Abstract:Objective To investigate the factors influencing the adjustment function of PI on spinal sagittal balance in normal subject.Methods 106 normal subjects were selected to perform a full standing spinal lateral radiograph including spine,pelvis and both hip joints from January 2008 to June 2008 in our outpatient department.The radiographs were digitized and 5 sagittal parameters were measured on lateral radiographs:pelvic incidence,sacral slope,pelvic tilt,lumbar lordosis and thoracic kyphosis,followed by the statistic analysis.Results 106 cases of normal subjects were separately divided into 38 cases,belonging to high SS/lower PT,with the mean value of PI and LL of(50.85±10.01)°and(49.41±9.84)°respectively and the regression equation of LL=0.571PI+20.395;68 cases,belonging to lower SS/high PT,with the mean value of PI and LL of(49.14±12.00)°and(39.17°±10.98)°respectively and the regression equation of LL=0.531PI+13.079°.The two groups demonstrated interclass positive correlation and linear regression between PI and SS,PT and LL,SS and LL respectively,while significant difference of SS,PT and LL between the two groups was showed except for that of PI.Conclusions(1)LL can't be concluded only by PI for the influence of pelvic posture.(2)Under the same PI,the more anteverted pelvis,the more large LL,otherwise,the more large PI,the more large LL under the same pelvic posture.
Sagittal Plane Analysis of the Spine and Pelvis in Degenerative Lumbar Scoliosis

Abstract:Background: Previous studies have reported the normative values of pelvic sagittal parameters, but no study has analyzed the sagittal spino-pelvic alignment in degenerative lumbar scoliosis (DLS) and its role in the pathogenesis. Methods: Retrospective analysis was applied to 104 patients with DLS, together with 100 cases of asymptomatic young adults as a control group and another control group consisting of 145 cases with cervical spondylosis. The coronal and sagittal parameters were measured on the anteroposterior and lateral radiograph of the whole spine in the DLS group as well as in the two control groups. Results: Statistical analysis showed that the DLS group had a higher pelvic incidence (PI) value (50.5° ± 10.2°), than the normal control group (with PI 47.2° ± 8.8°) and the cervical spondylosis group (46.9° ± 9.1°). In DLS group, there were 38 cases (36.5%) complicated with degenerative lumbar spondylolisthesis, who had higher PI values than patients without it. Besides, the lumbar lordosis (LL) and sacral slope (SS) of DLS group were lower; the scoliosis Cobb’s angle was correlated with pelvic tilt (PT); thoracic kyphosis was correlated with LL, SS, and PT; and LL was correlated with other sagittal parameters. Conclusion: Patients with DLS may have a higher PI, which may impact the pathogenesis of DLS. A high PI value is probably associated with the high prevalence of degenerative lumbar spondylolisthesis among DLS patients. In DLS patients, the lumbar spine maintains the ability of regulating the sagittal balance, and the regulation depends more on thoracic curve.
Radiological Analysis of Degenerative Lumbar Scoliosis in Relation to Pelvic Incidence.

Abstract:Purpose: The purpose of this study was to identify the influence of pelvic incidence (PI) on spinopelvic parameters in patients with degenerative lumbar scoliosis (DLS) and compare them with those of a normal population. Methods: There were two groups in this study. One group was composed by 136 patients with DLS and another was composed by 120 participants free of spinal disease. In each group there were three subgroups according to PI, which were low (PI less than 45 degrees), middle (PI between 45 degrees and 60 degrees) and high PI group (PI more than 60 degrees). Sagittal spinopelvic parameters were compared between the DLS patients and asymptomatic participants in each PI group. Results: The number of DLS patients with low, middle, and high PI were 38 (27.9%), 50 (36.8%), and 48 (35.3%), respectively. In the control group, the number of low, middle, and high PI participants were 52 (43.3%), 41 (34.2%), and 27 (22.5%), respectively. There were significant difference in PT, SS, LL, SVA and TLJ between the three subgroups in the DLS patients. Patients with high PI showed large TLJ, LL, PT, SS and small SVA. In the Control group and DLS group, PI determined pelvic orientation (PT, SS) and sagittal spinal parameters (LL, TLJ). In terms of correlation between SS and LL, between SS and TLJ, both DLS and Control groups showed significant correlations. In terms of correlation between PT and SVA, between PT and TLJ, only the DLS group showed a significant correlation. Compared with the asymptomatic participants, DLS patients showed a high PT and low SS as well as kyphotic TLJ, lumbar hypolordosis and thoracic hypokyphosis in all PI groups. Conclusions: The changes in spinopelvic parameters and pelvic compensatory mechanisms differ according to PI in patients with DLS, restoration of LL based on individual PI could help in accomplishing a balanced spinopelvic alignment.
Compensatory mechanisms in adult degenerative thoracolumbar spinal deformity – Radiographic patterns, their reversibility after corrective surgery, and the influence of pelvic morphology

Abstract:Objective: Loss of lumbar lordosis (LL) in degenerative deformity activates spinal compensatory mechanisms to maintain neutral C7 sagittal vertical axis (C7SVA), such as an increase in pelvic tilt (PT) and decreased thoracic kyphosis (TK). We study the extent to which PT increase and TK reduction contribute to the compensation of pelvic incidence (PI)-LL mismatch. Methods: A cohort of 43 adult patients with adult degenerative thoracolumbar deformity were included in this retrospective study. Radiographic spinopelvic measurements were obtained before and after corrective surgery. Pearson correlations were calculated. Results: Preoperative PI-LL mismatch significantly correlated with an increase in PT and a decrease in TK in the whole cohort r = +0.66 (95% confidence interval [CI] 0.44–0.8) and r = −0.67 (95% CI − 0.81–−0.47), respectively, at a relative rate of 0.37 (standard deviation [SD]: 0.07) and − 0.57 (SD: 0.09), respectively. In patients with low PI, only TK showed a significant correlation with PI-LL mismatch, r = −0.56 (95% CI − 0.8 to − 0.16), at a rate of − 0.57 (SD: 0.19). The high PI subgroup showed a significant correlation with PT, TK, and C7SVA, r = 0.62 (95% CI 0.26–0.82), r = −0.8 (95% CI − 0.9–−0.58), and r = 0.71 (95% CI 0.41–0.87) at rates of 0.48 (SD: 0.11), −0.72 (SD: 0.12), and 0.62 (SD: 1.27). Conclusions: Decreased TK represented a more consistent compensatory mechanism in patients with high and low PI when compared to an increase in PT. PI-LL mismatch induced more pronounced changes in TK than did PT in both subgroups. Patients with high PI relied more on increases in PT and a relative decrease in TK to compensate for PI-LL mismatch than patients with low PI.
Impact of Pelvic Anteversion on Spinopelvic Alignment in an Asymptomatic Population: a Dynamic Perspective of Standing and Sitting

Abstract:BACKGROUND CONTEXT: A subgroup of patients with pelvic anteversion can present with an unusually large degree of lumbar lordosis (LL), a highly sloped sacrum, and a relatively small pelvic incidence (PI). Prior to lumbar surgery, it can be important to consider such unique sagittal alignment. However, until now, there has been a lack of a predictive model considering different pelvic alignments. Furthermore, the dynamic characteristics of an anteverted pelvis (AP) subgroup have also been unclear. PURPOSE: To build linear predictive formulas for LL that take pelvic anteversion into consideration and to explore the dynamic characteristics of an AP subgroup. STUDY DESIGN: Monocentric, cross-sectional study. PATIENT SAMPLE: Five hundred sixty-five asymptomatic Chinese men and women between the ages of 18 and 80 years. OUTCOME MEASURES: Sagittal parameters including LL, lumbar lordosis minus thoracic kyphosis (LL-TK), PI, pelvic tilt (PT), pelvic incidence minus lumbar lordosis (PI-LL), sacral slope (SS), sacral slope divided by pelvic incidence (SS/PI), sagittal vertical axis (SVA), thoracic kyphosis (TK), and T1 (first thoracic vertebra) pelvic angle (TPA) were measured on whole spine radiographs obtained with participants in standing and sitting positions. METHODS: All participants underwent radiography in the standing position; 235 of them underwent additional radiography in the sitting position to allow measurement of sagittal parameters. The participants with pelvic anteversion were placed in an AP (anteverted pelvis) group. Sagittal parameters were compared between the AP group and the non-AP group, and predictive formulas for LL based on PI were created in both groups. In addition, changes in sagittal parameters from standing to sitting were compared in the AP group and a PI-matched control group. RESULTS: Of the 565 participants, 171 (30.3%) had pelvic anteversion. In comparison with the non-AP group, the AP group presented with larger LL, a larger SS, and a smaller PT, with relatively small PI. The predictive formulas for LL were LL=0.60 degrees x PI+21.60 degrees (R-2=0.268; p<.001) in the whole cohort, LL=0. 83xPI+18.75 degrees (R-2=0.427; p<.001) in AP group, and LL=0.79 degrees xPI+9.66 degrees (R-2=0.451; p<.001) in the non-AP group. In moving from standing to sitting, the AP group presented with a larger decrease in SS and LL compared with the control group, indicating different patterns of spinopelvic motion. CONCLUSIONS: In the cohort examined, 30.3% present with pelvic anteversion. Those with AP present with unique characteristics of spinopelvic alignment. In moving from standing to sitting, they exhibit different patterns of spinopelvic motion. We found that identifying the degree of anteversion in each person improves the accuracy of linear models for predicting the degree of LL, which in turn can make plans for spine surgery more accurate. The Role of Pelvic Compensation in Sagittal Balance and Imbalance: The Impact of Pelvic Compensation on Spinal Alignment and Clinical Outcomes Following Adult Spinal Deformity Surgery

Abstract:Background and objectives: The Scoliosis Research Society (SRS)-Schwab system does not include a pelvic compensation (PC) subtype, potentially contributing to gaps in clinical characteristics and treatment strategy for deformity correction. It also remains uncertain as to whether PC has differing roles in sagittal balance (SB) or imbalance (SI) status. To compare radiological parameters and SRS-22r domains between patients with failed pelvic compensation (FPC) and successful pelvic compensation (SPC) based on preoperative SB and SI. Methods: A total of 145 adult spinal deformity patients who received deformity correction were analyzed. Radiographic and clinical outcomes were collected for statistical analysis. Patients were classified into 4 groups based on the median value of PT/PI ratio (PTr) and the cutoff value of SB. Patients with low PTr and high PTr were defined as FPC and SPC, respectively. Radiographic and clinical characteristics of different groups were compared. Results: Patients with SPC exhibited significantly greater improvements in lumbar lordosis, pelvic tilt, PTr, and T1 pelvic angle as compared to patients with FPC, irrespective of SB or SI. No apparent differences in any of SRS-22r domains were observed at follow-up when comparing the SB-FPC and SB-SPC patients. However, patients with SI-SPC exhibited significantly better function, self-image, satisfaction, and subtotal domains at follow-up relative to those with SI-FPC. When SI-FPC and SI-SPC patients were subdivided further based on the degree of PI-LL by adjusting for age, the postoperative function and self-image domains were significantly better in the group with overcorrection of PI-LL than undercorrection of PI-LL in SI-FPC patients. However, no differences in these SRS-22r scores were observed when comparing the subgroups in SI-SPC patients. Conclusion: Flexible pelvic rotation is associated with benefits to the correction of sagittal parameters, irrespective of preoperative SB or SI status. However, PC is only significantly associated with clinical outcomes under SI. Patients with SI-FPC exhibit poorer postoperative clinical outcomes, which should be recommended to minimize PI-LL.
Correlation Between Sagittal Spinal and Pelvic Parameters in Degenerative Lumbar Scoliosis

Abstract:Objective To investigate the correlation between sagittal spinal and pelvic parameters in different types of degenerative lumbar scoliosis (DLS).Methods Standing anteroposterior and lateral radiographs of the whole spine including hip joints were carried out in 70 volunteers without spinal deformity and 110 patients with DLS.The following parameters were measured:thoracic kyphosis (TK),thoracolumbar kyphosis (TL),lumbar lordosis (LL),sagittal vertical axis (SVA),pelvic incidence (PI),pelvic tilt (PT) and sacral slope (SS).According to the sagittal spinal alignment,the patients with DLS were classified into 3 types:type Ⅰ (45 cases),type Ⅱ (48 cases) and type Ⅲ (17 cases).The sagittal spinal and pelvic parameters were compared between control group and different types of DLS group,and the relationship between the sagittal spinal parameters and pelvic parameters in different groups were also investigated.Results PI in type Ⅲ patients was lower than those in other groups; PT in type Ⅱ and Ⅲ patients was higher than those in controls and type Ⅰ patients,and there was a significant difference between type Ⅱ and Ⅲ patients; SS in type Ⅱ and Ⅲ patients was lower than those in controls and type Ⅰ patients,and there was no significant difference between type Ⅱ and Ⅲ patients.Sagittal spinal imbalance was found in 17.8％ of type Ⅱ patients and 29.4％ of type Ⅲ patients.There were significant correlations in sagittal spinal parameters,pelvic parameters and spinopelvic parameters in controls and type Ⅰ patients.However,in type Ⅱ and Ⅲ patients,the correlations in sagittal spinal parameters and spinopelvic parameters decreased,even disappeared,though significant correlations were still found in pelvic parameters.In any group,SVA showed a significant correlation with LL and PT,especially with PT.Conclusion The sagittal spinal alignment has a ladder-like change in patients with DLS,and the correlations in pelvic parameters and spinopelvic parameters also change in type Ⅱ and Ⅲ patients,for whom the sagittal spinal imbalance is more likely to occur.

Abstract:A recently discovered chlorinated flame retardant, Dechlorane Plus (DP), was reported in air and a sediment core within the North American Great Lakes region. To further reveal the fate of DP in the Great Lakes, 40 surficial sediments from Lakes Erie and Ontario and two additional cores were analyzed using newly available analytical grade DP isomer solutions. The maximum total concentration in Lake Ontario was over 60-fold higher than Lake Erie, 586 ng/g and 8.62 ng/g, respectively. Additionally, analysis of archived suspended sediments collected from the Niagara River (1980-2002) showed a declining total DP concentration of 89 ng/g to 7.0 ng/g, suggesting a possible decrease in production orthe reduction of free DP released into the environment during manufacturing. The average syn-DP fractional abundance (f(syn)) in our study was less than the commercial DP composition indicating a stereoselective enrichment of anti-DP in the environment Mean fyn profiles were uniquely similar to both Lake Ontario and the Niagara River in comparison to Lake Erie. During the course of our analysis we noticed an increasing f(syn) value in the calibration standard which became exacerbated as the liner got dirtier and suggested the prospect of DP degradation. Followup studies indicated these compounds were dechlorinated DP species produced on the injection liner. Using a clean injection liner, these degradates were also detected in sediments from the Niagara River and Lake Ontario;tentatively identified as [-Cl+H] and [-2Cl+2H] by high resolution mass spectrometry. The observed similarity of f(syn) profiles between Lake Ontario and Niagara River and the detection of the degradates only in their locations, suggest to us that the river is a major source to Lake Ontario's DP burden. To our knowledge, this is the first report of DP degradates in the environment.
[Restoration of the Difference Value of Pelvic Incidence and Lumbar Lordosis in Degenerative Scoliosis Patients: Its Influence in Maintaining Sagittal Profile and Improving Quality of Life].

Abstract:OBJECTIVE:To evaluate the role that post-operative difference value of pelvic incidence and lumbar lordosis (PI-LL) played on loss of correction, implant failure and health-related quality of life during follow-up in degenerative scoliosis patients. METHODS:Retrospective review of 62 patients (average age (57 ± 10) years, 11 male and 51 female patients) with degenerative scoliosis who underwent one stage posterior surgical instrumentation in the affiliated Drum Tower Hospital of Nanjing University Medical School from January 2005 to December 2011. The mean follow-up duration was 4.2 years. Long-cassette standing upright sagittal radiographs were obtained before and after operation and at the last follow-up. At the last follow-up, visual analogue scale and Oswestry disability index were collected. Based on post-operative PI-LL, patients were divided into two groups: group A (-9° < post-operative PI-LL<9°) and group B (post-operative PI-LL < -9°or post-operative PI-LL>9°). Independent t test and χ(2) test were performed for statistical analysis. For all statistical analysis, the level of significance was set at P < 0.05. RESULTS:No difference was observed in terms of loss of correction between two groups during follow-up. More implant failure were observed in group B (15.63% vs. 6.7%, χ(2) = 21.85, P = 0.012). In addition, patients with better PI-LL matching came with better visual analogue scale (3.9 ± 2.4 vs. 5.2 ± 3.3, F = 0.089, P = 0.024). CONCLUSION:Worse quality of life and increased risk for implant failure during follow-up may be related to mismatched PI-LL.
Sagittal Parameters of Spine‐Pelvis‐Hip Joints in Patients with Lumbar Spinal Stenosis

Abstract:Objective To explore sagittal compensation characteristics, including extra spinal‐pelvic parameters and distal hip parameters, for analysis in middle‐aged to the older patients with lumbar spinal stenosis (LSS) without spinal deformity and clarify the fitting relationship between the main sagittal parameters. Methods This retrospective single‐center study included 205 patients with LSS in our department from January 2016 to December 2018, including 153 women (74.6%), with an average age of 67.6 ± 7.1 years. Sagittal parameters were obtained on the whole spinal lateral radiograph. Spinal parameters include thoracic kyphosis (TK), thoracolumbar kyphosis (TLK), and lumbar lordosis (LL). Pelvic parameters include pelvic incidence (PI), pelvic tilt (PT), and sacral slope (SS). Hip parameters include pelvic rotation (PR) and hip inclination angle (HIA). Spine‐pelvic parameter is spino‐sacral angle (SSA). Sagittal balance parameter is sagittal vertical axis (SVA). PI‐LL, PT, SVA, HIA, and TLK were regarded as primary results, and the others were secondary outcomes. The independent sample t‐test was used to compare gender. Pearson correlation analysis was used to evaluate the correlation between primary results and secondary results. We take PI‐LL, PT, SVA, and TLK as dependent variables and include relevant factors for analysis. Results In the case of gender, PI, PI‐LL, and PT were found smaller in men than women, but TK is greater in men than women (27.3° ± 6.1° vs 23.3° ± 7.7°, p = 0.033). PI‐LL was positively correlated with PT (r = 0.608, p < 0.001) and negatively correlated with HIA (r = −0.193, p = 0.010); PT was negatively correlated with HIA (r = −0.289, p < 0.01). As to the relationship between primary and secondary results, HIA was positively correlated with SS and PR (p < 0.01). SVA was positively correlated with SSA (r = 0.341, p = 0.010). The positive influencing factors and risk factors of SVA were SSA, and PI‐LL played a negative regulatory role through proximal TK, the distal PT plays a positive regulatory role. The regulation of PI‐LL was compensated through both TK and PT, with a fitting relationship of PI‐LL = 0.5 × PT − 0.2 × TK. Conclusion There was a close interaction among spine‐pelvic‐hip sagittal parameters. We found the matching of PI‐LL in the domestic middle‐aged and elderly LSS population is regulated by thoracic spine and pelvis.
Significance of pelvic parameters in the evaluation of senile spinal sagittal balance

Abstract:Objective To investigate the value of pelvic parameters in the evaluation of senile spinal sagittal balance through studying the correlation of pelvic parameters such as the sacral slope (SS), pelvic tilt (PT) and pelvic incidence (PI) with senile spinal sagittal balance. Methods 26 patients with senile spinal sagittal deformity were adopted by our department from September 2010 to September 2011, including 14 cases of senile degenerative thoracolumbar kyphoscoliosis deformity and 12 cases of senile osteoporotic compression fractures and thoracolumbar kyphotic deformity. Among them, there were 9 males and 17 females, with an average age of 69.2 years old (range;56-79 years). The judgment standard for spinal sagittal balance included: (1) Standing patients who did not need the compensation of double hip hyperextension or double knee flexion could be judged as spinal sagittal balance; in contrast, he or she would be judged as spinal sagittal imbalance; (2) The sagittal vertical axis (SVA) value: patients with the distance between C7 plumb line of spinal sagittal plane on the lateral X-ray and the posteriosuperior corner of S1 was within ±2.5 cm could be judged as spinal sagittal balance; in contrast, he or she would be judged as spinal sagittal imbalance. The consistency and repeatability for judging spinal sagittal balance were investigated. According to spinal sagittal balance, all patients were divided into 2 groups: balance group and imbalance group. The SS, PT and PI of cases in both groups were measured, and independent-samples t test was conducted to compare the differences in SS, PT and PI between the 2 groups. Results The consistency of spinal balance judgment was 92.3% and Kappa coefficient was 0.861; The repeatability of spinal balance judgment was 96.1% and Kappa coefficient was 0.938. There were 14 and 12 cases in the balance group and imbalance group respectively. In balance group, SS was (37±11)°, PT was (22±10)° and PI was (63±13)°; In imbalance group, SS was (26±12)°, PT was (21±13)° and PI was (47±14)°;There were statistically significant differences in SS and PI between the 2 groups (P<0.01), without statistically significant differences in PT. Conclusions Spinal sagittal balance in patients with senile spinal sagittal deformity is correlated with SS and PI, and SS is a major factor, which could be a reference for evaluating spinal sagittal balance and operation strategy.

Abstract:Objective:To analyze the spinopelvic sagittal alignment in adult patients with idiopathic scoliosis according to curve types.Method:The sagittal spinopelvic parameters and scoliosis curves types were analyzed retrospectively in standing radiographs of 83 adult patients(mean age 43.0 years) with adult idiopathic scoliosis,these parameters included cobb angle of primary scoliosis,thoracic kyphosis(TK),thoracolumbar junction kyphosis(TLJ),lumbar lordosis(LL),pelvic incidence(PI),sacrum slope(SS),pelvic tilt(PT) and sagittal vertical axis(SVA).According to SRS classification,the patients were divided into two groups,thoracic curve and lumbar curve.All spinopelvic parameters between two groups were compared.The correlations between parameters as well as these parameters with age were analyzed.Result:The average age of 39 patients with thoracic curve was 40.9,while that for 44 patients with lumbar curve was 45.8.No age-related difference was noted between two groups.Also no difference for all spinopelvic parameters except SVA was noted.The SVA in patients with lumbar curve was significantly greater than that with thoracic curve(P<0.05).For patients less than 40 years old,all spinopelvic parameters between two groups showed no difference,while for patients older than 40 years old,SVA and PT in lumbar curve group were significant greater than those in thoracic curve group(P<0.05).In thoracic curve group,9 patients had SVA more than±50mm.Among them 7 patients were negative imbalance.In lumbar curve group,12 patients had SVA more than±50mm,and 8 patients of them were positive imbalance.In both two groups,there were significant correlations between LL and TK,LL and SS,PI and SS,PI and PT,SVA and LL,SVA and SS,SVA and PT,age and SVA,and age and PT.PI showed significant correlaion with LL in patients with thoracic curve while no correlation with LL in patients with lumbar curve.Unlike thoracic curve group,no relationship between PI and LL in lumbar curve group were noted,while there were significant correlations between age and LL,age and SS,PT and LL,and PT and SS.Conclusion:In adult patients with idiopathic scoliosis,there is no difference with respect to pelvic shape between thoracic and lumbar curves,and the pelvic is related to alignment of spine sagittal plane,age is related the imbalance of spine.Thoracic curve is apt to lead to negative imbalance,while lumbar curve is apt to lead to positive imbalance.

Abstract:BACKGROUND:The interactions between the spine, pelvis, and lower limbs are dynamic based on the "cone of economy" concept; thus, different global radiographic parameters could be regarded as reflections of different centers of gravity. We conducted this retrospective study to evaluate the offsets of different centers of gravity in asymptomatic populations and to investigate how the global sagittal alignment is supported.METHODS:The following parameters were measured: cervical lordosis, thoracic kyphosis (TK), lumbar lordosis (LL), pelvic incidence (PI), pelvic tilt (PT), the ratio between PT and PI (PT/PI), sacral slope, PI minus LL (PI-LL), the sagittal vertical axis (SVA), cranial SVA to ankle center (Cr-A), CrSVA to the femoral head center (Cr-FH), C2SVA to the femoral head center (C2-FH), pelvic translation (P. Shift), and knee angle (KA). Participants were divided into subgroups based on the PT/PI ratio. Mean values were compared using the t-test, and correlations were assessed using Pearson's coefficient.RESULTS:A total of 82 asymptomatic adults were enrolled. The average PT/PI in subgroup 1 was the smallest, showing that individuals in this group may have limited pelvic retroversion. No significant differences in Cr-FH, Cr-A, or C2-FH were found between subgroups (all P>0.1), implying that global alignment was well supported in each group. Specifically, C2-FH showed minor changes between subgroups (P=0.998), showing that C2-FH may be a target for sagittal compensation. There were positive correlations between PT/PI and both P. Shift and SVA (r=0.930 and r=0.606, respectively). However, Cr-FH, Cr-A, and C2-FH were not significantly correlated with P. Shift or PT/PI (all P>0.05). Weak correlations existed between Cr-A, Cr-FH, and age (all P>0.2).CONCLUSIONS:This study revealed that the Cr-FH and C2-FH offsets are stable across the population and could be maintained by regulating only the sagittal spinal curvature when pelvic compensation is limited. Cr-FH is not affected by age in the asymptomatic population. Thus, the stable Cr-FH and C2-FH could provide references for surgeons during the surgical decision-making process in patients with adult spinal deformity with sagittal malalignment.

Abstract:BACKGROUND:There has been an increasing recognition of the importance of sagittal spinopelvic alignment in patients with scoliosis as it relates to clinical outcomes. However, the changes seen in sagittal spinopelvic alignment in adult idiopathic scoliosis patients is poorly defined. This study was conducted to evaluate the sagittal alignment of pelvis and spine in adult idiopathic scoliosis patients.METHODS:The sagittal parameters of the spine and pelvis were analyzed in lateral standing radiographs of 124 patients (mean age 47.4 years) with adult idiopathic scoliosis, including thoracic kyphosis (TK), thoracolumbar junction kyphosis (TLJ), lumbar lordosis (LL), pelvic incidence (PI), sacrum slope (SS), pelvic tilt (PT) and C7 plumb line (C7PL). The patients were divided into three groups according to the age: 20 - 40 years, 41 - 64 years, and ≥ 65 years. The parameters were compared with those in normal adults and adolescent idiopathic scoliosis (AIS) patients. The relationship between all parameters as well as age and sagittal parameters were analyzed.RESULTS:The PI in patients with adult idiopathic scoliosis was 58.1° ± 13.0°, which was significantly higher than that in normal adults. The PT (19.9° ± 10.6°) was also higher than that in both normal adults and AIS patients, while the SS (38.1° ± 12.0°) was similar or smaller. As age increased, C7PL, PT and TJL increased while LL decreased. There was no relationship between age and both PI and TK. PT had the strongest statistical association with the C7PL.CONCLUSIONS:PI is higher in adult idiopathic scoliosis than normal subjects. The PT is the most relevant pelvic parameter to the global sagittal alignment of the spine. Age significantly influences sagittal parameters of the spine and pelvis except the PI and TK.

Abstract:Abstract Background Degenerative lumber spondylolisthesis (DLS), a common orthopaedic disease causing low back pain, seriously affects daily life and work. Although sagittal spinopelvic parameters have been studied in single-level DLS patients, investigations into different types of double-level DLS are scarce. This study aims to analyse the demographic and radiological parameters of patients with different types of double-level DLS and compare their differences to provide a reference for guiding surgical treatment and restoring sagittal balance of DLS patients. Methods From January 2014 to January 2020, double-level DLS patients’ records were retrospectively reviewed and divided into three types: anterior, posterior, and combined; the anterior and combined types were studied. Two spin surgeons measured the sagittal spinopelvic parameters: C7 tilt, maximal thoracic kyphosis (TKmax), maximal lumbar lordosis (LLmax), pelvic incidence (PI), pelvic tilt (PT), and sacral slope (SS). Following descriptive analysis, demographic and radiographic data were compared between the anterior and combined types. Results Patients in the anterior type group (n = 40) were older (67.68 ± 6.66 vs. 61.72 ± 10.06 years, P = 0.031), and the proportion of females were significantly higher (90% vs. 50%, P = 0.001) than the combined group (n = 18). Both groups had different levels of chronic lower back pain, but the incidence of radiating leg pain and neurogenic claudication was significantly higher in the anterior type. Owestry disability index and visual analogue scale lower back scores were also higher in the anterior type. In the anterior type, C7 tilt (7.14 ± 2.15 vs. 5.41 ± 2.28, P = 0.007), LLmax (50.02 ± 14.76 vs. 36.96 ± 14.56, P = 0.003), PI (68.28 ± 9.16 vs. 55.53 ± 14.19, P < 0.001), PT (28.68 ± 7.31 vs. 19.38 ± 4.70, P < 0.001), and PT/PI (42.45 ± 11.22 vs. 36.04 ± 9.87, P = 0.041) were significantly higher. In the anterior type, PI correlated significantly positively with LLmax ( r = 0.59) and SS ( r = 0.71). LLmax and SS ( r = 0.65) had a positive correlation. PT/PI and SS ( r = -0.77) had a negative correlation. In the combined type, PI correlated positively with LLmax ( r = 0.61) and SS ( r = 0.88), and PT/PI correlated negatively with SS ( r = -0.81). Conclusions In patients with double-level DLS, the sagittal spinopelvic parameters differ between the anterior and combined types. PI and LLmax were significantly higher in the anterior type, resulting in compensation by forward flexion and pelvic retroversion. Spinal surgeons should focus on correcting sagittal deformities, relieving postoperative clinical symptoms, and improving quality of life during fusion surgery, which is proven to guarantee long-term surgical results.
Effect of Paravertebral Muscle Degeneration on Lumbar and Pelvic Sagittal Alignment in Patients With Degenerative Scoliosis

Abstract:Study design: Retrospective study. Objectives: To explore the relationship between lumbar spine muscle mass and lumbar pelvic sagittal parameters in patients with degenerative scoliosis. Methods: This study included ADS patients who were treated in our hospital from 2019 to 2023. The spinal parameters were evaluated through X-rays, and the relative muscle volume (RMV) and fat infiltration (FI) were measured through three-dimensional reconstruction. Patients were categorized into 3 groups based on SRS-Schwab sagittal balance correction (0, +, ++), and into 3 groups based on GAP score (proportioned, moderately dis-proportioned, severely dis-proportioned). Finally, patients were classified into low-quality and high-quality groups based on the FI of Paraspinal muscles (PSM). Results: The study included a total of 63 patients. Significant statistical differences were observed in the FI and RMV of MF, ES and PS among patients classified by SRS-Schwab PT classification. Additionally, significant statistical differences were found in the RMV of MF and PS among patients classified by SRS-Schwab PI-LL classification and GAP score. Furthermore, a significant correlation was found between the FI and RMV of PSM and lumbopelvic sagittal parameters. The ordinal regression model analysis revealed that FI of ES significantly impacted PT imbalance, while RMV of MF significantly impacted PI-LL imbalance. Moreover, significant differences were noted in PT and PI between the low-quality and high-quality multifidus groups. Conclusions: As sagittal imbalance worsens, PSM degeneration also intensifies, primarily characterized by an increase in FI and a decrease in RMV. Notably, PT and PI-LL are positively correlated with RMV and negatively correlated with FI.

Abstract:Purpose: To analyze the relationships between coronal and sagittal spinopelvic parameters in degenerative lumbar kyphoscoliosis (DLKS). Methods: We enrolled 75 patients with DLKS for a radiographic study between January 2016 and September 2018. Correlations between coronal and sagittal spinopelvic radiographic parameters were analyzed. Then patients were divided into 2 groups: sagittal balanced group (SVA< = 5 cm, 30 patients) and sagittal imbalanced group (SVA >5 cm, 45 patients), and relevant parameters were compared. Results: The Cobb angle and lumbar lordosis of the DLKS patients were 24.87 ± 11.59° and 17.26 ± 12.24°, respectively. The average age was 68 years old (range: 42-82), and the sex ratio was 2.6:1 (female: 54 patients; male: 21 patients). 50 patients (66.7%) located convexity of the curve at left side, while 25 patients (33.3%) at right side. The Cobb angle correlated with LL-TK (r = −0.228, p = 0.049), LL (r = −0.255, p = 0.027) and SS (r = −0.232, p = 0.045). There were significant differences in PI-LL (t = −3.484, P = 0.001), LL-TK (t = 2.354, P = 0.023), PI (t = −3201, P = 0.002) and PT (t = −2.521, P = 0.014) between sagittal balanced and imbalanced group. Conclusions: In degenerative lumbar kyphoscoliosis, there are some correlations between coronal and sagittal spinopelvic parameters. Moreover, PI-LL, LL-TK, PI, PT were significantly different between sagittal balanced and imbalanced DLKS patients.
Sagittal Spinal and Pelvic Parameters in Patients With Scheuermann's Disease: A Preliminary Study

Abstract:BACKGROUND: Sagittal spinopelvic parameters remain poorly defined in patients with Scheuermann disease (SD). For example, although pelvic incidence (PI) should approximate lumbar lordosis (LL) by 10°, this is not true in patients with SD. This retrospective radiographic study was conducted to propose a new mathematical relationship between sagittal spinopelvic parameters in skeletally mature patients with SD.METHODS: The following formula (Δ) was proposed [(thoracic kyphosis - 45°) + (thoracolumbar kyphosis - 0°) + (PI - LL) = ± 10°] and validated with standard spino pelvic parameters in patients with skeletally mature SD without prior spine surgery at 2 centers between 2006 and 2015. The T1 pelvic angle (TPA) was used as a measure of global balance with normal maximum of 15°. Subgroup analysis was performed to compare Δ between balanced (TPA ≤ 15°) and unbalanced (TPA > 15°) patients with SD.RESULTS: In patients with SD (n = 30), half were female (n = 15), the average age was 39 years, and the average Δ was 2.4°. A significant correlation was discovered between Δ and both TPA (R 2 = 0.75) and PI (R 2 = 0.69). At TPA of 15°, average Δ was 9.2°. There was also a significant difference between balanced and unbalanced patients (-8.7° ± 11.6° versus 28.2° ± 19.7°, P = .0003).CONCLUSIONS: This study of a new formula (Δ) to evaluate global sagittal balance in patients with SD found that accounting for the kyphosis maintained Δ within ± 10°. Further study is planned to determine whether maintaining and/or restoring a normal Δ is associated with improved outcomes in patients with SD after surgery.

Abstract:Study Design. A retrospective radiographical study. Objective. To compare the prediction abilities of T1 pelvic angle (TPA) and other parameters for postoperative sagittal balance, and investigate the relationships between these parameters and health-related quality of life. Summary of Background Data. Using sagittal vertical axis (SVA) to assess sagittal alignment fails to take account of the pelvic compensation. A new parameter, TPA, has been recommended to represent the global sagittal balance of adult scoliosis. Methods. A retrospective review was performed on patients with adult scoliosis undergoing correction surgery from May 2009 to March 2013. The Spearman. was used to determine the correlations between the radiographical parameters (preoperative, postoperative, and changes) and the overall Oswestry Disability Index (ODI), visual analogue scale (VAS), and Scoliosis Research Society-22 (SRS-22) questionnaire scores. Results. Significant correlations were found between the changes of TPA and the changes of lumbar lordosis, pelvic tilt, sacral slope, pelvic incidence, SVA, spinosacral angle, ODI, VAS, SRS-22, and pedicle subtraction osteotomy (PSO) degrees (P < 0.05). The changes of SVA were significantly related to the changes of lumbar lordosis, TPA, C7-sacrofemoral distance, ODI, VAS, SRS-22 (P < 0.05) but not PSO degrees (P > 0.05). Significant correlations were found between the changes of spinosacral angle and the changes of thoracolumbar kyphosis, TPA, ODI, VAS, SRS-22, and PSO degrees (P < 0.05). The changes of C7 plumb line to sacrofemoral distance ratio were significantly related to the changes of SVA (P < 0.05), but not the changes of ODI, VAS, SRS-22, or PSO degrees (P > 0.05). Conclusion. TPA could better reflect the postoperative changes of sagittal alignment and health-related quality of life for patients with adult scoliosis. Moreover, the changes of TPA are strongly correlated to the osteotomy degrees for PSO and, TPA could be used as a reference parameter in surgical planning.
Dynamic changes of pelvic incidence after pelvic fixation with second sacral alar-iliac in patients with degenerative scoliosis

Abstract:Objective:To investigate how pelvic incidence (PI) would change during the follow-up in degenerative scoliosis (DS) patients who underwent second sacralalar-iliac (S 2AI) fixation and identify the possible factors associated with the changes in PI. Methods:The DS patients who underwent long fusion to pelvis with S 2AI fixation with a minimum follow-up of two years between November 2014 to January 2017 were retrospectively reviewed in this study. The following sagittal radiographic parameters were measured, including pelvic incidence (PI), lumbar lordosis (LL), pelvic tilt (PT), PI minus LL (PI-LL), and sagittal vertical axis (SVA) at pre-operation, post-operation and 2-year follow-up. Patients were divided into two groups at immediate post-operation: PI decreased less than 5° or increased (Group PI stabilization); PI decreased larger than 5° (Group PI activity). Descriptive statistics were calculated for all patients in the form of mean value and standard deviation (SD). Comparisons of means between variables were performed using an unpaired Student's t test. Pearson correlation coefficienttest was performed to determine the correlations between all radiographic variables. Inter- and intra-observer reliability was assessed using intraclass correlation coefficient (ICC). The internal consistency of the measurements was characterized as excellent ( ICC≥0.9), good (0.7≤ ICC<0.9), acceptable (0.6< ICC≤0.7), poor (0.5≤ ICC<0.6), or unpredictable ( ICC<0.5). Results:There were no significant differences in terms of age, sex, radiographic measurements and scores of SRS-22 between twogroups preoperatively ( P>0.05). 80 DS patients with a mean age of 55.3±16.2 years were enrolled in this study with a mean follow-up period of 34.6±8.7 months. At post-operation, 39 patients (38.8%) were in group PI stabilization whose PI decreased from 45.7°±11.4° to 45.3°±11.2° with no significant difference; while the other 41 (61.2%) were in group PI activity whose PI significantly decreased from 51.6°±14.5° to 40.9°±14.0°. At the last follow-up, 24 patients (49%) in group PI activity had PI returned with an increase of larger than 5°; while the other 25 (51%) showed no increase with a mean ΔPI change of -4.2°. Subgroup comparison revealed that ΔPI, post-operation PI, post-operation PT and age were significantly different between the two subgroups. Pre-operation PI, post-operation PI, post-operation PT, post-operation PI-LL were significantly correlated with ΔPI at the last follow-up. Logistic regression analysis showed that post-operation PI was the associated factor ( OR=0.87, P=0.024). Conclusion:PI decreased in more than half of DS patients after spinal surgery using S 2AI screws, while returned among 48% of them during 2-year follow-up. Lower pre-operation PI, post-operation PI and PT were strongly associated with the return of PI.
