Abstract:A novel nano-titania modified covalent organic frameworks (NTM-COFs) has been synthesized and characterized by transmission electron microscopy (TEM) and scanning electron micrographs (SEM). Besides, NTM-00Fs, as efficient sorbent, has also been evaluated in the on-line pass-through cleanup procedure prior to the analysis of local anesthetic drugs (lidocaine, bupivacaine and tetracaine) in human plasma by liquid chromatography-tandem quadrupole mass spectrometry (LC-MS/MS). Under optimum conditions, the level of matrix effects can be dramatically reduced by the NTM-00Fs based on-line pass-through cleanup procedure, with acceptable recoveries ranging from 88.8% to 103%. Satisfactory trueness and precision of the proposed method were also obtained, with the within- and between-run RSDs less than 7.0% and relative error (REs) less than 12%. The limits of detections (LODs) of lidocaine, bupivacaine and tetracaine were 0.029 mu g1(-1), 0.027 mu g.L-1 and 0.016 mu gl(-1), respectively. The analytical method has been successfully applied for the determination of the plasma concentrations of bupivacaine in five parturients who received an epidural administration of bupivacaine hydrochloride during vaginal delivery. Results demonstrate the applicability of the developed NTM-COFs on-line pass-through cleanup procedure coupled with LC-MS/MS method to clinical studies.
Propofol does not alter the protein binding and unbound concentration of lidocaine at clinically targeted plasma concentrations in vitro - A short communication

Abstract:Background: Intravenous lidocaine is increasingly used as an analgesic adjunct during general anaesthesia. Lidocaine is highly protein-bound and changes to binding can alter drug efficacy or toxicity. We aimed to measure the effect of various propofol and lidocaine plasma concentration combinations on the protein binding and concentration of lidocaine in vitro. Methods: Known targeted concentrations of propofol and lidocaine were added to drug-free human plasma in vitro. Samples were prepared and analysed in various clinically relevant concentration combinations; propofol at 0, 2, 4 and 6 µg/mL, and lidocaine at 1, 3 and 5 µg/mL. The total and unbound concentrations of lidocaine were measured by ultra-high performance liquid chromatography-mass spectrometry and percentage protein binding was determined. Data were presented as mean and standard deviation (SD) and differences between analysed groups. Results: The overall mean protein binding of lidocaine was 68.8% (SD 5.5, range 57.5-80.9%). Beta regression analysis revealed no statistically significant difference in lidocaine percentage binding across a range of propofol and lidocaine concentration combinations. Conclusion: Propofol did not alter the unbound and free pharmacologically active proportion of lidocaine at different clinically targeted concentrations of propofol and lidocaine in plasma in vitro. The percentage of plasma protein binding of lidocaine in this study was consistent with previously published results.
Monitoring Anesthetic Depth Based on Lattice Complexity of Electroencephalogram

Abstract:Based on fine-graining method and two partition methods(permutation partition and average partition), lattice complexity(LC)was used in EEG complexity analysis under anesthesia in this paper.By using the EEG of 40 patients who underwent general anesthesia with propofol,we calculated the Lattice complexity and lempel-ziv(LZ) complexity and compared Pearson's correlation coefficient between LC,LZ and BIS values. Results showed that during anesthesia induction period ,average partition with lattice complexity get better effect. While during anesthesia recovery period,permutation partition with lattice complexity get better result,and the Pearson's correlation coeffi?cient between LC and BIS values was 0.963 6. During the whole anesthesia period,Average partition with lattice com?plexity get better effect,and the the Pearson's correlation coefficient between LC and BIS values was 0.841 6. The fine-graining order take 3 can get good result. All these provide a new method for clinical anesthetic depth monitoring.
The effect of limiting molar conductivity on drug iontophoresis

Abstract:OBJECTIVE: To study the effect of limiting molar conductivity(Λ 0)on drug iontophoresis. METHOD: Dicaine hydrochloride, procaine hydrochloride, dyclocaine hydrochloride, lidocaine hydrochloride and bupivacaine hydrochloride were used as model drugs . The enhancing rate (ER) of model drugs through rat skin were determined respectively using valia-chien two-chamber diffusion cells. RESULTS: Linear regression of four drugs (dicaine hydrochloride, procaine hydrochloride, dyclocaine hydrochloride and lidocaine hydrochloride) showed that ER = -76.49 + 1.045 x 10 -2 Λ 0 (r = 0.9998). According to this equation, the ER of bupivacaine hydrochloride was 14.2. It was very close to the ER of bupivacaine hydrochloride determined in experiment (ER = 14). CONCLUSION: The limiting molar conductivity is proportion to enhancing rate.

Abstract:A HPLC method for determination of lidocaine in rabbit plasma was established. The pharmacokinetics of lidocaine hydrochloride by different routes of administration, i.e., intravenous injection, nasal gel, nasal solution and oral solution preparations was investigated in rabbits. A C18 column was used with the mobile phase of methanol-0.01mol/L sodium dihydrogen phosphate (pH 2.1) (30∶70), at the detection wavelength of 263nm. The pharmacokinetic parameters were t1/2 (62.06±8.72), (63.60±11.32), (59.16±5.58), (59.55±7.63)min, AUC0→180min(1233.74±390.82), (1230.38± 344.32), (721.09±268.60), (334.93±120.97)μg·min·ml-1, respectively. The maximum concentration(Cmax)and absolute bioavailabilities of the latter three preparations were (18.36±3.55), (13.68±2.58), (4.64±1.77)μg/ml, and(99.73± 20.92)%, (58.45±14.10)%, (27.15±6.47)%, respectively. The results of variance analysis and two one-side t test showed that lidocaine hydrochloride nasal gel and its injection were bioequivalent.

Abstract:Software-defined radios (SDR) require robust synchronization algorithms which are suitable for implementation on generic programmable platforms. In this paper, we propose and study a low-complexity digital clock recovery scheme for implementation on programmable digital signal processing (DSP) or field-programmable gate-array (FPGA) platforms. Performance is established in terms of mean-square timing error and the required computational complexity. It is shown that the proposed algorithm achieves a superior performance as compared with the existing algorithms for a wide range of operating parameters.
Serum lidocaine (lignocaine) concentrations during prolonged perioperative infusion in patients undergoing breast cancer surgery: A secondary analysis of a randomised controlled trial

Abstract:Perioperative lidocaine (lignocaine) infusions are being employed with increasing frequency. The determinants of systemic lidocaine concentrations during prolonged administration are unclear. In the Long-term Outcomes after Lidocaine Infusions for PostOperative Pain (LOLIPOP) pilot trial, the impact of infusion duration and body size metrics on serum lidocaine concentrations was examined with regression models in 48 women undergoing breast cancer surgery. Lidocaine was delivered as an intravenous bolus (1.5 mg/kg) and infusion (2 mg/kg per h) intraoperatively, followed by a 12-h subcutaneous infusion (1.33 mg/kg per h) postoperatively. Dosing was based on total body weight. Wound infiltration with other long-acting local anaesthetics was permitted. Protein binding and pharmacogenomic data were also collected. Lidocaine concentrations (median (interquartile range) (range)) during prolonged administration were in the safe and potentially therapeutic range: post-anaesthesia care unit 2.16 (1.73-2.82) (1.12-6.06) µg/ml; ward 1.41 (1.22-1.75) (0.64-2.81) µg/ml. Concentrations increased non-linearly during the early intravenous phase of administration (mean rise 1.21 µg/ml per hour of infusion, P = 0.007) but reached a pseudo steady-state during the later subcutaneous phase. Higher dose rates received per kilogram of lean (P = 0.004), adjusted (P = 0.006) and ideal body weight (P = 0.009) were associated with higher steady-state concentrations. The lidocaine free fraction was unaffected by the presence of ropivacaine, and phenotypes linked to slow metabolism were infrequent. Serum lidocaine concentrations reached a pseudo steady-state during a 12-h postoperative infusion. Greater precision in steady-state concentrations can be achieved by dosing on lean body weight versus adjusted or ideal body weight (equivalent lean body weight doses: intravenous bolus 2.5 mg/kg; intravenous infusion 3.33 mg/kg per h; subcutaneous infusion 2.22 mg/kg per h.

Abstract:Objective To study the effect on pharmacokinetics of lidocaine with partial hepatectomy during the operation. Methods Partial hepatectomy was chosen as the test group and abdominal operation was used as the control group. Venous blood plasma was collected before dosing and enactment time after first epidural dose of 4 mg·kg -1 and epidural infusion 2 mg·h·kg -1 of lidocaine for 2 h. The plasma concentration of lidocaine was measured by fluorescence polarization immunoassay. The pharmacokinetic parameters were calculated with pharmacokinetic non-linear program WINNOLIN. The effect on pharmacokinetics of lidocaine with partial hepatectomy was evaluated by comparing the parameters. Results The concentration-time curves of lidocaine fitted a one-compartment model for both test and control groups. The main pharmacokinetic parameters were as follow: (147.03±67.05) and (157.16±33.19) min for t 1/2 , (2.90±0.57) and (2.71±0.38) μg·mL -1 for C 120 , (1.60±0.40) and (1.65±0.26) L for V d, (844.32±295.82) and (896.09±240.77) μg·min·mL -1 for AUC, respectively. There was no significantly statistical difference between the two groups. Conclusion There was no evident effect of partial hepatectomy on pharmacokinetics of lidocaine.
A study of the in vitro clinical interaction between lidocaine and premedications using rat liver microsomes

Abstract:In this study, we have investigated the relationship between lidocaine metabolism and premedication, i.e., psychotropic and anti-anxiety agents (diazepam, midazolam), hypnotics (pentobarbital, thiamylal), depolarizing muscular relaxants (vecuronium, pancuronium and suxamethonium), an active anti-hypertensive (clonidine) and an H2 receptor antagonist (cimetidine) using rat hepatic microsomes in vitro. Lidocaine metabolism was noncompetitively inhibited by midazolam (Ki=29.0 microM). Thilamylal was a moderate competitive inhibitor of lidocaine metabolism (Ki=77.8 microM). Pentobarbital, diazepam and cimetidine weakly inhibited lidocaine metabolism formation in a concentration-dependent manner at high substrate concentrations. On the other hand, vecuronium, pancuronium, suxamethonium and clonidine did not inhibit lidocaine metabolism over the therapeutic range. These results show that the interaction between lidocaine and midazolam and thiamylal, catalyzed by a similar cytochrome P450, is of potential importance in toxicological and clinical studies.
Tumescent technique without epinephrine for endovenous laser therapy and serum lidocaine concentration

Abstract:Objective: Endovenous laser therapy (EVLT) requires tumescent lidocaine anesthesia. Although it is well known that the absorption of local anesthetic varies according to the injection site, little evidence exists establishing the maximum recommended safe dose for extravascular injections such as those used for EVLT. The aim of this study was to evaluate plasma concentration of lidocaine over time after administration of tumescent lidocaine during EVLT procedures in healthy volunteers. Methods: Between January 2011 and February 2013, 10 healthy patients scheduled for an EVLT procedure performed in a hospital setting were recruited to participate in an observational study. For each subject, a total of 10 venous samples were obtained for analysis after surgical injection of the tumescent lidocaine solution (0.1% concentration). Samples were collected at baseline (before the surgical procedure start) and then every 30 minutes for the first 2 hours after the initial lidocaine injection. Thereafter, venous samples were obtained every 2 hours, with the last sample drawn 12 hours after the surgeon's initial lidocaine injection. All specimens were drawn from a dedicated intravenous catheter, immediately placed in a heparinized blood collection tube, and centrifuged for 10 minutes at 3000 rpm. Plasma was then removed with a pipette and stored at -70 °C until analyzed. Total and free plasma lidocaine concentrations were determined by immunoassay. Plasma lidocaine concentrations were normalized by peak concentration for statistical comparisons. Results: Laboratory data were available for nine of the 10 volunteers. The mean total lidocaine dose administered was 6.38 (± 2.2) mg/kg (range, 3.57-10.7 mg/kg). The total lidocaine blood levels ranged from 0.48 (± 0.28) to 1.3 (± 0.49) mcg/mL. The free lidocaine blood levels ranged from nondetectable to 0.76 (± 0.43) mcg/mL. The average total time of injection for the group was 32.8 (± 10.0) minutes (range, 21-49 minutes). Among all dose ranges, both total and free lidocaine peak blood level ratios occurred at times 60 to 120 minutes (P < .05). No value considered in the statistical analysis exceeded 5 mcg/mL at any time. Conclusions: Tumescent lidocaine without epinephrine for EVLT procedures produces a peak serum concentration at 60 to 120 minutes. The peak plasma concentration as observed with the standard institutional dosing of tumescent lidocaine appeared below the threshold for human toxicity.
Time Course of Acute Neuroprotective Effects of Lidocaine Evaluated by Brain Impedanciometry in the Global Ischemia Model

Abstract:Voltage-activated sodium channels play a primary role during ischemic brain edema and thus are a pharmacological target for therapy. Lidocaine, a sodium channel blocker, was tested in male Sprague-Dawley rats anesthetized with thiobarbital (60 mg·kg(-1) i.p.) and perfused i.v. with Ringer's solution (n = 9) or lidocaine (0.75 mg·kg(-1), n = 9, or 1.5 mg·kg(-1), n = 6). Two tungsten microelectrodes were implanted in the cerebral cortex to register changes in tissue impedance in response to the voltage fall of a square wave electric pulse (100 μA, 10 ms), before and after infusion of lidocaine or Ringer's solution and during global cerebral ischemia due to a respiratory arrest induced by D-tubocurarine. Lidocaine infusion under normoxic conditions did not change voltage values (Mann-Whitney U = 51; p > 0.05). In animals infused with Ringer's solution, the voltage fall induced by global cerebral ischemia was fast for ∼8 min at -8.0 ± 2.3%·min(-1) followed by a slow decay at -0.96 ± 0.17%·min(-1). The time constant of voltage decay (λ) was 215.6 s (F = 547.4; p = 0.00000). Voltage values of lidocaine-infused animals were significantly higher than those of rats infused with Ringer's solution (U = 100; p = 0.000089). The decay rates were -4.97 ± 1.36%·min(-1) (fast phase) and -1.04 ± 0.3%·min(-1) (slow phase) with λ = 672.5 s (+211.9%; p = 0.000000). These results suggest that lidocaine significantly reduced cerebral impedance, hence exerting a strong early anti-edema effect probably by blocking voltage-activated sodium channels.

Profibrillatory effects of lidocaine in the acutely ischemic porcine heart

Abstract:Because recent clinical studies have failed to show evidence of the benefit of lidocaine in the arrhythmias occurring in the early stage of myocardial infarction and have even shown an increased mortality in patients thus treated, we investigated the value of lidocaine as a protective agent against ventricular fibrillation related to myocardial ischemia in the in situ heart of anesthetized open-chest pigs subjected to transient total occlusion of the proximal left anterior descending coronary artery (LAD) under ventricular pacing at a constant high rate. Vulnerability to the fibrillatory process induced by coronary occlusion was assessed both by time to onset of ventricular fibrillation (TF) and by electrical ventricular fibrillation threshold (EFT) determined after coronary occlusions of increasing duration (30, 60, 120, 180 s). Monophasic action potential (MAP) was recorded concurrently in the nonischemic and ischemic areas. Lidocaine, even in relatively high doses (2-4 mg.kg-1), did not prolong TF, nor did it increase EFT. On the contrary, TF was significantly shortened and EFT was significantly decreased (15-30%) at the maximal concentrations of lidocaine, with return to control values in 40-60 min. Therefore, lidocaine tends to increase the risk of ischemic ventricular fibrillation (VF): It fails to control the extreme enhancement of excitability and worsens conduction disorders, even though it decreases normal conduction only slightly. Use of lidocaine against rhythm disorders in acute myocardial infarction (AMI), is at least debatable and probably contraindicated.

Intermolecular Dynamics and Structure in Aqueous Lidocaine Hydrochloride Solutions

Abstract:We investigated the intermolecular dynamics and static structure in the aqueous solutions of lidocaine hydrochloride (LDHCl) in the concentration range of [LDHCl] = 0-2.00 M using femtosecond Raman-induced Kerr effect spectroscopy (fs-RIKES), small- and wide-angle X-ray scattering (SWAXS), and dynamic light scattering (DLS). For the fs-RIKES experiments, the concentration dependence of the difference low-frequency spectra of the aqueous LDHCl solutions relative to the neat water, which was mainly due to the intermolecular vibrations, was characterized using an exponential function with a characteristic concentration of ∼1 M. For the SWAXS experiments, we observed a manifestation of an excess scattering component centered within a range of 8-10 nm-1 in the aqueous LDHCl solutions. The results of Fourier inversion and further deconvolution analyses unambiguously demonstrated that lidocaines assemble into a nanometer-sized micelle-like structure with the innermost core (∼0.3 nm) and outer shell (∼0.5 nm), respectively. The DLS experiments also found nanometer-sized aggregates and further indicated evidence of the clusters of the aggregates. The results of viscosities, densities, and surface tensions of the solutions and the quantum chemistry calculations supported the unique features of the microscopic intermolecular interaction and the micelle-like aggregation.
Rate-dependent effects of lidocaine on cardiac dynamics: Development and analysis of a low-dimensional drug-channel interaction model

Abstract:State-dependent Na+ channel blockers are often prescribed to treat cardiac arrhythmias, but many Na+ channel blockers are known to have pro-arrhythmic side effects. While the anti and proarrhythmic potential of a Na+ channel blocker is thought to depend on the characteristics of its rate-dependent block, the mechanisms linking these two attributes are unclear. Furthermore, how specific properties of rate-dependent block arise from the binding kinetics of a particular drug is poorly understood. Here, we examine the rate-dependent effects of the Na+ channel blocker lidocaine by constructing and analyzing a novel drug-channel interaction model. First, we identify the predominant mode of lidocaine binding in a 24 variable Markov model for lidocaine-Na+ channel interaction by Moreno et al. We then develop a novel 3-variable lidocaine-Na+ channel interaction model that incorporates only the predominant mode of drug binding. Our low-dimensional model replicates the extensive voltage-clamp data used to parameterize the Moreno et al. model. Furthermore, the effects of lidocaine on action potential upstroke velocity and conduction velocity in our model are similar to those predicted by the Moreno et al. model. By exploiting the low-dimensionality of our model, we derive an algebraic expression for level of rate-dependent block as a function of pacing frequency, restitution properties, diastolic and plateau potentials, and drug binding rate constants. Our model predicts that the level of rate-dependent block is sensitive to alterations in restitution properties and increases in diastolic potential, but it is insensitive to variations in the shape of the action potential waveform and lidocaine binding rates.

Abstract:Objective To investigate in vitro percutaneous characteristics of compound lidocaine cream, and the consistenty of in - vitrope-rcutaneous with the Emla cream. Methods The concentration of lidocaine and prilocaine in receptor fluid was quantified by HPLC. The in - vitro percutaneous experiments of compound lidocaine cream through rat abdomen skin were perfromed by modified Franz′s cell. Results There were good linear relationship between the peak area and concentration in the range of 0. 224 - 16. 8 μg / mL for lidocaine, andin the range of 0. 192 - 14. 414 μg / mL for prilocaine. The accuracy and precision could meet the require-ments. Percutaneous characteristicsof lidocaine and prilocaine accord with zero order kinetics equation. The flux oflidocainefor test and control creamis (274. 6 ± 30. 25) μg / (cm2·h)and (266. 4 ± 21. 56 )μg / (cm2·h), and the flux ofprilocainefor test and control creamis (314. 2 ± 31. 08)μg / (cm2 ·h) and (309. 4 ± 23. 08) μg / (cm2 ·h) There were no significant difference( P > 0. 05) by t - test statis-tics. Conclusion In vitropercutaneous patternare consistent for test and control compound lidocaine creams.
Effects of bupivacaine and lidocaine on cardiac function in awake and pentobarbital-anesthetized rats

Abstract:Using an implanted Doppler crystal, we evaluated emodynamic changes induced by subconvulsive doses of bupivacaine and lidocaine in awake and pentobarbitalanesthetized rats. Low doses of lidocaine (2.0 mg·kg−1) and bupivacaine (0.5 mg·kg−1) changed hemodynamics minimally. However, a high dose of lidocaine (8.0 mg·kg−1) reduced heart rate, cardiac output, and regional myocardial wall thickening for a short period with or without anesthesia. In contrast, a high dose of bupivacaine (2.0 mg·kg−1) increased mean arterial pressure and did not change heart rate or regional myocardial wall thickening in the awake state. Under pentobarbital anesthesia, a high dose of lidocaine reduced mean arterial pressure significantly shortly after the injection, but bupivacaine did not. Thus, it is unlikely that bupivacaine has more potent cardiotoxicity than lidocaine in subconvulsive doses.

Abstract:Introduction Erector spinae plane (ESP) block is an emerging interfascial block with a wide range of indications for perioperative analgesia and chronic pain treatment. Recent studies have focused their attention on mechanisms of action of ESP block. However, the pharmacokinetics of drugs injected in ESP is, as of now, uninvestigated. The aim of this brief report is to investigate the pharmacokinetics of lidocaine in a series of 10 patients. Methods We are reporting a case series of 10 patients undergoing bilateral ESP block for multilevel lumbar spine surgery. ESP was performed with 3.5 mg/kg of lidocaine based on ideal body weight. Lidocaine concentration was dosed at 5, 15, 30 min and at 1, 2 and 3 hours. Results T max was 5 min for all the patients. C max ranged from 1.2 to 3.8 mg/L (mean: 2.59 mg/L). AUC 0-3 was high (76%, on average) suggesting an almost complete bioavailability. Age had a negative correlation with T1⁄2 of lidocaine. Conclusions Lidocaine pharmacokinetic after ESP block is well-described by a two-compartment model with a rapid and extensive rate of absorption. Nevertheless, its peak concentrations never exceeded the accepted toxicity limit. Elimination half-life was slightly prolonged, probably due to the advanced age of some patients.

Abstract:STUDY OBJECTIVE: To evaluate the effect of lidocaine inhalation on the circulatory response to direct laryngoscopy and endotracheal intubation.DESIGN: Prospective, randomized study.SETTING: Operating theater at a public hospital.PATIENTS: Eighty patients (ASA physical status I and II ages 25 to 45 years) scheduled for major abdominal surgery.INTERVENTIONS: In the first stage, 40 patients were randomly assigned to receive inhalation of either lidocaine 40 mg or a 0.9% solution of sodium chloride (placebo). In the second stage, the next 20 consecutive patients received inhalation of lidocaine 120 mg, and another 20 consecutive patients received intravenous (IV) lidocaine 1 mg/kg.MEASUREMENTS AND MAIN RESULTS: Mean arterial pressure rose significantly in the i.v. lidocaine group (21.2 mmHg; p < 0.05), the saline inhalation group (29.2 mmHg; p < 0.05), and the lidocaine 40 mg inhalation group (22.9 mmHg; p < 0.05), but not in the lidocaine 120 mg inhalation group (10.1 mmHg). The heart rate (HR) response to intubation with lidocaine inhalation was dose dependent. In the saline inhalation group, HR increased by 15.6 beats per minute (bpm) (p < 0.05); in the lidocaine 40 mg inhalation group, HR increased by 9.1 bpm (p < 0.05); and in the lidocaine 120 mg inhalation group, HR increased by only 3.1 bpm.CONCLUSION: Inhalation of lidocaine 120 mg prior to induction of anesthesia is an effective, safe, and convenient method to attenuate the circulatory response to laryngoscopy and endotracheal intubation.
