Abstract:Objective To investigate whether cardiac magnetic resonance (CMR) can assess the progression and regression of myocardial fibrosis in patients with aortic stenosis (AS). Methods Sixty-three patients with aortic stenosis were followed up for 2 years with serial CMR and echocardiography: 28 patients (63±13 years, 68% male, 50% asymptomatic severe AS) did not undergo intervention (natural history cohort) and 35 patients (67±8 years, 73% male, 83% symptomatic severe AS) underwent aortic valve replacement (AVR) within the first year (AVR cohort). Replacement and diffuse myocardial fibrosis were assessed using the late gadolinium enhancement (LGE) and T1 mapping techniques respectively. Annualised change was calculated for all measures. Results In the natural history cohort, left ventricular mass index (LVMi) increased over time (6%±1%, p<0.0001) and was accompanied by a fall in longitudinal systolic function (−4%±2% p=0.03). The indexed extracellular volume (iECV, a measure of diffuse fibrosis) also increased over time (7%±2%, p<0.0001). However, no changes were observed in native T1, post-contrast T1 or extracellular volume (ECV) fraction on serial imaging. Mid-wall LGE was observed in nine patients (32%). Absolute LGE mass increased by 3.8±0.8g (p<0.0001) amongst all the patients with baseline mid-wall LGE. Four patients with mid-wall LGE at baseline developed new areas of LGE in different myocardial segments. No patients without pre-existing mid-wall LGE developed new LGE during follow up. The AVR cohort displayed a fall in LVMi following surgery (−15%±2%, p<0.0001) mirrored by a reduction in diffuse fibrosis (iECV, −9%±2%, p<0.0001). The ECV fraction however was observed to increase (5% [2, 11], p<0.0001), with no change in native T1. Mid-wall LGE was present in 9 patients (26%). No patient went on to develop new LGE nor did existing LGE resolve in any patient and LGE mass did not change. Longitudinal systolic function increased following AVR (16%±6%, p=0.01). Conclusion Changes in diffuse fibrosis can be tracked using iECV and increase with time in patients with AS alongside LV mass and replacement fibrosis. Following AVR there is a fall in LV mass driven predominantly by regression of cellular hypertrophy with more gradual reduction in diffuse fibrosis burden but no change in replacement fibrosis. Abstract:Background Bicuspid aortic valve (BAV) is a common congenital heart disease and a major risk factor for aortic stenosis (AS). Myocardial remodelling response to AS plays an important prognostic role1. Cardiovascular magnetic resonance (CMR) can evaluate myocardial structure, function and perfusion in AS. Phosphorus magnetic resonance spectroscopy (31P-MRS) can also assess myocardial energetic consequences of AS2. Purpose Assess differences in myocardial remodelling between tricuspid aortic valve (TAV) and BAV-associated severe-AS, and their recovery 6 months post aortic valve replacement (AVR). Methods Eighty severe-AS patients (70[69-72]years) undergoing AVR (40 TAV, 40 BAV) and 21 demographically-matched controls were recruited. AS groups were matched for demographics, AS severity, surgical risk scores and comorbidities. One month before and 6 months post-AVR, patients underwent CMR and 31P-MRS for measuring left ventricular (LV) mass, concentricity-index (LV mass to LV end-diastolic volume ratio), global longitudinal shortening (GLS), rest and adenosine-stress myocardial blood flow (MBF), myocardial perfusion reserve (MPR) and energetics-index phosphocreatine to ATP ratio (PCr/ATP). Results Pre-AVR, there was no significant differences between the three cohorts when assessing LV ejection fraction, indexed LV end-diastolic volume and indexed end-systolic volume. Pre-AVR, both AS groups showed higher LV-concentricity (controls:0.53[0.51,0.60], TAV:0.95[0.82,1.07], BAV:0.92[0.82,1.08]g/ml;P<0.0001), higher mass-index (controls:45[39,50], TAV:73[67,78], BAV:74[68,80]g/m2;P<0.0001), impaired GLS (controls:18.8[16.5,19.5], TAV:12.6[11.3,13.9], BAV:13.6[10.9,16.6]%;P<0.0001). Reductions in both AS groups were also seen in PCr/ATP (controls:2.19[1.85,2.63], TAV:1.64[1.39,1.85], BAV:1.85[1.60,2.10];P=0.0004), stress-MBF (controls:2.16[1.88,2.27], TAV:1.35 [1.16,1.56], BAV:1.36[1.00,1.66]ml/min/g;P<0.0001), and MPR (controls:3.38[2.65,3.92], TAV:2.04 [1.78,2.46], BAV:1.83[1.58,2.78]ml/min/g;P<0.0001) against controls, with no significant differences between TAV and BAV. No significant difference was seen in the proportion of patients with TAV and BAV undergoing transcatheter or surgical AVR (table1). Post-AVR, the TAV and BAV groups experienced similar patterns of reverse LV-remodelling, with no significant differences in LV-concentricity (TAV:0.86[0.71,0.98], BAV:0.81[0.65,0.96];P=0.41), LV-mass-index (TAV:60[54,65], BAV:61[55,66];P=0.81), GLS (TAV 15.4[13.6,17.9], BAV 16.3[12.8,18.1];P=0.82. Perfusion and cardiac energetic improvements were also similar between the AS cohorts: PCr/ATP (TAV:1.96[1.68,2.26], BAV:1.96[1.79,2.13];P=0.91), stress-MBF (TAV:1.63[1.37,1.97], BAV:1.56[1.25,1.97];P=0.86), and MPR (TAV:2.64[1.91,3.26], BAV:2.55[2.07,3.28];P=0.70). Conclusions Patients with severe AS irrespective of BAV or TAV morphology, show a similar myocardial phenotype pre-AVR, with similar magnitudes of reverse remodelling post-AVR.

Abstract:Background Patients with moderate or asymptomatic severe aortic stenosis (AS) are at risk of cardiovascular events. Objectives The authors investigated the utility of cardiac magnetic resonance (CMR) to identify drivers of outcome in patients with moderate or asymptomatic severe AS. Methods A prospective, international, multicenter cohort (n = 457) of patients with moderate (aortic valve area [AVA]: 1.0-1.5 cm 2 ) or asymptomatic severe AS (AVA ≤1.0 cm 2 and NYHA functional class I-II) patients underwent CMR. Diffuse interstitial fibrosis and scar in the myocardium were analyzed with extracellular volume fraction (ECV%) and late gadolinium enhancement (LGE). The outcome was a composite of mortality and heart failure admission. Results Median ECV% was 26.6% (IQR: 24.4%-29.9%), and LGE was present in 31.5% (median 0.8%; IQR: 0.1%-1.7%). Greater AS severity was associated with greater left ventricular mass and diastolic dysfunction, but not with ECV% or LGE. During a median 5.7 years of follow-up, 83 events occurred. Patients with events had higher ECV% (median ECV% 26.3% vs 28.2%; P = 0.003). Patients in the highest ECV% tertiles (ECV% >28.6%) had worse outcomes both in the entire cohort and in those with NYHA functional class I moderate or severe AS, and ECV% was independently associated with outcome (adjusted HR: 1.05; P = 0.039). The ECV% had significant incremental prognostic value when added to parameters of AS severity and cardiac function, comorbidities, aortic valve replacement, and LGE ( P < 0.05). Conclusions Increased diffuse interstitial fibrosis of the myocardium is associated with poor outcomes in patients with moderate and asymptomatic severe AS and can help identify those who require closer surveillance for adverse outcomes.

Abstract:Left ventricular (LV) hypertrophy is a common finding in patients with severe aortic stenosis (AS). Cardiac magnetic resonance (CMR) is the gold-standard technique to evaluate LV remodeling. Our aim was to assess the prevalence and describe the patterns of LV adaptation in AS patients before and after surgical aortic valve replacement (AVR). Prospective study of 130 consecutive patients (71y [IQR 68–77y], 48% men) with severe AS, referred for surgical AVR. Patterns of LV remodeling were assessed by CMR. Besides normal LV ventricular structure, four other patterns were considered: concentric remodeling, concentric hypertrophy, eccentric hypertrophy, and adverse remodeling. At baseline CMR study: mean LV indexed mass: 81.8 ± 26.7 g/m 2 ; mean end-diastolic LV indexed volume: 85.7 ± 23.1 mL/m 2 and median geometric remodeling ratio: 0.96 g/mL [IQR 0.82–1.08 g/mL]. LV hypertrophy occurred in 49% of subjects (concentric 44%; eccentric 5%). Both normal LV structure and concentric remodeling had a prevalence of 25% among the cohort; one patient had an adverse remodeling pattern. Asymmetric LV wall thickening was present in 55% of the patients, with predominant septal involvement. AVR was performed in 119 patients. At 3–6 months after AVR, LV remodeling changed to: normal ventricular geometry in 60%, concentric remodeling in 27%, concentric hypertrophy in 10%, eccentric hypertrophy in 3% and adverse remodeling (one patient). Indexes of AS severity, LV systolic and diastolic function and NT-proBNP were significantly different among the distinct patterns of remodeling. Several distinct patterns of LV remodelling beyond concentric hypertrophy occur in patients with classical severe AS. Asymmetric hypertrophy is a common finding and LV response after AVR is diverse.

Adverse prognosis associated with asymmetric myocardial thickening in aortic stenosis

Abstract:Aims: Asymmetric wall thickening has been described in patients with aortic stenosis. However, it remains poorly characterized and its prognostic implications are unclear. We hypothesized this pattern of adaptation is associated with advanced remodelling, left ventricular decompenzation, and a poor prognosis. Methods and results: In a prospective observational cohort study, 166 patients with aortic stenosis (age 69, 69% males, mean aortic valve area 1.0 ± 0.4 cm2) and 37 age and sex-matched healthy volunteers underwent phenotypic characterization with comprehensive clinical, imaging, and biomarker evaluation. Asymmetric wall thickening on both echocardiography and cardiovascular magnetic resonance was defined as regional wall thickening ≥ 13 mm and > 1.5-fold the thickness of the opposing myocardial segment. Although no control subject had asymmetric wall thickening, it was observed in 26% (n = 43) of patients with aortic stenosis using magnetic resonance and 17% (n = 29) using echocardiography. Despite similar demographics, co-morbidities, valve narrowing, myocardial hypertrophy, and fibrosis, patients with asymmetric wall thickening had increased cardiac troponin I and brain natriuretic peptide concentrations (both P < 0.001). Over 28 [22, 33] months of follow-up, asymmetric wall thickening was an independent predictor of aortic valve replacement (AVR) or death whether detected by magnetic resonance [hazard ratio (HR) = 2.15; 95% confidence interval (CI) 1.29-3.59; P = 0.003] or echocardiography (HR = 1.79; 95% CI 1.08-3.69; P = 0.021). Conclusion: Asymmetric wall thickening is common in aortic stenosis and is associated with increased myocardial injury, left ventricular decompenzation, and adverse events. Background Recently, a new staging of patients with asymptomatic moderate to severe aortic stenosis (AS) based on extra-aortic valve cardiac damage has been described to stratify the prognostic of both, asymptomatic and symptomatic patients. Purpose To test the ability of this imaging based staging system to predict the development of symptoms in patients with moderate to severe asymptomatic AS. Methods We analyzed a retrospective cohort of 96 patients with at least moderate AS who were systematically studied with echocardiography and cardiac magnetic resonance imaging. Patients were classified according to the staging classification of extra-aortic valve damage: no cardiac damage (Stage 0), left ventricular (LV) damage: LV mass index >95 g/m2 in women and >115 g/m2 in men and or E/E ́ratio >14 and/or LV ejection fraction <50% (Stage 1), mitral valve or left atrial (LA) damage: LA volume >34 ml/m2, and/or atrial fibrillation and/or ≥ moderate mitral regurgitation (Stage 2), tricuspid valve or pulmonary vasculature damage: systolic pulmonary arterial pressure ≥60 mm Hg and/or moderate or greater tricuspid regurgitation (Stage 3), right ventricular damage: tricuspid annulus systolic velocity S ́< 9.5 cm/s, TAPSE < 17 mm, and/or RV ejection fraction < 50% (Stage 4). Given the small number of patients in Stages 3 and 4, patients were merged together in a single group. The primary endpoint was the development of symptoms during a follow-up of 5 years, regardless of whether the patient underwent aortic valve replacement. Kaplan-Meier curves and log-rank test of the time to development of symptoms data were used. Results At baseline patients had a mean age of 70.9 years old and 71.9% were male. Most of patients had hypertension (70.8%) and dyslipidemia (55.2%). 20.8% had diabetes mellitus and only 10.4% had previous coronary artery disease. The mean transvalvular gradient was 39 ± 14 mmHg and the mean aortic valve area was 1.04 ± 0.24 cm2. According to the staging scheme, 37 patients were classified in Stage 0 (38.5%), 32 in Stage 1 (33.3%), 23 in Stage 2 (24%) and 4 in the group of Stage 3-4 (4.2%). During 5 years of follow-up, 44 patients developed symptoms (33 dyspnea, 18 angina and 5 syncope), up to 40% of patients underwent aortic valve replacement and 20 patients died (20.8%), 7 of them due to cardiovascular causes. According to the staging scheme, the proportion of patients who remained asymptomatic at 5 years were: 50.3% in Stage 0, 45.8% in Stage 1, 27.3% in Stage 2, 0% in Stage 3-4 (p = 0.002). Every patient in stages 3 and 4 had developed symptoms by the third year while most patients in stage 0 remained asymptomatic within the first two years (figure 1). Conclusions Up to 45% of asymptomatic patients with moderate to severe AS will develop symptoms within a 5-year period. Symptoms are developed significantly earlier with incremental staging. The primary aim of this study was to evaluate left ventricular (LV) remodeling and its progression in asymptomatic patients with CAR and preserved LV ejection fraction (LVEF) by three-dimensional speckle-tracking echocardiography (3D-STE). The secondary aim was to identify the effect of management strategies on LV remodeling in severe CAR.135 subjects and 41 controls were enrolled, the subjects were divided according to regurgitation degree: mild (n = 48), moderate (n = 40), severe (n = 47). A routine follow-up was not possible in 13 patients for the severe CAR group, the remaining 34 patients were divided into three groups based on treatment: surgical (n = 13), drug (n = 11), and untreated (n = 10), and followed for 2.1 ± 0.37 years. All subjects underwent 3D-STE at baseline and follow-up, while 20 patients with CAR also underwent baseline two-dimensional speckle tracking echocardiography (2D-STE) and feature-tracking cardiovascular magnetic resonance imaging (CMR-FT). Volumetric and strain parameters were acquired.Compared with the global circumferential strain (GCS) derived from 2D-STE and CMR-FT, the 3D GCS was the largest (P < 0.001); however, no significant differences in the volumetric parameters, global longitudinal strain (GLS), and global radial strain (GRS) were identified at baseline. GLS, GRS, torsion, apical rotation, and twist were worse in the severe group (P < 0.05). During follow-up, LV volumetric indexes and sphericity indexes increased, while GLS, apical rotation, and twist worsened (P < 0.05) in the untreated group. In the surgical group, LV volumetric and sphericity indexes decreased, while GLS and GRS improved (P < 0.05). In the drug group, LV volumetric indexes increased, while LVEF, GLS, and GRS worsened (P < 0.05).3D-STE may be a reliable tool to monitor progression of ventricular remodeling in CAR. Abstract:Animals were orally immunized with horseradish peroxidase and bovine serum albumin, and absorption of these antigens was studied. In comparison with controls, a consistent and significant decrease in peroxidase uptake was noted in both germ-free and conventional rats immunized with peroxidase; a similar decrease in serum albumin uptake was also noted in animals immunized with serum albumin. There was no difference in the uptake of an unrelated macromolecule. These observations suggest that local immunization interferes specifically with the intestinal uptake of macromolecular antigens.

Abstract:Background The timing of surgery for aortic stenosis (AS) is imperfect, and the management of moderate AS and asymptomatic severe AS is still challenging. Myocardial fibrosis (MF) is the main pathological basis of cardiac decompensation in patients with AS and can be detected by cardiovascular magnetic resonance (CMR). The aim of this study was to evaluate the prognostic value of MF measured by CMR in patients with AS, which can provide a reference for the timing of aortic valve replacement (AVR). Methods We searched Medline, Embase, and Web of Science to include all studies that investigated the prognostic value of CMR in patients with AS. The search deadline is March 31, 2021. The pooled relative risk (RR) or hazard ratio (HR) and 95% confidence intervals (CI) of the biomarkers including late gadolinium enhancement (LGE), Native T1 or extracellular volume (ECV) were calculated to evaluate the prognostic value. Results 13 studies and 2,430 patients with AS were included in this study, the mean or medium follow-up duration for each study was ranged from 6 to 67.2 months. Meta-analysis showed the presence of LGE was associated with an increased risk for all-cause mortality (pooled RR: 2.14, 95% CI: 1.67–2.74, P < 0.001), cardiac mortality (pooled RR: 3.50, 95% CI: 2.32–5.30, P < 0.001), and major adverse cardiovascular events (MACEs) (pooled RR: 1.649, 95% CI: 1.23–2.22, P = 0.001). Native T1 was significantly associated with MACEs (pooled RR: 2.23, 95% CI: 1.00–4.95; P = 0.049), and higher ECV was associated with a higher risk of cardiovascular events (pooled HR: 1.69, 95% CI: 1.11–2.58; P = 0.014). Conclusion The use of CMR to detect MF has a good prognostic value in patients with AS. LGE, Native T1 and ECV measured by CMR can contribute to risk stratification of AS, thereby helping to optimize the timing of AVR.

Abstract:Abstract Introduction: Patients with paradoxical low-flow (LF) severe aortic stenosis (AS) despite preserved left ventricular ejection fraction (LVEF) appear distinct from normal-flow (NF) patients, showing worse prognosis, more concentric hypertrophy and smaller left ventricular (LV) cavities. The left ventricular remodelling index (LVRI) has been demonstrated to reliably discriminate between physiologically adapted athlete’s heart and pathological LV remodelling. Methods: We studied patients with index echocardiographic diagnosis of severe AS (aortic valve area <1 cm 2 ) with preserved LVEF (>50%). The LVRI was determined by the ratio of the LV mass to the end-diastolic volume, as previously reported, and was compared between patients with LF and NF AS. Patients were prospectively followed up for at least 3 years, and clinical outcomes were examined in association with LVRI. Results: Of the 450 patients studied, 112 (24.9%) had LF AS. While there were no significant differences in baseline clinical profile between LF and NF patients, LVRI was significantly higher in the LF group. Patients with high LVRI (>1.56 g/mL) had increased all-cause mortality (log-rank 9.18, P = 0.002) and were more likely to be admitted for cardiac failure (log-rank 7.61, P = 0.006) or undergo aortic valve replacement (log-rank 18.4, P < 0.001). After adjusting for the effect of age, hypertension, aortic valve area and mean pressure gradient on multivariate Cox regression, high LVRI remained independently associated with poor clinical outcomes (hazard ratio 1.64, 95% confidence interval 1.19–2.25, P = 0.002). Conclusion: Pathological LV remodelling (increased LVRI) was more common in patients with LF AS, and increased LVRI independently predicts worse clinical outcomes.

Abstract:Introduction: Aortic regurgitation (AR) leads to left ventricular (LV) remodeling by increasing LV load. We recently showed that LV dimensions, widely adopted marker of severe AR, are not reliable predictors of adverse CV outcomes in older patients and female sex. Here we sought to study the effects of chronic severe AR on left atrial (LA) size and function as a prognostic adjunct to LV dimensions. Methods: Adult patients with ≥3+ AR on routine surveillance with serial echocardiograms between 1/2010 and 12/2016 were included in this study. We assessed longitudinal trends of LA volume indexed to body surface area (LAVI) and LA reservoir strain (LAr) stratified by age and sex. Clinical endpoints included the occurrence of heart failure hospitalization, urgent aortic valve surgery, or all-cause mortality and were used to assess the prognostic value of our studied parameters. Results: A total of 525 patients with 1,687 serial echocardiograms conducted over a median (IQR) follow-up duration of 2.0 (1.0-3.6) years were included. As shown in figure 1, we observed a substantial increase in LAVI (1.0 [95% CI: 0.76-1.2] mL/m2 per year of follow-up) and a decrease in LAr (-1.3% [95% CI: -1.6 to -0.92%]) over the study period. There was no significant interaction related to sex or age (p ≥0.17). 210 adverse events were recorded and were significantly predicted by both LA parameters. Optimal discriminatory threshold for LAVI was determined to be 37 mL/m2 and LAr at 35% and were both found to be consistent across categories of sex and age. Conclusions: This study indicates that LA remodeling, unlike LV remodeling, shows a similar rate of progression across different categories of sex and age among patients with chronic severe AR. Furthermore, LA parameters provide an added prognostic value over LV parameters and may therefore be better suited to inform the timing of AR intervention.

Abstract:Background: Cross sectionally measured, elevated arterial stiffness is associated with unfavorable left ventricular (LV) remodeling, suggesting its important role in the pathophysiology of heart failure. However, data linking the degree of arterial stiffness progression with LV remodeling are scarce. Objectives: The purpose of this study was to investigate the association between longitudinal change in arterial stiffness and changes in LV remodeling. Methods: Serial measurements of arterial stiffness by cardio-ankle vascular index (CAVI) were performed in 317 participants without cardiovascular disease and with normal arterial stiffness. LV size, mass, and function were assessed by transthoracic echocardiography and including LV global longitudinal strain (LVGLS) by speckle-tracking and tissue Doppler velocity (e') of the mitral annulus (diastolic function). Results: During a median follow-up of 26.8 mo, there was a significant increase in CAVI (P < 0.001). Generalized estimating equation analyses showed that longitudinal increase in CAVI was associated with impaired LVGLS (estimate 0.46, 95% CI: 0.11-0.82; P = 0.010) after adjustment for demographics and baseline cardiovascular factors, but not with changes of LV mass index and e' velocity. When controlling for longitudinal change of covariates, CAVI progression remained associated with change in LVGLS (estimate 0.50, 95% CI: 0.16-0.85; P = 0.004). In sex stratified analysis, progression of CAVI was significantly associated with LVGLS deterioration only in women (estimate 0.92, 95% CI: 0.27-1.58; P = 0.006). Conclusions: Longitudinal increase in arterial stiffness is associated with deterioration in LVGLS. Vascular-ventricular coupling plays an important role in the progressive decline in ventricular function even at an early, subclinical stage.

Abstract:Circulation: Cardiovascular Imaging, Volume 17, Issue 8, Page e016489, August 1, 2024. BACKGROUND:Left ventricular (LV) hypertrophy occurs in both aortic stenosis (AS) and systemic hypertension (HTN) in response to wall stress. However, differentiation of hypertrophy due to these 2 etiologies is lacking. The aim was to study the 3-dimensional geometric remodeling pattern in severe AS pre- and postsurgical aortic valve replacement and to compare with HTN and healthy controls.METHODS:Ninety-one subjects (36 severe AS, 19 HTN, and 36 healthy controls) underwent cine cardiac magnetic resonance. Cardiac magnetic resonance was repeated 8 months post-aortic valve replacement (n=18). Principal component analysis was performed on the 3-dimensional meshes reconstructed from 109 cardiac magnetic resonance scans of 91 subjects at end-diastole. Principal component analysis modes were compared across experimental groups together with conventional metrics of shape, strain, and scar.RESULTS:A unique AS signature was identified by wall thickness linked to a LV left-right axis shift and a decrease in short-axis eccentricity. HTN was uniquely linked to increased septal thickness. Combining these 3 features had good discriminative ability between AS and HTN (area under the curve, 0.792). The LV left-right axis shift was not reversible post-aortic valve replacement, did not associate with strain, age, or sex, and was predictive of postoperative LV mass regression (R2=0.339,P=0.014).CONCLUSIONS:Unique remodeling signatures might differentiate the etiology of LV hypertrophy. Preliminary findings suggest that LV axis shift is characteristic in AS, is not reversible post-aortic valve replacement, predicts mass regression, and may be interpreted to be an adaptive mechanism.

Abstract:Background There are significant sex and age differences in left ventricular (LV) remodeling that may lead to disparity in outcomes when used to inform the timing of aortic regurgitation (AR) intervention. Objectives The aim of this study was to examine whether left atrial (LA) parameters might represent better criteria than LV parameters to inform the timing of AR intervention. Methods Using data on patients with moderate to severe or severe AR with serial echocardiography (2010-2016), the longitudinal trends in left atrial volume index (LAVI) and left atrial reservoir strain (LAr) were evaluated by sex and age. The incremental utility of these parameters in predicting adverse events over LV parameters was also determined. Results In 525 patients (25.7% women) with 1,687 echocardiograms over a median follow-up period of 2.0 years (Q1-Q3: 1.0-3.6 years), there was significant increase in LAVI (1.0 mL/m 2 per year [95% CI: 0.76-1.2 mL/m 2 per year]) and decrease in LAr (−1.3% per year [95% CI: −1.6% to −0.92%]), without a significant interaction by sex or age category ( P for interaction ≥ 0.17). In addition, both LAVI and LAr were significant predictors of adverse events independent of LV parameters. The optimal discriminatory thresholds were 37 mL/m 2 for LAVI and 35% for LAr. These thresholds were similar across categories of sex and age. Within the relatively short-term follow-up, surgery was associated with survival benefit among patients with LAVI ≥37 mL/m 2 (HR: 0.33 [95% CI: 0.15-0.72]; P = 0.006) but was not statistically significant among patients with LAVI <37 mL/m 2 (HR: 0.46 [95% CI: 0.18-1.17]; P = 0.09). Similarly, surgery was associated with survival for the subgroup with LAr ≤35% but not among those with LAr >35%. Conclusions Unlike LV remodeling, LA remodeling demonstrates a similar rate of progression between categories of sex and age among patients with AR. In addition, LA parameters provide incremental prognostic value over LV parameters.

Abstract:The possibility of using prerecorded words and 2-sec. delayed speech feedback as reinforcement in the reconditioning of intelligible verbal responses in a chronic, neologistic schizophrenic patient was investigated. A test situation was devised which involved daily presentation of 100 stimulus word-picture cards to elicit verbal responses. The results indicated that both singly presented prerecorded words and delayed speech feedback apparently strengthened correct and intelligible verbal responding.

Abstract:Reverse left ventricular (LV) remodeling after aortic valve replacement (AVR), in patients with aortic stenosis, is well-documented as an important prognostic factor. With this systematic review and meta-analysis, we aimed to characterize the response of the unloaded LV after AVR. We searched on MEDLINE/PubMed and Web of Science for studies reporting echocardiographic findings before and at least 1 month after AVR for the treatment of aortic stenosis. In total, 1,836 studies were identified and 1,098 were screened for inclusion. The main factors of interest were structural and dynamic measures of the LV and aortic valve. We performed a random-effects meta-analysis to compute standardized mean differences (SMD) between follow-up and baseline values for each outcome. Twenty-seven studies met the eligibility criteria, yielding 11,751 patients. AVR resulted in reduced mean aortic gradient (SMD: - 38.23 mmHg, 95% CI: - 39.88 to - 36.58 , I 2 = 92 % ), LV mass (SMD: - 37.24 g, 95% CI: - 49.31 to - 25.18 , I 2 = 96 % ), end-diastolic LV diameter (SMD: - 1.78 mm, 95% CI: - 2.80 to - 0.76 , I 2 = 96 % ), end-diastolic LV volume (SMD: - 1.6 ml, 95% CI: - 6.68 to 3.51, I 2 = 91 % ), increased effective aortic valve area (SMD: 1.10 cm2, 95% CI: 1.01 to 1.20, I 2 = 98 % ), and LV ejection fraction (SMD: 2.35%, 95% CI: 1.31 to 3.40%, I 2 = 94.1 % ). Our results characterize the extent to which reverse remodeling is expected to occur after AVR. Notably, in our study, reverse remodeling was documented as soon as 1 month after AVR.

Aortic stenosis (AS) stands as the most common valvular heart disease in developed countries and is characterized by progressive narrowing of the aortic valve orifice resulting in elevated transvalvular flow resistance, left ventricular hypertrophy, and progressive increased risk of heart failure and sudden death. This narrative review explores clinical challenges and evolving perspectives in moderate AS, where discrepancies between aortic valve area and pressure gradient measurements may pose diagnostic and therapeutic quandaries. Transthoracic echocardiography is the first‐line imaging modality for AS evaluation, yet cases of discordance may require the application of ancillary noninvasive diagnostic modalities. This review underscores the importance of accurate grading of AS severity, especially in low‐gradient phenotypes, emphasizing the need for vigilant follow‐up. Current clinical guidelines primarily recommend aortic valve replacement for severe AS, potentially overlooking latent risks in moderate disease stages. The noninvasive multimodality imaging approach—including echocardiography, cardiac magnetic resonance, computed tomography, and nuclear techniques—provides unique insights into adaptive and maladaptive cardiac remodeling in AS and offers a promising avenue to deliver precise indications and exact timing for intervention in moderate AS phenotypes and asymptomatic patients, potentially improving long‐term outcomes. Nevertheless, what we may have gleaned from a large amount of observational data is still insufficient to build a robust framework for clinical decision‐making in moderate AS. Future research will prioritize randomized clinical trials designed to weigh the benefits and risks of preemptive aortic valve replacement in the management of moderate AS, as directed by specific imaging and nonimaging biomarkers.

Predictors of LV reverse remodeling after SAVR: insights from a prospective TTE and CMR study in patients with severe aortic stenosis

Abstract:Background Left ventricular (LV) remodeling in patients with severe aortic stenosis (AS) is believed to be reversible after pressure overload relieve, as provided by both surgical (SAVR) or transcatheter aortic valve replacement. However, LV reverse remodeling (RR) is far from uniform in patients with AS and the same clinical indication for treatment. Aim To determine the prevalence and imaging predictors of LV RR after SAVR in patients with severe symptomatic AS. Methods single-center, prospective cohort study enrolling 119 patients with severe symptomatic AS (age 71±8y, 49% male; mean transaortic gradient 62±18mmHg, mean indexed aortic valve area (AVAi) 0.40±0.10 cm2/m2, mean LV ejection fraction by TTE 58±9%), with no previous history of ischemic cardiomyopathy, undergoing SAVR between 2019 and 2022. All patients underwent serial imaging assessment beyond clinical characterization (transthoracic echocardiogram – TTE, and 1.5T cardiac magnetic resonance - CMR) prior to surgery and at the 3rd to 6th month post-SAVR. Endomyocardial biopsy (EMB) for myocardial fibrosis quantification (Masson ́s Trichrome histochemistry) was performed during SAVR. LV RR was defined when in presence of at least one of the imaging criteria: > 15% reduction in LV end-diastolic indexed volume (LVEDV) by CMR; > 15% reduction in LV indexed mass (LVM) by CMR; > 15% reduction in LV geometric remodeling by CMR; >10% increase in LV ejection fraction (LVEF) either from TTE or CMR; > 50% increase in global longitudinal strain (GLS) at TTE. Clinical, imaging and histology derived data were compared in patients with post-SAVR LV RR and for each of the defined criteria. Results 107 patients (90%) met at least one criteria of LV RR. Morphological criteria were more prevalent than LV functional improvement after SAVR (Figure 1). In patients who met at least one criteria of LV RR, no differences were found between NT- proBNP, LV tissue characterization (by CMR) or myocardial fibrosis at EMB. Yet, those with LV RR had significantly smaller preoperative AVAi (0.4±0.9 vs 0.5±0.9 cm2/m2, p=0.007) and higher mean valvular gradients (62±18 vs 49±12 mmHg, p=0.017). Considering individual LV RR criteria, patients with higher NT-proBNP levels met LVEF and morphological (LVEDV and LVM) LV RR criteria (p=0.027, p=0.016 and p= 0.003, respectively). Patients with higher % of late gadolinium enhancement on pre-operative CMR [7.8 (IQR 2.5-11.3) vs. 1.6 (IQR 0-5.2)%, p=0.017] had significant GLS improvement after SAVR. Conclusion In a cohort of patients with classical severe, symptomatic AS, LV RR is common after SAVR, occurring in almost 9 out of every 10 patients. Both indexes of worse valve narrowing and pre-operative LV adaptation are associated with favorable RR after surgery. This underlines the benefits of a timely intervention even in patients with more advanced disease.

Abstract:Background: Aortic stenosis (AS) causes left ventricular (LV) pressure overload, leading to adverse LV remodeling and dysfunction. Identifying early subclinical markers of LV dysfunction in patients with significant AS is critical as this could provide support for earlier intervention, which may result in improved long-term outcomes. We therefore examined the impact of severe AS and its consequent increase in LV afterload on myocardial deformation and rotational mechanics by 2-dimensional (2D) and 3-dimensional (3D) speckle-tracking echocardiography. Methods: We prospectively measured various strain parameters in 168 patients (42% female, mean age 72 ± 12 years) with severe AS and LV ejection fraction (EF) ≥50%, and compared them to normal values found in literature. 2D and 3D images were analyzed for global longitudinal strain (GLS), global circumferential strain (GCS), global radial strain (GRS), basal rotation, apical rotation, and peak systolic twist. We further assessed the degree of concordance between 2D and 3D strain, and examined their association with measures of LV preload and afterload. Results: Patients with severe AS exhibited significantly lower GLS and GRS but higher GCS, apical rotation, and twist by 2D and 3D echocardiography compared with published normal values (P = 0.003 for 3D twist, P < 0.001 for all others). Agreement between 2D- and 3D-GLS by concordance correlation coefficient was 0.49 (95% confidence interval: 0.39-0.57). GLS was correlated with valvulo-arterial impedance, a measure of LV afterload (r = 0.34, p < 0.001 and r = 0.23, p = 0.003, respectively). Conclusion: Patients with severe AS demonstrated lower-than-normal GLS and GRS but appear to compensate with higher-than-normal GCS, apical rotation, and twist in order to maintain a preserved LVEF. GLS showed a modest correlation with valvulo-arterial impedance.
