Abstract:Background: Deep learning has always been at the forefront of scientific research. It has also been applied to medical research. Hereditary spinocerebellar ataxia (SCA) is characterized by gait abnormalities and is usually evaluated semi-quantitatively by scales. However, more detailed gait characteristics of SCA and related objective methods have not yet been established. Therefore, the purpose of this study was to evaluate the gait characteristics of SCA patients, as well as to analyze the correlation between gait parameters, clinical scales, and imaging on deep learning. Methods: Twenty SCA patients diagnosed by genetic detection were included in the study. Ten patients who were tested via functional magnetic resonance imaging (fMRI) were included in the SCA imaging subgroup. All SCA patients were evaluated with the International Cooperative Ataxia Rating Scale (ICARS) and Scale for the Assessment and Rating of Ataxia (SARA) clinical scales. The gait control group included 16 healthy subjects, and the imaging control group included seven healthy subjects. Gait data consisting of 10 m of free walking of each individual in the SCA group and the gait control group were detected by wearable gait-detection equipment. Stride length, stride time, velocity, supporting-phase percentage, and swinging-phase percentage were extracted as gait parameters. Cerebellar volume and the midsagittal cerebellar proportion in the posterior fossa (MRVD) were calculated according to MR. Results: There were significant differences in stride length, velocity, supporting-phase percentage, and swinging-phase percentage between the SCA group and the gait control group. The stride length and stride velocity of SCA groups were lower while supporting phase was longer than those of the gait control group. SCA group's velocity was negatively correlated with both the ICARS and SARA scores. The cerebellar volume and MRVD of the SCA imaging subgroup were significantly smaller than those of the imaging control group. MRVD was significantly correlated with ICARS and SARA scores, as well as stride velocity variability. Conclusion: SCA gait parameters were characterized by a reduced stride length, slower walking velocity, and longer supporting phase. Additionally, a smaller cerebellar volume correlated with an increased irregularity in gait. Gait characteristics exhibited considerable clinical relevance to hereditary SCA. We conclude that a combination of gait parameters, ataxia scales, and MRVD may represent more objective markers for clinical evaluations of SCA.

Abstract:Background Maintaining balance is crucial for independence and quality of life. Loss of balance is a hallmark of spinocerebellar ataxia (SCA). Objective The aim of this study was to identify which standing balance conditions and digital measures of body sway were most discriminative, reliable, and valid for quantifying balance in SCA. Methods Fifty‐three people with SCA (13 SCA1, 13 SCA2, 14 SCA3, and 13 SCA6) and Scale for Assessment and Rating of Ataxia (SARA) scores 9.28 ± 4.36 and 31 healthy controls were recruited. Subjects stood in six test conditions (natural stance, feet together and tandem, each with eyes open [EO] and eyes closed [EC]) with an inertial sensor on their lower back for 30 seconds (×2). We compared test completion rate, test–retest reliability, and areas under the receiver operating characteristic curve (AUC) for seven digital sway measures. Pearson's correlations related sway with the SARA and the Patient‐Reported Outcome Measure of Ataxia (PROM ataxia). Results Most individuals with SCA (85%–100%) could stand for 30 seconds with natural stance EO or EC, and with feet together EO. The most discriminative digital sway measures (path length, range, area, and root mean square) from the two most reliable and discriminative conditions (natural stance EC and feet together EO) showed intraclass correlation coefficients from 0.70 to 0.91 and AUCs from 0.83 to 0.93. Correlations of sway with SARA were significant (maximum r = 0.65 and 0.73). Correlations with PROM ataxia were mild to moderate (maximum r = 0.56 and 0.34). Conclusion Inertial sensor measures of extent of postural sway in conditions of natural stance EC and feet together stance EO were discriminative, reliable, and valid for monitoring SCA. © 2024 International Parkinson and Movement Disorder Society.

Abstract:Ataxia is a kind of external characteristics when the human body has poor coordination and balance disorder, it often indicates diseases in certain parts of the body. Many internal factors may causing ataxia; currently, observed external characteristics, combined with Doctor’s personal clinical experience play main roles in diagnosing ataxia. In this situation, different kinds of diseases may be confused, leading to the delay in treatment and recovery. Modern high precision medical instruments would provide better accuracy but the economic cost is a non-negligible factor. In this paper, novel non-contact sensing technique is used to detect and distinguish sensory ataxia and cerebellar ataxia. Firstly, Romberg’s test and gait analysis data are collected by the microwave sensing platform; then, after some preprocessing, some machine learning approaches have been applied to train the models. For Romberg’s test, time domain features are considered, the accuracy of all the three algorithms are higher than 96%; for gait detection, Principal Component Analysis (PCA) is used for dimensionality reduction, and the accuracies of Back Propagation (BP) neural Network, Support Vector Machine (SVM), and Random Forest (RF) are 97.8, 98.9, and 91.1%, respectively.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Objective</h3><p>Spinocerebellar ataxia type 3 (SCA3) is one of the most common hereditary neurodegenerative diseases, with balance instability as main symptom. Balance quantification is crucial for evaluating the efficacy of therapeutic interventions. However, balance evaluation in SCA3 is often subject to bias. Here, we aimed to quantitatively evaluate postural instability and investigate the relationship between postural instability and clinical characteristics in SCA3 patients.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>Sixty‐two SCA3 patients and 62 normal controls were recruited, and their postural balance was measured using a posturographic platform. Principal component analysis was performed as data reduction to identify postural instability factors. Multivariable linear regression was used to investigate potential risk factors for postural instability and to explore whether postural instability predicts the severity and progression of ataxia in SCA3 patients.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>We found SCA3 patients experience postural instability characterized by significant impairment in static and dynamic stability. The condition without visual feedback was the most sensitive measure in differentiating SCA3 from controls. Regression analyses revealed that ataxia severity predicted both static (<i>P </i> = 0.014) and dynamic stability (<i>P </i> = 0.001). Likewise, along with expanded CAG repeats (<i>P </i> < 0.001), both static (<i>P </i> < 0.001) and dynamic stability (<i>P </i> < 0.001) predicted ataxia severity, but not ataxia progression. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Interpretation</h3><p>Our findings demonstrate the validity of using the Pro‐kin system for assessing postural instability in SCA3 patients. This type of quantitative assessment of balance dysfunction can contribute to clinical trials and balance rehabilitation in SCA3 patients.</p></section>

Abstract:We use autoregressive hidden Markov models and a time-frequency approach to create meaningful quantitative descriptions of behavioral characteristics of cerebellar ataxias from wearable inertial sensor data gathered during movement. Wearable sensor data is relatively easily collected and provides direct measurements of movement that can be used to develop useful behavioral biomarkers. Sensitive and specific behavioral biomarkers for neurodegenerative diseases are critical to supporting early detection, drug development efforts, and targeted treatments. We create a flexible and descriptive set of features derived from accelerometer and gyroscope data collected from wearable sensors while participants perform clinical assessment tasks, and with them estimate disease status and severity. A short period of data collection ($<$ 5 minutes) yields enough information to effectively separate patients with ataxia from healthy controls with very high accuracy, to separate ataxia from other neurodegenerative diseases such as Parkinson's disease, and to give estimates of disease severity.

Abstract:Objective To examine the structural validity of the Mini-Balance Evaluation Systems Test (Mini-BESTest) in individuals with spinocerebellar ataxia (SCA). Design : Methodological research on data gathered in a cross-sectional study. A Rasch analysis was conducted (partial credit model). Setting : In-patients in a hospital rehabilitation setting. Participants : A pooled sample of patients with SCA (n=65 [total 110 data]; 23 women, 42 men; mean ± SD age 63.1 ± 9.9 yrs). Intervention : Not applicable. Main Outcome Measures : We evaluated the Mini-BESTest's category structure, unidimensionality, and measurement accuracy (0: unable to perform or requiring help, to 2: normal performance). Results : The Mini-BESTest rating scale fulfilled the category functioning criteria. The analysis of the standardized Rasch residuals showed the scale's unidimensionality, but there were seven-item pairs indicating local dependence. All of the items fit the underlying scale construct (dynamic balance) with the exception of item #1, "Sit to stand," which was an underfit. The Mini-BESTest demonstrated adequate reliability (person separation reliability = 0.87) and separated the patients into five strata. The item-difficulty measures ranged from −4.49 to 2.02 logits, and the person ability-item difficulty matching was very good (the mean of person ability = −0.07 logits and the mean of item difficulty = 0.00). No floor or ceiling effects were detected. The keyform identified items with small (#11, "Walk with head turns, horizontal") and large (#3, "Stand on one leg") item thresholds. Conclusions : The Mini-BESTest has a unidimensional balance assessment scale with good category structure and reliability even for individuals with SCA. However, it also has some inherent shortcomings such as fit statistics, local item dependencies, and item thresholds. The results obtained when the Mini-BESTest is administered to patients with cerebellar ataxia should thus be interpreted cautiously.

Abstract:Objectives To assess the neurocognitive performance while maintaining balance of patients experiencing CAI compared to healthy controls. In patients with CAI, the affected limb was also compared to the contralateral limb. Design A retrospective case-control study. Setting Laboratory study. Participants We included 27 patients with CAI and 21 healthy controls. Methods The study consisted of two sessions, namely familiarisation and experimentation, which were scheduled with a gap of at least one week between them. During the experimental trial, both groups performed the Y-Balance Test and Reactive Balance Test once on each limb. Main outcome measures The main outcome measures are accuracy and visuomotor response time (VMRT) calculated via video-analysis and with the FitlightTM-hardware and software respectively during the Reactive Balance Test (RBT). Results No data was excluded from the final analysis. Patients with CAI exhibited significantly lower accuracy than healthy controls, with a mean difference of 8.7% (±3.0)%. There were no differences for VMRT between groups. Additionally, no significant differences were observed between the affected and contralateral limb of the patient group for both accuracy and VMRT. Conclusions Patients with CAI showed lower accuracy, but similar VMRT compared to healthy controls during a neurocognitive balance task, indicating impaired neurocognitive function. Patients exhibit comparable speed to healthy individuals when completing neurocognitive balance tasks, yet they display a higher frequency of accuracy errors in accurately perceiving their environment and making decisions under time constraints. Future research should gain more insights in which other cognitive domains are affected in patients with CAI for a better grasp of this condition's underlying mechanism.

Abstract:INTRODUCTION: Gait and balance impairment are among the cardinal symptoms of Degenerative Cervical Myelopathy, however, utilizing objective measurements of these functions is highly challenging. METHODS: A prospective cross-sectional cohort study with manual and quantitative gait/balance testing using a Protokinetics Zeno Walkway electronic pressure mat (self/fast-paced, tandem gait/stance, Romberg, one foot standing). RESULTS: 95 DCM and 78 healthy subjects were included. For quantitave gait assesment, Velocity (p = 4.56x 10-8, 3.39x10-7), Gait Variability Index (p = 1.788x10-8, p = 0.00169), and stability ratio, single stance time/ double stance time, (p = 4.167x10^-7, p = 1.438x10^-5) were significantly different between groups for both self and fast-paced gait assessments, respectively. Balance was assessed by the variability of the center of pressure (COP). The standard deviation variability of COP in both the X (p = 0.00171) and Y directions (p = 0.000104) and cadence (p = 0.00175) were significant for tandem gait, walking heel-to-toe. Standing on either the Left or Right foot also showed a difference in the variability of COP in both directions X (p = 4.21x10^-7 Left Foot, p = 8.27x10^-7 Right Foot) and Y (p = 6.64x10^-4 Left Foot, p = 1.32x10^-4 Right Foot). On, manual assesment, DCM patients scored significantly worse in all parameters such as self-paced (p = 5.54x10^-2), fast-paced (p = 6.28x10^-2), tandem gait (p = 3.60x10^-8), Romberg eyes open (p = 1.58x10^-2), Romberg eyes closed (p = 1.90x10^-2), L foot stance (p = 6.78x10^-6), R foot stance (2.1x10^-6), and tandem stance (p = 8.83x10^-5). CONCLUSIONS: Quantitative parameters from an electronic pressure mat can be used to assess gait and balance impairment with high sensitivity and strong correlation with subjective measures. Manual scoring of tandem gait, standing on one foot, and tandem stance also provided robust differences and can be easily adopted into routine assessment of DCM patients.

Automated Romberg Test: Leveraging a CNN and Centre of Mass Analysis for Sensory Ataxia Diagnosis

Abstract:This paper proposes a novel method to diagnose sensory ataxia via an automated Romberg Test - the current de facto medical procedure used to diagnose this condition. It utilizes a convolutional neural network to predict joint locations, used for the calculation of various bio-mechanical markers such as the center of mass of the subject and various joint angles. This information is used in combination with data filtering techniques such as Kalman Filters, and center of mass analysis which helped make accurate inferences about the relative weight distribution in the lateral and anterior-posterior axes, and provide an objective, mathematically based diagnosis of this condition. In order to evaluate the performance of this method, testing was performed using dual weight scales and pre-annotated diagnosis videos taken from medical settings. These two methods both quantified the veritable weight distribution upon the ground surface with a ground truth and provided a real-world estimate of accuracy for the proposed method. A mean absolute error of 0.2912 percent was found for the calculated relative weight distribution difference, and an accuracy of 83.33 percent was achieved on diagnoses.

Abstract:To date, the human balance assessment is mainly achieved by measuring the center of pressure (COP) trajectory of upright posture. However, COP-based metrics provide inconclusive results under certain circumstances, due to the lack of relevance with the body balance mechanism. This paper developed a novel framework to measure the variability in body sway based on the Poincaré analysis to enhance the balance assessment. It provided nonlinear parameters SD1, SD2, and their ratio SD1/SD2 as quantitative indicators of the local and global balance regulation capacities. 136 healthy adults were categorized into young and elderly groups and received test on a force platform. COP signals were recorded for 30 s under eyes-opened and eyes-closed vision conditions. The results were analyzed using the two-way mixed-design analysis of variance and show that the technology is able to reveal the effect of age and vision on the balance control by SD1, SD2, and SD1/SD2. SD1 mainly responded to vision, and SD2 was more influenced by age. Due to the information about the subtle adjustment during quiet standing, these Poincaré indexes surpassed the conventional COP measurements in demonstrating the balance control behavior. It can be concluded that the supposed method of estimating the local and global variabilities is a valuable approach for the balance assessment.

Abstract:Background Context Cervical spondylotic myelopathy (CSM) is a degenerative disease caused by cervical cord compression and can lead to the significant impairment of motor function including gait and balance disturbances and changes in lower extremity muscle activity. Purpose This study aimed to characterize gait, balance and lower extremity muscle activity in patients with CSM compared to age-matched healthy controls (HCs) using wearable sensors in the clinical setting. Study Design Nonrandomized, prospective cohort study. Patient Sample Ten CSM patients and 10 age-matched HCs were recruited for this study. Outcome Measures Gait and balance function parameters contained spatial temporal parameters, step regularity (SR1), stride regularity (SR2) and harmonic ratio (HR). EMG muscle activity parameters included time to peak and peak value during loading, stance, and swing phase. Methods In this study, parameters of gait and balance function were extracted using triaxial accelerometer attached to the spinous processes of Lumbar 5 while participants performed an overground walking at a self-preferred speed. Moreover, muscular activity was simultaneously recorded via sEMG sensors attached to tibialis anterior (TA), rectus femoris (RF), bicep femoris (BF), and gastrocnemius lateral (GL). Independent sample t test was used to find the differences between CSM patients and HCs. Results Gait analysis showed cadence, step length and walking speed were statistically significantly lower in CSM patients than HCs. Stride time was significantly higher for CSM patients in comparison to HCs. Lower root mean square ratio (RMSR) of acceleration in the mediolateral (ML) direction, HR in the anteroposterior (AP) direction, SR1 in the AP direction and SR2 in all three directions were observed in CSM patients. For muscle activity analysis, EMG RMS for TA and RF during loading phase and RMS for GL during midstance phase was significantly lower for CSM patients, while significantly higher value was observed for RF RMS during midstance phase and GL RMS during swing phase in CSM patients. Conclusion Our pilot study shows that wearable sensors are able to detect the changes of gait, balance and lower extremity muscle activities of CSM patients in the clinical setting. This pilot study sets the stage for future researches on the diagnosis and monitor progression of CSM disease using wearable technology.

Abstract:Introduction: Ataxia is a neurological symptom that causes decreased balance, loss of coordination, and gait alterations. Innovative rehabilitation devices like virtual reality (VR) systems can provide task-oriented, repetitive and intensive training with multisensorial feedback, thus promoting neuroplastic processes. Among these VR technologies, the Computer Assisted Rehabilitation ENvironment (CAREN) associates a split belt treadmill on a 6-degrees of freedom platform with a 180° VR screen and a Vicon motion capture system to monitor patients' movements during training sessions. Methods: Eight patients affected by cerebellar ataxia were enrolled and received 20 sessions of CAREN training in addition to standard rehabilitation treatment. Each patient was evaluated at the beginning and at the end of the study with 3D gait analysis and clinical scales to assess balance, gait function and risk of falls. Results: We found improvements in kinematic, kinetic, and electromyographic parameters (as per pre-post- CAREN training), as well as in clinical outcomes, such as balance and risk of falls in ataxic patients. In addition, we found that trunk rotation improved, after CAREN intervention, approximating to the normative values. Discussion: Our results suggested that CAREN might be useful to improve specific biomechanical parameters of gait in ataxic patients.

Abstract:Balance ability assessment is crucial for screening mobility decline group and introducing early interventions. However, it is a big challenge to identify the population with moderate balance problems. Force platform test provides a tool for acquiring and analyzing the Center-of-Pressure (COP) signal of the human body. In this paper, the envelope curve area (ECA), and trajectory mean velocity (TMV) of COP signal were extracted as the magnitude and responsiveness parameters of COP. The tests on 136 subjects indicate that ECA and TMV offer comprehensive and efficient indices for assessing balance ability and suggest that the most reliable results are achieved under eyes opened condition.

Abstract:This study aimed to assess the responsiveness to the rehabilitation of three trunk acceleration-derived gait indexes, namely the harmonic ratio (HR), the short-term longest Lyapunov's exponent (sLLE), and the step-to-step coefficient of variation (CV), in a sample of subjects with primary degenerative cerebellar ataxia (swCA), and investigate the correlations between their improvements (∆), clinical characteristics, and spatio-temporal and kinematic gait features. The trunk acceleration patterns in the antero-posterior (AP), medio-lateral (ML), and vertical (V) directions during gait of 21 swCA were recorded using a magneto-inertial measurement unit placed at the lower back before (T0) and after (T1) a period of inpatient rehabilitation. For comparison, a sample of 21 age- and gait speed-matched healthy subjects (HS matched ) was also included. At T1, sLLE in the AP (sLLE AP ) and ML (sLLE ML ) directions significantly improved with moderate to large effect sizes, as well as SARA scores, stride length, and pelvic rotation. sLLE ML and pelvic rotation also approached the HS matched values at T1, suggesting a normalization of the parameter. HRs and CV did not significantly modify after rehabilitation. ∆sLLE ML correlated with ∆ of the gait subscore of the SARA scale (SARA GAIT ) and ∆stride length and ∆sLLE AP correlated with ∆pelvic rotation and ∆SARA GAIT . The minimal clinically important differences for sLLE ML and sLLE AP were ≥ 36.16% and ≥ 28.19%, respectively, as the minimal score reflects a clinical improvement in SARA scores. When using inertial measurement units, sLLE AP and sLLE ML can be considered responsive outcome measures for assessing the effectiveness of rehabilitation on trunk stability during walking in swCA.

Abstract:The aim of this study was to determine the time between the first detection of postural control impairments and the evident manifestation of ataxia in preclinical SCA1 individuals. Twenty five preclinical SCA1 mutation carriers: 13 with estimated disease onset ≤ 6 years (SCA1 +) aged 27.8 ± 8.1 years; 12 with expected disease onset > 6 years (SCA1-) aged 26.6 ± 3.1 years and 26 age and sex matched healthy controls (HCs) underwent static posturography during 5 years of observation. The movements of the centre of feet pressure (COP) during quiet standing with eyes open (EO) and closed (EC) were quantified by calculating the mean radius (R), developed surface area (A) and mean COP movement velocity (V). Ataxia was evaluated by use of the Scale for Assessment and Rating of Ataxia (SARA).

Abstract:Balance ability is one of the important factors in measuring human physical fitness and a common index for evaluating sports performance. Its quality directly affects the coordination ability of human movements and plays an important role in human productive activities. In the field of sports, balance ability is an important indicator of athletes' selection and training. How to objectively analyze balance performance becomes a problem for every non-professional sports enthusiast. Therefore, in this paper, we used a dataset of lower limb collected by inertial sensors to extract the feature parameters, then designed a RUS Boost classifier for unbalanced data whose basic classifier was SVM model to predict three classifications of balance degree, and, finally, evaluated the performance of the new classifier by comparing it with two basic classifiers (KNN, SVM). The result showed that the new classifier could be used to evaluate the balanced ability of lower limb, and performed higher than basic ones (RUS Boost: 72%; KNN: 60%; SVM: 44%). The results meant the established classification model could be used for and quantitative assessment of balance ability in initial screening and targeted training.

Abstract:Background: Similar to a modified star excursion balance test, the Y-balance test is recommended for use in clinical settings to evaluate dynamic balance, particularly in athletes with chronic ankle instability. However, due to the testing errors, there are certain restrictions. As a result, the modification of the centre of mass tracking system was developed in order to aid in the detection of the ability to control the dynamic balance. Therefore, the purpose of this study was to correlate the usage of an accelerometer for the shifting of the centre of mass during a dynamic balance test with a Y-balance test reach distance score.Methodology: Forty professional football athletes with CAI participated in this study by performing the Y-balance test three times while wearing an accelerometer. The jerk, RMS sway amplitude, mean velocity from the time domain, and the normalised reach distance scores of the Y-balance test in the anterior, posteromedial, and posterolateral directions were all collected.Results: There was a strong positive correlation of jerk and RMS sway amplitude with the normalised reach distance scores in the posteromedial direction (r = 0.706 and 0.777, respectively), a moderate positive correlation of jerk and RMS sway amplitude with the normalised reach distance scores in the posterolateral direction (r = 0.609 and 0.606, respectively), a moderate positive correlation of jerk and RMS sway amplitude with the composite reach distance scores (r = 0.531 and 0.573, respectively) and significant differences in the posteromedial, posterolateral and overall directions (p-value < 0.001).Conclusion: These findings indicate that the area of the centre of mass shifting as represented by the accelerometer can disclose the body's ability to control the centre of mass over the base of support when the body is moving. Furthermore, in this study, the RMS sway variable in the posteromedial direction appears to be the most prominent.
Impaired Lower Limb Proprioception in Spinocerebellar Ataxia Type 3 and Its Affected Factors

Abstract:Background Spinocerebellar ataxia type 3 (SCA3) is one of the most common hereditary neurodegenerative diseases. Postural control dysfunction is the main symptom of SCA3, and the proprioceptive system is a critical sensory component of postural control. Accordingly, proprioception quantification assessment is necessary in monitoring the progression of SCA3. Objective We aimed to quantitatively assess lower limb proprioception and investigate the relationship between proprioception and clinical characteristics in patients with SCA3. Methods A total of 80 patients with SCA3 and 62 health controls were recruited, and their lower limb proprioception was measured using the Pro-kin system. Clinical characteristics of the SCA3 patients were collected. Multivariable linear regression was used to investigate potential affected factors for lower limb proprioception. Results We found that the patients with SCA3 experience poorer lower limb proprioception characterized by significant impairment in the average trace error (ATE) and time to carry out the test time execution (TTE) compared to controls ( P < 0.05). Moreover, there were significant differences in TTE between the right and left lower limbs ( P < 0.05) of the patients. Regression analyses revealed that increasing age at onset (AAO) predicts poorer lower limb proprioception for both ATE (β = 2.006, P = 0.027) and TTE (β = 1.712, P = 0.043) and increasing disease duration predicts poorer lower limb proprioception for ATE (β = 0.874, P = 0.044). AAO (β = 0.328, P = 0.019) along with the expanded alleles (β = 0.565, P = 0.000) could affect the severity of ataxia. By contrast, ATE (β = 0.036, P = 0.800) and TTE (β = −0.025, P = 0.862) showed no significant predictors. Conclusions Lower limb proprioception in patients with SCA3 is significantly impaired when compared to healthy controls. Increasing AAO and disease duration are related to impaired lower limb proprioception.

Abstract:Ataxia rating scales are observer administered clinical outcome assessments (COAs) of the cerebellar motor syndrome. It is not known whether these COAs mirror patient experience of their disease. Here we test the hypothesis that ataxia COAs are related to and reflect patient reported symptoms and impact of illness. A concept library of symptoms and activities impacted by ataxia was created by reviewing (a) concept elicitation data from surveys completed by 147 ataxia patients and 80 family members and (b) cognitive debrief data from focus groups of 17 ataxia patients used to develop the Patient Reported Outcome Measure of Ataxia. These findings were mapped across the items on 4 clinical measures of ataxia (SARA, BARS, ICARS and FARS). Symptoms reported most commonly related to balance, gait or walking, speech, tremor and involuntary movements, and vision impairment. Symptoms reported less frequently related to hand coordination, loss of muscle control, dizziness and vertigo, muscle discomfort or pain, swallowing, and incontinence. There was a mosaic mapping of items in the observer-derived ataxia COAs with the subjective reports by ataxia patients/families of the relevance of these items to their daily lives. Most COA item mapped onto multiple real-life manifestations; and most of the real-life impact of disease mapped onto multiple COA items. The 4 common ataxia COAs reflect patient reported symptoms and impact of illness. These results validate the relevance of the COAs to patients' lives and underscore the inadvisability of singling out any one COA item to represent the totality of the patient experience.
