Abstract:ObjectiveTo compare the air disinfection effects of different human-machine coexistence disinfection methods in the high-risk areas of airborne diseases in medical and healthcare institutions, and to provide a reference for the prevention and control of airborne diseases in medical and healthcare institutions.MethodsField trials were conducted in the fever clinic, the infection disease department, and dental clinics of a tertiary hospital in Shanghai, respectively. The existing air disinfection methods (plasma air disinfection machine, circulating air ultraviolet disinfection machine or negative pressure ventilation system), upper-room 222 nm ultraviolet germicidal system, and the combination of the existing air disinfection methods and upper-room 222 nm ultraviolet germicidal system were all used in each location in the experiment group. The control group did not adopt specific air disinfection methods. Air sampling was conducted by the six-level sieve hole microbial sampler or the flat slab exposure method. The daily air sampling time was from 8:00 a.m. to 16:00 p.m., with one sample per hour, and a total of 9 samples were taken. The disinfection effects were compared by calculating the total number of airborne bacteria colonies and the sterilization rate for each disinfection method.ResultsThe total numbers of airborne bacteria colonies in the fever outpatient infusion room, the ward and nurse station of infection disease department of 222 nm group were lower than that in the control group (P=0.005, P<0.001, P<0.001). The total numbers of airborne bacteria colonies in the fever outpatient infusion room and the dental examination room of 222 nm group were lower than that in the control group or plasma air disinfection machine group (P=0.022, P=0.014). The total numbers of airborne bacteria colonies in the nucleic acid sampling room of plasma air disinfection machine group combined with 222 nm group were lower than that in plasma air disinfection machine group (P=0.019). The total numbers of airborne bacteria colonies in the CT examination room of fever clinic of the 222 nm group were lower than that in the circulating air ultraviolet disinfection machine group (P=0.002). The total numbers of airborne bacteria colonies in the CT examination room of 222 nm group combined with circulating air ultraviolet disinfection machine were lower than that of circulating air ultraviolet disinfection machine group and the control group (P=0.008, P<0.001). The air sterilization rate of upper-room 222 nm ultraviolet germicidal system ranged from 48.04% to 73.74%. The air sterilization rate of plasma air/circulating air ultraviolet disinfection machine combined with the upper-room 222 nm ultraviolet germicidal system ranged from 6.86% to 73.77%.ConclusionUpper-room 222 nm ultraviolet germicidal system could effectively reduce airborne colonies in the air and improve air hygiene quality in both clinic and ward environments with high airborne transmission risks.
Investigation and analysis on the infection control and radiation safety of radiodiagnostic workplace for COVID 19

Abstract:Objective:To survey and supervise the risk of infection control and radiation safety in the radiological diagnostic workplace for COVID-19, and provide data support for the safety protection of radiographers and related staff.Methods:4 emergency hospitals for COVID-19 including 2 makeshift hospitals, module hospital and brick pattern hospital in Hubei province were performed for testing and evaluation of imaging performance and radiological protection for the 8 new installed CT scanners and places according to the national standards of WS 519-2019 and GBZ 130-2013. The infection control safety factors such as the layout of the equipment room were monitored and investigated. Two COVID-19 designated hospitals including general hospital and infectious disease specialized hospital were selected to carry out field investigation and sampling of environmental biological samples for 4 CT rooms. Then the samples were detected for the nucleic acid of novel coronavirus. The results of radiodiagnostic workplace overall arrangement, infection prevention and the nucleic acid testing were analyzed, and the biological safety reliability and risk point were evaluated.Results:The indicators of imaging performance and radiation protection for 8 CT scanners in emergency hospitals could meet the requirements of national standards.Each of 2 makeshift hospitals had 3 CT rooms with the area of 38.8 m 2 and 4 mm Pb equivalent thickness of protective shielding. The CT rooms in module hospital and brick pattern hospital were 20.0 m 2, and 35.8 m 2 in areas, with 4 mm Pb equivalent and 3 mm Pb equivalent thickness of protection shielding, respectively. The 8 radiological diagnostic workplaces of the emergency hospitals were designed and constructed based on " three zones with two passage ways" . The result of the nucleic acid test indicated that the positive samples were found at the multiple sites such as scanning bed, internal of gantry and ground touched by patients in CT scanning room. The areas such as console panel and ground were risked of pollution by the virus infected hands and feet of radiographers. In addition, the similar positive samples were found in the areas in scanning room with no touch of patients, such as observation window and air outlet. Conclusions:8 CT scanners and rooms in 4 emergency hospitals basically meet the requirements of imaging performance and radiation protection. The disinfection of COVID-19 radiodiagnostic workplace should be standardized.
Increased recovery of salmonellae from environmental samples enriched with buffered peptone water

Abstract:The incidence and persistence of salmonellae in weather pools on the top of Stone Mountain were investigated with lactose and buffered peptone water used as pre-enrichment broths. A total of 162 samples were collected from 16 weather pools over a 3-month period. The use of buffered peptone water increased the recovery of salmonellae by approximately 25%. The combined use of direct enrichment in tetrathionate broth containing brilliant green dye and pre-enrichment in buffered peptone water followed by enrichment in tetrathionate broth made it possible to detect all 37 of the contaminated samples. All of the isolates were Salmonella bareilly, the only serotype recovered in a previous study. All but one of the isolations were made from moist or wet samples. S. bareilly was isolated from rabbit dung and litter collected near the weather pools, but attempts to trap rabbits for study were unsuccessful. Random samples taken along a side of the mountain yielded S. bareilly in weather pools within the upper third of the mountain; below this level, S. weslaco and S. memphis were recovered, but not S. bareilly.

Abstract:Background The COVID-19 pandemic has highlighted the need for effective infection control in outpatient healthcare settings. Germicidal Ultraviolet-C (GUV) light, known for inactivating microorganisms by damaging their DNA or RNA, offers a potential solution. This study examines the efficacy of GUV air disinfection systems in real-world outpatient environments. Methods We deployed upper-room and far-UV GUV fixtures in three outpatient facilities, assessing their impact on bacterial loads through air and surface sampling and bioindicator tests. Occupancy was also monitored. Results While manual air and surface sampling did not show a significant difference in bacterial loads between control and UV-C-treated groups, bioindicator tests demonstrated a high level of spore inactivation (up to 99.7% for upper-room GUV and 96.26% for far-UV). Occupancy levels did not significantly influence these outcomes. Discussion The discrepancy between bioindicator efficacy and environmental sampling results suggests limitations in the latter's ability to accurately capture environmental bioburden. Bioindicators proved to be reliable for in-situ validation of UV-C surface disinfection. Conclusions Bioindicators are effective for validating GUV surface disinfection efficacy in healthcare settings, though further research is needed to optimize environmental sampling methods for assessing GUV's impact on real-world bacterial loads.

Systematical Investigations on Disinfection Effectiveness of Far-UVC (222 Nm) Irradiation: from Laboratory Study to Field Tests

Abstract:The spread of pathogenic microorganisms in public spaces poses a great threat to human health. Far-UVC irradiation is regarded as an efficient method for inactivating most pathogenic microorganisms. Nevertheless, most of the studies on it have been done in laboratory, and there is still little knowledge about the disinfection effectiveness of far-UVC irradiation in real life. Here, systematic investigations were conducted to shed light on the disinfection effectiveness of far-UVC irradiation with laboratory studies and field tests in a real operating public lift cabin. The results of the laboratory study demonstrated that far-UVC irradiation can markedly promote the inactivation of the selected 7 types of microorganisms (including bacteria, viruses and fungus species) on steel surface compared to that on the glass surface, due to the synergistic effect of direct and reflected far-UVC light. This improvement was identified in the field test by employing the far-UVC device for disinfecting the surfaces of the lift cabin which are mostly made of stainless steel. Furthermore, the long-term field test of about 4 months also suggested that both environmental temperature and relative humidity (RH) affect the far-UVC disinfection effectiveness, where the best inactivation efficiency (99.9%) on the surface of lift button and handrail was achieved in the environmental temperature range of 23.5-25.5°C and the RH range of 60-70% at a low UVC dose of 5.2 mJ/cm2. In addition, >99.9% of airborne bacteria were inactivated in a chamber at a far-UVC dose of 15 mJ/cm2, showing a better performance of far-UVC for air disinfection compared to other technologies. This study systematically investigates the performance of far-UVC irradiation for surface and air disinfection in an actual environment, which provides helpful guidance for controlling the spread of pathogenic microorganisms in public spaces, especially in the ongoing COVID-19 pandemic.
Spatial Patterns of Viable Spore Deposition of Gibberella zeae in Wheat Fields.

Abstract:ABSTRACT An increased understanding of the epidemiology of Gibberella zeae will contribute to a rational and informed approach to the management of Fusarium head blight (FHB). An integral phase of the FHB cycle is the deposition of airborne spores, yet there is no information available on the spatial pattern of spore deposition of G. zeae above wheat canopies. We examined spatial patterns of viable spore deposition of G. zeae over rotational (lacking cereal debris) wheat fields in New York in 2002 and 2004. Viable, airborne spores (ascospores and macroconidia) of G. zeae were collected above wheat spikes on petri plates containing a selective medium and the resulting colonies were counted. Spores of G. zeae were collected over a total of 68 field environments (three wheat fields during 54 day and night sample periods over 2 years) from spike emergence to kernel milk stages of local wheat. Spatial patterns of spore deposition were visualized by contour plots of spore counts over entire fields. The spatial pattern of spore deposition was unique for each field environment during each day and night sample period. Spore deposition patterns during individual sample periods were classified by spatial analysis by distance indices (SADIE) statistics and Mantel tests. Both analyses indicated that the majority (93%) of the spore deposition events were random, with the remainder being aggregated. All of the aggregated patterns were observed during the night. Observed patterns of spore deposition were independent of the mean number of viable spores deposited during individual sample periods. The spatial pattern for cumulative spore deposition during anthesis in both years became aggregated over time. Contour maps of daily and cumulative spore deposition could be compared with contour maps of FHB incidence to gain insights into inoculum thresholds and the timing of effective inoculum for infection.

Abstract:OBJECTIVE. Because CT plays an important role in diagnosis, isolation, treatment, and effective evaluation of coronavirus disease (COVID-19), infection prevention and control management of CT examination rooms is important. CONCLUSION. We describe modifications to the CT examination process, strict disinfection of examination rooms, arrangement of waiting areas, and efforts to increase radiographers' awareness of personal protection made at our institution during the COVID-19 outbreak. In addition, we discuss the potential of using artificial intelligence in imaging patients with contagious diseases.

Abstract:BACKGROUND Advancement of urologic instruments through the genitourinary tract is associated with significant axial forces that likely contribute to patient discomfort, even after injection of a local anesthetic, and may lead to mucosal trauma, postprocedural dysuria and hematuria, and increased susceptibility to infection and strictures. Placing an everting urethral sheath prior to instrumentation may decrease these problems. MATERIALS AND METHODS Two 7-cm-long, 5-mm diameter urethral luminal models were created, one with and one without an artificial stricture. We measured the forces generated during advancement of a novel everting access sheath (Cystoglide; Percutaneous Systems, Mountain View, CA) through the models in comparison with a representative cystoscope and a urologic dilator simulating a traditional access sheath. RESULTS The mean force generated during advancement of the everting sheath was significantly less than that of both the representative cystoscope (P<0.01) and the traditional access sheath (P<0.01). This held true for the urethral models both with and without an artificial stricture (P<0.01) and with and without lubrication (P<0.01). CONCLUSIONS This novel introduction sheath markedly decreased the axial forces applied to an artificial urethral luminal wall. It is possible that the clinical use of this technology will decrease the discomfort and potential complications associated with lower urinary-tract endoscopy.

Abstract:Far-Ultraviolet-C (far-UVC) 222 nm was introduced during the Covid-19 pandemic to disinfect the airborne pathogens, particularly SARS-CoV-2. Although far-UVC 222 nm has shown promising results in surface disinfection, its efficacy in air disinfection remains uncertain due to additional variables in air. This study investigated the efficacy of far-UVC 222 nm UVGI in air disinfection experimentally, using total plate count, and input the measured far-UVC intensity into the computational fluid dynamic model to predict infection risk using the Wells-Riley model. Experimental results indicated that after 1 h of far-UVC irradiation, an equivalent far-UVC dosage of 21.31 J/m 2 , airborne bacteria concentration was reduced by 44.4 %–74.1 %, while airborne fungal concentration showed a marginal reduction. In a recirculating air-conditioned room, a 20 W far-UVC fixture installation, referred to as Case 1, demonstrated the highest available dosage of 7.66 % difference compared to Case 2. The use of far-UVC in Case 1 can reduce the risk of infection for the influenza H1N1, SARS-CoV-2, and Mycobacterium tuberculosis by 54.48 %, 60.06 %, and 83.73 %, respectively. The study suggests installing far-UVC near the recirculating air-conditioner supply air jet and away from surrounding walls for optimum effectiveness. These results provide recommendations for alternatives to upper-room UVGI, offering a viable option for continuous room air and surface disinfection.

Abstract:Abstract Background To detect the contamination of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) in the surroundings of coronavirus disease 2019 (COVID-19) patients and to evaluate the effectiveness of regular disinfectants and combinations against SARS-CoV-2 RNA. Methods We sampled the patients’ high contact surfaces in COVID-19 pediatric quarantine wards from April to June 2022. After conducting cleaning procedures using disinfectants, including trichloroisocyanuric acid (TCCA; 500, 1000, and 2000 mg/L), 5% hydrogen peroxide (H 2 O 2 ), 0.5% povidone-iodine (PI), 75% ethanol (EA), 0.2% chlorhexidine gluconate (CHG), 0.2% quaternary ammonia compound (QAC), and five combinations, environmental samples in bathroom were collected at 0, 30 s, 10, 30, and 60 min. All samples were delivered to the medical laboratory for SARS-CoV-2 nucleic acid ( ORF1ab and N ) detection using real-time PCR. Results SARS-CoV-2 RNA was largely detected on surfaces in the COVID-19 quarantine ward and was highest in the floor, bathroom, and bed sheet. The ORF1ab and N genes remained detectable after 60 min of treatment with QAC, PI, EA, and CHG. H 2 O 2 and TCCA2000 completely degraded SARS-CoV-2 RNA in 30 s, which was faster than TCCA1000 (10 min). Clearance of ORF1ab and N by TCCA500 required 10 and 60 min, respectively, whereas combination of TCCA500 with EA or PI destroyed ORF1ab and N faster at 30 s and 30 min, respectively. Conclusion The surroundings of patients with COVID-19 are contaminated by SARS-CoV-2 RNA. Effectiveness of disinfectants and combinations varies, N gene persists longer time than ORF1ab after some disinfection.

Abstract:Abstract Background Ultraviolet (UV) light disinfection following a manual terminal cleaning process for hospital rooms has been proposed as an additional method to reduce the bacterial burden on surfaces. The impact of UV light disinfection and the level of interdependence between the modalities remains unclear. Methods Samples were collected from 5 high touch surfaces from 10 patients room following discharge prior to manual disinfection, following manual disinfection, and following pulsed xenon UV disinfection using Rodac contact plates (total 150 samples). Colonies were identified using MALDI-TOF mass spectrometry. The bacterial colony counts were recorded and analyzed as pathogenic or commensal organisms (based on CDC criteria) to assess the efficacy of the disinfection process. Results Average colony counts for the rooms prior to disinfection, post disinfection, and post UV light were 185.8 CFU +/- SD 280, 43 CFU +/- 121, and 20 CFU +/- 36.7 respectively. The average drop in colony-forming units of the five high touch areas in patient’s rooms can be seen in table 1. Twelve commensal bacterial species were isolated: Bacillus species (sp.), Corynebacterium sp., Enhydrobacter sp., Kocuria sp., Lysinibacillus sp., Macrococcus sp., Micrococcus sp., Paenibacillus sp., Pantoea sp., Psychrobacter sp., Siccibacter sp., Coagulase negative staphylococcus. Seven pathogenic bacteria were isolated: Acinetobacter sp., Brucella sp., Proteus sp., Staphylococcus aureus, Escherichia sp., Enterococcus, and Pseudomonas aeruginosa. Reductions in the predominant bacterial species following disinfection modality are noted in table 2. Table 1: Colony forming units (CFUs) average Pre-disinfection (Pre-Dis), Post Disinfection (Post-Dis), and Post PX-UV Light (PX-UV). Table 2: Sub-analysis of commensal and pathogen isolation Pre-Disinfection (Pre-Dis), Post-Disinfection (Post-Dis), and Post PX-UV light (PX-UV). Conclusion A combination of manual disinfection and UV has shown a notable additional reduction in overall bacterial contamination of the patient rooms, including the majority of high touch areas as compared with manual disinfection alone. No additional reduction in commensal bacteria isolates was noted after UV light, however a further decrease in pathogenic bacteria (Acinetobacter and Enterococcus) was noted. UV light may be considered as an additional room disinfection method to reduce overall bacterial burden and pathogenic bacterial contamination of rooms as a comprehensive strategy to reduce nosocomial infections. Abstract:Since the COVID-19 pandemic, there has been persistent emphasis on the importance of indoor air disinfection and ventilation in isolation units in the hospital environment. Nevertheless, no optimal and concrete disinfection protocol has been proposed to inactivate the viruses as quickly as possible. In this study, we experimentally evaluated various ventilation and disinfection protocols based on the combination of negative-pressure ventilation, ultraviolet (UV) light illumination, and Hypochlorous acid (HOCl) spray against three active virus species in a 3.5 cubic meters isolation unit. This small-size unit has gained attention during the pandemic due to the high demand for compact mobile laboratory systems capable of rapid disease diagnosis. In accordance with the WHO laboratory biosafety guidance, which states that all enclosed units where diagnostic work is conducted must ensure proper ventilation and disinfection activities, we aim to propose virus removal protocols for units compact enough to be installed within a van or deployed outdoor. The results confirmed the superiority (in terms of virus removal rate and time required) of the virus removal methods in the order of UV light, ventilation, and HOCl spray. Ultimately, we propose two optimal protocols: (i) UV light alone for three minutes, and (ii) UV light with ventilation for three minutes, followed by one-minute ventilation only. The time span of three minutes in the latter protocol is based on the clinical practice such that the medical staffs have a sufficient time to process the samples taken in transition to next patient to care.

Abstract:Abstract Prolonged survival of SARS-CoV-2 on environmental surfaces and personal protective equipment (PPE) may lead to these surfaces transmitting disease to others. This article reports the effectiveness of a pulsed xenon ultraviolet (PX-UV) disinfection system in reducing the load of SARS-CoV-2 on hard surfaces and N95 respirators. Chamber slides and N95 respirator material were directly inoculated with SARS-CoV-2 and exposed to different durations of PX-UV disinfection. For hard surfaces, disinfection for 1, 2, and 5 minutes resulted in 3.53 Log 10 , >4.54 Log 10 , and >4.12 Log 10 reductions in viral load, respectively. For N95 respirators, disinfection for 5 minutes resulted in >4.79 Log 10 reduction in viral load. We found that PX-UV significantly reduces SARS-CoV-2 on hard surfaces and N95 respirators. With the potential to rapidly disinfectant environmental surfaces and N95 respirators, PX-UV devices are a promising technology for the reduction of environmental and PPE bioburden and to enhance both HCW and patient safety by reducing the risk of exposure to SARS-CoV-2.

Abstract:Lung cancer is one of malignant tumors with the highest morbidity and mortality in the world. At present, research of early diagnosis, treatment, prognosis, and metastasis associated biomarkers is most active. This article reviewed the research status of lung cancer biomarkers and analyzed the funding situation in the field of lung cancer markers in recent 10 years in China and abroad, to provide a reference for the future basic and clinical translational research of lung cancer biomarkers.

Abstract:[Objective] To a simple and high-performance disinfection method for air disinfection of direct rooming-in. [Methods] Compared three methods(method A: chlorine disinfectant,ventilation;method B: chlorine disinfectant,electronic germicidal lamp;method C: electronic germicidal lamp,ventilation) . [Results] Method A and B could make the bacteria in the air of direct rooming-in descend to the qualified level. However,the poorer effect was found when with method C. [Conclusion] Method A and B both have the good disinfection effect. And method A is more simple and easy.

Abstract:Objective: To evaluate the disinfection effect of high-energy pulse ultraviolet disinfection equipment in medical institution settings. Methods: The disinfection effect was evaluated through field tests and laboratory tests. Among them, 135 high-frequency contact points were selected from nine departments in the field test. Samples were collected before and after disinfection, and the disinfection effects of 75% alcohol wipes wiping disinfection, high-energy pulse ultraviolet disinfection robot disinfection and high-energy pulse ultraviolet handheld disinfection instrument were compared. In the laboratory test, 30 infected areas of the simulated test table were exposed to vertical ultraviolet irradiation and the bacterial-killing rate before and after disinfection was calculated. Results: In the field test, the bacteria-killing rates of 75% alcohol wipes, high-energy pulse ultraviolet disinfection robot and high-energy pulse ultraviolet handheld disinfection instrument were 94.99%, 91.53% and 95.94%, respectively, and the difference was statistically significant. The disinfection effect of the high-energy pulse ultraviolet handheld disinfection instrument was better than that of the high-energy pulse ultraviolet disinfection robot (P values <0.05). In the laboratory test, the killing log value of Staphylococcus aureus and Escherichia coli on the carrier were both greater than 3.00. In the simulated field test, the killing log value of Staphylococcus aureus on the surface samples were 4.99. Conclusion: Both the high-energy pulse ultraviolet handheld disinfection instrument and the high-energy pulse ultraviolet disinfection robot have good disinfection effects, which are similar to the disinfection effects of conventional 75% alcohol wipes.
