Abstract:In the 1899 6th edition of his influential textbook, Kraepelin proposed a diagnostic category of "Old-Age Paranoid Psychosis." In this 1930 article, Bruno Schulz studied the morbid risk (MR) of several disorders and traits in the parents, siblings, offspring, and nieces/nephews of 51 probands with "Old-Age Paranoid Psychosis." His results permitted an evaluation of the validity of Kraepelin's category of Old-Age Paranoid Psychosis, in particular, whether it was a form of psychosis resulting from "senile changes" or late-onset schizophrenia. The MR of schizophrenia in these four groups of relatives varied from 0 to 2.4% with 3 of 4 somewhat higher than population expectations but much lower than parallel results in relatives of schizophrenics. By contrast, the rates of eccentricity in these relatives were uniformly elevated over population rates, sometimes approaching those seen in relatives of schizophrenics. Schulz concluded, from his study, that Old-Age Paranoid Psychosis was a distinct disorder not closely related to schizophrenia. However, he suggested that a family history and/or a premorbid trait of eccentricity increases the risk of developing a paranoid psychosis in old age, particularly when associated with physical or mental decline. He was uncertain about whether the trait of eccentricity he found in this study was very similar or distinct from that observed in excess in relatives of schizophrenics. This study was the first, to the best of our knowledge, to use a family study design explicitly to address a nosologic question-in this case the familial relationship between Old-Age Paranoid Psychosis and schizophrenia.

Abstract:Schizophrenia (SZ) is a psychiatric disorder with complex genetic risk dictated by interactions between hundreds of risk variants. Epigenetic factors, such as histone posttranslational modifications (PTMs), have been shown to play critical roles in many neurodevelopmental processes, and when perturbed may also contribute to the precipitation of disease. Here, we apply an unbiased proteomics approach to evaluate combinatorial histone PTMs in human induced pluripotent stem cell (hiPSC)-derived forebrain neurons from individuals with SZ. We observe hyperacetylation of H2A.Z and H4 in neurons derived from SZ cases, results that were confirmed in postmortem human brain. We demonstrate that the bromodomain and extraterminal (BET) protein, BRD4, is a bona fide ‘reader’ of H2A.Z acetylation, and further provide evidence that BET family protein inhibition ameliorates transcriptional abnormalities in patient-derived neurons. Thus, treatments aimed at alleviating BET protein interactions with hyperacetylated histones may aid in the prevention or treatment of SZ.

Abstract:Biological psychiatry in the early twentieth century was based on interrelated disciplines, such as neurology and experimental biology. Neuropsychiatrist Franz Josef Kallmann (1897-1965) was a product of this interdisciplinary background who showed an ability to adapt to different scientific contexts, first in the field of neuromorphology in Berlin, and later in New York. Nonetheless, having innovative ideas, as Kallmann did, could be an ambiguous advantage, since they could lead to incommensurable scientific views and marginalization in existing research programs. Kallmann followed his Dr. Med. degree (1919) with training periods at the Charité Medical School in Berlin under psychiatrist Karl Bonhoeffer (1868-1948). Subsequently, he collaborated with Ernst Ruedin (1874-1952), investigating sibling inheritance of schizophrenia and becoming a protagonist of genetic research on psychiatric conditions. In 1936, Kallmann was forced to immigrate to the USA where he published The Genetics of Schizophrenia (1938), based on data he had gathered from the district pathological institutes of Berlin's public health department. Kallmann resumed his role as an international player in biological psychiatry and genetics, becoming president (1952) of the American Society of Human Genetics and Director of the New York State Psychiatric Institute in 1955. While his work was well received by geneticists, the idea of genetic differences barely took hold in American psychiatry, largely because of émigré psychoanalysts who dominated American clinical psychiatry until the 1960s and established a philosophical direction in which genetics played no significant role, being regarded as dangerous in light of Nazi medical atrocities. After all, medical scientists in Nazi Germany had been among the social protagonists of racial hygiene which, under the aegis of Nazi philosophies, replaced medical genetics as the basis for the ideals and application of eugenics.

Abstract:Objectives: Schizophrenia patients have a wide range of cognitive deficits. To explore the structure of these deficits and index their psychometric properties in order to define the major separable factors, exploratory factor analysis (EFA) was performed on a series of neuropsychological test results in a sample of Han Chinese first-episode schizophrenia patients, their relatives and controls without mental illness. Methods: The factors derived from the composite sample were tested with confirmatory factor analysis (CFA) in each of the subsamples. The heritability of each factor was estimated by using the mixed linear model in order to determine whether the common factor scores describe the familiarity of the data. Results: A six-factor model of general mental activity, sort and shift, attention and anti-interference, logical memory, reasoning and problem-solving/executive function, and visual reproduction were extracted and confirmed on EFA and CFA. There was a hierarchy in cognitive performance deficits in relatives depending on their relatedness to probands. Patients performed more poorly than siblings/offspring in general mental activity, sort and shift, attention and anti-interference, logical memory, planning and problem-solving, but not in visual reproduction. The logical memory domain was found to be significantly heritable (h<SU2</SU=0.34), and reasoning and problem-solving had a marginal heritability of around 0.19. Conclusions: Generalized cognitive deficit accounts for the majority of the overall variance of cognitive deficits in schizophrenia. Estimates of heritability were modest. The present results also support the hypothesis that neurocognitive deficits of schizophrenia are familial and could serve as endophenotypes, especially logical memory, which is the most heritable.

Abstract:Suzanne Retzinger's article, "A Theory of Mental Illness," provides further untangling of the complex links between social relationships and psychopathology. Those links have been demonstrated in such diverse areas as the impact of family relationships on relapse (e.g., Brown et al. 1972), the predictive importance of social relations to outcome (e.g., Strauss and Carpenter 1977), and the genetic connections between social relationship traits and vulnerability to psychopathology (Kendler et al. 1982). The research presented here focuses on social relationships with special emphasis on stigma in those relationships and its impact on disorder. It also has the valuable role of forging connections between the more abstract labeling theory and the actual behaviors of people as viewed in their interaction.

Abstract:Bipolar disorder (BD), schizophrenia (SZ) and schizoaffective disorder (SZA) can be disabling disorders associated with severe psychiatric symptomatology. Individual psychopathological features often overlap between these diagnostic groups and their severity can vary widely. More severe psychopathological features are generally associated with a less favorable outcome. Further, all three diseases are common complex genetic disorders with a polygenic genetic architecture in the majority of cases. The inherent heterogeneity with regard to disease severity has posed a significant challenge to both the study of the underlying disease mechanism and the clinical management. Therefore, stratification of cases into more homogeneous subgroups across diagnoses using both longitudinal clusters derived from psychometric data and genetic information could provide a means to identify individuals with higher risk for severe illness, mandating earlier and intensified clinical intervention.
Genetic Diversity in Schizophrenia: Developmental Implications of Ultra-Rare, Protein-Truncating Mutations

Abstract:The genetic basis of schizophrenia (SZ) remains elusive despite its characterization as a highly heritable disorder. This incomplete understanding has led to stagnation in therapeutics and treatment, leaving many suffering with insufficient relief from symptoms. However, recent large-cohort genome- and exome-wide association studies have provided insights into the underlying genetic machinery. The scale of these studies allows for the identification of ultra-rare mutations that confer substantial disease risk, guiding clinicians and researchers toward general classes of genes that are central to SZ etiology. One such large-scale collaboration effort by the Schizophrenia Exome Sequencing Meta-Analysis consortium identified ten, high-risk, ultra-rare, protein-truncating variants, providing the clearest picture to date of the dysfunctional gene products that substantially increase risk for SZ. While genetic studies of SZ provide valuable information regarding "what" genes are linked with the disorder, it is an open question as to "when" during brain development these genetic mutations impose deleterious effects. To shed light on this unresolved aspect of SZ etiology, we queried the BrainSpan developmental mRNA expression database for these ten high-risk genes and discovered three general expression trajectories throughout pre- and postnatal brain development. The elusiveness of SZ etiology, we infer, is not only borne out of the genetic heterogeneity across clinical cases, but also in our incomplete understanding of how genetic mutations perturb neurodevelopment during multiple critical periods. We contextualize this notion within the National Institute of Mental Health's Research Domain Criteria framework and emphasize the utility of considering both genetic variables and developmental context in future studies.
Dissecting Schizophrenia Biology Using Pleiotropy with Cognitive Genomics

Abstract:Given the increasingly large number of loci discovered by psychiatric GWAS, specification of the key biological pathways underlying these loci has become a priority for the field. We have previously leveraged the pleiotropic genetic relationships between schizophrenia and two cognitive phenotypes (educational attainment and cognitive task performance) to differentiate two subsets of illness-relevant SNPs: (1) those with “concordant” alleles, which are associated with reduced cognitive ability/education and increased schizophrenia risk; and (2) those with “discordant” alleles linked to reduced educational and/or cognitive levels but lower schizophrenia susceptibility. In the present study, we extend our prior work, utilizing larger input GWAS datasets and a more powerful statistical approach to pleiotropic meta-analysis, the Pleiotropic Locus Exploration and Interpretation using Optimal test (PLEIO). Our pleiotropic meta-analysis of schizophrenia and the two cognitive phenotypes revealed 768 significant loci (159 novel). Among these, 347 loci harbored concordant SNPs, 270 encompassed discordant SNPs, and 151 “dual” loci contained concordant and discordant SNPs. Competitive gene-set analysis using MAGMA related concordant SNP loci with neurodevelopmental pathways (e.g., neurogenesis), whereas discordant loci were associated with mature neuronal synaptic functions. These distinctions were also observed in BrainSpan analysis of temporal enrichment patterns across developmental periods, with concordant loci containing more prenatally expressed genes than discordant loci. Dual loci were enriched for genes related to mRNA translation initiation, representing a novel finding in the schizophrenia literature.

Abstract:Schizophrenia is a complex disorder, where family, twin and adoption studies have been demonstrating a high heritability of the disease and that this disease is not simply defined by several major genes but rather evolves from addition or potentiation of a specific cluster of genes, which subsequently determines the genetic vulnerability of an individual. Linkage and association studies suggest that a genetic vulnerablility, is not forcefully leading to the disease since triggering factors and environmental influences, i.e. birth complications, drug abuse, urban background or time of birth have been identified. This has lead to the assumption that schizophrenia is not only a genetically defined static disorder but a dynamic process leading to dysregulation of multiple pathways. There are several different hypothesis based on several facets of the disease, some of them due to the relatively well-known mechanisms of therapeutic agents. The most widely considered neurodevelopmental hypothesis of schizophrenia integrates environmental influences and causative genes. The dopamine hypothesis of schizophrenia is based on the fact that all common treatments involve antidopaminergic mechanisms and genes such as DRD2, DRD3, DARPP-32, BDNF or COMT are closely related to dopaminergic system functioning. The glutamatergic hypothesis of schizophrenia lead recently to a first successful mGlu2/3 receptor agonistic drug and is underpinned by significant findings in genes regulating the glutamatergic system (SLC1A6, SLC1A2 GRIN1, GRIN2A, GRIA1, NRG1, ErbB4, DTNBP1, DAAO, G72/30, GRM3). Correspondingly, GABA has been proposed to modulate the pathophysiology of the disease which is represented by the involvement of genes like GABRA1, GABRP, GABRA6 and Reelin. Moreover, several genes implicating immune, signaling and networking deficits have been reported to be involved in the disease, i.e. DISC1, RGS4, PRODH, DGCR6, ZDHHC8, DGCR2, Akt, CREB, IL-1B, IL-1RN, IL-10, IL-1B. However, molecular findings suggest that a complex interplay between receptors, kinases, proteins and hormones is involved in schizophrenia. In a unifying hypothesis, different cascades merge into another that ultimately lead to the development of symptoms adherent to schizophrenic disorders.

Abstract:Kleine-Levin syndrome (KLS) is a rare sleep disorder that affects ~ 1 person in a million and has been suggested to be more frequent in Ashkenazi Jewish. The disorder typically strikes adolescent males and improves with age, often resolving by age 30. KLS, patients have recurrent episodes sometimes lasting up to several weeks where they sleep nearly 24 hours per day. While awake, patients during episodes experience apathy, cognitive disturbances and occasionally hyperphagia and/or hypersexuality. Between episodes, patients are totally asymptomatic. Our aim was to identify genetic variants that contribute to KLS predisposition. We performed GWAS on 650 KLS cases and 15,000 controls as a part of an international collaboration. The sample contained KLS cases and matched controls from United States, Europe and Asia plus additional controls from the GERA consortium. Genotyping was done using Affy 6.0 and Affymetrix Axiom World Array with ethnicity specific platforms that were imputed to 1000 genomes. Analyses were controlled for population stratification and ethnicity (Caucasian, Ashkenazi Jewish, Asians, other). We also have pursued other approaches in understanding KLS by using Whole Exome Sequencing (WES) data and have identified 14 family pedigrees to pursue family association studies. Furthermore, we are also looking into any shared genetic architectures with other neuropsychiatric disorders. So far we have identified a Genome-wide significant loci near TRANK1. Most interestingly, the leading TRANK1 variant is also reported in other GWAS for bipolar disorder (BD). With this new insight we are looking into identifying any other shared genetic characteristics with BD. The findings give the first biological evidence for disease mechanisms in KLS. Importantly, these results suggest a partially overlapping genetic composition for schizophrenia and KLS. That these patients are not primarily depressed or psychotic during episodes and completely reverse to normality between episodes could suggest important pathophysiological clues linking sleep and episodic psychiatric conditions. none.

Abstract:Kraepelin's description of "dementia praecox" was a synthesizing influence in a field that had for too long been fragmented in its thinking about severe psychiatric disorders. Now, once again, the field is attempting to deal with some of the complexities involved in psychopathology and its evolution, complexities that have been reemphasized by newer descriptive research methods. To account for patients deviations from stereotyped diagnostic and prognostic concepts, multiple aspects of course of disorder have been defined and principles of multiaxial diagnosis supported. These principles can provide a basic structure for practice and research using a biopsychosocial orientation within the context of a system model.

Abstract:Abstract Background Impaired memory is among the most profound cognitive deficits observed in patients with schizophrenia. Evidence from twin studies suggests that memory is mainly influenced by genetics. Moreover, a few twin studies have demonstrated genetic overlap between specific memory functions and schizophrenia. Memory deficits in schizophrenia seem to involve abnormalities in frontal cortical areas and the middle temporal lobe, particularly the hippocampus. In the general population, twin studies have consistently demonstrated genetic influences on brain volumes, however, evidence from twin pairs discordant for schizophrenia suggests that hippocampus volumes may be more susceptible to environmental effects in patients. Methods Twin pairs concordant or discordant for a diagnosis in the schizophrenia spectrum were recruited nation-wide by linking The Danish Twin Register and The Danish Psychiatric Central Research Register. Both monozygotic (MZ) and dizygotic (DZ) proband pairs as well as healthy control (HC) pairs were identified. A total of 216 twins participated in this study consisting of 32 complete MZ and 24 complete DZ proband pairs, 29 complete MZ and 20 complete DZ HC pairs, and six twins from proband pairs were included without their sibling. Verbal memory was assessed using the list learning task from the Brief Assessment of Cognition in Schizophrenia (BACS), visual memory using the Rey Complex Figure Test (RCFT) and associative memory using 15 word pairs. Structural brain scans were acquired with T1-weighted sequence on a Philips 3.0 T Achieva MRI scanner with a 32-channel SENSE head coil. Images were processed using FreeSurfer (version 5.3) and the Desikan-Killiany atlas was used to extract the volumes of bilateral hippocampi, superior frontal, rostral and caudal middle frontal cortices as well as the whole brain volume. Structural equation modelling was applied to examine the genetic and environmental contributions to the variability in memory and brain measures and to quantify associations with schizophrenia spectrum liability. Results Significant heritability estimates were observed for verbal memory (h2=0.53), visual memory (h2=0.58) and associative memory (immediate h2=0.33, delayed h2=0.54), whereas the copy and recognition task from RCFT were only explained by unique environmental factors. Except for verbal memory, all memory measures were significantly associated with schizophrenia spectrum liability, and these were mainly due to overlapping genetic factors. Genetic factors also significantly contributed to whole brain (h2=0.36), right superior frontal (h2=0.48), left rostral middle frontal (h2=0.40) and hippocampus volumes (right h2=0.29, left h2=0.50). Common environmental factors significantly influenced whole brain (c2=0.51), right hippocampus (c2=0.51) and right rostral middle frontal (c2=0.47) volumes. Hippocampus volumes were significantly associated with schizophrenia spectrum liability, and for the left hippocampus this association was due to overlapping genetic factors. Discussion Specific memory measures and related brain areas were heritable, providing further evidence of the importance of genetics in memory functioning. Furthermore, the majority of the applied memory measures and left hippocampal volume were (genetically) associated with schizophrenia spectrum liability, suggesting a partially shared etiology. The heritable memory measures and related brain areas showing associations with disease may represent endophenotypes for schizophrenia spectrum disorders. In future analyses, we plan to examine the covariance between memory, brain volumes and schizophrenia.

Abstract:Abstract Neuroanatomical abnormalities have been reported along a continuum from at-risk stages, including high schizotypy, to early and chronic psychosis. However, a comprehensive neuroanatomical mapping of schizotypy remains to be established. The authors conducted the first large-scale meta-analyses of cortical and subcortical morphometric patterns of schizotypy in healthy individuals, and compared these patterns with neuroanatomical abnormalities observed in major psychiatric disorders. The sample comprised 3004 unmedicated healthy individuals (12–68 years, 46.5% male) from 29 cohorts of the worldwide ENIGMA Schizotypy working group. Cortical and subcortical effect size maps with schizotypy scores were generated using standardized methods. Pattern similarities were assessed between the schizotypy-related cortical and subcortical maps and effect size maps from comparisons of schizophrenia (SZ), bipolar disorder (BD) and major depression (MDD) patients with controls. Thicker right medial orbitofrontal/ventromedial prefrontal cortex (mOFC/vmPFC) was associated with higher schizotypy scores ( r = 0.067, p FDR = 0.02). The cortical thickness profile in schizotypy was positively correlated with cortical abnormalities in SZ ( r = 0.285, p spin = 0.024), but not BD ( r = 0.166, p spin = 0.205) or MDD ( r = −0.274, p spin = 0.073). The schizotypy-related subcortical volume pattern was negatively correlated with subcortical abnormalities in SZ (rho = −0.690, p spin = 0.006), BD (rho = −0.672, p spin = 0.009), and MDD (rho = −0.692, p spin = 0.004). Comprehensive mapping of schizotypy-related brain morphometry in the general population revealed a significant relationship between higher schizotypy and thicker mOFC/vmPFC, in the absence of confounding effects due to antipsychotic medication or disease chronicity. The cortical pattern similarity between schizotypy and schizophrenia yields new insights into a dimensional neurobiological continuity across the extended psychosis phenotype.

Abstract:Schizophrenia (SCZ) is a severe psychiatric disorder with a strong genetic component. High heritability of SCZ suggests a major role for transmitted genetic variants. Furthermore, SCZ is also associated with a marked reduction in fecundity, leading to the hypothesis that alleles with large effects on risk might often occur de novo. In this study, we conducted whole-genome sequencing for 23 families from two cohorts with unaffected siblings and parents. Two nonsense de novo mutations (DNMs) in GJC1 and HIST1H2AD were identified in SCZ patients. Ten genes (DPYSL2, NBPF1, SDK1, ZNF595, ZNF718, GCNT2, SNX9, AACS, KCNQ1, and MSI2) were found to carry more DNMs in SCZ patients than their unaffected siblings by burden test. Expression analyses indicated that these DNM implicated genes showed significantly higher expression in prefrontal cortex in prenatal stage. The DNM in the GJC1 gene is highly likely a loss function mutation (pLI=0.94), leading to the dysregulation of ion channel in the glutamatergic excitatory neurons. Analysis of rare variants in independent exome sequencing dataset indicates that GJC1 has significantly more rare variants in SCZ patients than in unaffected controls. Data from genome-wide association studies suggested that common variants in the GJC1 gene may be associated with SCZ and SCZ-related traits. Genes co-expressed with GJC1 are involved in SCZ, SCZ-associated pathways, and drug targets. These evidences suggest that GJC1 may be a risk gene for SCZ and its function may be involved in prenatal and early neurodevelopment, a vulnerable period for developmental disorders such as SCZ.

Abstract:Positive and negative schizotypy reflect distinct patterns of subclinical traits in the general population associated with neurodevelopmental and schizophrenia-spectrum pathologies. Yet, a comprehensive characterization of the unique and shared neuroanatomical signatures of these schizotypy dimensions is lacking. Leveraging 3D brain MRI data from 2,730 unmedicated healthy individuals, we identified neuroanatomical profiles of positive and negative schizotypy and systematically compared them to disorder-specific, micro-architectural, connectome, and neurotransmitter-level measures. Positive and negative schizotypy were associated with thinner frontal and thicker paralimbic cortical areas, respectively, and were differentially linked to cortical patterns of schizophrenia-spectrum and neurodevelopmental conditions. Furthermore, these schizotypal cortical patterns mapped onto local attributes of gene expression, cortical myelination, D1 and histamine receptor distributions. Network models identified cortical hub vulnerability to schizotypy-related thickness reduction and epicenters in sensorimotor-to-association and paralimbic areas. This study yields insights into the complex cortical signatures of schizotypy and their relationship to diverse features of cortical organization.

Abstract:Susceptibility to schizophrenia is inversely correlated with general cognitive ability at both the phenotypic and the genetic level. Paradoxically, a modest but consistent positive genetic correlation has been reported between schizophrenia and educational attainment, despite the strong positive genetic correlation between cognitive ability and educational attainment. Here we leverage published genome-wide association studies (GWASs) in cognitive ability, education, and schizophrenia to parse biological mechanisms underlying these results. Association analysis based on subsets (ASSET), a pleiotropic meta-analytic technique, allowed jointly associated loci to be identified and characterized. Specifically, we identified subsets of variants associated in the expected ("concordant") direction across all three phenotypes (i.e., greater risk for schizophrenia, lower cognitive ability, and lower educational attainment); these were contrasted with variants that demonstrated the counterintuitive ("discordant") relationship between education and schizophrenia (i.e., greater risk for schizophrenia and higher educational attainment). ASSET analysis revealed 235 independent loci associated with cognitive ability, education, and/or schizophrenia at p < 5 × 10-8. Pleiotropic analysis successfully identified more than 100 loci that were not significant in the input GWASs. Many of these have been validated by larger, more recent single-phenotype GWASs. Leveraging the joint genetic correlations of cognitive ability, education, and schizophrenia, we were able to dissociate two distinct biological mechanisms-early neurodevelopmental pathways that characterize concordant allelic variation and adulthood synaptic pruning pathways-that were linked to the paradoxical positive genetic association between education and schizophrenia. Furthermore, genetic correlation analyses revealed that these mechanisms contribute not only to the etiopathogenesis of schizophrenia but also to the broader biological dimensions implicated in both general health outcomes and psychiatric illness.

Abstract:Schizophrenia is a debilitating familial neuropsychiatric disorder which affects 1% of people worldwide. Although the heritability for schizophrenia approaches 80% only a small proportion of the overall genetic risk has been accounted for, and to date only a limited number of genetic loci have been definitively implicated. We have identified recently through genetic and in vitro functional studies, a novel serine/threonine kinase gene, unc-51-like kinase 4 (ULK4), as a rare risk factor for major mental disorders including schizophrenia. Now using the approach of in utero gene transfer we have discovered that Ulk4 plays a key modulatory role in corticogenesis. Knockdown of Ulk4 leads to significantly decreased cell proliferation in germinal zones and profound deficits in radial migration and neurite ramification. These abnormalities can be reversed successfully by Ulk4 gene supplementation. Ulk4 also regulated acetylation of α-tubulin, an important post-translational modification of microtubules. We conclude that Ulk4 plays an essential role in normal brain development and when defective, the risk of neurodevelopmental disorders such as schizophrenia is increased.

Abstract:Next to Karl Leonhard (1904-1988), Helmut Rennert (1920-1994) was the internationally best known representative of psychiatry from the former German Democratic Republic (GDR). He rose to prominence above all through his model of the universal genesis of endogenous psychoses, which constituted an antithesis to Leonhard's differentiated division. The 'polar opposite' aspects of Rennert and Leonhard are represented with an emphasis on their contrasting views of psychiatric nosology. In this respect, Rennert's model conceptions constitute the primary focus of attention. The fact that both concepts continue to possess topical perspectives to this day is reflected in the current discussion regarding nosological, categorical, syndromatological and dimensional approaches in relation to the further development of the classification systems of mental disorders. The preparatory work on the future classification systems will potentially omit the dichotomy between schizophrenic and affective disorders. These endeavors are in accordance with Rennert's unitarian psychopathological view and support his acceptance of the psychopathological continuity from affective to schizophrenic syndromes. The current discussion concerning the future classification is in line with Rennert, whose aim was to provide an unconventional model that unites findings from the different fields of psychiatric research. The classification of mental disorders without a previous establishment on the basis of fundamental theoretical assumptions could certainly foster a dynamic development in the future. A differentiated knowledge of the history of the ideas of unitary psychosis and their further development might be helpful in this development and can be of particular use when critically questioning explanatory approaches that are prone to simplification.

Abstract:Schizophrenia is routinely referred to as a neurodevelopmental disorder, but the role of brain development in a disorder typically diagnosed during early adult life is enigmatic. The authors revisit the neurodevelopmental model of schizophrenia with genomic insights from the most recent schizophrenia clinical genetic association studies, transcriptomic and epigenomic analyses from human postmortem brain studies, and analyses from cellular models that recapitulate neurodevelopment. Emerging insights into schizophrenia genetic risk continue to converge on brain development, particularly stages of early brain development, that may be perturbed to deviate from a typical, normative course, resulting in schizophrenia clinical symptomatology. As the authors explicate, schizophrenia genetic risk is likely dynamic and context dependent, with effects of genetic risk varying spatiotemporally, across the neurodevelopmental continuum. Optimizing therapeutic strategies for the heterogeneous collective of individuals with schizophrenia may likely be guided by leveraging markers of genetic risk and derivative functional insights, well before the emergence of psychosis. Ultimately, rather than a focus on therapeutic intervention during adolescence or adulthood, principles of prediction and prophylaxis in the pre- and perinatal and neonatal stages may best comport with the biology of schizophrenia to address the early-stage perturbations that alter the normative neurodevelopmental trajectory.
