Abstract:Purpose: To present the performance of a multi-point plastic scintillation detector (mPSD) as a tool in vivo dosimetry in brachytherapy. Methods: A previously optimized three-point sensor system was used for in vivo HDR brachytherapy measurements (using the scintillators BCF-60, BCF-12, and BCF-10). The light detection system of the mPSD consisted of compactly assembled photomultiplier tubes (PMTs) and dichroic mirrors and filters to achieve a highly sensitive scintillation light collection. The PMT signals were recorded using a NI-DAQ board at a rate of 100 kHz. Dose measurements covering a range of 0.5 to 10 cm from the 192Ir source were carried out according to TG-43 U1 recommendations in order to: (1) characterize the system's response in terms of angular dependence; (2) obtain the relative contribution of positioning and measurement uncertainties to the total system uncertainty; (3) assess the system's temporal resolution; and (4) track the source position in real time. Results: The positioning uncertainty dominated close to the source, whereas the measurement uncertainty dominated at larger distances. A maximum measurement uncertainty of 17 % was observed for the BCF-60 scintillator at 10 cm from the source. The average best compromise between positioning and measurement uncertainties were reached at 17.4 mm. The detector further exhibited no angular dependence. The system provided an average location with a standard deviation under 1.7 mm. The maximum observed differences between measured and expected source location was 1.82 mm. Dose deviations remained below 5% in all the explored measurement conditions. With regard to dwell time measurement accuracy, the maximum deviation observed at all distances was 0.56 s. Conclusions: The performance of the system demonstrated that it could be used for real-time dose, position and dwell time measurements during HDR brachytherapy.

Abstract:Brachytherapy is a radiotherapy procedure performed with radioactive sources implanted into the patient's body, close to the area affected by cancer. This is a reference procedure for the treatment of prostate and gynecologic cancer due to the reduction of the dose released close to organs at risk (e.g., rectum, bladder, colon). For this reason, real-time dose verification and source localisation are essential for an optimal treatment plan. The ORIGIN collaboration aims to achieve this goal through a 16-fibre sensor system, designed to house a small volume of scintillating material in a transparent fibre tip to enable point-like measurements. The selected scintillating materials feature a decay time of about 500 μs and the signal associated with the primary γ-ray interaction results in the emission of a sequence of single photons distributed over time. Therefore, the dosimeter requires a detector with single-photon sensitivity and a system designed to provide dose measurements by photon counting. Uniformity of fibre response, system stability and reproducibility of measurements are key features of the dosimeter. The characterisation of the 16-channel dosimeter system equipped with thermo-electrically cooled Silicon Photomultipliers, carried out in the laboratory using an X-ray cabinet, is discussed and the results are compared with an earlier version equipped with SiPMs operated at room temperature.

Abstract:Surface brachytherapy (BT) lacks standard quality assurance (QA) protocols. Commercially available treatment planning systems (TPSs) are based on a dose calculation formalism that assumes the patient is made of water, resulting in potential deviations between planned and delivered doses. Here, a method for treatment plan verification for skin surface BT is reported. Chips of thermoluminescent dosimeters (TLDs) were used for dose point measurements. High-dose-rate treatments were simulated and delivered through a custom-flap applicator provided with four fixed catheters to guide the Iridium-192 (Ir-192) source by way of a remote afterloading system. A flat water-equivalent phantom was used to simulate patient skin. Elekta TPS Oncentra Brachy was used for planning. TLDs were calibrated to Ir-192 through an indirect method of linear interpolation between calibration factors (CFs) measured for 250 kV X-rays, Cesium-137, and Cobalt-60. Subsequently, plans were designed and delivered to test the reproducibility of the irradiation set-up and to make comparisons between planned and delivered dose. The obtained CF for Ir-192 was (4.96 ± 0.25) μC/Gy. Deviations between measured and TPS calculated doses for multi-catheter treatment configuration ranged from −8.4% to 13.3% with an average of 0.6%. TLDs could be included in clinical practice for QA in skin BT with a customized flap applicator.

Silicon Photomultipliers for Deep Tissue Cerenkov Emission Detection During External Beam Radiotherapy

Abstract:Cerenkov Emission (CE) during external beam radiation therapy (EBRT) from a linear accelerator (Linac) has been demonstrated as a useful tool for radiotherapy quality assurance and potentially other applications for online tracking of tumors during treatment delivery. However, some of the current challenges that are impacting the potential of CE are related to the limited detection sensitivity and the lack of flexible tools to fit into an already complex treatment delivery environment. Silicon photomultiplier (SiPM) solid-state devices are new promising tools for low light detection due to their extreme sensitivity that mirrors photomultiplier tubes and yet have a form factor that is similar to silicon photodiodes, allowing for improved flexibility in device design that may help in the process of wider clinical applicability. In this work, we assess the feasibility of using SiPMs to detect CE during EBRT from a Linac and contrast their performance with commercially available silicon photodiodes (PDs). We demonstrate the feasibility of the SiPM based probes for standard dosimetry measurements. We also demonstrate that CE optical signals can be detected from tissue depths about five times greater than that for standard probes based on PDs, making our SiPM probe an enabling technology of CE measurements, particularly for deep tissue applications.
A Clinical Monte Carlo Dosimetric Evaluation in Accelerated Partial Breast Electronic Brachytherapy

Abstract:Electronic brachytherapy has been recently utilized in accelerated partial breast irradiation. Deficient tissue inhomogeneity correction in the dose calculation draws physicians' attentions. In this study, a clinical Monte Carlo based dose evaluation was assessed to demonstrate these dosimetric impacts. The dosimetric characteristics of Xoft S700 Axxent X-ray source (Xoft, Fremont, CA) were first evaluated using EGS4 Monte Carlo code to verify the calculation model and source parameter accuracy. A whole body female photographic phantom with defined structures derived from National Library of Medicine's Visible Human Project utilizing all clinical parameters was then used in the dose calculation. A 5cm diameter spherical balloon was inserted in the right breast of the phantom. The volume expanded 1cm from the balloon surface was designed to define as planning target volume (PTV). The prescribed dose per fraction was 3.4 Gy to 5 user-selected points (anterior, posterior, right lateral, superior and inferior) of the PTV. Dose, dose volume histograms of the PTV and normal tissues, isodose distributions in axial, sagittal and coronal planes crossing the center of the balloon were evaluated. Using measured 50 kVp source spectrum provided by Xoft Inc, the simulated radial dose function agrees well with the published data within 3%. After corrected for vender addressed 6% balloon attenuation, the PTV dose reported from the commercial treatment planning system is within 1% compared to our calculation result. The simulation shows 100% PTV covered by 90.6% of the prescription dose with excellent dose uniformity. When accounted for the tissue heterogeneity, changes in the isodose distribution were clearly observed. The two dimensional dose distributions show high skin and rib doses due to photoelectric effect with reasonably acceptable doses to all other surrounding normal tissues. Our preliminary EGS4 Monte Carlo simulation study demonstrates the dosimetric impact in the electronic brachytherapy, especially on the surrounding normal tissues, when tissue inhomogeneity consideration is taken into account.

Abstract:Preclinical studies have shown that radiotherapy treatments benefit from ultra-high dose rate (UHDR) irradiations. These trigger the FLASH-effect, resulting in a strong decrease in normal tissue toxicity with conserved tumor control probability, compared with conventional irradiations. However, the beam parameters to trigger the FLASH-effect and the radiobiological mechanisms behind it remain to be elucidated. A limiting factor in many studies is the lack of accurate real time dosimetry. In this work an innovative solution for 2D real time dosimetry in UHDR electron beams is presented and characterized. The in-house developed ImageDosis system consists of a scientific camera, with high temporal resolution, and a coating, containing 12% of Y 3 Al 5 O 12 :Ce 3+ as scintillating material. Reference dosimetry was performed by means of radiochromic film, and a (C 38 H 34 P 2 )MnBr 4 point scintillator was used to validate the pulse discrimination properties of the ImageDosis system. Irradiations were performed in two centers (Antwerp and Orsay), with an ElectronFlash accelerator. The ImageDosis system was tested with varying number of pulses, pulse length, pulse repetition frequency (PRF) and energy. In addition, its temporal resolution and 2D properties were investigated. The ImageDosis system showed negligible dose, dose rate and energy dependence for doses up to 13 Gy and average dose rates up to 140 Gy/s, with a dose per pulse up to 2 Gy and a PRF up to 300 Hz. The system is capable of discriminating and measuring the dose of individual pulses and shows promising 2D characteristics that need further optimization.

Seven-probe scintillator dosimeter for treatment verification in HDR-brachytherapy

Abstract:Purpose: We introduce a novel concept of a compact multiprobe scintillator detector and demonstrate its applicability in HDR-brachytherapy. Our fabricated seven-probe system is sufficiently narrow to be inserted in a brachytherapy needle or in a catheter. Methods: Our multiprobe detection system results from the parallel implementation of a miniaturized scintillator detector at the end of a bundle of seven fibers. The resulting system, which is narrower than 320 microns, is tested with a MicroSelectron 9.1 Ci Ir-192 HDR afterloader, in a water phantom. The detection signals from all seven probes are simultaneously read with an sCMOS camera (at a rate of 0.06 s). The camera is coupled to a chromatic filter to cancel Cerenkov signal induced within the fibers upon exposure. By implementing an aperiodic array of six scintillating cells along the bundle axis (one probe is kept bare to assess the stem effect), we first determine the range of inter-probe spacings leading to optimal source tracking accuracy. Then, three different source tracking algorithms involving sequentially or simultaneously all the scintillating probes are tested and compared. In each case, dwell positions are assessed from dose measurements and compared to the treatment plan. Dwell time is also determined and compared to the treatment plan. Results: The optimum inter-probe spacing for an accurate source tracking ranges from 15 mm to 35 mm. The optimum detection algorithm consists of adding the readout signals from all detector probes. In that case, the error to the planned dwell positions is of 0.01+/-0.14 mm and 0.02+/-0.29 mm at spacings between the source and detector axes of 5.5 and 40 mm, respectively. Using this approach, the average deviations to the expected dwell time are of -0.006+/-0.009 s and -0.008+/-0.058 s, at spacings between source and probe axes of 5.5 mm and 20 mm, respectively.

Abstract:Background/Objectives: Spatial fractionation of proton fields as sub-millimeter beamlets to treat cancer has shown better sparing of healthy tissue whilst maintaining the same tumor control. It is critical to ensure primary standard dosimetry is accurate and ready to support the modality's clinical implementation. Methods: This work provided a proof-of-concept, using the National Physical Laboratory's Primary Standard Proton Calorimeter (PSPC) to measure average absorbed dose-to-water in a pMBRT field. A 100 MeV mono-energetic field and a 2 cm wide SOBP were produced with a spot-scanned proton beam incident on a collimator comprising 15 slits of 400 μm width, each 5 cm long and separated by a center-to-center distance of 4 mm. Results: The results showed the uncertainty on the absorbed dose-to-water in the mono-energetic beam was dominated by contributions of 1.4% and 1.1% ( = 1) for the NPL PSPC and PTW Roos chambers, respectively, originating from the achievable positioning accuracy of the devices. In comparison, the uncertainty due to positioning in the SOBP for both the NPL PSPC and PTW Roos chambers were 0.4%. Conclusions: These results highlight that it may be more accurate and reliable to perform reference dosimetry measuring the Dose-Area Product or in an SOBP for spatially fractionated fields.

Abstract:Background and purpose: A sub-mm resolution Complementary Metal Oxide Semiconductor sensor has been developed for stereotactic radiotherapy quality assurance. Herein we evaluate its basic dosimetric performance and its application for linac C-arm stereotactic body radiotherapy (SBRT) plan quality assurance. Materials and methods: The detector was integrated into its accompanying phantom or in Water Equivalent Plastic (WEP). The measurement reproducibility, stability, dose linearity and dependence on angularity, dose rate and field size were investigated. Clinical plan measurements were compared to our radiotherapy treatment planning system and radiochromic film. Sensitivity to introduced Multi Leaf Collimator (MLC) offsets was evaluated by simulating single MLC offsets in SBRT plans and comparing measurements to expected doses. Results: Signal reproducibility was within ± 0.1 % and output calibration was stable over a 6 month period. Detector showed good linearity with dose (r2 = 1). Signal decreased by 5 % when dose rate was decreased from 1300 MU/min to 300 MU/min. Output factors agreed within 0.5 % of chamber measurements for 1x1 cm field sizes or greater. Angularity measurements showed good agreement with reference. For measurement of planned clinical doses, gamma pass-rates were 98.5 % ± 2.3 % (treatment planning system reference, 2 %/2mm) and 99.2 % ± 1.0 % (film reference, 2 %,2mm). The detector also showed sensitivity to errors of 1 mm offsets in MLC positioning. Conclusion: The detector performed well when used for pre-treatment SBRT plan quality assurance, offering a good alternative to radiochromic film.

The ImageDosis system showed capable of measuring the dose of the individual pulses up to a PRF of 250 Hz, but did not detect 3% of the pulses, because these pulses were delivered during the dead time of the camera. The maximal difference in FWHM of the field size between the ImageDosis system and the reference was 3.6%. For a nominal field size of 100 mm and 120 mm, a decreased output was observed on the superior part of the field. The ImageDosis system showed linear dose response and no dose rate nor energy dependence. It showed capable of discriminating and measuring the dose of individual pulses and promising 2D characteristics that need further optimization.

Abstract:Source localisation and real-time dose verification are at the forefront of medical research in brachytherapy, an oncological radiotherapy procedure based on radioactive sources implanted in the patient body. The ORIGIN project aims to respond to this medical community's need by targeting the development of a multi-point dose mapping system based on fibre sensors integrating a small volume of scintillating material into the tip and interfaced with silicon photomultipliers operated in counting mode. In this paper, a novel method for the selection of the optimal silicon photomultipliers to be used is presented, as well as a laboratory characterisation based on dosimetric figures of merit. More specifically, a technique exploiting the optical cross-talk to maintain the detector linearity in high-rate conditions is demonstrated. Lastly, it is shown that the ORIGIN system complies with the TG43-U1 protocol in high and low dose rate pre-clinical trials with actual brachytherapy sources, an essential requirement for assessing the proposed system as a dosimeter and comparing the performance of the system prototype against the ORIGIN project specifications.

Abstract:We report the experimental validation of a detection module based on a LaBr3(Ce+Sr) scintillator crystal, read by a matrix of Silicon Photomultipliers (SiPMs), for the development of a SPECT system for dose monitoring in Boron Neutron Capture Therapy (BNCT). The goal of the system is to detect the gamma rays at 478keV emitted by the excited 7Li produced in the 10B(n, α)7Li reaction, to have a real-time localization and quantification of the local dose released to a patient during BNCT treatment. The good energy resolution of the detector (<3% at 662keV) allows to resolve the photopeak at 478keV from the adjacent annihilation photons photopeak at 511keV, originated from pair production of high energy gamma rays, such as the hydrogen neutron capture gamma rays at 2.2MeV. The results shown in this article demonstrate the suitability of our detector in view of future application in BNCT real-time dose monitoring. Good linearity (RMSE < 2.5cps) between the number of events detected at 478keV and the boron concentration of the 10B-loaded samples has been achieved, down to a boron concentration of 62ppm with a neutron flux of approximately 1 × 105 n/cm2/s.

Abstract:We assessed the accuracy of a prototype radiation detector with a built in CMOS amplifier for use in dosimetry for high dose rate brachytherapy. The detectors were fabricated on two substrates of epitaxial high resistivity silicon. The radiation detection performance of prototypes has been tested by ion beam induced charge (IBIC) microscopy using a 5.5 MeV alpha particle microbeam. We also carried out the HDR Ir-192 radiation source tracking at different depths and angular dose dependence in a water equivalent phantom. The detectors show sensitivities spanning from (5.8 ± 0.021) × 10−8 to (3.6 ± 0.14) × 10−8 nC Gy−1 mCi−1 mm−2. The depth variation of the dose is within 5% with that calculated by TG-43. Higher discrepancies are recorded for 2 mm and 7 mm depths due to the scattering of secondary particles and the perturbation of the radiation field induced in the ceramic/golden package. Dwell positions and dwell time are reconstructed within ±1 mm and 20 ms, respectively. The prototype detectors provide an unprecedented sensitivity thanks to its monolithic amplification stage. Future investigation of this technology will include the optimisation of the packaging technique.

Abstract:(1) Background: High dose gradients and manual steps in brachytherapy treatment procedures can lead to dose errors which make the use of in vivo dosimetry (IVD) highly recommended for verifying brachytherapy treatments. A new procedure was presented to obtain a calibration factor which allows fast and robust calibration of plastic scintillation detector (PSD) probes for the geometry of a compact phantom using Monte Carlo simulations. Additionally, characterization of PSD energy, angular, and temperature dependences was performed. (2) Methods: PENELOPE/PenEasy code was used to obtain the calibration factor. To characterize the energy dependence of the PSD, the signal was measured at different radial and transversal distances. The sensitivity to the angular position was characterized in axial and azimuthal planes. (3) Results: The calibration factor obtained allows for an absorbed dose to water determination in full scatter conditions from ionization measured in a mini polymethylmethacrylate (PMMA) phantom. The energy dependence of the PSD along the radial distances obtained was (2.3 ± 2.1)% (k = 1). The azimuthal angular dependence measured was (2.6 ± 3.4)% (k = 1). The PSD response decreased by (0.19 ± 0.02)%/°C with increasing detector probe temperature. (4) Conclusions: The energy, angular, and temperature dependence of a PSD is compatible with IVD.

Abstract:Abstract The purpose of this study is to leverage radioluminescence imaging for the development of an automated high-dose-rate (HDR) brachytherapy quality assurance (QA) system that enables simultaneous measurements of dwell position, dwell time, wire velocity, and relative source strength in a single test. The system consists of a radioluminescence phosphor sheet (a mixture of Gd 2 O 2 S:Tb and PDMS) positioned atop a HDR needle applicator, a complementary metal-oxide-semiconductor digital camera used to capture the emitted radioluminescence signals from the scintillator sheet, and an in-house graphical user interface for signal processing. The signal processing was used to extract source intensity, location, and elapsed time, yielding the final measurements on dwell position, dwell time, and wire velocity. The source strength relative to the well chamber calibration (in unit of Air-Kerma strength, S k ) is measured by establishing a calibration curve that correlates S k with the detector response. Validation experiments are performed using three customized treatment plans. With these plans, the dwell position and dwell time are verified for a range of 110.0 cm–117.5 cm and 2 s–16 s, respectively, and the linear correlation with S k is demonstrated for the source strength varying between 28 348 U (cGy cm 2 h −1 ) and 41 906 U. The wire velocity, i.e. the speed of the radioactive source averaged over the distance in between dwell positions, is calculated for various distances ranging from 5 mm to 50 mm. Results show that the mean deviations of the measured dwell position and dwell time are 0.1 mm (range from 0 to 0.2 mm) and 32.5 ms (range from 0 to 60.0 ms) with respect to the planned values, respectively, and the system response is highly linear with S k ( R 2 = 0.998). Moreover, the measured wire velocities are comparable to previously reported values. Benefitting from the compact hardware design and image processing algorithms, the system provides a practical, reliable, and comprehensive solution for HDR QA.

Abstract:Background Non‐melanoma skin cancer is one of the most common types of cancer and one of the main approaches is brachytherapy. For small lesions, the treatment of this cancer with brachytherapy can be done with two commercial applicators, one of these is the Large Field Valencia Applicators (LFVA). Purpose The aim of this study is to test the capabilities of the LFVA to use clinically 60Co sources instead of the 192Ir ones. This study was designed for the same dwell positions and weights for both sources. Methods The Penelope Monte Carlo code was used to evaluate dose distribution in a water phantom when a 60Co source is considered. The LFVA design and the optimized dwell weights reported for the case of 192Ir are maintained with the only exception of the dwell weight of the central position, that was increased. 2D dose distributions, field flatness, symmetry and the leakage dose distribution around the applicator were calculated. Results When comparing the dose distributions of both sources, field flatness and symmetry remain unchanged. The only evident difference is an increase of the penumbra regions for all depths when using the 60Co source. Regarding leakage, the maximum dose within the air volume surrounding the applicator is in the order of 20% of the prescription dose for the 60Co source, but it decreases to less than 5% at about 1 cm distance. Conclusions Flatness and symmetry remains unaltered as compared with 192Ir sources, while an increase in leakage has been observed. This proves the feasibility of using the LFVA in a larger range of clinical applications.

Abstract:Objective.Detectors that can provide accurate dosimetry for microbeam radiation therapy (MRT) must possess intrinsic radiation hardness, a high dynamic range, and a micron-scale spatial resolution. In this work we characterize hydrogenated amorphous silicon detectors for MRT dosimetry, presenting a novel combination of flexible, ultra-thin and radiation-hard features.Approach.Two detectors are explored: an n-type/intrinsic/p-type planar diode (NIP) and an NIP with an additional charge selective layer (NIP + CSC).Results.The sensitivity of the NIP + CSC detector was greater than the NIP detector for all measurement conditions. At 1 V and 0 kGy under the 3T Cu-Cu synchrotron broadbeam, the NIP + CSC detector sensitivity of (7.76 ± 0.01) pC cGy-1outperformed the NIP detector sensitivity of (3.55 ± 0.23) pC cGy-1by 219%. The energy dependence of both detectors matches closely to the attenuation coefficient ratio of silicon against water. Radiation damage measurements of both detectors out to 40 kGy revealed a higher radiation tolerance in the NIP detector compared to the NIP + CSC (17.2% and 33.5% degradations, respectively). Percentage depth dose profiles matched the PTW microDiamond detector's performance to within ±6% for all beam filtrations except in 3T Al-Al due to energy dependence. The 3T Cu-Cu microbeam field profile was reconstructed and returned microbeam width and peak-to-peak values of (51 ± 1)μm and (405 ± 5)μm, respectively. The peak-to-valley dose ratio was measured as a function of depth and agrees within error to the values obtained with the PTW microDiamond. X-ray beam induced charge mapping of the detector revealed minimal dose perturbations from extra-cameral materials.Significance.The detectors are comparable to commercially available dosimeters for quality assurance in MRT. With added benefits of being micron-sized and possessing a flexible water-equivalent substrate, these detectors are attractive candidates for quality assurance,in-vivodosimetry and in-line beam monitoring for MRT and FLASH therapy.

Abstract:New radiotherapy treatment techniques make unprecedented demands on dosimetry. Water equivalence, radiation hardness, high temporal and spatial resolution in 4D are no longer desirable but essential for safe implementation of several techniques. Plastic scintillation dosimetryhas the capabilities to meet all of the above criteria. Despite several attempts, a system has not yet been commercialised for widespread availability in the clinic. Successful translation of good science to the benefits of clinical practice demands that the primary focus be shifted from the technological innovation to the patient needs. We will present the clinically beneficial features of scintillation dosimetry and some remaining challenges. Then we will discuss the barriers to widespread availability of scintillation dosimeters.
