Abstract:Background: Accurate segmentation of individual tooth and their substructures including enamel, pulp, and dentin from cone-beam computed tomography (CBCT) images is essential for dental diagnosis and treatment planning in digital dentistry. Existing methods for tooth segmentation based on CBCT images have achieved substantial progress; however, techniques for further segmentation into substructures are yet to be developed. Purpose: We aim to propose a novel three-stage progressive deep-learning-based framework for automatically segmenting 3D tooth from CBCT images, focusing on finer substructures, i.e., enamel, pulp, and dentin. Methods: In this paper, we first detect each tooth using its centroid by a clustering scheme , which efficiently determines each tooth detection by applying learned displacement vectors from the foreground tooth region. Next, guided by the detected centroid, each tooth proposal, combined with the corresponding tooth map, is processed through our tooth segmentation network. We also present an attention-based hybrid feature fusion mechanism, which provides intricate details of the tooth boundary while maintaining the global tooth shape, thereby enhancing the segmentation process. Additionally, we utilize the skeleton of the tooth as a guide for subsequent substructure segmentation. Results: Our algorithm is extensively evaluated on a collected dataset of 314 patients, and the extensive comparison and ablation studies demonstrate superior segmentation results of our approach. Conclusions: Our proposed method can automatically segment tooth and finer substructures from CBCT images, underlining its potential applicability for clinical diagnosis and surgical treatment.

Abstract:Accurate and automatic segmentation of individual tooth and root canal from cone-beam computed tomography (CBCT) images is an essential but challenging step for dental surgical planning. In this paper, we propose a novel framework, which consists of two neural networks, DentalNet and PulpNet, for efficient, precise, and fully automatic tooth instance segmentation and root canal segmentation from CBCT images. We first use the proposed DentalNet to achieve tooth instance segmentation and identification. Then, the region of interest (ROI) of the affected tooth is extracted and fed into the PulpNet to obtain precise segmentation of the pulp chamber and the root canal space. These two networks are trained by multi-task feature learning and evaluated on two clinical datasets respectively and achieve superior performances to several comparing methods. In addition, we incorporate our method into an efficient clinical workflow to improve the surgical planning process. In two clinical case studies, our workflow took only 2 min instead of 6 h to obtain the 3D model of tooth and root canal effectively for the surgical planning, resulting in satisfying outcomes in difficult root canal treatments.

Abstract:ABSTRACT Many scenarios in dental clinical diagnosis and treatment require the segmentation and identification of a specific tooth or the entire dentition in cone‐beam computed tomography (CBCT) images. However, traditional segmentation methods struggle to ensure accuracy. In recent years, there has been significant progress in segmentation algorithms based on deep learning, garnering considerable attention. Inspired by models from present neuro networks such as UCTransNet and DC‐Unet, this study proposes an MRCM‐UCTransNet for accurate three‐dimensional tooth segmentation from cone‐beam CT images. To enhance feature extraction while preserving the multi‐head attention mechanism, a multi‐scale residual convolution module (MRCM) is integrated into the UCTransNet architecture. This modification addresses the limitations of traditional segmentation methods and aims to improve accuracy in tooth segmentation from CBCT images. Comparative experiments indicate that, in the situation with a specific image size and small data volume, the proposed method exhibits certain advantages in segmentation accuracy and precision. Compared to traditional Unet approaches, MRCM‐UCTransNet's dice accuracy is improved by 7%, while its sensitivity is improved by about 10%. These findings highlight the efficacy of the proposed approach, particularly in scenarios with specific image size constraints and limited data availability. The proposed MRCM‐UCTransNet algorithm integrates the latest architectural advancements in the Unet model which achieves effective segmentation of six types of teeth within the tooth. It was proved to be efficient for image segmentation on small datasets, requiring less training time and fewer parameters.

Abstract:This paper presents a two-step method to automatically and accurately segment the dental crown components from CT images. Firstly, a multi-scale attention based U-Net model is proposed for pulp segmentation, which is embedded with global and local attention modules. The constructed attention modules can automatically aggregate pixel-wise contextual information and focus on catching the real dental pulp region. Secondly, two efficient level set models are proposed: one is the shape constraint-based level set model for enamel and dentin segmentation, the other is the region mutual exclusion-based level set model for neighboring teeth segmentation. The proposed shape constraint term can better handle topology changes of teeth and the region mutual exclusion term can more effectively avoid intersecting segmentation. Besides, a starting slice initialization method is introduced to achieve automatic segmentation, and an accurate contour propagation strategy is developed for slice-by-slice segmentation. We set up a series of comparative experiments for evaluation. Experimental results verify that the proposed method obtains promising performance for each crown component segmentation, and outperforms state-of-the-art tooth segmentation methods in terms of accuracy. This suggests that the proposed method can be used to accurately segment the crown components for precise tooth preparation treatment. Note to Practitioners—The motivation of this work is to reduce the burden on dentists during tooth preparation treatment, which requires accurate segmentation of crown components (i.e., enamel, dentin, and pulp) from dental CT images. Existing methods only focused on the segmentation of teeth or alveolar bone. Therefore, we present a novel automatic segmentation model for the dental crown components with high accuracy. A key strength of this study is the combination of a data-driven method (deep learning) and model-driven methods (level-set), which can provide good accuracy under limited training samples. This ability is highly desirable for practitioners by saving labor-intensive, costly labeling efforts. Furthermore, our proposed method will provide tools to help reduce subjectivity and human errors, as well as streamline and expedite the clinical workflow. This will significantly facilitate tooth preparation automation.

CBCT image based segmentation method for tooth pulp cavity region extraction

Abstract:Objectives:: A method was proposed to segment the tooth pulp cavity region in cone beam CT) images, which aimed to make the extraction process more efficient and generate more reliable results for further research. Methods:: Cone beam CT images of 50 teeth from 10 patients were randomly collected with the help of Peking University Hospital of Stomatology. All slice images have a ground truth tooth pulp cavity region delineated by two doctors manually. After necessary gamma transform in pre-processing stage, three kinds of information in an image such as greyscale, neighbour average greyscale and gradient were fused to search an optimal segmentation threshold by using plane intercept histogram of reciprocal cross entropy algorithm. With the optimal threshold, binarization was conducted and the tooth pulp cavity regions in slice images can be extracted. Qualitative and quantitative analyses compared to ground truth are involved with the evaluation criterion of average non-coincidence rate ( RANOA ). Independent repeated experiments were carried out to test the stability of this segmentation method. Results:: Accurate and complete segmentation results are obtained. The proposed method reaches the lowest RANOA values in most cases and owns more competitive robustness under various interferences compared with the other popular segmentation methods like reciprocal cross entropy method, active contour-based method, region growing method and level set method. Quantitative analysis verified the effectiveness of this method. Conclusions:: The proposed method can extract tooth pulp cavity regions from teeth efficiently. The segmentation results of this method are more accurate compared to other popular methods under different circumstances and can be used for subsequent applications.

Abstract:This study attempts to segment teeth and root-canals simultaneously from CBCT images, but there are very challenging problems in this process. First, the clinical CBCT image data is very large (e.g., 672 *688 * 688), and the use of downsampling operation will lose useful information about teeth and root canals. Second, teeth and root canals are very different in morphology, and it is difficult for a simple network to identify them precisely. In addition, there are weak edges at the tooth, between tooth and root canal, which makes it very difficult to segment such weak edges. To this end, we propose a coarse-to-fine segmentation method based on inverse feature fusion transformer and uncertainty estimation to address above challenging problems. First, we use the downscaled volume data (e.g., 128 * 128 * 128) to conduct coarse segmentation and map it to the original volume to obtain the area of teeth and root canals. Then, we design a transformer with reverse feature fusion, which can bring better segmentation effect of different morphological objects by transferring deeper features to shallow features. Finally, we design an auxiliary branch to calculate and refine the difficult areas in order to improve the weak edge segmentation performance of teeth and root canals. Through the combined tooth and root canal segmentation experiment of 157 clinical high-resolution CBCT data, it is verified that the proposed method is superior to the existing tooth or root canal segmentation methods.

Abstract:In the field of dental imaging, precise segmentation of individual teeth from 3D Cone Beam Computed Tomography (CBCT) images remains a challenging task. This paper proposes a novel coarse-to-fine segmentation framework specifically optimized for individual tooth segmentation in CBCT imagery. At its core, in the coarse stage, the framework employs an embedding grouping network that ensures accurate localization and establishes a strong tooth-specific prior. To address the intricacies of tooth morphology, in the fine stage, our architecture designs a Tripartite Fusion Module and a Decouple Body and Edge Module. These modules work in concert to refine the segmentation process, thereby improving the granularity of individual tooth delineation. We have tested our framework on a real-world dataset, and the results demonstrate improvement over existing methods.

Abstract:Deep learning allows automatic segmentation of teeth on cone beam computed tomography (CBCT). However, the segmentation performance of deep learning varies among different training strategies. Our aim was to propose a 3.5D U-Net to improve the performance of the U-Net in segmenting teeth on CBCT. This study retrospectively enrolled 24 patients who received CBCT. Five U-Nets, including 2Da U-Net, 2Dc U-Net, 2Ds U-Net, 2.5Da U-Net, 3D U-Net, were trained to segment the teeth. Four additional U-Nets, including 2.5Dv U-Net, 3.5Dv5 U-Net, 3.5Dv4 U-Net, and 3.5Dv3 U-Net, were obtained using majority voting. Mathematical morphology operations including erosion and dilation (E&D) were applied to remove diminutive noise speckles. Segmentation performance was evaluated by fourfold cross validation using Dice similarity coefficient (DSC), accuracy, sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV). Kruskal–Wallis test with post hoc analysis using Bonferroni correction was used for group comparison. P < 0.05 was considered statistically significant. Performance of U-Nets significantly varies among different training strategies for teeth segmentation on CBCT ( P < 0.05). The 3.5Dv5 U-Net and 2.5Dv U-Net showed DSC and PPV significantly higher than any of five originally trained U-Nets (all P < 0.05). E&D significantly improved the DSC, accuracy, specificity, and PPV (all P < 0.005). The 3.5Dv5 U-Net achieved highest DSC and accuracy among all U-Nets. The segmentation performance of the U-Net can be improved by majority voting and E&D. Overall speaking, the 3.5Dv5 U-Net achieved the best segmentation performance among all U-Nets.

Abstract:Abstract Digital dentistry plays a pivotal role in dental healthcare. A critical step in many digital dental systems is to accurately delineate individual teeth and the gingiva in the three-dimension (3D) intraoral scanned (IOS) mesh data. However, previous state-of-the-art methods are either time-consuming or error-prone, hence hinder their clinical applicability. In this paper, we present an accurate, efficient, and fully-automated deep learning model, trained on a dataset of 4,000 IOS data annotated by experienced human experts. On a hold-out dataset of 200 scans, our model achieves a per-face accuracy, average-area accuracy and area under the receiver operating characteristic curve (AUC) of 96.94%, 98.26%, and 0.9991, respectively, significantly outperforming the state-of-the-art baseline. In addition, our model only takes about 24 seconds to generate segmentation outputs, as compared to over 5 minutes by the baseline and 15 minutes by human experts. A clinical performance test of 500 patients with malocclusion or/and abnormal teeth shows that 96.9% of the segmentations are satisfactory for clinical applications, 2.9% automatically trigger alarms for human improvement, and only 0.2% of them need rework. Our research demonstrates the potential for deep learning to improve the efficacy and efficiency of dental treatment and digital dentistry.

Abstract:Accurate and automatic segmentation of three-dimensional (3D) individual teeth from cone-beam computerized tomography (CBCT) images is a challenging problem because of the difficulty in separating an individual tooth from adjacent teeth and its surrounding alveolar bone. Thus, this paper proposes a fully automated method of identifying and segmenting 3D individual teeth from dental CBCT images. The proposed method addresses the aforementioned difficulty by developing a deep learning-based hierarchical multi-step model. First, it automatically generates upper and lower jaws panoramic images to overcome the computational complexity caused by high-dimensional data and the curse of dimensionality associated with limited training dataset. The obtained 2D panoramic images are then used to identify 2D individual teeth and capture loose- and tight- regions of interest (ROIs) of 3D individual teeth. Finally, accurate 3D individual tooth segmentation is achieved using both loose and tight ROIs. Experimental results showed that the proposed method achieved an F1-score of 93.35% for tooth identification and a Dice similarity coefficient of 94.79% for individual 3D tooth segmentation. The results demonstrate that the proposed method provides an effective clinical and practical framework for digital dentistry.

Abstract:Accurate segmentation of the teeth from Cone Beam Computed Tomography (CBCT) images is a critical step towards building the personalized 3D digital model as it can provide important information to orthodontists for clinical treatments. However, the teeth CBCT image segmentation is a challenging task, especially for the root parts, because the root contour of a tooth may be degenerated by noise and surrounding alveolar bone or neighboring teeth. Most existing methods employ semi-automatic or interactive methods and there are few automatic and high-precision methods for teeth root segmentation. In this paper, we design a lightweight CNN architecture to accomplish this task as an end-to-end framework which can automatically segment the teeth from CBCT images. Specifically, we use ordinary convolutions, dilated convolutions and residual connections as the basic module to build the network. After that, a geodesic active contour model is employed to refine the CNN's outputs which can further improve the segmentation results. The whole pipeline is fully automatic and without any image-specific fine tune. The method is evaluated on a dental CBCT segmentation challenge and achieves state-of-the-art results.

Abstract:Semantic segmentation on medical Computed Tomography (CT) images is of great significance to research and clinical diagnosis. However, methods based on neural network have competitive advantages for segmentation of dental CT images. In this paper, a 3D multi-feature fusion method for tooth segmentation is proposed. In order to obtain the body space of the data, first of all, the dental CT training set is compressed in NII format, and the body space data is processed; then the proposed 3D convolution network is used to train the data, extract the feature vectors, and obtain the probability distribution; to handle the situation that 3D neural network always leads to fuzzy boundary and unclear topology, the new CRF algorithm is used to refine the probability distribution which removes the redundant information generated by the neural network model, and makes the segmentation results more accurate. Compared with diverse contemporary segmentation algorithms, the effectiveness and superiority of our proposed method are verified, proving the conclusion that the supervision mechanism, neural network model components, and optimization proposed methods can improve the accuracy of tooth segmentation is reliable and valid.

Abstract:OBJECTIVES:Instance-level tooth segmentation extracts abundant localization and shape information from panoramic radiographs (PRs). The aim of this study was to evaluate the performance of a mask refinement network that extracts precise tooth edges.METHODS:A public dataset which consists of 543 PRs and 16211 labelled teeth was utilized. The structure of a typical Mask Region-based Convolutional Neural Network (Mask RCNN) was used as the baseline. A novel loss function was designed focus on producing accurate mask edges. In addition to our proposed method, 3 existing tooth segmentation methods were also implemented on the dataset for comparative analysis. The average precisions (APs), mean intersection over union (mIoU), and mean Hausdorff distance (mHAU) were exploited to evaluate the performance of the network.RESULTS:A novel mask refinement region-based convolutional neural network was designed based on Mask RCNN architecture to extract refined masks for individual tooth on PRs. A total of 3311 teeth were correctly detected from 3382 tested teeth in 111 PRs. The AP, precision, and recall were 0.686, 0.979, and 0.952, respectively. Moreover, the mIoU and mHAU achieved 0.941 and 9.7, respectively, which are significantly better than the other existing segmentation methods.CONCLUSIONS:This study proposed an efficient deep learning algorithm for accurately extracting the mask of any individual tooth from PRs. Precise tooth masks can provide valuable reference for clinical diagnosis and treatment. This algorithm is a fundamental basis for further automated processing applications.

Deep learning approaches for tooth segmentation employ convolutional neural networks (CNNs) or Transformers to derive tooth feature maps from extensive training datasets. Tooth segmentation serves as a critical prerequisite for clinical dental analysis and surgical procedures, enabling dentists to comprehensively assess oral conditions and subsequently diagnose pathologies. Over the past decade, deep learning has experienced significant advancements, with researchers introducing efficient models such as U-Net, Mask R-CNN, and Segmentation Transformer (SETR). Building upon these frameworks, scholars have proposed numerous enhancement and optimization modules to attain superior tooth segmentation performance. This paper discusses the deep learning methods of tooth segmentation on dental panoramic radiographs (DPRs), cone-beam computed tomography (CBCT) images, intro oral scan (IOS) models, and others. Finally, we outline performance-enhancing techniques and suggest potential avenues for ongoing research. Numerous challenges remain, including data annotation and model generalization limitations. This paper offers insights for future tooth segmentation studies, potentially facilitating broader clinical adoption.

Abstract:To solve the problem of low efficiency, the complexity of the interactive operation, and the high degree of manual intervention in existing methods, we propose a novel approach based on the sparse voxel octree and 3D convolution neural networks (CNNs) for segmenting and classifying tooth types on the 3D dental models. First, the tooth classification method capitalized on the two-level hierarchical feature learning is proposed to solve the misclassification problem in highly similar tooth categories. Second, we exploit an improved three-level hierarchical segmentation method based on the deep convolution features to conduct segmentation of teeth-gingiva and inter-teeth, respectively, and the conditional random field model is used to refine the boundary of the gingival margin and the inter-teeth fusion region. The experimental results show that the classification accuracy in Level_1 network is 95.96%, the average classification accuracy in Level_2 network is 88.06%, and the accuracy of tooth segmentation is 89.81%. Compared with the existing state-of-the-art methods, the proposed method has higher accuracy and universality, and it has great application potential in the computer-assisted orthodontic treatment diagnosis.

Abstract:The process of constructing precise geometry of human jaws from cone beam computed tomography (CBCT) scans is crucial for building finite element models and treatment planning. Despite the success of deep learning techniques, they struggle to accurately identify delicate features such as thin structures and gaps between the tooth-bone interfaces where periodontal ligament resides, especially when trained on limited data. Therefore, segmented geometries obtained through automated methods still require extensive manual adjustment to achieve a smooth and organic 3D geometry that is suitable for simulations. In this work, we require the model to provide anatomically correct segmentation of teeth and bones which preserves the space for the periodontal ligament layers. To accomplish the task with few accurate labels, we pre-train a modified MultiPlanar UNet as the backbone model using inferior segmentations, i.e., tooth-bone segmentation with no space in the tooth-bone interfaces, and fine-tune the model with a dedicated loss function over accurate delineations that considers the space. We demonstrate that our approach can produce proper tooth-bone segmentations with gap interfaces that are fit for simulations when applied to human jaw CBCT scans. Furthermore, we propose a marker-based watershed segmentation applied on the MultiPlanar UNet probability map to separate individual tooth. This has advantages when the segmentation task is challenged by common artifacts caused by restorative materials or similar intensities in the teeth-teeth interfaces in occurrence of crowded teeth phenomenon. Abstract:Recent advances in deep learning are making it possible to segment dental Cone beam Computer Tomography (CBCT) images automatically and effectively. However, complex root morphological features, fuzzy boundaries between tooth roots and alveolar bone, and costly annotation of dental CBCT images limit the capability of existing deep learning models. To address the above issues, we collected dental CBCT data from 200 patients and labeled 45 of them for network training. We proposed the CNN-Transformer Architecture UNet (CTA-UNet) network, which can effectively extract local features by CNN and capture feature dependencies remotely by Transformer. Multiple spatial attention modules enhance the spatial information extraction and representation ability of the network. Further, we proposed a novel Masked image modeling (MIM) method to pre-train CNN and transformer modules simultaneously, to mitigate the limitations caused by a smaller amount of labeled training data. To the best of our knowledge, this is the first study that applied self-supervised learning methods to CNN-Transformer architecture network on dental CBCT images segmentation task. Experimental results show that the proposed method is superior to current dental CBCT image segmentation techniques and has real-world applicability in orthodontics and dental implants.

Conformational dynamics of bis(BF2)‐2,2′‐bidipyrrins revealed by through‐space 13C19F and 19F19F couplings

Abstract:The conformation of [bis‐(N,N′‐difluoroboryl)]‐3,3′‐diethyl‐4,4′,8,8′,9,9′,10,10′‐octamethyl‐2,2′‐bidipyrrin (1) in solution was studied by analyzing the 13C19F and 19F19F through‐space spin–spin couplings. The 1H and 13C NMR spectra were assigned on the basis of nuclear Overhauser effect spectroscopy (NOESY), heteronuclear single‐quantum correlation (HSQC), and heteronuclear multiple‐bond correlation (HMBC) experiments. The 19F spectrum of 1 was compared with that of 2‐ethyl‐1,3,5,6,7‐pentamethyl‐4,4‐difluoro‐4‐bor‐3a,4a‐diaza‐s‐indacen (2). The 19F19F through‐space spinspin coupling in 1 was thus assigned and the coupling constant was obtained by simulating the coupling patterns. The obtained conformation of 1 was compared with those of the known complexes [bis‐(N,N′‐difluoroboryl)]‐3,3′,8,8′,9,9′‐hexaethyl‐4,4′,10,10′‐tetramethyl‐6,6′‐(4‐methylphenyl)‐2,2′‐bidipyrrin (3)and [bis‐(N,N′‐difluoroboryl)]‐9,9′‐diethyl‐4,4′,8,8′,10,10′‐hexamethyl‐3,3′‐bis(methoxycarbonylethyl)‐2,2′‐bidipyrrin (4). The conformational dynamics of 1, 3, and 4 was surveyed by observing the temperature dependence of the through‐space coupling constants between 253 and 333 K. The 13C19F and 19F19F through‐space spin–spin couplings thus confirm similar conformations of different BisBODIPYs in solution in contrast to earlier findings in the solid state.
