Abstract:Hemiplegic shoulder pain (HSP), which occurs in most patients with hemiplegia, causes considerable distress and worsens outcomes in rehabilitation. Although they have received the treatments such as anti-inflammatory drugs or physical therapy, many of the individuals remain suffering from shoulder pain 6 months after acute stroke event. In this retrospective study, we evaluated the effectiveness of ultrasound guided subacromial-subdeltoid (SASD) bursa injections with botulinum toxin type A (BoNT/A) compared to steroids for refractory HSP. The data were collected retrospectively by reviewing the patient's medical records and pain questionnaires in our rehabilitation center. In total, 38 patients who received ultrasound guided SASD bursa injection (BoNT/A group, n=18; corticosteroid group, n=20) were included. The pain visual analog scale (VAS) score at rest and during arm passive abduction, Fugl-Meyer score of upper limbs (F-M score) were evaluated before, 2, 4, 8, and 12 weeks after injection. Both 2 groups obtained a significant improvement of VAS score at rest or during arms passive abduction compared to baseline score (within group compare, P<.05). There were no significant differences of pain score improvement between two groups at week 2, 4, 8, and 12 after injection either at rest or during passive arm abduction (between 2 groups compare, P>.05). There were also no differences in results of the post treatment F-M score between 2 groups (between 2 groups compare, P>.05). Similarly, during the follow-up period no collateral effects were reported after BoNT/A injection. SASD bursa BoNT/A injection can substantially reduce the pain as corticosteroid in patients with HSP. BoNT/A injection could be a useful strategy for replacing steroids as a treatment for refractory HSP especially in the patients who cannot tolerate the steroids injection.

Abstract:Introduction Long head of biceps brachii tendinopathy (LHBT) is characterised by persistent pain and disability of shoulder joint, impairing patients' quality of life. Extracorporeal shock wave therapy (ESWT) is a non-invasive treatment, which promotes tissue regeneration and repair. However, ESWT has a side effect that often causes short-term pain and swelling in the treatment area. It is known that the effects of Kinesio taping (KT) on relieving swelling and pain. Due to insufficient clinical evidence from current limited studies, this randomised controlled study aims to explore the effects of ESWT combined with KT on upper limb function during individuals with LHBT. Methods and analysis A 2x2 factorial design, double-blind, randomised controlled trial will be conducted. A total of 144 participants will be randomly allocated into one of four groups (KT+ESWT, KT+sham ESWT, sham KT+ESWT or sham KT+sham ESWT) to participate in a 4-week treatment programme. Measurements will be taken at pretreatment (baseline), immediately after treatment and 6 weeks after treatment. The primary endpoint will be the Constant-Murley score (CMS), the secondary endpoints will include the pain Numerical Rating Scale, range of motion, pressure pain threshold and soft tissue hardness of biceps, speed test and global rating of change. Repeated measures analysis of variance will be used to compare differences among the effects of different interventions. Ethics and dissemination Ethics approval was obtained from the Ethics Committee of the Shanghai Sixth People's Hospital Affiliated to Shanghai Jiao Tong University School of Medicine. In addition to international conference reports, findings will be disseminated through international publications in peer-reviewed journals. Trial registration number ChiCTR2100051324.

Comparative study of ultrasonic-guided betamethasone local injection and extracorporeal shock wave therapy in post-stroke hemiplegic shoulder pain: a randomized clinical trial

Abstract:Objective: This study aimed to compare the efficacy and safety of ultrasound-guided local injection (UGLI) of betamethasone around the shoulder and extracorporeal shock wave therapy (ESWT) in patients with hemiplegic shoulder pain. Method: Forty-two patients with hemiplegic shoulder pain were randomly divided into the UGLI group (N = 21) and the ESWT group (N = 21). In the UGLI group, betamethasone was injected at the pain point around the shoulder under ultrasonic localization. In the ESWT group, an extracorporeal shock wave was performed at the pain points around the shoulder for 20 min of time, once a week, for 4 consecutive weeks. Both groups received rehabilitation training. The visual analog scale (VAS) evaluation was performed at baseline, 1 h, 1 week, and 1 month after treatment. Furthermore, Neer shoulder joint function scores, upper limb Fugl-Meyer assessment (FMA), modified Barthel index (MBI), Hamilton Depression Scale (HAMD), the MOS-item short-form health survey (SF-36) scores, and serum expression level of cytokine were evaluated at baseline and 1 month after treatment. Results: After 1-h treatment, the UGLI group showed a greater effect on the degree of pain than the ESWT group (P = 0.017). After 4 consecutive weeks of intervention, the UGLI group showed a significant improvement in the serum level of cytokine expression compared with the ESWT group (P < 0.05). The range of motion (ROM) of the hemiplegic shoulder (P < 0.05) has no difference between the two groups (P > 0.05). Conclusion: The ultrasonic-guided betamethasone local injection and extracorporeal shock wave both can improve hemiplegic shoulder pain. However, the UGLI can induce a more cytokine expression level.

Abstract:Objective: The purpose of this study was to evaluate the clinical efficacy of extracorporeal shock waves combined with traditional Chinese medicine bone-setting manipulation for external humeral epicondylitis. Methods: Ninety-two patients with external humeral epicondylitis were randomly divided into an observation group and a control group. Patients in the control group were treated with extracorporeal shock waves while those in the observation group with traditional Chinese medicine bone-setting manipulation based on the control group. Patients in both groups were evaluated by the Visual Analogue Scale (VAS), Mayo Elbow Performance Score (MEPS), and Disabilities of the Arm, Shoulder, and Hand Questionnaire (DASH) before and after treatment. The inflammatory factors such as IL-6, IL-10, TNF-ɑ, and clinical outcomes were contrasted before and after treatment. Results: There were statistically significant differences in VAS score, MEPS score, and DASH score between the two groups before and after treatment (p < 0.05). The observation group exhibited a more pronounced improvement in each score compared to the control group. Post-treatment, the inflammatory factors of both groups were significantly lower than pre-treatment levels (p < 0.05), with the observation group showing a more noticeable decrease. The overall effectiveness of the observation group was higher than that of the control group, and the difference was statistically significant (p < 0.05). Conclusion: The combination of extracorporeal shock wave therapy and traditional Chinese medicine bone-setting manipulation can effectively alleviate pain symptoms and improve dysfunction caused by external humeral epicondylitis, while also reducing inflammatory factor expression. This combined treatment may prove more effective than extracorporeal shock wave therapy alone. Clinical Trial: Registration: ChiCTR2200066075.
An analysis of hospital-acquired bacteraemia in intensive care unit patients in a university hospital in Kuwait.

Abstract:An analysis of hospital-acquired bacteraemia among ICU patients was carried out over a two-year period in order to determine the incidence, associated mortality rate and susceptibility pattern of causative pathogens. There was a high incidence of bacteraemia, occurring in 127 (18.4%) of 692 patients. Mortality attributable to nosocomial bacteraemia was 52% of the total 79 deaths from all causes. The highest mortality rate (58.5%) occurred in patients with fungal infections, whilst death from Gram-negative bacteraemia was only 17%. Over 98% of patients had underlying disease. Nearly half (46.8%) of 267 organisms isolated were Gram-positive. In comparison, Gram-negative bacteria accounted for 36.6% and the rest (17.6%) were fungi (mainly Candida albicans). The majority of the bactereamic episodes were monomicrobial (90.2%). Coagulase-negative staphylococci (CNS) were the commonest pathogens isolated, representing 32.6% of all organisms. Inducible beta-lactamase producing organism (Enterobacter spp. 9.7%, Serratia marcescens 6.7%, Klebsiella pneumoniae 6% and Pseudomonas aeruginosa 6%) formed the bulk of Gram-negative bacteria. In contrast, Escherichia coli (7.5%) and K. pneumoniae (4%) were the commonest Gram-negative bacteria from hospital-acquired bacteraemia in the general hospital population. The majority (80%) of CNS were resistant to methicillin (MRSE) but susceptible to vancomycin; they were relatively resistant to erythromycin, clindamycin and beta-lactams antibiotics. Whilst Gram-negative organisms were relatively susceptible to imipenem (85%), ciprofloxacin (88%) and amikacin (87%), they had unacceptably low levels of susceptibility to cefuroxime (59.3%), cefotaxime (71%), ceftazidime (60.9%), and piperacillin (51.1%). This study shows that hospital-acquired bacteraemia in ICU patients carries a poor prognosis. Information regarding the infective agents and their susceptibility in the ICU setting is valuable for the selection of empirical therapy before culture and susceptibility results are known.
A Comparative Study on the Clinical Outcomes of Arthroscopic Extensor Carpi Radialis Brevis Insertional Debridement and Extracorporeal Shock Wave Therapy in the Treatment of Refractory Tennis Elbow

Abstract:Objective To compare the clinical outcomes of arthroscopic extensor carpi radialis brevis（ECRB） insertional debridement and extracorporeal shock wave therapy（ESWT） in the treatment of lateral epicondylitis.Methods Fifty patients with chronic external humeral epicondylitis between year 2017 and 2020 were selected, and randomly divided into an arthroscopic ECRB insertional debridement surgery group（n=25） and ESWT group（n=25）,according to a random number table. Before the treatment,as well as 3,6 and 12 months after the treatment,both groups were evaluated the elbow function using the visual analog scale（VAS）, disabilities of the arm, shoulder and hand（DASH）score, and Mayo elbow performance score（MEPS）. Spearman correlation coefficient was used to analyze the correlation between the functional score and basic data of patients.Results After the treatment, the average DASH and VAS scores of both groups decreased significantly, while the average MEPS score increased significantly. Three months after the treatment, no significant difference were found between the two groups in the average DASH score（46 ± 12 vs 49 ± 11,P=0.39）,while the average MEPS score of the ESWT group was significantly higher than the surgery group（87 ± 7 vs 80± 7,P=0.002）,with the average VAS score of the ESWT group significantly lower than the latter（2± 1 vs 3 ± 1,P=0.002）. However,6 and 12 months after the treatment,there was no significant difference between the two groups in the average DASH, MEPS and VAS scores. Correlation analysis showed that the elbow joint function scores were related to gender,age and body mass index of the ESWT group,while the functional scores of the arthroscopic debridement group were related to age,the number of local seals and the degree of ECRB lesions.Conclusions Both treatments in this study are effective,and ESWT is superior to the other in terms of relieving pain in the early stage after treatment.
Extracorporeal Shock Wave in Combination with Arthroscopy for Calcified Supraspinatus Tendonitis

Abstract:Objective: Exploring the therapeutic effect of extracorporeal shockwave combined with Arthroscopic Surgery on calcified supraspinatus tendinitis. Materials and Methods: Sixty patients with calcific supraspinatus tendinitis who received treatment in our hospital from June 2022 to June 2023 were randomly divided into two groups. All patients had disease lasting more than 6 months. The control group received extracorporeal shockwave therapy (ESWT), while the observation group, after undergoing arthroscopic debridement of calcific deposits in the joint, began receiving the same ESWT as the control group after 2 weeks. The differences in Visual Analog Scale (VAS) score, University of California at Los Angeles (UCLA) score, and Constant–Murley score between the two groups before and after treatment were recorded and compared. Results: Before treatment, there was no significant difference in VAS score, UCLA score, and Constant–Murley Scale (CMS) score between the two groups of patients (P > 0.05); compared with before treatment, both groups of patients showed a significant decrease in VAS scores after 1 and 2 months of treatment (P < 0.05). After 1 and 2 months of treatment, the VAS scores of the observation group were significantly lower than the ones of the control group (P < 0.05). Compared with before treatment, the UCLA score and CMS score of both groups of patients significantly increased after 1 and 2 months of treatment (P < 0.05). After 1 and 2 months of treatment, the UCLA score and CMS score of the observation group were significantly higher than those of the control group (P < 0.05). Conclusion: The combination of extracorporeal shockwave and arthroscopy has a significant therapeutic effect on calcified supraspinatus tendinitis, helping to improve shoulder joint function and effectively alleviate pain in patients. Effectiveness of Extracorporeal Shock Wave Therapy and kinesio taping in calcific tendinopathy of the shoulder: a randomized controlled trial

Abstract:Background: Extracorporeal Shock Wave Therapy (ESWT) is effective in the treatment of calcific tendinopathy of the rotator cuff, eliciting an analgesic/anti-inflammatory action and promoting tissue regeneration. Kinesio taping (KT), another recently-introduced rehabilitative tool, exerts an analgesic and biomechanical action on joints and muscles. ESWT and KT may have a synergic effect when used in combination, but the effectiveness of the association has not been established. Aim: The aim of this study was to test if the association of KT with ESWT is superior to ESWT alone in the treatment of rotator cuff calcific tendinopathy. Design: Randomized controlled trial. Setting: Rehabilitation Institute outpatients. Population: Forty-two patients with rotator cuff calcific tendinopathy were randomly assigned to the experimental group (ESWT+KT, N.=21) or control (ESWT, N.=21). Methods: In the experimental group, patients underwent three sessions (once a week for 3 weeks) of ESWT with KT applied at the end of each session. Controls underwent three sessions of ESWT only. All patients were assessed before treatment (T0) and at 1 (T1), 4 (T2) and 12 weeks (T3) after the end of treatment with the following outcome measures: a visual analogue scale (VAS), the Disabilities of the Arm, Shoulder and Hand (DASH) questionnaire, Subjective Shoulder Rating Questionnaire (SSRQ), and Oxford Shoulder Score (OSS). Results: Both groups showed significant improvement in all outcome measures, but the time course differed between the two groups. At T1 vs. T0, the improvement was significantly better in ESWT+KT than ESWT on VAS (P=0.007), DASH (P<0.0001) and SSRQ (P=0.0001). Successive improvements at T2 vs. T1 and T3 vs. T2 did not differ significantly between the groups. At the end of follow-up, ESWT+KT still showed significantly greater improvement than ESWT on VAS (P=0.02) and SSRQ (P=0.038). Conclusions: KT associated with ESWT seems to improve the recovery in rotator cuff calcific tendinopathy with a faster therapeutic response compared to ESWT only. Clinical rehabilitation impact: Our results suggest the effectiveness of using KT as adjuvant therapy to ESWT in rotator cuff calcific tendinopathy, through enhancing the short-term analgesic action and the medium- to long-term biological-regenerative effects.
Comparison of Ultrasound-guided Transverse Carpal Ligament Release via Different Approaches in Carpal Tunnel Syndrome: A Prospective, Randomized, Controlled Trial

Abstract:Background: Ultrasound-guided transverse carpal ligament (TCL) needle release has been demonstrated to be an effective treatment for carpal tunnel syndrome (CTS). However, no existing evidence has investigated the comparative efficacy of different release approaches. Objective: To compare the efficacy of ultrasound-guided TCL needle release via different approaches for patients with mild to moderate CTS over a 12-month follow-up. Study design: A prospective, randomized, controlled trial. Setting: Outpatient clinic at a university hospital. Methods: Sixty-four patients with mild to moderate CTS (> 3 months' duration) were randomly assigned to either the long-axis group (one session of ultrasound-guided corticosteroid injection plus long-axis TCL needle release) or the short-axis group (one session of ultrasound-guided corticosteroid injection plus short-axis TCL needle release) in a one-to-one ratio. The primary outcomes were the symptom severity scale (SSS) and functional severity scale (FSS) scores of the Boston Carpal Tunnel Questionnaire (BCTQ). The secondary outcomes were electrophysiological studies, including distal motor latency (DML) and sensory nerve conduction velocity (SNCV), cross-sectional area (CSA) of the median nerve (MN), and patient-reported successful clinical response. Assessments were performed before treatment and at one, 3, 6, and 12 months after treatment. Results: A total of 60 patients (30 per group) completed the trial. Compared to the baseline, both groups exhibited improvement in SSS, FSS, SNCV, DML, and CSA at all follow-up time points, with statistical differences for SSS, FSS, and SNCV at 3, 6, and 12 months (P < 0.05), DML at 6 and 12 months (P < 0.05), and CSA at each follow-up time point (P < 0.05). Compared to the short-axis group, the long-axis group exhibited more improvement in SSS and FSS at all follow-up time points, with statistical differences at 3, 6, and 12 months (P < 0.05), and in SNCV and DML at 6 and 12 months (P < 0.05). Although the long-axis patients exhibited more improvement in their wrists' CSAs, the intergroup differences were nonsignificant at all follow-up time points (P > 0.05). Four patients in the short-axis group experienced recurrent symptoms and underwent surgery at 12 months, whereas no recurrence was observed in the long-axis group. Limitations: A relevant future trial with a longer follow-up period than this one used is still necessary. Conclusions: Ultrasound-guided TCL needle release via the long-axis approach appears to be more effective than the short-axis approach for treating mild to moderate CTS.
Short-term outcomes of extracorporeal shock wave therapy for the treatment of chronic non-calcific tendinopathy of the supraspinatus: a double-blind, randomized, placebo-controlled trial

Abstract:Background: There is evidence supporting the use of extracorporeal shock wave therapy (ESWT) in calcific tendinopathy of the rotator cuff, but the best current evidence does not support its use in non-calcifying tendinopathy. We conducted a randomized placebo-controlled trial to investigate the efficacy and safety of low energy ESWT for non-calcifying tendinopathy of the rotator cuff. Methods: 20 patients with non-calcifying supraspinatus tendinopathy (NCST) were randomized to an active or a sham treatment group. Physical, blood, roentgenographic, and MRI examinations of the shoulder were conducted to verify that patients met the inclusion and exclusion criteria. These examinations were repeated six and twelve weeks after treatments. Effectiveness was determined by comparison of the mean improvement in the Constant and Murley score (CMS) between the treatment and the placebo groups at three months. Safety was assessed by analyzing the number and severity of adverse events. Results: All the patients completed the investigation protocol. At the final follow-up, significant improvement in the total CMS score and most of the CMS subscales was observed in the ESWT group when compared to the baseline values. Significantly higher total CMS, and significantly higher scores for CMS pain and ROM were observed in the ESWT group when compared to the placebo. No serious adverse events were noted after ESWT. Conclusions: Patients suffering from NCST may benefit from low energy ESWT, at least in short-term. The application protocol of ESWT is likely to play a key-role in a successful treatment. Future investigations should be undertaken on the long-term effects of this technique for the treatment of NCST. Trial registration: Current Controlled Trials ISRCTN41236511.
Probiotic Supplementation in Mothers of Very Low Birth Weight Infants

Abstract:Abstract Objective Oral probiotic supplementation reduces the risk of necrotizing enterocolitis (NEC) in preterm infants. Concerns about safety and purity of probiotic preparations have limited their use in preterm infants. The authors administered probiotic bacteria to mothers of preterm infants, thereby avoiding the risks of direct exposure of infants to probiotic bacteria. Design This prospective, randomized, double blind, placebo-controlled trial at the Tel Aviv Medical Center (June 2007–November 2009) examined the effects of maternal oral probiotic supplementation on the incidence of NEC, death, and sepsis in very low birth weight (VLBW) infants fed with maternal breast milk. Mothers were assigned to supplementation with Lactobacillus acidophilus and Bifidobacteria lactis 2 × 1010 CFU/d or to placebo starting from 1 to 3 days postpartum. The primary outcome measures were NEC, sepsis, and death. Results In total 49 mothers of 58 VLBW infants were recruited. A total of 25 infants were in the probiotic group and 33 in the placebo group. The overall incidence of Bell stage II to III NEC was 12%, with an incidence of 4% in the infants of the probiotic group and 18.2% in the placebo group (p = 0.12), respectively. Sepsis and mortality rates were similar. Conclusion Postpartum maternal supplementation with probiotic bacteria may decrease the incidence of NEC in breastfed infants.

Abstract:Background: The present study compared the clinical effect of extracorporeal shock wave therapy (ESWT) with that of ultrasound (US)-guided shoulder steroid injection therapy in patients with supraspinatus tendinitis. We hypothesized that the two treatments would show comparable results. Methods: The inclusion criteria were age over 20 years and diagnosis of supraspinatus tendinitis using US. Ultimately, 26 patients were assigned using blocked randomization: 13 in the US-guided shoulder injection group and 13 in the ESWT group. Treatment outcomes were evaluated using the pain visual analog scale (pVAS), the American Shoulder and Elbow Society (ASES) score, and the Constant score at baseline and at 1 and 3 months after the procedure. Results: At 1 month after the intervention, pVAS, ASES, and constant score were significantly higher in the US-guided shoulder injection group than in the ESWT group, but not at 3 months after the intervention. Both groups showed clinically significant treatment effects at 3 months after the intervention compared to baseline. No significance was shown using equivalence testing. Conclusions: US-guided shoulder injection therapy was not superior to ESWT therapy. Considering the complications and rebound phenomenon of steroid injections, interventions using ESWT may be a good alternative to treat patients with supraspinatus tendinitis.
Clinical, sonographic and molecular changes in calcific tendinitis of the shoulder following extracorporeal shockwave therapy: a prospective case-control study

Abstract:Background: Extracorporeal shockwave therapy (ESWT) is the primary treatment for calcific tendinitis of the shoulders, but what are the effects of clinical, sonographic, and molecular markers following ESWT in treating calcific tendinitis of the shoulder? Methods: Twenty-eight patients were categorized into radiodense and radiolucent subgroups. In addition, clinical assessments included the visual analogue scale (VAS), Constant-Murley (CM) score, American Shoulder and Elbow Surgeon (ASES) score, sonographic evaluation, and serum enzyme-linked immunosorbent assay (ELISA). The participants completed a one-year follow-up. All data were collected before and after treatment. Results: After one year of follow-up, all patients showed notable improvement in VAS, CM, and ASES scores, with no significant clinical variations among the subgroups. However, the radiolucent group showed significant complete resorption and size reduction at the final follow-up. Sonographic evaluation revealed improved tissue perfusion and reduced calcification from 3 to 12 months in all patients, including those in the radiolucent group, but complete resorption of calcific deposits did not occur. The percentage of tissue perfusion was improved at 1 and 3 months after ESWT. There were no significant differences in the levels of the molecular markers interleukin-1 beta (IL-1 β) or IL-33, but the level of insulin-like growth factor 1 (IGF-1) was notably increased at 1 and 3 months post-ESWT. The BMP7 level was increased at 3 months and was then decreased significantly at 6 and 12 months. Conclusion: ESWT improved symptoms, reduced calcification, enhanced tissue perfusion, and promoted angiogenesis and BMP7 activity. In particular, it benefited radiolucent type patients with better calcification resorption. Partial resorption led to improvements in transparency, and a second ESWT session at 3 months was recommended for optimal results.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Objectives</h3><p>To evaluate whether the therapeutic effect of ultrasound-guided injections with 10 mg or 40 mg triamcinolone acetonide (TA) was dose-dependent in patients with idiopathic mild to moderate carpal tunnel syndrome (CTS).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Design</h3><p>Prospective, double-blinded, randomized controlled study with 12 weeks of follow-up.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Setting</h3><p>Rehabilitation outpatient clinic of a single medical center.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Participants</h3><p>Patients with CTS (N=56).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Intervention</h3><p>Participants were randomly assigned to two treatment groups for injection: (A) 40 mg TA + 2% lidocaine hydrochloride or (B) 10 mg TA + 2% lidocaine hydrochloride.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Main outcome measures</h3><p>Participants were evaluated using Visual Analogue Scale (VAS) and Boston Carpal Tunnel Questionnaire (BCTQ, including Symptom Severity Scale (SSS) and Functional Status Scale (FSS)) at baseline, 6 and 12 weeks after injection). Nerve conduction studies (NCS), including parameters of distal motor latency, amplitude of compound motor action potential, amplitude of sensory nerve action potential and sensory nerve conduction velocity of median nerve, and the patient's subjective impression of improvement were recorded before injection and 6 and 12 weeks after injection.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>No significant differences were observed in baseline demographic characteristics and clinical evaluations. The parameters in group A and B at the baseline, 6, and 12 weeks were: (1) SSS: 2.17±0.14, 1.19±0.04, and 1.34±0.09 and 1.87±0.11, 1.21±0.07, and 1.26±0.04; (2) FSS: 1.63±0.07, 1.27±0.06, and 1.33±0.08 and 1.50±0.10, 1.18±0.05, and 1.26±0.05; VAS: 6.4±0.3, 2.2±0.3, and 3.0±0.1 and 6.7±0.3, 2.0±0.3, and 3.1±0.3, respectively, and significantly decreased after 6 and 12 weeks in both treatment groups (p<0.05). All parameters of NCS improved in both groups after 12 weeks (p<0.05). VAS, BCTQ, and NCS did not show significant inter-group differences after 6 and 12 weeks.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>In patients with idiopathic mild to moderate CTS, ultrasound-guided injection with 10 and 40 mg TA yield similar improvements in BCTQ, VAS, and NCS at the 12-week follow-up.</p>

Assessment of Clinical Analgesic Levels and Serum Biomarkers in Patients with Rheumatoid Arthritis: A Randomized Controlled Trial Comparing the Efficacy of Diclofenac and Methotrexate Combined Therapy with Extracorporeal Shockwave Therapy

Abstract:Background: Rheumatoid arthritis (RA) is one of the most common forms of arthritis. Extracorporeal shockwave therapy (ESWT) has been identified as a viable alternative therapeutic approach in light of the present protracted clinical course of pharmacological treatment, and changes in levels of marker proteins in the blood samples of RA patients can be utilized to assess treatment outcomes. Methods: A randomized controlled trial was conducted involving forty patients diagnosed with rheumatoid arthritis (RA) who were assigned randomly to two groups. The first group received a combination of diclofenac and methotrexate (MTX) consisting of 25 mg of diclofenac administered thrice daily and 15 mg of MTX administered once weekly. Individual follow-up assessments were carried out after 7 and 14 days. Meanwhile, patients in the second group underwent two sessions of Extracorporeal Shockwave Therapy (ESWT), with a 7-day interval between sessions. Evaluations were conducted on day 7 and day 14. Patients who displayed pain control and stability were advised to continue the treatment, whereas those who had inflammation and discomfort were administered specific medications, and their progress was closely monitored until day 28. Blood samples were collected from both groups prior to treatment, after the first treatment, and after the second treatment. Four marker proteins (NRP-1, CELF-6, COX-2, and RGS-1) and two inflammatory cytokines (IL-6 and IL-17) were measured using western blot and RT-PCR techniques. A statistical analysis was conducted on the levels of specific proteins and inflammatory factors before and after treatment to evaluate its impact. Result: Both groups exhibited statistically significant differences in the serum level of target biomarkers before and after the intervention. However, the ESWT group demonstrated a more noticeable effect, while the diclofenac + MTX group exhibited a delayed anti-inflammatory effect compared to ESWT. Conclusion: Both treatments significantly improved joint function, relieved pain, and reduced inflammation in patients. However, ESWT demonstrated a more prominent clinical analgesic effect compared to the combination treatment of diclofenac and MTX. Furthermore, ESWT produced a more immediate and noteworthy anti-inflammatory impact by regulating NRP-1 expression, a trophic factor receptor that facilitates vascular endothelial cell migration and tissue repair through angiogenesis, and regulating RGS-1 to limit inflammatory signal transmission and immune cell activation.
Comparison of Radial Extracorporeal Shock Wave Therapy and Traditional Physiotherapy in Rotator Cuff Calcific Tendinitis Treatment

Abstract:Objectives: This study aims to investigate the efficacy of radial extracorporeal shock wave therapy (rESWT) in relieving pain and improving range of motion (ROM) and functionality besides conventional physiotherapy methods in the treatment of chronic rotator cuff calcific tendinitis (RCCT). Patients and methods: We studied 80 patients (35 males, 45 females; mean age 53.3±9.6 years; range, 40 to 70 years) with chronic RCCT. Patients were randomly divided into two groups: rESWT group (n=40) treated with conventional physiotherapy and rESWT, and control group (n=40) treated only with a conventional physiotherapy program. The traditional physiotherapy program included ultrasound, transcutaneous electrical nerve stimulation, shoulder joint ROM and stretching exercises, and ice applications. All patients received a total of 20 treatments, five days a week for four weeks. rESWT was applied once a week for four weeks in total. Before and after treatment, all patients were evaluated for age, height, weight, Body Mass Index (BMI), pain intensity with a Visual Analog Scale, shoulder ROM, and functional disability status with the shortened version of the Disabilities of the Arm, Shoulder and Hand questionnaire (QuickDASH). Results: Mean BMI value of the participants was 26.1±3.0 kg/m2. Although all parameters of the patients in both groups improved significantly, patients in the rESWT group had a statistically significant improvement in pain, ROM and QuickDash scores (p<0.001, p<0.001, and p<0.001, respectively). Conclusion: We assume that rESWT is an effective and noninvasive method of reducing pain and increasing ROM and functional status without the need for surgery.
Rehabilitation of An Analgesic Bracelet Based on Wrist-Ankle Acupuncture in Patients with Rotator Cuff Injury: A Randomized Trial

Abstract:Objective. To evaluate the effect of transcutaneous electrical nerve stimulation (C-TENS) in the rehabilitation of rotator cuff injury. Methods. A total of 60 patients diagnosed with mild to moderate rotator cuff injury were randomly divided into the control group and test group. Both groups received conventional rehabilitation treatment including physical therapy, joint mobilization, interventional therapy, and family recovery training. The test group additionally received TENS treatment on the upper area 4 or 5 of the wrist, based on the wrist-ankle acupuncture (WAA) theory. The results of the visual analogue scale (VAS), shoulder range of motion (ROM), and Constant-Murley score (CMs) were collected before and after the 1st treatment and after the 5th treatment. The differences of those indicators between the two groups were analyzed statistically. Results. The VAS scores of measurement points after treatment were all improved compared with those at baseline. There was a significant difference between the two groups after the 1st and the 5th treatment ( p < 0.05 ), and the improvements in the test group were better than those in the control group. The ROM of flexion, extension, abduction, adduction, internal rotation, and external rotation after the 1st treatment and 5th treatment in both groups were all improved compared with those at baseline. There was no significant difference between two groups. The CMs of the two groups after the 5th treatment were all improved compared with those at baseline. There was no significant difference between two groups. No adverse events occurred during the treatment. Conclusion. Electrical stimulation on the wrist combined with conventional rehabilitation is more effective in relieving pain than the conventional rehabilitation alone. Electrical stimulation on the wrist combined with conventional rehabilitation has no obvious effect in improving shoulder joint mobility and shoulder function.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Objectives</h3><p>The study aimed to investigate whether the shoulder tendons changed their elasticity following ultrasound guided peri-tendinous or intra-bursal corticosteroid injections.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Design</h3><p>Post-hoc secondary analysis of a double-blinded, randomized controlled study with 3 months of follow-up.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Setting</h3><p>Outpatient rehabilitation clinic.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Participants</h3><p>Patients with subacromial impingement syndrome (N=60).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Interventions</h3><p>Patients with unilateral shoulder pain were randomly assigned to receive standard ultrasound guided subacromial or dual-target corticosteroid injections. The supraspinatus tendons were exposed to 40 mg triamcinolone acetonide in the formal group, whereas the long head of the biceps brachii tendons (LHBT) and supraspinatus tendons were individually infiltrated by 20 mg triamcinolone acetonide in the latter group. Patients' bilateral shoulder were divided into Group 1 (n=30, receiving standard subacromial injections), Group 2 (n=30, receiving dual-target injections) and Group 3 (n=60, without injections).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Main Outcome Measures</h3><p>Strain ratio of LHBT and supraspinatus tendons using ultrasound elastography.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>The repeated-measures analysis of variance revealed no intra-group difference of the strain ratio of the LHBT (<em>p</em> = 0.412 for Group 1; <em>p</em> = 0.936 for Group 2; <em>p</em>= 0.131 for Group 3) and supraspinatus tendon (<em>p</em> = 0.309 for Group 1; <em>p</em> =0.067 for Group 2; <em>p</em>= 0.860 for Group 3) across the three time points. Treating Group 3 as the reference, the linear mixed model revealed no significant changes in tendon elasticity after either the standard subacromial injection (<em>p</em>= 0.205 for the LHBT and <em>p</em>= 0.529 for the supraspinatus tendon) or the dual-target injection (<em>p</em>=0.961 for the LHBT and <em>p</em>= 0.831 for the supraspinatus tendon).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>Elasticity of the LHBT and supraspinatus tendons, is unlikely to change after a single dose of peri-tendinous or intra-bursal corticosteroid injections. Future studies with a shorter follow-up interval are needed to validate whether corticosteroid injections can cause transient changes of the tendon's elasticity.</p>

Extracorporeal Shock Wave Therapy versus Local Corticosteroid Injection and Platelet-Rich Plasma in The Treatment of Supraspinatus Tendinopathy

Abstract:Background: A chronic degenerative musculoskeletal condition known as tendinopathy is widespread in both the general public and sportsmen. The most typical therapies used to facilitate tendon repair and regeneration are extracorporeal shockwave therapy (ESWT), platelet-rich plasma (PRP), and local corticosteroid injection. Objective: The aim of the current study was to evaluate the efficacy of ESWT to corticosteroid and PRP injections in the treatment of supraspinatus tendonitis. Patients and methods: A total of 60 subjects suffering from supraspinatus tendinopathy were recruited and divided into 3 groups: 20 patients who received platelet-rich plasma (PRP) injections, 20 patients who received local corticosteroid injection, and 20 patients who received ESWT using radial ESWT. Results: In the PRP group results showed a statistically significant improvement in visual analogue scale (VAS), tendon thickness, tear size, and American Shoulder and Elbow Surgeons patient self-report section (ASES-p). The VAS, US examination, and ASES-p all indicated statistically significant improvements after corticosteroid injection. The VAS, US test, and ASES-p all revealed statistically significant improvements in the ESWT group. Conclusion: In comparison to steroid injection and ESWT, PRP therapy offers an extra favorable short-term benefit for the treatment of supraspinatus tendinopathy. ESWT is also a simple, effective, and noninvasive alternative for the treatment of supraspinatus tendinopathy. Two months following therapy, local corticosteroid injections reduced discomfort and increased functional abilities.
