Abstract:Urinary stones composed of uric acid can be treated medically. Prediction of uric acid stone type is, therefore, desirable when a urinary stone is diagnosed with unenhanced CT. The purpose of the present study was to describe single-energy thin slice quantitative CT parameters of urinary stones correlated to chemical stone type and to develop a method to distinguish pure uric acid stones (UA) from other stones (non-UA/Mix). Unenhanced thin slice single-energy CT images of 126 urinary stones (117 patients) with known chemical stone type were retrospectively included in the study. Among the included stones, 22 were UA and 104 were non-UA/Mix. The included CT images and Laplacian filtered images of the stones were quantitatively analyzed using operator-independent methods. A post hoc classification method for pure UA stones was created using a combination of cutoff values for the peak attenuation and peak point Laplacian. The stone types differed in most quantitative image characteristics including mean attenuation (p < 0.001), peak attenuation (p < 0.001), and peak point Laplacian (p < 0.001). The sensitivity for the post hoc-developed peak attenuation-peak point Laplacian method for classifying pure UA stones was 95% [21/22, 95% CI (77-100%)] and the specificity was 99% [103/104, 95% CI (95-100%)]. In conclusion, quantitative image analysis of thin slice routine single-energy CT images is promising for predicting pure UA content in urinary stones, with results comparable to double energy methods.

Abstract:Abstract Objectives To assess the accuracy of low-dose dual-energy computed tomography (DECT) to differentiate uric acid from non-uric acid kidney stones in two generations of dual-source DECT with stone composition analysis as the reference standard. Methods Patients who received a low-dose unenhanced DECT for the detection or follow-up of urolithiasis and stone extraction with stone composition analysis between January 2020 and January 2022 were retrospectively included. Collected stones were characterized using X-ray diffraction. Size, volume, CT attenuation, and stone characterization were assessed using DECT post-processing software. Characterization as uric acid or non-uric acid stones was compared to stone composition analysis as the reference standard. Sensitivity, specificity, and accuracy of stone classification were computed. Dose length product (DLP) and effective dose served as radiation dose estimates. Results A total of 227 stones in 203 patients were analyzed. Stone composition analysis identified 15 uric acid and 212 non-uric acid stones. Mean size and volume were 4.7 mm × 2.8 mm and 114 mm 3 , respectively. CT attenuation of uric acid stones was significantly lower as compared to non-uric acid stones ( p < 0.001). Two hundred twenty-five of 227 kidney stones were correctly classified by DECT. Pooled sensitivity, specificity, and accuracy were 1.0 (95%CI: 0.97, 1.00), 0.93 (95%CI: 0.68, 1.00), and 0.99 (95%CI: 0.97, 1.00), respectively. Eighty-two of 84 stones with a diameter of ≤ 3 mm were correctly classified. Mean DLP was 162 ± 57 mGy*cm and effective dose was 2.43 ± 0.86 mSv. Conclusions Low-dose dual-source DECT demonstrated high accuracy to discriminate uric acid from non-uric acid stones even at small stone sizes. Abstract:The resaturation curve, a noninvasive indicator-dilution test using an ear oximeter to detect rates of change in arterial oxygen saturation during breathing of various concentrations of oxygen, was used to assess cardiac performance in normal subjects and in 108 patients with cardiac valvular disease. Measurements made during exercise included the time constant of resaturation (tau) and beat-to-beat changes in arterial oxygen saturation (the left heart clearance fraction). At maximum rates of voluntary work, patients had a significantly reduced clearance fraction and longer tau than normal subjects. Clearance fraction and tau improved in patients after aortic valve replacement; deterioration occurred in tau and clearance fraction over time in patients treated medically, as compared to normal subjects who showed little change during a ten-year period. Clearance fraction and tau correlated with hemodynamic data obtained during cardiac catheterization. The resaturation curve provides an objective measure of cardiac impairment that can be readily repeated during follow-up of patients with heart disease.

Abstract:BackgroundUric acid stone diagnosis is presently done primarily with in vitro analysis of stones. In vivo diagnosis with dual-energy CT (DECT) would allow earlier initiation of therapy with urine alkalinization and avoid surgical intervention.ObjectiveTo evaluate if DECT, using stone analysis as reference standard, is sufficiently accurate to replace stone analysis for diagnosis of uric acid stones.MethodsOriginal studies in patients with urolithiasis examined with DECT with stone analysis as the reference standard were eligible for inclusion. MEDLINE (1946–2018), Embase (1947–2018), CENTRAL (August 2018), and multiple urology and radiology conferences were searched. QUADAS-2 was used to assess risk of bias and applicability. Meta-analyses were performed using a bivariate random-effects model.ResultsA total of 21 studies (1105 patients, 1442 stones) were included. Fourteen studies containing 662 patients (944 stones) were analyzed in the uric acid dominant target condition (majority of stone composition uric acid): mean sensitivity was 0.88 (95% CI 0.79–0.93) and specificity 0.98 (95% CI 0.96–0.99). Thirteen studies (674 patients, 760 stones) were analyzed in the uric acid–containing target condition (< majority of stone composition uric acid): mean sensitivity was 0.82 (95% CI 0.73–0.89) and specificity 0.97 (95% CI 0.94–0.98). Meta-regression showed no significant variability in test accuracy. Two studies had one or more domains at high risk of bias and there were no concerns regarding applicability.ConclusionDECT is an accurate replacement test for diagnosis of uric acid calculi in vivo, such that stone analysis could be replaced in the diagnostic pathway. This would enable earlier initiation of urine alkalinization.Key Points• DECT for uric acid dominant stones has sensitivity of 0.88 (95% CI 0.79–0.93) and specificity of 0.98 (95% CI 0.96–0.99); uric acid–containing stones had mean sensitivity of 0.82 (95% CI 0.73–0.89) and specificity of 0.97 (95% CI 0.94–0.98).• Meta-regression did not identify any variables (study design, reference standard, dual-energy CT type, dose, risk of bias) that influenced test accuracy.• Only 2 of the 21 included studies had 1 or more domain considered to be at high risk of bias with the majority of domains considered at low risk of bias; there were no concerns regarding applicability in any of the included studies.

Abstract:PurposeTo assess the non-inferiority of dual-layer spectral detector CT (SDCT) compared to dual-source dual-energy CT (dsDECT) in discriminating uric acid (UA) from non-UA stones.MethodsFifty-seven extracted urinary calculi were placed in a cylindrical phantom in a water bath and scanned on a SDCT scanner (IQon, Philips Healthcare) and second- and third-generation dsDECT scanners (Somatom Flash and Force, Siemens Healthcare) under matched scan parameters. For SDCT data, conventional images and virtual monoenergetic reconstructions were created. A customized 3D growing region segmentation tool was used to segment each stone on a pixel-by-pixel basis for statistical analysis. Median virtual monoenergetic ratios (VMRs) of 40/200, 62/92, and 62/100 for each stone were recorded. For dsDECT data, dual-energy ratio (DER) for each stone was recorded from vendor-specific postprocessing software (Syngo Via) using the Kidney Stones Application. The clinical reference standard of X-ray diffraction analysis was used to assess non-inferiority. Area under the receiver-operating characteristic curve (AUC) was used to assess diagnostic performance of detecting UA stones.ResultsSix pure UA, 47 pure calcium-based, 1 pure cystine, and 3 mixed struvite stones were scanned. All pure UA stones were correctly separated from non-UA stones using SDCT and dsDECT (AUC = 1). For UA stones, median VMR was 0.95–0.99 and DER 1.00–1.02. For non-UA stones, median VMR was 1.4–4.1 and DER 1.39–1.69.ConclusionSDCT spectral reconstructions demonstrate similar performance to those of dsDECT in discriminating UA from non-UA stones in a phantom model.

Abstract:Objective To evaluate the ability of dual-source,dual-energy CT in differentiating uric acid stones from non-uric acid stones with infrared spectroscopy as reference standard.Materials and Methods Urinary calculus from 308 patients were scanned in first generation dual-source CT with dual-energy mode between July 2011 and June 2012.Renal Stone application was used to analyze their composition.The uric acid stones color were coded red and non-uric acid stones were blue.CT values were measured in 60 selective urinary calculus including 30 uric acid stones and 30 non-uric acid stones.The accuracy of dual energy CT to differentiate uric acid and no-uric acid stones was calculated.Results Of 308 patients,60 patients had uric acid stones and 248 non-uric acid stones.No difference was found for uric acid stone at 80 kV and 140 kV(375.8±69.2 HU vs.374.1±69.4 HU;t=-0.217,P=0.830),while CT values of non-uric acid stones were higher at 80 kV than those at 140 kV(1 455.1±312.4 HU vs.1 039.6±194.4 HU;t=-12.16,P0.001).CT values of non-uric acid stones at 80 kV,140 kV,and average weighted images(1 455.1±312.4 HU,1 039.6±194.4 HU,and 882.0±176.4 HU,respectively) were higher than those of uric acid stones(375.8±69.2 HU,374.1±69.4 HU,and 366.3±80.1 HU,respectively;P0.001).With infrared spectrum findings as reference standard,the accuracy of dual energy CT in differentiating uric acid stones from non-uric acid stones was 100%.Conclusions Dual-source,dual-energy CT can accurately differentiate uric acid stones from non-uric acid stones,and plays an important role in treatment planning of renal stones.
Diagnosis of uric acid calculi by dual energy CT:with ex vivo calculi spectroscopy as the reference standard

Abstract:Objective:To prospectively determine the capability of dual energy CT Gemstone spectral imaging and advanced post-processing techniques to identify uric acid calculi,with ex vivo urinary calculi spectroscopy as the reference standard.Methods:Sixty-three urinary stones were obtained by endoscopic lithotripsy.The chemical composition of the stones was confirmed by spectroscopy(including 9 cases of uric acid calculus,54 cases of non-uric acid stones).These urinary calculi were placed in the fresh porcine kidney pelvis dipped in physiological saline.Both routine CT scan and dual energy CT scan were performed.All the images of 63 urinary calculi were analyzed at workstation.CT value of the calculi was measured on the images of routine scan,40keV monoenergic(MONO) images,and 140keV MONO images,respectively.Effective Z(Eff-Z) was measured for each calculus.The spectral HU curve and histogram of 63 urinary calculi were created.Kruskal-Wallis U test and ROC curve analysis were used to evaluate the capability of these parameters to differentiate the urinary calculi components.Results:CT values of uric acid stones were as follows:(403±112)HU on routine scan images,(341±127)HU on 40keV MONO images,(404±119)HU on 140keV MONO images.CT values of non-uric acid stones were as follows:(854±335)HU on routine scan images,(1689±853)HU on 40keV MONO images,(445±130)HU on 140keV MONO images.The CT values of uric acid stone and non-uric acid stone were different on both routine scan(P0.01) and 40keV MONO images(P0.01).But the CT values of uric acid stone and non-uric acid stone were not significantly different(P=0.436) on 140keV MONO images.Eff-Z of uric acid stones and non-uric acid stones were significantly different(P0.01).Seven in 9(77.8%) uric acid stones demonstrated rising spectral energy curve,whereas the other 56 non-uric acid stones all demonstrated reversed spectral energy curve.Conclusion:Dual energy CT is a promising noninvasive imaging modality that can show the characterization of uric acid and non-uric acid stones.

Efficacy of single-source rapid kV-switching dual-energy CT for characterization of non-uric acid renal stones: a prospective ex vivo study using anthropomorphic phantom

Abstract:Purpose: To investigate the accuracy of rapid kV-switching single-source dual-energy computed tomography (rsDECT) for prediction of classes of non-uric-acid stones. Materials and methods: Non-uric-acid renal stones retrieved via percutaneous nephrolithotomy were prospectively collected between January 2017 and February 2018 in a single institution. Only stones ≥ 5 mm and with pure composition (i.e., ≥ 80% composed of one component) were included. Stone composition was determined using Fourier Transform Infrared Spectroscopy. The stones were scanned in 32-cm-wide anthropomorphic whole-body phantom using rsDECT. The effective atomic number (Zeff), the attenuation at 40 keV (HU40), 70 keV (HU70), and 140 keV (HU140) virtual monochromatic sets of images as well as the ratios between the attenuations were calculated. Values of stone classes were compared using ANOVA and Mann-Whitney U test. Receiver operating curves and area under curve (AUC) were calculated. A p value < 0.05 was considered statistically significant. Results: The final study sample included 31 stones from 31 patients consisting of 25 (81%) calcium-based, 4 (13%) cystine, and 2 (6%) struvite pure stones. The mean size of the stones was 9.9 ± 2.4 mm. The mean Zeff of the stones was 12.01 ± 0.54 for calcium-based, 11.10 ± 0.68 for struvite, and 10.23 ± 0.75 for cystine stones (p < 0.001). Zeff had the best efficacy to separate different classes of stones. The calculated AUC was 0.947 for Zeff; 0.833 for HU40; 0.880 for HU70; and 0.893 for HU140. Conclusion: Zeff derived from rsDECT has superior performance to HU and attenuation ratios for separation of different classes of non-uric-acid stones.

Abstract:Objective: To evaluate the diagnostic accuracy of a new energy and radiation dose-reduced protocol for noncontrast computed tomography (NCCT) with dual-energy CT (DECT) analysis and its potential for the compositional analysis of uric acid (UA)- or non-uric acid (N-UA)-containing calculi. Methods: A retrospective evaluation was carried out in 61 patients who underwent dose-reduced DECT (tube A: 140 kV/55 mAs; tube B: 80 kV/303 mAs) with a tube current 38.8% lower than that set by the manufacturer. A protocol combining low-dose CT and targeted DE scans was used. Urinary stones were detected and classified as UA- or N-UA-containing or mixed based on DE software results. The accuracy of the compositional analysis was controlled by correlation with conventional infrared-based analysis. Results: The compositional stone differentiation was correct in 58 of 61 (95.1%) patients. The sensitivity of detecting pure UA-containing and pure N-UA-containing stones was 100%. The specificity of detecting UA- and pure N-UA-containing stones was 100% and 78.57%, respectively, as 3 of 7 mixed urinary stones (small fragments <4 mm) were classified as N-UA calculi. The total radiation dose in patients with body mass index <25 and >25 kg/m(2) was 1.2 and 2.5 mSv, respectively. Conclusion: Lowering the DECT tube current by up to 38% of the manufacturer's recommendations allows a reduced radiation dose without impairing detection accuracy and stone compositional analysis. Compared with previous studies, this protocol might significantly decrease patient radiation exposure without affecting the quality of results.

Optimal dual-energy computed tomography scan parameters to detect small-sized urinary stones and their composition

Abstract:To investigate the optimal scanning parameters of dual-energy computed tomography (DECT), which can accurately determine sensitivity (the detectability of urinary stones) and accuracy (the composition matching of urinary stones), and to apply them to clinical trials. Fifteen urinary stones were chemically analyzed, and their chemical compositions were considered a reference standard with which we compared the uric acid (UA) and non-UA compositions determined using DECT. The urinary stones were placed inside a bolus and scanned with a dual-source CT scanner under various selected dual-energy conditions (A to X) using various solid water phantom thicknesses. These datasets were analyzed using the Siemens syngo .via software tool (integrated into the CT system) for matching the sensitivity and accuracy assessments. This study showed that 80% of the highest sensitivity (detection of urinary stones) and 92% of the highest accuracy (composition matching of urinary stones) were achieved under condition A (a collimation beam width setting of 2 × 32 × 0.6 mm, an automatic exposure control setting of 80/sn140 peak kilovoltage, and a slice thickness of 0.5/0.5 mm) ( P < 0.05). Application of the DECT energy parameters presented in the study will help identify the sensitivity and accuracy of UA and non-UA stone analysis, even in patients with small-sized urinary stones and in conditions difficult for analysis.

Abstract:Objectives: To predict the main component of pure and mixed kidney stones using dual-energy computed tomography and machine learning. Methods: 200 kidney stones with a known composition as determined by infrared spectroscopy were examined using a non-anthropomorphic phantom on a spectral detector computed tomography scanner. Stones were of either pure (monocrystalline, n = 116) or compound (dicrystalline, n = 84) composition. Image acquisition was repeated twice using both, normal and low-dose protocols, respectively (ND/LD). Conventional images and low and high keV virtual monoenergetic images were reconstructed. Stones were semi-automatically segmented. A shallow neural network was trained using data from ND1 acquisition split into training (70%), testing (15%) and validation-datasets (15%). Performance for ND2 and both LD acquisitions was tested. Accuracy on a per-voxel and a per-stone basis was calculated. Results: Main components were: Whewellite (n = 80), weddellite (n = 21), Ca-phosphate (n = 39), cysteine (n = 20), struvite (n = 13), uric acid (n = 18) and xanthine stones (n = 9). Stone size ranged from 3 to 18 mm. Overall accuracy for predicting the main component on a per-voxel basis attained by ND testing dataset was 91.1%. On independently tested acquisitions, accuracy was 87.1-90.4%. Conclusions: Even in compound stones, the main component can be reliably determined using dual energy CT and machine learning, irrespective of dose protocol. Abstract:Background: Dual-energy computed tomography (DECT) utilises the technique of simultaneous image acquisition of a body part at two different energy levels of the X-ray tubes. We used the DECT in this project to study its accuracy in non-invasive in vivo differentiation of uric acid (UA) calculi from non-UA calculi using the 3rd generation dual-source computed tomography (CT) scanner. Materials and methods: The DECT ratios of the various stones were noted and compared with the postextraction analysis of stones for composition. Sixty stones from 53 patients were included in the study, and their DECT results were compared with the post-extraction analysis (by infrared spectroscopy as the standard comparative method). Results: DECT was highly sensitive and specific in the diagnosis of various types of calculi based on their dual-energy ratio. It was found to be 100% sensitive and specific for differentiating UA stones from the non-UA stones. The DE ratio for the UA stones was found to be 0.98-1.13. Most of the stones belonged to non-UA category (85%), and only 15% were UA calculi. The sensitivity and specificity in differentiating a calcium oxalate from non-calcium oxalate calculus was 97.8% and 92.3%, respectively. Conclusion: DECT using the 3rd generation CT scanners is highly sensitive in the determination of urinary calculi composition which will be very helpful to the treating surgeon in selecting the optimal modality of treatment as UA stones are amenable to medical therapy while most of the non-UA stones require surgical intervention.
Characterization of urinary stones with dual-energy CT: improved differentiation using a tin filter

Abstract:Purpose: To assess image quality and capability of stone differentiation between UA-containing and non-UA-containing uroliths with the latest dual-energy (DE) computed tomography (CT) system equipped with a tin filter (TF) using various data acquisition parameters in a work bench model. Methods and materials: One hundred ten urinary stones (4.2 +/- 3.0 mm, 0.4-12 mm) of 15 compositions were examined in an ex vivo phantom, using DE dual-source CT (Definition Flash, Siemens Healthcare) equipped with a TF. Phantom was scanned in a water tank and contained stones in acrylic elliptic spheres filled with a parenchyma substitute. Scans were performed at 3 different settings: at 80 and 140 kVp without TF, at 80 and 140 kVp with TF, and at 100 and 140 kVp with TF. Tube current time products were adapted to yield constancy in CT dose indices (CTDIvol = 18.84 mGy, 18.95 mGy, and 18.90 mGy, respectively). CT numbers of urinary stones and image noise were electronically measured by placing regions of interest. DE indices (DEI) were calculated and compared using analysis of variances for repeated measures and paired t tests; image noise (IN) using the Friedman test. The stones were classified as UA-containing or non-UA-containing on color-coded images based on the DEI. Diagnostic accuracy was calculated using crystallographic analysis as standard of reference. Results: Of the 110 stones (60%), 65 contained UA; 45 stones (40%) contained no UA. DEI was greatest at 80 and 140 kVp when using the TF (DEI80 kVp/TF140 kVp = 0.038 vs. DEI80 kVp/140 kVp = 0.028, DEI100 kVp/TF140 kVp = 0.025; P < 0.01). IN of high kVp acquisitions were similar (P = 0.15), whereas IN of low kVp acquisitions were significantly (P < 0.001) different being lowest at 100 kVp. The semiautomated DE software correctly classified all stones at all settings with a diagnostic accuracy of 100% (95% confidence interval: 97%-100%). Conclusion: DECT with TF and 80-140 kVp tube voltage settings significantly improves the discrimination between UA-containing and non-UA containing urinary stones as compared with DECT without using the TF on the basis of DEI. The 100/140 kVp setting with TF is associated with lower IN but demonstrates similar discrimination abilities as compared with 80/140 kVp setting without the use of the TF.

Abstract:OBJECTIVE:To study the value of Hounsfield unit (HU) on CT in prediction of stone-free rate of upper urinary calculi after extracorporeal shockwave lithotripsy (ESWL).METHODS:Fifty-two patients suffering from solitary stone in the upper urinary tract 0.5 - 1.5 cm in size, 34 males and 18 females, aged 15 - 77, underwent non-contrast helical computerized tomography (NCHCT), and then ESWL. One day, 2 weeks, 4 weeks, and 3 months after the ESWL the patients underwent kidney, ureter, and bladder examination and B mode ultrasonography. The patients with the residual fragments less than 3 mm in size were categorized into the stone-free group. The relationship among the HU, number of shock wave, and the characteristics of the patients were analyzed.RESULTS:Thirty-six of the 52 patients were categorized into the stone-free group (69.2%) and 16 of the 52 were categorized into the residual stone group (30.8%). The CT value of the stone-free group was 579.65 +/- 194.65 HU, significantly lower than that of the residual stone group (1032.18 +/- 270.49 HU, t = 6.842, P < 0.01). The number of shock wave of the stone-free group was 1526.67 +/- 723.01, significantly lower than that of the residual stone group (2612.50 +/- 518.81, t = 5.407, P < 0.01). The OR values for the HU and number of shock wave were 0.993 (95% CI: 0.988 - 0.998) and 0.998 (95% CI: 0.997 - 1.000).CONCLUSION:The HU value on pretreatment NCHCT of the upper urinary tract stones can be used to predict the stone-free rate after ESWL. ESWL is not indicated to the patients of upper urinary tract stones with high values of HU.
