Abstract:Despite interacting with health care professionals 3 times weekly, people with kidney diseases have higher rates of emergency department utilization, hospitalization and mortality than other Medicare beneficiaries. This disparity was exaggerated by the COVID-19 pandemic. 1 Among the factors identified as increasing the risk of mortality for Medicare beneficiaries with kidney failure are weakened immune systems, multiple comorbidities, and an unmet need for in-person health services. During the pandemic, the 18 ESRD Network Programs implemented interventions highlighting recommendations from the Centers for Disease Control and Prevention (CDC) to slow the transmission of SARS-CoV-2. Despite these efforts, high death rates persisted in parallel with the known areas of high COVID-19 transmission. These patterns suggest that kidney disease provides an opportunity to implement public health interventions aimed at the risk factors for excess mortality in people with kidney disease. Analyses of the causes and patterns of poorer outcomes can identify new public health focuses and guide the planning and implementation of interventions for the larger Medicare population. Priorities for preventing the spread of infectious diseases are immunization and contact tracing. The current political environment is associated with a decreased rate of immunization especially against SARS-CoV-2 2 ; public health efforts are urgently needed to prevent upsurges in preventable deaths. Early during the pandemic, contact tracing was performed under the auspices of the CDC; as tests became more readily available, state and local governments took the lead. Factors critical to successful screening and tracing include testing frequency and individual willingness to undergo testing as well as the sensitivity and specificity of the test, community prevalence of disease and the cost of the test. 3 When willingness to undergo testing for SARS-CoV-2 decreased from 50% to 20%, the cost per infection avoided increased from 22 200. Willingness to undergo testing depends on numerous factors including perceived risks and benefits of testing. Social determinants of health play a large role in an individual's trust of health care professionals. Disparities greatly influence a patient's willingness to accept screening for health conditions including infectious diseases. 4 Historic disparities in access to health care worsened as a result of the pandemic, which in turn worsened existing mistrust of the health care system among underserved individuals. Montez-Rath and colleagues 5 studied SARS-CoV-2 screening among patients undergoing maintenance dialysis. They found no differences in willingness to undergo testing when offered a dynamic testing strategy with frequency based on local SARS-CoV-2 activity compared with a static strategy on a fixed testing frequency. The authors noted higher rates of test acceptance among women, individuals identified as Hispanic in the medical record, patients with diabetes, those living in the West and Midwest compared with the South and Northeast, and patients living in counties in the third quartile of the CDC Social Vulnerability Index. Notably, patients identified as Black in the medical record were less likely to accept testing as were those living in a zip code tabulation area with a Republican representative in the US Congress. The authors note that at the time they completed their study, public perception was that the risks associated with COVID-19 infection had abated, which impacted test acceptance. Their study looked at patients receiving maintenance dialysis but can be applied more broadly. Improving public health requires addressing social determinants of health and educating the public regarding the risks and benefits of screening for health conditions. Improving the health of the US population is challenging, particularly in historically underserved populations. The public relies on public officials for open, honest, straightforward communication about health threats. Confusion resulting from discrepancies between official public statements and scientific evidence causes mistrust and undermines public health resulting in harm to the health of the public. 6 In the Montez-Rath study, 5 disparities based on geography and representation in the US Congress suggest a relationship to information received by the public. This discrepancy may result in an avoidable tragedy through increased vulnerability to preventable illness. Lessons from the kidney community can shed light on a successful approach 7 to improving public health.
