Abstract:The COVID-19 pandemic has caused huge impact on public health and significantly changed our lifestyle. This is due to the fast airborne oro-nasal transmission of SARS-CoV-2 from the infected individuals. The generation of liquid aerosolized particles occurs when the COVID-19 patients speak, sing, cough, sneeze, or simply breathe. We have developed a novel aerosol barrier mask (ABM) to mitigate the spread of SARS-CoV-2 and other infectious pathogens. This Aerosol Barrier Mask is designed for preventing SARS-CoV-2 transmission while transporting patients within hospital facilities. This mask can constrain aerosol and droplet particles and trap them in a biofilter, while the patient is normally breathing and administrated with medical oxygen. The system can be characterized as an oxygen delivery and mitigation mask which has no unfiltered exhaled air dispersion. The mask helps to prevent the spread of SARS-CoV-2, and potentially other infectious respiratory pathogens and protects everyone in general, especially healthcare professionals.

Abstract:The Coronavirus Disease 2019 (COVID-19) pandemic renewed interest in infectious aerosols and reducing risk of airborne respiratory pathogen transmission, prompting development of devices to protect healthcare workers during airway procedures. However, there are no standard methods for assessing the efficacy of particle containment with these protective devices. We designed and built an aerosol bio-containment device (ABCD) to contain and remove aerosol via an external suction system and tested the aerosol containment of the device in an environmental chamber using a novel, quantitative assessment method. The ABCD exhibited a strong ability to control aerosol exposure in experimental and computational fluid dynamic (CFD) simulated scenarios with appropriate suction use and maintenance of device seals. Using a log-risk-reduction framework, we assessed device containment efficacy and showed that, when combined with other protective equipment, the ABCD can significantly reduce airborne clinical exposure. We propose this type of quantitative analysis serves as a basis for rating efficacy of aerosol protective enclosures.

Infectious Aerosol Capture Mask as Environmental Control to Reduce Spread of Respiratory Viral Particles

Abstract:Negative pressure isolation of COVID-19 patients is critical to limiting the nosocomial transmission of SARS-CoV-2; however, airborne isolation rooms are limited. Alternatives to traditional isolation procedures are needed. The evaluation of an Infectious Aerosol Capture Mask (IACM) that is designed to augment the respiratory isolation of COVID-19 patients is described. Efficacy in capturing exhaled breath aerosols was evaluated using laboratory experimentation, computational fluid dynamics (CFD) and measurements of exhaled breath from COVID-19 patients and their surroundings. Laboratory aerosol experiments indicated that the mask captured at least 99% of particles. Simulations of breathing and speaking showed that all particles between 0.1 and 20 µm were captured either on the surface of the mask or in the filter. During coughing, no more than 13% of the smallest particles escaped the mask, while the remaining particles collected on the surfaces or filter. The total exhaled virus concentrations of COVID-positive patients showed a range from undetectable to 1.1 × 106 RNA copies/h of SARS-CoV-2, and no SARS-CoV-2 aerosol was detected in the samples collected that were adjacent to the patient when the mask was being worn. These data indicate that the IACM is useful for containing the exhaled aerosol of infected individuals and can be used to quantify the viral aerosol production rates during respiratory activities.
In vitro model for investigating aerosol dispersion in a simulated COVID-19 patient during high-flow nasal cannula treatment

Abstract:The use of high-flow nasal cannula in the treatment of COVID-19 infected patients has proven to be a valuable treatment option to improve oxygenation. Early in the pandemic, there were concerns for the degree of risk of disease transmission to health care workers utilizing these treatments that are considered aerosol generating procedures. This study developed an in vitro model to examine the release of simulated patient-derived bioaerosol with and without high-flow nasal cannula at gas flow rates of 30 and 50 L/min. Aerosol dispersion was evaluated at 30 and 90 cm distances. Reduction of transmission risk was assessed using a surgical facemask on the manikin. Results indicated that the use of a facemask facilitated a 94–95% reduction in exhaled aerosol concentration at 30 cm and 22–60% reduction for 90 cm distance across both gas flow rates. This bench study confirms that this in vitro model can be used as a tool to assess the risk of disease transmission during aerosol generating procedures in a simulated patient and to test factors to mitigate the risk.

Abstract:Oxygen therapy is an essential treatment for patients with coronavirus disease 2019, although there is a risk of aerosolization of additional viral droplets occurring during this treatment that poses a danger to healthcare professionals. High-flow oxygen through nasal cannula (HFNC) is a vital treatment bridging low-flow oxygen therapy with tracheal intubation. Although many barrier devices (including devices without negative pressure in the barrier) have been reported in the literature, few barrier devices are suitable for HFNC and aerosol infection control procedures during HFNC have not yet been established. Hence, we built a single cough simulator model to examine the effectiveness of three protective measures (a semi-closed barrier device, a personalized exhaust, and surgical masks) administered in isolation as well as in combination using particle counter measurements and laser sheet visualization. We found that the addition of a personalized exhaust to a semi-closed barrier device reduced aerosol leakage during HFNC without negative pressure. This novel combination may thus reduce aerosol exposure during oxygen therapy, enhance the protection of healthcare workers, and likely reduce nosocomial infection risk.

Abstract:Patients with respiratory infections (e.g., COVID-19, antimicrobial resistant bacteria) discharge pathogens to the environment, exposing healthcare workers and inpatients to deleterious complications. This study tested the performance of SPEAR-P1 (synchronized personal exhaled air removal system – prototype 1), which actively detects expiration and removes exhaled air using an open, non-sealing lightweight facemask connected to a deep vacuum generating unit (DVGU). Fourteen healthy examinees practiced breathing through facemasks at 30, 25 and 20 breaths per minute; oxygen and nebulized saline were added at later steps. To test the efficacy of removing exhaled air, CO2 was used as a proxy and its level was measured from the outer surface of the open facemask. Compared to the baseline recording, SPEAR-P1 reduced CO2 escaping from the facemask by 66% on average for all study steps and respiratory rates (p<0.001), reaching 85.55% on average at 20 breaths per minute (p<0.001). This study shows that removing exhaled air from examinees using an open, non-sealing lightweight facemask is feasible. Future development of this system will enhance its efficacy and provide a method to remove a patient's contaminating aerosol without the need to "seal" the patient, especially in settings where isolation rooms are not readily available.

Abstract:Introduction: The "Severe Acute Respiratory Syndrome Coronavirus 2″ (SARS-CoV2) pandemic has led to a worldwide shortage of personal protection devices (PPD) for medical and paramedical personnel. Adaptation of commercially available snorkel masks to serve as full face masks has been proposed. Even not formally approved as PPD, they are publicized on social media as suitable for this use. Concerns about actual protection levels and risk of carbon dioxide (CO2) accumulation while wearing them for extended periods made us perform a systematic testing of various brands, in order to verify whether they are as safe and effective as claimed. Methods: A 'fit' test was performed, analogous to gas mask testing. Respiratory safety was evaluated by measuring end-tidal CO2 and oxygen saturation while wearing the masks in rest and during physical exercise. Masks were tested with 3D adaptors to mount regular bacterial-viral ventilator filters when available, or with snorkel openings covered with N95/FFP2 cloth. Results: Modified masks performed reasonably well on the fit test, comparable to regular N95/FFP2 masks. Not all ventilator filters are equally protective. For all masks, a small initial increase in end-tidal CO2 was noted, remaining within physiological limits. 3D printed adaptors are safer, have more flexibility and reliability than makeshift adaptations. Conclusions: These masks can offer benefit as a substitute for complete protective gear as they are easier to don and remove and offer full-face protection. They may be more comfortable to wear for extended periods. Proper selection of mask size, fit testing, quality of 3D printed parts, and choice of filter are important.

Abstract:At the front lines of the world's response to the COVID-19 pandemic are hero-clinicians facing a lack of critical supplies including protective medical grade breathing masks and filtering materials. At the same time, the general public is now being advised to wear masks to help stop the spread. As a result, in the absence of centrally coordinated production and distribution efforts, supply chains for masks, respirators, and materials for advanced filtration technology are immensely burdened. Here we describe experimental efforts to nondestructively quantify three vital characteristics of mask approaches: breathability, material filtration effectiveness, and sensitivity to fit. We focus on protection against water aerosols $>$0.3$\mu$m using off-the-shelf particulate, flow, and pressure sensors, permitting rapid comparative evaluation of these three properties. We present and discuss both the pressure drop and the particle transmission as a function of flow to permit comparison of relative protection for a set of proposed filter and mask designs. The design considerations of the testing apparatus can be reproduced by university laboratories and medical facilities and used for rapid local quality control of respirator masks which are of uncertified origin, monitoring the long-term effects of various disinfection schemes, and evaluating improvised products not designed or marketed for filtration.
A novel face mask design with improved properties for COVID-19 prevention

Abstract:Textile Research Journal, Ahead of Print. Novel cloth face masks to mitigate the spread of COVID-19 have been developed and tested for particle (0.1 μm in size) filtration efficiency, bacterial filtration efficiency, breathability, leakage, heart rate, and blood oxygen level, and then compared with the available N95 masks and surgical masks. It was found that this novel mask had better filtration efficiency than that of surgical masks and was very close to that of N95 masks. The breathability was also improved and was in the range of the designated levels for barrier face coverings. The flow visualization technique was utilized to study the leakage of the mask and it was found to have significantly lower leakage as compared to surgical masks. Heart rate and blood oxygen level tests were performed by wearing the mask during 10-minute walking sessions and it was found that wearing the mask did not adversely affect heart rate or blood oxygen levels or add any other strain on the wearer. It is believed that this novel face mask would reduce the spread of COVID-19 as well as provide an environmentally and economically conscious alternative to the N95 respirators for the public. The mask developed in this study can be washed, reused, and therefore worn for longer periods of time.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Objective</h3><p>All respiratory care represents some risk of becoming an Aerosol Generating Procedure (AGP) during COVID‐19 patient management. Personal Protective Equipment (PPE) and Environmental Control/Engineering is advised. High Velocity Nasal Insufflation (HVNI) and High Flow Nasal Cannula (HFNC) deliver High Flow Oxygen (HFO) therapy, established as a competent means of supporting oxygenation for acute respiratory distress patients, including that precipitated by COVID‐19. Although unlikely to present a disproportionate particle dispersal risk, AGP from HFO continues to be a concern. Previously, we published a preliminary model. Here, we present a subsequent high‐resolution simulation (higher complexity/reliability) to provide a more accurate and precise particle characterization on the effect of surgical masks on patients during HVNI, Low‐Flow Oxygen therapy (LFO2), and tidal breathing.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>This <i>in‐silico </i> modeling study of HVNI, LFO2, and tidal breathing presents ANSYS Fluent Computational Fluid Dynamics simulations that evaluate the effect of Type I surgical mask use over patient face on particle/droplet behavior. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>This <i>in‐silico </i> modeling simulation study of HVNI (40L∙min<sup>‐1</sup>) with a simulated surgical mask suggests 88.8% capture of exhaled particulate mass in the mask, compared to 77.4% in LFO2 (6L∙min<sup>‐1</sup>) capture, with particle distribution escaping to the room (>1m from face) lower for HVNI+Mask versus LFO2+Mask (8.23% versus 17.2%). The overwhelming proportion of particulate escape was associated with mask‐fit designed model gaps. Particle dispersion was associated with lower velocity. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions</h3><p>These simulations suggest employing a surgical mask over the HVNI interface may be useful in reduction of particulate mass distribution associated with AGPs.</p><p>This article is protected by copyright. Abstract:The COVID-19 is caused by the SARS-CoV-2, which is extremely infectious. Numerous virologist suggestions and guidelines advised using P2/N95 masks, gloves, goggles, face-shields, and frocks or gowns as routine specific protective tools during airway management to protect healthcare personnel from infection (PPE). However, numerous imitation research has indicated that conventional PPE cannot adequately protect healthcare personnel. Since then, numerous firms and healthcare professionals have created their personal reformed devices 'aerosol containment devices' (ACD). Their usage has expanded throughout the world without being properly evaluated for usefulness, efficacy, or safety. The practice of 'ACD' has been shown to make tracheal intubation (TI) more problematic in several simulated tests. Furthermore, the device should limit the transmission of droplets from a patient; however, it might put healthcare personnel at danger of being exposed to greater levels of viral aerosols. Consequently, the existing state of information suggests that 'ACD' deprived of a vacuum mechanism can simply protect healthcare personnel against viral transmission to a limited extent. We search various databases for the literature with keywords 'COVID-19,' 'aerosol box,' 'aerosol contaminations,' and 'droplet contaminations.' The current review focused on the aerosol box from various perspectives, including their mechanism, optimum time of use, the spread of aerosol control, current gaps, and future perspective for bridging those gaps.

Abstract:The controversy surrounding the transmission of COVID-19 in 2020 has revealed the need to better understand the airborne transmission route of respiratory viruses to establish appropriate strategies to limit their transmission. The effectiveness in protecting against COVID-19 has led to a high demand for face masks. This includes the single-use of non-degradable masks and Filtering Facepiece Respirators by a large proportion of the public, leading to environmental concerns related to waste management. Thus, nanocellulose-based membranes are a promising environmental solution for aerosol filtration due to their biodegradability, renewability, biocompatibility, high specific surface area, non-toxicity, ease of functionalization and worldwide availability. Although the technology for producing high-performance aerosol filter membranes from cellulose-based materials is still in its initial stage, several promising results show the prospects of the use of this kind of materials. This review focuses on the overview of nanocellulose-based filter media, including its processing, desirable characteristics and recent developments regarding filtration, functionalization, biodegradability, and mechanical behavior. The porosity control, surface wettability and surface functional groups resulting from the silylation treatment to improve the filtration capacity of the nanocellulose-based membrane is discussed. Future research trends in this area are planned to develop the air filter media by reinforcing the filter membrane structure of CNF with CNCs. In addition, the integration of sol–gel technology into the production of an air filter can tailor the pore size of the membrane for a viable physical screening solution in future studies.

Abstract:The SARS-CoV-2 virus is primarily transmitted through virus-laden fluid particles ejected from the mouth of infected people. Face covers can mitigate the risk of virus transmission but their outward effectiveness is not fully ascertained. <i>Objective:</i> by using a background oriented schlieren technique, we aim to investigate the air flow ejected by a person while quietly and heavily breathing, while coughing, and with different face covers. <i>Results:</i> we found that all face covers without an outlet valve reduce the front flow through by at least 63% and perhaps as high as 86% if the unfiltered cough jet distance was resolved to the anticipated maximum distance of 2-3 m. However, surgical and handmade masks, and face shields, generate significant leakage jets that may present major hazards. <i>Conclusions:</i> the effectiveness of the masks should mostly be considered based on the generation of secondary jets rather than on the ability to mitigate the front throughflow.

Abstract:In the unfortunate event of current ongoing pandemic COVID-19, where vaccination development is still at the initial stage, several preventive control measures such as social distancing, hand-hygiene, and personal protective equipment have been recommended by health professionals and organizations. Among them, the safe wearing of facemasks has played a vital role in reducing the likelihood and severity of infectious respiratory disease transmission. The reported research in facemasks has covered many of their material types, fabrication techniques, mechanism characterization, and application aspects. However, in more recent times, the focus has shifted towards the theoretical investigations of fluid flow mechanisms involved in the virus-laden particles prevention by facemasks. This exciting research domain aims to address the complex fluid transport that led to designing a facemask with a better performance. This review paper discusses the recent updates on fluid flow dynamics through the facemasks. Key design aspects such as thermal comfort and flow resistance are discussed. Furthermore, the recent progress in the investigations on the efficacy of facemasks for prevention of COVID 19 spread and the impact of wearing facemasks are presented. Finally, the potential research directions for analyzing the fluid flow behavior are highlighted.

An Individual Barrier Enclosure Actively Removing Aerosols for Airborne Isolation: A Vacuum Tent

Abstract:BACKGROUND:Aerosol barrier enclosure systems have been designed to prevent airborne contamination, but their safety has been questioned. A vacuum tent was designed with active continuous suctioning to minimize risks of aerosol dispersion. We tested its efficacy, risk of rebreathing, and usability on a bench, in healthy volunteers, and in an ergonomic clinical assessment study.METHODS:First, a manikin with airway connected to a breathing simulator was placed inside the vacuum tent to generate active breathing, cough, and CO2 production; high-flow nasal cannula (HFNC) was applied in the manikin's nares. Negative pressure was applied in the vacuum tent's apex port using wall suction. Fluorescent microparticles were aerosolized in the vacuum tent for qualitative assessment. To quantify particles inside and around vacuum tent (aerosol retention), an airtight aerosol chamber with aerosolized latex microparticles was used. The vacuum tent was tested on healthy volunteers breathing with and without HFNC. Last, its usability was assessed in 5 subjects by 5 different anesthesiologists for delivery of full anesthesia, including intubation and extubation.RESULTS:The vacuum tent was adjusted until no leak was visualized using fluorescent particles. The efficacy in retaining microparticles was confirmed quantitatively. CO2 accumulation inside the vacuum tent showed an inverse correlation with the suction flow in all conditions (normal breathing and HFNC 30 or 60 L/min) in bench and healthy volunteers. Particle removal efficacy and safe breathing conditions (CO2, temperature) were reached when suctioning was at least 60 L/min or 20 L/min > HFNC flow. Five subjects were successfully intubated and anesthetized without ergonomic difficulties and with minimal interference with workflow and an excellent overall assessment by the anesthesiologists.CONCLUSIONS:The vacuum tent effectively minimized aerosol dispersion. Abstract:In several countries wearing multiple surgical masks or N95 respirators was mandatory in public during the COVID pandemic. In this study, we investigated the transportation and filtering mechanism of heterogeneous nanoparticles and viruses through surgical masks and N95 respirators. We conducted experiments in vitro using aerosol spray paints containing nanoparticles and validated the findings in vivo on a human volunteer. Scanning electron microscopy was employed to analyse the transportation and distribution of nanoparticles in different mask layers and on pristine silicon substrates placed on human skin. We provide analytical insights into the pressure distribution and fluid velocity profiles within the complex polymer network. Remarkably, our results showed that both single surgical masks and N95 respirators demonstrated similar efficiency in filtering colloidal and jet-stream nanoparticles in the air. These comprehensive findings have significant implications for policymakers in defining regulations for airborne pandemics and air pollution control.

Abstract:The ongoing respiratory COVID-19 pandemic has heavily impacted the social and private lives of the majority of the global population. This infection is primarily transmitted via virus-laden fluid particles (i.e., droplets and aerosols) that are formed in the respiratory tract of infected individuals and expelled from the mouth in the course of breathing, talking, coughing, and sneezing. To mitigate the risk of virus transmission, in many places of the world, the public has been asked or even obliged to use face covers. It is plausible that in the years ahead we will see the use of face masks, face shields and respirators become a normal practice in our life. However, wearing face covers is uncomfortable in some situations, like, for example, in summer heat, while staying on beaches or at hotel swimming pools, doing exercises in gyms, etc. Also, most types of face cover become contaminated with time and need to be periodically replaced or disinfected. These nuisances are caused by the fact that face covers are based on material barriers, which prevent inward and outward propagation of aerosol and droplets containing the pathogen. Applying well established gas-particle flow formalism, we study a non-material based protection barrier created by a flow of well directed down stream of air across the front of the open face. The~protection is driven by dragging virus-laden particles inside the width of the air flow and hence, as a consequence, displacing them away from their primary trajectories. The study, shows that such, potentially portable, air curtains can effectively provide both inward and outward protection and serve as an effective personal protective equipment (PPE) mitigating human to human transmission of virus infection like COVID-19.

Abstract:The spread of many infectious diseases relies on aerosol transmission to the respiratory tract. Here we design an intranasal mask comprising a positively-charged thermosensitive hydrogel and cell-derived micro-sized vesicles with a specific viral receptor. We show that the positively charged hydrogel intercepts negatively charged viral aerosols, while the viral receptor on vesicles mediates the entrapment of viruses for inactivation. We demonstrate that when displaying matched viral receptors, the intranasal masks protect the nasal cavity and lung of mice from either severe acute respiratory syndrome coronavirus 2 or influenza A virus. With computerized tomography images of human nasal cavity, we further conduct computational fluid dynamics simulation and three-dimensional printing of an anatomically accurate human nasal cavity, which is connected to human lung organoids to generate a human respiratory tract model. Both simulative and experimental results support the suitability of intranasal masks in humans, as the likelihood of viral respiratory infections induced by different variant strains is dramatically reduced.
