Abstract:Aims and background Isolated vaginal recurrences of endometrial carcinoma are rare, and prognostic factors that predict treatment outcome are still not well defined. The aim of the present study was to evaluate the results of brachytherapy in isolated vaginal recurrences from endometrial carcinoma. Methods Thirty-five patients with isolated vaginal recurrences were treated with brachytherapy with intravaginal ovoids or cylinders that were calculated to deliver 6000 to 7000 cGy at the surface. Patients were assessed for size and location of recurrence at presentation, response and complications from therapy. Results Treatment was well tolerated by most patients. Grade 2 toxicity occurred in 4 patients (3 cases of partial vaginal stenosis and one proctitis). Complete response to radiation was observed in all patients, and an overall 9 failures were observed (4 local, 4 distant and 1 local plus distant). Twenty patients (57%) were alive without evidence of disease at 3 to 11 years following treatment. Site of vaginal recurrence (upper third versus others) and long (more than 12 months versus less than 12 months) interval from hysterectomy were the only factors significantly related to local failures. Conclusions Isolated vaginal recurrences following hysterectomy for endometrial carcinoma can be treated with brachytherapy with a low rate of severe toxicity.
Automated Detection of Focal Cortical Dysplasia Based on Magnetic Resonance Imaging and Positron Emission Tomography.

Abstract:PURPOSE:Focal cortical dysplasia (FCD) is a common etiology of drug-resistant focal epilepsy. Visual identification of FCD is usually time-consuming and depends on personal experience. Herein, we propose an automated type II FCD detection approach utilizing multi-modal data and 3D convolutional neural network (CNN).METHODS:MRI and positron emission tomography (PET) data of 82 patients with FCD were collected, including 55 (67.1%) histopathologically, and 27 (32.9%) radiologically diagnosed patients. Three types of morphometric feature maps and three types of tissue maps were extracted from the T1-weighted images. These maps, T1, and PET images formed the inputs for CNN. Five-fold cross-validations were carried out on the training set containing 62 patients, and the model behaving best was chosen to detect FCD on the test set of 20 patients. Furthermore, ablation experiments were performed to estimate the value of PET data and CNN.RESULTS:On the validation set, FCD was detected in 90.3% of the cases, with an average of 1.7 possible lesions per patient. The sensitivity on the test set was 90.0%, with 1.85 possible lesions per patient. Without the PET data, the sensitivity decreased to 80.0%, and the average lesion number increased to 2.05 on the test set. If an artificial neural network replaced the CNN, the sensitivity decreased to 85.0%, and the average lesion number increased to 4.65.SIGNIFICANCE:Automated detection of FCD with high sensitivity and few false-positive findings is feasible based on multi-modal data. PET data and CNN could improve the performance of automated detection.
Visual Explanation for Identification of the Brain Bases for Dyslexia on fMRI Data

Abstract:Brain imaging of mental health, neurodevelopmental and learning disorders has coupled with machine learning to identify patients based only on their brain activation, and ultimately identify features that generalize from smaller samples of data to larger ones. However, the success of machine learning classification algorithms on neurofunctional data has been limited to more homogeneous data sets of dozens of participants. More recently, larger brain imaging data sets have allowed for the application of deep learning techniques to classify brain states and clinical groups solely from neurofunctional features. Deep learning techniques provide helpful tools for classification in healthcare applications, including classification of structural 3D brain images. Recent approaches improved classification performance of larger functional brain imaging data sets, but they fail to provide diagnostic insights about the underlying conditions or provide an explanation from the neural features that informed the classification. We address this challenge by leveraging a number of network visualization techniques to show that, using such techniques in convolutional neural network layers responsible for learning high-level features, we are able to provide meaningful images for expert-backed insights into the condition being classified. Our results show not only accurate classification of developmental dyslexia from the brain imaging alone, but also provide automatic visualizations of the features involved that match contemporary neuroscientific knowledge, indicating that the visual explanations do help in unveiling the neurological bases of the disorder being classified.

Abstract:Objective dyslexia diagnosis is not a straighforward task since it is traditionally performed by means of the intepretation of different behavioural tests. Moreover, these tests are only applicable to readers. This way, early diagnosis requires the use of specific tasks not only related to reading. Thus, the use of Electroencephalography (EEG) constitutes an alternative for an objective and early diagnosis that can be used with pre-readers. In this way, the extraction of relevant features in EEG signals results crucial for classification. However, the identification of the most relevant features is not straighforward, and predefined statistics in the time or frequency domain are not always discriminant enough. On the other hand, classical processing of EEG signals based on extracting EEG bands frequency descriptors, usually make some assumptions on the raw signals that could cause indormation loosing. In this work we propose an alternative for analysis in the frequency domain based on Singluar Spectrum Analysis (SSA) to split the raw signal into components representing different oscillatory modes. Moreover, correlation matrices obtained for each component among EEG channels are classfied using a Convolutional Neural network.

Abstract:Diagnosis of dyslexia often occurs in late schooling years, leading to academic and psychological challenges. Furthermore, diagnosis is time-consuming, costly, and reliant on arbitrary cutoffs. On the other hand, automated algorithms hold great potential in medical and psychological diagnostics. The aim of the present study was to develop a machine learning tool for the detection of dyslexia in children based on the intrinsic connectivity patterns of different brain networks underlying perception and attention. Here, 117 children (8-12 years old; 58 females; 52 typical readers; TR and 65 children with dyslexia) completed cognitive and reading assessments and underwent 10 minutes of resting-state fMRI. Functional connectivity coefficients between 264 brain regions were used as features for machine learning. Different supervised algorithms were employed for classification of children with and without dyslexia. A classifier trained on dorsal attention network features exhibited the highest performance (accuracy .79, sensitivity .92, specificity .64). Auditory, visual, and fronto-parietal network-based classification showed intermediate accuracy levels (70-75%). These results highlight significant neurobiological differences in brain networks associated with visual attention between TR and children with dyslexia. Distinct neural integration patterns can differentiate dyslexia from typical development, which may be utilized in the future as a biomarker for the presence and/or severity of dyslexia.

Abstract:Background Dyslexia is a neurological disorder that affects an individual's language processing abilities. Early care and intervention can help dyslexic individuals succeed academically and socially. Recent developments in deep learning (DL) approaches motivate researchers to build dyslexia detection models (DDMs). DL approaches facilitate the integration of multi-modality data. However, there are few multi-modality-based DDMs. Methods In this study, the authors built a DL-based DDM using multi-modality data. A squeeze and excitation (SE) integrated MobileNet V3 model, self-attention mechanisms (SA) based EfficientNet B7 model, and early stopping and SA-based Bi-directional long short-term memory (Bi-LSTM) models were developed to extract features from magnetic resonance imaging (MRI), functional MRI, and electroencephalography (EEG) data. In addition, the authors fine-tuned the LightGBM model using the Hyperband optimization technique to detect dyslexia using the extracted features. Three datasets containing FMRI, MRI, and EEG data were used to evaluate the performance of the proposed DDM. Results The findings supported the significance of the proposed DDM in detecting dyslexia with limited computational resources. The proposed model outperformed the existing DDMs by producing an optimal accuracy of 98.9%, 98.6%, and 98.8% for the FMRI, MRI, and EEG datasets, respectively. Healthcare centers and educational institutions can benefit from the proposed model to identify dyslexia in the initial stages. The interpretability of the proposed model can be improved by integrating vision transformers-based feature extraction.

Dyslexia Diagnosis by EEG Temporal and Spectral Descriptors: An Anomaly Detection Approach

Abstract:Diagnosis of learning difficulties is a challenging goal. There are huge number of factors involved in the evaluation procedure that present high variance among the population with the same difficulty. Diagnosis is usually performed by scoring subjects according to results obtained in different neuropsychological (performance-based) tests specifically designed to this end. One of the most frequent disorders is developmental dyslexia (DD), a specific difficulty in the acquisition of reading skills not related to mental age or inadequate schooling. Its prevalence is estimated between 5% and 12% of the population. Traditional tests for DD diagnosis aim to measure different behavioral variables involved in the reading process. In this paper, we propose a diagnostic method not based on behavioral variables but on involuntary neurophysiological responses to different auditory stimuli. The experiments performed use electroencephalography (EEG) signals to analyze the temporal behavior and the spectral content of the signal acquired from each electrode to extract relevant (temporal and spectral) features. Moreover, the relationship of the features extracted among electrodes allows to infer a connectivity-like model showing brain areas that process auditory stimuli in a synchronized way. Then an anomaly detection system based on the reconstruction residuals of an autoencoder using these features has been proposed. Hence, classification is performed by the proposed system based on the differences in the resulting connectivity models that have demonstrated to be a useful tool for differential diagnosis of DD as well as a method to step towards gaining a better knowledge of the brain processes involved in DD. The results corroborate that nonspeech stimulus modulated at specific frequencies related to the sampling processes developed in the brain to capture rhymes, syllables and phonemes produces effects in specific frequency bands that differentiate between controls and DD subjects. The proposed method showed relatively high sensitivity above 0.6, and up to 0.9 in some of the experiments.
Detecting Phase-Synchrony Connectivity Anomalies in EEG Signals. Application to Dyslexia Diagnosis

Abstract:Objective Dyslexia diagnosis is a challenging task, since traditional diagnosis methods are not based on biological markers but on behavioural tests. Although dyslexia diagnosis has been addressed by these tests in clinical practice, it is difficult to extract information about the brain processes involved in the different tasks and, then, to go deeper into its biological basis. Thus, the use of biomarkers can contribute not only to the diagnosis but also to a better understanding of specific learning disorders such as dyslexia. In this work, we use Electroencephalography (EEG) signals to discover differences among controls and dyslexic subjects using signal processing and artificial intelligence techniques. Specifically, we measure phase synchronization among channels, to reveal the functional brain network activated during auditory processing. On the other hand, to explore synchronicity patterns risen by low-level auditory processing, we used specific stimuli consisting in band-limited white noise, modulated in amplitude at different frequencies. The differential information contained in the functional (i.e., synchronization) network has been processed by an anomaly detection system that addresses the problem of subjects variability by an outlier-detection method based on vector quantization. The results, obtained for 7 years-old children, show that the proposed method constitutes an useful tool for clinical use, with the area under ROC curve (AUC) values up to 0.95 in differential diagnosis tasks.

Abstract:Background: Dyslexia is a type of specific learning disability (SLD) which has neurobiological origin. It is characterized by difficulties with accurate and/or fluent word recognition and by poor spelling and decoding abilities. The impaired reading in dyslexia is associated with inability to process the sensory input that enters the nervous system. Functional magnetic resonance imaging (fMRI) has emerged as a potential source in understanding the neurobiology and to identify the brain basis of sensory stimuli processed in dyslexic patients. Methodology: The present study was conducted to assess the difference in neural changes using fMRI in children and adolescents with SLD compared with normal children and also the correlation of clinical parameters with BOLD - fMRI changes. Sixteen children and adolescents diagnosed as dyslexia were assessed with All India Institute of Medical Sciences (AIIMS) SLD Battery, Mini-International Neuropsychiatric Interview for Children and Adolescents (MINI KID) and Aggregated Neurobehavioral Student Health and Educational Review (ANSER) system and compared with 15 controls matched with age and sex. Participants of both groups were asked to perform 3 tasks during the fMRI acquisition (phonological, picture-naming and semantic tasks). Result: As compared to control group, the participants with dyslexia show phonological decoding problem. During picture task, the participants with dyslexia use more areas of brain involve in recalling the memory events while during semantic tasks processing the occipito-temporal (fusiform) gyrus was less activated when in contrast to control. Conclusion: This study shows that participants with dyslexia fail to use normal brain regions specialized in language processing, but rather use different areas.
Dyslexia and Dysgraphia prediction: A new machine learning approach

Abstract:Learning disabilities like dysgraphia, dyslexia, dyspraxia, etc. interfere with academic achievements but have also long terms consequences beyond the academic time. It is widely admitted that between 5% to 10% of the world population is subject to this kind of disabilities. For assessing such disabilities in early childhood, children have to solve a battery of tests. Human experts score these tests, and decide whether the children require specific education strategy on the basis of their marks. The assessment can be lengthy, costly and emotionally painful. In this paper, we investigate how Artificial Intelligence can help in automating this assessment. Gathering a dataset of handwritten text pictures and audio recordings, both from standard children and from dyslexic and/or dysgraphic children, we apply machine learning techniques for classification in order to analyze the differences between dyslexic/dysgraphic and standard readers/writers and to build a model. The model is trained on simple features obtained by analysing the pictures and the audio files. Our preliminary implementation shows relatively high performances on the dataset we have used. This suggests the possibility to screen dyslexia and dysgraphia via non-invasive methods in an accurate way as soon as enough data are available.
Multimodal investigation of the neurocognitive deficits underlying dyslexia in adulthood

Abstract:Dyslexia is a neurobiological disorder characterised by reading difficulties, yet its underlying causes remain unclear. Neuroimaging and behavioural studies found anomalous responses in tasks requiring phonological processing, motion perception, and implicit learning, and showed gray and white matter abnormalities in several brain regions of dyslexics compared to controls, indicating that dyslexia is a heterogeneous condition and promoting a multifactorial approach. In order to evaluate whether the combination of behavioural and multimodal MRI can have greater sensitivity in identifying neurocognitive traits of dyslexia compared to monocomponential approaches, in 19 dyslexic and 19 control subjects we acquired behavioural cognitive assessments, multiple (phonological, visual motion, rhythmic) mismatch-response functional MRI tasks, structural diffusion-weighted and T1-weighted images. To examine between-group differences in the multimodal neurocognitive measures, we applied univariate and multivariate approaches. Results showed that dyslexics performed worse than controls in behavioural phonological tasks. Neuroimaging analyses revealed that individuals with dyslexia present reduced cerebellar responses to mismatching rhythmic stimuli, as well as structural disorganization in several white matter tracts and cortical regions previously implicated in dyslexia. Most importantly, in line with the view of dyslexia as a multifactorial phenomenon, a machine learning model trained with features from all three MRI modalities (functional, diffusion, and T1-weighted) discriminated between dyslexics and controls with greater accuracy than models including just one modality. The individual classification scores in the multimodal machine learning model correlated with behavioural reading accuracy. These results confirm that dyslexia should be approached as a composite condition characterised by multiple distinctive cognitive and brain features.

Unraveling Brain Synchronisation Dynamics by Explainable Neural Networks using EEG Signals: Application to Dyslexia Diagnosis

Abstract:The electrical activity of the neural processes involved in cognitive functions is captured in EEG signals, allowing the exploration of the integration and coordination of neuronal oscillations across multiple spatiotemporal scales. We have proposed a novel approach that combines the transformation of EEG signal into image sequences, considering cross-frequency phase synchronisation (CFS) dynamics involved in low-level auditory processing, with the development of a two-stage deep learning model for the detection of developmental dyslexia (DD). This deep learning model exploits spatial and temporal information preserved in the image sequences to find discriminative patterns of phase synchronisation over time achieving a balanced accuracy of up to 83%. This result supports the existence of differential brain synchronisation dynamics between typical and dyslexic seven-year-old readers. Furthermore, we have obtained interpretable representations using a novel feature mask to link the most relevant regions during classification with the cognitive processes attributed to normal reading and those corresponding to compensatory mechanisms found in dyslexia.

Abstract:Resting-state functional MRI (rs-fMRI) scans hold the potential to serve as a diagnostic or prognostic tool for a wide variety of conditions, such as autism, Alzheimer's disease, and stroke. While a growing number of studies have demonstrated the promise of machine learning algorithms for rs-fMRI based clinical or behavioral prediction, most prior models have been limited in their capacity to exploit the richness of the data. For example, classification techniques applied to rs-fMRI often rely on region-based summary statistics and/or linear models. In this work, we propose a novel volumetric Convolutional Neural Network (CNN) framework that takes advantage of the full-resolution 3D spatial structure of rs-fMRI data and fits non-linear predictive models. We showcase our approach on a challenging large-scale dataset (ABIDE, with N > 2,000) and report state-of-the-art accuracy results on rs-fMRI-based discrimination of autism patients and healthy controls.
Features and Machine Learning for Correlating and Classifying between Brain Areas and Dyslexia

Abstract:We develop a method that is based on processing gathered Event Related Potentials (ERP) signals and the use of machine learning technique for multivariate analysis (i.e. classification) that we apply in order to analyze the differences between Dyslexic and Skilled readers. No human intervention is needed in the analysis process. This is the state of the art results for automatic identification of Dyslexic readers using a Lexical Decision Task. We use mathematical and machine learning based techniques to automatically discover novel complex features that (i) allow for reliable distinction between Dyslexic and Normal Control Skilled readers and (ii) to validate the assumption that the most of the differences between Dyslexic and Skilled readers located in the left hemisphere. Interestingly, these tools also pointed to the fact that High Pass signals (typically considered as "noise" during ERP/EEG analyses) in fact contains significant relevant information. Finally, the proposed scheme can be used for analysis of any ERP based studies.

Abstract:Developmental Dyslexia (DD) is a learning disability related to the acquisition of reading skills that affects about 5% of the population. DD can have an enormous impact on the intellectual and personal development of affected children, so early detection is key to implementing preventive strategies for teaching language. Research has shown that there may be biological underpinnings to DD that affect phoneme processing, and hence these symptoms may be identifiable before reading ability is acquired, allowing for early intervention. In this paper we propose a new methodology to assess the risk of DD before students learn to read. For this purpose, we propose a mixed neural model that calculates risk levels of dyslexia from tests that can be completed at the age of 5 years. Our method first trains an auto-encoder, and then combines the trained encoder with an optimized ordinal regression neural network devised to ensure consistency of predictions. Our experiments show that the system is able to detect unaffected subjects two years before it can assess the risk of DD based mainly on phonological processing, giving a specificity of 0.969 and a correct rate of more than 0.92. In addition, the trained encoder can be used to transform test results into an interpretable subject spatial distribution that facilitates risk assessment and validates methodology.

Abstract:Several methods have been developed to extract information from electroencephalograms (EEG). One of them is Phase-Amplitude Coupling (PAC) which is a type of Cross-Frequency Coupling (CFC) method, consisting in measure the synchronization of phase and amplitude for the different EEG bands and electrodes. This provides information regarding brain areas that are synchronously activated, and eventually, a marker of functional connectivity between these areas. In this work, intra and inter electrode PAC is computed obtaining the relationship among different electrodes used in EEG. The connectivity information is then treated as a graph in which the different nodes are the electrodes and the edges PAC values between them. These structures are embedded to create a feature vector that can be further used to classify multichannel EEG samples. The proposed method has been applied to classified EEG samples acquired using specific auditory stimuli in a task designed for dyslexia disorder diagnosis in seven years old children EEG's. The proposed method provides AUC values up to 0.73 and allows selecting the most discriminant electrodes and EEG bands.

This mechanistic and phenotypic variation, as well as relatively modest sample sizes, may have limited the development of accurate neuroimaging-based classifiers for reading disability, including because of the large feature space of neuroimaging datasets. An unsupervised learning model was used to reduce deformation-based data to a lower-dimensional manifold and then supervised learning models were used to classify these latent representations in a dataset of 96 reading disability cases and 96 controls (mean age: 9.86 ± 1.56 years). A combined unsupervised autoencoder and supervised convolutional neural network approach provided an effective classification of cases and controls (accuracy: 77%; precision: 0.75; recall: 0.78). Brain regions that contributed to this classification accuracy were identified by adding noise to the voxel-level image data, which showed that reading disability classification accuracy was most influenced by the superior temporal sulcus, dorsal cingulate, and lateral occipital cortex. Regions that were most important for the accurate classification of controls included the supramarginal gyrus, orbitofrontal, and medial occipital cortex. The contribution of these regions reflected individual differences in reading-related abilities, such as non-word decoding or verbal comprehension. Together, the results demonstrate an optimal deep learning solution for classification using neuroimaging data. In contrast with standard mass-univariate test results, results from the deep learning model also provided evidence for regions that may be specifically affected in reading disability cases.
Towards Accessible Learning: Deep Learning-Based Potential Dysgraphia Detection and OCR for Potentially Dysgraphic Handwriting

Abstract:Dysgraphia is a learning disorder that affects handwriting abilities, making it challenging for children to write legibly and consistently. Early detection and monitoring are crucial for providing timely support and interventions. This study applies deep learning techniques to address the dual tasks of dysgraphia detection and optical character recognition (OCR) on handwriting samples from children with potential dysgraphic symptoms. Using a dataset of handwritten samples from Malaysian schoolchildren, we developed a custom Convolutional Neural Network (CNN) model, alongside VGG16 and ResNet50, to classify handwriting as dysgraphic or non-dysgraphic. The custom CNN model outperformed the pre-trained models, achieving a test accuracy of 91.8% with high precision, recall, and AUC, demonstrating its robustness in identifying dysgraphic handwriting features. Additionally, an OCR pipeline was created to segment and recognize individual characters in dysgraphic handwriting, achieving a character recognition accuracy of approximately 43.5%. This research highlights the potential of deep learning in supporting dysgraphia assessment, laying a foundation for tools that could assist educators and clinicians in identifying dysgraphia and tracking handwriting progress over time. The findings contribute to advancements in assistive technologies for learning disabilities, offering hope for more accessible and accurate diagnostic tools in educational and clinical settings.
