Abstract:BACKGROUND: Most of the acute exacerbations of chronic obstructive pulmonary disease (COPD) are due to infections, mostly due to bacteria and viruses. There is a need to study the outcome of microbe-induced airway inflammation.MATERIALS AND METHODS: It is an observational follow-up study from the pulmonary medicine department of Kalinga Institute of Medical Sciences with the participation of the Regional Medical Research Center, Bhubaneswar, from October 2018 to February 2022. Patients who were admitted with acute exacerbation of COPD and treated as per GOLD (Global Initiative for Chronic Obstructive Lung Disease) 2021 guidelines were included in the study. Those patients in the severe category, who had clinically recovered, had undergone pulmonary physiotherapy, were on prescribed medications and home oxygen therapy after discharge, were followed up every three months by telephone calls. Any exacerbation, clinical stability, or mortality information was recorded.RESULTS: Out of 197 cases, the majority were elderly, males, smokers, and belonged to urban areas; in total, 102 (51.8%) microbes were isolated as etiological agents of infective exacerbation in which 19.79% were viruses and 23.35% were bacteria, while coinfection was found in 8.62% cases. Among the viruses, rhinovirus, influenza virus, and respiratory syncytial virus were the major isolates. Among the bacteria, mostly gram-negative organisms such as Acinetobacter baumannii, Klebsiella pneumoniae, and Pseudomonas aeruginosa were isolated. Readmission was more among patients with coinfection.CONCLUSION: Acute exacerbation of COPD was mostly seen in males in the age group of 61-80 years. Rhinovirus and influenza A virus were the two most common viral isolates, and among the bacterial isolates, Acinetobacter baumannii and Klebsiella pneumoniae were predominantly detected. Poor clinical outcomes were noticed more among the coinfection group.

Abstract:ObjectiveTo investigate viral infection in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD) in Shanghai, and to analyze the clinical characteristics and biomarkers in viral infection. MethodsThis study included all consecutive patients who were admitted for a diagnosis of AECOPD during June 2013 to May 2015. Thirty-one stable COPD patients and 31 healthy controls were also recruited. Oropharyngeal samples were assessed, PCR for respiratory viruses were performed. Patients were divided into AECOPD virus-positive (+) group and AECOPD virus-negative (-) group according to viral detection. Luminex was used to detect the concentrations of inflammatory cytokines in the serum. ResultsA total of 264 patients were included with a mean age of 750.5 years. There were 72 patients (27.3%) identified with viral positive, of whom two patients were detected with double viral infections (FluA+FluB and RSVA+HRV, respectively). The rate of viral detection was associated with season, highest in winter. Comparisons of clinical characteristics showed no significant differences between AECOPD virus+ group and AECOPD virus- group. However, serum concentrations of interferon-inducible protein-10 (IP-10) and interferon-gamma (IFN-) in virus+ AECOPD patients were significantly higher than those in the virus- AECOPD, stable COPD and healthy control groups (P<.05). ConclusionViral infection was an important pathogen in AECOPD patients; the most common viruses included FluA, HRV and FluB. It was very difficult to diagnose the viral infection according to clinical characteristics. The increased of serum IP-10 and IFN- levels might be value to indicate viral infection in AECOPD.
Prevalence and risk of viral infection in patients with acute exacerbation of chronic obstructive pulmonary disease: a meta-analysis

Abstract:Exacerbations of chronic obstructive pulmonary disease (COPD) lead to substantial morbidity and mortality. Viral infections could be an important cause of acute exacerbations of COPD (AECOPD) and only a few studies report the prevalence of respiratory viruses on this disease. We aimed to update the review on the prevalence of respiratory viral infection in patients with AECOPD with a meta-analysis. We reviewed the prevalence of respiratory viruses on this disease by searching PubMed systematically to identify primary studies published from Jan 1990 to March 2012. Studies met with seven criteria were extracted for meta-analysis. A total of 17 studies were eligible for the meta-analysis. Weighted overall prevalence of respiratory viruses in patients with AECOPD was 39.3 % (95 % CI 36.9–41.6) with a high degree of a heterogeneity ( I 2 > 75 %). In contrast, the rate in stable COPD patients from four studies was 13.6 % (95 % CI 9.0–18.2) without any apparent heterogeneity. Pooled risk ratio for respiratory viral infection was 4.1 (95 % CI 2.0–8.5) for AECOPD as compared with stable COPD. Rhinovirus was the most common virus and with a weighted prevalence of 14.8 % (95 % CI 13.3–16.5). Respiratory viruses probably are important etiological agents in patients with AECOPD as compared with the stable COPD patients. This result would help to provide better strategies for management of AECOPD and health-care planning.

Abstract:Objective To study the conditions of microbiological infection of lower respiratory tract in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods Thirty two samples of induced sputum were cultured from 32 AECOPD patients.Potential pathogenic bacteria were regarded as significant only if they reached a growth of≥10~7CFU/ml indicating the presence of bacterial exacerbation of COPD.And such common 7 kinds of viruses as influenza A,influenza B,parainfluenzaⅠ,parainfluenzaⅡ,parainfluenzaⅢ,respiratory syncytial virus(RSV) and adenovirus were examined at the same time by indirect immunofluorescence assay and virus isolation from culture.Results A total of 40 trains of bacteria were isolated.The growth of 22 trains from 16 AECOPD patients was more than or equal to 10~7CFU/ml.Mixed infection was identified in 8 AECOPD patients.Single infection was identified in 8 AECOPD patients.Gram-positive cocci occupied 22.7%(5 strains).Gram-negative cocci occupied 13.6%(3 strains).Gram-negative rods occupied 50%(11 strains),with 22.7%(5 strains) of Pseudomonas aeruginosa being the highest.Fungi occupied 13.7%(3 strains).Among 32 sputum samples none of the 7 kinds of viruses were detected by indirect immunofluorescence assay,in the viral culture undertaken only 1 sample had positive finding for parainfluenzaⅢ,and the other samples were negative.The positive findings of pathogenic bacteria culture in sputum from AECOPD patients were occupied 50%,and the positive findings of viral culture were 3.1%.Mixed infection of pathogenic bacteria and viruses was 3.1%.Conclusion Gram-negative bacteria are the main pathogenic bacteria for AECOPD,and pseudomonas aeruginosa is the most common.The rate of positive findings for 7 kinds of common viruses in samples of induced sputum is lower.

Abstract:Objective To study the relationship between distribution and drug resistance of AECOPD pathogens and comprehensive evaluation classification of AECOPD patients. Methods The sputum specimens of 142 AECOPD patients who hospitalized in Jinhua Fifth Hospital from 2008 to 2012 were conducted by sputum culture and drug sensitivity test. After comprehensive assessment classification according to GOLD( 2011),all patients were divided into A,B,C and D level. Results One hundred and twenty-nine pathogen strains were isolated from 99 AECOPD patients( 99 /142). In bacteria distribution,Gram-negative bacteria were predominant,accounting for 59.6%,most of which were Pseudomonas aeruginosa,Klebsiella pneumonia,Acinetobacter baumannii and Escherichia coli. The susceptibility tests in vitro showed that most Gram-negative bacteria were sensitive to Imipenem. Gram-positive bacteria infection accounted for 20. 9%,most of which were Staphylococcus aureus,Enterococcus and Streptococcus pneumoniae. Most Gram-positive bacteria were sensitive to vancomycin. Fungous infection accounted for 19. 3%,including Candida albicans and C. Tropicalis. They had a higher sensitivity to Fluorocystine,Amphotricin B,Itraconazole and Voriconazole. Mixed infection accounted for 27. 2%. The ratios of Gram-negative bacteria in AECOPD patients with A,B,C and D level were 46. 6%,50. 0%,59. 0%,66. 0%,respectively; fungi were 6. 2%,12. 5%,20. 4%and 24. 0%,respectively; and mixed infection were 15. 3%,23. 0%,22. 9% and 36. 8%,respectively. The detection rates of Gramnegative bacteria and fungi as well a s the mixed infection rates of patients in C,D level were higher than A,B level( P < 0. 05).Conclusion The dominant pathogen of AECOPD patients is Gram-negative bacteria.,fungous infection and mixed infection are also high. The situation of drug resistance is serious,and the comprehensive evaluation classification of the patients is high.
Airway pathogens detected in stable and exacerbated COPD in patients in Asia-Pacific

Abstract:Background: The burden of chronic obstructive pulmonary disease (COPD) in the Asia-Pacific region is projected to increase. Data from other regions show bacterial and viral infections can trigger acute exacerbations of COPD (AECOPD). Methods: This 1-year prospective epidemiological study (ClinicalTrials.gov identifier: NCT03151395) of patients with moderate to very severe COPD in Hong Kong, the Philippines, South Korea and Taiwan assessed the prevalence in sputum samples (by culture and PCR) of bacterial and viral pathogens during stable COPD and AECOPD. The odds of experiencing an exacerbation was evaluated for pathogen presence, acquisition and apparition. Health-related quality of life (HRQOL) was assessed. Results: 197 patients provided 983 sputum samples, with 226 provided during exacerbation episodes. The mean yearly AECOPD incidence rate was 1.27 per patient. The most prevalent bacteria by PCR at exacerbation were Haemophilus influenzae (Hi) and Moraxella catarrhalis (Mcat); Mcat prevalence was higher at exacerbation than at stable state. Virus prevalence was low, other than for human rhinovirus (HRV) (8.1%, stable state; 16.6%, exacerbation). The odds ratio (95% CI) for an exacerbation (versus stable state) was statistically significant for the presence, acquisition and apparition of Hi (2.20, 1.26-3.89; 2.43, 1.11-5.35; 2.32, 1.20-4.46, respectively), Mcat (2.24, 1.30-3.88; 5.47, 2.16-13.86; 3.45, 1.71-6.98, respectively) and HRV (2.12, 1.15-3.91; 2.22, 1.09-4.54; 2.09, 1.11-3.91, respectively). HRQOL deteriorated according to the number of exacerbations experienced. Conclusion: In patients with COPD in the Asia-Pacific region, the presence of Hi, Mcat or HRV in sputum samples significantly increased the odds of an exacerbation, providing further evidence of potential roles in triggering AECOPD.

Abstract:Free secretory component (FSC) was purified from guinea-pig milk by gel-filtration and immunoabsorption on anti-SC antibodies. Guinea-pig FSC cross-reacted with antisera to human FSC, and the reverse. Guinea-pig and human FSC resembled each other in molecular size, electrophoretic mobility and heterogeneity, as well as by the existence of antigenic determinants restricted to the unassociated form of the molecule. Guinea-pig FSC associates in vitro with guinea-pig IgM. Distribution of disulphide links is required to set free guinea-pig FSC from secretory IgA.
Time-course of upper respiratory tract viral infection and COPD exacerbation

Abstract:Viral respiratory tract infections have been implicated as the predominant risk factor for acute exacerbations of chronic obstructive pulmonary disease (AECOPD). We aimed to evaluate, longitudinally, the association between upper respiratory tract infections (URTI) caused by viruses and AECOPD. Detection of 18 viruses was performed in naso- and orοpharyngeal swabs from 450 COPD patients (Global Initiative for Chronic Obstructive Lung Disease stages 2–4) who were followed for a mean of 27 months. Swabs were taken during stable periods (n=1909), at URTI onset (n=391), 10 days after the URTI (n=356) and during an AECOPD (n=177) and tested using a multiplex nucleic acid amplification test. Evidence of at least one respiratory virus was significantly higher at URTI onset (52.7%), 10 days after the URTI (15.2%) and during an AECOPD (38.4%), compared with the stable period (5.3%, p<0.001). During stable visits, rhinovirus accounted for 54.2% of all viral infections, followed by coronavirus (20.5%). None of the viruses were identified in two consecutive stable visits. Patients with a viral infection at URTI onset did not have a higher incidence of exacerbation than patients without viral infection (p=0.993). Τhe incidence of any viral infection during an AECOPD was similar between URTI-related AECOPD and non-URTI-related AECOPD (p=0.359). Only 24% of the patients that had a URTI-related AECOPD had the same virus at URTI onset and during an AECOPD. Detection of parainfluenza 3 at URTI onset was associated with a higher risk of an AECOPD (p=0.003). Rhinovirus and coronavirus were the most frequently detected viruses during AECOPD visits, accounting for 35.7% and 25.9% of all viral infections, respectively. The prevalence of viral infection during the stable period of COPD was low. The risk of exacerbation following the onset of URTI symptoms depends on the particular virus associated with the event and was significant only for parainfluenza 3.

Abstract:Background Bacteria-induced respiratory infection has been long considered to be the major cause of acute exacerbation of chronic obstructive pulmonary disease (AECOPD). Therefore, a clear picture about the distribution and drug-resistance of pathogenic bacteria in the lower airways should be helpful for treatment of the disease. So far, data on this topic among Chinese are lacking. Methods A surveillance study was performed in consecutive patients with AECOPD at five areas in China between October 2006 and April 2008. The sputum from these patients was cultured and isolated for bacteria. Agar dilution method was used to determine the minimal inhibitory concentrations (MICs) of levofloxacin and other 15 antibiotics against these strains. Results Three hundred and fifty-nine pathogenic bacterial strains were isolated among 884 patients with AECOPD. The predominant bacteria were Pseudomonas aeruginosa (21.7%), Klebsiella pneumoniae (12.3%), Haemophilus influenzae (14.2%) and Streptococcus pneumoniae (11.7%), followed by Haemophilus parainfluenzae (9.5%), Acinetobacter baumannii (7.8%), Moraxella catarrhalis (6.4%) and Escherichia coli (3.6%). The majority of bacterial pathogens isolated in this study were susceptible to fluoroquinolones, ceftazidime, cefepime and imipenem. Conclusions Gram-negative bacilli are the leading pathogens in patients with AECOPD in China. Haemophilus parainfluenzae may be one of the most important pathogens in AECOPD. This study provides evidence for local surveillance of AECOPD pathogens and appropriate choice of antimicrobials in China.
The roles of bacteria and viruses in COPD-Bronchiectasis association: A prospective cohort study

Abstract:Background: Exacerbations are implicated in bronchiectasis and COPD, which frequently co-exist [COPD-Bronchiectasis association (CBA)]. We aimed to determine the bacterial and viral spectrum at stable-state and exacerbation onset of CBA, and their association with exacerbations and clinical outcomes of CBA as compared with bronchiectasis. Methods: We prospectively collected spontaneous sputum from adults with CBA, bronchiectasis with (BO) and without airflow obstruction (BNO) for bacterial culture and viral detection at stable-state and exacerbations. Results: We enrolled 76 patients with CBA, 58 with BO, and 138 with BNO (711 stable and 207 exacerbation visits). Bacterial detection rate increased from BNO, CBA to BO at steady-state (P = 0.02), but not at AE onset (P = 0.91). No significant differences in viral detection rate were found among BNO, CBA and BO. Compared with steady-state, viral isolations occurred more frequently at exacerbation in BNO (15.8 % vs 32.1 %, P = 0.001) and CBA (19.5 % vs 30.6 %, P = 0.036) only. In CBA, isolation of viruses, human metapneumovirus and bacteria plus viruses was associated with exacerbation. Repeated detection of Pseudomonas aeruginosa (PA) correlated with higher modified Reiff score (P = 0.032) in CBA but not in BO (P = 0.178). Repeated detection of PA yielded a shorter time to the first exacerbation in CBA [median: 4.3 vs 11.1 months, P = 0.006] but not in BO (median: 8.4 vs 7.6 months, P = 0.47). Conclusions: Isolation of any viruses, human metapneumovirus and bacterialplus viruses was associated with CBA exacerbations. Repeated detection of PA confers greater impact of future exacerbations on CBA than on BO.

Emerging evidence suggests an association between alterations in the respiratory microbiome flora species and airway inflammation in patients with AECOPD. The present study aimed to describe the inflammatory cells and bacterial microbiome distributions in respiratory tract in Egyptian patients with AECOPD. Subjects and Methods: The present cross-sectional study included 208 patients with AECOPD. Sputum and broncho-alveolar lavage samples from the studied patients were submitted to microbial cultures using appropriate media. Total and differential leukocytic counts and were done via automated cell counter. Results: The present study included 208 AECOPD patients. They comprised 167 males (80.3%) and 41 females (19.7%) with an age of 57.9 ± 4.9 years. AECOPD was categorized as mild, moderate and severe in 30.8%, 43.3% and 26%, respectively. Sputum samples had significantly higher TLC, neutrophil percent and eosinophil percent when compared with BAL samples. In contrast, lymphocyte percent was significantly higher in BAL samples. Sputum specimens had significantly lower frequency of positive growths (70.2% versus 86.5%, p = 0.001). Among the identified organisms, sputum specimens had significantly lower frequency of Strept. pneumoniae (14.4% versus 30.3%, p = 0.001), Klebsiella pneumoniae (19.7% versus 31.7%, p = 0.024), Haemophilus influenzae (12.5% versus 26.9%, p = 0.011), Pseudomonas aeruginosa (2.9% versus 10%, p = 0.019) and Acinetobacter spp . (1.9% versus 7.2%, p = 0.012) growths when compared with BAL samples. Conclusion: The present study could identify a distinctive pattern of inflammatory cell distribution in sputum and BAL samples of AECOPD patients. The most commonly isolated organisms were Klebsiella pneumoniae and Strept. pneumoniae . The most commonly isolated microbes are H. influenzae, M. catarrhalis, S. pneumoniae, S. aureus, and P. aeruginosa . 3 Acute exacerbation of chronic obstructive pulmonary disease (AECOPD) is an acute and sustained worsening of a patient's condition from a stable state. 4 Possible risk factors include air pollution, seasonal change, ethnic background, associated comorbidities, poor exercise capacity and obesity. 5 Management strategy usually includes bronchodilators and corticosteroids as first-line treatments. In patients with severe exacerbations, noninvasive ventilation, magnesium, ketamine, and epinephrine may be considered. Mechanical ventilation may be needed in selected situations. 6 AECOPD has profound effects on disease progression and patients' quality of life. Abstract:Acute exacerbation of chronic obstructive pulmonary disease (AECOPD) is one of the leading causes of admission to the intensive care unit, often triggered by a respiratory tract infection of bacterial or viral aetiology. Managing antibiotic therapy in this context remains a challenge. Respiratory panel molecular tests allow identifying viral aetiologies of AECOPD. We hypothesized that the systematic use of a respiratory multiplex PCR (mPCR) would help antibiotics saving in severe AECOPD. Our objectives were to describe the spectrum of infectious aetiologies of severe AECOPD, using a diagnostic approach combining conventional diagnostic tests and mPCR, and to measure antibiotics exposure. The study was bicentric, prospective, observational, and included 105 critically ill patients with a severe AECOPD of presumed infectious aetiology, in whom a respiratory mPCR with a viral panel was performed in addition to conventional microbiological tests. Altogether, the microbiological documentation rate was 50%, including bacteria alone (19%), respiratory viruses alone (16%), and mixed viruses and bacterial species (16%). The duration of antibiotic therapy was shorter in patients without documented bacterial infection (5.6 vs . 9 days; P = 0.0006). This pilot study suggests that molecular tests may help for the proper use of anti-infective treatments in critically ill patients with severe AECOPD.

Abstract:Objective: To determine the pattern of respiratory pathogens at bronchiectasis exacerbation and its associations with disease severity. Methods: A total of 119 steady-state bronchiectasis patients [42 males, 77 females, age range 19 to 74 years, mean age (45±14)years], diagnosed by a compatible history combined with evidence of bronchial dilatation on high-resolution computed tomography (HRCT), were recruited prospectively from out-patient clinics in the First Affiliated Hospital of Guangzhou Medical University between September 2012 and March 2013. A comprehensive history taking, radiologic appearance, spirometry, sputum bacterial culture and 16 respiratory viruses in nasopharyngeal swabs and sputum samples by PCR assays were collected at steady-state bronchiectasis. All bronchiectasis patients were followed up one year and assessed for bacteriology, virology and systemic inflammatory indices [including white blood cell, C-reactive protein (CRP), interleukin-6, 8 and tumor necrosis factor-α] during bronchiectasis exacerbation. Results: Fifty-eight bronchiectasis patients [20 males, 38 females, age range 19 to 74 years, mean age (44±14) years] reported 100 exacerbations (1 to 5 exacerbation events per patient) during one year follow-up. Respiratory viruses were found more frequently in sputum and nasal swab during exacerbation [35.0% (35/100) and 39% (39/100)] than those during steady-state in bronchiectasis [sputum: 13.8% (8/58), nasal swab: 8.6% (5/58)] (χ(2)=8.33,χ(2)=13.51; respectively, all P<0.05). The rate of bacterial detection during exacerbation in sputum was 56% (56/100), which was not significantly different compared with those at steady-state (35/58, 60.3%;χ(2)=0.284, P=0.59). Of these respiratory infections, viral-bacterial co-infection accounted for 30% exacerbation events. The most common bacteria and viruses during exacerbation in mild bronchiectasis (n=18, with 25 exacerbation events) were Haemophilus parainfluenzae (4 cases) in sputum and influenza A in nasal swab or sputum (4 cases), respectively. In patients with moderate (n=17, with 29 exacerbation events)-severe bronchiectasis (n=23, with 46 exacerbation events), pseudomonas aeruginosa was the most common bacteria in sputum (35 cases), and the most common respiratory viruses were rhinovirus in nasal swab or sputum (11 cases). In these 100 exacerbation events, patients with bacterial and viral co-infection, pure bacteria infection, pure virus infection, no bacteria and virus infection accounted for 30, 29, 16 and 25 exacerbation events, respectively. And patients with co-infection had higher serum CRP (45±23) mg/L and IL-8 [9.0 (4.4-15.5) ng/L] (F=23.32, F=9.81,respectively; all P<0.05), and increased risk of hospitalization (30% vs. 0] compared with those in non-infectious group(χ(2)=9.0, P=0.003). Conclusions: Pseudomonas aeruginosa, rhinovirus and influenza A were common causative agents of exacerbation in bronchiectasis.In patients with moderate-severe bronchiectasis, pseudomonas aeruginosa was the most common bacterium in sputum, and the most common respiratory virus was rhinovirus in nasal swab or sputum, compared to Haemophilus parainfluenzae in sputum and influenza A in nasal swab or sputum in mild bronchiectasis. Patients with co-infection had more severe systemic inflammatory response and higher risk of hospitalization during exacerbation.

Abstract:Objective To investigate the distribution and antibiotic resistance of bacterial pathogens in patients with acute exacerbation of chronic obstructive pulmonary diseases(AECOPD).Methods Sputum and tracheal secretions were collected by bronchoscopy in 625 patients with AECOPD who were hospitalized during the period from March 2007 to March 2009 in our hospital.The results of pathogen identification and susceptibility testing were analyzed.Results A total of 478 strains of microorganisms were isolated from 440 of the 625 patients.Gram-negative bacteria accounted for 65.27%,including P.aeruginosa,K.pneumoniae,Acinetobacter spp.,E.coli,and S.maltophilia.Gram-positive bacteria accounted for 20.71%,including S.aureus,S.pneumoniae,and Enterococcus spp.Fungi accounted for 14.02%,including C.albicans(6.69%),C.tropicalis and other yeasts.Susceptibility testing results showed that gram-negative bacteria were highly resistant to third generation cephalosporins,but relatively susceptible to piperacillin-tazobactam and carbapenems.Gram-positive cocci were highly resistant to penicillin,clindamycin,and erythromycin.No vancomycin-resistant strain was found in gram-positive isolates.Conclusions The main pathogens in lower respiratory tract infection in AECOPD patients are gram-negative bacteria.The bacterial isolates are highly resistant to antibiotics.Clinicians should pay attention to isolation and culture of pathogens and antimicrobial susceptibility test for more rational antibiotic therapy.

Abstract:Objective: To explore the underlying mechanisms the airway microbiome contributes to Acute Exacerbation of Chronic Obstructive Pulmonary Disease(AECOPD). Methods: We enrolled 31 AECOPD patients and 26 stable COPD patients, their sputum samples were collected for metagenomic and RNA sequencing, and then subjected to bioinformatic analyses. The expression of host genes was validated by Quantitative Real-time PCR(qPCR) using the same batch of specimens. Results: Our results indicated a higher expression of Rothia mucilaginosa ( p =0.015) in the AECOPD group and Haemophilus influenzae ( p =0.005) in the COPD group. The Different expressed genes(DEGs) detected were significantly enriched in "type I interferon signaling pathway"( p <0.001, q =0.001) in gene function annotation, and "Cytosolic DNA-sensing pathway"( p =0.002, q =0.024), "Toll-like receptor signaling pathway"( p =0.006, q =0.045), and "TNF signaling pathway"( p =0.006, q =0.045) in KEGG enrichment analysis. qPCR amplification experiment verified that the expression of OASL and IL6 increased significantly in the AECOPD group. Conclusion: Pulmonary bacteria dysbiosis may regulate the pathogenesis of AECOPD through innate immune system pathways like type I interferon signaling pathway and Toll-like receptor signaling pathway.

Abstract:Aim: To establish symptoms, lung function and to evaluate subsequent exacerbations of chronic obstructive pulmonary disease (COPD) during a year after virus-induced COPD exacerbations. Materials and methods: Patients hospitalized with viral (n=60), bacterial (n=60) and viral-bacterial (n=60) COPD exacerbations were enrolled to single-center prospective observational study. COPD was diagnosed according spirography criteria. Viral infection was established in bronchoalveolar lavage fluid or sputum by real-time reverse transcription-polymerase chain reaction for RNA of influenza A and B virus, rhinovirus, respiratory syncytial virus and SARS-CoV-2. Symptoms, lung function, COPD exacerbations were assessed. Patients were investigated at the hospitalization onset and then 4 and 52 weeks following the discharge from the hospital. Results: After 52 weeks in viral and viral-bacterial COPD exacerbations groups the rate of forced expiratory volume in one second (FEV1) decline were maximal - 71 (68; 73) ml/year and 69 (67; 72) ml/year versus 59 (55; 62) ml/year after bacterial exacerbations. Low levels of diffusion lung capacity for carbon monoxide (DLco/Va) - 52.5% (45.1%; 55.8%), 50.2% (44.9%; 56.0%) and 75.3% (72.2%; 80.1%) respectively, of 6-minute walk distance; p<0.001 in relation to bacterial exacerbations. In Cox proportional hazards regression analyses viral and viral-bacterial exacerbations were associated with increased risk of subsequent COPD exacerbations by 2.4 times independent of exacerbations rate before index event and FEV1. In linear regression models the relationships between airflow limitation and respiratory syncytial virus, rhinovirus and influenza virus infection, between low DLco/Va and rhinovirus, influenza virus and SARS-CoV-2 infection. Conclusion: COPD after virus-induced exacerbations were characterized by progression of airflow limitation, low DLco/Va, low 6-minute walking test distance, subsequent COPD exacerbations risk.

Design: Retrospective cohort study; analysis of hospital medical records and linked pathology and radiology reports. Setting: Tertiary health network in Victoria, Australia; January 2022–August 2022. Main Outcome Measures: Key clinical information including comorbidities, vaccination status, treatments administered and outcomes such as hospital length of stay, ICU admission, non-invasive ventilation usage and inpatient mortality. Results: One hundred ninety-nine viral AECOPDs - 125 SARS-CoV-2 and 74 other viruses were identified. Of the SARS-CoV-2 group. 13.6% were unvaccinated, 17.6% partially and 68.0% fully vaccinated. The SARS-CoV-2 group were older (77.2 vs 68.9, p < 0.00001) with more comorbidities (1[1– 2] vs 1[0– 2], p = 0.008) and lower candidacy for full resuscitation (25.6% vs 56.8%, p < 0.0001). Mortality tended to be higher among SARS-CoV2 admission (9.6% v 2.7%, p = 0.066) but rates of ICU admission (10.4% v 13.5%, p = 0.507), length of hospitalisation (5[3– 8] vs 5[3– 9], p = 0.9) and readmission within 30 days (25% vs 33.3%, p = 0.184) were similar. Conclusion: In a highly vaccinated population in the Omicron era, COPD patients requiring hospitalisation with SARS-CoV-2 are older with more comorbidities than those admitted with other respiratory viruses. Length of hospitalisation and ICU utilisation was similar. Inpatient mortality may be higher. Keywords: SARS-CoV2, coronavirus, COPD, viral exacerbations Patients with chronic obstructive pulmonary disease (COPD) are vulnerable to severe acute exacerbations (AECOPDs) when they contract respiratory viruses. 1 Exacerbations of COPD are cardinal events in the disease, associated with accelerated decline in lung function, reduced quality of life, hospitalization and mortality. 2–4 Viral infection appears to be responsible for up to 50% of AECOPDs. 5,6 The prognosis of virus-associated exacerbation appears different to other exacerbation etiotypes, with a slower recovery and prolonged hospitalization. 7,8 In general, studies examining hospitalized AECOPDs have considered all respiratory viruses as a generic grouping, with outcomes compared between "viral" and "non-viral" exacerbations or versus alternative broad etiologies such as bacterial or eosinophilic exacerbations. 9–11 Studies of individual viruses such as rhinovirus ("common cold"), influenza and Respiratory Syncytial Virus (RSV) have been performed, although there are very limited data to indicate whether specific respiratory viruses have different impacts as causes of severe AECOPDs. The relative impact of SARS-CoV-2 versus previously circulating respiratory viruses as a cause of severe AECOPD has been little studied. One prospective observational cohort study in the United Kingdom compared outcomes from AECOPDs secondary to SARS-CoV-2 versus groups with either alternative infections (bacterial or viral), or non-infective AECOPDs. 10 They reported higher 30-day mortality, longer hospital stays and more frequent requirement for positive pressure ventilatory support among the SARS-CoV-2 group. Much of this data was collected during the early phases of the pandemic with circulating wild-type, alpha and delta strains. Since the onset of the COVID pandemic, widespread vaccination and development of effective therapeutics have been achieved, limiting applicability of early pandemic data to the current era. SARS-CoV-2 is now an endemic community respiratory virus, and it is therefore necessary to evaluate the clinical phenotype of AECOPD secondary to SARS-CoV-2 contextualized alongside other commonly circulating respiratory viruses. With the introduction of new research methods and large-sample prospective studies, people have a deeper understanding of the role of respiratory infections in AECOPD. Abstract:Abstract Bacterial infection is a major cause of acute exacerbations of chronic obstructive pulmonary disease (AECOPD), which are associated with significantly increased airway and systemic inflammation. However, the relationship among bacteriology, the resolution of inflammation and clinical outcomes is largely unknown. In this study, we recruited consecutive patients hospitalized for AECOPD with purulent sputum. We measured the airway and systemic inflammation levels, the COPD assessment test (CAT) score and adverse outcomes between patients with and without potentially pathogenic microorganisms (PPM). Among sputum samples collected from the 135 episodes of AECOPD, 42 (31.1%) were PPM-positive at admission. Compared with those in the PPM-negative group, more patients in the PPM-positive group had ≥2 exacerbations in previous year and Anthonisen type I at admission and higher drop in sputum neutrophil, serum hs-CRP and CAT value from exacerbation to the subsequent baseline. No significant differences in the adverse outcomes between the two groups were observed. Among the 38 PPM-positive patients who survived and were discharged from hospital, 19 remained PPM-positive (bacterial persistence group) and 19 PPM-negative (bacterial clearance group). Both inflammation indices and CAT score decreased compared to admission in the two groups, regardless of the bacteriology at discharge. Our data suggest uncultivated bacteria and/or virus might also play important roles in causing inflammation and AECOPD.

Abstract:Patients with frequent exacerbations represent a chronic obstructive pulmonary disease (COPD) subgroup requiring better treatment options. The aim of this study was to determine the innate immune mechanisms that underlie susceptibility to frequent exacerbations in COPD. We measured sputum expression of immune mediators and bacterial loads in samples from patients with COPD at stable state and during virus-associated exacerbations. In vitro immune responses to rhinovirus infection in differentiated primary bronchial epithelial cells (BECs) sampled from patients with COPD were additionally evaluated. Patients were stratified as frequent exacerbators (≥2 exacerbations in the preceding year) or infrequent exacerbators (<2 exacerbations in the preceding year) with comparisons made between these groups. Frequent exacerbators had reduced sputum cell mRNA expression of the antiviral immune mediators type I and III interferons and reduced interferon-stimulated gene (ISG) expression when clinically stable and during virus-associated exacerbation. A role for epithelial cell-intrinsic innate immune dysregulation was identified: induction of interferons and ISGs during in vitro rhinovirus (RV) infection was also impaired in differentiated BECs from frequent exacerbators. Frequent exacerbators additionally had increased sputum bacterial loads at 2 wk following virus-associated exacerbation onset. These data implicate deficient airway innate immunity involving epithelial cells in the increased propensity to exacerbations observed in some patients with COPD. Therapeutic approaches to boost innate antimicrobial immunity in the lung could be a viable strategy for prevention and treatment of frequent exacerbations.
