Control of Risk Factors for Cardiovascular Disease in Patients with Type 2 Diabetes in Beijing Communities

Abstract:Objective To explore the status of risk factors for cardiovascular diseases in patients with type 2 diabetes in Beijing communities. Methods We conducted a cross?sectional survey in 27 Beijing communities of adults aged 20 years or older. According to operation specification, all of the patients were collected medical history and taken both physical and correlated laboratory examinations. Glycated haemoglobin A1c(HbAlc), lipid profile, blood pressure and body mass index(BMI) were assessed according to the Chinese guidelines for type 2 diabetes prevention and treatment(2013). Chi square test was used in comparison between groups. Results A total of 2 213 T2DM patients were enrolled, which included 895 male ( (59±10) yrs) and 1 318 female participants ( (61±8) yrs) with type 2 diabetes. No statistic significance was found in the difference on HbA1c(A) between the two groups (47.6%for male vs 49.5%for female,χ2=0.747,P=0.387). The blood pressure(B) (28.9% vs 35.2%) and body mass index (26.3% vs 31.6%, χ2=9.515,7.429, both P<0.05) were significantly lower whereas the serum low density lipid chor (LDL?C) level (C) (46.9% vs 39.8%, χ2=10.964, P<0.05) was higher in male patients compare to female patients. Only7.5%of the patients achieved all 3(ABC) target goals. The HbA1c was significantly lower in group with longer diabetes duration. Compared to younger diabetes patients, the older patients had significantly higher rates in HbA1c(40.5%, 49.9%, 54.5%), blood pressure(27.1%, 31.9%, 38.8%) and body mass index(25.8%, 29.4%, 32.9%, χ2=26.297, 20.446, 7.866, all P<0.05). Conclusion The control rates of diabetes, hypertension, dyslipidemia and obesity in type 2 diabetes patients in Beijing communities are lower than those in the top three hospitals, especially in male and younger patients. The propaganda and intervention should be strengthen for type 2 diabetes complications in community.
Glycemic Control Rate of T2DM Outpatients in China: A Multi-Center Survey

Abstract:Background: Type 2 diabetes mellitus (T2DM)-associated mortality and morbidity are strongly dependent on glycemic control. With T2DM prevalence increasing in China, we aimed to assess glycemic control rates in Chinese T2DM outpatients.Material/Methods: A total of 9065 adult T2DM outpatients (5035 men) were assessed in 26 Chinese medical centers between August 2010 and April 2012. Patients were stratified according to BMI (kg/m(2)): <24, 24-28, and >28. Successful glycemic control was defined as glycated hemoglobin A1c (HbA1c) <= 7% or fasting plasma glucose (FPG) <7.0 mmol/L.Results: Among the participants included in this study, 2939 had BMI <24, 3361 had BMI of 24-28, and 2764 had BMI >28. The glycemic control rate was only 32.6%, and the triple control rate for glycemia, blood pressure, and lipidemia was only 11.2%. Glycemic control rates by BMI group were 33.7% (<24), 33.8% (24-28), and 30.2% (>28) (p=0.005), and corresponding incidences of cardiovascular diseases (CVD) were 12.2%, 15.7%, and 15.9% (p<0.001). Multivariate logistic regression analysis demonstrated that older age (p<0.001), higher BMI (p=0.026), larger waist circumference (p<0.001), less education (p<0.001), and recent diagnosis (p<0.001) were independent risk factors for poor glycemic control.Conclusions: The T2DM glycemic control rate in China is currently low, especially in older obese patients with poor education and recent diagnosis.
The Prevalence and Control of Type 2 Diabetes Mellitus in Residents of a Rural Town, Shandong Province, China

Abstract:Objective: To understand the prevalence and control of type 2 diabetes mellitus (T2DM) in a rural area, a cross-sectional study was carried out. Methods: Blood glucose was determined in residents over 60 years during the National Basic Public Health Service project carrying on in a rural town of Shandong province. Combined with detail information of diabetic history, the prevalence rate of this disease was calculated with a cutoff of more than 7.0 mmol/L of fasting plasma glucose (FPG). Furthermore, according to FPG levels, the control rate of diabetes was evaluated. Basic information and clinical laboratory tests were analyzed with proper methods for univariate analysis. Multivariable logistic analysis was used to explore multiple variables associated with the control effect of T2DM. Results: The prevalence rate of T2DM was 13.5% (95% CI 12.4%~14.7%) without age difference in residents over 60-year old (X 2= 1.078, P = 0.898). And the rate was higher in the female (16.1%, 95% CI 14.4%~17.8%) than that in the male (11.3%, 95% CI 9.7%~12.9%). The control rate of diabetes was 44.5% (95% CI 39.4-49.6%, cutoff <6.1 mmol/L for FPG) and 59.6% (95% CI 54.5-64.6%, cutoff <7.0 mmol/L for FPG), respectively. Most T2DM residents had balance foods, and a high proportion of drugs or insulin were used; however, the proportion of keeping exercise and modifying behaviors was low. Nearly 80% of the diabetic individuals used at least one drug or insulin. However, the more drugs or insulin used, the higher the FPG level was. Conclusion: A relative high level of T2DM prevailed in the rural area under fast economy-transforming. T2DM was controlled poor and measures should be further optimized to achieve ideal effect, especially for the use of anti-diabetic drugs.
Management Status of Type 2 Diabetes Mellitus in Tertiary Hospitals in Beijing: Gap Between Guideline and Reality.

Abstract:BACKGROUND Diabetes has become one of the most common chronic diseases and the third leading cause of death in China. Many programs have been initiated at national and local levels to address the illness. However, the effect of these programs in daily outpatient clinics is still unclear. The objective of this study was to investigate the management status of type 2 diabetes mellitus (T2DM) and factors associated with it in diabetes clinics of tertiary hospitals in Beijing. METHODS A cross-sectional survey was conducted in six tertiary hospitals in Beijing. Control criteria were defined based on 2007 China guideline for type 2 diabetes (CGT2D). RESULTS A sample of 1151 patients, age (60.8 ± 9.2) years, and with a median disease duration of 7.3 years was included. The hemoglobin A1c (HbA1c) mean level was (7.15 ± 1.50)%, the percentage of patients achieving the targets for HbA1c was 37.8%, blood pressure 65.6%, triglyceride (TG) 48.8%, high-density lipoprotein (HDL) 59.2%, low-density lipoprotein (LDL) 34.0%, and total cholesterol (TC) 42.0%. The factors independently associated with glycemic control were diabetes duration (odds ratio (OR) = 0.95; 95% confidence interval (CI): 0.919 - 0.982, P < 0.01), body mass index (BMI) (OR = 0.914, 95%CI: 0.854 - 0.979, P = 0.01) and smoking (OR = 0.391, 95%CI: 0.197 - 0.778, P < 0.01). The factors independently associated with blood pressure control were BMI (OR = 0.915, 95%CI: 0.872 - 0.960, P < 0.01) and male gender (OR = 0.624, 95%CI: 0.457 - 0.852, P < 0.01). The factor independently associated with LDL control was education level (OR = 1.429, 95%CI: 1.078 - 1.896, P = 0.013). CONCLUSIONS The management status of T2DM patients in tertiary hospitals in Beijing has improved remarkably. However, there is still room for further improvement to reach the guideline target. Long diabetes duration, high BMI, smoking, male gender and low education level were independently associated with poor metabolic control.

Abstract:ObjectiveThe purpose of this study is to evaluate the prevalence, awareness, treatment and glycemic control of diabetes mellitus (DM) in a Chinese population. The findings from this study are expected to offer scientific evidence to better prevent and control the growing number of reported and untreated cases.MethodsA cross-sectional survey was conducted in Jiangsu, China. We recruited permanent residents over 18 years of age from eight towns in Jintan (JT) and six towns in Yangzhong (YZ) using a three-stage stratified cluster sampling method. The rates of DM prevalence, awareness, treatment and control as well as their related factors were analyzed.ResultsA total number of 15404 people were entered into the analysis. The DM prevalence, awareness, treatment and control rates were 7.31%, 58.35%, 51.87% and 14.12%, respectively. Multivariable logistic regression analysis showed that being female was positively related to prevalence (OR = 1.21, 95% CI: 1.07-1.37), awareness (OR = 1.52, 95% CI: 1.19-1.93), treatment (OR = 1.48, 95% CI: 1.17-1.88) and control (OR = 1.87, 95% CI: 1.30-2.67) of DM. Having a family history of diabetes was significantly correlated with DM risk (OR = 1.86, 95% CI: 1.37-2.54) and increased awareness (OR = 3.12, 95% CI: 2.19-4.47), treatment (OR = 3.47, 95% CI: 2.45-4.90) and control (OR = 1.81, 95% CI: 1.22-2.68) of DM. Former smoking status (OR = 1.82, 95% CI: 1.23-2.71), overweight (OR = 2.11, 95% CI: 1.72-2.60) and obesity (OR = 3.46, 95% CI: 2.67-4.50) were related to the risk of DM. Additionally, we found current drinking status to be positively correlated with DM risk (OR = 1.30, 95% CI: 1.01-1.66) and negatively correlated with DM awareness (OR = 0.41, 95% CI: 0.29-0.59) and treatment (OR = 0.41, 95% CI: 0.29-0.59). Our study highlights the high prevalence and inadequate awareness, treatment and control of DM in the Chinese population.ConclusionsManagement and prevention of DM-related complications should be considered an essential strategy by governments and society. This study assessed the reasons why DM has been increasing and established the first step in determining where to start regarding preventative methods.

Abstract:Objective To investigate the state of T2DM control in tertiary first-class hospitals in Beijing.Methods 1151 patients with T2DM were recruited as the study subjects in 6 tertiary first-class hospitals in Beijing.The message was collected to evaluate the state of T2DM control via interview,medical records and electronic laboratory enquiry system.The evaluation criteria were defined based on 2007 China Guideline for T2DM.Results The age was(60.8±9.2) years old,the disease duration was 7.3 years,the BMI was(24.6±3.3)kg/m2,and the HbA1c level was(7.15±1.50)%.The target arrival rates of HbA1c,BP,TG,HDL-C,LDL-C,and TC were 37.8%,65.6%,48.8%,59.2%,34.0%,and 42.0%,respectively.Conclusions The metabolic control of T2DM in tertiary first-class hospital is better in Beijing than in the domestic and abroad,but there is a wide gap as compared with 2007 China Guideline for T2DM.
Prevalence and related knowledge of diabetes mellitus among residents of Beijing

Abstract:Objective To determine the prevalence of diabetes mellitus,rates of awareness,treatment and control of diabetes mellitus(DM) in Beijing citizens.Methods Totally 10 054 subjects were randomly selected from different districts of Beijing in 2007.DM was diagnosed according to WHO criteria of 1999.Results The prevalence of DM was 11.0%(11.0% for male and 10.9% for female,P>0.05).The prevalence of DM was the highest in urban residents and lowest in suburban residents(P<0.001),and increased with age(P<0.001).The rate of awareness,treatment,and control of DM was 62.0%,55.2%,and 32.2%,respectively,with no gender difference(P>0.05),but increased with age and education level of the residents(P<0.001).Conclusion The prevalence of DM is high among the residents in Beijing,but the rates of awareness,treatment,and control are low.The education,screening,and treatment of DM are urgently recommended.

Abstract:Objective: To describe the current situation of blood glucose control in 18-64 year-old people with type 2 diabetes in the Shenzhen community and analyze the influencing factors to provide a reference for enhancing the management effect of type 2 diabetes in community health service institutions. Methods: The data were from the Shenzhen Community Health Service Information System. A cross-sectional survey design was adopted to include 18-64 year-old patients with type 2 diabetes in Shenzhen community health service institutions in 2022. The blood glucose control rate was calculated through the physical examination data recorded by the information system in 2022, and the influencing factors of blood glucose control in this group of patients were analyzed using a multivariate logistic regression model. Results: A total of 120 174 patients were included in the study. The blood glucose control rate was 53.04%. The results of multivariate logistic regression analysis showed that women (OR=1.07, 95%CI: 1.04-1.10), ages 45-54 (OR=1.10, 95%CI: 1.01-1.19), 55-64 (OR=1.24, 95%CI: 1.14-1.35), middle school (OR=1.09, 95%CI: 1.05-1.13), high school and technical secondary school (OR=1.26, 95%CI: 1.21-1.31), junior college or above (OR=1.75, 95%CI: 1.67-1.83), basic medical insurance for urban employees (OR=1.05, 95%CI: 1.01-1.08), weekly exercise (OR=1.26, 95%CI: 1.22-1.31), daily exercise (OR=1.31, 95%CI: 1.28-1.35) and combined hypertension (OR=1.21, 95%CI: 1.18-1.24) were more likely to reach the standard of blood glucose control; while unmarried (OR=0.85, 95%CI: 0.77-0.95), divorced (OR=0.84, 95%CI: 0.73-0.97), insurance status was fully self-funded (OR=0.95, 95%CI: 0.91-0.99), disease duration was 5- years (OR=0.65, 95%CI: 0.63-0.66), ≥10 years (OR=0.41, 95%CI: 0.39-0.42), the treatment method was diabetes drug therapy (OR=0.74, 95%CI: 0.71-0.76), low weight (OR=0.89, 95%CI: 0.80-1.00), obese group (OR=0.85, 95%CI: 0.82-0.88), combined with central obesity (OR=0.83, 95%CI: 0.81-0.86), combined with dyslipidemia (OR=0.69, 95%CI: 0.68-0.71), current smoking (OR=0.74, 95%CI: 0.72-0.77), and current drinking (OR=0.97, 95%CI: 0.93-1.00) were less likely to reach the standard of blood glucose control. Conclusions: The blood glucose control rate of 18-64 year-old people with type 2 diabetes in Shenzhen still has room for improvement. More attention should be paid to 18-64 year-old patients with type 2 diabetes, especially for patients with dyslipidemia, central obesity, and diabetes with a long course, and supervision and guidance should be strengthened for patients with bad habits such as smoking, drinking alcohol, and lack of exercise.

Abstract:Objective To investigate the prevalence of diabetes mellitus(DM),to analyze its risk factors among permanent inhabitants in Chaoyang District,Beijing,and to provide methods to control DM in Chaoyang District.Methods A survey with multi-stage random cluster sampling was performed in 20 sub-districts among permanent inhabitants aged 18-79 years.Questionaire,physical examination and laboratory test were evaluated.Results The prevalencerate of DM in Chaoyang District was 8.80%,and 10.11% after standerlized with age.The rates of awareness,treatment and control of DM were 57.65%,41.45% and 30.25%,respectively.Male(OR=1.77,95%CI:1.39-2.27),age(OR=1.93,95%CI:1.78-2.19),obesity(OR=1.54,95%CI:1.22-1.94),family(OR=3.12,95%CI:2.46-3.94) history were the risk factors.The prevalence of DM was decreased with the increase of the degree of education.Hypertension and dyslipidemic were significantly associated with DM.Conclusion The prevalence of DM is still high in Chaoyang District,and measures to control DM are needed.
Risk Factors Associated with the Presence of Diabetes in Chinese Communities in Beijing.

Abstract:Objective: To identify risk factors associated with the presence of diabetes.Methods: A diabetes survey was conducted in 2801 citizens aged 35-79 years living in 10 communities in Beijing, China. Participants were recruited by residents committees. 75 g oral glucose tolerance test was performed to define diabetes according to the WHO 1999 criteria. Logistic regression analysis was used to estimate odds ratios (OR) for diabetes.Results: The prevalence of diabetes and impaired fasting glucose (IFG) and/or impaired glucose tolerance (IGT) was 24.2% and 20.4% in men, respectively and 19.4% and 19.0% in women, respectively. The multivariate adjusted OR (95% CI) corresponding to a one standard deviation increase in age (year) was 1.52 (1.25,1.86), waist circumference (cm) 1.35 (1.12,1.63), serum triglycerides (mmol/L) 1.29 (1.09,1.54) and high density lipoprotein cholesterol (mmol/L) 0.74 (0.61,0.91) in men and 2.03 (1.77,2.33),1.21 (1.06,1.38), 1.34 (1.21,1.49),0.74 (0.66,0.84) in women, respectively. For diabetes family history they were 2.46 (1.66,3.65) in men and 2.39 (1.84,3.10) in women, and for hypertension 1.14 (0.77,1.68) in men and 1.54 (1.18,2.01) in women. There were no significant associations between the presence of diabetes and occupation, education level, household income, leisure time physical activities, current smoking and drinking status.Conclusions: Age, diabetes family history, obesity, dyslipidemia and hypertension were all associated with the presence of diabetes in this study population. Abstract:Background The prevalence of type 2 diabetes in China is increasing rapidly. Appropriate management of glycemia, blood pressure and dyslipidemia in this population is a major public health concern. Objective The aim of this study was to assess metabolic control including glycated hemoglobin A(1c) (HbA(1c)), blood pressure (BP) and low density lipoprotein cholesterol (LDL-c), in a large sample of patients with type 2 diabetes in China and to identify factors that correlated with the achievement of HbA(1c), BP and LDL-c goals (ABCs). Method A nationwide survey was conducted in 50 medical centres across China from April to July of 2010. Baseline information on demographics, medical history, HbA(1c), BP and LDL-c levels were measured in 5961 patients with type 2 diabetes. Results Mean age, body mass index (BMI) and HbA(1c) were 59.5 +/- 1.3 years, 24.5 +/- 4.1 kg/m(2) and 8.3 +/- 2.2%, respectively. With respect to generally accepted ABC treatment goals, 35.2% of participants had HbA(1c) <7%; 35.5% had BP<140/80 mmHg, and 45.1% had LDL-c<100 mg/dl. The proportion of patients who met all three targets was only 5.4%. Logistic regression revealed that smoking (P=0.000), higher BMI (P=0.001) and insulin use (P=0.000) were statistically significant predictors of failing to meet ABC targets. Conclusion The percentage of Chinese patients with type 2 diabetes who met recommended targets for HbA(1c), BP and LDL-c in 2010 was low. Smoking, higher BMI and insulin use were the strongest determinants of failing to meet ABC targets. Copyright (C) 2015 John Wiley & Sons, Ltd.

Abstract:Objective To survey prevalence rate, treatment rate and control rate of diabetes mellitus in Xuzhou. Methods With multistage cluster sampling, a total of 23742 residents aged above 15 years were selected and investigated by a standard questionnaire on diabetes mellitus epidemiology. Results The prevalence rate of diabetes mellitus in Xuzhou was 5.35% and its standardized prevalence rate was 4.04%. It was increasing with the age( P =0. 000). Its prevalence rate in urban was 6.35% higher than that in rural area with 4.58% and its standardized prevalence rate was 4.27% and 3.76% respectively. Its prevalence rate in male and female were 5.04% and 5.62% respectively and its standardized prevalence rate were 3.92% and 4. 14% respectively. There were significant difference between male and female prevalence rate ( P 0.05). The rate of awareness, treatment and control were 45.20%, 32.36% and 45.30%respectively. Conclusions The prevalence rate of diabetes mellitus in Xuzhou was higher than the national average. The percentage rate of people with impaired fasting glucose was higher and the high risk population has youthful trend. In addition, the rate of awareness, treatment and control were above national average. In conclusion, it was in urgent need to carry out and improve community-based comprehensive prevention and control of diabetes mellitus in Xuzhou. Abstract:Objective To study the awareness of disease and the status of control or treatment for hypertension,diabetes and hypercholesterolemia in a community population.Methods In 2007,a cross-sectional survey was carried out in the community residents of Xishan,which was located in Shijingshan district of Beijing city.One thousand two hundred and fifty six participants included 643 men with age ranged between 35-74 and 613 women with age ranged between 40-74.A standard structure questionnaire was used to collect the history and current treatment or control status of hypertension,diabetes and hypercholesterolemia,which were respectively defined by national guidelines.Results The prevalence and the proportion of disease awareness were 40.6% and 78.4% for hypertension;10.3% and 80.6% for diabetes;8.9% and 34.8% for hypercholesterolemia,respectively.62.0% and 27.1% of the participants reported the status of treatment and control for hypertension;68.2% and 25.6% for diabetes;32.1% and 12.5% for hypercholesterolemia,respectively.Patients with cardio-/cerebro-vascular diseases were associated with higher proportion of awareness for hypertension and diabetes,but not for hypercholesterolemia.Conclusion The study suggested that the community population with the medical conditions were better treated and controlled for hypertension and diabetes than for hypercholesterolemia.It is important to increase the awareness of disease,the proportion of treatment and control for hypercholesterolemia.

Abstract:Objective To study control situation of type 2 diabetes and its influence factors in different grade hospitals in Beijing. Methods Blood pressure,blood glucose and lipid fractions of 2077 subjects with type 2 diabetes mellitus from grade I,Ⅱ,Ⅲhospitals in Beijing were measured in the cross sectional study,multilevel analysis was employed to investigate the relationship between diabetes control situation and different hospital grades. Results Among 2077 patients,22.7%,30.5%,and 42.7% respectively reached normal levels of fasting plasma glucose(FPG),total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C)after treatment in grade Ⅲ hospital. In contrast,the percentages of patients with normal FPG,TC and LDL-C levels in grade I hospitals (17.2%,25.1%,32.7% respectively)and grade Ⅱ hospitals(14.4%,22.4%,37.1% respectively)were greatly lower than those in grade III hospitals (P0.01);And,the percentages of patients with normal high-density lipoprotein cholesterol in grade Ⅲ hospitals and grade Ⅰ hospitals were 84.8% and 70.9% respectively,which were significantly higher than 68.0% in grade II hospitals (P0.01); However,there was no significant difference in the percentages of patients with the normal level of 2h-PG,glycosylated hemoglobin (HbA1c),systolic blood pressure or diastolic blood pressure between grade Ⅰ,Ⅱ and Ⅲ hospitals(P0.05). The main causes of failure in control of blood sugar are as follow in order:unreasonable diet control,not taking enough prescribed medicine,not doing exer-cise persistently,not seeing a doctor on time,not taking medicine according to the doctor's advice and not using insulin. Conclusions Although control situation of type 2 diabetes in grade Ⅲ hospital is better than that in grade Ⅰ and Ⅱ hospitals,the percentages of type 2 diabetes patients with normal blood glucose,blood pressure and lipid levels are still quite low. Therefore,it is necessary to further strengthen the guideline of diabetes patients in different grade hospitals and thus improve diabetes therapy and decrease its complications.

Abstract:Objective: To explore the self-management of diabetes among type 2 diabetes residents in a community in Beijing.Methods: Used random cluster sampling method.Type 2 diabetes patients among 1775 community residents above or equal to 18 years old were enrolled in the questionnaire survey.Results: Screened out a total of 137 patients with diabetes(7.7%).Majority of them was elder.Overweight or obesity accounted for 61.3%.73.7% of the patients had cardiovascular and cerebrovascular diseases.28.3% of the patients fasting blood glucose control and 19.2% of patients postprandial blood glucose control were not ideal.14.2% of the patients did not monitor their glucose.Only 51.7% of the patients checked their glycosylated hemoglobin every 3 ～ 6 months.19.7% patients were drinking and 16.8% patients were smokers.The salt and oil consumption of some patients were more.Only 48.2% patients exercise 150 minutes weekly.Conclusion: Community medical staff need to residents for health education and behavior intervention,from disease surveillance and lifestyle changes to proceed,to help diabetic residents to establish a good way of life,better control of blood sugar,reduce the incidence of diabetic complications,improve the living quality.

Abstract:Objective To investigate the prevalence of diabetes-hypertension and related cardiovascular complications of these comorbid conditions.Methods Data were collected by physical examinations and an administered questionnaire from people older than 40 years old in rural areas of Fangshan district,Beijing City.Analysis of variance,chi-square test and logistic regression model were used.Results Of the 8 642 subjects,the prevalence rates of diabetes mellitus,hypertension and both conditions were 57.6%,11.3% and 7.9%,respectively.The prevalence rate of hypertension and diabetes together was higher among women than men.The rates of awareness, treatment and control of hypertension among people affected by both hypertension and diabetes were 71.1%,44.0% and 9.9%,respectively.The prevalence rates of stroke among people who had neither hypertension nor diabetes,hypertension only,diabetes only and two diseases together were 2.9%,5.2%,7.9% and 12.9%,respectively,while 16.1%,26.8%,39.0% and 46.0% of coronary heart disease.Conclusions The people who were 40 years old and above have a high prevalence of hypertension and diabetes together,which are important risk factors for cardiovascular diseases,tight control of the blood pressure should be needed.
The hypertension and hyperlipidemia status among type 2 diabetic patients in the community and influencing factors analysis of glycemic control

Abstract:Objective: To understand the prevalence of hypertension and hyperlipidaemia as well as the current status of glycaemic control and its influencing factors among type 2 diabetes mellitus patients in the community in South China, and to provide recommendations for the prevention and control of diabetes. Methods: Questionnaires, physical examinations and laboratory tests were conducted on patients with type 2 diabetes mellitus who participated in the National Basic Public Health Service Programme in Guangzhou in 2020. The chi-square test, t-test and multi-factor unconditional logistic regression analysis were performed using R 4.1.2 software. Result: Among 127,423 type 2 diabetic patients in Guangzhou, 57,695 achieved glycemic control standards, with a glycemic control rate of 45.28%.In this study, the proportion of T2DM patients with hypertension and hyperlipidaemia together was 27.79%, The percentage of T2DM patients with hypertension alone and hyperlipidaemia alone was 28.34% and 20.53% respectively, and the rate of no complications was 23.34%. There was a statistically significant difference in the rate of glycaemic control between the different disease combination states (P < 0.05). The glycaemic control rate was 47.67% in diabetic patients without hypertension and hyperlipidaemia, 52.54% and 37.24% in those with combined hypertension alone and hyperlipidaemia alone respectively, compared to 41.80% in diabetic patients with hypertension and hyperlipidaemia. After adjusting for all covariates, multivariate analysis showed that combined hypertension alone was associated with good glycaemic control (OR 0.817, 95% CI 0.791, 0.843, P < 0.001),when using comorbid T2DM as a control group, combined hyperlipidaemia alone, combined hypertension and hyperlipidaemia were associated with poor glycaemic control (OR 1.521, 95% CI 1.470,1.574, P < 0.001 and OR 1.250, 95% CI 1.211,1.291, P < 0.001), Subgroup analyses as well as multifactorial unconditional logistic regression analyses showed that patients with type 2 diabetes who were overweight and obese, smoked, drank alcohol, had a diagnosis of diabetes for ≥ 6 years, had fair or poor adherence and had a family history of diabetes had lower rates of glycaemic control. Conclusion: The results of this study showed that the co-morbidity of hypertension and hyperlipidaemia was high and prevalent among diabetic patients in Guangzhou. Moreover, glycaemic control of T2DM patients with hyperlipidaemia was lower than other diabetic patients. Obesity and overweight, poor lifestyle and dietary habits are also major factors affecting the treatment and control of T2D patients in this region. Therefore, comprehensive measures should be actively taken to control blood glucose levels in type 2 diabetic patients by also incorporating lipid management into the community and strictly controlling lipid levels.

Abstract:BACKGROUND:The prevalence of type 2 diabetes in China is increasing rapidly. Appropriate management of glycemia, blood pressure and dyslipidemia in this population is a major public health concern. OBJECTIVE:The aim of this study was to assess metabolic control including glycated hemoglobin A1c (HbA1c ), blood pressure (BP) and low density lipoprotein cholesterol (LDL-c), in a large sample of patients with type 2 diabetes in China and to identify factors that correlated with the achievement of HbA1c, BP and LDL-c goals (ABCs). METHOD:A nationwide survey was conducted in 50 medical centres across China from April to July of 2010. Baseline information on demographics, medical history, HbA1c , BP and LDL-c levels were measured in 5961 patients with type 2 diabetes. RESULTS:Mean age, body mass index (BMI) and HbA1c were 59.5 ± 1.3 years, 24.5 ± 4.1 kg/m(2) and 8.3 ± 2.2%, respectively. With respect to generally accepted ABC treatment goals, 35.2% of participants had HbA1c <7%; 35.5% had BP < 140/80 mmHg, and 45.1% had LDL-c < 100 mg/dl. The proportion of patients who met all three targets was only 5.4%. Logistic regression revealed that smoking (P=0.000), higher BMI (P=0.001) and insulin use (P=0.000) were statistically significant predictors of failing to meet ABC targets. CONCLUSION:The percentage of Chinese patients with type 2 diabetes who met recommended targets for HbA1c , BP and LDL-c in 2010 was low. Smoking, higher BMI and insulin use were the strongest determinants of failing to meet ABC targets.

Abstract:Abstract This study aimed at understanding the diabetic prevalence, awareness, treatment and control rates and their influencing factors among people aged ≥ 40 years in Shenyang, China. A face-to-face cross-sectional epidemiological survey was conducted on the respondents using the national unified questionnaire. A total of 3922 respondents were enrolled, including 609 cases of diabetes. The diabetic prevalence rate was 15.5%, and was higher in rural areas than that in urban areas (17.7% vs . 14.2%, p = 0.004), while no difference was observed between men and women (14.8% vs . 16.1%, p = 0.242). Advanced age, hypertension and dyslipidemia were the diabetes influencing factors. Among the 609 respondents with diabetes, the diabetic awareness and treatment rates, and the control rate of fasting plasma glucose were 82.3%, 36.6% and 17.1%, respectively. In different age groups, the diabetic awareness rate was higher in men than that in women, and the treatment rate was higher in women than that in men. The diabetic patients, who consumed fruit for ≥ 5 days a week, accounted for 16.3%, and their diabetic treatment (28.1%) and control rates (44.1%) were lower. Shenyang people aged ≥ 40 years have higher diabetic prevalence and awareness rates, and lower diabetic treatment and control rates. Finally, it is necessary to enhance awareness and education about diabetes, to improve its treatment and control rates.

Abstract:Objectives To investigate the relationship between different medical payment methods and control rate of glucose, blood lipid and blood pressure, and to analyze its influence on the prevalence of microvascular complications in patients with type 2 diabetes mellitus (T2DM) in communities. Methods A total of 2 213 patients with T2DM from 27 community clinics in Beijing were enrolled in this study. For each patient, the information of medical history, physical examination, blood biochemistry parameter, and medical insurance status were collected. The glycated hemoglobin A1c<7%, systolic blood pressure<140 mmHg (1 mmHg=0.133 kPa), diastolic blood pressure<80 mmHg, and low-density lipoprotein<2.6 mmol/L were set as the target goals. The patients were divided into good medical care group(reimbursed by government/company or citizen medical insurance) and poor medical care group(self-paid or paid in other ways) according to the medical payment methods. The control rates of blood glucose, blood lipid and blood pressure were compared separatedly and integratedly between the two groups. The association of diabetic complications, including diabetic neuropathy, diabetic retinopathy and diabetic nephropathy, with medicine payment status was evaluated. The single factor variance analysis, t test, Chi square test and Logistic regression analysis were used for statistical analysis. Results Among the 2 213 patients with T2DM, 1 419 (64.1%) belonged to good medical care group, and 794 (35.9%) belonged to poor medical care group. The average age and the duration of diabetes were (61±9) vs (58±9) years and (8±6) vs (7±5) years, respectively (t=6.644, 5.237, both P<0.05). The control rates of blood glucose, blood pressure, blood lipids and integrated of above all were 52.9%(750/1 419), 38.8%(551/1 419), 44.7%(635/1 419) and 9.7%(138/1 419) in the good medical care group , and 41.3%(328/794), 21.7%(172/794), 39.0%(310/794) and 3.7%(29/794) in the poor medical care group, respectively (χ2=27.160, 68.220, 6.780 and 26.910, all P<0.01). The prevalence of diabetic complications of peripheral neuropathy, retinopathy and nephropathy were 20.4%, 31.6% and 34.3%in poor medical care group, and 12.4%, 21.7%and 27.8%in good medical care group, respectively (χ2=25.180, 26.470 and 10.200, all P<0.01). Conclusion The control of T2DM and its complications is better in patients with good medical care.
