected adults in rural areas of Hanzhong, in Northwest China. Data on blood pressure, body mass index, family history of hypertension, and socio-demographic and lifestyle characteristics were collected in similar way by trained investigators in four surveys. Data of 8575 participants aged 35-64 years was analyzed. Averages and proportions were adjusted for age and sex. RESULTS: Average blood pressure in the population has increased since 1982 from 76.9 mm Hg to 79.6 mm Hg in 2010 (diastolic) and from 120.9 to 129.7 mm Hg (systolic). Prevalence of hypertension increased from 18.4% in 1982 to 30.5% in 2010, and awareness of hypertension increased from 16.8% to 38.4% in 2010. Treatment of hypertension increased from 1.0% in 1982 to 17.4% in 2010, and control of hypertension increased from 0.1% in 1982 to 3.5% in 2010. All these gradients were statistically significant (P<0.01 for trend). Population blood pressure and prevalence, awareness and treatment of hypertension were positively associated with increasing age, body mass index and having family history of hypertension. CONCLUSIONS: Average blood pressure levels and the prevalence, awareness, treatment and control of hypertension among adults in rural areas of Hanzhong have increased since 1982. However, awareness, treatment and control rates remain low. Public health programs and practical strategies are required to improve prevention and control of hypertension in rural Northwest China. In particular, attention should be given to the elderly and obese, and to those with a family history of hypertension, while raising awareness and treatment among younger adults.

Abstract:BACKGROUND:The aim of this study was to investigate trends in the distribution, prevalence, awareness, treatment, and control of hypertension in a Chinese population that has had the fastest growing gross domestic product in the world over the past 3 decades.METHODS:Four standardized cross-sectional health surveys were conducted between 2002 and 2010 in a population consisting of 85 million residents in Guangdong Province. Multistage cluster sampling was adopted to recruit representative samples. The data were obtained through on-site health examinations and face-to-face interviews.RESULTS:The aging trend in this population was not significant (P = 0.17) during the survey period, whereas body mass index/waist circumference increased significantly (P = 0.047 for body mass index and P < 0.001 for waist circumference). The age-standardized prevalence of hypertension increased from 10.5% to 13.3%, averaging a 0.35% increase per year. A higher risk was observed in younger residents over the survey period. Awareness and treatment increased by 22.0% and 19.0%, respectively, in the rural areas, whereas there was no significant change in the urban area (the corresponding figures were 1.8% and -3.1%, respectively). There was no improvement in hypertension control (the age-standardized control prevalence in 2002 was 50.3%, whereas it was 43.2% in 2010).CONCLUSIONS:The prevalence of hypertension increased slightly in this population with the fastest economic development. This increase mirrored the trend of increasing obesity. Awareness and treatment of hypertension have improved in the rural areas, although there were no significant changes in the urban areas. The prevalence of awareness and treatment remains at unacceptably low levels, suggesting that urgent and aggressive strategies are necessary to improve hypertension control and intervention.

Abstract:Background: Although the prevalence of hypertension (HTN) continues to increase in developing countries, including China, recent data are lacking. A nationwide survey was conducted from October 2012 to December 2015 to assess the prevalence of HTN in China. Methods: A stratified multistage random sampling method was used to obtain a nationally representative sample of 451 755 residents ≥18 years of age from 31 provinces in mainland China from October 2012 to December 2015. Blood pressure (BP) was measured after resting for 5 minutes by trained staff using a validated oscillometric BP monitor. HTN was defined as systolic BP (SBP) ≥140 mm Hg/or diastolic BP (DBP) ≥90 mm Hg or use of antihypertensive medication within 2 weeks. Pre-HTN was defined as SBP 120 to 139 mm Hg and DBP 80 to 89 mm Hg without antihypertensive medication. HTN control was defined as SBP <140 mm Hg and DBP<90 mm Hg. In addition, the prevalence of HTN (SBP ≥130 or DBP ≥80 mm Hg) and control rate (SBP <130 and DBP <80 mm Hg) of HTN were also estimated according to the 2017 American College of Cardiology/American Heart Association High Blood Pressure Guideline. Results: Overall, 23.2% (≈244.5 million) of the Chinese adult population ≥18 years of age had HTN, and another 41.3% (≈435.3 million) had pre-HTN according to the Chinese guideline. There were no significant differences of HTN prevalence between urban and rural residents (23.4% versus 23.1%, P=0.819). Among individuals with HTN, 46.9% were aware of their condition, 40.7% were taking prescribed antihypertensive medications, and 15.3% had controlled HTN. Calcium channel blockers were the most commonly used antihypertensive medication (46.5%) as monotherapy, and 31.7% of treated hypertensive patients used ≥2 medications. The prevalence of HTN based on the 2017 American College of Cardiology/American Heart Association guideline was twice as high as that based on 2010 Chinese guideline (46.4%), whereas the control rate fell to 3.0%. Conclusions: In China, there is a high prevalence of HTN and pre-HTN, and awareness, treatment, and control of HTN were low. Management of medical therapy for HTN needs to improve.

Abstract:Background-The present article aims to provide accurate estimates of the prevalence, awareness, treatment, and control of hypertension in adults in China.Methods and Results-Data were obtained from sphygmomanometer measurements and an administered questionnaire from 141 892 Chinese adults >= 18 years of age who participated in the 2002 China National Nutrition and Health Survey. In 2002, approximate to 153 million Chinese adults were hypertensive. The prevalence was higher among men than women (20% versus 17%; P<0.001) and was higher in successive age groups. Overall, the prevalence of hypertension was higher in urban compared with rural areas in men (23% versus 18%; P<0.01) and women (18% versus 16%; P<0.001). Of the 24% affected individuals who were aware of their condition, 78% were treated and 19% were adequately controlled. Despite evidence to suggest improved levels of treatment in individuals with hypertension over the past decade, compared with estimates from 1991, the ratio of controlled to treated hypertension has remained largely unchanged at 1:4.Conclusions-One in 6 Chinese adults is hypertensive, but only one quarter are aware of their condition. Despite increased rates of blood pressure-lowering treatment, few have their hypertension effectively controlled. National hypertension programs must focus on improving awareness in the wider community, as well as treatment and control, to prevent many tens of thousands of cardiovascular-related deaths. Abstract:The definition, recommended treatment thresholds and targets of hypertension have been updated in the recent 2017 American College of Cardiology/American Heart Association (ACC/AHA) Guideline for the Prevention, Detection, Evaluation and Management of High Blood Pressure in Adults. However, the impacts of this guideline on Chinese population are currently unknown. In this study, we aim to provide updated data in China using criteria from the updated guideline. A multistage, stratified sampling method was used to obtain a representative sample of 50,171 adults aged 18 years and above. Hypertension was defined as an average systolic blood pressure ≥130 mm Hg, or an average diastolic blood pressure ≥80 mm Hg, or self-reported use of antihypertensive medications in past 2 weeks. The weighted prevalence of hypertension was 60.1% (95% confidence interval [CI] 59.3%-61.0%). The treatment rate and recommended treatment rate were 16.8% (95% CI 16.1%-17.6%) and 53.7% (95% CI 52.6%-54.9%), respectively; the gap was more prominent among men and participants from rural areas. Hence, the adoption of the 2017 ACC/AHA guideline will lead to a substantial increase in both prevalence and number of patients needing treatment in China. The applicability of this guideline should be carefully evaluated based on evidence from Chinese population.
Prevalence and characteristics of hypertension in mainland Chinese adults over decades: a systematic review

Abstract:Hypertension is considered to be the leading risk factor for cardiovascular mortality and accounts for a large proportion of premature deaths in China. An empirical understanding of the pattern of disease burden, such as reliable information about the trend and prevalence of hypertension, is essential in developing effective prevention and control strategies and is also important in the decision-making and planning processes of health. However, there are limited comprehensive reviews or nationwide studies that reveal the current burden of hypertension in China. This review is to systematically evaluate hypertension prevalence and determinants as well as its awareness, treatment and control over decades in China. A systematic search was performed for epidemiological studies on hypertension, published between 1982 and January 2013. MeSH major topic terms ‘hypertension’ and ‘prevalence’ and ‘China’ were used in the search. Data on hypertension prevalence, determinants and awareness, as well as its treatment and control, were extracted and compared. Sixty articles were included for data extraction. The prevalence reported in the studies varied from 7.17 to 51.2%. Awareness, treatment and control rates also varied regionally. Except in the central region of China, where a declining trend in hypertension prevalence was witnessed, a rising trend in hypertension prevalence over time could still be observed in other parts of China. Higher hypertension prevalence was found among men and in the urban population, whereas lower prevalence was found in the southern region. Hypertension awareness and treatment rate varied extensively, with a low control rate, nationwide.

[Effect of pulse wave velocity on coronary flow reserve in patients with essential hypertension].

Abstract:OBJECTIVE To observe the effect of pulse wave velocity (PWV) on coronary flow reserve (CFR) in patients with essential hypertension (EH). METHODS 108 patients with EH were divided into three groups according to the blood pressure (BP) status and antihypertensive drug therapy (ADT): group A (n = 41, with ADT and BP failing to reach the standard), group B (n = 27, with high BP and without ADT), and group C (n = 40, with ADT and BP reaching the standard). Twenty-five sex, age, body mass index, waist/hip ratio, heart rate, fasting glycemia and lipid level-matched healthy persons were used as controls (group D). PWV, CFR, left ventricle mass index (LVMI), E/A ratio, left ventricle isovolumetric relaxation time (IVRT), left ventricular ejection fraction (LVEF) were detected in each group. Correlation analysis was conducted. RESULTS (1) the PWV values in group A, B, and C were 12.7 m/s +/- 1.3 m/s, 11.9 m/s +/- 2.2 m/s, and 9.6 m/s +/- 0.9 m/s respectively, all significantly higher than that in the group D (8.9 m/s +/- 0.7 m/s, all P < 0.05) and with significant differences between group A and group C and between group B and group C (both P < 0.05). The CFR values in group A, B, and C were 2.56 +/- 0.18, 2.54 +/- 0.19, and 2.97 +/- 0.19 respectively, all significantly lower than that in group D (3.26 +/- 0.26, all P < 0.05), with significant differences between group A and group C and between group B and group C (both P < 0.05). The values of average peak velocity of baseline (APVb) in groups A, B, and C were 38 cm/s +/- 4 cm/s, 36 cm/s +/- 4 cm/s, and 34 cm/s +/- 4 cm/s respectively, significantly higher than that in group D (29 cm/s +/- 4 cm/s, all P < 0.05), without significant difference among group A, B, and C. (2) The values of LVMI in group A, B, and C were 117 g/m(2) +/- 8 g/m(2), 113 g/m(2) +/- 11 g/m(2), and 98 g/m(2) +/- 11 g/m(2) respectively, all significantly higher than that of group D (88 g/m(2) +/- 7 g/m(2), all P < 0.05), with significant differences between group A and group C and between group B and group C. The E/A ratios in groups A and B were 0.76 +/- 0.19 and 0.87 +/- 0.18 respectively, significantly lower than those in group C and D (1.08 +/- 0.18 and 1.19 +/- 0.15 respectively, all P < 0.05). The values of IVRT in group A, B and C were 134 ms +/- 11 ms, 138 ms +/- 11 ms, and 109 ms +/- 12 ms respectively, all significantly higher than that in group D (84 ms +/- 12 ms, all P < 0.05) with a significant difference between group A and group C and between group B and group C (both P < 0.05). There was no significant difference of LVEF among different groups. (3) Stepwise linear regression of PWV showed that the factors significantly influencing PWV were age, LVMI, E/A ratio and SBP, the factors significantly influencing CFR were LVMI and PWV, and PWV was significant negatively correlated to CFR and significant positively correlated to APVb. CONCLUSION PWV is increased in hypertensive patients. The increase of LVMI and increase of PWV contribute to the decrease of CFR in hypertensive patients. Increase of PWV that attributes to left ventricular hypertrophy and decrease of E/A ratio in patients with hypertension leads to further decrease of CFR. Decrease of CFR in hypertensive patients may cause cardiovascular diseases. Early detection and prevention of increased PWV may decrease the morbidity of cardiovascular diseases.
Prevalence, Awareness, Treatment, and Control of Hypertension in China: Results from a National Survey.

Abstract:BACKGROUND:Hypertension is one of the major risk factor for cardiovascular disease worldwide. The objective of this study was to investigate the prevalence, awareness, treatment, and control of hypertension in China.METHODS:A multistage, stratified sampling method was used to obtain a representative sample of persons aged 18 years or older in the general population of China. Blood pressure (BP) was measured by sphygmomanometer 3 times at 5-minute intervals. Hypertension was defined as a systolic BP ≥ 140mm Hg, or diastolic BP ≥ 90mm Hg, or self-reported use of antihypertensive medications in the last 2 weeks irrespective of the BP.RESULTS:Altogether 50,171 subjects finished the survey across the entire country. The adjusted prevalence of hypertension was 29.6% (95% confidence interval (CI) = 28.9%-30.4%) and was higher among men than among women (31.2%, 95% CI = 30.1%-32.4%; vs. 28.0%, 95% CI = 27.0%-29.0%). The awareness, treatment among all hypertensive participants, control among all hypertensive participants, and control among treated hypertensive participants were 42.6%, 34.1%, 9.3%, and 27.4%, respectively. Multiple lifestyle factors were independently associated with presence of hypertension, including physical inactivity, habitual drinking, chronic use of nonsteroidal anti-inflammatory drugs, high body mass index, and central obesity.CONCLUSIONS:Hypertension is an important public health burden in China, and control of hypertension is still suboptimal. Several modifiable lifestyle activities were associated with hypertension and thus should be considered potential targets for intervention, with special attention to socioeconomically disadvantaged subpopulations in China.
Factors associated with prevalence, awareness, treatment and control of hypertension in urban adults from 33 communities in China: the CHPSNE Study

Abstract:Using a randomized multistage cluster sampling, we studied the factors associated with the prevalence, awareness, treatment and control of hypertension (defined as systolic and diastolic blood pressure (BP) measurements ⩾140 and 90 mm Hg, respectively, or current drug treatment for hypertension) in a representative sample of the urban Chinese population. The participants were 18–74 years of age and had lived for at least 5 years in an area comprising 33 communities in three cities (Shenyang, Anshan and Jinzhou) in China. A total of 28 830 people were selected; the overall response rate was 87.4% (25 196/28 830). The overall prevalence of hypertension was 28.7% (7237/25 196). Of those with hypertension, 42.9% (3107/7237) were aware of their condition, 28.2% (2042/7237) were receiving treatment and 3.7% (264/7237) were considered controlled. More than half (57.8%) of the hypertensives did not think that high BP would endanger their lives. Multivariate analysis revealed that age, gender, education, occupation, income, body mass index, waist circumference and a family history of hypertension correlated significantly with the prevalence of hypertension. Among all the hypertensives, higher awareness (determined by odds ratios; 95% confidence intervals) was noted for persons who were white-collar workers (1.29; 1.08, 1.53, respectively), overweight (1.48; 1.30, 1.69), obese (3.37; 2.76, 4.11) or had a family history of hypertension (3.07; 2.76, 3.42). Among the individuals aware of their hypertension, treatment was more common in those with a higher level of education and less common among individuals consuming ⩾2 alcoholic drinks per day (0.65; 0.52, 0.83). Controlled hypertension was much less common among older persons, and participants who were former smokers (0.49; 0.26, 0.91). The results indicate that more attention is needed to improve the awareness of the potentially fatal nature of hypertension in urban China.

Abstract:Hypertension is an important modifiable risk factor for cardiovascular diseases. This study aimed to determine the up-to-date prevalence, awareness, treatment, and control of hypertension, and their related influencing factors in adults in Beijing, China. A cross-sectional study was conducted in 2008. A total of 5760 adults aged 18-79 years were selected from the general population. Data from a standardized questionnaire, physical examination, and blood sample were obtained. The prevalence of hypertension was 35.5% (41.8% in men, 30.9% in women) and increased with age in both sexes. The prevalence was similarly high in urban (35.6%) and in rural (34.9%) areas. Body mass index (BMI) and waist circumference were independently associated with hypertension. Participants with diabetes and/or dyslipidemia had higher prevalence of hypertension than their counterparts. Among hypertensive patients, 42.5% were aware of the diagnosis, 35.9% received treatment, and 11.8% had hypertension control. Awareness and treatment of hypertension were low in men, young people, and rural residents. Control of hypertension was lower in older people and those with lower educational attainment. High waist circumference (central obesity) resulted in low control of hypertension (odds ratio = 0.45). Our findings underscore the urgent need to develop a comprehensive health strategy for the prevention, detection, and control of hypertension to avert the cardiovascular disease epidemic in Beijing.

Abstract:The aim of this study is to evaluate the developments in the treatment and prevalence of hypertension by demographic subgroups in least developed area of China in 2012 and 2022. This population‐based cross‐sectional study was conducted in 2012 and 2022, we applied stratified multistage random sampling to investigate residents aged 18 years or older in Gansu, the least developed province in the northwest of China. Questionnaires and anthropometric measurements were given to all respondents. The standardized prevalence of hypertension in adults in Gansu increased from 26.1% in 2012 to 28.8% in 2022. Compared with 2012, the control rate remains decreased despite the significantly improved awareness and treatment rates of hypertension in 2022. Apart from the reversal of the control rate, the trend of higher prevalence in men and higher awareness and treatment rates in women has not changed. There was an obviously increase in the proportion of participants who had received health education and hypertension management services from medical workers. The treatment was still primarily monotherapy, and there was no significant improvement in the prescription of medication. The prevalence of hypertension has increased mildly in the least developed region of China over the past decade, and the challenge of hypertension management has shifted from increasing awareness and treatment rates to increasing control rates. The onset and control of hypertension are affected by education methods, BMI, local economic conditions and other factors, and targeted strategies can be adopted to strengthen the management of hypertension in economically underdeveloped areas of China.

Abstract:OBJECTIVE: To assess the prevalence, awareness, treatment, and control of hypertension and their associated factors in an urban Chinese population.METHODS: A cross-sectional study was conducted in three cities in northeast China from 2009 to 2010, using a multistage cluster sampling method to select a representative sample. A total of 25 196 adults, aged 18-74 years, were examined in 33 communities. Hypertension was defined as a mean SBP of at least 140 mmHg, DBP at least 90 mmHg, and/or use of antihypertensive medications.RESULTS: Overall, the prevalence of hypertension was 28.7% for urban residents, and 39.1% for middle-aged and elderly residents (aged ≥35 years). Among all the hypertensive patients examined in the study (n = 7237), 42.9% were aware of their condition, 28.2% were receiving treatment, and only 3.7% had their blood pressure adequately controlled. Female hypertensive patients had more effectively controlled blood pressure than their male counterparts. Among the study participants, 37.9% did not think that high blood pressure would endanger their lives. Among hypertensive patients aware of their conditions, the primary reason for not taking antihypertensive medication was a lack of money (34.8%). Age, sex, education, occupation, income, body mass, waist circumference, and family hypertension history significantly correlated with the prevalence of hypertension.CONCLUSION: Hypertension is highly prevalent in the urban population of China, and the effects of being overweight/obesity on hypertension were much larger than any other examined factors. The percentage of hypertensive patients aware of their condition, receiving proper treatment, and keeping their hypertension under control is unacceptably low.

Abstract:BACKGROUND Hypertension is the most important risk factor for cardiovascular diseases. Little information exists on the status of hypertension among southern Chinese. We therefore investigated the hypertension prevalence, awareness, treatment, control, and associated factors in a southern Chinese population with 85 million residents. METHODS Stratified multistage cluster sampling with probability proportional to size method was used in this survey. A representative sample of 13,889 residents aged 20 years or above with completed questionnaire and blood pressure (BP) measurement was obtained. BP was measured in accordance with the 1999 World Health Organization/International Society of Hypertension Guidelines. Information related to history of diagnosis and treatment of hypertension was collected through questionnaire. RESULTS The prevalence of hypertension in this population was 20.5% (16.5%, 24.4%), which translated to 9.8 million adults suffering from hypertension in Guangdong province. The urban population had higher prevalence of hypertension than the rural population (25.1 vs. 16.1%). The prevalence of awareness, treatment, and control of hypertension in hypertensive patients living in urban regions were 42.8, 37.9, and 13.5%, respectively, which were higher than those in rural regions (the corresponding figures were 17.6, 10.4, and 3.4%, respectively). Nearly 50% urban adults and 80% rural adults did not measure their BP in the last 12 months. Frequency of BP measurement was associated with both awareness and treatment. CONCLUSIONS Hypertension was prevalent in southern China. The prevalence of awareness, treatment, and control of hypertension is low. Urgent strategies are needed to improve prevention, detection, and treatment of hypertension in this large Chinese population.

Abstract:The past two to three decades have seen a steady increase in the prevalence of hypertension in China, largely owing to increased life expectancy and lifestyle changes (particularly among individuals aged 35–44 years). Data from the China hypertension survey conducted in 2012–2015 revealed a high prevalence of grade 3 hypertension (systolic blood pressure ≥180 mmHg and diastolic blood pressure ≥110 mmHg) in the general population, which increased with age to up to 5% among individuals aged ≥65 years. The risk profile of patients with hypertension in China has also been a subject of intense study in the past 30 years. Dietary sodium and potassium intake have remained largely the same in China in the past three decades, and salt substitution strategies seem to be effective in reducing blood pressure levels and the risk of cardiovascular events and death. However, the number of individuals with risk factors for hypertension and cardiovascular disease in general, such as physical inactivity and obesity, has increased dramatically in the same period. Moreover, even in patients diagnosed with hypertension, their disease is often poorly managed owing to a lack of patient education and poor treatment compliance. In this Review, we summarize the latest epidemiological data on hypertension in China, discuss the risk factors for hypertension that are specific to this population, and describe several ongoing nationwide hypertension control initiatives that target these risk factors, especially in the low-resource rural setting.
