Abstract:To investigate changes in anterior chamber depth (ACD) and the distance between the Visian implantable Collamer lens (ICL) and the crystalline lens during pharmacologic accommodation in high myopia.Prospective, comparative case series.Thirty-three phakic eyes of 18 high myopic patients (range, -8.63 to -23.86 diopters) with a mean age of 29 years (range, 20-44 years) were examined at least 1 year after ICL implantation.Ultrasound biomicroscopy was used to measure distance changes between the corneal endothelium, the ICL, and the crystalline lens after inducing pharmacologic accommodation with topical pilocarpine in 1 eye. The contralateral eye served as the control.Mean changes of ACD measured from the posterior corneal surface to the crystalline lens (ACD-L), from the posterior corneal surface to the anterior surface of the ICL (ACD-ICL), and the distance between the ICL and the crystalline lens (ICL-L) at the central and peripheral regions of the eye.For each eye, the ICL was in contact with the iris, but it was never in contact with the crystalline lens. At baseline, the mean distance between the ICL and the crystalline lens was 0.609 ± 0.165 mm at the central horizontal meridian, 0.588 ± 0.157 mm at the central vertical meridian, 0.281 ± 0.106 mm at the peripheral temporal sulcus, and 0.290 ± 0.098 mm at the peripheral nasal sulcus. After instillation of pilocarpine, a significant decrease in ICL-L was accompanied by a significant reduction in ACD-L and an increase in ACD-ICL (P<0.01). There were no significant changes in the control eyes (P>0.05). The central ICL-L reduction in the study group was significantly larger than that in the control group (P<0.01), but the peripheral ICL-L changes in the study group were not significantly different from those in the controls (P>0.05).During pharmacologic accommodation, the ICL and the crystalline lens came closer as the ICL was pushed backward by the iris as a result of pupillary constriction. Simultaneously, the anterior surface of the crystalline lens became more convex and moved forward. Accommodation Changes after Visian Implantable Collamer Lens with Central Hole for High Myopia A STROBE-compliant Article

Abstract:To characterize the accommodative changes in high-myopic patients after the implantation of the Visian implantable collamer lens with a central hole (ICL V4c). This prospective study enrolled 30 patients (60 eyes) with uneventful surgery of ICL V4c implantation. Parameters including amplitude of accommodation (AA), monocular and binocular facility of accommodation (FA), positive relative accommodation (PRA), negative relative accommodation (NRA), near point convergence (NPC), accommodative response, and accommodation convergence/accommodation (AC/A) ratio were assessed before surgery, at 1 and 3 months postoperatively. Mean preoperative SEwas -10.86 +/- 3.87 diopter (D) (range, -6.5D to -22D), which improved to 0.27 +/- 0.51D at 1 month and 0.09 +/- 0.47D at 3 months after surgery (P<.001). Significant improvements in AA, NRA, PRA, NPC, monocular, and binocular FA were seen at 1 month and 3 months postoperatively compared to the values before surgery (P<.05), but the difference between 1 month and 3 months were not obvious (P>.05) except for binocular FA (P=.002). However, no significant changes were seen in either AC/A ratio or accommodative response at any postoperative follow-ups in contrast to those before surgery (P>.05). Similar changes in accommodative function were found in patients with less myopia (> -10.00D) and those with more myopia (<= 10.00D) (P>.05). The accommodative function of eyes after the implantation of ICL V4c is enhanced and stabilizes at 1 month, except for the AC/A ratio and accommodative response. The clearer vision and increased amount of accommodation for near target account for the majority of the improvement.
Observation on Changes of Anterior Chamber and Chamber Angle Structures after Posterior Chamber Phakic Intraocular Lens Implantation by Ultrasound Biomicroscopy

Abstract:OBJECTIVE:To study the changes of anterior chamber and chamber angle structures after implantable contact lens (ICL) implantation in high myopia by using ultrasound biomicroscopy (UBM).METHODS:A prospective series case study was conducted on 15 high myopia patients (30 eyes) treated with ICL implant. These patients were followed for one year postoperatively. Intraocular pressure (IOP) was measured by Goldmann applanation tonometer after surgery. All eyes were examined by UBM one year after the surgery. Central anterior chamber depth (ACD), trabecular-iris angle (TIA), the distance between the posterior surface of ICL and the lens and the distance between peripheral surface of ICL and the lens were measured. Nonparametric test was used to compare TIA. One-way ANOVA was used to assess the distance between peripheral surface of ICL and the lens. Repeated Measure ANOVA and Bonferroni test was performed to compare the IOP level before and after surgery. Paired t-test was used to compare ACD.RESULTS:Intraocular pressure changed from (13.75 +/- 2.27) mm Hg (1 mm Hg = 0.133 kPa) preoperatively to (14.27 +/- 1.70), (14.70 +/- 2.07), (14.07 +/- 2.24), (14.00 +/- 2.69) and (13.97 +/- 2.95) mm Hg at 1 week, 1, 3, 6 and 12 months after the surgery, respectively. There was a statistically significant increase of the IOP one month after the surgery, which was normalized completely at 3 months after surgery. The distances between central posterior surface of the cornea to the ICL and the lens were (2.24 +/- 0.21) and (2.97 +/- 0.11) mm, respectively. There was significant difference between these two data (t = 20.63, P < 0.01). TIA was measured in 12:00, 3:00, 6:00 and 9:00. There were no statistically differences between them. TIA was greater than 30 degree in 55.8% of cases. The distance between the central anterior surface of the ICL to the lens (central section) was (0.63 +/- 0.16) mm. No contact was found between the ICL and the crystalline lens.CONCLUSIONS:Posterior chamber phakic intraocular lens implantation for the surgical correction of high myopia is a safe procedure as evaluated with immediate visual and refractive results. We found that the iridocorneal angle and the anterior chamber depth are decreased after the surgery. There is no contact between the crystalline lens and ICL. The ICL contacts with the posterior iris surface.

Abstract:The purpose of this article is to evaluate the clinical outcomes of a posterior chamber phakic intraocular lens (pIOL) (Visian Implantable Collamer Lens V4c) for the correction of moderate to high myopia in Chinese eyes.The article is designed as a retrospective case series.This study included the first consecutive eyes that had implantation of a new pIOL design with a central hole, at our department by the same surgeon. The safety, efficacy, predictability, stability, and adverse events of the surgery were evaluated over 6 months.The study enrolled 63 eyes (32 patients). The mean spherical equivalent decreased from -12.813.11 diopters (D) preoperatively to -0.05 +/- 0.27D 6 months postoperatively; 96.8% of eyes were within +/- 0.50D of the target and 100% of eyes were within +/- 1.00D. All eyes had a decimal uncorrected distance visual acuity of 0.5 (20/40) or better at every follow-up visit. The safety and efficacy indices were 1.42 +/- 0.34 and 1.11 +/- 0.19, respectively. Postoperatively, the intraocular pressure (IOP) remained stable over time. No significant rises in IOP (including pupillary block) and no secondary cataract were found. After 6 months, the mean vault was 505.2 +/- 258.9m (range 120-990m), and the mean endothelial cell loss was 2.0%.Implantation of the pIOL was safe, effective, predictable, and stable in the correction of moderate-to-high myopia in Han Chinese patients, even without peripheral iridectomy.
Posterior Chamber Phakic Intraocular Lens Implantation for High Myopia

Abstract:Objective. To evaluate the efficacy, safety and stability of posterior chamber phakic intraocular lens (PIOLs) implantation for the correction of high myopia.Methods. Thirty-nine eyes of twenty patients with high myopia ( between -11.75 and -25.75 diopters) had a posterior chamber PIOL (Staar ICL) implanted. During 6 - 48 months' follow-up, visual acuity, refraction, intraocular pressure (IOP), corneal reaction and space between crystal lens and intraocular lens (IOLs) were tested.Results. Successful implantation was achieved in all patients. Visual acuity without correction greater than 0.5 was found in 34 eyes at 1 day and 3 months postoperatively. Thirty-five eyes maintained a low negative power of refraction (-1.42 +/- 1.32 doipters), which did not prevent the patients from most of their daily activities. During 3 - 48 months' follow-up, refraction was stable and no cornea edema and glaucoma was found. Two eyes of one patient had corticosteroid glaucoma and another eye showed cataractogenesis under anterior capsular membrane.Conclusion. Posterior chamber PIOL implantation is predictable, safe, and effective in the correction of high myopia, and its indications should be carefully selected.
Comparative Study of Implantable Collamer Lens Implantation in Treating Four Degrees of Myopia: Six-Month Observation of Visual Results, Higher-Order Aberrations, and Amplitude of Accommodation

Abstract:Purpose: To compare the visual and refractive outcomes, higher-order aberrations (HOAs), and amplitude of accommodation (AA) after implantable collamer lens (ICL) model V4c implantation in four degrees of myopia. Methods: One hundred and thirty-seven myopic eyes (137 patients) undergoing ICL implantation were included and divided into four groups: Group 1 with spherical equivalent (SE) <=-6.0D, Group 2 with SE from -6.13D to -9.0D, Group 3 with SE from -9.13D to -12.0D, and Group 4 with SE from -12.13D to -18.0D. The postoperative visits were scheduled at 1 day, 1 week, 1 month, 3 months, and 6 months. Visual and refractive outcomes, HOAs and AA were observed and compared. Results: At 6 months postoperatively, the uncorrected distance visual acuity (UDVA) in Group 4 was worse than the values in the other groups (all p < .05). Meanwhile, Group 4 had more eyes with corrected distance visual acuity (CDVA) improvement than the other groups. Furthermore, 96%, 100%, 100%, and 81% of eyes had an SE within +/- 0.5D in Group 1, Group 2, Group 3, and Group 4 (p < .05 between Group 4 and the other groups), respectively. The postoperative UDVA and SE remained stable in all groups. No significant change in total HOAs was found between preoperative and postoperative values for each group. However, quatrefoil increased in each group, whereas trefoil was induced in all groups except for Group 1. Negative spherical aberration was induced in Group 3 and Group 4. AA significantly decreased 1 week postoperatively and gradually improved in each group. Although AA improved to the preoperative level in Group 1 at 3 months postoperatively, it was still lower than the preoperative level in the other groups. Conclusions: In treating different degrees of myopia with ICL implantation, differences were observed in terms of CDVA improvement, SE predictability, HOAs induction, and accommodation recovery.
Comparison of accommodation and accommodative micro-fluctuation after implantable collamer lens and LASIK surgery for myopia

Abstract:Background: To longitudinally analyze and compare the accommodative micro-fluctuation (MFs) and accommodative function between myopic patients after implantable collamer lens (ICL) implantation and laser in situ keratomileusis (LASIK). Methods: Patients with good corrected visual acuity (20/20 or better) and underwent ICL (V4c) and LASIK for myopic-correction (ranging from - 3.50 to - 8.50 D) were recruited. Refraction, amplitude of accommodation (AMP), accommodative lag, higher-order aberration (HOA), and MFs were recorded before surgery and 1 and 3 months after surgery. The ACOMEREF automatic refractor was used to measure the high-frequency component (HFC) of the MFs, which suggested tension of the ciliary muscle. Results: The study comprised 120 eyes. At 3 months after surgery, the manifest refractive spherical equivalent of the ICL and LASIK groups were - 0.11 and - 0.09 D, respectively (p = 0.46). HFC values were significantly higher at 1 month (p = 0.03) and 3 months postoperatively (p = 0.03) in the ICL group compared to that in the LASIK group. The ocular HOA of the ICL group was 1.08 ± 0.43 μm, which was lower than the LASIK group 1.45 ± 0.54 μm (p = 0.01). No significant differences in AMP and accommodative lag between groups were noted at 3 months postoperatively. There was a positive correlation between HFC and vault of the ICL lens (r2 = 0.14, p = 0.005). There were no correlations between HFC and ocular HOA and postoperative MRSE in the two groups (all p>0.05). Conclusions: The HFC increased significantly after an early period of ICL implantation compared to laser in situ keratomileusis for myopic correction, which indicated increased tension of the ciliary muscle, and had a positive correlation on the vault of the ICL lens; However, studies with longer follow-up time and more structural evaluation are needed.
Short-term Evaluation of Visual Quality, Amplitude of Accommodation, and Stereoacuity Between Patients with Moderate-to-High Myopia Who Underwent ICLV4c Implantation and SMILE.

Abstract:PURPOSE: To compare the visual quality, accommodation, and stereoacuity between patients with a myopia magnitude of-3.00 to-8.50 diopters (D) who underwent ICLV4c (STAAR Surgical) implantation and small incision lenticule extraction (SMILE). METHODS: The visual quality parameters, amplitude of accommodation (AMP), and stereoacuity were compared between and within the ICLV4c and SMILE groups (40 eyes/ group) with a mean spherical equivalent (SE) of-6.14 D. Differences in these parameters before and after surgery were analyzed using SPSS 22.0 software (SPSS, Inc).RESULTS: Modulation transfer function (MTF) cut-off increased significantly by 3.24 cycles/degree (t =-2.111, P =.041) and AMP decreased significantly by 1.66 D (t = 7.519, P < .001) from baseline to 3 months after surgery in the ICLV4c group. However, the MTF cut-off was significantly lower (t = 2.123, P = .037) and the AMP was significantly better (t =-3.605, P = .001) in the SMILE group than in the ICLV4c group at 3 months postoperatively. Other parameters such as Objective Scatter Index, Strehl ratio, and Optical Quality Analysis System (Visiometrics) under different contrast values did not show significant changes (P > .05). Stereoacuity did not improve significantly, except in a patient with anisometropia (2.50 D) in the ICLV4c group.CONCLUSIONS: The ICLV4c implantation yielded a better visual quality but worse AMP than did SMILE in patients with a myopia magnitude of-3.00 to-8.50 D in the short term.
Psychometric analysis of the Functional Independence Score in Haemophilia (FISH)

Abstract:Summary. Joint morbidity in haemophilia has traditionally been measured using clinical and radiological scores. There have been no reliable, validated tools for the assessment of functional independence in persons with haemophilia till recently. The Functional Independence Score in Haemophilia (FISH) has been developed as a performance based assessment tool to address this need. The FISH is designed to measure the patient’s independence in performing activities of daily living (grooming and eating, bathing and dressing), transfers (chair and floor), and mobility (walking, step climbing and running). On assessment of its psychometric properties in 63 patients with haemophilia (mean age 14 years), FISH was found to have good internal consistency (Cronbach’s alpha of 0.85). It had moderate correlation with the World Federation of Hemophilia clinical score (r = −0.61), and a correlation with the Pettersson score of −0.38. It had good correlation with other self‐rated functional scores, such as the Stanford Health Assessment Questionnaire (r = −0.75); the Western Ontario and McMaster Universities Osteoarthritis Index (r = −0.66) and the Haemophilia Activities List (HAL) (r = −0.66). It had good reliability with a pooled intra class correlation of 0.98. On assessing responsiveness following treatment of flexion deformities of the knee in 12 patients, the FISH showed significant changes in the score with a standardized responsiveness mean of −1.93. In conclusion, the FISH was found to be a reliable and valid tool with good internal consistency and responsiveness to therapy, for the assessment of functional independence in persons with haemophilia.
Effects of Lighting Conditions and Accommodation on the Three-Dimensional Position of Visian Implantable Collamer Lens

Abstract:Background To investigate the effects of lighting conditions and accommodation on the three-dimensional position of Visian implantable collamer lens (ICL V4c). Methods This observational study recruited 62 eyes of 31 myopia patients underwent ICL V4c implantation. Anterior segment optical coherence tomography (AS-OCT) assessed the anterior chamber depth (ACD), ACD-ICL (distance from the corneal endothelium to anterior surface of the ICL V4c), vault (distance between the posterior ICL V4c surface and anterior crystalline lens surface), and crystalline lens tilt under various lighting conditions and accommodation relative to the corneal topographic axis at one year after ICL V4c implantation. Baseline was defined as the scotopic condition, which was also the non-accommodative stimulus condition. The ICL V4c tilt was analyzed using MATLAB. The significance level was set at P < 0.05. Results The ACD-ICL values were similar under various lighting conditions ( P = 0.978) but decreased during accommodation ( P < 0.001). The vault was significantly smaller under mesopic and photopic conditions than the baseline ( P = 0.044 and P < 0.001, respectively) but remained unchanged during accommodation ( P = 0.058). The inferotemporal proportion of ICL V4c (88.7%, 55 eyes) and crystalline lens (74.2%, 46 eyes) tilts were not significantly different ( P = 0.063). Crystalline lens under various lighting conditions and accommodation exhibited similar tilts. The vertical tilt of ICL V4c was significantly larger under photopic conditions than the baseline ( P = 0.038). The horizontal and total tilts were significantly decreased during accommodation ( P = 0.043 and 0.013, respectively). Conclusions The axial position of ICL V4c in the anterior chamber was stable under various lighting conditions. Lighting conditions and accommodation may influence vertical, horizontal and total tilts of ICL V4c.
Visual function assessment of posterior-chamber phakic implantable collamer lenses with a central port

Abstract:Background: To investigate the short-term visual function of the V4c implantable collamer lens (ICL) for myopia using a Binoptometer 4P.Methods: Eighty eyes from 40 patients (age, 28.75±6.57 years) who underwent ICL V4c implantation at Shenzhen Eye Hospital from August 2020 to June 2021 were enrolled. Uncorrected visual acuity (UCVA), best corrected visual acuity (BCVA), subjective manifest refraction, intraocular pressure (IOP), and corneal endothelial cell density (ECD) were evaluated 1 month after surgery. Corrected distance visual acuity (CDVA), corrected near visual acuity (CNVA), contrast sensitivity (CS), near stereoacuity (NSA), twilight vision, and glare sensitivity were measured using a Binoptometer 4P.Results: The average logMAR (logarithmic minimum angle of resolution) UCVA postoperative was 0.07±0.13, which was significantly better than the preoperative BCVA (P=0.028). At 1 month postoperatively, the mean safety and efficacy indexes were 1.14±0.24 and 0.98±0.16, respectively. The mean spherical equivalent (SE) of all eyes was -8.98±3.20 D preoperatively, and -0.16±0.48 D postoperatively. None of the patients lost one or more lines of BCVA, 62.1% remained unchanged, 10.6% gained one line, and 27.3% gained two or more lines. CDVA, CNVA, CS, and NSA of both eyes were measured using a Binoptometer 4P, which were significantly better than the preoperative values. In addition, some patients had subjective symptoms in the early postoperative period, such as halo, glare, etc.Conclusions: ICL V4c implantation is a safe, effective, and predictable solution for myopia. We found that the short-term visual function of patients was improved significantly. The Binoptometer 4P is an effective and convenient visual testing device for assessing postoperative visual function after ICL V4c implantation.

Abstract:PurposeTo investigate visual outcomes early after implantable collamer lens (ICL) V4c implantation between patients with fully corrected and under-corrected spectacles preoperatively.MethodsPatients who implanted ICL V4c were divided into the full correction (46 eyes/23 patients) and under-correction groups (48 eyes/24 patients) based on preoperative differences between the spherical diopter of the spectacles and the actual spherical diopter. Refractive outcomes, scotopic pupil size, higher-order aberrations, and subjective visual outcomes as assessed using a validated questionnaire were compared between the two groups 3 months postoperatively. Moreover, the relationships between the severity of haloes and postoperative ocular or ICL parameters were analyzed.ResultsAt the 3-month follow-up, the efficacy indices in the full correction and under-correction groups were 0.99 +/- 0.12 and 1.00 +/- 0.10, respectively; the safety indices were 1.15 +/- 0.16 and 1.15 +/- 0.15, respectively. Total-eye spherical aberration (p < 0.0001) and internal spherical aberration (p = 0.0005) were significantly different pre- and post-operatively in the under-correction group, while no differences were found in the full correction group. Total-eye spherical aberration (p = 0.002) and the severity of haloes (p = 0.03) were postoperatively different between the two groups. The severity of haloes was associated with postoperative spherical aberration (total-eye spherical aberration: r = -0.32, p = 0.002; internal spherical aberration: r = -0.24, p = 0.02).ConclusionGood efficacy, safety, predictability, and stability were obtained early after surgery regardless of preoperative spectacle correction. Patients in the under-correction group possessed a shift to negative spherical aberration and reported greater severity of haloes at the 3-month follow-up. Haloes were the most common visual symptoms after ICL V4c implantation and the severity of them was correlated with postoperative spherical aberration.

Abstract:Objective: To observe the impact of implantable Collamer lens (ICL) implantation surgery on choroidal thickness and blood flow density in myopic patients. Methods: This was a prospective cohort study. Patients undergoing ICL surgery at Qingdao University Affiliated Hospital between June 2021 and May 2023 were consecutively enrolled. Patients were categorized into high myopia (HM) and super high myopia (SHM) groups based on whether their spherical equivalence power exceeded 10.00 D. Comprehensive ophthalmic examinations, including optical coherence tomography, optical coherence tomography angiography, visual acuity assessment, intraocular pressure measurement, and optometry, were performed preoperatively and at 1 week, 1 month, and 3 months postoperatively. Results: A total of 42 patients (84 eyes), with an average age of (25.27±3.18) years, comprising 11 males and 31 females, were enrolled in the study. Among them, 20 patients belonged to the HM group, while 22 patients were in the SHM group. Both choroidal thickness and blood flow density exhibited significant increases at postoperative 1 week and 1 month compared to preoperative levels (P<0.05), but returned to baseline levels by postoperative 3 months. Specifically, the subfoveal choroidal thickness increased from (169.49±61.57) μm preoperatively to (180.16±66.61) μm at 1 week, (186.69±63.32) μm at 1 month, and then reverted to (169.58±60.82) μm at 3 months. The central choroidal blood flow density showed changes from 60.03%±1.60% preoperatively to 61.04%±1.17% at 1 week, 60.42%±1.81% at 1 month, and 60.22%±1.57% at 3 months. Furthermore, the HM group exhibited more pronounced changes in both choroidal thickness and blood flow density across all time points compared to the SHM group. Significant differences were observed in choroidal thickness changes at various areas at 1 month, while changes in blood flow density in specific areas were significant. However, no significant differences were noted at 3 months postoperatively. Correlation analysis revealed a negative correlation of changes in subfoveal choroidal thickness and central choroidal blood flow density postoperatively at 1 week and 3 months with preoperative choroidal blood flow density. Notably, no correlation was found between preoperative choroidal thickness and postoperative changes. Conclusions: In the early period following ICL implantation, the increase in choroidal thickness and blood flow density may be more pronounced in HM compared to SHM, but the two parameters can return to baseline levels by 3 months. ICL implantation transiently affects the fundus microenvironment in myopic patients, with implications of preoperative choroidal blood flow.
Retinal and Choroidal Changes Following Implantable Collamer Lens V4c Implantation in High Myopia Patients-A 1-Year Follow-Up Study

Abstract:This prospective study aimed to evaluate the impact of Visian Implantable Collamer Lens (ICL) V4c implantation on retinal and choroidal morphology in patients with high myopia. A total of 97 eyes from 52 high myopic patients who underwent ICL V4c implantation were followed up for 12 months. Preoperative and postoperative evaluations included comprehensive ophthalmic assessments and enhanced depth imaging optical coherence tomography (EDI-OCT) to analyze changes in central retinal thickness (CRT), retinal volume (CRV), choroidal thickness (ChT), total choroidal area (TCA), luminal area (LA), and choroidal vascular index (CVI). Repeated measures mixed-effects models were used for comparing pre- and postoperative measurement variables and exploring relationships among age, axial length (AL), spherical equivalent refraction (SER), and postoperative retinal and choroidal changes, with statistical significance set at p < 0.05. Follow-up assessments were conducted at various time points, with participation rates ranging from 21% to 98%. Baseline characteristics showed a median age of 26.7 years, -10.14 diopters of SER, and an AL of 27.44 mm. Throughout the 12-month follow-up, CRT and 3.0 mm CRV consistently increased compared to the baseline, with statistically significant rises observed at postoperative day 1, week 1, and month 12. Most ChT measurements, including subfoveal ChT, declined over the 12 months, except at postoperative 6 months. Horizontal and vertical TCA and LA values significantly increased throughout the follow-up, except for month 6. After surgery, both horizontal and vertical CVI parameters exhibited an increase compared to the baseline, with some changes reaching statistical significance. Correlation analysis performed by repeated measures mixed-effects models showed that no relationship was found between age, AL, and SER and changes in postoperative retinal parameters and CVI parameters. However, postoperative changes in ChT and choroidal area parameters showed a negative correlation with AL and a positive correlation with SER. Our research demonstrated that ICL V4c implantation resulted in noteworthy alterations in retinal and choroidal morphology over a 1-year follow-up period. Moreover, in patients with high myopia, individuals with longer AL and higher degrees of myopia exhibited more pronounced postoperative changes in the choroid and retina. Further studies with extended follow-up durations are necessary to comprehensively understand the long-term effects of ICL implantation on retinal and choroidal morphology and function.

Abstract:Purpose: To investigate the refractive stability, axial length changes and their related factors in a high myopia population after Implantable Collamer Lens (ICL) implantation.Methods: This prospective study included 116 eyes of 116 patients divided into several groups based on the spherical equivalent refractive error (SE)—SE > -6D, -12 ≤ SE < -6D and SE < -12D groups—and axial length (AL)—AL < 28mm and AL ≥ 28mm groups. The uncorrected and corrected distance visual acuity, refraction, axial length and intraocular pressure were followed for 1 year.Results: SE changed from -11.53 ± 5.25D preoperatively to -0.33 ± 0.70D at 1 week, and further changed to -0.48 ± 0.77D at 1 year after ICL implantation, with average progression being -0.15 ± 0.37D from 1 week to 1 year after surgery. Axial length changed from 27.95 ± 2.33mm preoperatively to 27.98 ± 2.36mm 1 year after surgery, with an average axial elongation of 0.03 ± 0.12mm. The mean axial elongation rate was 0.05mm/year in the SE < -12D group, being significantly faster than the other SE groups (P < 0.05); it was 0.06mm/year in the AL ≥ 28mm group, being significantly faster than the AL < 28mm group (P < 0.05).Conclusion: Patients with high myopia and long axial length showed a continuous myopic progression and axial elongation at an adult age after ICL surgery, especially those with myopia higher than -12.0D and AL longer than 28.00mm.

Abstract:PURPOSE:To investigate two-year outcomes of Visian Implantable Collamer Lens with a central hole (ICL V4c) implantation for correcting high myopia. METHODS:Sixty-one eyes of 32 patients went through ICL V4c implantation. Safety, efficacy, predictability, and intraocular pressure were evaluated 2 years postoperatively. Anterior chamber volume (ACV), anterior chamber depth (ACD), anterior chamber angle width (ACAW), and vault were measured using a Scheimpflug tomography imaging system. RESULTS:At 2 years, the spherical equivalent refraction decreased from preoperative -14.62 ± 4.29 D to -0.90 ± 0.95 D, with 79% of the eyes within ±0.50 D and 98% within ±1.00 D of the intended correction. The efficacy index was 1.03 ± 0.23, and the safety index was 1.24 ± 0.26. Corrected distance visual acuity (CDVA) remained unchanged in 23% of the eyes, 75% gained one or more lines of CDVA, and 2% lost one line. The ACV, ACD, and ACAW, respectively, decreased from 193.28 ± 29.15 mm3, 3.15 ± 0.23 mm, and 36.51 ± 6.54 degree to 112.48 ± 17.01 mm3, 2.99 ± 0.23 mm, and 22.54 ± 5.27 degree (p=0.0008, 0.008, and 0.0003, resp.). Intraocular pressure was 15.39 ± 2.88 mmHg before surgery and was 15.86 ± 4.11 mmHg at 2 years (p=0.11). CONCLUSION:Implantation of ICL V4c is a safe, effective, and predictable procedure for correcting high myopia. Reduction of anterior chamber space after surgery did not induce intraocular pressure increase during the 2-year follow-up.

Abstract:OBJECTIVE:To observe clinical effect of accommodative IOL on different age patients.METHODS:Retrospective case series of fifty-four patients implanted with accommodative IOL were divided into three groups according to their age: 39-60, 60-75 and older than 75 years. Three months after the cataract surgery, distant visual acuity, near visual acuity and defocusing with minus glasses (D) were examined and depth of anterior chamber was measured by ultrasound biomicroscope.RESULT:There were no statistically significant differences in distant visual acuity among these three groups. Near visual acuity in older than 75 group was significantly lower than the other two groups. There were no statistically significant differences in the depth of anterior chamber among these three groups. When patients watched the target moving from 5 meters away to near, the change of depth of anterior chamber in older than 75 group was significantly less than that in the other two groups. Accommodation power in older than 75 group was significantly lower than that in the other two groups as measured by defocusing with minus glass (D).CONCLUSION:Patients implanted with accommodative IOL can get good distant and near visual acuities. Younger patients may obtain more benefit from accommodative IOL such as obtaining higher accommodative power.

Abstract:Objective To evaluate accommodation and visual function after implantation of the 1CU accommodative posterior chamber intraocular lens and compare it to conventional intraocular lenses.Methods The prospective study analyzed 37 eyes of 32 patients (mean age=67.6 years) after cataract surgery and PC-IOL implantation (1CU, Human Optics) 1, 3 and 6 months after surgery. The results were compared to those for a randomly selected control group (mean age=65.3 years) with implants consisting of the same material (SN60AT Alcon, Human Optics IOL). The following methods were used to measure accommodation: dynamic retinoscopy and a subjective method (Stataper's method); static change in anterior chamber depth (ACD) (with A-scan) with pharmacologic stimulation after 1% pilocarpine eye drops measuring accommodation and visual function.Results Visual acuity and mean accommodation improved after 3 and 6 months in the study group compared to the control group, respectively, as follows: uncorrected near visual acuity, 4.71±0.15 vs 4.34±0.12 and 4.70±0.16 vs 4.30±0.08; amplitude (D), by retinoscopy, 0.93±0.44 vs 0.36±0.12 and 0.92±0.52 vs 0.35±0.13. By Stataper method, it was 3.61±0.31 vs 2.07±0.59 and 3.59±0.36 vs 2.21±0.57. The mean ACD decrease (mm) was 0.55±0.12 vs 0.34±0.20 and 0.56±0.11 vs 0.30±0.12. The differences between preoperative measurements and those at 3 and 6 months after surgery were not statistically significant for the two groups.Conclusion The 1CU accommodating IOL can provide better near visual acuity and accommodative ability compared to the commonly implanted monofocal PC-IOL. But long-term follow-up is necessary to evaluate the stability of accommodation and near vision.

Abstract:56 Abstract A novel switched-capacitor inverter is prop osed. The proposed inverter outputs larger voltage than the i nput voltage by switching the capacitors in series and in parallel. The maximum output voltage is determined by the number of the c apacitors. The proposed inverter, which does not need any inductors , can be smaller than a conventional two-stage unit which cons ist of a boost converter and an inverter bridge. Its output harmonics are reduced compared to a conventional voltage source s ingle phase full bridge inverter. In this paper, the circuit co nfiguration, the theoretical operation, the simulation results with M ATLAB/ SIMULINK, and the experimental results are shown. The experimental results accorded with the theoretical c culation and the simulation results.
