Abstract:The first outbreak of dengue occurred in Madeira Island on 2012, featuring one virus serotype. Aedes aegypti was the vector of the disease and it is unlikely that it will be eliminated from the island. Therefore, a new outbreak of dengue fever can occur and, if it happens, risk to the population increases if two serotypes coexist. In this paper, mathematical modeling and numerical simulations are carried out to forecast what may happen in Madeira Island in such scenario.

Abstract:Various ocular manifestations have been described in Wegener's granulomatosis. Orbital involvement is common and manifestations in the anterior segment of the eye such as keratitis, scleritis, and conjunctivitis may occur. Involvement of the posterior segment presenting with major vascular occlusions or anterior ischemic neuropathy is rare. This case report presents a patient with bilateral cotton-wool spots at the posterior pole associated with histologically proven Wegener's granulomatosis. This finding, as an initial ocular manifestation of the disease, is very uncommon and may be interpreted as focal retinal inflammatory vasculitis. This was supported by the rapid positive response to systemic combined immunosuppressive and steroidal treatment during a 30-month period of review. The presence of cotton-wool spots in association with systemic signs of the disease should be considered in establishing the diagnosis of Wegener's granulomatosis.

Abstract:Dengue is a viral infection transmitted mainly by the Aedes aegypti mosquito and to a lesser extent by the Aedes albopictus. This infectious disease generally causes flu-like symptoms, but it can also lead to life-threatening symptoms. Unfortunately, the number of cases increases every year and about a third part of the world’s population is at risk of contracting this disease. To generate tools capable of containing dengue transmission, we present a novel stochastic model for the Aedes aegypti life cycle and the dengue virus transmission, taking into account all the mechanisms of transmission and parameters estimated experimentally to date. This new model describes in detail all the interactions in the stages of the life cycle of the mosquito. It also considers the environmental conditions, i.e., the breeding sites and the temperature, which are very important factors for the mosquito survival. The results show that the contagion by bite only does not provoke an epidemic outbreak when five infected, pregnant, and fed females, looking for lay eggs, arrive to a susceptible human population. However, if in addition to the bite transmission, the virus is also transmitted in vertical transmission and sexual ways, then an outbreak arises. Altogether, the transmission mechanisms and the adequate environmental conditions could explain the virus persistence in the population. Under these conditions and by considering fumigation as a way to control the mosquito population, in this new model the outbreak and the virus propagation could be avoided—but only if the control is implemented within the first two weeks of the presence of the virus.

Abstract:The influence of climate on mosquito-borne diseases like dengue and chikungunya is well-established, but comprehensively tracking long-term spatial and temporal trends across large areas has been hindered by fragmented data and limited analysis tools. This study presents an unprecedented analysis, in terms of breadth, estimating the SIR transmission parameters from incidence data in all 5,570 municipalities in Brazil over 14 years (2010-2023) for both dengue and chikungunya. We describe the Episcanner computational pipeline, developed to estimate these parameters, producing a reusable dataset describing all dengue and chikungunya epidemics that have taken place in this period, in Brazil. The analysis reveals new insights into the climate-epidemic nexus: We identify distinct geographical and temporal patterns of arbovirus disease incidence across Brazil, highlighting how climatic factors like temperature and precipitation influence the timing and intensity of dengue and chikungunya epidemics. The innovative Episcanner tool empowers researchers and public health officials to explore these patterns in detail, facilitating targeted interventions and risk assessments. This research offers a new perspective on the long-term dynamics of climate-driven mosquito-borne diseases and their geographical specificities linked to the effects of global temperature fluctuations such as those captured by the ENSO index.

Abstract:Dengue is an infectious disease that is transmitted by the Aedes aegypti mosquito. Each stage of the life cycle is influenced by climate variation. The transmission of the dengue virus can be related to increased mosquito survival due to rain and temperature conditions that are optimal for the mosquito’s maturation. The aim of this paper is to propose a mathematical model to study how temperature influences each stage of the mosquito’s life cycle dynamics by representing transitions and death rates as an explicit function of temperature. The model is thus able to show the influence of temperature on dengue transmission. It can also be used as an operational tool due to its simplicity regarding data requirements and computational effort. The model demonstrates that an expected increase in global temperature will influence the mosquito’s life cycle and, consequently, increase the incidence of dengue cases in areas that were previously free from the disease.

Abstract:Dengue is a climate-sensitive mosquito-borne disease with increasing geographic extent and human incidence. Although the climate-epidemic association and outbreak risks have been assessed using both statistical and mathematical models, local mosquito population dynamics have not been incorporated in a unified predictive framework. Here, we use mosquito surveillance data from 2005 to 2015 in China to integrate a generalized additive model of mosquito dynamics with a susceptible-infected-recovered (SIR) compartmental model of viral transmission to establish a predictive model linking climate and seasonal dengue risk. The findings illustrate that spatiotemporal dynamics of dengue are predictable from the local vector dynamics, which in turn, can be predicted by climate conditions. On the basis of the similar epidemiology and transmission cycles, we believe that this integrated approach and the finer mosquito surveillance data provide a framework that can be extended to predict outbreak risk of other mosquito-borne diseases as well as project dengue risk maps for future climate scenarios.

Abstract:System Dynamics (SD) models have been used to understand complex, multi-faceted dengue transmission dynamics, but a gap persists between research and actionable public health tools for decision-making. Spain is an at-risk country of imported dengue outbreaks, but only qualitative assessments are available to guide public health action and control. We propose a modular SD model combining temperature-dependent vector population, transmission parameters, and epidemiological interactions to simulate outbreaks from imported cases accounting for heterogeneous local climate-related transmission patterns. Under our assumptions, 15 provinces sustain vector populations capable of generating outbreaks from imported cases, with heterogeneous risk profiles regarding seasonality, magnitude and risk window shifting from late Spring to early Autum. Results being relative to given vector-to-human populations allow flexibility when translating outcomes between geographic scales. The model and the framework are meant to serve public health by incorporating transmission dynamics and quantitative-qualitative input to the evidence-based decision-making chain. It is a flexible tool that can easily adapt to changing contexts, parametrizations and epidemiological settings thanks to the modular approach.

Abstract:Vectors of Dengue, Chikungunya, Zika, and Yellow Fever are emerging in new areas, posing increasing public health risks. Aedes albopictus, a key vector for these diseases, is expanding its range beyond its tropical and subtropical origins, driven by suitable climate, population mobility, trade, and urbanization. Since its introduction to Europe, Ae. albopictus has rapidly spread and triggered recurrent outbreaks. Past model attempts have handled vector suitability and vector introduction as independent drivers. Here we develop a novel, highly predictive spatio-temporal vector diffusion model based on minimum temperature, median temperature, relative humidity and human population as predictors. The model predicts areas of presence or absence with an accuracy of 99% and 79% for new established vector populations. The model explains how short- and long-range spread of Ae. albopictus interacts with vector suitability. These results show that the expansion of Ae. albopictus in Europe is predictable and closely linked to climate suitability, and human population. The new model integrates in one simultaneous model framework the climate and mobility drivers, providing a better basis for anticipating future outbreaks in situations of dependent interacting co-drivers.

Imported dengue in Spain: a nationwide analysis with predictive time series analyses

Abstract:Background: Of febrile illnesses in Europe, dengue is second only to malaria as a cause of travellers being hospitalized. Local transmission has been reported in several European countries, including Spain. This study assesses the evolution of dengue-related admissions in Spain in terms of time, geographical distribution and individuals' common characteristics; it also creates a predictive model to evaluate the risk of local transmission. Methods: This is a retrospective study using the Hospital Discharge Records Database from 1997 to 2016. We calculated hospitalization rates and described clinical characteristics. Spatial distribution and temporal behaviour were also assessed, and a predictive time series model was created to estimate expected cases in the near future. Figures for resident foreign population, Spanish residents' trips to endemic regions and the expansion of Aedes albopictus were also evaluated. Results: A total of 588 dengue-related admissions were recorded: 49.6% were women, and the mean age was 34.3 years. One person died (0.2%), 82% presented with mild-to-moderate dengue and 7-8% with severe dengue. We observed a trend of steady and consistent increase in incidence (P < 0.05), in parallel with the increase in trips to dengue-endemic regions. Most admissions occurred during the summer, showing significant seasonality with 3-year peaks. We also found important regional differences. According to the predictive time series analysis, a continuing increase in imported dengue incidence can be expected in the near future, which, in the worst case scenario (upper 95% confidence interval), would mean an increase of 65% by 2025. Conclusion: We present a nationwide study based on hospital, immigration, travel and entomological data. The constant increase in dengue-related hospitalizations, in combination with wider vector distribution, could imply a higher risk of autochthonous dengue transmission in the years to come. Strengthening the human and vector surveillance systems is a necessity, as are improvements in control measures, in the education of the general public and in fostering their collaboration in order to reduce the impact of imported dengue and to prevent the occurrence of autochthonous cases.

Abstract:In this paper we present a model to estimate the density of aedes mosquitoes in a community affected by dengue. The model is based on the fitting of a continuous function to the incidence of dengue infections, from which the density of infected mosquitoes is derived straightforwardly. Further derivations allows the calculation of the latent and susceptible mosquitoes' densities, the sum of the three equals the total mosquitoes' density. The model is illustrated with the case of the risk of urban yellow fever resurgence in dengue infested areas but the same methods apply for other aedes-transmitted infections like Zika and chikungunya viruses.

Abstract:In the last decades, dengue has become one of the most widespread mosquito-borne arboviruses in the world, with an increasing incidence in tropical and temperate regions. The mosquito Aedes aegypti is the dengue primary vector and is more abundant in highly urbanized areas. Traditional vector control methods have showing limited efficacy in sustaining mosquito population at low levels to prevent dengue virus outbreaks. Considering disease transmission is not evenly distributed in the territory, one perspective to enhance vector control efficacy relies on identifying the areas that concentrate arbovirus transmission within an endemic city, i.e., the hotspots. Herein, we used a 13-month timescale during the SARS-Cov-2 pandemic and its forced reduction in human mobility and social isolation to investigate the spatiotemporal association between dengue transmission in children and entomological indexes based on adult Ae. aegypti trapping. Dengue cases and the indexes Trap Positive Index (TPI) and Adult Density Index (ADI) varied seasonally, as expected: more than 51% of cases were notified on the first 2 months of the study, and higher infestation was observed in warmer months. The Moran's Eigenvector Maps (MEM) and Generalized Linear Models (GLM) revealed a strong large-scale spatial structuring in the positive dengue cases, with an unexpected negative correlation between dengue transmission and ADI. Overall, the global model and the purely spatial model presented a better fit to data. Our results show high spatial structure and low correlation between entomological and epidemiological data in Foz do Iguaçu dengue transmission dynamics, suggesting the role of human mobility might be overestimated and that other factors not evaluated herein could be playing a significant role in governing dengue transmission.

Abstract:After their re-emergence in the last decades, dengue fever and other vector-borne diseases are a potential threat to the lives of millions of people. Based on a data set of dengue cases in the Brazilian city of Fortaleza, collected from 2011 to 2016, we study the spatio-temporal characteristics of dengue outbreaks to characterize epidemic and non-epidemic years. First, we identify regions that show a high prevalence of dengue cases and mosquito larvae in different years and also analyze their corresponding correlations. Our results show that the characteristic correlation length of the epidemic is of the order of the system size, suggesting that factors such as citizen mobility may play a major role as a drive for spatial spreading of vector-borne diseases. Inspired by this observation, we perform a mean-field estimation of the basic reproduction number and find that our estimated values agree well with the values reported for other regions, pointing towards similar underlying spreading mechanisms. These findings provide insights into the spreading characteristics of dengue in densely populated areas and should be of relevance for the design of improved disease containment strategies.

Abstract:Dengue virus (DENV) infection is endemic in many places of the tropical and subtropical regions, which poses serious public health threat globally. We develop and analyze a mathematical model to study the transmission dynamics of the dengue epidemics. Our qualitative analyzes show that the model has two equilibria, namely the disease-free equilibrium (DFE) which is locally asymp- totically stable when the basic reproduction number (R0) is less than one and unstable if R0 > 1, and endemic equilibrium (EE) which is globally asymp-totically stable when R0 > 1. Further analyzes reveals that the model exhibit the phenomena of backward bifurcation (BB) (a situation where a stable DFE co-exists with a stable EE even when the R0 > 1), which makes the disease control more diffi-cult. The model is applied to the real dengue epidemic data in Kaohsiung and Tainan cities in Taiwan, China to evaluate the fitting performance. We propose two reconstruction approaches to estimate the time-dependent R0 , and we find a consistent fitting results and equivalent goodness-of-fit. Our findings highlight the similarity of the dengue outbreaks in the two cities. We find that despite the proximity in Kaohsiung and Tainan cities, the estimated transmission rates are neither completely synchronized, nor periodically in-phase perfectly in the two cities. We also show the time lags between the seasonal waves in the two cities likely occurred. It is further shown via sensitivity analysis result that proper sanitation of the mosquito breeding sites and avoiding the mosquito bites are the key control measures to future dengue outbreaks in Taiwan.

Abstract:BackgroundDengue is a vector-borne disease caused by the dengue virus (DENV). Despite the crucial role of Aedes mosquitoes in DENV transmission, pure vector indices poorly correlate with human infections. Therefore there is great need for a better understanding of the spatial and temporal scales of DENV transmission between mosquitoes and humans. Here, we have systematically monitored the circulation of DENV in individual Aedes spp. mosquitoes and human patients from Caratinga, a dengue endemic city in the state of Minas Gerais, in Southeast Brazil. From these data, we have developed a novel stochastic point process pattern algorithm to identify the spatial and temporal association between DENV infected mosquitoes and human patients.MethodsThe algorithm comprises of: (i) parameterization of the variogram for the incidence of each DENV serotype in mosquitoes; (ii) identification of the spatial and temporal ranges and variances of DENV incidence in mosquitoes in the proximity of humans infected with dengue; and (iii) analysis of the association between a set of environmental variables and DENV incidence in mosquitoes in the proximity of humans infected with dengue using a spatio-temporal additive, geostatistical linear model.ResultsDENV serotypes 1 and 3 were the most common virus serotypes detected in both mosquitoes and humans. Using the data on each virus serotype separately, our spatio-temporal analyses indicated that infected humans were located in areas with the highest DENV incidence in mosquitoes, when incidence is calculated within 2.5–3 km and 50 days (credible interval 30–70 days) before onset of symptoms in humans. These measurements are in agreement with expected distances covered by mosquitoes and humans and the time for virus incubation. Finally, DENV incidence in mosquitoes found in the vicinity of infected humans correlated well with the low wind speed, higher air temperature and northerly winds that were more likely to favor vector survival and dispersal in Caratinga.ConclusionsWe have proposed a new way of modeling bivariate point pattern on the transmission of arthropod-borne pathogens between vector and host when the location of infection in the latter is known. This strategy avoids some of the strong and unrealistic assumptions made by other point-process models. Regarding virus transmission in Caratinga, our model showed a strong and significant association between high DENV incidence in mosquitoes and the onset of symptoms in humans at specific spatial and temporal windows. Together, our results indicate that vector surveillance must be a priority for dengue control. Nevertheless, localized vector control at distances lower than 2.5 km around premises with infected vectors in densely populated areas are not likely to be effective.

Abstract:Epidemiological models may give some basic guidelines for public health practitioners, allowing the analysis of issues that can influence the strategies to prevent and fight a disease. To be used in decision making, however, a mathematical model must be carefully parameterized and validated with epidemiological and entomological data. Here an SIR (S for susceptible, I for infectious, and R for recovered individuals) and ASI (A for the aquatic phase of the mosquito, S for susceptible, and I for infectious mosquitoes) epidemiological model describing a dengue disease is presented, as well as the associated basic reproduction number. A sensitivity analysis of the epidemiological model is performed in order to determine the relative importance of the model parameters to the disease transmission.

Abstract:Dengue is a neglected tropical vector-borne disease that is spreading rapidly and increasing worldwide. Climate change has been considered one of the primary factors contributing to this escalation. In Brazil, a vast and heterogeneous country, each epidemic has set new records, with the disease currently spreading widely across the country. In this study, we used predictive modeling techniques, grounded on a Bayesian hierarchical framework, to forecast the spatio-temporal dynamics of dengue in Brazil under future climate scenarios. We used monthly historical data on dengue incidence from 2000 to 2021, collected over 557 Brazilian microregions, to learn about its association with population, environmental, climatic, and socioeconomic conditions. By integrating projections of precipitation, minimum temperature and population based on different Shared Socioeconomic Pathway scenarios, we obtained spatially resolved projections of dengue incidence for 2021-2040 and 2041-2060. The model’s predictive performance was ensured via robust cross-validation. We observed an increase in dengue incidence with rising precipitation, urban infrastructure, and Normalized Difference Water Index, and a decrease with increas-ing elevation and deprivation index. Minimum temperature exhibited a nonlinear and positive association behavior. Our forecasts show great regional variability. In the south region, some traditionally non-endemic microregions are expected to see a clear increase in dengue incidence rate. Conversely, other regions show geographic variability of patterns, suggesting that in some cases elevated temperatures may exceed the viable threshold for dengue transmission. Investing in reducing greenhouse gas emissions and implementing measures to address the rising incidence of dengue due to climate change is crucial.

Abstract:As the world's fastest spreading vector-borne disease, dengue was estimated to infect more than 390 million people in 2010, a 30-fold increase in the past half century. Although considered to be a non-endemic country, mainland China had 55,114 reported dengue cases from 2005 to 2014, of which 47,056 occurred in 2014. Furthermore, 94% of the indigenous cases in this time period were reported in Guangdong Province, 83% of which were in Guangzhou City. In order to determine the possible determinants of the unprecedented outbreak in 2014, a population-based deterministic model was developed to describe dengue transmission dynamics in Guangzhou. Regional sensitivity analysis (RSA) was adopted to calibrate the model and entomological surveillance data was used to validate the mosquito submodel. Different scenarios were created to investigate the roles of the timing of an imported case, climate, vertical transmission from mosquitoes to their offspring, and intervention. The results suggested that an early imported case was the most important factor in determining the 2014 outbreak characteristics. Precipitation and temperature can also change the transmission dynamics. Extraordinary high precipitation in May and August, 2014 appears to have increased vector abundance. Considering the relatively small number of cases in 2013, the effect of vertical transmission was less important. The earlier and more frequent intervention in 2014 also appeared to be effective. If the intervention in 2014 was the same as that in 2013, the outbreak size may have been over an order of magnitude higher than the observed number of new cases in 2014.The early date of the first imported and locally transmitted case was largely responsible for the outbreak in 2014, but it was influenced by intervention, climate and vertical transmission. Early detection and response to imported cases in the spring and early summer is crucial to avoid large outbreaks in the future.
