Abstract:OBJECTIVE To investigate the clinical and molecular genetics characteristics of a patient with mild androgen insensitivity syndrome (MAIS). METHODS Clinical data of the patient was collected, and DNA was isolated from peripheral blood sample. Eight exons of AR gene were amplified by PCR with specific primers and directly sequenced by Sanger method. The results were compared with standard sequences from GenBank. Online Polyphen-2 software was applied to predict the effect of mutation on the protein function and compare the conservation of the sequence at the mutation site in various species. The exon of the AR gene containing the mutated site was analyzed in 90 unrelated normal males using PCR and restrictive digestion with Sfa NI. RESULTS Sequence analysis has detected a novel missense mutation in codon 176 of exon 1 (Ser176Arg) of the AR gene. Analysis with polyphen-2 software has indicated the codon to be highly conserved across various species, and that the S176A mutation has caused damage to the protein structure and function (prediction score=0.999). The same mutation was not detected in 90 healthy males. CONCLUSION The S176A mutation of the AR gene may contribute to the mild androgen insensitivity syndrome.
Trace elements and micropollutant anions in the dialysis and reinfusion fluid prepared on-line for haemodiafiltration.

Abstract:Exposure to the trace elements and micropollutions of tap water may be very considerable in dialysis patients. As few data on trace elements in reinfusion and dialysis fluid for haemodiafiltration (HDF) have been reported, we studied nine trace elements (microg/l; Al, As, Cd, Cr, Cu, Hg, Pb, Se, Zn) and five anions (mg/l; F-, NO2-, NO3-, PO4(3-), SO4(2-)) in tap water, in water after two passages of reverse osmosis (2RO), in dialysate and in on-line prepared reinfusate. NO3- and SO4(2-) were somewhat elevated in our tap water (22.2+/-7.6 and 21.8+/-11.3 mg/l) but decreased (P<0.001) after 2RO (1.4+/-1.5 and 0.9+/-1.1 mg/l); the other anions, which were at a very low level, remained unchanged. All trace elements decreased, with statistical significance only for Al, Cr and Zn from 14.9+/-19.9, 2.6+/-0.6 and 35.1+/-41.1 microg/ to 3.2+/-2.1, 0.2+/-0.2 and 3.5+/-4.8 microg/l, respectively. Due to impurities in concentrate salts for Al (5.4+/-3.1), Cr (0.5+/-0.4) and SO4(2-) (2.4+/-1.8), greater concentrations were found in dialysate and reinfusate than in tap water after 2RO (P<0.03). For all measurements, trace elements and anions were at acceptable levels according to international standards. Simultaneous determinations of trace elements at inflow (Din) and outflow (Dout) of the dialysate as well as in plasma or in whole blood at the beginning of on-line HDF documented Dout/Din>1 for Al, Cu and Zn and a positive gradient between the concentration in blood and dialysate inlet. In conclusion, our dialysate and reinfusate can be considered safe regarding trace elements and micropollution: two passages through reverse osmosis reduces the concentrations of trace elements and anions. The impurities of concentrates are acceptable. Accumulation or depletion of trace elements should be evaluated after longitudinal studies of plasma concentrations.
Diffusion-weighted Imaging in Assessing Renal Pathology of Chronic Kidney Disease: A Preliminary Clinical Study

Abstract:To investigate the clinical potential of diffusion-weighted imaging (DWI) in assessing renal pathology of chronic kidney disease (CKD).Seventy-one CKD patients and twelve healthy volunteers were examined using DWI with prospective acquisition correction. Renal biopsy specimens from the CKD patients were scored based on the severity of renal pathology and to confirm pathology type. CKD patients were divided into three groups according to pathology scores: mild, moderate, or severe. The association between renal apparent diffusion coefficient (ADC) values and pathology scores was investigated using Pearson's correlation and single factor analysis of variance. Multiple linear regression analysis was performed to explore associations between renal ADC values and pathology score, glomerular filtration rate, serum creatinine, and age. The Kruskal-Wallis H test was conducted to compare ADC values and pathology type.Renal ADC values correlated negatively with pathology scores (r=-0.633, P<0.001). The ADC values among the four groups (mild, moderate, severe impairment, and controls) were significantly different (F=19.512, P<0.001). However, when patients were stratified by pathology type, no significant differences were found in ADC values among these groups (χ(2)=9.929, P=0.270). Further multiple linear regression analysis showed that only the pathology score and ADC values were related (t=-4.586, P=0.000).DWI has clinical potential in assessing the severity of renal pathology in CKD and shows promise as a non-invasive and effective technique to guide therapy and follow-up.
Pathological assessment of chronic kidney disease with DWI: Is there an added value for diffusion kurtosis imaging?

Abstract:Background Chronic kidney disease (CKD) is a worldwide health problem, precise functional and pathological assessment is beneficial to better treatment. Diffusion kurtosis imaging (DKI) can evaluate non-Gaussian diffusion and may help to assess renal pathology and function. Purpose To assess pathological and functional alterations in CKD using DKI compared with diffusion-weighted imaging (DWI). Study Type Prospective study. Population 70 CKD patients and 20 healthy volunteers. Field Strength 1.5 T. Assessment All participants underwent DKI, and apparent diffusion coefficient (ADC), mean diffusivity (MD), and mean kurtosis (MK) of renal parenchyma were acquired. Correlation between renal parenchymal ADC, MD, MK, and estimated glomerular filtration rate (eGFR), pathological scores were assessed. The diagnostic efficacy of ADC, MD, and MK for assessing the degree of renal pathological injury were compared. Statistical Tests ANOVA, Spearman correlation analysis, and ROC curve analysis. Results The cortical ADC, MD were significantly higher than medulla for all participants, whereas medullary MK was significantly higher than cortex (P < 0.01). Whether eGFR reduced or not, renal parenchymal MK were significantly higher in patients than controls (P < 0.05). Positive correlation was found between eGFR and ADC (cortex, r = 0.562; medulla, r = 0.527), and negative correlation between eGFR and MK (cortex, r = -0.786; medulla, r = -0.709) (all P < 0.001). There was positive correlation between MK and glomerular injury (cortex, r = 0.681; medulla, r = 0.652), tubulointerstitial lesion (cortex, r = 0.650; medulla, r = 0.599) (all P < 0.001). For discrimination between mild and m-s renal injury group, the AUC values of ADC, MD, MK were cortex: 0.723, 0.655, 0.864 and medulla: 0.718, 0.581, 0.829. The AUC values of ADC, MD, MK were cortex: 0.708, 0.679, 0.770 and medulla: 0.713, 0.830, 0.780 for differentiating control group from mild renal injury group. Data Conclusion DKI is practicable for noninvasive assessment of renal pathology and function of CKD, DKI offer better diagnostic performance than DWI. 1 2 Evidence Level Technical Efficacy

Abstract:Erythrocyte acetylcholinesterase (AchE) activity was measured in the blood of 36 alcoholic subjects and 41 healthy volunteers. The mean activity in the alcoholics was significantly lower than that in the control subjects. In vitro experiments showed that ethanol inhibited the AchE activity immediately and in proportion to the concentration of ethanol used. Incubation times up to 6 h did not increase the inhibition significantly. Incubation of normal red cells with ethanol for 15 h, followed by washing, showed also that AchE activity was inhibited by the previous exposure to ethanol and that washing did not reduce the inhibitory effect. The possibility is considered that depressed erythrocyte AchE activity may be an early indication of potential disturbances of the autonomic nervous system. The importance of reporting ethanol intake in patients with organophosphorus poisoning is stressed.

Abstract:Purpose: To investigate the potential of Intravoxel incoherent motion diffusion-weighted imaging(IVIM-DWI) for the assessment of renal fibrosis in chronic kidney disease (CKD), using histopathology as a reference standard. Methods: Eighty-five CKD patients and twenty healthy volunteers were recruited in this study. IVIM-DWI was performed in all of the participants, and all of the CKD patients underwent renal biopsy. The mean values of the true diffusion coefficient (D), pseudo diffusion coefficient (D*) and perfusion fraction (1) in the renal cortex and medulla were compared between the CKD patients and healthy volunteers. The Spearman correlation coefficient was calculated to assess the relationship between the D, D*, f values and the estimated glomerular filtration rate (eGFR), serum creatinine level (SCr), 24 h urinary protein level (24 h-UPRO), histopathological fibrosis scores. Results: The D, D* and f values were significantly lower in medulla than in the cortex for all of the participants. All of the IVIM parameters were significantly lower in the CKD patients than in the healthy controls. In the CKD patients, a significant negative correlation was found between the renal parenchymal D, D*,f values and the 24 h-UPRO, as well as between the renal parenchymal 0, f values and the SCr. There was a significant positive correlation between all of the IVIM parameters and the eGFR. All of the IVIM parameters exhibited a significant negative correlation with the histopathological fibrosis score. Conclusion: IVIM-DWI shows great potential in the noninvasive assessment of renal fibrosis in CKD.

Abstract:Dear Sir I read with interest the article by Aleem et al. (1) reporting significantly higher ratings of tardive dyskinesia (TD) and parkinsonism in dexamethasone suppression test (DST) nonsupressors and a significant positive correlation between the post-DST plasma cortisol levels and the movement disorder scales. These findings are significant, as they further support the recently presented hypothesis that diminished melatonin secretion facilitates the emergence of TD (2), and additionally may in part explain the increased risk of TD in patients with a history of affective disorder and in patients with advanced age (3). Attention has recently been focused on the role of melatonin in the regulation of the hypothalamicpituitary-adrenal (HPA) axis. An increased incidence of DST nonsuppression has been reported in depressed patients who also consistently showed the lowest melatonin plasma levels (4,5), and it has been suggested that HPA disinhibition is associated with diminished pineal secretory activity (6). In addition, the incidence of DST nonsuppression has been reported to increase with age (7) and it is possible that this may be associated with the physiological decline of melatonin secretion with aging (8, 9). A history of depression and advanced age have recently been recognized as major risk factors for the development of TD and parkinsonism (3). In addition, several studies have reported spontaneous mood-dependent alterations in the severity of TD where depression was associated with spontaneous exacerbation of TD and mania (which is associated with increased melatonin secretion) (10) with its attenuation (1 1,12). These studies raised the possibility that diminished melatonin secretion facilitates the emergence of TD, and that the increased risk of TD in depressed patients may in part be related to reduced melatonin secretion (13). This hypothesis gained further support by the recent observations in which pinealectomy was associated with an increased incidence and severity of spontaneous perioral movements in drug-naive rats and administration of haloperidol produced further augmentation of the perioral dyskinesias in the pinealectomized rats (2). Numerous studies have consistently found an increased incidence of TD in elderly patients (3, 14), and it has been suggested that TD emerges in the setting of an underlying diffuse organic brain dysfunction (15). An increased incidence of “soft” neurological signs, primitive (developmental) reflexes and cognitive deficits have been reported in schizophrenic patients with TD compared with those without TD (1 5). The incidence of DST nonsuppression increases with age and is markedly elevated in patients with senile dementia compared with age-matched normal controls (16). In addition, melatonin plasma levels have been reported to be markedly diminished in patients with senile dementia (17). Interestingly, the incidence of hypercortisolism was reported to be increased in pinealectomized rats (9) and it has been suggested that pinealectomy may be regarded (with some reservation) as an age-inducing process (9). Thus, the emergence of spontaneous perioral movements in pinealectomized rats, the increased incidence of TD in depressed and elderly schizophrenic patients may all be associated with diminished melatonin-mediated disinhibition of the HPA axis.
Differentiating between malignant and benign renal tumors: do IVIM and diffusion kurtosis imaging perform better than DWI?

Abstract:<h3>Objective</h3>To quantitatively compare the diagnostic values of conventional diffusion-weighted imaging (DWI), intravoxel incoherent motion (IVIM), and diffusion kurtosis imaging (DKI) in differentiating between malignant and benign renal tumors.<h3>Methods</h3>Multiple b value DWIs and DKIs were performed in 180 patients with renal tumors, which were divided into clear cell renal cell carcinoma (ccRCC), non-ccRCC, and benign renal tumor group. The apparent diffusion coefficient (ADC), true diffusivity (D), pseudo-diffusion coefficient (D*), perfusion fraction (f), mean kurtosis (MK), and mean diffusivity (MD) maps were calculated. The diagnostic efficacy of various diffusion parameters for predicting malignant renal tumors was compared.<h3>Results</h3>The ADC, D, and MD values of ccRCCs were higher, while D*, f, and MK values were lower than those of benign renal tumors (all p D* and f values of non-ccRCCs were lower than those of benign renal tumors (p = 0.002 and p D, MD, and MK values between non-ccRCCs and benign renal tumors was not statistically significant (p > 0.05). The ADC, D, MD, and f values of ccRCCs were higher, while MK values were lower than those of non-ccRCCs (all p D, D*, f, MK, and MD were 0.849, 0.891, 0.708, 0.656, 0.862, and 0.838 for differentiating ccRCCs from benign renal tumors, respectively. The AUC values of D* and f were 0.772 and 0.866 for discrimination between non-ccRCCs and benign renal tumors, respectively.<h3>Conclusion</h3>IVIM parameters are the best, while DWI and DKI parameters have similar performance in differentiating malignant and benign renal tumors.<h3>Key Points</h3> • The D value is the best parameter for differentiating ccRCC from benign renal tumors. • The f value is the best parameter for differentiating non-ccRCC from benign renal tumors. • Conventional DWI and DKI have similar performance in differentiating malignant and benign renal tumors.

Abstract:Abstract Objectives To explore the diagnostic performance of diffusion kurtosis imaging (DKI) in evaluating the clinical and pathological characteristics of patients with immunoglobulin A nephropathy (IgAN) compared with conventional DWI. Materials and methods A total of 28 IgAN patients and 14 healthy volunteers prospectively underwent MRI examinations including coronal T2WI, axial T1WI, T2WI, and DWI sequences from September 2020 to August 2021. We measured mean kurtosis (MK), mean diffusivity (MD), and apparent diffusion coefficient (ADC) by using MR Body Diffusion Toolbox v1.4.0 (Siemens Healthcare). Patients were divided into three groups according to their estimated glomerular filtration rate (eGFR) (Group1, healthy volunteers without kidney disease or other diseases that affect renal function; Group2, IgAN patients with eGFR > 60 mL/min/1.73 m 2 ; Group3, IgAN patients with eGFR < 60 mL/min/1.73 m 2 ). One-way analysis of variance, Pearson or Spearman correlation, and receiver operating characteristic curves were applied in our statistical analysis. Results MK Cortex and ADC Cortex showed significant differences between the Group1 and Group2. MK Cortex , MD Cortex , ADC Cortex , MK Medulla , and ADC Medulla showed significant differences between Group2 and Group3. MK Cortex had the highest correlation with CKD stages ( r = 0.749, p < 0.001), and tubulointerstitial lesion score ( r = 0.656, p < 0.001). MD Cortex had the highest correlation with glomerular lesion score ( r = − 0.475, p = 0.011). MK Cortex had the highest AUC (AUC = 0.923) for differentiating Group1 from Group2, and MD Cortex had the highest AUC (AUC = 0.924) for differentiating Group2 from Group3, followed by MK Medulla (AUC = 0.923). Conclusions DKI is a feasible and reliable technique that can assess the clinical and pathological characteristics of IgAN patients and can provide more valuable information than conventional DWI, especially MK Cortex .

Diffusion kurtosis imaging for assessing endometrial fibrosis: a preliminary clinical study

Abstract:PurposeTo investigate the feasibility of using diffusion kurtosis imaging (DKI) for the assessment of endometrial fibrosis.Methods40 patients with hysteroscopy confirmed endometrial fibrosis and 30 healthy women underwent MR examination including the DKI sequence (b = 0, 500, 1000, 1500, and 2000 s/mm2). Endometrial thickness (ET), apparent diffusion coefficient (ADC), corrected ADC (D), and kurtosis of diffusion (K) were measured and compared, and the diagnostic performance of those parameters was evaluated using ROC curves analysis.ResultsPatients with endometrial fibrosis had a thinner endometrium than the healthy controls (P < 0.001). They also had significantly lower ADC and D values and significantly higher K values of the endometrium than the healthy controls (all P < 0.001). ADC, D, K, and ET all performed excellently in diagnosing endometrial fibrosis, with areas under the curve (AUCs) of 0.940, 0.879, 0.860, and 0.853, respectively. ADC showed the highest AUC, demonstrating better diagnostic accuracy than K (z = 2.307, P < 0.05). However, there were no differences in AUC between D, K, and ET, or ADC, D, and ET (all P > 0.05). The reproducibility of ADC, D, and K values in patients with endometrial fibrosis and healthy controls was excellent (ICC 0.951–0.991).ConclusionDKI of the endometrium has promising potential for the noninvasive assessment of endometrial fibrosis.

Economical Surface Balloon Deployment System Concept for Future Scout Class Mars Missions

Abstract:(Abstract) The United States has established a vision for space exploration that includes human missions to the Moon, and eventually to Mars. In order for this vision to turn into reality, the Martian environment must be understood thoroughly to minimize the risk to humans. Although Mars rovers and orbiting satellites are able to gather a significant amount of planetary data, neither platform provides enough information to assess all risks to human exploration, nor do they provide a good balance between mobility and precise, in- situ measurements. Fortunately, many of these requirements can be fulfilled with simple balloon missions. This fact has driven the University of Michigan team to invest in the development of the Mars Balloon Scout (MBS) mission. The objectives of the MBS mission include the search for organic compounds and toxic elements in the atmosphere, meteorology and dust electrification measurements, and exploration of the local Mars geology. The balloon system complements landed and orbital missions because of its ability to perform in-situ atmospheric measurements, survey the surface from low altitude, and its relative simplicity and long mission life span when compared to airplanes. However, since the perception of risk is high, balloons or airplanes have not been utilized in a Mars mission yet. The riskiest parts of a Mars balloon mission are the entry, descent, and deployment (EDD) phases. Typical EDD phases of previous missions, such as the Mars Exploration Rovers (MER) mission, lasted approximately six minutes from entry to touchdown, which leaves limited time for balloon deployment and inflation. In the past, most Mars Balloon Mission Concepts have proposed the idea of inflating the balloon during the entry-descent phase. This method eliminates the risk of rupture of the balloon envelope during deployment on the rocky Martian surface. Although balloon inflation during entry-descent was initially believed to be the best method, tests conducted by NASA's Jet Propulsion Laboratory (JPL) and Wallops Flight Facility (WFF) suggests that the technology has not reached the minimum Technology Readiness Level (TRL) required for a flight project yet. Moreover, the number of tests was limited because of the high costs of stratospheric tests. As a result of the low TRL of midair balloon inflation systems and the high costs and risks associated with the technology development, our group at the University of Michigan has been studying a new concept, the balloon inflation at the Martian surface after landing. This idea is more economical when compared to the conventional deployment during descent, and allows the technology to become flight ready faster and at lower cost. Successful implementation of the new surface balloon inflation technology will allow for a low-risk balloon mission, and could ultimately advance NASA's goal of sending humans to Mars.

The rats in hepatic fibrosis group were randomly divided into 4 subgroups and seven rats per group, the rats were administrated 50％ CCl4 intraperitoneally twice a week to establish hepatic fibrosis , and the four subgroups were injected 2, 4, 6, and 8 weeks, respectively. The rats in the control group were administrated same dose of olive oil for 8 weeks. One rat in hepatic fibrosis group was died of liver failure in the 7th week, and a total of 27 fibrosis experimental rats and 7 control rats were finally included in this study. DKI was performed at the end of the injection period for all rats, the apparent diffusion(D)and kurtosis(K)values were evaluated. Rats were sacrificed immediately after MRI scan and liver specimens were collected. The liver tissues were examined by pathology, liver fibrosis degree, which was graded from S0 to S4, and inflammatory activity, which was graded from G0 to G3 were graded. The difference of D value and K value between different liver fibrosis and inflammatory activity scores was compared by one-way ANOVA(normal distribution)or Kruskal-Wallis test(skewed distribution). Spearman correlation analysis and multiple regression analysis were used to reveal the correlation between DKI parameters and fibrosis staging/necroinflammatory activity grade. To confirm the efficiency of using the ROC curve of DKI parameters to qualify the liver fibrosis grade, which grade was≥3. Results Seven, 6, 6, 7, 8 rats were diagnosed as S0 to S4, respectively. The difference of D value and K value among different fibrosis grades was statistically significant(P<0.05). D value and the degree of fibrosis was negatively correlated(r=-0.650, P<0.01);K value and liver fibrosis grade no correlation(r=0.336, P=0.080). Thirteen, 6, 8, 7 rats were diagnosed as G0 to G3, respectively. D value was negatively correlated with inflammatory activity(r=-0.590, P=0.001);K value was no correlation with inflammatory activity(r=0.169, P=0.389). Compared with inflammatory activity, fibrosis classification was an independent factor in determining D values(P=0.001). ROC analyses demonstrated an area under the curve(AUC)of D value, K value, D value combined with K value in the diagnosis of liver fibrosis grading ≥ 3 level were 0.781, 0.672 and 0.833, respectlively. The sensitivity and specificity of D value combined with K value were 83.3％ and 75.0％, respectively. Conclusion DKI imaging is of great value in the classification of hepatic fibrosis and can be used as an effective method for the diagnosis of fibrosis.
Capability of Arterial Spin Labeling and Intravoxel Incoherent Motion Diffusion-Weighted Imaging to Detect Early Kidney Injury in Chronic Kidney Disease.

Abstract:OBJECTIVES:To prospectively investigate the capability of arterial spin labeling (ASL) and intravoxel incoherent motion diffusion-weighted imaging (IVIM-DWI) for the identification of early kidney injury in chronic kidney disease (CKD) patients with normal estimated glomerular filtration rate (eGFR). METHODS:Fifty-four CKD patients confirmed by renal biopsy (normal eGFR group [eGFR ≥ 90 mL/min/1.73 m2]: n = 26; abnormal eGFR group [eGFR < 90 mL/min/1.73 m2]: n = 28) and 20 healthy volunteers (HV) were recruited. All subjects were examined by IVIM-DWI and ASL imaging. Renal blood flow (RBF) derived from ASL, true diffusion coefficient (D), pseudo-diffusion coefficient (D*), and perfusion fraction (f) derived from IVIM-DWI were measured from the renal cortex. One-way analysis of variance was used to compare MRI parameters among the three groups. The correlation between eGFR and MRI parameters was evaluated by Spearman correlation analysis. Diagnostic performances of MRI parameters for detecting kidney injury were assessed by receiver operating characteristic (ROC) curves. RESULTS:The renal cortical D, D*, f, and RBF values showed statistically significant differences among the three groups. eGFR was positively correlated with MRI parameters (D: r = 0.299, D*: r = 0.569, f: r = 0.733, RBF: r = 0.586). The areas under the curve (AUCs) for discriminating CKD patients from HV were 0.725, 0.752, 0.947, and 0.884 by D, D*, f, and RBF, respectively. D, D*, f, RBF, and eGFR identified CKD patients with normal eGFR with AUCs of 0.735, 0.612, 0.917, 0.827, and 0.733, respectively, and AUC of f value was significantly larger than that of eGFR. CONCLUSION:IVIM-DWI and ASL were useful for detecting underlying pathologic injury in early CKD patients with normal eGFR. KEY POINTS:• The renal cortical f and RBF values in the control group were significantly higher than those in the normal eGFR group. • A negative correlation was observed between the renal cortical D, D*, f, and RBF values and SCr and 24 h-UPRO, while eGFR was significantly positively correlated with renal cortical D, D*, f, and RBF values. • The AUC of renal cortical f values was statistically larger than that of eGFR for the discrimination between the CKD with normal eGFR group and the control group.

Abstract:Objective To investigate the value of intravoxel incoherent motion diffusion-weighted imaging (IVIM-DWI) in the staging of chronic kidney disease(CKD).Methods From May 2016 to April 2017,seventy-two patients diagnosed as CKD according to the criteria of clinical diagnostic and 20 healthy volunteers (control group) underwent routine MRI and IVIM-DWI ( 8 b values, 0 to 800 s/mm2).CKD patients were divided into two groups based on their estimated glomerular filtration rate (eGFR): mild CKD group(45 cases,eGFR≥60 ml·min-1·1.73m-2)and moderate to severe CKD group(27 cases,eGFR<60 ml·min-1·1.73 m-2).The ADC,true diffusion coefficient(D),pseudo-diffusion coefficient(D*),perfusion fraction (f) were measured on both cortex and medulla. The paired-samples t test was used to compare the cortico-medullary difference of the ADC,D,D*and f values in three groups.Differences of the ADC,D, D*and f values among three groups were compared using the one-way analysis of variance (ANOVA). Correlations between eGFR and the IVIM-DWI parameters in CKD were evaluated by using Pearson correlation analysis. ROC was performed to evaluate the diagnostic efficiency of using IVIM-DWI parameters to distinguish CKD with moderate to severe renal impairment from mild renal impairment, as well as distinguish CKD with mild renal impairment from healthy volunteers.Results The cortical ADC,D, D*and f values were significantly higher than that in the medulla in healthy volunteers(all P<0.05). The cortical ADC,D*and f values were significantly higher than that in the medulla in mild CKD group(all P<0.05). The cortical ADC,D and f values were significantly higher than that in the medulla in moderate to severe CKD group (all P<0.05). The ADC,D,D*and f values of cortex and medulla showed significantly differences among three groups(all P<0.05).In CKD patients,no significant correlation was found between medullary D*,f values and eGFR, there was a significant positive correlation between eGFR and cortical ADC,D,D*and f values(r=0.475,0.362,0.625,0.276;all P<0.05),as well as between eGFR and medullary ADC,D values(r=0.427,0.615;P<0.05). The results of the ROC analysis for distinguishing the mild CKD group from the moderate to severe CKD group revealed that the cortical D*value had the highest area under the ROC curve (AUC=0.965), cortical f value showed high sensitivity(92.6%) to distinguish CKD with different degree of renal impairment, with the threshold of 32.99%, and cortical D*value showed high specificity(97.8%)with the threshold of 17.07×10-3mm2/s;the results of the ROC analysis for distinguishing the mild CKD group from healthy volunteers revealed that the cortical D*value had the highest AUC(0.885), medullary ADC value showed high sensitivity (82.2%) to distinguish mild CKD group from healthy volunteers,with the threshold of 1.83×10-3mm2/s,and medullary f value showed high specificity(100.0%)with the threshold of 21.70%,as well as medullary D value showed high specificity(100.0%)with the threshold of 1.75× 10-3mm2/s.Conclusion IVIM-DWI may be useful for CKD early diagnosis and assessing renal function.

Abstract:Purpose: To investigate the utility of diffusion kurtosis imaging (DKI) MRI for evaluation of renal fibrosis in rats with unilateral ureteral obstruction (UUO). Methods: Twenty-five rats had UUO, and ten rats were subjected to sham operation as control. DKI was performed on a 3.0 T MRI scanner on days 1, 3, 5, and 7 after ligation. All rats then underwent F-18-FDG dynamic PET to evaluate unilateral renal function, followed by histological analysis to examine a-smooth muscle actin (alpha-SMA) expression. DKI metrics were assessed among the time points and between two sides, and compared with maximum standardized uptake value (SUVmax), serum levels of creatinine and urea, and fibrosis marker alpha-SMA. Results: Mean kurtosis (MK) on day 7, axial kurtosis (Ka) on days 3 and 7, mean diffusivity (MD) on days 1, 3, 5, and 7, and fractional anisotropy (FA) on days 3, 5, and 7 of cortex and medulla between the UUO and contralateral sides were significantly different (all p < 0.05). Over the course of UUO progression, there were significant changes in Ka, MD and FA of medulla (all p < 0.05). FA of medulla was positively correlated with SUVmax (r = 0.641, p < 0.001), and MD of cortex was negatively correlated with urea (r = - 0.534, p = 0.001). MD of cortex was negatively correlated with alpha-SMA on UUO sides (r = - 0.710, p < 0.001). Conclusions: DKI shows the potential for noninvasive assessment of renal fibrosis and unilateral renal function induced by UUO.

Abstract:Objective Our objective was to evaluate pathological and functional changes in chronic kidney disease (CKD) using diffusion tensor imaging (DTI) at 3 T. Methods There were fifty-one patients with CKD who required biopsy and 19 healthy volunteers who were examined using DTI at 3 T. The mean values of fractional anisotropy (FA) and the apparent diffusion coefficient (ADC) were obtained from the renal parenchyma (cortex and medulla). Correlations between imaging results and the estimated glomerular filtration rate (eGFR), as well as pathological damage (glomerular lesion and tubulointerstitial injury), were evaluated. Results The renal cortical FA was significantly lower than the medullary in both normal and affected kidneys ( p < 0.001). The parenchymal FA was significantly lower in patients than healthy controls, regardless of whether eGFR was reduced. There were positive correlations between eGFR and FA (cortex, r = 0.689, p = 0.000; and medulla, r = 0.696, p = 0.000), and between eGFR and ADC (cortex, r = 0.310, p = 0.017; and medulla, r = 0.356, p = 0.010). Negative correlations were found between FA and the glomerular lesion (cortex, r = -0.499, p = 0.000; and medulla, r = -0.530, p = 0.000), and between FA and tubulointerstitial injury (cortex, r = -0.631, p = 0.000; and medulla, r = -0.724, p = 0.000). Conclusion DTI is valuable for noninvasive assessment of renal function and pathology in patients with CKD. A decrease in FA could identify the glomerular lesions, tubulointerstitial injuries, and eGFR. Preoperative identification of cytokeratin 19 status of hepatocellular carcinoma based on diffusion kurtosis imaging

Abstract:Purpose: To explore the potential value of diffusion kurtosis imaging (DKI) for identification of cytokeratin 19 (CK19) status of HCCs. Methods: This study was approved by the local institute review board and written informed consent was obtained. 73 patients with pathologically confirmed HCCs were included in this prospective study. All the diffusion-weighted (DW) images were acquired using a 3.0-T MR scanner with 4 b-values (0, 800, 1500 and 2000 s/mm2). The mean diffusion value (MD) and mean kurtosis coefficient (MK) from DKI, apparent diffusion coefficient (ADC) from DW imaging (b = 0, 500 s/mm2), and tumor-to-liver signal intensity ratios on ADC map (SIRADC) and DW images with b-value of 500 s/mm2 (SIRb500) were calculated and compared between CK19-positive (n = 23) and CK19-negative (n = 50) HCC groups. Univariate and multivariate logistic regression analyses were used to identify risk factors for the positive expression of CK19. Results: Increased a-fetoprotein level (p = 0.021) and SIRb500 (p = 0.006) and decreased ADC (p = 0.021) and MD (p < 0.001) were significantly correlated with CK19-positive HCCs at univariate analysis. Decreased MD value (odds ratio: 0.042, p = 0.002) and a-fetoprotein level (odds ratio: 5.139, p = 0.015) were the independent risk factors for CK19-positive HCCs at multivariate analysis. The area under the curve of MD value by receiver operating characteristic analysis was 0.823 with a sensitivity of 86.96% and a specificity of 76% for the prediction of CK19-positive HCCs. Conclusion: The decreased MD value derived from DKI is potential quantitative biomarker for predicting CK19-positive HCCs.
