Abstract:BACKGROUND AND PURPOSE: MR imaging provides critical information about fetal brain growth and development. Currently, morphologic analysis primarily relies on manual segmentation, which is time-intensive and has limited repeatability. This work aimed to develop a deep learning?based automatic fetal brain segmentation method that provides improved accuracy and robustness compared with atlas-based methods. MATERIALS AND METHODS: A total of 106 fetal MR imaging studies were acquired prospectively from fetuses between 23 and 39?weeks of gestation. We trained a deep learning model on the MR imaging scans of 65 healthy fetuses and compared its performance with a 4D atlas-based segmentation method using the Wilcoxon signed-rank test. The trained model was also evaluated on data from 41 fetuses diagnosed with congenital heart disease. RESULTS: The proposed method showed high consistency with the manual segmentation, with an average Dice score of 0.897. It also demonstrated significantly improved performance (P?<?.001) based on the Dice score and 95% Hausdorff distance in all brain regions compared with the atlas-based method. The performance of the proposed method was consistent across gestational ages. The segmentations of the brains of fetuses with high-risk congenital heart disease were also highly consistent with the manual segmentation, though the Dice score was 7% lower than that of healthy fetuses. CONCLUSIONS: The proposed deep learning method provides an efficient and reliable approach for fetal brain segmentation, which outperformed segmentation based on a 4D atlas and has been used in clinical and research settings.
M-DDC: MRI Based Demyelinative Diseases Classification with U-Net Segmentation and Convolutional Network

Abstract:Childhood demyelinative diseases classification (DDC) with brain magnetic resonance imaging (MRI) is crucial to clinical diagnosis. But few attentions have been paid to DDC in the past. How to accurately differentiate pediatric-onset neuromyelitis optica spectrum disorder (NMOSD) from acute disseminated encephalomyelitis (ADEM) based on MRI is challenging in DDC. In this paper, a novel architecture M-DDC based on joint U-Net segmentation network and deep convolutional network is developed. The U-Net segmentation can provide pixel-level structure information, that helps the lesion areas location and size estimation. The classification branch in DDC can detect the regions of interest inside MRIs, including the white matter regions where lesions appear. The performance of the proposed method is evaluated on MRIs of 201 subjects recorded from the Children’s Hospital of Zhejiang University School of Medicine. The comparisons show that the proposed DDC achieves the highest accuracy of 99.19% and dice of 71.1% for ADEM and NMOSD classification and segmentation, respectively.

Abstract:The ability to determine if the brain is developing normally is a key component of pediatric neuroradiology and neurology. Brain magnetic resonance imaging (MRI) of infants demonstrates a specific pattern of development beyond simply myelination. While radiologists have used myelination patterns, brain morphology and size characteristics in determining if brain maturity matches the chronological age of the patient, this requires years of experience with pediatric neuroradiology. Due to the lack of standardized criteria, estimation of brain maturity before age three remains fraught with interobserver and intraobserver variability. An objective measure of brain developmental age estimation (BDAE) could be a useful tool in helping physicians identify developmental delay as well as other neurological diseases. We investigated a three-dimensional convolutional neural network (3D CNN) to rapidly classify brain developmental age using common MRI sequences. MRI datasets from normal newborns were obtained from the National Institute of Mental Health Data Archive from birth to 3 years. We developed a BDAE method using T1-weighted, as well as a fusion of T1-weighted, T2-weighted, and proton density (PD) sequences from 112 individual subjects using 3D CNN. We achieved a precision of 94.8% and a recall of 93.5% in utilizing multiple MRI sequences in determining BDAE.

Abstract:Abstract With the rapid growth and increasing use of brain MRI, there is an interest in automated image classification to aid human interpretation and improve workflow. We aimed to train a deep convolutional neural network and assess its performance in identifying abnormal brain MRIs and critical intracranial findings including acute infarction, acute hemorrhage and mass effect. A total of 13,215 clinical brain MRI studies were categorized to training (74%), validation (9%), internal testing (8%) and external testing (8%) datasets. Up to eight contrasts were included from each brain MRI and each image volume was reformatted to common resolution to accommodate for differences between scanners. Following reviewing the radiology reports, three neuroradiologists assigned each study to abnormal vs normal, and identified three critical findings including acute infarction, acute hemorrhage, and mass effect. A deep convolutional neural network was constructed by a combination of localization feature extraction (LFE) modules and global classifiers to identify the presence of 4 variables in brain MRIs including abnormal, acute infarction, acute hemorrhage and mass effect. Training, validation and testing sets were randomly defined on a patient basis. Training was performed on 9845 studies using balanced sampling to address class imbalance. Receiver operating characteristic (ROC) analysis was performed. The ROC analysis of our models for 1050 studies within our internal test data showed AUC/sensitivity/specificity of 0.91/83%/86% for normal versus abnormal brain MRI, 0.95/92%/88% for acute infarction, 0.90/89%/81% for acute hemorrhage, and 0.93/93%/85% for mass effect. For 1072 studies within our external test data, it showed AUC/sensitivity/specificity of 0.88/80%/80% for normal versus abnormal brain MRI, 0.97/90%/97% for acute infarction, 0.83/72%/88% for acute hemorrhage, and 0.87/79%/81% for mass effect. Our proposed deep convolutional network can accurately identify abnormal and critical intracranial findings on individual brain MRIs, while addressing the fact that some MR contrasts might not be available in individual studies.

Deep Learning to Predict Neonatal and Infant Brain Age from Myelination on Brain MRI Scans

Abstract:Background Assessment of appropriate brain myelination on T1- and T2-weighted MRI scans is based on gestationally corrected age (GCA) and requires subjective visual inspection of the brain with knowledge of normal myelination milestones. Purpose To develop a convolutional neural network (CNN) capable of estimating neonatal and infant GCA based on brain myelination on MRI scans. Materials and methods In this retrospective study from one academic medical center, brain MRI scans of patients aged 0-25 months with reported normal myelination were consecutively collected between January 1995 and June 2019. The GCA at MRI was manually calculated. After exclusion criteria were applied, T1- and T2-weighted MRI scans were preprocessed with skull stripping, linear registration, z scoring for normalization, and downsampling. A three-dimensional regression CNN was trained to predict GCA using mean absolute error (MAE) as its loss function. Attention maps were calculated using layer-wise relevance propagation. Models were validated on an external test set from the National Institutes of Health (NIH). Model MAEs were compared using Kruskal-Wallis and Mann-Whitney tests. Results A total of 518 neonates and infants (mean GCA, 67 weeks ± 33 [SD], 56% male) was included, comprising 469 T1-, 438 T2-, and 389 T1- and T2-weighted studies. Across 10 runs, MAEs of T1-, T2-, and T1- and T2-weighted networks were 9.8 ± 2.3, 9.1 ± 1.9, and 7.7 ± 1.7 weeks, respectively. Attention map analysis demonstrated increased network attention to the cerebellum, posterior white matter, and basal ganglia signal in neonates with GCA of less than 40 weeks and the anterior white matter signal in infants with GCA of more than 120 weeks, corresponding to the known progression of myelination. The T1- and T2-weighted network tested on the external NIH test set had an MAE of 9.1 weeks, which was reduced to 5.9 weeks with further training using half the external test set (P < .001). Conclusion A three-dimensional convolutional neural network can predict the gestationally corrected age of neonates and infants aged 0-25 months based on brain myelination patterns on T1- and T2-weighted MRI scans. © RSNA, 2022 Online supplemental material is available for this article.

Abstract:Determining if the brain is developing normally is a key component of pediatric neuroradiology and neurology. Brain magnetic resonance imaging (MRI) of infants demonstrates a specific pattern of development beyond simply myelination. While radiologists have used myelination patterns, brain morphology and size characteristics to determine age-adequate brain maturity, this requires years of experience in pediatric neuroradiology. With no standardized criteria, visual estimation of the structural maturity of the brain from MRI before three years of age remains dominated by inter-observer and intra-observer variability. A more objective estimation of brain developmental age could help physicians identify many neurodevelopmental conditions and diseases earlier and more reliably. Such data, however, is naturally hard to obtain, and the observer ground truth not much of a gold standard due to subjectivity of assessment. In this light, we explore the general feasibility to tackle this task, and the utility of different approaches, including two- and three-dimensional convolutional neural networks (CNN) that were trained on a fusion of T1-weighted, T2-weighted, and proton density (PD) weighted sequences from 84 individual subjects divided into four age groups from birth to 3 years of age. In the best performing approach, we achieved an accuracy of 0.90 [95% CI:0.86-0.94] using a 2D CNN on a central axial thick slab. We discuss the comparison to 3D networks and show how the performance compares to the use of only one sequence (T1w). In conclusion, despite the theoretical superiority of 3D CNN approaches, in limited-data situations, such approaches are inferior to simpler architectures. Abstract:The performance of deep neural networks typically increases with the number of training images. However, not all images have the same importance towards improved performance and robustness. In fetal brain MRI, abnormalities exacerbate the variability of the developing brain anatomy compared to non-pathological cases. A small number of abnormal cases, as is typically available in clinical datasets used for training, are unlikely to fairly represent the rich variability of abnormal developing brains. This leads machine learning systems trained by maximizing the average performance to be biased toward non-pathological cases. This problem was recently referred to as hidden stratification. To be suited for clinical use, automatic segmentation methods need to reliably achieve high-quality segmentation outcomes also for pathological cases. In this paper, we show that the state-of-the-art deep learning pipeline nnU-Net has difficulties to generalize to unseen abnormal cases. To mitigate this problem, we propose to train a deep neural network to minimize a percentile of the distribution of per-volume loss over the dataset. We show that this can be achieved by using Distributionally Robust Optimization (DRO). DRO automatically reweights the training samples with lower performance, encouraging nnU-Net to perform more consistently on all cases. We validated our approach using a dataset of 368 fetal brain T2w MRIs, including 124 MRIs of open spina bifida cases and 51 MRIs of cases with other severe abnormalities of brain development.

Abstract:This paper presents a deep learning framework for image classification aimed at increasing predictive performance for Cytotoxic Edema (CE) diagnosis in infants and children. The proposed framework includes two 3D network architectures optimized to learn from two types of clinical MRI data , a trace Diffusion Weighted Image (DWI) and the calculated Apparent Diffusion Coefficient map (ADC). This work proposes a robust and novel solution based on volumetric analysis of 3D images (using pixels from time slices) and 3D convolutional neural network (CNN) models. While simple in architecture, the proposed framework shows significant quantitative results on the domain problem. We use a dataset curated from a Childrens Hospital Colorado (CHCO) patient registry to report a predictive performance F1 score of 0.91 at distinguishing CE patients from children with severe neurologic injury without CE. In addition, we perform analysis of our systems output to determine the association of CE with Abusive Head Trauma (AHT) , a type of traumatic brain injury (TBI) associated with abuse , and overall functional outcome and in hospital mortality of infants and young children. We used two clinical variables, AHT diagnosis and Functional Status Scale (FSS) score, to arrive at the conclusion that CE is highly correlated with overall outcome and that further study is needed to determine whether CE is a biomarker of AHT. With that, this paper introduces a simple yet powerful deep learning based solution for automated CE classification. This solution also enables an indepth analysis of progression of CE and its correlation to AHT and overall neurologic outcome, which in turn has the potential to empower experts to diagnose and mitigate AHT during early stages of a childs life.
Can deep learning classify cerebral ultrasound images for the detection of brain injury in very preterm infants?

Abstract:Objectives: Cerebral ultrasound (CUS) is the main imaging screening tool in preterm infants. The aim of this work is to develop deep learning (DL) models that classify normal vs abnormal CUS to serve as a computer-aided detection tool providing timely interpretation of the scans. Methods: A population-based cohort of very preterm infants (220-306 weeks) born between 2004 and 2016 in Nova Scotia, Canada. A set of nine sequential CUS images per infant was retrieved at three specific coronal landmarks at three pre-identified times (first, sixth weeks, and term age). A radiologist manually labeled each image as normal or abnormal. The dataset was split into training/development/test subsets (80:10:10). Different convolutional neural networks were tested, with filtering of the most uncertain prediction. The model's performance was assessed using precision/recall and the receiver operating area under the curve. Results: Sequential CUS retrieved for 538/665 babies (81% of the cohort). Four thousand one hundred eighty images were used to develop and test the model. The model performance was only discrete at the beginning but, through different machine learning strategies was boosted to good levels averaging 0.86 ROC AUC (95% CI: 0.82, 0.90) and 0.87 PR AUC (95% CI: 0.84, 0.90) (model uncertainty estimation filters using normalized entropy threshold = 0.5). Conclusion: This study offers proof of the feasibility of applying DL to CUS. This basic diagnostic model showed good discriminative ability to classify normal versus abnormal CUS. This serves as a CAD and a framework for constructing a prognostic model. Clinical relevance statement: This DL model can serve as a computer-aided detection tool to classify CUS of very preterm babies as either normal or abnormal. This model will also be used as a framework to develop a prognostic model. Key points: Binary computer-aided detection models of CUS are applicable for classifying ultrasound images in very preterm babies. This model acts as a step towards developing a model for predicting neurodevelopmental outcomes in very preterm babies. This model serves as a tool for interpretation of CUS in this patient population with a heightened risk of brain injury.
Deep learning-based detection of murine congenital heart defects from µCT scans

Abstract:Congenital heart defects (CHD) result in high morbidity and mortality rates, but their origins are poorly understood. Mouse models of heart morphogenesis are required to study the pathological mechanisms of heart development compared to normal. In mouse fetuses, CHD can be observed and detected in 3D images obtained by thoracic micro-computed tomography (μCT). However, diagnosis of CHD from μCT scans is a time-consuming process that requires the experience of senior experts. An automated alternative would thus save time, empower less experienced investigators and could broaden analysis to larger numbers of samples. Here, we describe and validate an approach based on deep learning to automatically segment the heart and screen normal from malformed hearts in mouse μCT scans. In an initial cohort, we collected 139 μCT scans from thorax and abdomen of control and mutant perinatal mice. We trained a self-configurating neural network (nnU-Net) to segment hearts from body μCT scans and validated its performance on expert segmentations, achieving a Dice coefficient of 96%. To identify malformed hearts, we developed and trained a 3D convolutional neural network (CNN) that uses segmented μCT scans as inputs. Despite the relatively small training data size, our diagnosis model achieved a sensitivity, specificity (for a 0.5 threshold), and area under the curve (AUC) of 92%, 96%, and 97% respectively, as determined by 5-fold cross-validation. As further validation, we analyzed two additional cohorts that were collected after the model was trained: a ‘prospective’ cohort, using the same experimental protocol as the initial cohort, and containing a subset of its genotypes, and a ‘divergent’ cohort in which mice were subjected to a different treatment for heart arrest (cardioplegia) and that contained a new mouse line. Performance on the prospective cohort was excellent, with a sensitivity of 92%, a specificity of 100%, and an AUC of 100%. Performance on the divergent cohort was moderate (sensitivity: 69%, specificity: 80% and AUC: 81%), but was much improved when the model was finetuned on (a subset of) the cohort (sensitivity: 79%, specificity: 88% and AUC: 91%). These results showcase our model’s robustness and adaptability to technical and biological differences in the data, highlighting its usefulness for practical applications. In order to facilitate the adoption, adaptation and further improvement of these methods, we built a user-friendly (available at ) that allows users without programming skills to utilize the segmentation and diagnosis models and re-train the latter on their own data and resources. The plugin also highlights the cardiac regions used for the diagnosis. Our automatic and retrainable pipeline, which can be employed in high-throughput genetic screening, will accelerate diagnosis of heart anomalies in mice and facilitate studies of the mechanisms of CHD.

Deep learning microstructure estimation of developing brains from diffusion MRI: A newborn and fetal study

Abstract:Diffusion-weighted magnetic resonance imaging (dMRI) is widely used to assess the brain white matter. Fiber orientation distribution functions (FODs) are a common way of representing the orientation and density of white matter fibers. However, with standard FOD computation methods, accurate estimation requires a large number of measurements that usually cannot be acquired for newborns and fetuses. We propose to overcome this limitation by using a deep learning method to map as few as six diffusion-weighted measurements to the target FOD. To train the model, we use the FODs computed using multi-shell high angular resolution measurements as target. Extensive quantitative evaluations show that the new deep learning method, using significantly fewer measurements, achieves comparable or superior results than standard methods such as Constrained Spherical Deconvolution and two state-of-the-art deep learning methods. For voxels with one and two fibers, respectively, our method shows an agreement rate in terms of the number of fibers of 77.5% and 22.2%, which is 3% and 5.4% higher than other deep learning methods, and an angular error of 10° and 20°, which is 6° and 5° lower than other deep learning methods. To determine baselines for assessing the performance of our method, we compute agreement metrics using densely sampled newborn data. Moreover, we demonstrate the generalizability of the new deep learning method across scanners, acquisition protocols, and anatomy on two clinical external datasets of newborns and fetuses. We validate fetal FODs, successfully estimated for the first time with deep learning, using post-mortem histological data. Our results show the advantage of deep learning in computing the fiber orientation density for the developing brain from in-vivo dMRI measurements that are often very limited due to constrained acquisition times. Our findings also highlight the intrinsic limitations of dMRI for probing the developing brain microstructure.

An electron microscopic study of the changes induced by oestrogens on the lymph‐node post‐capillary venules

Abstract:The changes induced in the lymph-node post-capillary venules of male and female mice, 1, 2, 4, 7, 10 and 14 days after oestrogen administration were studied in 1 micron sections and with the electron microscope. The endothelial cells of the post-capillary venules are higher in female than in male control mice. On the first and second days after Hexoestrol administration there is an increase in the number of ribosomes and polyribosomes lying free in the cytoplasm of the endothelial cells. These cells were significantly increased in height between day 2 and day 7, in both male and female Hexoestrol-receiving mice, remaining at high levels thereafter. The increase in height was accompanied by an increase in the Golgi elements which occupied the major part of the cytoplasm of the endothelial cells. Lysosomes, multivesicular bodies and signs of phagocytosis were more often seen. In contrast, the numbers of lymphocytes passing through the post-capillary venule wall were significantly decreased, probably due to a direct effect of Hexoestrol on the thymus. The relation of an oestrogenic effect on the ultrastructure of the post-capillary venules to the sex differences seen in the immune response is discussed. The possibility that this may constitute a part of a general oestrogenic effect on the reticuloendothelial system is also considered.
Automatic linear measurements of the fetal brain on MRI with deep neural networks

Abstract:Timely, accurate and reliable assessment of fetal brain development is essential to reduce short and long-term risks to fetus and mother. Fetal MRI is increasingly used for fetal brain assessment. Three key biometric linear measurements important for fetal brain evaluation are Cerebral Biparietal Diameter (CBD), Bone Biparietal Diameter (BBD), and Trans-Cerebellum Diameter (TCD), obtained manually by expert radiologists on reference slices, which is time consuming and prone to human error. The aim of this study was to develop a fully automatic method computing the CBD, BBD and TCD measurements from fetal brain MRI. The input is fetal brain MRI volumes which may include the fetal body and the mother's abdomen. The outputs are the measurement values and reference slices on which the measurements were computed. The method, which follows the manual measurements principle, consists of five stages: 1) computation of a Region Of Interest that includes the fetal brain with an anisotropic 3D U-Net classifier; 2) reference slice selection with a Convolutional Neural Network; 3) slice-wise fetal brain structures segmentation with a multiclass U-Net classifier; 4) computation of the fetal brain midsagittal line and fetal brain orientation, and; 5) computation of the measurements. Experimental results on 214 volumes for CBD, BBD and TCD measurements yielded a mean $L_1$ difference of 1.55mm, 1.45mm and 1.23mm respectively, and a Bland-Altman 95% confidence interval ($CI_{95}$) of 3.92mm, 3.98mm and 2.25mm respectively. These results are similar to the manual inter-observer variability. The proposed automatic method for computing biometric linear measurements of the fetal brain from MR imaging achieves human level performance. Automated MRI Lung Segmentation and 3D Morphologic Features for Quantification of Neonatal Lung Disease

Abstract:Purpose: To analyze the performance of deep learning (DL) models for segmentation of the neonatal lung in MRI and investigate the use of automated MRI-based features for assessment of neonatal lung disease.Materials and Methods: Quiet-breathing MRI was prospectively performed in two independent cohorts of preterm infants (median gestational age, 26.57 weeks; IQR, 25.3-28.6 weeks; 55 female and 48 male infants) with (n = 86) and without (n = 21) chronic lung disease (bronchopulmonary dysplasia [BPD]). Convolutional neural networks were developed for lung segmentation, and a three-dimensional reconstruction was used to calculate MRI features for lung volume, shape, pixel intensity, and surface. These features were explored as indicators of BPD and disease-associated lung structural remodeling through correlation with lung injury scores and multinomial models for BPD severity stratification.Results: The lung segmentation model reached a volumetric Dice coefficient of 0.908 in cross-validation and 0.880 on the independent test dataset, matching expert-level performance across disease grades. MRI lung features demonstrated significant correlations with lung injury scores and added structural information for the separation of neonates with BPD (BPD vs no BPD: average area under the receiver operating characteristic curve [AUC], 0.92 ± 0.02 [SD]; no or mild BPD vs moderate or severe BPD: average AUC, 0.84 ± 0.03).Conclusion: This study demonstrated high performance of DL models for MRI neonatal lung segmentation and showed the potential of automated MRI features for diagnostic assessment of neonatal lung disease while avoiding radiation exposure.Keywords: Bronchopulmonary Dysplasia, Chronic Lung Disease, Preterm Infant, Lung Segmentation, Lung MRI, BPD Severity Assessment, Deep Learning, Lung Imaging Biomarkers, Lung Topology Supplemental material is available for this article. Harmonized Segmentation of Neonatal Brain MRI

Abstract:Deep learning based medical image segmentation has shown great potential in becoming a key part of the clinical analysis pipeline. However, many of these models rely on the assumption that the train and test data come from the same distribution. This means that such methods cannot guarantee high quality predictions when the source and target domains are dissimilar due to different acquisition protocols, or biases in patient cohorts. Recently, unsupervised domain adaptation techniques have shown great potential in alleviating this problem by minimizing the shift between the source and target distributions, without requiring the use of labeled data in the target domain. In this work, we aim to predict tissue segmentation maps on T 2-weighted magnetic resonance imaging data of an unseen preterm-born neonatal population, which has both different acquisition parameters and population bias when compared to our training data. We achieve this by investigating two unsupervised domain adaptation techniques with the objective of finding the best solution for our problem. We compare the two methods with a baseline fully-supervised segmentation network and report our results in terms of Dice scores obtained on our source test dataset. Moreover, we analyse tissue volumes and cortical thickness measures of the harmonized data on a subset of the population matched for gestational age at birth and postmenstrual age at scan. Finally, we demonstrate the applicability of the harmonized cortical gray matter maps with an analysis comparing term and preterm-born neonates and a proof-of-principle investigation of the association between cortical thickness and a language outcome measure.
NEOCIVET: Towards accurate morphometry of neonatal gyrification and clinical applications in preterm newborns

Abstract:Cerebral cortical folding becomes dramatically more complex in the fetal brain during the 3rd trimester of gestation; the process continues in a similar fashion in children who are born prematurely. To quantify this morphological development, it is necessary to extract the interface between gray matter and white matter, which is particularly challenging due to changing tissue contrast during brain maturation. We employed the well-established CIVET pipeline to extract this cortical surface, with point correspondence across subjects, using a surface-based spherical registration. We then developed a variant of the pipeline, called NEOCIVET, that quantified cortical folding using mean curvature and sulcal depth while addressing the well-known problems of poor and temporally-varying gray/white contrast as well as motion artifact in neonatal MRI. NEOCIVET includes: i) a tissue classification technique that analyzed multi-atlas texture patches using the nonlocal mean estimator and subsequently applied a label fusion approach based on a joint probability between templates, ii) neonatal template construction based on age-specific sub-groups, and iii) masking of non-interesting structures using label-fusion approaches. These techniques replaced modules that might be suboptimal for regional analysis of poor-contrast neonatal cortex. The proposed segmentation method showed more accurate results in subjects with various ages and with various degrees of motion compared to state-of-the-art methods. In the analysis of 158 preterm-born neonates, many with multiple scans (n=231; 26-40weeks postmenstrual age at scan), NEOCIVET identified increases in cortical folding over time in numerous cortical regions (mean curvature: +0.003/week; sulcal depth: +0.04mm/week) while folding did not change in major sulci that are known to develop early (corrected p<0.05). The proposed pipeline successfully mapped cortical structural development, supporting current models of cerebral morphogenesis, and furthermore, revealed impairment of cortical folding in extremely preterm newborns relative to relatively late preterm newborns, demonstrating its potential to provide biomarkers of prematurity-related developmental outcome.

Abstract:Resting-state functional MRI (rs-fMRI) scans hold the potential to serve as a diagnostic or prognostic tool for a wide variety of conditions, such as autism, Alzheimer's disease, and stroke. While a growing number of studies have demonstrated the promise of machine learning algorithms for rs-fMRI based clinical or behavioral prediction, most prior models have been limited in their capacity to exploit the richness of the data. For example, classification techniques applied to rs-fMRI often rely on region-based summary statistics and/or linear models. In this work, we propose a novel volumetric Convolutional Neural Network (CNN) framework that takes advantage of the full-resolution 3D spatial structure of rs-fMRI data and fits non-linear predictive models. We showcase our approach on a challenging large-scale dataset (ABIDE, with N > 2,000) and report state-of-the-art accuracy results on rs-fMRI-based discrimination of autism patients and healthy controls.
3D CNN for neuropsychiatry: Predicting Autism with interpretable Deep Learning applied to minimally preprocessed structural MRI data

Abstract:Predictive modeling approaches are enabling progress toward robust and reproducible brain-based markers of neuropsychiatric conditions by leveraging the power of multivariate analyses of large datasets. While deep learning (DL) offers another promising avenue to further advance progress, there are challenges related to implementation in 3D (best for MRI) and interpretability. Here, we address these challenges and describe an interpretable predictive pipeline for inferring Autism diagnosis using 3D DL applied to minimally processed structural MRI scans. We trained 3D DL models to predict Autism diagnosis using the openly available ABIDE I and II datasets (n = 1329, split into training, validation, and test sets). Importantly, we did not perform transformation to template space, to reduce bias and maximize sensitivity to structural alterations associated with Autism. Our models attained predictive accuracies equivalent to those of previous machine learning (ML) studies, while side-stepping the time- and resource-demanding requirement to first normalize data to a template. Our interpretation step, which identified brain regions that contributed most to accurate inference, revealed regional Autism-related alterations that were highly consistent with the literature, encompassing a left-lateralized network of regions supporting language processing. We have openly shared our code and models to enable further progress towards remaining challenges, such as the clinical heterogeneity of Autism and site effects, and to enable the extension of our method to other neuropsychiatric conditions.

Abstract:Quality assessment of prenatal ultrasonography is essential for the screening of fetal central nervous system (CNS) anomalies. The interpretation of fetal brain structures is highly subjective, expertise-driven, and requires years of training experience, limiting quality prenatal care for all pregnant mothers. With recent advancement in Artificial Intelligence (AI), computer assisted diagnosis has shown promising results, being able to provide expert level diagnosis in matter of seconds and therefore has the potential to improve access to quality and standardized care for all. Specifically, with advent of deep learning (DL), assistance in precise anatomy identification through semantic segmentation essential for the reliable assessment of growth and neurodevelopment, and detection of structural abnormalities have been proposed. However, existing works only identify certain structures (e.g., cavum septum pellucidum [CSP], lateral ventricles [LV], cerebellum ) from either of the axial views (transventricular [TV], transcerebellar [TC]), limiting the scope for a thorough anatomical assessment as per practice guidelines necessary for the screening of CNS anomalies. Further, existing works do not analyze the generalizability of these DL algorithms across images from multiple ultrasound devices and centers, thus, limiting their real-world clinical impact. In this study, we propose a deep learning (DL) based segmentation framework for the automated segmentation of 10 key fetal brain structures from 2 axial planes from fetal brain USG images (2D). We developed a custom U-Net variant that uses inceptionv4 block as a feature extractor and leverages custom domain-specific data augmentation. Quantitatively, the mean (10 structures; test sets 1/2/3/4) Dice-coefficients were: 0.827, 0.802, 0.731, 0.783. Irrespective of the USG device/center, the DL segmentations were qualitatively comparable to their manual segmentations. The proposed DL system offered a promising and generalizable performance (multi-centers, multi-device) and also presents evidence in support of device-induced variation in image quality (a challenge to generalizibility) by using UMAP analysis. Its clinical translation can assist a wide range of users across settings to deliver standardized and quality prenatal examinations.

Abstract:Objectives. MRI is pivotal in diagnosing brain injuries in infants. However, the dynamic development of the brain introduces variability in infant MRI characteristics, posing challenges for MRI-based classification in this population. Furthermore, manual data selection in large-scale studies is labor-intensive, and existing algorithms often underperform with thick-slice MRI data. To enhance research efficiency and classification accuracy in large datasets, we propose an advanced classification model. Approach. We introduce the Dual-Branch Attention Information Interactive Neural Network (DBAII-Net), a cutting-edge model inspired by radiologists' use of multiple MRI sequences. DBAII-Net features two innovative modules: (1) the Convolutional Enhancement Module (CEM), which leverages advanced convolutional techniques to aggregate multi-scale features, significantly enhancing information representation; and (2) the Cross-Modal Attention Module (CMAM), which employs state-of-the-art attention mechanisms to fuse data across branches, dramatically improving positional and channel feature extraction. Performances (accuracy, sensitivity, specificity, area under the curve, etc.) of DBAII-Net were compared with eight benchmark models for brain MRI classification in infants aged 6 months to 2 years. Main results. Utilizing a self-constructed dataset of 240 thick-slice brain MRI scans (122 with brain injuries, 118 without), DBAII-Net demonstrated superior performance. On a test set of approximately 50 cases, DBAII-Net achieved average performance metrics of 92.53% accuracy, 90.20% sensitivity, 94.93% specificity, and an area under the curve (AUC) of 0.9603. Ablation studies confirmed the effectiveness of CEM and CMAM, with CMAM significantly boosting classification metrics. Significance. DBAII-Net with CEM and CMAM outperforms existing benchmarks in enhancing the precision of brain MRI classification in infants, significantly reducing manual effort in infant brain research.
