Abstract:Purpose To investigate the feasibility of reducing the acquisition time for continuous dynamic positron emission tomography (PET) while retaining acceptable performance in quantifying kinetic metrics of 2-[ 18 F]-fluoro-2-deoxy-D-glucose ([ 18 F]FDG) in tumors. Methods In total, 78 oncological patients underwent total-body dynamic PET imaging for ≥ 60 min, with 8, 20, and 50 patients receiving full activity (3.7 MBq/kg), half activity (1.85 MBq/kg), and ultra-low activity (0.37 MBq/kg) of [ 18 F]FDG, respectively. The dynamic data were divided into 21-, 30-, 45- and ≥ 60-min groups. The kinetic analysis involved model fitting to derive constant rates ( V B , K 1 to k 3 , and Ki ) for both tumors and normal tissues, using both reversible and irreversible two-tissue-compartment models. One-way ANOVA with repeated measures or the Freidman test compared the kinetic metrics among groups, while the Deming regression assessed the correlation of kinetic metrics among groups. Results All kinetic metrics in the 30-min and 45-min groups were statistically comparable to those in the ≥ 60-min group. The relative differences between the 30-min and ≥ 60-min groups ranged from 12.3% ± 15.1% for K 1 to 29.8% ± 30.0% for V B , and those between the 45-min and ≥ 60-min groups ranged from 7.5% ± 8.7% for Ki to 24.0% ± 24.3% for V B . However, this comparability was not observed between the 21-min and ≥ 60-min groups. The significance trend of these comparisons remained consistent across different models (reversible or irreversible), administrated activity levels, and partial volume corrections for lesions. Significant correlations in tumor kinetic metrics were identified between the 30-/45-min and ≥ 60-min groups, with Deming regression slopes > 0.813. In addition, the comparability of kinetic metrics between the 30-min and ≥ 60-min groups were established for normal tissues. Conclusion The acquisition time for dynamic PET imaging can be reduced to 30 min without compromising the ability to reveal tumor kinetic metrics of [ 18 F]FDG, using the total-body PET/CT system.
Phantom-based image quality assessment of clinical 18F-FDG protocols in digital PET/CT and comparison to conventional PMT-based PET/CT

Abstract:Abstract Background We assessed and compared image quality obtained with clinical 18 F-FDG whole-body oncologic PET protocols used in three different, state-of-the-art digital PET/CT and two conventional PMT-based PET/CT devices. Our goal was to evaluate an improved trade-off between administered activity (patient dose exposure/signal-to-noise ratio) and acquisition time (patient comfort) while preserving diagnostic information achievable with the recently introduced digital detector technology compared to previous analogue PET technology. Methods We performed list-mode (LM) PET acquisitions using a NEMA/IEC NU2 phantom, with activity concentrations of 5 kBq/mL and 25 kBq/mL for the background (9.5 L) and sphere inserts, respectively. For each device, reconstructions were obtained varying the image statistics (10, 30, 60, 90, 120, 180, and 300 s from LM data) and the number of iterations (range 1 to 10) in addition to the employed local clinical protocol setup. We measured for each reconstructed dataset: the quantitative cross-calibration, the image noise on the uniform background assessed by the coefficient of variation (COV), and the recovery coefficients (RCs) evaluated in the hot spheres. Additionally, we compared the characteristic time-activity-product (TAP) that is the product of scan time per bed position × mass-activity administered (in min·MBq/kg) across datasets. Results Good system cross-calibration was obtained for all tested datasets with < 6% deviation from the expected value was observed. For all clinical protocol settings, image noise was compatible with clinical interpretation (COV < 15%). Digital PET showed an improved background signal-to-noise ratio as compared to conventional PMT-based PET. RCs were comparable between digital and PMT-based PET datasets. Compared to PMT-based PET, digital systems provided comparable image quality with lower TAP (from ~ 40% less and up to 70% less). Conclusions This study compared the achievable clinical image quality in three state-of-the-art digital PET/CT devices (from different vendors) as well as in two conventional PMT-based PET. Reported results show that a comparable image quality is achievable with a TAP reduction of ~ 40% in digital PET. This could lead to a significant reduction of the administered mass-activity and/or scan time with direct benefits in terms of dose exposure and patient comfort.

Abstract:BACKGROUND. Digital PET scanners with increased sensitivity may allow shorter scan acquisition times or reductions in administered radiopharmaceutical activities. OBJECTIVE. The purpose of this study was to evaluate in children and young adults the impact of shorter simulated acquisition times on the quality of whole-body FDG PET images obtained using a digital PET/CT system. METHODS. This retrospective study included 27 children and young adults (nine male and 18 female patients) who underwent clinically indicated whole-body FDG PET/CT examinations performed using a 25-cm axial FOV PET/CT system at 90 s per bed position (expressed hereafter as seconds per bed). Raw list-mode data were reprocessed to simulate acquisition times of 60, 55, 50, 45, 40, and 30 s/bed. Three radiologists independently reviewed reconstructed images and assigned Likert scores for lesion conspicuity, normal structure conspicuity, image quality, and image noise. A separate observer recorded the SUVmax, SUVmean, and SD of the SUV (SUVSD) for liver, thigh, and the most FDG-avid lesion. The SUVSD/SUVmean (the SUVSD divided by the SUVmean) was calculated as a surrogate of image noise. ANOVA, the Friedman test, and the Dunn test were used to compare qualitative measures (combining reader scores) and SUV measurements. RESULTS. The mean patient age was 10.8 ± 8.3 (SD) years, mean BMI was 18.7 ± 2.9, and mean administered FDG activity was 4.44 ± 0.37 MBq/kg (0.12 ± 0.01 mCi/kg). No qualitative measure showed a significant difference versus 90 s/bed for the simulated acquisition at 60 s/bed (all p > .05). Significant differences (all p < .05) versus 90 s/bed were observed for lesion conspicuity at at most 40 s/bed, conspicuity of normal structures and overall image quality at at most 45 s/bed, and image noise at at most 55 s/bed. SUVmean was not significantly different from 90 s/bed for any site for any reduced-count simulation (all p > .05). SUVSD/SUVmean and SUVmax showed gradual increases with decreasing acquisition times and were significantly different from 90 s/bed only for liver at 60 s/bed (for SUVmax: 1.00 ± 0.00 vs 1.05 ± 0.03, p = .02; for SUVSD/SUVmean: 0.09 ± 0.02 vs 0.11 ± 0.02, p = .04). CONCLUSION. Favorable findings for the simulated acquisition at 60 s/bed suggest that, in children and young adults who undergo imaging performed using a 25-cm FOV digital PET scanner, acquisition time or administered FDG activity may be decreased by approximately 33% from the clinical standard without significantly impacting image quality. CLINICAL IMPACT. A 25-cm axial FOV digital scanner may allow FDG PET/CT examinations to be performed with reduced radiation exposure or faster scan acquisition times.

Abstract:Positron emission tomography (PET) represents molecular imaging for non-invasive phenotyping of physiological and biochemical processes in various oncological diseases. PET imaging with 18F-fluorodeoxyglucose (18F-FDG) for glucose metabolism evaluation is the standard imaging modality for the clinical management of lymphoma. One of the 18F-FDG PET applications is the detection and pre-treatment staging of lymphoma, which is highly sensitive. 18F-FDG PET is also applied during treatment to evaluate the individual chemo-sensitivity and accordingly guide the response-adapted therapy. At the end of the therapy regiment, a negative PET scan is indicative of a good prognosis in patients with advanced Hodgkin’s lymphoma and diffuse large B-cell lymphoma. Thus, adjuvant radiotherapy may be alleviated. Future PET studies using non-18F-FDG radiotracers, such as 68Ga-labeled pentixafor (a cyclic pentapeptide that enables sensitive and high-contrast imaging of C–X–C motif chemokine receptor 4), 68Ga-labeled fibroblast activation protein inhibitor (FAPI) that reflects the tumor microenvironment, and 89Zr-labeled atezolizumab that targets the programmed cell death-ligand 1 (PD-L1), may complement 18F-FDG and offer essential tools to decode lymphoma phenotypes further and identify the mechanisms of lymphoma therapy.

Abstract:Aim: High-sensitivity hybrid positron emission tomography (PET) imaging using advanced whole-body (WB) or total-body PET/computed tomography (CT) systems permits reducing injected tracer activity while preserving diagnostic quality. Such approaches are promising for healthy control studies or exploring inter-organ communication in systemic diseases. This study assessed test/retest variations in the fluoro-2-deoxy-D-glucose (FDG) uptake in key organs from low-dose (LD) and standard-dose (STD) [18F]FDG-PET/CT imaging protocols in healthy controls and lung cancer patients. Methods: A total of 19 healthy controls (19–62 years, 46–104 kg, 10 M/9 F) and 7 lung cancer patients (47–77 years, 50–88 kg, 4 M/3 F) underwent [18F]FDG-PET/CT imaging. All subjects were first injected ("test," LD) with 28 ± 2 MBq FDG and underwent a dynamic (0–67 min post-injection) WB imaging protocol with LD-CT. Then, 90 min post-LD injection, the subjects were repositioned and injected with 275 ± 16 MBq FDG ("retest," STD). Second LD-CT and STD-CT scans were acquired for healthy controls and patients, respectively. Static images (55–67 min post-injection) were considered for subsequent analysis. The CT images were used to automatically segment the target volumes of interest. Standardized uptake values normalized to the body weight (SUV BW ) were extracted for each volume of interest. The mean SUV BW were compared for both LD/STD conditions with paired t-tests. In patients, FDG-avid lesions were manually delineated on LD and STD static images. Effective dose levels were estimated from both the CT and PET acquisitions. Results: Organ-based mean SUV BW were similar between the LD and STD (mean %difference ≤5%) in both healthy controls and cancer patients, except in the heart. Intra-control test/retest variability was significant in the brain, heart, and skeletal muscle ( p < 0.05). While 17 lesions were delineated on the STD images of the patients, only 10/17 lesions were identified on the LD images due to increased image noise. Lesion-based mean SUV BW were similar between LD and STD acquisitions ( p = 0.49, %difference = 10%). In patients, the effective doses were (1.9 ± 0.2) mSv (LD-CT), (16.6 ± 5.4) mSv (STD-CT), (0.5 ± 0.1) mSv (LD-PET), and (4.6 ± 0.3) mSv (STD-PET). Conclusion: LD and STD [18F]FDG injections in healthy controls and lung cancer patients yielded comparable mean SUV BW , except in the heart. Dose levels may be reduced for [18F]FDG-PET imaging without a loss in mean SUV BW accuracy, promoting LD-PET/CT protocols for studying multi-organ metabolic patterns. In oncology patients, this approach may be hindered by a lower diagnostic quality in the presence of significant noise.

Inhibition by cyclothiazide of neuronal nicotinic responses in bovine chromaffin cells

Abstract:1 The desensitizing acetylcholine (ACh) response of bovine chromaffin cells maintained in culture was examined using rapid agonist applications (of 2 s duration) which imposed nominal drug concentrations within 50 ms. This study was aimed, firstly, at identifying which of the α3, α4 and α7 subunits known to be present in these cells is predominant in the ACh‐evoked response and secondly, on the effects on these neuronal nicotinic ACh receptors (AChR) of cyclothiazide (CT), an agent acting as a modulator of a gating desensitization site on other ligand‐gated channels. 2 Locally applied 100 μm ACh evoked peak currents (IAch) of −1.5 ± 0.1 nA (n = 83) at a holding potential of −60 mV. The ACh dose‐response curve yielded an estimated EC50 of 60 μm. This current was not sustained but desensitized during the application period; it displayed strong inward rectification, but desensitized equally whether the evoked current was inward or outward going. The latter observation excludes α4 as a major contributor to the recorded current. Because the response was almost insensitive to a 1 μm α‐bungarotoxin pretreatment (IACg = −12 ± 0.1 nA; n = 6), and because 1, 1‐dimethyl‐4‐phenylpiperazinium (DMPP) works as a potent agonist (peak current = −1.9 nA, n = 2 for 100 μm DMPP), the α7 subunit is also a minor contributor to the response. Taken together, these observations suggest a dominant α3 type of response. 3 Triple exponential fits were used to describe the characteristics of the ACh‐evoked currents; one component to fit the rising phase, with 2 components to describe the decay phase. The decay times were 100 ms and 4 s for the fast and slow components respectively. The rate of the slow decay component increased systematically with recording time, approximately doubling from its initial value within 20–40 min. Furthermore there was a gradual rundown of the response, seen first as a loss of the late component of the current, measured at 2 s, with the peak current amplitude decreasing later in the recording. 4 CT, when coapplied with ACh, produced a dose‐dependent inhibition of the ACh‐evoked peak current. The effect showed little voltage‐dependency with 100 μm CT producing 46 ± 5% (s.d.; n = 3) and 47 ± 8% (s.d.; n = 7) inhibition at −100 and −60 mV respectively. At +60 mV, inhibition was estimated to be 26 ± 7% (s.d.; n = 3). 5 After pre‐exposure of the cells to CT by bath application, 10 and 30 μm CT produced poorly reversible 20 ± 9% (n = 7) and 42 ± 5% (n = 4) inhibitions of the peak current respectively. There were no discernible effects on the fitted decay constants at any CT concentration tested, although an increased inhibitory effect of CT was observed at higher concentrations (100 μm) on the amplitude of the late component measured at 2 s. 6 Similar effects were observed in conditions chosen to isolate the α3 type of receptor: namely when using DMPP as an agonist, or after α‐bungarotoxin pretreatment. 7 The 2,3‐benzodiazepine, GYKI 53655, is known to antagonize the action of CT on AMPA receptors. Coapplication of 50 μm GYKI 53655 with ACh (100 μm) produced a 29 ± 4% inhibition of the peak ACh‐evoked current and 44 ± 6% inhibition of its amplitude at 2 s (n = 4). This response was fully reversible. Brief applications of both CT (100 μm) and GYKI 53655 (50 μm) with ACh via the microperfusion system produced a fully reversible inhibition that was not significantly different from the values obtained with either CT or GYKI 53655 alone, with 37 ± 6% inhibition of peak and 61 ± 9% inhibition of the amplitude at 2 s (n = 3). 8 The results obtained suggest that the CT effect is to impede recovery from a slow desensitization, with a more pronounced effect with longer CT applications. Globally, CT favours the ‘rundown state’ of the neuronal nicotinic AChR.
Optimal activity of [18F]FDG for Hodgkin lymphoma imaging performed on PET/CT camera with BGO crystals

Abstract:Background: We aimed to find the minimum feasible activity of fluorodeoxyglucose ([18F]FDG) in positron emission tomography/computed tomography (PET/CT) of Hodgkin lymphoma patients performed on a camera with bismuth germanate (BGO) crystals. Material and methods: Ninety-one [18F]FDG PET/CT scans (each in seven Bayesian Penalized Likelihood [BPL] reconstructions with varying acquisition time per bed position - 2 min, 1.5 min, 1 min, 50 s, 40 s, 30 s, and 20 s) were independently assessed by three physicians to evaluate image quality. Mean administered activity was 3.0 ± 0.1 MBq/kg and mean uptake time was 54.0 ± 8.7 min. The series quality was subjectively marked on a 1-10 scale and then ranked 1-7 based on the mean mark. Interobserver rank correlation and intraclass correlation within each series for the three observers were calculated. Phantom studies were also performed to determine if reduced acquisition time can be directly translated into a reduced activity. Results: Time series were marked and ranked unanimously - the longer the time of acquisition the higher the mark and rank. The interobserver agreement in the ranking was excellent (100%) with a kappa coefficient of 1.00 (95% CI [0.83-1.0]). The general intraclass correlation coefficient (agreement between the marks observers gave each time series) was very high (0.945, 95% CI [0.936-0.952]) and was higher the shorter the time per bed. According to all three observers only the series with 2 min and 1.5 min acquisition time were appropriate for assessment (mean mark ≥ 7). In phantom studies there was a linear correlation between time per bed, administered activity, and number of total prompts detected by a scanner. Hence, a reduction of acquisition time of 25% (from 2 min to 1.5 min) could be directly translated into a 25% activity reduction (from 3.0 to 2.25 MBq/kg). Conclusions: In patients with HL, [18F]FDG activity can be reduced by up to 25% when using a BGO crystal camera, without substantial impact on image quality.
Benefit of Improved Performance with State-of-the Art Digital PET/CT for Lesion Detection in Oncology

Abstract:The latest digital whole-body PET scanners provide a combination of higher sensitivity and improved spatial and timing resolution. We performed a lesion detectability study on two generations of Biograph PET/CT scanners, the mCT Flow and the Vision, to study the impact of improved physical performance on clinical performance. Our hypothesis was that the improved performance of the Vision would result in improved lesion detectability, allowing shorter imaging times or, equivalently, a lower injected dose. Methods: Data were acquired with the Society of Nuclear Medicine and Molecular Imaging Clinical Trials Network torso phantom combined with a 20-cm-diameter cylindrical phantom. Spherical lesions were emulated by acquiring sphere-in-air data and combining them with the phantom data to generate combined datasets with embedded lesions of known contrast. Two sphere sizes and uptakes were used: 9.89-mm-diameter spheres with 6:1 (lung) and 3:1 (cylinder) local activity concentration uptakes and 4.95-mm-diameter spheres with 9.6:1 (lung) and 4.5:1 (cylinder) local activity concentration uptakes. Standard image reconstruction was performed: an ordinary Poisson ordered-subsets expectation maximization algorithm with point-spread function and time-of-flight modeling and postreconstruction smoothing with a 5-mm gaussian filter. The Vision images were also generated without any postreconstruction smoothing. Generalized scan statistics methodology was used to estimate the area under the localized receiver-operating-characteristic curve (ALROC). Results: The higher sensitivity and improved time-of-flight performance of the Vision leads to reduced contrast in the background noise nodule distribution. Measured lesion contrast is also higher on the Vision because of its improved spatial resolution. Hence, the ALROC is noticeably higher for the Vision than for the mCT Flow. Conclusion: Improved overall performance of the Vision provides a factor of 4-6 reduction in imaging time (or injected dose) over the mCT Flow when using the ALROC metric for lesions at least 9.89 mm in diameter. Smaller lesions are barely detected in the mCT Flow, leading to even higher ALROC gains with the Vision. The improved spatial resolution of the Vision also leads to a higher measured contrast that is closer to the real uptake, implying improved quantification. Postreconstruction smoothing, however, reduces this improvement in measured contrast, thereby reducing the ALROC for small, high-uptake lesions.

Abstract:Positron emission tomography/computed tomography (PET/CT) based on fibroblast activation protein inhibitors (FAPI) has shown complementary values to 2-[18F]-fluoro-2-deoxy-D-glucose ([18F]FDG) in cancer imaging. This study aimed to investigate the feasibility of a one-stop FDG-FAPI dual-tracer imaging protocol with dual-low activity for oncological imaging. Nineteen patients with malignancies underwent one-stop [18F]FDG (0.37 MBq/kg) PET (PETFDG) and dual-tracer PET 30–40 and 50–60 min (hereafter, PETD30–40 and PETD50–60, respectively) after additional injection of [68Ga]Ga-DOTA-FAPI-04 (0.925 MBq/kg), with a single diagnostic CT to generate the PET/CT. The lesion detection rate and tumor-to-normal ratios (TNRs) of tracer uptake were compared between PETFDG/CT and PETD50–60/CT and between PETD50–60/CT and PETD30–40/CT. In addition, a visual scoring system was established to compare the lesion detectability. The dual-tracer PETD50–60 and PETD30–40/CT showed similar performance in detecting primary tumors but presented significantly higher lesion TNRs than PETFDG. Significantly, more metastases with higher TNRs were identified on PETD50–60 than PETFDG (491 vs. 261, P < 0.001). The dual-tracer PETD50–60 received significantly higher visual scores than single PETFDG (111 vs. 10) in demonstrating both primary tumors (12 vs. 2) and metastases (99 vs. 8). However, these differences were not significant between PETD50–60 and PETD30–40. These resulted in tumor upstaging in 44.4% patients taking PET/CT for initial assessment, and more recurrences (68 vs. 7) were identified in patients taking PET/CT for restaging, both on PETD50–60 and PETD30–40, compared to PETFDG. The reduced effective dosimetry per patient (26.2 ± 2.57 mSv) was equal to that of a single standard whole-body PET/CT. The one-stop dual-tracer dual-low-activity PET imaging protocol combines the strengths of [18F]FDG and [68Ga]Ga-DOTA-FAPI-04 with shorter duration and lesser radiation and is thus clinically applicable.
Improved Discrimination Between Benign and Malignant LDCT Screening-Detected Lung Nodules with Dynamic over Static 18F-FDG PET As a Function of Injected Dose

Abstract:Lung cancer mortality rate can be significantly reduced by up to 20% through routine low-dose computed tomography (LDCT) screening, which, however, has high sensitivity but low specificity, resulting in a high rate of false-positive nodules. Combining PET with CT may provide more accurate diagnosis for indeterminate screening-detected nodules. In this work, we investigated low-dose dynamic F-18-FDG PET in discrimination between benign and malignant nodules using a virtual clinical trial based on patient study with ground truth. Six patients with initial LDCT screening-detected lung nodules received 90 min single-bed PET scans following a 10 mCi FDG injection. Low-dose static and dynamic images were generated from under-sampled list-mode data at various count levels (100%, 50%, 10%, 5%, and 1%). A virtual clinical trial was performed by adding nodule population variability, measurement noise, and static PET acquisition start time variability to the time activity curves (TACs) of the patient data. We used receiver operating characteristic (ROC) analysis to estimate the classification capability of standardized uptake value (SUV) and net uptake constant K-i from their simulated benign and malignant distributions. Various scan durations and start times (t*) were investigated in dynamic Patlak analysis to optimize simplified acquisition protocols with a population-based input function (PBIF). The area under curve (AUC) of ROC analysis was higher with increased scan duration and earlier t*. Highly similar results were obtained using PBIF to those using image-derived input function (IDIF). The AUC value for K-i using optimized t* and scan duration with 10% dose was higher than that for SUV with 100% dose. Our results suggest that dynamic PET with as little as 1 mCi FDG could provide discrimination between benign and malignant lung nodules with higher than 90% sensitivity and specificity for patients similar to the pilot and simulated population in this study, with LDCT screening-detected indeterminate lung nodules.
Head-to-head Intra-Individual Comparison of Total-Body 2-[18F]FDG PET/CT and Digital PET/CT in Patients with Malignant Tumor: How Sensitive Could It Be?

Abstract:To comparatively evaluate the lesion-detecting ability of 2-[18F]FDG total-body PET/CT (TB PET/CT) and conventional digital PET/CT. This study enrolled 67 patients (median age, 65 years; 24 female and 43 male patients) who underwent a TB PET/CT scan and a conventional digital PET/CT scan after a single 2-[18F]FDG injection (3.7 MBq/kg). Raw PET data for TB PET/CT were acquired over the course of 5 min, and images were reconstructed using data from the first 1, 2, 3, and 4 min and the entire 5 min (G1, G2, G3, G4, and G5, respectively). The conventional digital PET/CT scan acquired in 2–3 min per bed (G0). Two nuclear medicine physicians independently assessed subjective image quality using a 5-point Likert scale and recorded the number of 2-[18F]FDG-avid lesions. A total of 241 lesions (69 primary lesions; 32 liver, lung, and peritoneum metastases; and 140 regional lymph nodes) among 67 patients with various types of cancer were analyzed. The subjective image quality score and SNR (signal-to-noise ratio) increased gradually from G1 to G5, and these values were significantly higher than the values at G0 (all p < 0.05). Compared to conventional PET/CT, G4 and G5 of TB PET/CT detected an additional 15 lesions (2 primary lesions; 5 liver, lung, and peritoneum lesions; and 8 lymph node metastases). TB PET/CT was more sensitive than conventional whole-body PET/CT in detecting small (4.3 mm, maximum standardized uptake value (SUVmax) of 1.0) or low-uptake (tumor-to-liver ratio of 1.6, SUVmax of 4.1) lesions. This study explored the gain of the image quality and lesion detectability of TB PET/CT, compared to conventional PET/CT, and recommended the appropriate acquisition time for TB PET/CT in clinical practice with an ordinary 2-[18F] FDG dose. • TB PET/CT increases the effective sensitivity to approximately 40 times that of conventional PET scanners. • The subjective image quality score and signal-to-noise ratio of TB PET/CT from G1 to G5 were better than those of conventional PET/CT. • 2-[ 18 F]FDG TB PET/CT with a 4-min acquisition time at a regular tracer dose detected an additional 15 lesions compared to conventional PET/CT.
Ultralow-dose [18F]FDG PET/CT Imaging: Demonstration of Feasibility in Dynamic and Static Images

Abstract:Validation of [18F]FDG PET/CT at ultralow-dose (0.37 MBq/kg) and compared to imaging at half-dose (1.85 MBq/kg). This prospective head-to-head intraindividual study compared dynamic and static parameters of ultralow-dose with half-dose [18F]FDG total-body PET/CT. In static imaging, the ultralow-dose groups of PET images were denoted ULD5, 60–65 min; ULD8, 60–68 min; ULD10, 60–70 min; and ULD15, 60–75 min. The half-dose group images were reconstructed to 60–61, 60–62, 60–63, and 60–75 min, defined as LD1, LD2, LD3, and LD15, respectively. A 5-point Likert scale was used to subjectively evaluate the quality of static PET images, with a score greater than 3 considered to meet the requirements for clinical diagnosis. Thirty participants were included in this study, and in terms of kinetic indicators, no special differences were found between the two groups of normal organs and lesions. In static images, those in groups ULD8 and LD2 achieved scores of ≥ 3.0, meeting the requirements for clinical diagnosis. In static imaging, four lesions were missed in the LD1 group with a lesion detectability of 89.7 • A prospective single-center study showed that the total-body PET scanner allows ultralow-dose [ 18 F]FDG imaging with acceptable image quality and lesion detectability. • For the participant, radiation exposure can be reduced with ultralow-dose [ 18 F]FDG total-body PET/CT imaging.
Voxel-based Dynamic FDG PET with Simplified Data Acquisition Protocols and Shortened Scan Duration

Abstract:644 Objectives: Dynamic FDG PET provides more accurate quantitative information than static PET. The net uptake constant Ki estimated from dynamic PET has shown better performance in oncological diagnosis than standardized uptake value (SUV). However, conventional dynamic imaging requires longer scan duration. A simplified data acquisition protocol with shortened scan duration could make dynamic FDG PET more clinically flexible. Our previous work has shown good results in lung cancer study using ROI-based dynamic FDG PET with simplified protocols (presented in the 2017 IEEE NSS/MIC conference). The objective of this study is to investigate the voxel-based dynamic FDG PET with simplified data acquisition protocols and shortened scan duration. Methods: The voxel-based dynamic FDG PET analysis was performed through indirect and direct ways. A population-based input function (PBIF) template was used in Patlak analysis. Patient-specific PBIF was generated by scaling the PBIF template with an image-derived input function (IDIF) estimated from the blood pool of left ventricle in dynamic images. Dynamic images were reconstructed with an OSEM algorithm, incorporating attenuation, crystal sensitivity normalization, random, decay and scatter corrections. Voxel-based Patlak analysis with scaled PBIF was used to indirectly generate Ki images. As for direct dynamic PET analysis, parametric images were reconstructed directly from the acquired dynamic data. We used existing dynamic images with known Ki to simulate sinograms and performed direct parametric reconstruction. A nested algorithm was applied to accelerate the convergence. 10 inner loops of parametric image update were nested into each outer loop of dynamic image update.The developed dynamic image methods were validated on simulated and clinical data with simplified protocols. We used single-bed FDG PET lung cancer patient data to generate preliminary results. Accurate simulation and more patient study has been planned. Four acquisition protocols with different scan duration (t) and start time (t[asterisk]) were compared in the indirect analysis: (a) t=70 min, t[asterisk]=20 min; (b) t=30 min, t[asterisk]=20 min; (c) t=30 min, t[asterisk]=30 min; and (d) t=40 min, t[asterisk]=30 min. Results: The 8-mm lung nodule could be obviously observed in the parametric images generated from indirect voxel-based Patlak analysis. The images became noisier with shorter scan duration. Direct Ki images were smoother than indirect Ki images. The mean Ki values of the nodule in direct image showed higher bias than indirect images, compared to the ground truth generated from ROI-based irreversible 2-tissue modeling. The larger bias may be caused by the inaccuracy in preliminary simulation and slow convergence in direct reconstruction. Further phantom simulation is ongoing to validate the accuracy of direct reconstruction.Conclusion: The voxel-based dynamic analysis with simplified protocols were validated to be feasible. Preliminary parametric images were reconstructed in both indirect and direct approaches. The optimized design of acquisition protocols requires further investigation.
Feasibility of Acquisitions Using Total-Body PET/CT with an Ultra-Low 18F-FDG Activity.

Abstract:The present study aimed to evaluate the feasibility of ultra-low 18F-FDG activity in total-body PET/CT oncologic studies. Methods: Thirty patients with cancer were enrolled prospectively and underwent a total-body PET/CT scan 60 min after injection of an ultra-low 18F-FDG activity (0.37 MBq/kg). Of the 30 enrolled patients, 11 were diagnosed with colorectal cancer (CRC). PET raw data were acquired within 15 min and reconstructed using data from the first 1, 2, 4, 8, and 10 min and the entire 15 min (G1, G2, G4, G8, G10, and G15, respectively). Image quality was qualitatively assessed twice by 2 readers using a 5-point Likert scale. The Cohen κ-test was used to investigate the intra- and interreader agreement. The SUVmax of lesions; the SUVmax, SUVmean, and SD of the livers; the tumor-to-background ratio; and the signal-to-noise ratio were measured and compared. The acquisition time for a clinically acceptable image quality using an ultra-low-activity injection was determined. In a matched-pair study, 11 patients with CRC who received a full 18F-FDG activity (3.7 MBq/kg) with an acquisition time of 2 min were selected retrospectively by matching sex, height, weight, body mass index, glucose level, uptake time, and pathologic types with the 11 CRC subjects in the prospective study. Qualitative and quantitative analyses were performed and compared between the 11 patients with CRC in the ultra-low-activity group and their matched full-activity controls. Results: Qualitative analysis of image quality showed good intra- and interreader agreements (all κ > 0.7). All the images acquired for 8 min or longer scored over 3 (indicating clinical acceptability). There was no significant difference in tumor-to-background ratio and liver signal-to-noise ratio among all the images acquired for 8 min or longer. In the matched study, no significant difference was found in the image quality score and quantitative parameters between the ultra-low-activity group with an 8-min acquisition and the full-activity group with a 2-min acquisition. Conclusion: An ultra-low 18F-FDG activity with an 8-min acquisition in a total-body PET/CT study can achieve acceptable image quality equivalent to that in the full-activity group after a 2-min acquisition.
Optimizing Acquisition Times for Total-Body Positron Emission Tomography/computed Tomography with Half-Dose 18F-Fluorodeoxyglucose in Oncology Patients

Abstract:The present study aimed to explore the boundary of acquisition time and propose an optimized acquisition time range for total-body positron emission tomography (PET)/computed tomography (CT) oncological imaging using half-dose (1.85 MBq/kg) 18F-fluorodeoxyglucose activity based on diagnostic needs. In this retrospective study based on a total-body PET system (uEXPLORER), an exploration cohort (October 2019–December 2019) of 46 oncology patients was first studied. The acquisition time for all patients was 15 min, and the acquired images were reconstructed and further split into 15-, 8-, 5-, 3-, 2-, and 1-min duration groups (abbreviated as G15, G8, G5, G3, G2, and G1). The image quality and lesion detectability of reconstructed PET images with different acquisition times were evaluated subjectively (5-point scale, lesion detection rate) and objectively (standardized uptake values, tumor-to-background ratio). In the same way, the initial optimized acquisition times were further validated in a cohort of 147 oncology patients (December 2019–June 2021) by using the Gs images (the images obtained using the 15- and 10-min acquisition times) as controls. In the exploration cohort, the subjective scores for G1, G2, G3, G5, and G8 images were 2.0 ± 0.2, 2.9 ± 0.3, 3.0 ± 0.0, 3.9 ± 0.2, and 4.2 ± 0.4, respectively. Two cases in G1 were rated as 1 point. No significant difference in scores was observed between G5 and G8 (p > 0.99). In general, groups with a longer acquisition time showed lower background uptake and lesion conspicuity. Compared with G15, lesion detection rate significantly reduced to 85.3% in G1 (p < 0.05). In the validation cohort, the subjective score was 3.0 ± 0.2 for G2, 3.0 ± 0.1 for G3, 3.6 ± 0.5 for G5, 4.0 ± 0.3 for G8, and 4.4 ± 0.5 for Gs. Only the scores between G2 and G3 were not significantly different (p > 0.99). The detection rates (204 lesions) significantly reduced to 94.1–90.2% in G3 and G2 (all p < 0.05). A 2-min acquisition time provided acceptable performance in certain groups and specific medical situations. And protocols with acquisition times ≥ 5 min could provide comparable lesion detectability as regular protocols, showing better compatibility and feasibility with clinical practice.
Role of 2-(fluorine-18) Fluoro-2-deoxy-D-glucose Positron Emission Tomography/Computed Tomography in Initial Staging and Bone Marrow Involvement Prediction in Patients with Newly Diagnosed Lymphoma in Correlation with Bone Marrow Study

Abstract:Background: Lymphoma is a common malignant proliferative disease in which bone marrow infiltration will upstage the disease and thus affect prognosis of the disease. As of now bone marrow biopsy is considered as a reference standard to find out bone marrow involvement in lymphoma. Performing an invasive and painful intervention in all newly diagnosed lymphoma patients is controversial. PET-CT is a non-invasive technique that gives functional information about the cells using the glucose metabolism. It can detect early bone marrow and extra medullary organ involvement which can lead to restaging of the disease. These advantages make PET-CT a valuable adjunct in diagnosis of lymphoma. Aims and objectives: Our study aims to evaluate the usefulness of 18 F-FDG PET-CT, a non-invasive, semi quantitative whole body imaging technique for detection of early bone marrow and extra medullary organ involvement in lymphoma patients which in turn can obviate the need for bone marrow study (BMS). The primary objective of study is to categorise FDG uptake in bone marrow as diffuse /unifocal /multifocal / no uptake and to correlate pattern of FDG uptake to bone marrow study. Our study also assesses the role of FDG PET/CT in staging of lymphoma. Materials and methods: Thirty patients with newly diagnosed lymphoma in the age group 18 to 75 years of both sexes within 3 months of diagnosis and who have not been started on any treatment was included in the study. Marrow uptake on FDG PET/CT has been categorized as diffuse, unifocal, multifocal and no uptake. Agreement between bone marrow study and FDG PET/CT has been assessed by reliability analysis using Cohen's kappa. Sensitivity, specificity, PPV, NPV of PET/CT in detecting marrow involvement have been calculated. Results: The sensitivity, specificity, PPV, NPV and accuracy of 18 F-FDG PET-CT in detecting marrow involvement of lymphoma cases are 86.6%, 77.7%, 68.4%, 91.3% and 80.9% respectively. 18 F-FDG PET-CT detected bone marrow involvement in 86.6% (13 out of 15 total positive cases) cases of lymphoma which included both HL and NHL. Reliability analysis using Cohen's kappa is used to test the agreement between bone marrow study and 18F-FDG PET/CT. k value of 0.6 was obtained which showed a moderate agreement between bone marrow study and 18F-FDG PET/CT in marrow assessment. Conclusion: 18F-FDG PET/CT is a highly sensitive imaging modality which can pick up extra-nodal organ and BMI in patients with lymphoma and can upstage the disease and alter treatment strategies. PET-CT cannot completely replace the bone marrow study. However, being an invasive painful procedure, BMB can be avoided in cases with unifocal or multifocal marrow involvement on PET-CT.
Exploration the feasibility and additional value of [18F]FDG/[68Ga]Ga-FAPI-04 dual-low-activity-tracer one-stop total-body PET imaging at 34 min post-injection of [68Ga]Ga-FAPI-04

Abstract:Objectives: To validate the feasibility of one-stop 2-[18F]fluoro-2-deoxy-D-glucose ([18F]FDG) and [68Ga]Ga-fibroblast activation protein inhibitor-04 ([68Ga]Ga-FAPI-04) dual-low-activity-tracer positron emission tomography/computed tomography (PET/CT) at 34 min post-injection of [68Ga]Ga-FAPI-04 and explore its additional value. Methods: Thirty pairs of patients with suspected malignancies who underwent dual-tracer imaging were enrolled in this retrospective study. The images were reconstructed at 34-39 and 50-60 min after additional injection of [68Ga]Ga-FAPI-04 (in one-stop FDG-FAPI PET/CT, named PETFDG, PETD34-39, and PETD50-60; in the 2-day protocol, named PETFDG', PETF34-39, and PETF50-60, respectively). Tumour-to-normal ratios (TNR) of lesions in PETFDG, PETD34-39, and PETD50-60 and TNR of lesions in PETF34-39 and PETF50-60 were evaluated separately. To evaluate the potential added value of one-stop FDG-FAPI PET/CT over the 2-day protocol, TNRs of PETFDG, PETD34-39, and PETD50-60 were compared with PETF34-39. The lesion detectability of the two imaging protocols was evaluated by chi-square test. Results: Comparing FAPI-weighted PET (PETD34-39 and PETD50-60) and single-tracer imaging (PETFDG) in one-stop FDG-FAPI PET/CT, TNRs of FAPI-weighted PET were higher than those of PETFDG. PETD34-39 and PETD50-60 showed similar performance in lesion detectability and TNRs (all P > 0.05). In the 2-day protocol, there are no statistically significant differences in TNRs of all lesions at PETF34-39 and PETF50-60. Comparing one-stop FDG-FAPI PET/CT with the 2-day protocol, TNRs of PETF34-39 were significantly higher than those of PETFDG but lower than those of PETD34-39 and PETD50-60. Lesion detectability in the one-stop FDG-FAPI PET/CT was higher than that in the 2-day protocol. The average radiation dose in one-stop FDG-FAPI PET/CT was significantly lower than that in the 2-day protocol (P<0.001). Conclusion: One-stop FDG-FAPI PET/CT at 34 min could provide sufficient information to meet clinical diagnosis and showed better lesion detectability than that in the 2-day protocol.
Comparative effectiveness of standard vs. AI-assisted PET/CT reading workflow for pre-treatment lymphoma staging: a multi-institutional reader study evaluation

Abstract:Background: Fluorine-18 fluorodeoxyglucose (FDG)-positron emission tomography/computed tomography (PET/CT) is widely used for staging high-grade lymphoma, with the time to evaluate such studies varying depending on the complexity of the case. Integrating artificial intelligence (AI) within the reporting workflow has the potential to improve quality and efficiency. The aims of the present study were to evaluate the influence of an integrated research prototype segmentation tool implemented within diagnostic PET/CT reading software on the speed and quality of reporting with variable levels of experience, and to assess the effect of the AI-assisted workflow on reader confidence and whether this tool influenced reporting behaviour. Methods: Nine blinded reporters (three trainees, three junior consultants and three senior consultants) from three UK centres participated in a two-part reader study. A total of 15 lymphoma staging PET/CT scans were evaluated twice: first, using a standard PET/CT reporting workflow; then, after a 6-week gap, with AI assistance incorporating pre-segmentation of disease sites within the reading software. An even split of PET/CT segmentations with gold standard (GS), false-positive (FP) over-contour or false-negative (FN) under-contour were provided. The read duration was calculated using file logs, while the report quality was independently assessed by two radiologists with >15 years of experience. Confidence in AI assistance and identification of disease was assessed via online questionnaires for each case. Results: There was a significant decrease in time between non-AI and AI-assisted reads (median 15.0 vs. 13.3 min, p < 0.001). Sub-analysis confirmed this was true for both junior (14.5 vs. 12.7 min, p = 0.03) and senior consultants (15.1 vs. 12.2 min, p = 0.03) but not for trainees (18.1 vs. 18.0 min, p = 0.2). There was no significant difference between report quality between reads. AI assistance provided a significant increase in confidence of disease identification (p < 0.001). This held true when splitting the data into FN, GS and FP. In 19/88 cases, participants did not identify either FP (31.8%) or FN (11.4%) segmentations. This was significantly greater for trainees (13/30, 43.3%) than for junior (3/28, 10.7%, p = 0.05) and senior consultants (3/30, 10.0%, p = 0.05). Conclusions: The study findings indicate that an AI-assisted workflow achieves comparable performance to humans, demonstrating a marginal enhancement in reporting speed. Less experienced readers were more influenced by segmentation errors. An AI-assisted PET/CT reading workflow has the potential to increase reporting efficiency without adversely affecting quality, which could reduce costs and report turnaround times. These preliminary findings need to be confirmed in larger studies.
Preliminary application of 18F-FDG PET/CT in cutaneous lymphoma

Abstract:Objective Cutaneous lymphoma is the common group of extranodal non-Hodgkin lymphomas.18F-fluorodeoxyglucose(FDG) positron emission tomography(PET/CT) have been widely used in lymphoma.This research was to investigate the usefulness of FDG PET/CT in the management of cutaneous lymphoma.Methods The 12 patients with cutaneous lymphoma [nine T/NK cell non-Hodgkin lymphoma(NHL) and three B-cell NHL] were examined by PET/CT retrospectively.There were 9 men and 3 women(age range 16-79 years) who were performed PET/CT for initial staging and restaging following therapy.The patients were divided into 2 groups.Group A with 6 cases who had performed therapy before PET/CT scans,and Group B with 6 cases without therapy.All cases were followed up at least 6 months.Results Among group A,the 6 cases had performed 8 PET/CT scans,2 of 6 cases with negative PET/CT scan had not relapsed with follow-up more than 6 months.Four cases had positive PET/CT scans post-therapy.In group B,the 6 cases had performed with positive PET/CT results,however,2 of 6 cases with cutaneous lesions displayed negative FDG.Combined group A and B with positive PET/CT scans,there were cutaneous lesions in eight patients by visual inspection,however,2 patients with negative FDG PET/CT and another 2 patients with positive FDG PET/CT of partial cutaneous lesions;FDG PET had a sensitivity of 75% for the evaluation of cutaneous lesions by patients.For staging of cutaneous lymphoma,two patients were upstaged by FDG PET/CT compared with conventional methods.Conclusion FDG PET/CT has a potential value for initial staging and evaluating therapy reaction in patients with cutaneous lymphoma.
