Abstract:Objective: Prior to 2007, forced sex with male children in South Africa did not count as rape but as "indecent assault", a much less serious offence. This study sought to document prevalence of male sexual violence among school-going youth. Design: A facilitated self-administered questionnaire in nine of the 11 official languages in a stratified (province/metro/urban/rural) last stage random national sample. Setting: Teams visited 5162 classes in 1191 schools, in October and November 2002. Participants: A total of 269,705 learners aged 10-19 years in grades 6-11. Of these, 126,696 were male. Main outcome measures: Schoolchildren answered questions about exposure in the last year to insults, beating, unwanted touching and forced sex. They indicated the sex of the perpetrator, and whether this was a family member, a fellow schoolchild, a teacher or another adult. Respondents also gave the age when they first suffered forced sex and when they first had consensual sex. Results: Some 9% (weighted value based on 13915/127097) of male respondents aged 11-19 years reported forced sex in the last year. Of those aged 18 years at the time of the survey, 44% (weighted value of 5385/11450) said they had been forced to have sex in their lives and 50% reported consensual sex. Perpetrators were most frequently an adult not from their own family, followed closely in frequency by other schoolchildren. Some 32% said the perpetrator was male, 41% said she was female and 27% said they had been forced to have sex by both male and female perpetrators. Male abuse of schoolboys was more common in rural areas while female perpetration was more an urban phenomenon. Conclusion: This study uncovers endemic sexual abuse of male children that was suspected but hitherto only poorly documented. Legal recognition of the criminality of rape of male children is a first step. The next steps include serious investment in supporting male victims of abuse, and in prevention of all childhood sexual abuse.

Abstract:Background: There is limited research in South Africa that has analysed the experiences of child and youth victims of crime and sexual victimisation who accessed formal response services through the victim empowerment programme. Objective: The primary aim of this article is to explore children and youth's experiences of accessing the Victim Empowerment Programmatic services, through the criminal justice system in South Africa. The focus of the article is on childrens' disclosure, reporting and social support. Participants and setting: The participants in the study are male and female victim/survivor of crime between the ages of 12-17 years who has been through a Victim Empowerment Programme (VEP) for at least 12 months. The Victim Empowerment Programme is a governmental programme located within the National Department of Social Development in South Africa. Method: The full study is a mixed method study but the children's(youth) participation in the study is limited to qualitative methods. The data analysis utilised a thematic approach and ATLAS.ti software. Results: The findings revealed four themes, namely, (1) children's (as youth) perceptions of the presence and frequency of rape in communities, (2) disclosure and reporting rape; (3) interlinkage of disclosure and reporting of child rape and child sexual victimisation; and (4) seeking social and professional support to deal with the trauma of child (youth) rape and child sexual victimisation. The findings showed that children (youth) perceive their community environments as unsafe spaces where they are exposed to crimes such as rape and burglary; that there are delays in disclosure of sexual victimisation and victims/survivors are still dealing with the trauma of rape as they report the crime and navigate the pathways of the criminal justice system. Conclusion: In conclusion, this study confirms the delay in disclosure of childhood (youth) sexual victimisation and identifies the complex tension for children (youth) who disclose, that they are often pressured to report the crime, and journey through an adversarial criminal justice system, and so 'breaking the silence' is often unpredictable and emotionally and psychologically costly for children and youth.
Who is killing South African men? A retrospective descriptive study of forensic and police investigations into male homicide

Abstract:Not much is known about the perpetrators of male homicide in South Africa, which has rates seven times the global average. For the country’s first ever male homicide study we describe the epidemiology of perpetrators, their relationship with victims and victim profiles of men killed by male versus female perpetrators. We conducted a retrospective descriptive study of routine data collected through forensic and police investigations, calculating victim and perpetrator homicide rates by age, sex, race, external cause, employment status and setting, stratified by victim-perpetrator relationships. For perpetrators, we reported suspected drug and alcohol use, prior convictions, gang-involvement and homicide by multiple perpetrators. Perpetrators were acquaintances in 63% of 5594 cases in which a main perpetrator was identified. Sharp objects followed by guns were the main external causes of death. The highest rates were recorded in urban informal areas among unemployed men across all victim-perpetrator relationship types. Recreational settings including bars featured prominently. Homicides clustered around festive periods and weekends, both of which are associated with heavy episodic drinking. Perpetrator alcohol use was reported in 41% of homicides by family members and 50% by acquaintances. Other drug use was less common (9% overall). Of 379 men killed by female perpetrators, 60% were killed by intimate partners. Perpetrator alcohol use was reported in approximately half of female-on-male murders. Female firearm use was exclusively against intimate partners. No men were killed by male intimate partners. Violence prevention, which in South Africa has mainly focused on women and children, needs to be integrated into an inclusive approach. Profiling victims and perpetrators of male homicide is an important and necessary first step to challenge prevailing masculine social constructs that men are neither vulnerable to, nor the victims of, trauma and to identify groups at risk of victimisation that could benefit from specific interventions and policies.

Rape Myth Acceptance: Gender and Cross-National Comparisons Across the United States, South Africa, Ghana, and Nigeria

Abstract:Many studies indicate that rape-supportive beliefs persist and influence sexually aggressive behaviors and hostility toward women. Despite the plethora of studies, cross-cultural knowledge remains sparse. The present study examined rape myth acceptance across gender and countries (i.e., United States, South Africa, Ghana, and Nigeria). An online questionnaire was administered to a convenience sample of 699 respondents in the four countries. Results suggested that respondents in Nigeria were the most likely and respondents in United States were the least likely to endorse rape myths. Respondents in South Africa were less likely than respondents in Ghana to endorse the myth that the female victim of rape "asked for it" and that the male perpetrator "didn't mean to" rape the female victim. Although men were more likely than women to endorse rape myths, female respondents in Nigeria endorsed the myths "she asked for it" and "he didn't mean to" more than did male respondents in Nigeria. In general, exposure to various patriarchal structures and ideologies; differences in preventive, protective, and punitive policy responses to gender-based violence; repressive cultural and religious practices; reinforcement of demeaning stereotypes against women; and psychological assimilation of oppressive policy, values and beliefs may be responsible for cross-national differences. Cross-country differences in rape myths suggest the need for formal and informal intervention in vulnerable countries. International transfer of effective policies and programs for combating gender-based violence in protective countries might lead to considerable changes in vulnerable countries and help to shift the focus from patriarchal to egalitarian views of women.

There remains a paucity of research examining the barriers to reporting sexual assault among gay and bisexual male assault survivors, despite much higher sexual assault victimization rates for gay and bisexual men. Few researches have examined barriers for gay Latino and/or Black male sexual assault survivors, specifically focusing on the role of racism and homophobia. To address this gap in literature, our study examined the specific barriers to reporting sexual assault for gay Latino (including Black) male sexual assault survivors, with a focus on how their multiple marginalized identities influenced their decision-making process. The study consisted of 14 gay Latino sexual assault survivors from six cities in the United States who participated in one-on-one semistructured in-depth interviews. Our data revealed three themes: (a) Bias and Discrimination, which describes how racism, homophobia, and gendered ideology serve as barriers to reporting sexual assault; (b) the Unjust Reporting Process focuses on the unnecessarily difficult, complex, and unclear timely reporting process; and (c) Retraumatization, which describes how participants were likely to be retraumatized by reporting. Content analysis was conducted and methodology consistent with this analysis was completed. Our findings provide recommendations to enhance the reporting process for gay Latino male sexual assault survivors which include law enforcement practices (e.g., Training tailored for engaging with LGBTQIA+ individuals and male survivors) as well as agency-wide practices (e.g., trainings and groups specifically for gay and bisexual men of color). All practices should be survivor centered and trauma-informed.

Abstract:Monte Carlo techniques are the gold standard for studying light propagation in turbid media. Traditional Monte Carlo techniques are unable to include wave effects, such as diffraction; thus, these methods are unsuitable for exploring focusing geometries where a significant ballistic component remains at the focal plane. Here, a method is presented for accurately simulating photon propagation at the focal plane, in the context of a traditional Monte Carlo simulation. This is accomplished by propagating ballistic photons along trajectories predicted by Gaussian optics until they undergo an initial scattering event, after which, they are propagated through the medium by a traditional Monte Carlo technique. Solving a known problem by building upon an existing Monte Carlo implementation allows this method to be easily implemented in a wide variety of existing Monte Carlo simulations, greatly improving the accuracy of those models for studying dynamics in a focusing geometry.
Men after the sexual assault: The journey from medical service to court

Abstract:Background: There may be significant physical and psychological consequences and impacts for males who experience sexual assault as adults, however, published literature in this context is sparse, specifically for the investigative outcomes in the criminal legal process. Method: This clinical audit tracked 138 adult males who presented for forensic and medical sexual assault care from initial presentation to ACT Police investigation and court outcome from 2004 to 2022. Results: There were 103/138 (74.6%) males who attended for medical and forensic care within 72 h of the reported assault. Pre-existing mental health conditions were self-reported in 59/138 (42.7%) males. Nearly half of males 67/138 (48.5%) had a medical evidence kit collected. Males presenting for medical care 44/138 (31.2%) went on to report to ACT Policing, 36/44 (81.2%) of cases did not proceed to court. The most common clearance type was inadequate evidence to proceed (17/44, 38.6%). In 8/44 (18.2%) of adult male cases who subsequently reported to ACT Police, the investigation resulted in entry to the judicial process with a suspect charged with a sexual offence. Five of those cases resulting in a conviction. Conclusions: Many adult male patients presented within timeframes that enabled the provision of time critical medical care and provided an opportunity for forensic medical evidence collection. Many adult males 59 (42.7%) self-reported pre-existing mental health conditions upon initial presentation. Attrition occurred throughout the patient journey with many males not reporting to police and thus not presenting in the legal process. This further emphasises the need for independent sexual violence patient advisors to support adult male victims through the medical and criminal justice process.

Abstract:Journal of Interpersonal Violence, Ahead of Print. When examining sexual assault, men are often hidden from the literature. The current study employed a mixed-methods, convergent parallel design to examine the experiences of 22 male survivors of sexual assault (mean age = 44.19, SD = 13.28, range 18–65; 91% European American; 50% heterosexual). Survivors were administered an online survey using quantitative and qualitative questions to assess rape myths, gender roles, self-esteem, self-efficacy, resiliency, coping, overall mental and physical health, and diagnostic criteria for alcohol abuse, post traumatic stress disorder (PTSD), and sexual dysfunction. Three key themes emerged from the integration of quantitative and qualitative data: (a) Help is Elusive, (b) Internal Struggles and External Strengths, and (c) Living with Clinical Diagnoses. This study adds to an important area of the literature that increases understanding of men's experiences with sexual violence and honors the voices of these survivors.

Rape Myth Acceptance and Other Barriers to Formally Reporting Sexual Assault Among College Students With and Without Sexual Assault Histories

Abstract:Low rates of reporting sexual assault to law enforcement have been attributed to a culture of rape myth acceptance. Yet, rape myth acceptance rates and specific barriers to reporting have not been examined by sexual assault and reporting histories. This study compared the rape myth acceptance levels of reporting survivors, non-reporting survivors, and individuals without sexual assault histories. The sample consisted of 579 undergraduate students (68.0% White, 72.5% women) at a public university in the southeastern U.S. Differences in non-reporting survivors' experienced barriers to reporting and the perceived barriers of those without sexual assault histories were also examined. Results indicate differences in rape myth endorsement by sexual assault and reporting status, with the highest rape myth adherence rates endorsed by individuals without a sexual assault history and the lowest endorsed by reporting survivors. While non-reporting survivors and those without sexual assault histories shared two of the top three barriers to reporting (i.e., wanted to avoid thinking or talking about it, did not want family or friends to find out), non-reporting survivors were more likely to endorse personal reasons for not reporting, and those without sexual assault histories were more likely to endorse concerns about the law enforcement response to rape. Several barrier factors were identified among non-reporting survivors (i.e., fear of law enforcement involvement, personal reasons, ambiguity of the event, responsibility) and among individuals without sexual assault histories (i.e., risks outweigh benefits, victim blaming, ambiguity). Although both survivors and individuals without sexual assault histories acknowledge the personal, social, and legal risks of reporting a sexual assault to law enforcement, survivors' experienced barriers differed in nuanced ways from the presumed barriers of individuals without sexual assault histories. Findings may enhance sexual assault prevention trainings and awareness campaigns by targeting both rape myth beliefs and specific barriers to reporting of those with and without sexual assault histories.
Adverse Childhood Experiences, Aggression, Empathy, and Psychopathology in Adult Males Accused of Rape

Abstract:Background: Investigating the underlying psychosocial factors is a cornerstone of planning need-based intervention for adult males accused of rape. Unresolved debates on its etiology, mediation, or interaction among causal psychosocial variables fuel curiosity to scrutinize it further. Hence, we studied potential influence of and relation between adverse childhood experiences, aggression, empathy, and psychopathology in adult males accused of rape in India and investigated the risk factors for the same. Methods: With a correlational research design, 40 literate and consenting adult males medically confirmed for rape were recruited using convenient sampling. The assessment was done on Adverse Childhood Experiences, Aggression Questionnaire, Symptom Checklist-90, and Interpersonal Reactivity Index. Descriptive statistics, Pearson's product-moment correlation, and stepwise linear regression analysis were calculated. Results: Approximately 75% of the participants experienced at least one category of Adverse Childhood Experiences. Scores above cut-off points were obtained on anger, hostility, fantasy, and personal distress. Significant correlations were obtained between adverse childhood experiences and psychopathology; between hostility and psychopathology, perspective taking, and personal distress; and in case of indirect aggression, with perspective taking and empathetic concerns. Regression analysis revealed that an increase in Symptom Checklist-90 global scores increases hostility and that lower personal distress predicts higher scores on hostility on Aggression Questionnaire. Conclusions: Adverse childhood experiences, aggression, and psychopathology play a critical role and, therefore, should be included as core components of the prevention of rape or relapse prevention programs at the community level.

Abstract:This study compared the qualitative nature of women and men's sexual violence (SV) victimization, the types of experiences captured and missed on the Sexual Experiences Survey-Short Form Victimization (SES-SFV) across genders, and common interpretations of the SES-SFV items. Fifty-four university students (31 women, 21 cis men, 2 trans men) who had recent unwanted (but not necessarily nonconsensual) sexual experiences thought out loud while privately completing the SES-SFV. They also typed descriptions of experiences reported on SES-SFV items or similar experiences when nothing was reported on an item. Results indicated that women's victimization was more frequent and severe than cis men's, except when men were victimized by men. Although verbal coercion was common across genders, event descriptions indicated that women's verbal coercion experiences were more often harsh and part of a partner's ongoing SV or coercive control. The findings suggest that quantitative measurement can mask important gender differences in victimization and (based on analysis of false positives and negatives) may underestimate rape and attempted rape experiences, especially women's. Findings suggested that responding to the SES-SFV was not traumatic or distressing. However, participants sometimes expressed confusion about the items and interpreted them in unintended ways.

Abstract:Abstract Background Gender-based violence (GBV) is a crucial global public health challenge disenfranchising women and girls from enjoying their fundamental human rights, thereby threatening their well-being. While the concept of GBV does not imply that violence is always unidirectional, literature shows that women and girls are the most common victims of this type of violence. One in three women, globally, have suffered physical or sexual violence by an intimate partner or non-partner. Evidence has shown that a number of women who experience GBV varies widely, with 37% being in Eastern Mediterranean, 37.7% in South-East Asia, 29.8% in America, 36.6% in Africa, 44% in sub-Saharan Africa (SSA) and 53% in South Africa. Methods Using a semi-structured face-to-face interview with fifteen conveniently sampled adult males, who met the selection criteria, this study explored men’s conceptualisation of GBV in Alexandra Township, using qualitative research methods. Results Socio-economic factors and evolving cultural dynamics were perceived to be among the key factors aggravating gender-based violence. Participants viewed poverty and substance abuse as the main causes of violence towards women, a phenomenon tied to the growing frustration emanating from men’s inability to provide for their families. Cultural factors related to the patriarchal system and diminishing value of respect between men and women were identified as root causes of GBV. The participants also blamed the government for what they considered to be “too many rights” for women, resulting in men exerting their authority through abuse. Participants also expressed concerns over feminisation of GBV, asserting that men fall prey to GBV too. Partner infidelity and insecurities also contributed to GBV. Conclusion The study results provided important insights on how men conceptualize GBV in Alexandra Township, South Africa. These results revealed that socio-economic conditions, alongside some gender stereotypes are pervasive and shape how men view GBV in Alexandra Township. This evidence is necessary for developing interventions aimed at curbing GBV and may also be suggestive of the need to redesign programmes targeting men, so that certain stereotypes can be uprooted.

Abstract:Background: Sexually exploited young men are prevalent, yet underrepresented in clinical practice, policy and research. There are multiple barriers that often prevent young men to disclose and to seek or receive support, such as gender norms, limited awareness of victimization and feelings of guilt and shame. Objective: By gaining more insight into the background characteristics of young men who experienced sexual exploitation and their needs, this study aims to raise awareness and to better inform policymakers, care- and educational professionals on adequate prevention and intervention efforts. Methods: Twenty-six young men (age 14–32) who experienced sexual exploitation or other forms of sexual violence in their youth or were at high-risk, participated in this qualitative study that was conducted in The Netherlands. By means of semi-structured interviews and case-file analyses, data was collected to identify risk and protective factors in their life-course and support needs. Results: Several vulnerabilities (e.g. previous experiences of abuse and neglect, household dysfunction, social rejection, running away, substance use) and a lack of positive and supportive relationships led young men into high-risk situations. Among these were involvement in pay dates, criminality and having to survive from day to day, which contributed to victimization. Prevailing gender norms and experiences of stigmatization were often a barrier to express vulnerabilities and to disclose victimization. There was a wide variety in support needs, including peer-to-peer support, therapy, support with day-to-day practices and anonymous support. Conclusions: These results will contribute to adequate prevention and trauma-informed intervention strategies that meet the unique needs of young men at risk for, or victim of sexual exploitation.

Abstract:OBJECTIVE To investigate the influence of left heart function in obstructive sleep apnea by CIH, and explore left heart function and blood pressures before or after CPAP. METHODS 75 OSA were divided into two groups: 40 patients blood pressures were coincident with diagnostic standard of hypertension recording; 35 patients blood pressures were non-hypertension recording. The control group included 30. healthy adults. The blood pressures before and also after sleep, left ventricular ejection fraction (LVEF), shortening fraction (FS), E, A and E/A were compared with those of normal control subjects. Analyzed BP, LVEF and E/A after CPAP. RESULTS (1) The blood pressure soon after getting up in the morning (150.80+/-20.73/108.0+/-15.34) mm Hg (1 mm Hg=0.133 kPa) was significantly higher than that before sleep (134.16+/-18.33/90.09+/-11.24) mm Hg (P<0.001). (2) Multiple parameters including LVEF, FS and E/A, were impaired in OSA patients relative to the control subjects (P<0.05 or P<0.01), OSA affected cardiovascular function directly; E/A was significantly decreased in obstructive sleep apnea unallied hypertension than that in normal control subjects; E/A was conspicuously decreased in obstructive sleep apnea associated hypertension comparing with that in obstructive sleep apnea unallied hypertension. These showed OSA affected cardiovascular function directly, hypertension aggravated the drop in cardiovascular function. 3. After CPAP in 6 months, it (142.59+/-15.34/96.52+/-9.81) mm Hg was significantly decreased than that before treatment (150.80+/-20.73/108.0+/-15.34) (P<0.001); LVEF (59.70+/-11.1)% was increased than that before treatment (56.40+/-9.74)% (P<0.05) and E/A 1.16+/-0.25 was increased. (2) CPAP treatment had important sense in the treatment of hypertension and cardiovascular function, and improved the life quality.

Abstract:Male rape is a relatively under-discussed topic in scientific literature, despite its significant relevance and prevalence worldwide, including in Brazil. To inform public health and safety policies, this study aimed to analyze cases of male rape using data from SINAN, the Information System for Notifiable Diseases, a division of the Brazilian Ministry of Health, for the years 2010–2022. Our findings reveal a 469 % increase in male rape cases in the country over the study period, with a predominance of cases in the state of São Paulo. Also, a higher prevalence of cases was observed among individuals aged 5–9 years. Furthermore, we identified a correlation between the occurrence of rape and certain factors, including individuals with less than 4 years of formal education if they were over the age of 50, and those who had a history of being sexually abused between the ages of 5 and 14. Additionally, this study reinforces the prevalence of rape cases occurring within domestic environments and perpetrated by individuals closely related to the victims. This study contributes to filling the gap in research on male rape in Brazil, laying the foundation for the development of strategies to prevent and combat rape in the country.

Abstract:Journal of Interpersonal Violence, Ahead of Print. The vast majority of theoretical and empirical research assessing sexual assault (SA) focuses on man-against-woman SA (MWSA), leaving other forms such as woman-against-woman SA (WWSA) understudied. Relatively simple questions, such as the relative frequency of WWSA for women of different sexual orientations, the tactics (e.g., coercion, force), or forms (e.g., groping, vaginal penetration) employed in WWSA remain unanswered. To address these issues, 268 women (approximately one-third of each bisexual, lesbian, and heterosexual) completed an online survey reporting on whether or not they had experienced a variety of nonconsensual sexual experiences in their adult lifetime, asking specifically about perpetration by men and women. Results indicated that although SA perpetrated by men was far more common, a substantial minority of participants reported having experienced WWSA. In general, tactics and forms employed were similar, regardless of the gender of the perpetrator or the woman's sexual orientation, with two exceptions: bisexual women had experienced higher rates of MWSA, and lesbian women had experienced more SA in the form of oral sex compared to other groups. Implications for theory, empirical research, and especially community and clinical practice are discussed.
