Transcranial Photobiomodulation Treatment: Significant Improvements in Four Ex-Football Players with Possible Chronic Traumatic Encephalopathy

Abstract:Background: Chronic traumatic encephalopathy, diagnosed postmortem (hyperphosphorylated tau), is preceded by traumatic encephalopathy syndrome with worsening cognition and behavior/mood disturbances, over years. Transcranial photobiomodulation (tPBM) may promote improvements by increasing ATP in compromised/stressed cells and increasing local blood, lymphatic vessel vasodilation. Objective: Aim 1: Examine cognition, behavior/mood changes Post-tPBM. Aim 2: MRI changes - resting-state functional-connectivity MRI: salience, central executive, default mode networks (SN, CEN, DMN); magnetic resonance spectroscopy, cingulate cortex. Methods: Four ex-players with traumatic encephalopathy syndrome/possible chronic traumatic encephalopathy, playing 11- 16 years, received In-office, red/near-infrared tPBM to scalp, 3x/week for 6 weeks. Two had cavum septum pellucidum. Results: The three younger cases (ages 55, 57, 65) improved 2 SD (p < 0.05) on three to six neuropsychological tests/subtests at 1 week or 1 month Post-tPBM, compared to Pre-Treatment, while the older case (age 74) improved by 1.5 SD on three tests. There was significant improvement at 1 month on post-traumatic stress disorder (PTSD), depression, pain, and sleep. One case discontinued narcotic pain medications and had reduced tinnitus. The possible placebo effect is unknown. At 2 months Post-tPBM, two cases regressed. Then, home tPBM was applied to only cortical nodes, DMN (12 weeks); again, significant improvements were seen. Significant correlations for increased SN functional connectivity (FC) over time, with executive function, attention, PTSD, pain, and sleep; and CEN FC, with verbal learning/memory, depression. Increased n-acetyl-aspartate (NAA) (oxygen consumption, mitochondria) was present in anterior cingulate cortex (ACC), parallel to less pain and PTSD. Conclusion: After tPBM, these ex-football players improved. Significant correlations of increased SN FC and CEN FC with specific cognitive tests and behavior/mood ratings, plus increased NAA in ACC support beneficial effects from tPBM.
Traumatic Brain Injury Recovery with Photobiomodulation: Cellular Mechanisms, Clinical Evidence, and Future Potential

Abstract:Traumatic Brain Injury (TBI) remains a significant global health challenge, lacking effective pharmacological treatments. This shortcoming is attributed to TBI's heterogeneous and complex pathophysiology, which includes axonal damage, mitochondrial dysfunction, oxidative stress, and persistent neuroinflammation. The objective of this study is to analyze transcranial photobiomodulation (PBM), which employs specific red to near-infrared light wavelengths to modulate brain functions, as a promising therapy to address TBI's complex pathophysiology in a single intervention. This study reviews the feasibility of this therapy, firstly by synthesizing PBM's cellular mechanisms with each identified TBI's pathophysiological aspect. The outcomes in human clinical studies are then reviewed. The findings support PBM's potential for treating TBI, notwithstanding variations in parameters such as wavelength, power density, dose, light source positioning, and pulse frequencies. Emerging data indicate that each of these parameters plays a role in the outcomes. Additionally, new research into PBM's effects on the electrical properties and polymerization dynamics of neuronal microstructures, like microtubules and tubulins, provides insights for future parameter optimization. In summary, transcranial PBM represents a multifaceted therapeutic intervention for TBI with vast potential which may be fulfilled by optimizing the parameters. Future research should investigate optimizing these parameters, which is possible by incorporating artificial intelligence.

Abstract:Traumatic brain injury (TBI) is a common cause of neurologic morbidity for which few effective therapies exist, especially during the chronic stage. A potential therapy for chronic TBI is transcranial photobiomodulation (tPBM). tPBM is a noninvasive neuromodulation technique that uses light to stimulate the cortex and increase blood flow and metabolism while also enhancing cognition and improving affect. There has been much work focusing on the efficacy of tPBM in acute TBI in small animals, but much less work has focused on chronic TBI. Patients with chronic TBI manifest microvascular injury, which may serve as a modifiable treatment target for tPBM. There is a need to study and improve tPBM, as the currently implemented protocols targeting microvascular injury have been relatively unsuccessful. This review includes 16 studies, which concluded that after tPBM application, there were improvements in neuropsychological outcomes in addition to increases in cerebral blood flow. However, these conclusions are confounded by differing tPBM parameters, small sample sizes, and heterogenous TBI populations. While these results are encouraging, there is a need to further understand the therapeutic potential of tPBM in chronic TBI.

Abstract:Mild traumatic brain injury (mTBI) is a common consequence of head injury but there are no recognized interventions to promote recovery of the brain. We previously showed that photobiomodulation (PBM) significantly reduced the number of apoptotic cells in adult rat hippocampal organotypic slice cultures. In this study, we first optimized PBM delivery parameters for use in mTBI, conducting cadaveric studies to calibrate 660 and 810 nm lasers for transcutaneous delivery of PBM to the cortical surface. We then used an in vivo weight drop mTBI model in adult rats and delivered daily optimized doses of 660, 810 nm, or combined 660/810 nm PBM. Functional recovery was assessed using novel object recognition (NOR) and beam balance tests, whilst histology and immunohistochemistry were used to assess the mTBI neuropathology. We found that PBM at 810, 660 nm, or 810/660 nm all significantly improved both NOR and beam balance performance, with 810 nm PBM having the greatest effects. Histology demonstrated no overt structural damage in the brain after mTBI, however, immunohistochemistry using brain sections showed significantly reduced activation of both CD11b+ microglia and glial fibrillary acidic protein (GFAP)+ astrocytes at 3 days post‐injury. Significantly reduced cortical localization of the apoptosis marker, cleaved caspase‐3, and modest reductions in extracellular matrix deposition after PBM treatment, limited to choroid plexus and periventricular areas were also observed. Our results demonstrate that 810 nm PBM optimally improved functional outcomes after mTBI, reduced markers associated with apoptosis and astrocyte/microglial activation, and thus may be useful as a potential regenerative therapy.

High-frequency repetitive transcranial magnetic stimulation for treating moderate traumatic brain injury in rats: A pilot study

Abstract:Transcranial magnetic stimulation (TMS) is a method of noninvasive brain stimulation that causes neuromodulation by activating neurons or changing excitability in a certain brain area. Considering the known effects of TMS and the pathophysiology of traumatic brain injury (TBI), TMS was proposed to have potential for treating this condition. Moderate TBI was induced in adult male Sprague Dawley rats using Feeney's weight-dropping method. Injured rats were divided into a TMS group and a control group. Repetitive TMS (rTMS) was administered to rats in the TMS group from post-TBI day 2. At post-TBI days 7, 14 and 28, three or four of the rats were sacrificed, and harvested brains were embedded in paraffin and sectioned. Sections were then treated with hematoxylin and eosin and immunohistochemical staining. Three rats from each group underwent fludeoxyglucose F 18 micro-positron emission tomography scanning on post-TBI day 2 and 13. The unexpected mortality rate after injury was lower in the TMS group than in the control group. The modified neurological severity score of the TMS group was lower compared with the control group at post-TBI day 14. According to the results of hematoxylin eosin staining, relative cerebral parenchyma loss was lower at post-TBI day 28 in the TMS group compared with the control group. However, the aforementioned differences were not found to be statistically significant. There was also no significant difference in glucose metabolism between the two groups. According to immunohistochemical staining, the TMS group showed a significantly higher level of proliferation (indicated by bromodeoxyuridine) in the subventricular zone, as compared with the control group (P<0.05). A significantly higher rate of neuron survival at day 28 (P<0.05; indicated by NeuN) and a significantly reduced rate of apoptosis at days 7 and 14 (P<0.05; indicated by caspase-3) were observed in the perilesional zone of the TMS group, as compared with the control group. The current findings suggest that high-frequency rTMS may promote neurogenesis and provide a basis for further studies in this area.

Abstract:Transcranial photobiomodulation (PBM) also known as low level laser therapy (tLLLT) relies on the use of red/NIR light to stimulate, preserve and regenerate cells and tissues. The mechanism of action involves photon absorption in the mitochondria (cytochrome c oxidase), and ion channels in cells leading to activation of signaling pathways, up-regulation of transcription factors, and increased expression of protective genes. We have studied PBM for treating traumatic brain injury (TBI) in mice using a NIR laser spot delivered to the head. Mice had improved memory and learning, increased neuroprogenitor cells in the dentate gyrus and subventricular zone, increased BDNF and more synaptogenesis in the cortex. These highly beneficial effects on the brain suggest that the applications of tLLLT are much broader than at first conceived. Other groups have studied stroke (animal models and clinical trials), Alzheimer's disease, Parkinson's disease, depression, and cognitive enhancement in healthy subjects.

Abstract:Photobiomodulation (PBM) therapy on the brain employs red to near-infrared (NIR) light to treat various neurological and psychological disorders. The mechanism involves the activation of cytochrome c oxidase in the mitochondrial respiratory chain, thereby enhancing ATP synthesis. Additionally, light absorption by ion channels triggers the release of calcium ions, instigating the activation of transcription factors and subsequent gene expression. This cascade of events not only augments neuronal metabolic capacity but also orchestrates anti-oxidant, anti-inflammatory, and anti-apoptotic responses, fostering neurogenesis and synaptogenesis. It shows promise for treating conditions like dementia, stroke, brain trauma, Parkinson's disease, and depression, even enhancing cognitive functions in healthy individuals and eliciting growing interest within the medical community. However, delivering sufficient light to the brain through transcranial approaches poses a significant challenge due to its limited penetration into tissue, prompting an exploration of alternative delivery methods such as intracranial and intranasal approaches. This comprehensive review aims to explore the mechanisms through which PBM exerts its effects on the brain and provide a summary of notable preclinical investigations and clinical trials conducted on various brain disorders, highlighting PBM's potential as a therapeutic modality capable of effectively impeding disease progression within the organism—a task often elusive with conventional pharmacological interventions.

Abstract:Photobiomodulation (PBM) therapy is an innovative treatment for neurological and psychological conditions. Complex IV of the mitochondrial respiratory chain can be stimulated by red light, which increases ATP synthesis. Additionally, the ion channels' light absorption causes the release of Ca2+, which activates transcription factors and changes gene expression. Neuronal metabolism is improved by brain PBM therapy, which also promotes synaptogenesis and neurogenesis as well as anti‐inflammatory. Its depression‐treating potential is attracting attention for other conditions, including Parkinson's disease and dementia. Giving enough dosage for optimum stimulation using the transcranial PBM technique is challenging because of the rapidly increasing attenuation of light transmission in tissue. Different strategies like intranasal and intracranial light delivery systems have been proposed to overcome this restriction. Suprapubic Catheters—A Shared Understanding, From the Other Side Looking In

Abstract:Objective The objective of this study was to explore the experiences of people living with a suprapubic catheter. Method This descriptive study was guided by the philosophy of Gadamerian hermeneutics. Data were gathered via indepth interviews with 6 adults living in the community who had a suprapubic catheter for long-term urinary bladder drainage. Interpretation of the data occurred via thematic analysis of the participants' stories. Results Two distinct but interrelated themes emerged. Participants shifted, over time, from negative to positive experiences (psychologically, physically, and practically) to adjustment to life with a catheter. Participants also related that health professionals had not adequately prepared or supported them as they learned to live with a suprapubic catheter. Many of the participants' initial negative experiences were augmented by this deficiency. Issues revolved around perceptions of being psychologically unprepared, the changed body image, altered sexuality, and support needs. Conclusion The findings of this study revealed that the insertion of a suprapubic catheter brought about significant life changes for the participants. The data highlighted disparities between the needs of people who have a supra-pubic catheter and health professionals' perceptions of their needs.
Photobiomodulation therapy for repeated closed head injury in rats

Abstract:Repeated traumatic brain injury, leads to cumulative neuronal injury and neurological impairments. There are currently no effective treatments to prevent these consequences. Growing interest is building in the use of transcranial photobiomodulation (PBM) therapy to treat traumatic brain injury. Here, we examined PBM in a repeated closed head injury (rCHI) rat model. Rats were administered a total of three closed head injuries, with each injury separated by 5 days. PBM treatment was initiated 2 hours after the first injury and administered daily for a total of 15 days. We found that PBM-treated rCHI rats had a significant reduction in motor ability, anxiety and cognitive deficits compared to CHI group. PBM group showed an increase of synaptic proteins and surviving neurons, along with a reduction in reactive gliosis and neuronal injury. These findings highlight the complexity of gliosis and neuronal injury following rCHI and suggest that PBM may be a viable treatment option to mitigate these effects and their detrimental consequences.
[Buccal epithelium reactivity in children].

Abstract:The purpose of this study was the analysis of the buccal smears from 200 healthy children and children with chronic tonsillitis and chicken pox. The smears were stained using Pappenheim's method. The peculiarity of buccal mucosa cytograms of children of different ages was the significant increase in mature forms of the cells (presence of differentiation stage 5 and 6 cells) in adolescents. They also had a lower area of epitheliocytes at all the stages of differentiation and of their nuclei. In chronic tonsillitis and chicken pox, the complex of cytological changes was detected, that included the increase of the leukocyte number, changes in the proportions of epitheliocytes in cell population, augmentation of inflammation-destruction and destruction indexes.
Transcranial photobiomodulation: an emerging therapeutic method to enhance brain bioenergetics

Abstract:Beginning in the 1960s, animal and cell culture studies have demonstrated that transcranial photobiomodulation (t-PBM), which involves photons in the red and near-infrared spectrum, can penetrate the cerebral cortex and enhance the mitochondrial electron transport chain [1] (fNIRS similarly uses near infrared photons to image the brain, but at lower light intensities and exposure, resulting in minimal impact on brain function). Emerging human t-PBM data have demonstrated behavioral and clinical effects in multiple patient populations, along with effects on cerebral metabolism and hemodynamics, highlighting its potential therapeutic efficacy [1]. For example, small studies suggested that t-PBM delivered with both high (700 mW/cm 2 ) and low (36 mW/cm 2 ) irradiance [2] is effective in the treatment of major depressive disorder (MDD). However, since other studies with low t-PBM irradiance (55 mW/cm 2 ) did not replicate the positive results in MDD [3], it becomes critically important to further understand the irradiance dose-dependent neurobiological mechanisms of t-PBM, as we are attempting in our ongoing clinical trial [4]. t-PBM has an excellent safety profile [1,2,3], and it can be performed in multiple settings, including during magnetic resonance imaging (MRI) or other neuroimaging modalities, to quantify the impact on brain bioenergetics noninvasively and in real-time. However, work is still underway to determine the optimal parameters for clinical utility. Our team has recently demonstrated the effect of t-PBM, at medium irradiance (300 mW/cm 2 ), on regional intrinsic brain activity in 19 individuals with early Alzheimer's disease [AD] [5]. In this publication [5], we found that t-PBM altered the fractional amplitude of low-frequency fluctuations (fALFF) across all laser-irradiated brain cortical regions, which are limited to frontal regions due to skull density and hair interference. fALFF, assessed non-invasively during resting-state MRI, is a voxel-based measure of intrinsic brain activity measuring the regional brain metabolism: it is strongly correlated with cerebral glucose metabolism measured by [ 18 F]Fluorodeoxyglucose positron emission tomography. Similarly, other studies have reported increases in MRI BOLD signal (also in t-PBM-irradiated brain areas, at 300 mW/cm 2 irradiance) in healthy volunteers [6]. The increase in fALFF we detected in t-PBM-irradiated brain areas during stimulation [5], and the similar increases in MRI BOLD signal [6], suggest that t-PBM can increase intrinsic brain activity (and specifically cerebral metabolism) in real time; these effects continue briefly after t-PBM administration has ended.

Abstract:Spinal cord injury (SCI) is a cause of profound and irreversible damage, with no effective therapy to promote functional recovery. Photobiomodulation (PBM) may provide a viable therapeutic approach using red or near‐infrared light to promote recovery after SCI by mitigating neuroinflammation and preventing neuronal apoptosis. Our current study aimed to optimize PBM dose regimens and develop and validate the efficacy of an invasive PBM delivery paradigm for SCI. Dose optimization studies were performed using a serum withdrawal model of injury in cultures of primary adult rat dorsal root ganglion neurons (DRGN). Implantable and transcutaneous PBM delivery protocols were developed and validated using cadaveric modeling. The efficacy of PBM in promoting recovery after SCI in vivo was studied in a dorsal column crush injury model of SCI in adult rats. Optimal neuroprotection in vitro was achieved between 4 and 22 mW/cm2. 11 mW/cm2 for 1 min per day (0.66 J/cm2) increased cell viability by 45% over 5 days (p <0.0001), increasing neurite outgrowth by 25% (p <0.01). A method for invasive application of PBM was developed using a diffusion‐tipped optogenetics fiber optic. Delivery methods for PBM were developed and validated for both invasive (iPBM) and noninvasive (transcutaneous) (tcPBM) application. iPBM and tcPBM (24 mW/cm2 at spinal cord, 1 min per day (1.44 J/cm2) up to 7 days) increased activation of regeneration‐associated protein at 3 days after SCI, increasing GAP43+ axons in DRGN from 18.0% (control) to 41.4% ± 10.5 (iPBM) and 45.8% ± 3.4 (tcPBM) (p <0.05). This corresponded to significant improvements at 6 weeks post‐injury in functional locomotor and sensory function recovery (p <0.01), axonal regeneration (p <0.01), and reduced lesion size (p <0.01). Our results demonstrated that PBM achieved a significant therapeutic benefit after SCI, either using iPBM or tcPBM application and can potentially be developed for clinical use in SCI patients.

Abstract:PBM therapy promotes glial cell polarization toward the neuroprotective A2/M2 phenotype, preserves mitochondrial dynamics, alleviates secondary injury and neuronal senescence, and produces anxiolytic effects. Repeated closed head injury (rCHI) is one of the most common brain injuries. Although extensive studies have focused on how to treat rCHI‐induced brain injury and reduce the possibility of developing memory deficits, the prevention of rCHI‐induced anxiety has received little research attention. The current study was designed to assess the effects of photobiomodulation (PBM) therapy in preventing anxiety following rCHI. The rCHI disease model was constructed by administering three repeated closed‐head injuries within an interval 5 days. 2‐min daily PBM therapy using an 808 nm continuous wave laser at 350 mW/cm2 on the scalp was implemented for 20 days. We found that PBM significantly ameliorated rCHII‐induced anxiety‐like behaviors, neuronal apoptosis, neuronal injury, promotes astrocyte/microglial polarization to anti‐inflammatory phenotype, preserves mitochondrial fusion‐related protein MFN2, attenuates the elevated mitochondrial fission‐related protein DRP1, and mitigates neuronal senescence. We concluded that PBM therapy possesses great potential in preventing anxiety following rCHI.

Abstract:Abstract Light therapy techniques, such as photobiomodulation therapy (PBMT), photodynamic therapy (PDT), and laser photoablation, have gained widespread attention and become indispensable physiotherapy methods in clinical practice. PBMT involves the application of low‐level laser/LED to modulate the function of nerve cells, relieve neuroinflammation, promote neurogenesis and vascular growth. Recent studies have shown that PBMT holds promise as a complementary or alternative treatment of Alzheimer's disease (AD), traumatic brain injury (TBI), major depressive disorder (MDD), etc. However, the therapeutic effect of PBMT is influenced by various factors, such as the patients' condition, brain structure and function, illumination parameters, etc. Therefore, the optimized parameters, personalized therapeutic schedules, and precise evaluation of the therapeutic effect are crucial to the treatment success. In this review, we identified the recent experimental and clinical successes, existing obstacles, and future opportunities for PBMT in the treatment of the brain diseases. As a non‐invasive, side‐effect‐free, and highly accessible technique, PBMT brings a glimmer of light for the treatment of neuropsychiatric disorders and the neuro‐rejuvenation of human brains.
The effect of phototherapy on sleep during acute rehabilitation after traumatic brain injury: a randomized controlled trial

Abstract:Objective: To examine the impact of bright white light (BWL) exposure on sleep quality in persons with recent traumatic brain injury (TBI). Design: Randomized, controlled device-sham study Setting: 3 TBI Model System inpatient rehabilitation units Participants: 131 participants (mean 40.9 years, 68% male) Intervention: Intervention group (N = 65) received BWL (1260 lux at 20 inches, 440–480 nanometers length) for 30 minutes each morning at 12–24 inches from the face. Control group (N = 66) received red light (<450 lux, no light between 440 and 480 nanometers) for the same period. Planned intervention was maximum of 10 treatments or until discharge. Main Outcome Measure: Sleep duration and quality using actigraphic recording. Results: There were no differences found between groups on the primary outcomes nor on the secondary outcomes (sleepiness, mood, cooperation with therapy). Conclusion: BWL treatment during acute rehabilitation hospitalization does not appear to impact sleep or measures commonly associated with sleep. While studies have indicated common complaints of sleep difficulties after TBI, we were unable to document an effect for phototherapy as a treatment. With growing evidence of the effect of sleep on neural repair and cognition, further study is needed to understand the nature and treatment of sleep disorders after TBI.Clinicaltrials.gov Identifier: NCT02214212

Transcranial photobiomodulation increases cognition and serum BDNF levels in adults over 50 years: A randomized, double-blind, placebo-controlled trial

Abstract:Background: There is a significant lack of therapeutic options for mild cognitive impairment (MCI), which is rapidly becoming a global epidemic due to aging. Transcranial photobiomodulation (t-PBM) involves delivering near-infrared light (NIR) to the scalp, targeting cortical areas of the brain. NIR t-PBM has recently emerged as a potential therapy for various neurodegenerative conditions, including memory issues. Aims: This study aimed to evaluate cognition scores (primary outcome), depression, anxiety, resilience scores, neuroplasticity, and neurodegeneration biomarkers (secondary outcomes) in individuals with MCI undergoing t-PBM therapy or receiving a placebo. Materials and methods: A total of 93 older adult individuals with MCI were randomly assigned to either a t-PBM (n = 47) or Placebo (n = 46) group. Clinical assessments were conducted at baseline, 60 days post-treatment, and a 150-day follow-up. We also measured serum levels of brain-derived neurotrophic factor (BDNF), a neuroplasticity biomarker, as well as neuron-specific enolase (NSE) and calcium-binding protein B (S100B), which are neurodegeneration biomarkers. Intervention effects were analyzed using repeated measures (RM) two-way ANOVA followed by Tukey post hoc test. Fischer's exact test and Generalized Estimating Equations (GEE) were also applied. Results: Of the 93 older adults individuals invited to participate, 76 (t-PBM: 40, placebo: 36) completed the study. The t-PBM significantly improved cognition as measured by the Montreal Cognitive Assessment (MoCA) compared to placebo (p = 0.0301). The delta values for MoCA scores were 3.20 in the t-PBM group and 1.97 in the placebo group. This effect persisted until the three-month follow-up, accompanied by increased BDNF levels in the t-PBM group but not in the placebo group (p = 0.0046). The delta values for BDNF were 821.94 in the t-PBM group and 359.41 in the placebo group. t-PBM did not alter depression, anxiety, resilience scores, nor the levels of NSE and S100B in individuals with MCI. Conclusion: The t-PBM increases cognitive function and BDNF levels in adults with MCI. Its application as an adjunctive treatment may play a crucial role in preventing neurodegenerative diseases.

Abstract:Journal of Innovative Optical Health Sciences, Ahead of Print. Acquired brain injury (ABI) is an injury that affects the brain structure and function. Traditional ABI treatment strategies, including medications and rehabilitation therapy, exhibit their ability to improve its impairments in cognition, emotion, and physical activity. Recently, near-infrared (NIR) photobiomodulation (PBM) has emerged as a promising physical intervention method for ABI, demonstrating that low-level light therapy can modulate cellular metabolic processes, reduce the inflammation and reactive oxygen species of ABI microenvironments, and promote neural repair and regeneration. Preclinical studies using ABI models have been carried out, revealing the potential of PBM in promoting brain injury recovery although its clinical application is still in its early stages. In this review, we first inspected the possible physical and biological mechanisms of NIR-PBM, and then reported the pathophysiology and physiology of ABI underlying NIR-PBM intervention. Therefore, the potential of NIR-PBM as a therapeutic intervention in ABI was demonstrated and it is also expected that further work can facilitate its clinical applications.
