Abstract:Healthcare quality and safety span multiple topics across the spectrum of academic and clinical disciplines. Keeping abreast of the rapidly growing body of work can be challenging. In this series, we provide succinct summaries of selected relevant studies published in the last several months. Some articles will focus on a particular theme, whereas others will highlight unique publications from high-impact medical journals. From closed to open access: A case study of flipped journals

Abstract:In recent years, increased stakeholder pressure to transition research to Open Access has led to many journals "flipping" from a toll access to an open access publishing model. Changing the publishing model can influence the decision of authors to submit their papers to a journal, and increased article accessibility may influence citation behaviour. The aim of this paper is to show changes in the number of published articles and citations after the flipping of a journal. We analysed a set of 171 journals in the Web of Science (WoS) which flipped to open access. In addition to comparing the number of articles, average relative citation (ARC) and normalized impact factor (IF) are applied, respectively, as bibliometric indicators at the article and journal level, to trace the transformation of flipped journals covered. Our results show that flipping mostly has had positive effects on journal's IF. But it has had no obvious citation advantage for the articles. We also see a decline in the number of published articles after flipping. We can conclude that flipping to open access can improve the performance of journals, despite decreasing the tendency of authors to submit their articles and no better citation advantages for articles.

Abstract:While writing this abstract I received an email, which I promptly answered. When I returned my attention to the abstract, I struggled to regain my flow of writing. In order to understand this deficit in performance associated with switching from one task to another, or "switch cost", cognitive neuroscientists use task switching paradigms to recreate similar experiences. However, many researchers may be familiar with the difficulties that accompany modifying an established paradigm to suit their experimental design, or even the challenge of creating a new, unvalidated paradigm to perturb a particular aspect of cognitive function. This software tool article introduces a novel task switching paradigm for use and adaptation in online and neuroimaging task switching studies. The paradigm was constructed with a flexible, easily-adapted framework that can accommodate a variety of designs. This paradigm utilizes three psychometrically opposed but visually similar tasks- the Digit Span, the Spatial Span, and the Spatial Rotation. In two Use Cases we demonstrate the reliable nature of overall task performance and the dependence of switch costs on certain task parameters. This task framework can be adapted for use across different experimental designs and environment, and we encourage researchers to modify the task switching game for their experiments.

Abstract:Objective To study the impact of converting from subscription‐based publishing to open access ("flipping") in three obstetrics and gynecology (OBGYN) journals. Methods We compared original articles in three OBGYN journals during a matched subscription‐based and open access publishing period. We analyzed citation metrics and country of authorship. Results Overall, 1522 studies were included; of those, 869 (57.1%) were before flipping and 653 (42.9%) were after flipping. Flipped classroom or Inverted classroom is one such innovation that can empower a learner to develop critical thinking skills and master ways to imbibe vast information by engaging students in active learning process. Reading and understanding are carried at home, and the class-time is utilized for higher levels of learning like analyzing, evaluating, and application of the basic information. This review article is aimed to guide the educators in applying the concept of flipped classroom in their teaching learning armamentarium.

Abstract:Objectives: Nonmedical switching (NMS) is a change in a patient's treatment regimen for reasons other than lack of efficacy, intolerance, adverse effects, or poor adherence. We describe the impact of NMS on patients, health care workers, and health systems, focusing on NMS to in-class biologic alternatives in US patients with chronic, immune-mediated rheumatic and dermatologic conditions. Additionally, we evaluate the ways in which the COVID-19 pandemic may exacerbate the physical, psychological, and economic impacts of NMS. Study design: Narrative review. Methods: We performed a search of MEDLINE's PubMed database from October 2015 to October 2020, with a repeat search in October 2021. Search terms included relevant keywords pertaining to NMS, biologics, and disease areas. Results were supplemented by a search of key congress abstracts from 2015 to 2021 and a targeted internet search. Results: NMS increases medication abandonment, errors, and adverse effects, and it can lead to longer patient visits, increased follow-up visits, additional laboratory tests and procedures, and greater overall health care resource utilization (HCRU). The increased HCRU associated with NMS increases patients' financial burden due to additional co-pays and out-of-pocket costs. Conclusions: The decision to switch treatments should result from shared decision-making between health care providers (HCPs) and patients to achieve the best clinical outcomes and optimal HCRU. The issues related to NMS may be compounded by the financial and psychosocial stress on HCPs and patients created by the COVID-19 pandemic. HCPs should advocate for continuous patient treatment and be familiar with continuity of care legislation, appeals processes, and manufacturer assistance programs.

Abstract:Background: Compared with multiple-inhaler triple therapy (MITT), single-inhaler triple therapy (SITT) with fluticasone furoate/umeclidinium/vilanterol (FF/UMEC/VI) demonstrated improved lung function and meaningful improvements in chronic obstructive pulmonary disease (COPD) Assessment Test score. This real-world study compared the effectiveness of switching patients with COPD in England from MITT to once-daily SITT with FF/UMEC/VI by evaluating rates of COPD exacerbation, healthcare resource use (HCRU) and associated direct medical costs. Methods: Retrospective cohort pre-post study using linked primary care electronic health record and secondary care administrative datasets. Patients diagnosed with COPD at age ≥35 years, with smoking history, linkage to secondary care data and continuous GP registration for 12 months pre-switch and 6 months post-switch to FF/UMEC/VI were included. Index date was the first initiation of an FF/UMEC/VI prescription immediately following MITT use from 15 November 2017 to 30 September 2019. Baseline was 12 months prior to index, with outcomes assessed 6/12 months pre-switch and post-switch, and stratified by prior COPD exacerbation status. Results: We included 2533 patients (mean [SD] age: 71.1 [9.9] years; 52.1% male). In the 6 months post-switch, there were significant decreases in the proportion of patients experiencing ≥1 moderate-to-severe (36.2%-28.9%), moderate only (24.4%-19.8%) and severe only (15.4%-11.8%) COPD exacerbation (each, p<0.0001) compared with the 6 months pre-switch. As demonstrated by rate ratios, there were significant reductions in exacerbation rates of each severity overall (p<0.01) and among patients with prior exacerbations (p<0.0001). In the same period, there were significant decreases in the rate of each COPD-related HCRU and total COPD-related costs (-24.9%; p<0.0001). Conclusion: Patients with COPD switching from MITT to once-daily SITT with FF/UMEC/VI in a primary care setting had significantly fewer moderate and severe exacerbations, and lower COPD-related HCRU and costs, in the 6 months post-switch compared with the 6 months pre-switch.

Abstract:Many medical educators, out of necessity imposed by the COVID-19 pandemic, had to flip their classrooms. However, instead of adapting to this new teaching style, many have proceeded in the manner that they have always used to create content, opening a slide deck and inserting content until they are satisfied with the result. When in fact, we know based on evidence and our own experience, that educators should first plan, organize, and storyboard before collecting information into a presentation [1,2]. If educators need to replace real-world teaching and interaction, we believe the next best option is video education, although there are other forms of prework that can be utilized to flip medical classrooms, including short readings and exercises. We discuss the case for flipping medical classrooms, the limitations, and how educators can get started flipping their classrooms today.
