Abstract:ABSTRACT Introduction Forensic nurses routinely provide services to sexual assault victims who are uncertain about reporting their assault to police. The purpose of this study was to determine whether assault characteristics are related to the concerns about police reporting expressed by sexual assault victims who have forensic evidence collected but do not report their assault to police at that time. Methods We analyzed medical records of patients who received services at a hospital-based forensic nursing program between 2010 and 2021. Records were included if a sexual assault evidence kit was collected, the patient declined to report the assault to police, and the patient completed a nonreport sexual assault evidence kit supplement form that included a question asking why they chose not to report the assault ( N = 296). We qualitatively analyzed patients' reasons for not reporting the assault and then used two-variable case-ordered matrices and chi-square analyses to explore relationships between reasons for not reporting and assault characteristics. Results Identified reasons for not reporting included lacking information about the assault, fear of harm/retaliation, and self-blame/minimization. Physical force, drug/alcohol consumption, and victim–offender relationship were related to patients referencing lacking information and fearing harm/retaliation as reasons for not reporting, but not related to the frequency of patients referencing self-blame/minimization. Implications Results indicate that assault characteristics are related to reasons for not reporting at the time of the medical forensic examination. Being aware of these relationships may help forensic nurses provide patient-centered services and anticipatory guidance.

Abstract:An analysis of social media posts using the #WhyIDidntReport hashtag reveals six themes regarding the reasons why survivors of sexual violence do not report the incident to health or social organisations such as police or supervisors. Using just-world theory as a means to examine social reactions to posts of victim's stories, we suggest the reasons for not reporting could be divided into clusters of internal or external barriers. Within the first cluster, three themes reflect survivors who did not report because of external reasons (e.g. victim blaming by the police or other institutions; minimisation of the seriousness of the crime; and reporting costs). In the second cluster, three themes reflect survivors who did not report because of internal reasons (e.g. self-blame, protecting others, and naivety). We find that survivors who did not report sexual violence because of external reasons received significantly more social support, whereas survivors who did not report because of internal reasons received significantly less social support in the form of shares and likes. Overall, these findings support our theorising that the reasons why survivors do not report sexual violence are impactful because, consistent with just-world theorising, they change perceptions of victimhood and therefore the level of social support.

Abstract:Objectives To assess the characteristics of all persons reporting sexual assault at the obstetrics and gynecology emergency departments of Geneva University Hospital and Lausanne University Hospitals (CHUV), between 2018 and 2021. Methods Retrospective analysis of medico-legal reports for all persons reporting sexual assault at the obstetrics and gynecology emergency departments of the HUG and CHUV between 2018 and 2021. Results 962 sexual assault records during the 48-month study, 740 were retained for the analysis. Median age of victims: 24 years (IQR: 19 to 33 years; range: 14 to 93 years) On weekends, the number of assaults is twice as high as weekdays During summer, the monthly number of sexual assault consultations doubled (34%) 58% of the victims said they knew their assaulter, 28% did not, and 14% did not know whether they knew their attacker or not because of amnesia 24% of the patients were unable to recall or specify which types of penetration they were subjected to (if any), because of amnesia 67% of patients reported vaginal penetration, 17% anal, and 21% oral 63% of victims reported some type of substance use (alcohol, drugs) prior to assault Police ordered 40% of the sexual assault medico-legal examinations, while 60% of the victims came to the emergency department seeking care on their own 56% of assaults take place within the home 83% of patients were examined within 72 hours of the sexual assault Ano-genital injuries were found in 28% of patients who underwent a gynecological exam (n=705). 21% of patients who underwent a gynaecological exam and reported anal penetration presented with anal injury. 28% of patients who underwent a gynaecological exam and reported vaginal penetration presented with genital injury Conclusions Such data have shaped the creation of the informatic medical file and prospective study to inform services after sexual assault, prevention and health education strategies for all. Rape Myth Acceptance and Other Barriers to Formally Reporting Sexual Assault Among College Students With and Without Sexual Assault Histories

Abstract:Low rates of reporting sexual assault to law enforcement have been attributed to a culture of rape myth acceptance. Yet, rape myth acceptance rates and specific barriers to reporting have not been examined by sexual assault and reporting histories. This study compared the rape myth acceptance levels of reporting survivors, non-reporting survivors, and individuals without sexual assault histories. The sample consisted of 579 undergraduate students (68.0% White, 72.5% women) at a public university in the southeastern U.S. Differences in non-reporting survivors' experienced barriers to reporting and the perceived barriers of those without sexual assault histories were also examined. Results indicate differences in rape myth endorsement by sexual assault and reporting status, with the highest rape myth adherence rates endorsed by individuals without a sexual assault history and the lowest endorsed by reporting survivors. While non-reporting survivors and those without sexual assault histories shared two of the top three barriers to reporting (i.e., wanted to avoid thinking or talking about it, did not want family or friends to find out), non-reporting survivors were more likely to endorse personal reasons for not reporting, and those without sexual assault histories were more likely to endorse concerns about the law enforcement response to rape. Several barrier factors were identified among non-reporting survivors (i.e., fear of law enforcement involvement, personal reasons, ambiguity of the event, responsibility) and among individuals without sexual assault histories (i.e., risks outweigh benefits, victim blaming, ambiguity). Although both survivors and individuals without sexual assault histories acknowledge the personal, social, and legal risks of reporting a sexual assault to law enforcement, survivors' experienced barriers differed in nuanced ways from the presumed barriers of individuals without sexual assault histories. Findings may enhance sexual assault prevention trainings and awareness campaigns by targeting both rape myth beliefs and specific barriers to reporting of those with and without sexual assault histories.
Women attending the sexual assault treatment unit services in the Republic of Ireland: A 7‐year review

Abstract:Objective Sexual assault is pervasive in today's society, with the numbers of those reporting it increasing. In Ireland, 50% of women will experience some form of sexual violence in their lifetime. We sought to describe the incident details of females presenting to the Sexual Assault Treatment Unit (SATU) network in the Republic of Ireland and to determine associations between incident characteristics and: (1) victim age, (2) presence of injury, (3) victim‐perpetrator relationship, and (4) number of assailants. Methods This was a retrospective cross‐sectional study of all females who attended between 2017 and 2023. Descriptive bivariate analysis was performed. Results There were 5942 female attendances, with an average age of 26 years. The largest age group was women between 18 and 25 years (38.1%, n = 2263). Forensic examinations were performed in 76.6% (n = 4549). Assailants were male in 92% (n = 5469) of incidents, with multiple assailants disclosed in 7.3% (n = 435). Strangers or recent acquaintances were the assailant in 38.5% (n = 2290) of incidents, and close associates in 22.9% (n = 1359). Incidents occurred at the survivor's home in 22% (n = 1306) of incidents, and in the assailant's home in 22.6% (n = 1342). Drug use within 24 h was reported in 15.1% (n = 897) of cases, and alcohol use in 72% (n = 4276). Drug‐facilitated assault was suspected by 16.1% (n = 955). Men after the sexual assault: The journey from medical service to court

Abstract:Background: There may be significant physical and psychological consequences and impacts for males who experience sexual assault as adults, however, published literature in this context is sparse, specifically for the investigative outcomes in the criminal legal process. Method: This clinical audit tracked 138 adult males who presented for forensic and medical sexual assault care from initial presentation to ACT Police investigation and court outcome from 2004 to 2022. Results: There were 103/138 (74.6%) males who attended for medical and forensic care within 72 h of the reported assault. Pre-existing mental health conditions were self-reported in 59/138 (42.7%) males. Nearly half of males 67/138 (48.5%) had a medical evidence kit collected. Males presenting for medical care 44/138 (31.2%) went on to report to ACT Policing, 36/44 (81.2%) of cases did not proceed to court. The most common clearance type was inadequate evidence to proceed (17/44, 38.6%). In 8/44 (18.2%) of adult male cases who subsequently reported to ACT Police, the investigation resulted in entry to the judicial process with a suspect charged with a sexual offence. Five of those cases resulting in a conviction. Conclusions: Many adult male patients presented within timeframes that enabled the provision of time critical medical care and provided an opportunity for forensic medical evidence collection. Many adult males 59 (42.7%) self-reported pre-existing mental health conditions upon initial presentation. Attrition occurred throughout the patient journey with many males not reporting to police and thus not presenting in the legal process. This further emphasises the need for independent sexual violence patient advisors to support adult male victims through the medical and criminal justice process.

Abstract:Journal of Interpersonal Violence, Ahead of Print. As intimate partner violence (IPV) often remains unknown to police, bystanders can play a crucial role in prevention and further escalation of IPV. However, little is known about what brings them into action by reporting incidents of IPV to authorities. As such, we use statements of bystanders who filed reports about IPV incidents to an official domestic violence agency in the Netherlands (N = 78), to investigate the reasoning and motivations for reporting their suspicions. Results show that the reasons for bystanders to report IPV differ depending on the relational dynamics between partners. In situations perceived as intimate terrorism, involving a hierarchical abusive relationship between a man offender and a woman victim, bystanders primarily reported when previous helping initiatives proved inefficient, and they did so to prevent further harm, often particularly in relation to the woman victim. In situations perceived as situational couple violence, involving a symmetrical abusive relationship, bystanders primarily reported when escalation appeared, and they did so to prevent further harm to involved children. We conclude that bystanders report IPV incidents when the need for help is clear, and their motivation for acting concerns the well-being of victims. Our findings add to the growing body of evidence about real-life bystander intervention in emergencies and highlight the need for understanding intervention as context-specific in order to design effective intervention initiatives.

Establishing Sexual Assault Care Centres in Belgium: health professionals' role in the patient-centred care for victims of sexual violence

Abstract:Background: Having ratified the Convention of Istanbul, the Belgian federal government commits itself to the foundation of Sexual Assault Care Centres (SACC). In the light of researching the feasibility of these centres, this study aimed to evaluate the care for victims of sexual violence (SV) in Belgian hospitals anno 2016 as well as to formulate recommendations for the intended model. Methods: Between April and October 2016, a questionnaire was distributed to 159 key health professionals active in 17 different hospitals attached to an AIDS Referral Centre. The survey covered four parts, i.e. the health professionals' profile, their knowledge, attitude and practices, an assessment of the hospital's policy and the caregivers' opinion on the care for victims of SV and on the intended SACCs. Subsequently, a descriptive analysis using 'IBM SPSS Statistics 23' was performed. Results: A total of 60 key health professionals representing 15 different hospitals completed the questionnaire resulting in a response rate of 38%. The results showed a lack of knowledge and practical experience of caregivers' regarding the care for SV victims. Approximately 30% of responders face personal or professional difficulties upon provision of care to victims of SV. Participants evaluate the current care as good, despite the limited psychosocial support, follow-up, insight for the needs of vulnerable groups and support for family, relatives and health professionals. Yet, the majority of health professionals appraise the SACCs as the best approach for both victims and caregivers. Conclusions: By introducing a SACC, the Belgian federal government aims to provide holistic and patient-centred care for victims of SV. Essential in patient-centred health care is an extensive and continuous education, training and supervision of health professionals concerning the care for victims, support for family, relatives and caregivers. At the end and as a result of a participatory process with many professional experts as well as victims, a specific Belgian model, adjusted to the health care system anno 2016 was developed for piloting. The main challenges in establishing SACCs are situated at the institutional and policy level. Collaborating with other institutions and further research are herewith required.
Perceived Reportability of Intimate Partner Violence against Women to the Police and Help-seeking: A National Survey

Abstract:The literature has consistently found that victims of intimate partner violence (IPV) against women are reluctant to make their situation visible and report it to the police. Whether victims perceive and define IPV behaviors as reportable is key to understanding in which cases the police are seen as potential providers of help or whether other potential sources of help (family, friends, professionals, etc.) are preferred. The purpose of the present study was to analyze the influence of perceived reportability of IPV on women's selection of the police as a source of help compared to not seeking help or selecting other sources of help. Structural equation modeling was applied to data from 479 female participants drawn from a nationally representative sample of 1,112 male and female Chilean adolescents and young adults conducted by the National Youth Institute of Chile. The results showed that perceived reportability was a significant predictor of choosing the police as a source of help compared to other informal sources of help, such as family and friends. However, perceived reportability did not affect help-seeking choices between the police and other formal help sources (psychologists, public services). Participants' IPV victimization and attitudes toward IPV also played a specific role in selecting the police as a source of help. These findings highlight the complexity of the visibility of women's IPV victimization to the police and the need to examine its explanatory mechanisms.

Reporting sexual assault in the military: who reports and why most servicewomen don't

Abstract:Background: Public and congressional attention to the Department of Defense's (DoD's) efforts to prevent and respond to sexual assault in the military (SAIM) is increasing. To promote reporting, the DoD offers (1) restricted reporting, allowing confidential reporting to designated military personnel without triggering an official investigation, and (2) unrestricted reporting, which initiates a criminal investigation. Purpose: To identify factors associated with officially reporting SAIM by examining demographic, military, and sexual assault characteristics and survey reporting perceptions and experiences. Differences between active component (AC) (full-time active duty) and Reserve and National Guard (RNG) were explored. Methods: A Midwestern community sample of currently serving and veteran servicewomen (1,339) completed structured telephone interviews. RNG interviews were conducted March 2010 to September 2010 and AC interviews from October 2010 to December 2011. Data were analyzed in 2013. Logistic regression analyses examined demographic, military, and SA characteristics related to SAIM reporting. Bivariate statistics tested differences between AC and RNG. Results: A total of 205 servicewomen experienced SAIM and 25% reported. More AC servicewomen experienced SAIM, but were no more likely to report than RNG servicewomen. Restricted reporting was rated more positively, but unrestricted reporting was used more often. Reporters' experiences corroborated non-reporters' concerns of lack of confidentiality, adverse treatment by peers, and beliefs that nothing would be done. Officers were less likely to report than enlisted servicewomen. Conclusions: Actual and perceived reporting consequences deter servicewomen from reporting. SAIM undermines trust in military units, mission readiness, and the health and safety of all service members.

Abstract:Introduction In the United States, Title IX requires employees of federally funded universities to report any knowledge of university-related nonconsensual sexual experiences (NSEs), whether or not the students involved desire to file a report. Research assessing students' likelihood of reporting sexual violence (SV) to someone at their university under these policies remains sparse; however, extant research indicates students are more likely to report when SV is penetrative, the perpetrator is a professor, and the event is similar to stereotypes of SV but they are less likely to report when alcohol is present or a student is the perpetrator. Alcohol is present in roughly half of all college SV and alcohol is the greatest rape-risk factor for female students. Despite the widespread implementation of MR policies, there is no consensus on how these policies actually impact students' reporting. None of the previous studies on MR have used an experimental design to evaluate the effects of MR on students' likelihood of reporting sexual violence to universities. Objective The current study aimed to fill these gaps by (1) using an experimental design to assess students' likelihood of reporting sexual violence and (2) by evaluating how contextual factors, like perpetrator, violence type and alcohol consumption, impact likelihood of reporting under MR policy Methods 171 female undergraduate students were recruited for the current study. Participants were randomized into two groups: one group was instructed to imagine that their university had MR policies in place while the other was instructed to imagine that university employees were all confidential reporting (CR) sources-- meaning that students could disclose without creating a report. Students read four vignettes alternating perpetrator (professor vs. student) and violence type (penetrative vs. non-penetrative). After each vignette, students indicated their likelihood of reporting if they were the student in the vignette with and without the presence of alcohol. Results Student answers were dichotomized to "would" versus "would not" report and binomial logistic regressions analyzed differences between conditions and vignettes with and without alcohol. Significant differences arose across vignettes, conditions, and alcohol consumption. Conclusions Overall, students who said they would report the SV vignette without alcohol were roughly 4 times more likely to report it with alcohol (p <0.001). Students were significantly more likely to say they would report a professor perpetrating a penetrative assault (PR) over any other vignette; however, perhaps most importantly, students in the MR condition were significantly less likely to do so than students in the CR group (p= 0.032). Students in the MR group were also significantly less likely to report than CR students when alcohol was present (p= 0.004). Despite being most similar to stereotypical SV (e.g., power differential, penetration), students in the MR group reported it significantly less than CR students. As this type of SV is historically most commonly reported, this could have important implications for real-world reporting patterns. MR policy could significantly decrease reporting overall. Abstract:Sexual violence and abuse (SVA) is highly prevalent globally, has devastating and wide-ranging effects on victim-survivors, and demands the provision of accessible specialist support services. In the UK, Rape Crisis England & Wales (RCEW), a voluntary third sector organization, is the main provider of specialist SVA services. Understanding the profile of victim-survivors who are referred to RCEW and their referral outcomes is important for the effective allocation of services. Using administrative data collected by three Rape Crisis Centres in England between April 2016 and March 2020, this study used multinomial regression analysis to examine the determinants of victim-survivors' referral outcomes, controlling for a wide range of potentially confounding variables. The findings demonstrate that support needs, more so than the type of abuse experienced, predicted whether victim-survivors were engaged with services. Particularly, the presence of mental health, substance misuse and social, emotional, and behavioral needs were important for referral outcomes. The referral source also influenced referral outcomes, and there were some differences according to demographic characteristics and socioeconomic factors. The research was co-produced with stakeholders from RCEW, who informed interpretation of these findings. That victim-survivors' engagement with services was determined by their support needs, over and above demographic characteristics or the type of abuse they had experienced, demonstrates the needs-led approach to service provision adopted by RCEW, whereby resources are allocated effectively to those who need them most.

Abstract:Background: Sexually exploited young men are prevalent, yet underrepresented in clinical practice, policy and research. There are multiple barriers that often prevent young men to disclose and to seek or receive support, such as gender norms, limited awareness of victimization and feelings of guilt and shame. Objective: By gaining more insight into the background characteristics of young men who experienced sexual exploitation and their needs, this study aims to raise awareness and to better inform policymakers, care- and educational professionals on adequate prevention and intervention efforts. Methods: Twenty-six young men (age 14–32) who experienced sexual exploitation or other forms of sexual violence in their youth or were at high-risk, participated in this qualitative study that was conducted in The Netherlands. By means of semi-structured interviews and case-file analyses, data was collected to identify risk and protective factors in their life-course and support needs. Results: Several vulnerabilities (e.g. previous experiences of abuse and neglect, household dysfunction, social rejection, running away, substance use) and a lack of positive and supportive relationships led young men into high-risk situations. Among these were involvement in pay dates, criminality and having to survive from day to day, which contributed to victimization. Prevailing gender norms and experiences of stigmatization were often a barrier to express vulnerabilities and to disclose victimization. There was a wide variety in support needs, including peer-to-peer support, therapy, support with day-to-day practices and anonymous support. Conclusions: These results will contribute to adequate prevention and trauma-informed intervention strategies that meet the unique needs of young men at risk for, or victim of sexual exploitation.

Abstract:Background Sexual violence is a crime that affects people of all genders. While focus is frequently on female survivors, it is crucial to acknowledge that males also experience sexual violence and to ensure that gender-sensitive services are available to all survivors. Understanding the prevalence of, and factors associated with, sexual violence against males is a critical first step in addressing this issue. We aim to address the lack of data in relation to sexual violence against males. Methods A cross-sectional study of all male attendances at 6 Sexual Assault Treatment Units (SATU) in the Republic of Ireland over a 6-year period and, where applicable, comparison with corresponding female attendances. Results There were 381 male attendances with an average age of 28.5 years over the study period, representing 7 % of all SATU patients. There was a 24 % increase in male attendances during the study period. 39.1 % presented within 24 h of the assault. 61.9 % reported the crime to the police. Employment status included 37.3 % employed, 24.9 % unemployed, and 26.2 % students, with 86.7 % being Irish nationals. Most incidents occurred on weekdays (53.3 %) and at night (56.7 %). Referrals were primarily from police (55.9 %), and psychological support was provided in 62.3 % of cases. Alcohol (60.4 %) and illicit drugs (20.5 %) were reported before assaults. 18.6 % suspected drug-facilitated assaults. Male assailants constituted 90.1 %, with 13.9 % involving multiple assailants. Male attenders were significantly more likely than females to be assaulted in their assailant's home and to be assaulted by more than one assailant. They were significantly less likely than females to report the crime to the police or to have consumed alcohol. Conclusion To our knowledge, this is the largest case series of male patients attending a sexual assault treatment service yet to be published in the international literature. Male patients are a distinct group that are increasingly accessing SATU services. Significant differences exist between male and female patients' reported experiences of sexual violence. Knowledge of these factors will support appropriate tailoring of treatment & service provision, prevention and awareness strategies to help modify the impact and reduce the incidence of sexual violence in this cohort.

The Impact of Participation in Research About Abuse and Intimate Partner Violence: An Investigation of Harms, Benefits, and Regrets in Young Adolescents in the Western Cape of South Africa

Abstract:There is very little evidence whether recalling and answering questions about abuse or interpersonal violence has a positive or negative impact on participants of such research. This is an important ethical dilemma to ensure an appropriate risk-benefit ratio in research with young people is maintained. We assessed reported harms, benefits, and regrets of young adolescents who participated in a sensitive research project, and compared the harms and benefits in those who had and had not been victims and/or perpetrators of abuse or intimate partner violence. Participants were 3,264 adolescents aged 12 to 15 years in 41 public schools in the Western Cape, South Africa, who completed a survey about intimate partner violence, verbal, physical, and sexual abuse, as part of an HIV prevention cluster randomized controlled trial. The majority of participants reported research participation as beneficial (70.3%), while 27.7% reported harms and 14% regrets. Victims of abuse were more likely than non-victims to report benefits (71.9% vs. 67.1%; p = .02) and harms (31% vs. 20.9%; p < .01) and were less likely to report regret (13.1% vs. 16.7%; p = .02). Perpetrators of abuse were less likely than non-perpetrators to report benefits (67.4% vs. 72.8%; p = .01) and more likely to report harms (36.4% vs. 26.1%; p < .01) and regrets (17.4% vs. 13.3%; p = .01). Our findings suggested that research participation was more likely to have a positive rather than a negative emotional impact on young adolescents and that relatively few regretted participating. Victims and perpetrators of abuse were more likely to report benefits than harms, supporting the ethical appropriateness of ongoing research on abuse and violence. We recommend that further research is required to clarify and standardize terminology and instruments to quantify these kinds of evaluations, including measurement of the severity and intensity of reported benefits, harms and regrets, and the longer term impact of participation in sensitive research.

Abstract:More than 80,000 prisoners each year are sexually victimized during incarceration, but only about 8% report victimization to correctional authorities. Complicating reporting is the fact that half of the perpetrators are staff members. Given the restrictive and highly regulated prison environment, studies that examine reporting behaviors are difficult to conduct and to date information available relied on those who have reported or hypothetical victimization studies. This study uses an ecological framework and archival data from a class action lawsuit of sexual misconduct to determine predictors of reporting. Relying on a subsample of 179 women, chosen because they have all experienced at least 1 penetration offense, we use bivariate and multivariable mixed effects logistic regression analyses to examine individual, assault, and context-level predictors of reporting on 397 incidents of staff sexual misconduct. The final model revealed that that 6 predictors (age at time of assault, physical injury, multiple incidents, perpetrator with multiple victims, the year the abuse began, and the number of years women have left on their sentence) account for 58% of the variance in reporting. Disclosure to inmate peers and/or family and friends was significant in the bivariate results. These findings indicate the need for stronger and more systematic implementation of Prison Rape Elimination Act guidelines and remedies that create and enforce sanctions, including termination, for staff violating policy and state law. Abstract:Sexual assault is a widespread and serious problem in our society. Yet instead of delivering justice to its victims the criminal justice system can too often be a source of further distress for victims. It is well known that sexual assault complainants have too often experienced the criminal justice system as a place that re-traumatizes and even harms them and that many victim-survivors choose not to report the crimes of sexual violence committed against them as a result.Sexual assault is very often an experience of trauma. Trauma has a neurobiological impact – it affects our brains and our nervous-systems. For this reason, it is imperative that those working within the criminal justice system have a basic appreciation of the effects and impact of trauma in relation to victims of sexual assault in order to process sexual assault cases more effectively and receive evidence in these cases in a more fair and impartial manner.Recently, an important and significant paradigm shift in our knowledge about victim reactions to traumatic events like sexual assault, including impact on memory, has deepened understanding and led to improved practices which assist with developing more effective criminal justice system responses to sexual assault cases. Insights from the neurobiology of trauma has assisted professionals working in a wide range of fields to better understand the psychological and physiological responses of crimes such as sexual assault, and how these affect victim response. This in turn, has facilitated more trauma-informed service delivery and more appropriate and effective interventions, from first responders like police through to advocates and legal personnel in court rooms.The focus of this report is on outlining some of the key findings from the body of knowledge of neuroscience, and applying them to the issue of sexual assault and its impacts on victims. More specifically, we review and highlight some of the significant developments which have emerged from the field of the neurobiology of trauma as they relate to the unique crime of sexual assault and apply their relevance to the many challenges surrounding the criminal processing of sexual assault cases. Victim reactions to sexual assaults are still not well understood in society at large. These misunderstandings unfortunately continue to persist in the legal system. In fact, they contribute to ongoing deficiencies in criminal justice system processing of sexual assault cases, leading to imperfect justice for victims and survivors. This has been described as the “justice gap” for sexual assault cases. We argue that this justice gap can, in part, be closed by moving towards a more trauma-informed criminal justice system. This will lead to more just outcomes for sexual assault complainants and also allow for the fuller realization of the impartiality and fairness that criminal trials are supposed to provide for all participants.
Barriers to Seeking Medical Care Among Youth Victims of Sexual Violence

Abstract:Purpose: Youth victims of sexual violence often experience physical health problems but are unlikely to receive medical care. However, victims' reasons for not accessing medical care have been understudied. We examined barriers to seeking medical care among youth victims who contacted the National Sexual Assault Online Hotline. Methods: We used archival data about one-on-one chat sessions with youth victims between June 2018 and February 2020. Hotline staff described victims' reasons for not accessing medical care via an online assessment. We coded and qualitatively examined these reasons using data about 520 victims with physical health concerns who had not received medical care. Results: Victims' barriers were rooted in individual beliefs and contextual realities reflected in three categories: (1) perception that medical care was not needed, (2) anticipated consequences of seeking medical care, and (3) inability to physically access medical care. Victims who perceived care as unnecessary did not understand the health implications of abuse or minimized their need for care. Anticipated consequences included privacy and control over disclosure, stigmatization, retaliation from the perpetrator, family disruptions, and retraumatizing medical treatment. Victims unable to physically access care were uncertain how to access care independently, lacked social support, or were prevented from care by the perpetrator. Discussion: Medical treatment can ameliorate acute health concerns and increase safety, but youth victims perceived numerous barriers to care. Several barriers implied unintended consequences of child maltreatment policies, including mandatory reporting. Trauma-informed policy and practice are critical for improving victims' access to medical care and other support services.

Abstract:In 1973/74 in the district of Traunstein a study was conducted which, like a corresponding study in Mannheim, aimed at the determination of the frequency of mental disorders among patients in general practices. Three research psychiatrists evaluated 1.274 patients in 18 general practices by means of an interview developed by Goldberg et al. (1970) and diagnosed patients according to the ICD (International Classification of Diseases). The study was based on a representative sample of all patients who consulted a physician during a 14-day period. About 1/3 of all patients (32%) showed psychiatric symptoms according to the evaluation of the interviewers, but only about 1/4 (26%) according to the opinion of the general practitioners. The bulk lies in neurotic and psychosomatic illnesses, comprising more than half (57%) of the mentally ill. About 10% each are listed for psychogeriatric disorders, affective and other psychoses as well as personality disorders. The results largely coincide with those for comparable studies in Mannheim and Norway.
