Abstract:Proton and heavy ion therapy offer superior relative biological effectiveness (RBE) in the treatment of deep-seated tumors compared with conventional photon radiotherapy due to its Bragg-peak feature of energy deposition in organs. Many proton and carbon ion therapy centers are active all over the world. At present, five particle radiotherapy institutes have been built and are receiving patient in China, mainly including Wanjie Proton Therapy Center (WPTC), Shanghai Proton Heavy Ion Center (SPHIC), Heavy Ion Cancer Treatment Center (HIMM), Chang Gung Memorial Hospital (CGMH), and Ruijin Hospital affiliated with Jiao Tong University. Many cancer patients have benefited from ion therapy, showing unique advantages over surgery and chemotherapy. By the end of 2020, nearly 8,000 patients had been treated with proton, carbon ion or carbon ion combined with proton therapy. So far, there is no systemic review for proton and carbon ion therapy facility and clinical outcome in China. We reviewed the development of proton and heavy ion therapy, as well as providing the representative clinical data and future directions for particle therapy in China. It has important guiding significance for the design and construction of new particle therapy center and patients’ choice of treatment equipment.

Abstract:There are very strong demand for mid-energy of proton machine recent years due to the surging cancer patients and fast progress of the space science in China. For the applications of proton therapy and proton irradiation, the energy range of proton beam usually is from 200 MeV to 250 MeV, or even higher for astronavigation. Based on the R&D starting from 2009, two construction projects of 230 MeV and 250 MeV superconducting cyclotron, have been implemented recently at China Institute of Atomic Energy (CIAE). That was started in Jan 2015 for the 230 MeV machine, for the program of proton therapy and space science launched by China National Nuclear Corporation (CNNC), and in Jan 2016 for the 250 MeV machine, for the program of proton therapy launched by the Ministry of Science and Technology of China (MOST). In this paper, the designs for the two SC cyclotrons and their key components, including the superconducting main magnet, RF system, internal ion source and central region, extraction system, etc, and the construction progress of the machines will be presented.

Abstract:Protons and heavy ion radiation therapy have proven highly effective against a wide range of cancers and in recent decades there have been rapid advances. Due to the physical characteristics of the Bragg-peak and superior biological properties, proton and heavy ion radiation is able to focus its energy on the tumor while minimizing exposure to surrounding normal tissues and organs, which is expected help enhance the tumor dose and reduce normal tissue damage. In recent years, many studies have explored the efficacy and safety of proton and heavy ion radiotherapy on various malignancies, such as head and neck cancer, lung cancer, esophageal cancer, and liver cancer. Abstract:To perform an evaluation of the on-going prospective clinical trials on particle radiation therapy and their impact on the current clinical practice as well as future clinical research and development.Furthermore,to briefly present the results of the registration trial of the IONTRIS particle therapy system at the Shanghai Proton and Heavy Ion Center.We used data from the clinicaltrials,gov and the Particle Therapy Collaborative Organization Group (PTCOG) website.After excluding retrospective and in silico studies,we examined and analyzed the prospective clinical trials for their ion type,targeting disease site,and nature.At the time of this analysis,149 prospective trials were identified on proton and carbon-ion radiation therapy,including 20 were carbon-ion and 129 trials were proton radiation focused,respectively.Except for 15 randomized phase Ⅱ/Ⅲ and Ⅲ trials,134 trials were phase 0-Ⅱ trials.Tumors from nearly all body parts were covered by the on-going trials,but trials on pediatric,GI,lung,prostate,and breast cancer account for the majority.The majority of the currently on-going trials focus on the efficacy and adverse-effects of the new dose/fractionation schemes of particle therapy as well as the use of particle therapy on new indications.Few studies invcstigate the addition of adjuvant therapy or imaging technology used in adjunct with particle therapy.Randomized trials that compare particle radiation therapy versus photon radiation is relatively uncommon.Despite the prevailing use of proton and heavy-ion radiation therapy for cancer treatment,～ 150 prospective clinical trials associated with particle radiation therapy are identified.As the majority of trials aim to investigate more efficacious dose/fractionation and the application of particle therapy on new indications,improved outcome from and expanded utilization of particle radiation therapy can be expected.

Abstract:Carbon ions have unique physical and biological properties that allow for precise targeting of tumors while minimizing damage to surrounding healthy tissues. The emitted neutrons dominate the radiation field in the treatment room and pose challenges for radiological shielding. Concrete is extensively utilized in the construction of radiotherapy facilities due to its good shielding characteristics, and it can be easily poured into the desired shapes and thickness. The difference in composition of concrete affects the characteristics of neutron attenuation and activation performance. Therefore, the purpose of this study is to clarify the shielding properties and activation performances of four types of concrete for carbon ion therapy facilities. The Monte Carlo method is used to analyze the neutron spectra from thick targets upon carbon ion bombardment. Furthermore, the deep attenuation efficiency of the secondary neutron in different compositions of concrete is discussed. The shielding design is developed to ensure compliance with the prescribed dose limit outside the shielding during operation. Finally, the induced radioactivity in concrete is estimated for both short-term and long-term operation. The produced radionuclides inventories and depth profiling are determined. This study reveals the shielding and radioactivity issue of carbon ion therapy facilities and is expected to aid in the design or construction of similar facilities.

Abstract:Objective. Non-primary radiation doses to normal tissues from proton therapy may be associated with an increased risk of secondary malignancies, particularly in long-term survivors. Thus, a systematic method to evaluate if the dose level of non-primary radiation meets the IEC standard requirements is needed. Approach. Different from the traditional photon radiation therapy system, proton therapy systems are composed of several subsystems in a thick bunker. These subsystems are all possible sources of non-primary radiation threatening the patient. As a case study, 7 sources in the P-Cure synchrotron-based proton therapy system are modeled in Monte Carlo (MC) code: tandem injector, injection, synchrotron ring, extraction, beam transport line, scanning nozzle and concrete reflection/scattering. To accurately evaluate the synchrotron beam loss and non-primary dose, a new model called the torus source model is developed. Its parametric equations define the position and direction of the off-orbit particle bombardment on the torus pipe shell in the Cartesian coordinate system. Non-primary doses are finally calculated by several FLUKA simulations. Main results. The ratios of summarized non-primary doses from different sources to the planned dose of 2 Gy are all much smaller than the IEC requirements in both the 15 cm to 50 cm and 50 cm to 200 cm regions. Thus, the P-Cure synchrotron-based proton therapy system is clean and patient-friendly, and there is no need an inner shielding concrete between the accelerator and patient. Significance. Non-primary radiation dose level is a very important indicator to evaluate the quality of a PT system. This manuscript provides a feasible Monte Carlo procedure for synchrotron-based proton therapy with new beam loss model. Which could help people figure out precisely whether this level complies with the IEC standard before the system put into clinical treatment. Abstract:The use of carbon ion beam therapy for tumor treatment offers unique physical and biological advantages, high efficiency, low toxicity, and a short treatment cycle. Therefore, it is considered to be the most ideal radiation therapy for cancer in the 21st century. However, construction and equipment operation costs for a carbon ion center are prohibitive. Currently, carbon ions are only used for clinical purposes in Japan, Germany, China, Italy, Austria, and a few other countries. In Japan, patients with cancer have been treated with carbon ions since 1994, with strong positive effects. To date, over 46,800 patients worldwide have undergone carbon ion radiotherapy (CIRT) for conditions such as malignant head and neck tumors as well as lung cancer, liver cancer, pancreatic cancer, and other cancer types. We conducted a review encompassing over 20 years of global clinical experience with CIRT to summarize treatment outcomes, recent developments, and the merits of CIRT as an emerging cancer treatment modality. We also compare its efficacy and toxicity with those of photon radiotherapy. These findings can serve as a useful reference in decision making and planning regarding new heavy ion centers to better serve patients with cancer.

Abstract:This paper provides the physical design of a low-energy proton irradiation facility for space radiation effect research, based on an existing linear accelerator (linac)-based compact neutron source. The beam requirement of the low-energy proton irradiation consists of a proton flux range of 10(8)-10(9)p/cm(2)/s with a nonuniformity of less than 10% in the target diameter area of 2 cm, and optional proton energies of < 1 MeV to 13 MeV. Based on the existing high-current proton linac, the flux range and nonuniformity of the proton beam at the target can be achieved by inserting beam apertures, adjusting quadrupole fields to expand the beam size, and decreasing the repetition rate of the beam pulse. To measure the energy distribution of the protons, one CdZnTe detector can be employed. The above approach is performed on the physical design of the low-energy proton facility for the Compact Pulsed Hadron Source at Tsinghua University. The proton flux at the target can be reduced from 6.2 x 10(14) p/cm(2)/s to 1.2 x 10(8) - 3.0 x 10(9) p/cm(2)/s, and proton energies of 0.56, 5.0, 8.2, and 10.5 MeV can be obtained separately with the energy degrader (aluminum) thicknesses of 75, 800, 550, and 300 mu m. Irradiation nonuniformity is less than 10%. Energy spread is less than 10% for the proton energy of 3.0, 5.0, 8.2, and 10.5 MeV, except for the low energy of 0.56 MeV.

Abstract:With the development of social economy and radiotherapy technique, proton/heavy ion radiotherapy has been applied widely to clinical practices. At present, there are at least 29 hospitals in China at various stages of planning, construction, commissioning or clinical operation of medical proton/heavy ion beam radiotherapy equipment. Compared with common radiotherapy accelerators used in conventional external beam radiotherapy, the proton/heavy ion therapy system has more stringent requirements for quality control so as to achieve an optimum therapeutic effect. In order to protect the health rights of patients undergoing radiotherapy, to facilitate the relevant administrative supervision departments to carry out standard-based approval and routine supervision and to promote the development of related medical undertakings, the standard for testing of quality control for medical proton/heavy ion beam radiotherapy equipment is drafted to fill the gap in this regard in China and even worldwide. The standard contains five indicators and corresponding testing methods for radiological protection and safety and 16 indicators for quality control of equipment performance. The standard is a mandatory standard and is based on the relevant Chinese legal requirements for the testing of radiotherapy equipment, so all the indicators listed in the standard shall be tested. During the drafting of the standard, the opinions from hospitals that are currently using proton/heavy ion medical accelerators for radiotherapy purpose and from the related equipment manufacturers were taken into account. The draft standard was revised with reference to these opinions and the feasibility of the related quality control requirements. The official version of the standard was released on March 7, 2023, and implementation is scheduled to begin on March 1, 2024.
Analysis of the Proton Therapy Controlling System and Application

Abstract:In recent years, the cancel therapy remains a key issue that needs to be addressed for human beings. As a matter of fact, the proton beam therapy (PBT) is a new kind of way to treat tumor by emitting proton beam. Different from conventional radiation therapy, the main principal is to cut the DNA of the tumor cells and prevent further development. In this case, this study will discuss the proton therapy controlling system as well as demonstrate the applications. Comparing to the X-rays, protons can align tumors more precisely and decrease the effect of other normal tissues, which also decrease the side effects. Nevertheless, the proton therapy is still be developed further. Many proton centers are being built and will begin operating in the future. This article popularizes the basic knowledge of proton therapy, from the basic principle, the applications to the limitations. These results shed light on guiding further exploration of proton therapy.
STUDY ON THE SHIELDING AND DOSE RATE DISTRIBUTIONS OF THERAPEUTIC PROTON SYNCHROTRON ACCELERATOR BASED ON FLUKA

Abstract:Some therapeutic proton synchrotron accelerators will be built for cancer treatment in China. The radiation produced by proton must be carefully evaluated and shielded for occupational disease hazard assessment and environmental impact assessment. Adopting the FLUKA code, a therapeutic room model, a synchrotron hall model and a high energy transport line tunnel model are constructed to get the ambient dose equivalent rate distributions. The ambient dose equivalent rates are also calculated with an empirical formula in some regions. The results calculated by the two ways are consistent with each other. The advantages and disadvantages of the two methods have been analyzed. The ambient dose equivalent rates are no more than 2.5 μSv/h at 30 cm beyond the shielding wall and the maze door. The dose rates are <25 μSv/h at 30 cm beyond the roof of the therapeutic room. These results comply with the National Occupational Health Standard requirements.
Research Status and Prospect of Tumor Therapy by Using Laser-Accelerated Proton Beams

Abstract:Proton radiotherapy is one of the most important modalities to treat cancers, which attracts a lot of attention for that it can precisely target the tumor with reduced side effects. However, due to the financial pressures of constructing and maintaining a proton radiotherapy center, nowadays this treatment method is not wildly available worldwide. In this context, the novel proton acceleration techniques based on high-power laser facilities are believed to become a competitive acceleration method, mainly for the advantages of compact size and low complexity. Meanwhile, the laser accelerated protons own some special features like broad energy spectrum, ultra-short pulse duration and ultra-high peak current, which would favor the research about radiobiological effects under ultra-high dose rate radiotherapy. As for laser-accelerator-based proton radiotherapy, the fundamental challenge nowadays lies on the proton energy that can be achieved (<100 MeV), which is not enough to completely meet the needs of human treatments, while we have seen the potential of the advanced target structure and cascaded acceleration to enhance the beam energy. Besides, quantities of in vitro and in vivo biological experiments have been carried out, benefiting from the adjunct development of beam transportation elements that are suitable for laser-accelerated protons, and biological phenomena that differ from the case of using conventional proton beams have been observed. Furthermore, compared with the standard dose rate radiotherapy, there are evidences showing the significant advantages of ultra-high dose rate radiotherapy in regulating immunity, which paves the way to a multidisciplinary field of radiobiology, immunology, and advanced acceleration technology, where there should be a place for laser accelerators. Additionally, the past two decades have witnessed the worldwide booming developments of petawatt class laser facilities, accompanied by the increasing peak power and declining cost per petawatt. This progress might promote the clinical use of laser accelerators in the future. In the past few years, we have carried out theoretical and experimental research concerned with laser proton acceleration, beam transportation, radiobiology and immunology based on the platforms of the Compact Laser Plasma Accelerator of Peking University and Beijing Laser Acceleration Innovation Center, in order to investigate the availability and probable advantages of tumor therapy by using laser-accelerated proton beams. In this review, the research status from these aspects at home and abroad is introduced, and the progress as well as problems is discussed especially in the dose delivery and biological effects. Finally, the future development of laser-accelerator-based proton therapy is discussed and prospected.

Abstract:Carbon ions offer significant advantages for deep-seated local tumors therapy due to their physical and biological properties. Secondary particles, especially neutrons caused by heavy ion reactions should be carefully considered in treatment process and radiation protection. For radiation protection purposes, the FLUKA Code was used in order to evaluate the radiation field at deep tumor therapy room of HIRFL in this paper. The neutron energy spectra, neutron dose and energy deposition of carbon ion and neutron in tissue-like media was studied for bombardment of solid water target by 430MeV/u C ions. It is found that the calculated neutron dose have a good agreement with the experimental date, and the secondary neutron dose may not exceed one in a thousand of the carbon ions dose at Bragg peak area in tissue-like media.
Particle Therapy for Cancers: a New Weapon in Radiation Therapy.

Abstract:Particle irradiation started to draw attention in the past decade and has now become a hotspot in the radiation oncology community. This article reviews the most advanced developments in particle irradiation, focusing on the characteristics of proton and carbon ions in radiation physics and radiobiology. The Bragg peak of physical dose distribution causes proton and carbon beams to optimally meet the requirement for cancer irradiation because the Bragg peak permits the accurate concentration of the dose on the tumor, thus sparing the adjacent normal tissues. Moreover, carbon ion has more radiobiological benefits than photon and proton beams. These benefits include stronger sterilization effects on intrinsic radio-resistant tumors and more effective killing of hypoxic, G(0), and S phase cells. Compared with the most advanced radiation techniques using photon, such as three-dimensional conformal radiation therapy and intensity-modulated radiation therapy, proton therapy has yielded more promising outcomes in local control and survival for head and neck cancers, prostate carcinoma, and pediatric cancers. Carbon therapy in Japan showed even more promising results than proton therapy. The local controls and overall survivals were as good as that treated by surgery in early stages of non-small cell lung cancer, hepatocellular carcinoma, prostate carcinoma, and head and neck cancers, especially for such highly resistant tumors as melanoma. The non-invasive nature of particle therapy affords more patients with chances to receive and benefit from treatment. Particle therapy is gradually getting attention from the oncology community. However, the cost of particle therapy facilities has limited the worldwide use of this technology.

Abstract:Radiotherapy should have low toxicity in the entrance channel (normal tissue) and be very effective in cell killing in the target region (tumour). In this regard, ions heavier than protons have both physical and radiobiological advantages over conventional X-rays. Carbon ions represent an excellent combination of physical and biological advantages. There are a dozen carbon-ion clinical centres in Europe and Asia, and more under construction or at the planning stage, including the first in the USA. Clinical results from Japan and Germany are promising, but a heated debate on the cost-effectiveness is ongoing in the clinical community, owing to the larger footprint and greater expense of heavy ion facilities compared with proton therapy centres. We review here the physical basis and the clinical data with carbon ions and the use of different ions, such as helium and oxygen. Research towards smaller and cheaper machines with more effective beam delivery is necessary to make particle therapy affordable. The potential of heavy ions has not been fully exploited in clinics and, rather than there being a single ‘silver bullet’, different particles and their combination can provide a breakthrough in radiotherapy treatments in specific cases.

Dose Escalated Carbon Ion Radiotherapy for Localized Prostate Cancer in Shanghai Proton and Heavy Ion Center: Toxicity and Efficacy Outcomes At Two Years.

Abstract:Abstract Purpose: The purpose of this study was to prospectively analyze the safety and feasibility of spot scanning carbon ion radiotherapy (CIRT) for patients with localized prostate cancer.Methods: 118 localized prostate cancer patients treated with spot scanning CIRT at Shanghai Proton and Heavy Ion Center (SPHIC) were enrolled in this dose escalated study. The dose was gradually increased from 59.2GyE to 65.6GyE in 16 fractions. The primary endpoint was the acute and late toxicities. Secondary endpoints were biochemical relapse free survival (bRFS), distant metastasis free survival (DMFS), prostate cancer-specific survival (PCSS), and overall survival (OS).Results: The median follow-up time was 30.2 months (4.8-62.7 months). Acute grade 1 and 2 genitourinary (GU) toxicities were 15.3% and 18.6%, while acute grade 1 and 2 gastrointestinal (GI) toxicities were 2.5% and 0%, respectively. Late grade 1 and 2 GU toxicities were 4.2% and 1.7%, respectively. No late GI toxicity were observed. There were no cases of severe acute or late toxicity (≥grade 3). The significant association was not found between the factors and the acute GU toxicities except for CTV volume (p=0.031) on multivariate analysis. The 2-year bRFS, DMFS, PCSS, OS were 100%, 100%, 100% and 98.8%, respectively.Conclusion: The 2 years’ outcomes are encouraging, providing additional and useful information on the feasibility and safety of spot scanning CIRT for prostate cancer. Long term follow-up and prospective multi-institutional data are warranted to reinforce the role of CIRT in the management of localized prostate cancer.Trial registration: Clinicaltrial, NCT02739659. Abstract:Laser plasma acceleration has made remarkable progress in the last few decades, but it also faces many challenges. Although the high gradient is a great potential advantage, the beam quality of the laser accelerator has a certain gap, or it is different from that of traditional accelerators. Therefore, it is important to explore and utilize its own features. In this article, some recent research progress on laser proton acceleration and its irradiation application, which was carried out on the compact laser plasma accelerator (CLAPA) platform at Peking University, have been introduced. By combining a TW laser accelerator and a monoenergetic beamline, proton beams with energies of less than 10 MeV, an energy spread of less than 1%, and with several to tens of pC charge, have been stably produced and transported in CLAPA. The beamline is an object–image point analyzing system, which ensures the transmission efficiency and the energy selection accuracy for proton beams with large initial divergence angle and energy spread. A spread-out Bragg peak (SOBP) is produced with high precision beam control, which preliminarily proved the feasibility of the laser accelerator for radiotherapy. Some application experiments based on laser-accelerated proton beams have also been carried out, such as proton radiograph, preparation of graphene on SiC, ultra-high dose FLASH radiation of cancer cells, and ion-beam trace probes for plasma diagnosis. The above applications take advantage of the unique characteristics of laser-driven protons, such as a micron scale point source, an ultra-short pulse duration, a wide energy spectrum, etc. A new laser-driven proton therapy facility (CLAPA II) is being designed and is under construction at Peking University. The 100 MeV proton beams will be produced via laser–plasma interaction by using a 2-PW laser, which may promote the real-world applications of laser accelerators in malignant tumor treatment soon.

Matter and Radiation at Extremes

Abstract:The newly built Compact Laser Plasma Accelerator–Therapy facility at Peking University will deliver 60 J/1 Hz laser pulses with 30 fs duration. Driven by this petawatt laser facility, proton beams with energy up to 200 MeV are expected to be generated for tumor therapy. During high-repetition operation, both prompt radiation and residual radiation may cause safety problems. Therefore, human radiological safety assessment before commissioning is essential. In this paper, we simulate both prompt and residual radiation using the Geant4 and FLUKA Monte Carlo codes with reasonable proton and as-produced electron beam parameters. We find that the prompt radiation can be shielded well by the concrete wall of the experimental hall, but the risk from residual radiation is nonnegligible and necessitates adequate radiation cooling. On the basis of the simulation results, we discuss the constraints imposed by radiation safety considerations on the annual working time, and we propose radiation cooling strategies for different shooting modes.
