Abstract:The metabolic rate of oxygen (MRO2) is fundamental to tissue metabolism. Determination of MRO2 demands knowledge of the arterio-venous difference in hemoglobin-bound oxygen concentration, typically expressed as oxygen extraction fraction (OEF), and blood flow rate (BFR). MRI is uniquely suited for measurement of both these quantities, yielding MRO2 in absolute physiologic units of µmol O2 min-1/100 g tissue. Two approaches are discussed, both relying on hemoglobin magnetism. Emphasis will be on cerebral oxygen metabolism expressed in terms of the cerebral MRO2 (CMRO2), but translation of the relevant technologies to other organs, including kidney and placenta will be touched upon as well. The first class of methods exploits the blood's bulk magnetic susceptibility, which can be derived from field maps. The second is based on measurement of blood water T2, which is modulated by diffusion and exchange in the local-induced fields within and surrounding erythrocytes. Some whole-organ methods achieve temporal resolution adequate to permit time-series studies of brain energetics, for instance, during sleep in the scanner with concurrent electroencephalogram (EEG) sleep stage monitoring. Conversely, trading temporal for spatial resolution has led to techniques for spatially resolved approaches based on quantitative blood oxygen level dependent (BOLD) or calibrated BOLD models, allowing regional assessment of vascular-metabolic parameters, both also exploiting deoxyhemoglobin paramagnetism like their whole-organ counterparts.
Imaging effective oxygen diffusivity in the human brain with multiparametric magnetic resonance imaging

Abstract:Cerebrovascular diseases can impair blood circulation and oxygen extraction from the blood. The effective oxygen diffusivity (EOD) of the capillary bed is a potential biomarker of microvascular function that has gained increasing interest, both for clinical diagnosis and for elucidating oxygen transport mechanisms. Models of capillary oxygen transport link EOD to measurable oxygen extraction fraction (OEF) and cerebral blood flow (CBF). In this work, we confirm that two well established mathematical models of oxygen transport yield nearly equivalent EOD maps. Furthermore, we propose an easy-to-implement and clinically applicable multiparametric magnetic resonance imaging (MRI) protocol for quantitative EOD mapping. Our approach is based on imaging OEF and CBF with multiparametric quantitative blood oxygenation level dependent (mq-BOLD) MRI and pseudo-continuous arterial spin labeling (pCASL), respectively. We evaluated the imaging protocol by comparing MRI-EOD maps of 12 young healthy volunteers to PET data from a published study in different individuals. Our results show comparably good correlation between MRI- and PET-derived cortical EOD, OEF and CBF. Importantly, absolute values of MRI and PET showed high accordance for all three parameters. In conclusion, our data indicates feasibility of the proposed MRI protocol for EOD mapping, rendering the method promising for future clinical evaluation of patients with cerebrovascular diseases.
Evaluation of calibrated and uncalibrated optical imaging approaches for relative cerebral oxygen metabolism measurements in awake mice

Abstract:Objective. The continuous delivery of oxygen is critical to sustain brain function, and therefore, measuring brain oxygen consumption can provide vital physiological insight. In this work, we examine the impact of calibration and CBF measurements on the computation of the relative changes in the cerebral metabolic rate of oxygen consumption (rCMRO2) from hemoglobin-sensitive intrinsic optical imaging data. Using these data, we calculate rCMRO2, and calibrate the model using an isometabolic stimulus. Approach. We used awake head-fixed rodents to obtain hemoglobin-sensitive optical imaging data to test different calibrated and uncalibrated rCMRO2 models. Hypercapnia was used for calibration and whisker stimulation to further tests the models and gauge the impact of calibration. Main results. We found that typical uncalibrated models can provide reasonable estimates of rCMRO2 with differences as small as 7-9% compared to their calibrated models. However, calibrated models showed lower variability and less dependence on baseline hemoglobin concentrations. Lastly, we found that supplying the model with measurements of cerebral blood flow (CBF) significantly reduced error and variability in CMRO2 change calculations. Significance. The effect of calibration on rCMRO2 calculations remains understudied, and we systematically evaluated different rCMRO2 calculation scenarios that consider including different measurement combinations. This study provides a quantitative comparison of these scenarios to evaluate trade-offs that can be vital to the design of blood oxygenation sensitive imaging experiments for rCMRO2 calculation.

Abstract:The human brain constitutes 2% of the body's total mass but uses 20% of the oxygen. The rate of the brain's oxygen utilization can be derived from a knowledge of cerebral blood flow and the oxygen extraction fraction (OEF). Therefore, OEF is a key physiological parameter of the brain's function and metabolism. OEF has been suggested to be a useful biomarker in a number of brain diseases. With recent advances in MRI techniques, several MRI‐based methods have been developed to measure OEF in the human brain. These MRI OEF techniques are based on the T2 of blood, the blood signal phase, the magnetic susceptibility of blood‐containing voxels, the effect of deoxyhemoglobin on signal behavior in extravascular tissue, and the calibration of the BOLD signal using gas inhalation. Compared to 15O PET, which is considered the "gold standard" for OEF measurement, MRI‐based techniques are non‐invasive, radiation‐free, and are more widely available. This article provides a review of these emerging MRI‐based OEF techniques. We first briefly introduce the role of OEF in brain oxygen homeostasis. We then review the methodological aspects of different categories of MRI OEF techniques, including their signal mechanisms, acquisition methods, and data analyses. The strengths and limitations of the techniques are discussed. Finally, we review key applications of these techniques in physiological and pathological conditions.

Abstract:Oxygen extraction fraction (OEF), the fraction of oxygen that tissue extracts from blood, is an essential biomarker used to directly assess tissue viability and function in neurologic disorders. In ischemic stroke, for example, increased OEF can indicate the presence of penumbra-tissue with low perfusion yet intact cellular integrity-making it a primary therapeutic target. However, practical OEF mapping methods are not currently available in clinical settings, owing to the impractical data acquisitions in positron emission tomography (PET) and the limitations of existing MRI techniques. Recently, a novel MRI-based OEF mapping technique, termed QQ, was proposed. It shows high potential for clinical use by utilizing a routine sequence and removing the need for impractical multiple gas inhalations. However, QQ relies on the assumptions of Gaussian noise in susceptibility and multi-echo gradient echo (mGRE) magnitude signals for OEF estimation. This assumption is unreliable in low signal-to-noise ratio (SNR) regions like disease-related lesions, risking inaccurate OEF estimation and potentially impacting clinical decisions. Addressing this, our study presents a novel multi-echo complex QQ (mcQQ) that models realistic Gaussian noise in mGRE complex signals. We implemented mcQQ using a deep learning framework (mcQQ-NET) and compared it with the existing QQ-NET in simulations, ischemic stroke patients, and healthy subjects, using identical training and testing datasets and schemes. In simulations, mcQQ-NET provided more accurate OEF than QQ-NET. In the subacute stroke patients, mcQQ-NET showed a lower average OEF ratio in lesions relative to unaffected contralateral normal tissue than QQ-NET. In the healthy subjects, mcQQ-NET provided uniform OEF maps, similar to QQ-NET, but without unrealistically high OEF outliers in areas of low SNR, such as SNR ≤ 15 (dB). Therefore, mcQQ-NET improves OEF accuracy by more accurately reflecting realistic Gaussian noise in complex mGRE signals. Its enhanced sensitivity to OEF abnormalities, based on more realistic biophysics modeling, suggests that mcQQ-NET has potential for investigating tissue variability in neurologic disorders.

Abstract:Streamlined qBOLD acquisitions enable experimentally straightforward observations of brain oxygen metabolism. $R_2^\prime$ maps are easily inferred; however, the Oxygen extraction fraction (OEF) and deoxygenated blood volume (DBV) are more ambiguously determined from the data. As such, existing inference methods tend to yield very noisy and underestimated OEF maps, while overestimating DBV. This work describes a novel probabilistic machine learning approach that can infer plausible distributions of OEF and DBV. Initially, we create a model that produces informative voxelwise prior distribution based on synthetic training data. Contrary to prior work, we model the joint distribution of OEF and DBV through a scaled multivariate logit-Normal distribution, which enables the values to be constrained within a plausible range. The prior distribution model is used to train an efficient amortized variational Bayesian inference model. This model learns to infer OEF and DBV by predicting real image data, with few training data required, using the signal equations as a forward model. We demonstrate that our approach enables the inference of smooth OEF and DBV maps, with a physiologically plausible distribution that can be adapted through specification of an informative prior distribution. Other benefits include model comparison (via the evidence lower bound) and uncertainty quantification for identifying image artefacts. Results are demonstrated on a small study comparing subjects undergoing hyperventilation and at rest. We illustrate that the proposed approach allows measurement of gray matter differences in OEF and DBV and enables voxelwise comparison between conditions, where we observe significant increases in OEF and $R_2^\prime$ during hyperventilation.
Oxygen extraction fraction mapping with multi-parametric quantitative BOLD MRI: Reduced transverse relaxation bias using 3D-GraSE imaging

Abstract:Magnetic resonance imaging (MRI)-based quantification of the blood-oxygenation-level-dependent (BOLD) effect allows oxygen extraction fraction (OEF) mapping. The multi-parametric quantitative BOLD (mq-BOLD) technique facilitates relative OEF (rOEF) measurements with whole brain coverage in clinically applicable scan times. Mq-BOLD requires three separate scans of cerebral blood volume and transverse relaxation rates measured by gradient-echo (1/T2*) and spin-echo (1/T2). Although the current method is of clinical merit in patients with stroke, glioma and internal carotid artery stenosis (ICAS), there are relaxation measurement artefacts that impede the sensitivity of mq-BOLD and artificially elevate reported rOEF values.We posited that T2-related biases caused by slice refocusing imperfections during rapid 2D-GraSE (Gradient and Spin Echo) imaging can be reduced by applying 3D-GraSE imaging sequences, because the latter requires no slice selective pulses. The removal of T2-related biases would decrease overestimated rOEF values measured by mq-BOLD. We characterized effects of T2-related bias in mq-BOLD by comparing the initially employed 2D-GraSE and two proposed 3D-GraSE sequences to multiple single spin-echo reference measurements, both in vitro and in vivo. A phantom and 25 participants, including young and elderly healthy controls as well as ICAS-patients, were scanned. We additionally proposed a procedure to reliably identify and exclude artefact affected voxels. In the phantom, 3D-GraSE derived T2 values had 57% lower deviation from the reference. For in vivo scans, the formerly overestimated rOEF was reduced by -27% (p<0.001). We obtained rOEF=0.51, which is much closer to literature values from positron emission tomography (PET) measurements. Furthermore, increased sensitivity to a focal rOEF elevation in an ICAS-patient was demonstrated.In summary, the application of 3D-GraSE improves the mq-BOLD-based rOEF quantification while maintaining clinically feasible scan times. Thus, mq-BOLD with non-slice selective T2 imaging is highly promising to improve clinical diagnostics of cerebrovascular diseases such as ICAS.

Abstract:Oxygenation-sensitive cardiovascular magnetic resonance (OS-CMR) is a novel, powerful tool for assessing coronary function in vivo. The data extraction and analysis however are labor-intensive. The objective of this study was to provide an automated approach for the extraction, visualization, and biomarker selection of OS-CMR images. We created a Python-based tool to automate extraction and export of raw patient data, featuring 3336 attributes per participant, into a template compatible with common data analytics frameworks, including the functionality to select predictive features for the given disease state. Each analysis was completed in about 2 min. The features selected by both ANOVA and MIC significantly outperformed ( p < 0.001) the null set and complete set of features in two datasets, with mean AUROC scores of 0.89eatures f 0.94lete set of features in two datasets, with mean AUROC scores that our tool is suitable for automated data extraction and analysis of OS-CMR images.

Intraosseous access in the military operational setting.

Abstract:INTRODUCTION In an operational military environment, circulatory access can prove difficult for a variety of reasons including profound hypovolaemia, and limited first responder experience. With many injuries that cause catastrophic haemorrhage, such as traumatic limb amputations, circulatory access is needed as quickly as possible. Since 2006, the Defence Medical Services have been using the EZ-IO and FAST1 devices as a means of obtaining circulatory access. METHODS A prospective observational study was conducted between March and July 2011 at the Emergency Department, Camp Bastion, Afghanistan. All patients with an intraosseous device had data recorded that included if the device successfully flushed (functionality) and if any problems were encountered. RESULTS 117 patients presented with a total of 195 devices: 149 were EZ-IO (76%) and 46 were FAST1 (24%). Functionality was recorded for 111 (57%), with 17 failing to function, yielding an overall success rate of 84.7%. Device failure was observed to be more prevalent in the humerus; inability to flush the device was the leading cause, followed by mechanical failure. There were 2 complications, device breaking on removal being the reason for both. CONCLUSIONS The devices in the study were tested for a period of time following insertion (median 32 minutes), and still the success rates mirror those found in the literature. Observed differences between sites were not found to be significant with confidence intervals overlapping. Further work is proposed to investigate the long-term complications of intraosseous devices.

Abstract:Journal of Cerebral Blood Flow &Metabolism, Ahead of Print. Quantitative BOLD (qBOLD) MRI allows evaluation of oxidative metabolism of the brain based purely on an endogenous contrast mechanism. The method quantifies deoxygenated blood volume (DBV) and hemoglobin oxygen saturation level of venous blood (Yv), yielding oxygen extraction fraction (OEF), and along with a separate measurement of cerebral blood flow, cerebral metabolic rate of oxygen (CMRO2) maps. Here, we evaluated our recently reported 3D qBOLD method that rectifies a number of deficiencies in prior qBOLD approaches in terms of repeat reproducibility and sensitivity to hypercapnia on the metabolic parameters, and in comparison to dual-gas calibrated BOLD (cBOLD) MRI for determining resting-state oxygen metabolism. Results suggested no significant difference between test-retest qBOLD scans in either DBV and OEF. Exposure to hypercapnia yielded group averages of 38 and 28% for OEF and 151 and 146 μmol/min/100 g for CMRO2 in gray matter at baseline and hypercapnia, respectively. The decrease of OEF during hypercapnia was significant (p ≪ 0.01), whereas CMRO2 did not change significantly (p = 0.25). Finally, baseline OEF (37 vs. 39%) and CMRO2 (153 vs. 145 μmol/min/100 g) in gray matter using qBOLD and dual-gas cBOLD were found to be in good agreement with literature values, and were not significantly different from each other (p > 0.1).

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3><p>Perfusion MRI is an important modality in many brain imaging protocols, since it probes cerebrovascular changes in aging and many diseases; however, it may not be always available.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">New method</h3><p>We introduce a new method that seeks to estimate regional perfusion properties using spectral information of resting-state functional MRI (rsfMRI) via machine learning. We used pairs of rsfMRI and arterial spin labeling (ASL) images from the same individuals with normal cognition and mild cognitive impairment (MCI), and built support vector machine models aiming to estimate regional cerebral blood flow (CBF) from the rsfMRI signal alone.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>This method demonstrated higher associations between the estimated CBF and actual CBF (ASL-CBF) at the total lobar gray matter (r = 0.40; FDR-p = 1.9e-03), parietal lobe (r = 0.46, FDR-p = 8e-04), and occipital lobe (r = 0.35; FDR-p = 0.01) using rsfMRI signals of frequencies [0.01-0.15] Hertz compared to frequencies [0.01-0.10] Hertz and [0.01-0.20] Hertz. We further observed significant associations between the estimated CBF and actual CBF in 24 regions of interest (p < 0.05), with the highest association observed in the superior parietal lobule (r = 0.50, FDR-p = 0.002). Moreover, the estimated CBF at superior parietal lobule showed significant correlation with the mini-mental state exam (MMSE) score (r = 0.27; FDR-p = 0.04) and decreased in MCI with lower MMSE score compared to NC group (FDR-p = 0.04).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Comparison with existing methods</h3><p>Consistent with previous findings, this new method also suggests that rsfMRI signals contain perfusion information.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>The proposed framework can obtain estimates of regional perfusion from rsfMRI, which can serve as surrogate perfusion measures in the absence of ASL.</p>

Predicting Hypoxia in Brain Tumors from Multiparametric MRI

Abstract:This research paper presents a novel approach to the prediction of hypoxia in brain tumors, using multi-parametric Magnetic Resonance Imaging (MRI). Hypoxia, a condition characterized by low oxygen levels, is a common feature of malignant brain tumors associated with poor prognosis. Fluoromisonidazole Positron Emission Tomography (FMISO PET) is a well-established method for detecting hypoxia in vivo, but it is expensive and not widely available. Our study proposes the use of MRI, a more accessible and cost-effective imaging modality, to predict FMISO PET signals. We investigate deep learning models (DL) trained on the ACRIN 6684 dataset, a resource that contains paired MRI and FMISO PET images from patients with brain tumors. Our trained models effectively learn the complex relationships between the MRI features and the corresponding FMISO PET signals, thereby enabling the prediction of hypoxia from MRI scans alone. The results show a strong correlation between the predicted and actual FMISO PET signals, with an overall PSNR score above 29.6 and a SSIM score greater than 0.94, confirming MRI as a promising option for hypoxia prediction in brain tumors. This approach could significantly improve the accessibility of hypoxia detection in clinical settings, with the potential for more timely and targeted treatments.

Two distinct modes of hemodynamic responses across the human cortex

Abstract:Functional magnetic resonance imaging (fMRI) detects changes in brain activity by measuring fluctuations in blood oxygenation. This interpretation relies on the mechanism of neurovascular coupling, which closely links changes in neuronal activity to changes in cerebral blood flow (CBF). Whether uniform neurovascular coupling applies to the entire human cortex is yet to be proven. We used blood-oxygenation-level-dependent (BOLD) fMRI along with quantitative fMRI to assess the cerebral metabolic rate of oxygen (CMRO2) associated with both positive and negative BOLD signal changes (∆BOLD). In contradiction to the canonical hemodynamic model of BOLD signal responses, we found discordant CMRO2 in 12-36% of voxels with significant positive and in 52-66% of voxels with significant negative ∆BOLD. This implies that a considerable number of voxels with negative ∆BOLD exhibit increased metabolic activity and vice versa. During the baseline state, we found a lower oxygen extraction fraction (OEF) and oxygen metabolism in discordant compared to concordant voxels. During task states, ∆OEF replaces ∆CBF as the primary mechanism of oxygen supply in discordant voxels. Thus, we propose OEF as a complementary hemodynamic response type to CBF in regulating oxygen demand in the human brain. In such conditions, the traditional interpretation of positive and negative BOLD signal changes in terms of increased or decreased neural activity will be misleading, and we recommend quantitative fMRI or additional CBF measurements for a more valid interpretation of regional BOLD signal changes in relation to neuronal activity.

Machine learning‐based estimation of respiratory fluctuations in a healthy adult population using resting state BOLD fMRI and head motion parameters

Abstract:Purpose External physiological monitoring is the primary approach to measure and remove effects of low‐frequency respiratory variation from BOLD‐fMRI signals. However, the acquisition of clean external respiratory data during fMRI is not always possible, so recent research has proposed using machine learning to directly estimate respiratory variation (RV), potentially obviating the need for external monitoring. In this study, we propose an extended method for reconstructing RV waveforms directly from resting state BOLD‐fMRI data in healthy adult participants with the inclusion of both BOLD signals and derived head motion parameters. Methods In the proposed method, 1D convolutional neural networks (1D‐CNNs) used BOLD signals and head motion parameters to reconstruct the RV waveform for the whole fMRI scan time. Resting‐state fMRI data and associated respiratory records from the Human Connectome Project in Young Adults (HCP‐YA) dataset are used to train and test the proposed method. Results Compared to using only BOLD‐fMRI data for a CNN input, this approach yielded improvements of 14% in mean absolute error, 24% in mean square error, 14% in correlation, and 12% in dynamic time warping. When tested on independent datasets, the method demonstrated generalizability, even in data with different TRs and physiological conditions. Conclusion This study shows that the respiratory variations could be reconstructed from BOLD‐fMRI data in the young adult population, and its accuracy could be improved using supportive data such as head motion parameters. The method also performed well on independent datasets with different experimental conditions.

Test-retest analysis of cerebral oxygen extraction estimates in healthy volunteers: comparison of methods based on quantitative susceptibility mapping and dynamic susceptibility contrast magnetic resonance imaging

Abstract:Background: Estimation of the oxygen extraction fraction (OEF) by quantitative susceptibility mapping (QSM) magnetic resonance imaging (MRI) is promising but requires systematic evaluation. Extraction of OEF-related information from the tissue residue function in dynamic susceptibility contrast MRI (DSC-MRI) has also been proposed. In this study, whole-brain OEF repeatability was investigated, as well as the relationships between QSM-based OEF and DSC-MRI-based parameters, i.e., mean transit time (MTT) and an oxygen extraction index, referred to as apparent OEF (AOEF). Method: Test-retest data were obtained from 20 healthy volunteers at 3 T. QSM maps were reconstructed from 3D gradient-echo MRI phase data, using morphology-enabled dipole inversion. DSC-MRI was accomplished using gradient-echo MRI at a temporal resolution of 1.24 s. Results: The whole-brain QSM-based OEF was (40.4±4.8) % and, in combination with a previously published cerebral blood flow (CBF) estimate, this corresponds to a cerebral metabolic rate of oxygen level of CMRO2 = 3.36 ml O2/min/100 g. The intra-class correlation coefficient [ICC(2,1)] for OEF test-retest data was 0.73. The MTT-versus-OEF and AOEF-versus-OEF relationships showed correlation coefficients of 0.61 (p = 0.004) and 0.52 (p = 0.019), respectively. Discussion: QSM-based OEF showed a convincing absolute level and good test-retest results in terms of the ICC. Moderate to good correlations between QSM-based OEF and DSC-MRI-based parameters were observed. The present results constitute an indicator of the level of robustness that can be achieved without applying extraordinary resources in terms of MRI equipment, imaging protocol, QSM reconstruction, and OEF analysis.

Abstract:The Magnetic Resonance Fingerprinting (MRF) approach aims to estimate multiple MR or physiological parameters simultaneously with a single fast acquisition sequence. Most of the MRF studies proposed so far have used simple MR sequence types to measure relaxation times (T1, T2). In that case, deep learning algorithms have been successfully used to speed up the reconstruction process. In theory, the MRF concept could be used with a variety of other MR sequence types and should be able to provide more information about the tissue microstructures. Yet, increasing the complexity of the numerical models often leads to prohibited simulation times, and estimating multiple parameters from one sequence implies new dictionary dimensions whose sizes become too large for standard computers and DL <a class="link-external link-http" href="http://architectures.In" rel="external noopener nofollow">this http URL</a> this paper, we propose to analyze the MRF signal coming from a complex balance Steady-state free precession (bSSFP) type sequence to simultaneously estimate relaxometry maps (T1, T2), Field maps (B1, B0) as well as microvascular properties such as the local Cerebral Blood Volume (CBV) or the averaged vessel Radius (R).To bypass the curse of dimensionality, we propose an efficient way to simulate the MR signal coming from numerical voxels containing realistic microvascular networks as well as a Bidirectional Long Short-Term Memory network used for the matching process.On top of standard MRF maps, our results on 3 human volunteers suggest that our approach can quickly produce high-quality quantitative maps of microvascular parameters that are otherwise obtained using longer dedicated sequences and intravenous injection of a contrast agent. This approach could be used for the management of multiple pathologies and could be tuned to provide other types of microstructural information.

Abstract:Health Informatics Journal, Volume 30, Issue 2, April-June 2024. This study develops machine learning-based algorithms that facilitate accurate prediction of cerebral oxygen saturation using waveform data in the near-infrared range from a multi-modal oxygen saturation sensor. Data were obtained from 150,000 observations of a popular cerebral oximeter, Masimo O3TM regional oximetry (Co., United States) and a multi-modal cerebral oximeter, Votem (Inc., Korea). Among these observations, 112,500 (75%) and 37,500 (25%) were used for training and test sets, respectively. The dependent variable was the cerebral oxygen saturation value from the Masimo O3TM (0–100%). The independent variables were the time of measurement (0–300,000 ms) and the 16-bit decimal amplitudes values (infrared and red) from Votem (0–65,535). For the right part of the forehead, the root mean square error of the random forest (0.06) was much smaller than those of linear regression (1.22) and the artificial neural network with one, two or three hidden layers (2.58). The result was similar for the left part of forehead, that is, random forest (0.05) vs logistic regression (1.22) and the artificial neural network with one, two or three hidden layers (2.97). Machine learning aids in accurately predicting of cerebral oxygen saturation, employing the data from a multi-modal cerebral oximeter.

Abstract:The aim of this study was to investigate the various MRI biophysical models in the measurements of local cerebral metabolic rate of oxygen (CMRO(2)) and the corresponding relationship with cerebral blood flow (CBF) during brain activation. This aim was addressed by simultaneously measuring the relative changes in CBF, cerebral blood volume (CBV), and blood oxygen level dependent (BOLD) MRI signals in the human visual cortex during visual stimulation. A radial checkerboard delivered flash stimulation at five different frequencies. Two MRI models, the single-compartment model (SCM) and the multicompartment model (MCM), were used to determine the relative changes in CMRO(2) using three methods: [1] SCM with parameters identical to those used in a prior MRI study (M = 0.22; alpha = 0.38); [2] SCM with directly measured parameters (M from hypercapnia and alpha from measured deltaCBV and deltaCBF); and [3] MCM. The magnitude of relative changes in CMRO(2) and the nonlinear relationship between CBF and CMRO(2) obtained with Methods [2] and [3] were not in agreement with those obtained using Method [1]. However, the results of Methods [2] and [3] were aligned with positron emission tomography findings from the literature. Our results indicate that if appropriate parameters are used, the SCM and MCM models are equivalent for quantifying the values of CMRO(2) and determining the flow-metabolism relationship.
